Student mental health was a growing concern globally prior to the onset of the COVID-19 pandemic. The aim of this study was to assess the impact of the pandemic and associated restrictions on the psychological wellbeing of college students. Baseline data were collected pre-pandemic in September 2019 among students attending a university in Northern Ireland and an Institute of Technology in the Republic of Ireland. Surveys were also conducted with this cohort during the pandemic, at the start of the academic years 2020 and 2021 (499 students fully completed all three waves). A follow-up survey was conducted at the end of their third year, in summer 2022 (n = 229). High levels of mental health problems were already present among students commencing college. The subsequent pandemic had a very negative impact on student's academic experience and other aspects of life. Rates of depression (PHQ-9) increased significantly from the onset of the pandemic and remained high. Anxiety (GAD-7) initially decreased but then escalated at the end of college. The study highlights the importance of early intervention and makes recommendations for addressing the needs of students during times of stress. The underpinnings of mild to moderate neurodevelopmental delay remain elusive, often leading to late diagnosis and interventions. Here, we present data on exome and genome sequencing as well as array analysis of 13 individuals that point to pathogenic, heterozygous, mostly de novo variants in WDFY3 (significant de novo enrichment P = 0.003) as a monogenic cause of mild and non-specific neurodevelopmental delay. Nine variants were protein-truncating and four missense. Overlapping symptoms included neurodevelopmental delay, intellectual disability, macrocephaly, and psychiatric disorders (autism spectrum disorders/attention deficit hyperactivity disorder). One proband presented with an opposing phenotype of microcephaly and the only missense-variant located in the PH-domain of WDFY3. Findings of this case are supported by previously published data, demonstrating that pathogenic PH-domain variants can lead to microcephaly via canonical Wnt-pathway upregulation. In a separate study, we reported that the autophagy scaffolding protein WDFY3 is required for cerebral cortical size regulation in mice, by controlling proper division of neural progenitors. Here, we show that proliferating cortical neural progenitors of human embryonic brains highly express WDFY3, further supporting a role for this molecule in the regulation of prenatal neurogenesis. We present data on Wnt-pathway dysregulation in Wdfy3-haploinsufficient mice, which display macrocephaly and deficits in motor coordination and associative learning, recapitulating the human phenotype. Consequently, we propose that in humans WDFY3 loss-of-function variants lead to macrocephaly via downregulation of the Wnt pathway. The gamma-aminobutyric acid (GABA) receptor, RDL, plays important roles in neuronal signalling and is the target of highly effective insecticides. A mutation in RDL, commonly A296S, underlies resistance to several insecticides such as cyclodienes. Even though the use of cyclodienes has been banned, the occurrence of mutations substituting A296 is notably high in mosquitoes from several countries. Here we report a survey investigating the prevalence of the Rdl mutant allele in mosquitoes from Laos, a country where mosquito-borne diseases such as malaria and dengue fever are health concerns. Anopheles and Aedes mosquitoes were collected from twelve provinces in Laos. Adult bioassays on Ae. aegypti (Linnaeus) (Diptera: Culicidae) and Ae. albopictus (Skuse) showed that all the populations tested were susceptible to dieldrin (4%) following WHO protocols. Exon 7 from a total of 791 mosquitoes was sequenced to identify the amino acid encoded for at 296 of RDL. Only one of these mosquitoes, Anopheles maculatus rampae (Diptera: Culicidae) from Attapeu, carried the mutant allele being heterozygous for A296S. We therefore found a general lack of the Rdl mutant allele indicating that mosquitoes from Laos are not exposed to insecticides that act on the GABA receptor compared to mosquitoes in several other countries. Identifying the prevalence of the Rdl mutation may help inform the potential use of alternative insecticides that act on the GABA receptor should there be a need to replace pyrethroids in order to prevent/manage resistance.

