Abstract: 19 augustus 2023, Contains fulltext : 296246.pdf (Publisher’s version ) (Open Access), Welfare landscapes are key sites for the everyday enactment and negotiation of citizenship, but they have remained relatively marginal in debates about migrants' belonging in postcolonial Europe. Drawing on ethnographic studies of Egyptian migrant parents' engagements with the welfare state in Paris, Amsterdam and Milan, this paper examines how citizenship is enacted, negotiated and experienced in welfare encounters. In contrast to the racialised citizenship produced by public discourses and bordering institutions, such welfare encounters articulate a social citizenship built on the idea that all citizens and legalised residents have a right to social welfare. This social citizenship is worked out in concrete encounters that revolve around material provisions and assistance, which are often deeply personal and intimate. Such encounters do not transact abstract notions of the citizen but draw on dense ideas and valuations of good lives, roles, responsibilities and deservingness on the part of welfare actors and beneficiaries.

Par la mise en perspective de trois enquêtes de terrain réalisées avec des professionnel·les et des bénévoles du soutien à la parentalité à Amsterdam, Milan et Paris, nous montrons comment cette forme de gouvernement est mise en œuvre et comment elle fait émerger de nouveaux enchevêtrements aux effets incontrôlables. Plus précisément, nous montrons comment les politiques sociales de proximité : (1) empêtrent les professionnel·les du welfare dans des réseaux locaux complexes, (2) brouillent la frontière entre les professionnel·les et les citoyen·nes ainsi qu’entre l’État et la société civile, (3) s’appuient sur des relations personnalisées et du travail affectif. La réorganisation de l’intervention sociale via des maillages territoriaux locaux produit un écheveau inextricable de réseaux et de partenariats. En outre, plutôt que de susciter des communautés résilientes qui prendraient soin d’elles-mêmes, les nouveaux programmes de protection sociale impliquent de plus en plus de citoyen·nes dans la mise en œuvre des modes de gouvernance qui les visent. Abstract: ABSTRACT ChatGPT, a language-learning model chatbot, has garnered considerable attention for its ability to respond to users’ questions. Using data from 14 countries and 186 institutions, we compare ChatGPT and student performance for 28,085 questions from accounting assessments and textbook test banks. As of January 2023, ChatGPT provides correct answers for 56.5 percent of questions and partially correct answers for an additional 9.4 percent of questions. When considering point values for questions, students significantly outperform ChatGPT with a 76.7 percent average on assessments compared to 47.5 percent for ChatGPT if no partial credit is awarded and 56.5 percent if partial credit is awarded. Still, ChatGPT performs better than the student average for 15.8 percent of assessments when we include partial credit. We provide evidence of how ChatGPT performs on different question types, accounting topics, class levels, open/closed assessments, and test bank questions. We also discuss implications for accounting education and research.

Abstract: Digital tools have been increasingly applied in the field of psychology and well-being, such as Virtual Reality (VR), that provides vivid and realistic illusions of environments and stimuli to users’ senses. Currently, one of the possible applications of VR in the field of psychology is the stimulation of creativity. Stimulating creativity might indeed enhance emotional regulation and promote positive feelings, which in turn might influence cognitive processes and past autobiographical memories. The present study aimed at evaluating the benefits and changes related to a creative VR experience on emotion regulation and the decision made during unpleasant past events. 25 healthy volunteers were enrolled and randomly assigned to one of two conditions: 1) painting VR, 2) neutral control condition. Before the experience, all participants were asked to think about an unpleasant past decision. Pre and post the exposure, participants evaluated the past decision and how much they felt the decision was based on System 1 or System 2. Results showed that participants in the creativity condition rated themselves as significantly more curious and felt that their creativity was stimulated compared to the control group. Furthermore, participants in the creative VR changed the perception of the how much they used System 1 in their choices, when controlling for emotional intelligence. Lastly, the creative experience led participants to feel less satisfied with their previous decision, possibly because it induced creative reasoning on possible alternatives. Results highlighted that VR for creativity can enhance creative feelings and shape emotions and individual relationship with decision-making.

