1. Same-day discharge protocol for laparoscopic treatment of adnexal disease: management and acceptance

Abstract: Objective: Develop a ‘same-day discharge’ setting for laparoscopic treatment of adnexal disease. Setting: Preventive Gynecology, European Institute of Oncology, Milan, Italy. Population: Two hundred patients undergoing laparoscopic procedures. Material and methods: Data were retrospectively collected through clinical, surgical and laboratory reports. After discharge patients were contacted by phone and e-mail. Main outcome measures: The rate of discharge, adverse events and readmission was measured. The need for adjunctive care provided by our on-call service or by a primary care physician and the acceptability of the same-day discharge protocol were also investigated. Results: One hundred and sixty-five patients out of 200 were discharged on the same day. Of the 35 patients hospitalized, the most frequent causes for overnight admission were: uncontrolled pain, surgical length or complexity of the procedure in nine patients, nausea/vomit in four patients. One hundred and one out of 200 patients answered the mailed questionnaire. None of the discharged patients were readmitted. Eighty-five percent of the answering patients evaluated the length of their hospital stay as adequate or moderately adequate. Ninety-two percent of the patients would recommend the day surgery to other patients. Conclusions: our experience demonstrates that the same-day discharge protocol for laparoscopic treatment of adnexal disease is safe and acceptable.

The 100 keV antiproton beam delivered by the AD/ELENA facility was further decelerated with a pulsed drift tube. A 9 MeV electron beam from a linear accelerator produced a low energy positron beam. The positrons were accumulated in a set of two Penning-Malmberg traps. The positronium target cloud resulted from the conversion of the positrons extracted from the traps. The antiproton beam was steered onto this positronium cloud to produce the antiatoms. Abstract: In Computer Graphics and Computer Vision, shape co-segmentation and shape-matching are fundamental tasks with diverse applications, from statistical shape analysis to human-robot interaction. These problems respectively target establishing segment-to-segment and point-to-point correspondences between shapes, which are crucial task for numerous practical scenarios. Notably, co-segmentation can aid in point-wise correspondence estimation in shape-matching pipelines like the functional maps framework. Our paper introduces an innovative shape segmentation pipeline which provides coherent segmentation for shapes within the same class. Through comprehensive evaluation on a diverse test set comprising shapes from various datasets and classes, we demonstrate the coherence of our segmentation approach. Moreover, our method significantly improves accuracy in shape matching scenarios, as evidenced by comparisons with the original functional maps approach. Importantly, these enhancements come with minimal computational overhead. Our work not only introduces a novel coherent segmentation method and a valuable tool for improving correspondence accuracy within functional maps, but also contributes to the theoretical foundations of this impactful field, inspiring further research.

7. Adjoint Bijective ZoomOut: Efficient Upsampling for Learned Linearly-invariant Embedding

Abstract: In this paper, we present a novel method for refining correspondences between 3D point clouds. Our method is compatible with the functional map framework, so it relies on the spectral representation of the correspondence. Although, differently from other similar approaches, this algorithm is specifically for a particular functional setting, being the only refinement method compatible with a recent data-driven approach, more suitable for point cloud matching. Our algorithm arises from a different way of converting functional operators into point-to-point correspondence, which we prove to promote bijectivity between maps, exploiting a theoretical result. Iterating this procedure and performing spectral upsampling in the same way as other similar methods, ours increases the accuracy of the correspondence, leading to more bijective correspondences. We tested our method over different datasets. It outperforms the previous methods in terms of map accuracy in all the tests considered.

Abstract: The paper examines the provisions of the new public procurement code on contracts relating to cultural heritage. First of all, the research focuses on the profiles of continuity and discontinuity in the legal discipline of the public contracts in this field, comparing both the previous and the actual code. Then, the analysis moves on the field of application, especially about the prohibition of availment, and on the qualification of the executors. In the end, the paper deals with the planning, the variations and the testing, and finally on the special forms of public-private partnership.

Abstract: We report on the first production of an antihydrogen beam by charge exchange of 6.1 keV antiprotons with a cloud of positronium in the GBAR experiment at CERN. The antiproton beam was delivered by the AD/ELENA facility. The positronium target was produced from a positron beam itself obtained from an electron linear accelerator. We observe an excess over background indicating antihydrogen production with a significance of 3-4 standard deviations.