The purpose of this transcendental phenomenological study was to describe the experiences of teachers who form positive relationships with at-risk students in Title I middle schools. The theory guiding this study was Nel Nodding’s ethic of care as it relates to the teacher as a caregiver interested in the expressed needs of the students. Using a qualitative transcendental phenomenological design, 15 southeastern Title I middle school teachers were selected to participate in the study. The central question guiding this research was: How do teachers describe their experiences of forming positive relationships with at-risk students in Title I middle schools? Sub-questions to further refine the central question were: (a) What perceptions do teachers have about the factors that contribute to building and maintaining positive relationships with at-risk students? (b) How does modeling an ethic of care impact teachers’ relationships with at-risk students? (c) When modeling of an ethic of care, what do teachers perceive as the specific behaviors that help create a sense of security or belonging for at-risk students? Data collection included face to face interviews, a questionnaire, photo narratives, and a focus group. Data analysis used the Moustakas (1994) method for transcendental phenomenological research. This included the epoche, listing significant statements, writing a textural description, structural description, and composite description.

Purpose: The craniosynostoses are characterized by premature fusion of one or more cranial sutures. The relative contribution of previously reported genes to craniosynostosis in large cohorts is unclear. Here we report on the use of a massively parallel sequencing panel in individuals with craniosynostosis without a prior molecular diagnosis. Methods: A 20-gene panel was designed based on the genes’ association with craniosynostosis, and clinically validated through retrospective testing of an Australian and New Zealand cohort of 233 individuals with craniosynostosis in whom previous testing had not identified a causative variant within FGFR1-3 hot-spot regions or the TWIST1 gene. An additional 76 individuals were tested prospectively. Results: Pathogenic or likely pathogenic variants in non-FGFR genes were identified in 43 individuals, with diagnostic yields of 14% and 15% in retrospective and prospective cohorts, respectively. Variants were identified most frequently in TCF12 (N = 22) and EFNB1 (N = 8), typically in individuals with nonsyndromic coronal craniosynostosis or TWIST1-negative clinically suspected Saethre–Chotzen syndrome. Clinically significant variants were also identified in ALX4, EFNA4, ERF, and FGF10. Conclusion: These findings support the clinical utility of a massively parallel sequencing panel for craniosynostosis. TCF12 and EFNB1 should be included in genetic testing for nonsyndromic coronal craniosynostosis or clinically suspected Saethre–Chotzen syndrome.

Background: Glycosylphosphatidylinositol biosynthesis defects (GPIBDs) cause a group of phenotypically overlapping recessive syndromes with intellectual disability, for which pathogenic mutations have been described in 16 genes of the corresponding molecular pathway. An elevated serum activity of alkaline phosphatase (AP), a GPI-linked enzyme, has been used to assign GPIBDs to the phenotypic series of hyperphosphatasia with mental retardation syndrome (HPMRS) and to distinguish them from another subset of GPIBDs, termed multiple congenital anomalies hypotonia seizures syndrome (MCAHS). However, the increasing number of individuals with a GPIBD shows that hyperphosphatasia is a variable feature that is not ideal for a clinical classification. Methods: We studied the discriminatory power of multiple GPI-linked substrates that were assessed by flow cytometry in blood cells and fibroblasts of 39 and 14 individuals with a GPIBD, respectively. On the phenotypic level, we evaluated the frequency of occurrence of clinical symptoms and analyzed the performance of computer-assisted image analysis of the facial gestalt in 91 individuals. Results: We found that certain malformations such as Morbus Hirschsprung and diaphragmatic defects are more likely to be associated with particular gene defects (PIGV, PGAP3, PIGN). However, especially at the severe end of the clinical spectrum of HPMRS, there is a high phenotypic overlap with MCAHS. Elevation of AP has also been documented in some of the individuals with MCAHS, namely those with PIGA mutations. Although the impairment of GPI-linked substrates is supposed to play the key role in the pathophysiology of GPIBDs, we could not observe gene-specific profiles for flow cytometric markers or a correlation between their cell surface levels and the severity of the phenotype. Background: Childhood obesity is a public health problem with significant long-term implications and racial/ethnic disparities. African American extended family members play a significant role in child rearing and socialization, and research suggests that grandparents, in general, may influence children’s weight-related behaviors. There is, however, a lack of research exploring how urban African American children’s relationships with extended family members may influence children’s weight-related behaviors. Therefore, this study examines how extended family members’ roles and responsibilities