Abstract: No robust evidence is provided by literature regarding the management of intracranial hypertension following severe traumatic brain injury (TBI). This is mostly due to the lack of prospective randomized controlled trials (RCTs), the presence of studies containing extreme heterogeneously collected populations and controversial considerations about chosen outcome. A scientific society should provide guidelines for care management and scientific support for those areas for which evidence-based medicine has not been identified. However, RCTs in severe TBI have failed to establish intervention effectiveness, arising the need to make greater use of tools such as Consensus Conferences between experts, which have the advantage of providing recommendations based on experience, on the analysis of updated literature data and on the direct comparison of different logistic realities. The Italian scientific societies should provide guidelines following the national laws ruling the best medical practice. However, many limitations do not allow the collection of data supporting high levels of evidence for intracranial pressure (ICP) monitoring and decompressive craniectomy (DC) in patients with severe TBI. This intersociety document proposes best practice guidelines for this subsetting of patients to be adopted on a national Italian level, along with joint statements from “TBI Section” of the Italian Society of Neurosurgery (SINch) endorsed by the Neuroanesthesia and Neurocritical Care Study Group of the Italian Society of Anesthesia, Analgesia, Resuscitation and Intensive Care (SIAARTI). Presented here is a recap of recommendations on management of ICP and DC supported a high level of available evidence and rate of agreement expressed by the assemblies during the more recent consensus conferences, where members of both groups have had a role of active participants and supporters. Abstract: Rivers serve many societal functions and are one of the most intensively human influenced ecosystems worldwide, and, due to their importance, are included under the monitoring programs of the Water Framework Directive across Europe. Macroinvertebrates play an important role when monitoring running waters for the assessment of their environmental quality due to their reliability as bioindicators and utility in long-term studies. Macroinvertebrates do not constitute a systematic unit but they are formed by a set of different taxa, grouped according to taxonomic ranks, size and habitat preferences. They represent the base of the aquatic food chain, serving as a food source for amphibians, birds, reptiles, fish and humans, and contributing in the organic matter processing. Despite the large amount of data collected on Italian river macroinvertebrates and the increased interest in the study of this group, only few data are available for research scientist and managers. In this paper, we collected and homogenized knowledge on the presence, distribution and abundances of macroinvertebrates taxa inhabiting the River Po catchment (Northern Italy) in the last decade. The data set includes 130,727 records collected between 2007 and 2018 including 143 taxa of macroinvertebrates, mostly identified at family rank level. Moreover, the data set provides information on the geographic distribution of these families and their abundance by sub-catchment, altitude, meso- and micro-habitat.

Abstract: In forensic evaluation of charred corpses, internal detrimental signs may result as more significant of those observed during external examination and is often arduous to state if a victim was exposed to fire before or after death. When the histological analysis of the remaining internal viscera is necessary, the massive destruction caused by the lesion, the charring and the coarctation of the samples don't allow to give further information or to determine the remaining organic components of the viscera. This limit is determined by the intrinsic characteristics of this thermal lesivity of self-maintenance even after the exitus of the subject, worsening the initial detrimental framework. The Authors, with the purpose of improving the microscopic visualization of the samples collected from cadavers with peculiar deterioration, as in case of carbonization, suggest the use of a specific technical protocol based on the use of Sandison's rehydrating solution since the samples treated with this solution showed, at microscopic examination, a substantial histological-morphological improvement.

Abstract: We present a retrospective study of prostitutes’ homicide in the Milanese area over a 26-year period (1993–2018), and compare the results with general women’s homicides: 294 women were killed of whom 71 were prostitutes. In the general population, the type-victim is an Italian woman aged between 31 and 40 years suffering from no particular pathologies or drug dependence. Prostitutes are 10 years younger, mostly Italian, suffering from pathologies probably related to their activities. Prostitutes remain at high risk of violence, due to gaps in the crime prevention system.