Abstract: Cell membrane perturbation is a common way to stimulate cells by using external actuators. Recently, nanotechnology has added a number of new strategies for doing this, enlarging the scope and the range of mechanisms involved. Here, we describe a number of possible perturbation actions that are driven by light, and we try to capture the underlying phenomena. The discussion is based on the simple equivalent circuit model for the cell membrane.

Abstract: We present a description of the GBAR positron (e+) trapping apparatus, which consists of a three stage Buffer Gas Trap (BGT) followed by a High Field Penning Trap (HFT), and discuss its performance. The overall goal of the GBAR experiment is to measure the acceleration of the neutral antihydrogen (H¯) atom in the terrestrial gravitational field by neutralising a positive antihydrogen ion (H¯+), which has been cooled to a low temperature, and observing the subsequent H¯ annihilation following free fall. To produce one H¯+ ion, about 1010 positrons, efficiently converted into positronium (Ps), together with about 107 antiprotons (p¯), are required. The positrons, produced from an electron linac-based system, are accumulated first in the BGT whereafter they are stacked in the ultra-high vacuum HFT, where we have been able to trap 1.4(2) × 109 positrons in 1100 s.

Abstract: Clinical features and natural history of coronavirus disease 2019 (COVID-19) differ widely among different countries and during different phases of the pandemia. Here, we aimed to evaluate the case fatality rate (CFR) and to identify predictors of mortality in a cohort of COVID-19 patients admitted to three hospitals of Northern Italy between March 1 and April 28, 2020. All these patients had a confirmed diagnosis of SARS-CoV-2 infection by molecular methods. During the study period 504/1697 patients died; thus, overall CFR was 29.7%. We looked for predictors of mortality in a subgroup of 486 patients (239 males, 59%; median age 71 years) for whom sufficient clinical data were available at data cut-off. Among the demographic and clinical variables considered, age, a diagnosis of cancer, obesity and current smoking independently predicted mortality. When laboratory data were added to the model in a further subgroup of patients, age, the diagnosis of cancer, and the baseline PaO2/FiO2 ratio were identified as independent predictors of mortality. In conclusion, the CFR of hospitalized patients in Northern Italy during the ascending phase of the COVID-19 pandemic approached 30%. The identification of mortality predictors might contribute to better stratification of individual patient risk.

Abstract: Coronavirus disease 2019 (COVID-19) is a major worldwide threat caused by a novel coronavirus, severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), rapidly spreading to a global pandemic. As of May 11, 2020, 4,176,346 cases have been reported worldwide, 219,814 in Italy, and of them, 81,871 occurred in the Lombardy region.1 Although the respiratory manifestations of COVID-19 have been widely described, the impact on the gastrointestinal (GI) system remains less clear. The reported prevalence of digestive symptoms ranges from 3% to 79%, depending on the setting,2–5 but data on GI endoscopic and histologic findings in COVID-19 patients are lacking. Therefore, the aim of this study is to describe the GI endoscopic and histologic findings in COVID-19 patients.

Abstract: For the GBAR (Gravitational Behaviour of Antihydrogen at Rest) experiment at CERN's Antiproton Decelerator (AD) facility we have constructed a source of slow positrons, which uses a low-energy electron linear accelerator (linac). The driver linac produces electrons of 9 MeV kinetic energy that create positrons from bremsstrahlung-induced pair production. Staying below 10 MeV ensures no persistent radioactive activation in the target zone and that the radiation level outside the biological shield is safe for public access. An annealed tungsten-mesh assembly placed directly behind the target acts as a positron moderator. The system produces 5 x 10(7) slow positrons per second, a performance demonstrating that a low-energy electron linac is a superior choice over positron-emitting radioactive sources for high positron flux.