may influence urban African American children’s weight-related behaviors, how extended family members socialize children to adopt weight-related behaviors, and how extended family members’ socialization practices may differ from those of primary caregivers. Methods: This study builds upon and extends the work of a previous, mixed-methods study of 31 primary caregiver-child dyads, which was designed to examine household and neighborhood factors related to childhood obesity. In Phase 2, individual semi-structured, in-depth interviews were conducted with 8 Baltimore City children; paired interviews were conducted with their primary caregivers and one adult member of each child’s extended family. Manuscript 1 combines qualitative data from both studies to present case studies of the 4 families that participated in both studies. Manuscripts 2 and 3 focus on data collected from Phase 2’s 8 family units, and present detailed analyses of familial influences on children’s physical activity and dietary behaviors, respectively. African Americans, particularly those who reside in rural areas, are at increased risk of developing several preventable health conditions, including type 2 diabetes, cardiovascular disease, and overweight/obesity. Because of several professional and personal experiences, I was inspired to use this thesis as an opportunity to propose an intervention that addresses these preventable health conditions. The proposed intervention, Project HEAL (Healthy Eating and Activity for Life), is a faith-based, theory driven education and comprehensive lifestyle management intervention for rural African Americans. This program is of public health importance because it may be particularly useful in positively impacting the health of rural African Americans, a traditionally hard-to-reach population, and in significantly reducing racial/ethnic and geographic health disparities.Project HEAL is informed by the Social Cognitive Theory and Health Belief Model. These theories were chosen because of their emphasis on self-efficacy, the individual, and the environment. Much of the content of Project HEAL was derived from the curriculum of the evidence-based lifestyle modification intervention of the Diabetes Prevention Program. The Project HEAL curriculum is a modified version that addresses specific constructs of the previously mentioned theories and that is more appropriate for rural African Americans churches and for small group settings. To ensure a comprehensive evaluation of the program, Project HEAL's evaluation strategy will be guided by the Reach, Efficacy, Adoption, and Implementation, and Maintenance framework for health behavior programs.Project HEAL will be implemented with the guidance of an advisory committee and with the help of lay health educators. Therefore, while this thesis describes the Project HEAL development and implementation in detail, there are several aspects of the program will need additional modifications to suit the participating church and its congregants.

African Americans, particularly those who reside in rural areas, are at increased risk of developing several preventable health conditions, including type 2 diabetes, cardiovascular disease, and overweight/obesity. Because of several professional and personal experiences, I was inspired to use this thesis as an opportunity to propose an intervention that addresses these preventable health conditions. The proposed intervention, Project HEAL (Healthy Eating and Activity for Life), is a faith-based, theory driven education and comprehensive lifestyle management intervention for rural African Americans. This program is of public health importance because it may be particularly useful in positively impacting the health of rural African Americans, a traditionally hard-to-reach population, and in significantly reducing racial/ethnic and geographic health disparities.Project HEAL is informed by the Social Cognitive Theory and Health Belief Model. These theories were chosen because of their emphasis on self-efficacy, the individual, and the environment. Much of the content of Project HEAL was derived from the curriculum of the evidence-based lifestyle modification intervention of the Diabetes Prevention Program. The Project HEAL curriculum is a modified version that addresses specific constructs of the previously mentioned theories and that is more appropriate for rural African Americans churches and for small group settings. This evaluation examines the effect of the Atlanta Urban League�s Housing Program on low income African Americans� ability to secure and obtain homes. The Atlanta Urban League has several programs that assist African Americans with becoming homeowners. This evaluation focuses on the housing program�s ability to assist the target population with securing and maintaining home ownership through implementation of a one-day buying seminar. The sample consists of participants who attended the home buying seminar for the year 2001 to 2002. Participants were selected from the Atlanta Urban League�s database. The purpose was to see how the Atlanta Urban League met its goals of: (1) increasing knowledge about the mortgage filing process, (2) participants� ability to secure new housing within six months of completing the seminar, and (3) new homeowners� ability to maintain stable homeownership six months after buying their home.