Abstract: Bowel perforation is an uncommon complication of the administration of sodium polystirene sulfonate. We describe a case of bowel perforation occurring soon after the performance of a right colectomy in a patient with stage 3 chronic renal failure treated with sodium polystyrene sulfonate. The perforation was revealed during the course of revision surgery. histological examination demonstrated perivisceritis with ischemic wall damage and hemorrhagic mucosa filled with numerous blackish, sharp-angled crystalloids. After reviewing the literature concerning the complications associated with the administration of SPS, we discuss some aspects of professional medical liability with particular reference to the etiopathogenesis of bowel perforation.

Abstract: Contains fulltext : 219585.pdf (Publisher’s version ) (Open Access), This special issue examines welfare programs as sites where Europe's increasingly diverse societies are being shaped and negotiated. It zooms in on parenting as a central governmental domain where concerns about, and hopes for, the future of society intersect with notions of citizenship, family care, welfare, and deservingness of public resources. In this introduction to the special issue, we draw out three paradoxical orders that shape the encounters between migrant parents and welfare actors we have studied. One is concerned with the tension between the universal and difference, the other with the re-articulation of the public and the private, and a third with irreconcilable social and institutional demands. This helps us understand how Europe's diverse societies are being shaped on the ground, beyond the often strongly racialized, nationalist rhetoric that has come to dominate public debates.

The paper introduces the notion of the "intimate public," a spatial and relational milieu for activating an increasingly diverse public in the absence of traditional welfare resources. Through the lens of the intimate public, a number of effects come into view: First, the incitement to participate recasts the private as simultaneously a source of new forms of solidarity and a potential obstacle to social cohesion and integration. Second, assumptions about the organic and sustainable nature of citizen-based solidarity effectively erase the affective labor required to produce and sustain it, especially in marginalized neighborhoods. Finally, the intense relationality that characterizes the intimate public produces uncertainty and entanglements that bind professionals and participants in compelling ways.

It zooms in on parenting as a central governmental domain where concerns about, and hopes for, the future of society intersect with notions of citizenship, family care, welfare, and deservingness of public resources. In this introduction to the special issue, we draw out three paradoxical orders that shape the encounters between migrant parents and welfare actors we have studied. One is concerned with the tension between the universal and difference, the other with the re-articulation of the public and the private, and a third with irreconcilable social and institutional demands. This helps us understand how Europe's diverse societies are being shaped on the ground, beyond the often strongly racialized, nationalist rhetoric that has come to dominate public debates.

Abstract: Literature on electrocution in Italy is limited. The authors have focused their attention on the use of scanning electron microscope/energy-dispersive X-ray analysis in order to evaluate the current mark even on paraffin-embedded samples. A total of 24,104 autopsies were performed at the Section of Legal Medicine of Milan (1993-2017); all cases of death caused by electrocution (low and high voltage) were selected. We assessed a regular histological examination, a toxicological examination and detected metallisation with energy-dispersive X-ray spectroscopy analysis on paraffin-embedded tissues of typical current marks. We collected 27 high-voltage fatalities and five cases of low-voltage suicide electrocution; the technique revealed itself as sensitive. In all cases, microscopic examination highlighted the typical signs of electric current and thermal damage but the histochemical specific stainings for copper and iron gave negative results. Scanning electron microscope/energy-dispersive X-ray analysis, assessed on these same samples, even when paraffin-embedded, however, enabled us to detect these elements and differentiate between the various metal residues involved in the electrocution process. Based on their experience, authors advise the use of scanning electron microscope/energy-dispersive X-ray analysis on electrocution deaths to provide information for forensic diagnosis.

We show that the reorganization of governance through neighborhood-based networks produces an unwieldy quagmire of networks and partnerships. Moreover, rather than creating self-caring communities, new welfare programs primarily draw increasing numbers into governmental roles. Finally, instead of being released from its welfare and social responsibilities, locally embedded professionals turn out to be particularly effective at bringing the welfare state back in.