Abstract: The GBAR (Gravitational Behavior of Antihydrogen at Rest) experiment at CERN requires efficient deceleration of 100 keV antiprotons provided by the new ELENA synchrotron ring to synthesize antihydrogen. This is accomplished using electrostatic deceleration optics and a drift tube that is designed to switch from -99 kV to ground when the antiproton bunch is inside – essentially a charged particle “elevator” – producing a 1 keV pulse. We describe the simulation, design, construction and successful testing of the decelerator device at -92 kV on-line with antiprotons from ELENA.

Abstract: ALS (amyotrophic lateral sclerosis), the most common motor neuron disease, causes muscle denervation and rapidly fatal paralysis. While motor neurons are the most affected cells in ALS, studies on the pathophysiology of the disease have highlighted the importance of non-cell autonomous mechanisms, which implicate astrocytes and other glial cells. In ALS, subsets of reactive astrocytes lose their physiological functions and become toxic for motor neurons, thereby contributing to disease pathogenesis. Evidence of astrocyte contribution to disease pathogenesis are well established in cellular and animal models of familial ALS linked to mutant SOD1, where astrocytes promote motor neuron cell death. The mechanism underlying astrocytes reactivity in conditions of CNS injury have been shown to involve the MTOR pathway. However, the role of this conserved metabolic signaling pathway, and the potential therapeutic effects of its modulation, have not been investigated in ALS astrocytes. Here, we show elevated activation of the MTOR pathway in human-derived astrocytes harboring mutant SOD1, which results in inhibition of macroautophagy/autophagy, increased cell proliferation, and enhanced astrocyte reactivity. We demonstrate that MTOR pathway activation in mutant SOD1 astrocytes is due to post-transcriptional upregulation of the IGF1R (insulin like growth factor 1 receptor), an upstream positive modulator of the MTOR pathway. Importantly, inhibition of the IGF1R-MTOR pathway decreases cell proliferation and reactivity of mutant SOD1 astrocytes, and attenuates their toxicity to motor neurons. Abstract: Background: Mixed symptoms in depression may underlie bipolar diathesis rather than unipolarity. Uncovering mixed depression (MxD) is crucial for appropriate management, especially in the perinatal period, as it may affect treatment planning and impact future child development. We used a scale specific for identifying MxD and tested its validity in pregnant and postpartum women with depression. Methods: Women developing a major depressive episode (MDE) during their perinatal period extending from pregnancy to one year postpartum from November-2012 through June-2019 were assessed with BPRS-18, EPDS, CGI-S, GAF, HAM-A, HAM-D, Koukopoulos’ Mixed Depression Rating Scale (KMDRS), TEMPS, and YMRS. They were classified, based on KMDRS criteria, as with mixed (MxD) or without (nonMxD) mixed symptoms. We conducted ROC analysis and performed factor analysis of the KMDRS. Results: Of 45 included, MxD (N = 19) were biased towards diagnosis of bipolar disorder and nonMxD (N = 26) towards major depressive disorder. Other sociodemographic variables did not differ significantly between MxD and nonMxD. MxD scored higher on total YMRS, BPRS, and KMDRS, and on KMDRS-6 Subjective Feelings of Irritability and KMDRS-12 Suicidal Impulsiveness items. The KMDRS correlated in the entire sample, in MxD and nonMxD, with the YMRS and the BPRS, while correlating with the HAM-D in nonMxD only. The KMDRS showed acceptable AUC distribution, with a 68% sensitivity and 58% specificity. Best-fit was three-factor-structure, explaining 54.66% of cumulative variance. Limitations: Small sample and cross-sectional design. Conclusions: The KMDRS is fit for investigating MxD along with the YMRS and the BPRS in perinatal women with a MDE.

Abstract: The usage of magnetic nanoparticles (NPs) in applications necessitates a precise mastering of their properties at the single nanoparticle level. There has been a lot of progress in the understanding of the magnetic properties of NPs, but incomparably less when interparticle interactions govern the overall magnetic response. Here, we present a quantitative investigation of magnetic fields generated by small clusters of NPs assembled on a dielectric non-magnetic surface. Structures ranging from individual NPs to fifth-fold particulate clusters are investigated in their magnetization saturation state by magnetic force microscopy and numerical calculations. It is found that the magnetic stray field does not increase proportionally with the number of NPs in the cluster. Both measured and calculated magnetic force fields underline the great importance of the exact spatial arrangement of NPs, shedding light on the magnetic force field distribution of particulate clusters, which is relevant for the quantitative evaluation of their magnetization and perceptibly for many applications.