Abstract: L’attività del Chirurgo deve svolgersi tra gli intricati binari dei doveri e dei divieti stabiliti dalle varie norme penali, civili e quelle derivanti dai codici deontologici, oltre che dai codici di comportamento aziendali. Quando si incorre in una violazione delle regole si è chiamati a risponderne. La responsabilità nasce dal mancato adempimento dei doveri, salvo che sia provata l’impossibilità di tale adempimento per fattori inevitabili: in generale, le aspettative (non di rado, le pretese) dell’utenza sono accresciute nel tempo, molto di più di quanto la Sanità sia spesso in grado di offrire e di garantire. Con il progredire delle conoscenze e delle competenze sanitarie è aumentata l’insoddisfazione verso le prestazioni ricevute: infatti più attività si è chiamati a svolgere, più aumenta la probabilità di cadere in errore e, più alternative vi sono, più cresce il rischio di non scegliere quella più appropriata e di venire poi chiamati a rispondere della scelta adottata. I Chirurghi sono oggi chiamati sempre più a prendere coscienza di questa problematica e ad affrontarla con lucida fermezza. Questo libro offre una sezione generale, dedicata all’analisi dei vari principi e delle plurime regole della responsabilità professionale sanitaria, con i richiami di legge e dell’alta giurisprudenza, e una sezione speciale, con numerosi casi-tipo peritali chirurgici, formulati ad hoc sull’esperienza maturata nel corso degli anni di concreta pratica professionale nell’ambito del contenzioso sanitario. L’opera, in definitiva, si propone di venir in aiuto ai Colleghi per affrontare i problemi della responsabilità professionale, con qualche timore in meno e qualche conoscenza in più.

Abstract: No robust evidence is provided by literature regarding the management of intracranial hypertension following severe traumatic brain injury (TBI). This is mostly due to the lack of prospective randomized controlled trials (RCTs), the presence of studies containing extreme heterogeneously collected populations and controversial considerations about chosen outcome. A scientific society should provide guidelines for care management and scientific support for those areas for which evidence-based medicine has not been identified. However, RCTs in severe TBI have failed to establish intervention effectiveness, arising the need to make greater use of tools such as Consensus Conferences between experts, which have the advantage of providing recommendations based on experience, on the analysis of updated literature data and on the direct comparison of different logistic realities. The Italian scientific societies should provide guidelines following the national laws ruling the best medical practice. However, many limitations do not allow the collection of data supporting high levels of evidence for intracranial pressure (ICP) monitoring and decompressive craniectomy (DC) in patients with severe TBI. This intersociety document proposes best practice guidelines for this subsetting of patients to be adopted on a national Italian level, along with joint statements from “TBI Section” of the Italian Society of Neurosurgery (SINch) endorsed by the Neuroanesthesia and Neurocritical Care Study Group of the Italian Society of Anesthesia, Analgesia, Resuscitation and Intensive Care (SIAARTI). Presented here is a recap of recommendations on management of ICP and DC supported a high level of available evidence and rate of agreement expressed by the assemblies during the more recent consensus conferences, where members of both groups have had a role of active participants and supporters. Abstract: Background: To date, 750 000 patients with COVID-19 worldwide have required mechanical ventilation and thus are at high risk of acute brain dysfunction (coma and delirium). We aimed to investigate the prevalence of delirium and coma, and risk factors for delirium in critically ill patients with COVID-19, to aid the development of strategies to mitigate delirium and associated sequelae. Methods: This multicentre cohort study included 69 adult intensive care units (ICUs), across 14 countries. We included all patients (aged ≥18 years) admitted to participating ICUs with severe acute respiratory syndrome coronavirus 2 infection before April 28, 2020. Patients who were moribund or had life-support measures withdrawn within 24 h of ICU admission, prisoners, patients with pre-existing mental illness, neurodegenerative disorders, congenital or acquired brain damage, hepatic coma, drug overdose, suicide attempt, or those who were blind or deaf were excluded. We collected de-identified data from electronic health records on patient demographics, delirium and coma assessments, and management strategies for a 21-day period. Additional data on ventilator support, ICU length of stay, and vital status was collected for a 28-day period. The primary outcome was to determine the prevalence of delirium and coma and to investigate any associated risk factors associated with development of delirium the next day. We also investigated predictors of number of days alive without delirium or coma. These outcomes were investigated using multivariable regression. Findings: Between Jan 20 and April 28, 2020, 4530 patients with COVID-19 were admitted to 69 ICUs, of whom 2088 patients were included in the study cohort. Abstract: The morphological analysis of stab wounds may often not be accurate enough to link it with the type of wounding weapon, but a further evaluation may be performed with the search for metallic residues left during the contact between the instrument and the skin. In this study, Scanning Electron Microscopy-Energy Dispersive Spectroscopy (SEM-EDS) was applied to the study of cadaveric stab wounds performed with kitchen knives composed of iron, chromium and nickel, in order to verify the presence of metallic residues on the wound's edge. Two groups of 10 corpses were selected: group A, including victims of stab wounds and a control group B (died of natural causes). Samplings were performed on the lesions and in intact areas of group A, whereas in group B sampling were performed in non-exposed intact skin. Samples were then analysed with optical microscopy and SEM-EDS. In group A, optical microscopic analysis showed the presence of vital haemorrhagic infiltration, while SEM-EDS showed evidence of microscopic metal traces, isolated or clustered, consisting of iron, chromium and nickel. Moreover, in two cases organic residues of calcium and phosphate were detected, as a probable sign of bone lesion. Control samples (group A in intact areas and group B), were negative for the search of exogenous material to optical microscopy and SEM-EDS. The results show the utility and possible application of the SEM-EDS in theidentification of metallic residues from sharp weapons on the skin.