Abstract: The GBAR experiment aims to measure the gravitational acceleration of antihydrogen (H) over bar. It will use (H) over bar (+) ions formed by the interaction of antiprotons with a dense positronium cloud, which will require about 1010 positrons to produce one (H) over bar (+). We present the first results on the positron accumulation, reaching 3.8 +/- 0.4x10(8) e(+) collected in 560 s.

Abstract: We have developed a PbWO4 (PWO) detector with a large dynamic range to measure the intensity of a positron beam and the absolute density of the ortho-positronium (o-Ps) cloud it creates. A simulation study shows that a setup based on such detectors may be used to determine the angular distribution of the emission and reflection of o-Ps to reduce part of the uncertainties of the measurement. These will allow to improve the precision in the measurement of the cross-section for the (anti)hydrogen formation by (anti)proton-positronium charge exchange and to optimize the yield of antihydrogen ion which is an essential parameter in the GBAR experiment.

Abstract: Background: Depression is an important risk factor for suicide. However, other dimensions may contribute to the suicidal risk and to the transition from ideas to acts. We aimed to test the relative involvement of hopelessness, temperament, childhood trauma, and aggression in suicide risk in a large sample of patients with mood disorders. Methods: We assessed 306 patients with major depressive and bipolar disorders for clinical characteristics including hopelessness, temperament, childhood trauma, and aggression. We tested their associations with suicidal ideation and acts using standard univariate/bivariate methods, followed by multivariate logistic regression models. Results: In multivariate analyses, the loss of expectations subscore of the hopelessness scale was associated with lifetime suicidal ideation but not suicide attempt. Childhood emotional abuse, severity of current depression, and female gender were associated with lifetime suicide attempts, whereas hyperthymic temperament was protective. Only hyperthymic temperament differentiated patients with a history of suicidal ideas vs. those with a history of suicide attempt. Conclusions: Findings support the association of hopelessness with suicidal ideation and point to considering in suicidal acts not only depression, but also childhood emotional abuse, hyperthymic temperament, and gender.

Lo scritto esamina i contenuti del decreto, segnalando in particolare che essi appaiono senz’altro adeguati al fine di garantire la partecipazione dei cittadini, e dunque l’attuazione di una forma di democrazia deliberativa in materia di opere pubbliche, mentre si può dubitare che risultino sempre funzionali a prevenire i conflitti di prossimità.

Abstract: The essay deals with the phenomenon of ‘new philanthropy’ from a juridical point of view, wondering if Italian civil law that rules the contract of donation is able to reach a fair balance between the interests of donors and those of donees. Some legal scholars reckon that there are too many limits to the act of donation, such as those arising from the constraint of a solemn form and above all those related to forced heirship (‘successione necessaria’), according to which an indefeasible portion of the estate passes to the deceased’s next-of-kin. On the contrary, the author argues the reasons why these limits aim, on the one hand, to make the donor aware of the seriousness of the choice of undergoing a pauperization, and, on the other, to fulfil the constitutional protection of the nuclear family. Moreover, although forced heirs can use a specific legal action (called ‘azione di riduzione’) that permits them to restore the reserved portion, donations can be abated only after the testamentary dispositions exceeding the disposable portion have been reduced.

Abstract: Nell’ambito delle società altomedievali, il dono, l’atto di liberalità, non risponde completamente a logiche di gratuità, ma, come l’antropologia moderna insegna a proposito delle società “primitive”, si inserisce in un fitto reticolo di relazioni intersoggettive. Un esempio emblematico in tal senso può essere rappresentato dal fenomeno delle donationes pro anima, tipico della civiltà longobarda fra VII e VIII secolo, che si segnala per la natura ‘comunitaria’ dell’atto dispositivo, ma che, al tempo stesso, apre (o meglio: riapre) la strada a forme di successione volontaria che sembravano essere scomparse con la civilitas romana a vantaggio di regole improntate ad una dimensione super-individuale della vita associata., In the early medieval society, the gift, the donation, does not fully respond to the logic of gratuitousness, but, as modern anthropology teaches about the "primitive" societies, is part of a dense network of interpersonal relationships. An emblematic exemple in this sense can be represented by the case of the donationes pro anima, typical of Lombard civilization between VII and VIII century, which is notable for the 'Communal’ nature of this act, but which, at the same time, opens (or rather re-opens) the way to forms of voluntary succession that seemed to have disappeared with the Roman civilitas, for the benefit of rules inspired by a super-individual dimension of social life.

Abstract: Background: Cyclicity is the essential feature of Bipolar disorder, but the effect of different cycle patterns on the clinical features is poorly understood. Moreover, no studies investigated the relationship between mania and depression inside the manic-depressive cycle. Objective: The aim of this study is to verify the presence of a relationship between the manic and the depressive phase during the course of bipolar disorder Method: 160 consecutive patients with BD type I were recruited and followed for a mean period of 10 years. During the follow-up period, four types of euthymic phases were collected: free intervals present between a depressive and a manic/hypomanic episode (D-M); free intervals present between a manic/hypomanic and a depressive episode (M-D); free intervals present between two depressive episodes (D-D); free intervals present between two manic/hypomanic episodes (M-M). One-way ANOVA using the groups as independent variable and the duration of the free intervals as dependent variables was used. Furthermore, ANOVA was followed by Fisher's Protected Least Significant Difference post-hoc test to measure between-group differences. Results: M-D-free interval phases were shorter than D-M-free intervals. M-D intervals were the shortest ones, the D-D and D-M did not differ, and the M-M were the longest. Conclusion: The strict temporal link between manic and depressive phases supports the idea that the manic-depressive cycle usually begins with a manic episode, and that the subsequent depression is often the consequence of subsiding mania.

Abstract: This essay addresses from a philosophical-legal perspective the concept of ‘gift’ and its close relation with the communitarian dimension. Starting from the theory elaborated by René Girard, in particular his idea of ‘scapegoat’, it is possible to highlight three dimensions of the gift: the gift as a social fundament, the gift as a violence dimension and the gift as a politicalinstitutional bond. This theoretical model allows to compare the gift and the communitarian dimension, paradigmatically the Greek experience and the early Christian communities, and to clarify the dialectical relation between the nexus gift-community and law (also with regard to New Philanthropy). In spite of the current opposition between donative agency and strategicinstrumental reason, the relation gift-law should be understood in a more complex manner. Having an immediate and direct social-institutional projection, gift intrinsically ‘creates law’: more widely it implies the implementation of a political-legal bond.

Abstract: The aim of the GBAR (Gravitational Behavior of Antimatter at Rest) experiment is to measure the free fall acceleration of an antihydrogen atom, in the terrestrial gravitational field at CERN and therefore test the Weak Equivalence Principle with antimatter. The aim is to measure the local gravity with a 1% uncertainty which can be reduced to few parts of 10(-3).

Abstract: Background: Cyclicity is the essential feature of Bipolar disorder, but the effect of different cycle patterns on the clinical features is poorly understood. Moreover, no studies investigated the relationship between mania and depression inside the manic-depressive cycle. Objective: The aim of this study is to verify the presence of a relationship between the manic and the depressive phase during the course of bipolar disorder Method: 160 consecutive patients with BD type I were recruited and followed for a mean period of 10 years. During the follow-up period, four types of euthymic phases were collected: free intervals present between a depressive and a manic/hypomanic episode (D-M); free intervals present between a manic/hypomanic and a depressive episode (M-D); free intervals present between two depressive episodes (D-D); free intervals present between two manic/hypomanic episodes (M-M). One-way ANOVA using the groups as independent variable and the duration of the free intervals as dependent variables was used. Furthermore, ANOVA was followed by Fisher's Protected Least Significant Difference post-hoc test to measure between-group differences. Results: M-D-free interval phases were shorter than D-M-free intervals. M-D intervals were the shortest ones, the D-D and D-M did not differ, and the M-M were the longest. Abstract: The GBAR project (Gravitational Behaviour of Anti hydrogen at Rest) at CERN, aims to measure the free fall acceleration of ultracold neutral anti hydrogen atoms in the terrestrial gravitational field. The experiment consists preparing anti hydrogen ions (one antiproton and two positrons) and sympathetically cooling them with Be (+) ions to less than 10 mu K. The ultracold ions will then be photo-ionized just above threshold, and the free fall time over a known distance measured. We will describe the project, the accuracy that can be reached by standard techniques, and discuss a possible improvement to reduce the vertical velocity spread.

Abstract: This technology review expresses the current view of the European Society of Gastrointestinal Endoscopy (ESGE) on the available techniques for intraductal biliopancreatic imaging. The three cholangioscopy techniques are described: the "dual-operator" and " single-operator" mother-baby approaches using dedicated instruments, and the "direct" technique using currently available ultrathin gastroscopes. The mother-baby method is standardized and reproducible, while direct cholangioscopy is technically demanding and its safety requires further evaluation. As well as direct visualization of the bile ducts, cholangioscopy has the further advantage of allowing targeted biopsy. Image quality is still suboptimal for single-operator cholangioscopy, while the other techniques have achieved adequately detailed imaging. The costs of mother-baby cholangioscopy are high and its application in clinical practice should be restricted to selected cases (i.e. indeterminate biliary strictures/intraluminal lesions, difficult biliary stones) and to the setting of tertiary care centers. Peroral pancreatoscopy may find an indication in situations where other imaging modalities (mainly EUS) are inconclusive (i.e. delineation of main duct intraductal papillary mucinous neoplasia extension, sampling of indeterminate main pancreatic duct strictures). Intraductal ultrasonography (IDUS) has a poorer performance than EUS in the staging of pancreatic malignancies and can increase the risk of pancreatitis. A promising indication for IDUS could be the evaluation of indeterminate biliary strictures and ampullary tumors. Probe-based confocal laser endomicroscopy (pCLE) of the bile ducts is a difficult and expensive technique. Appropriate training needs to be established, since interpretation of images is challenging. pCLE can be an important diagnostic tool in the setting of indeterminate biliary strictures.

Abstract: A 3-month-old girl was admitted to the hospital because of hypotonia and frequent vomiting. She had severe metabolic acidosis and her liver function was abnormal. Hepatomegaly and rapidly progressive liver failure developed, and she died at 4 months of age. Two half-siblings from a different mother had died in infancy of an undiagnosed myopathy. The liver was fatty and hepatocytes were filled with large and small lipid droplets. Other tissues were morphologically normal. The respiratory chain enzymes containing subunits encoded by mitochondrial DNA were markedly decreased in liver, partially decreased in muscle, but normal in other tissues. Southern blot analysis showed 90% depletion of mitochondrial DNA in liver, 53% depletion in muscle, and normal amounts in other tissues. This is the second case of fatal infantile liver failure associated with mitochondrial DNA depletion. This pathogenetic mechanism should be considered in infants with multiple respiratory chain defects and variable tissue expression. © 1992 Mosby-Year Book, Inc.

Abstract: We documented the presence of a newly described point mutation in the tRNA Leu(UUR) gene of mitochondrial DNA in five postmortem tissues from a patient with MELAS syndrome. The mutation was heteroplasmic, but the percentage of mutant genomes was similar (79 to 88%) in both clinically affected and unaffected tissues. © 1991 American Academy of Neurology.

Abstract: We studied a large family with a dominantly inherited mitochondrial myopathy characterized by progressive external ophthalmoplegia, dysphagia, cataract, lactic acidosis, exercise intolerance, and early death. Morphologic studies of muscle biopsies suggested mitochondrial heteroplasmy and revealed ragged-red fibers and decreased histochemical reactions for cytochrome c oxidase and succinate dehydrogenase. Biochemistry showed a partial defect of cytochrome c oxidase and a mild generalized reduction of other mitochondrial enzymes requiring mitochondrial DNA-encoded subunits. Southern blot analysis and PCR amplification showed mitochondrial DNA deletions in muscle of all affected members, but not in lymphocytes or fibroblasts, suggesting a tissue-specific distribution. Deletions were multiple and seemed to increase with time and to correlate with the severity of the disease. © 1991 American Academy of Neurology.