It zooms in on parenting as a central governmental domain where concerns about, and hopes for, the future of society intersect with notions of citizenship, family care, welfare, and deservingness of public resources. In this introduction to the special issue, we draw out three paradoxical orders that shape the encounters between migrant parents and welfare actors we have studied. One is concerned with the tension between the universal and difference, the other with the re-articulation of the public and the private, and a third with irreconcilable social and institutional demands. This helps us understand how Europe's diverse societies are being shaped on the ground, beyond the often strongly racialized, nationalist rhetoric that has come to dominate public debates.

Abstract: 23 februari 2020, Contains fulltext : 219413.pdf (Publisher’s version ) (Open Access), This paper examines the enactment of relational welfare in Milan through a focus on how new welfare professionals operating in deprived neighborhoods in Milan work to stimulate participation and relational energies among migrant and low-income parents. The paper introduces the notion of the "intimate public," a spatial and relational milieu for activating an increasingly diverse public in the absence of traditional welfare resources. Through the lens of the intimate public, a number of effects come into view: First, the incitement to participate recasts the private as simultaneously a source of new forms of solidarity and a potential obstacle to social cohesion and integration. Second, assumptions about the organic and sustainable nature of citizen-based solidarity effectively erase the affective labor required to produce and sustain it, especially in marginalized neighborhoods. Finally, the intense relationality that characterizes the intimate public produces uncertainty and entanglements that bind professionals and participants in compelling ways.

Abstract: This chapter presents a systematic review of the social, psychological, and economic factors relating to involvement and recruitment into organized crime groups (OCGs), including mafias, drug trafficking organizations (DTOs), gangs, and other criminal organizations. This review has three objectives: (i) identifying the most commonly reported factors leading to recruitment into OCGs, (ii) highlighting whether these factors are independent of one other or they are correlated, and (iii) assessing the validity and generalizability of research findings. The review searched all possible relevant studies indexed in selected databases and published in five languages (i.e. English, French, German, Italian, and Spanish), without limitations as to their year of publication or geographic origin. Starting from 48,731 potentially eligible records, integrated with experts’ suggestions, this systematic review includes 47 empirical studies employing quantitative, qualitative, or mixed-methods approaches. The findings show that social and economic factors are the most commonly reported factors relating to recruitment into OCGs, while psychological factors are marginal. Although factors are highly interrelated and shared across OCGs, their predominance varies across types of criminal organizations. For instance, individuals join mafias and gangs attracted by strong group identity, whereas individuals enter DTOs mainly for financial gain. In light of these findings, recommendations for future research and implications for prevention policies are discussed.

Abstract: Congenital diverticula of the left ventricle, very rare malformations, are determined by an abnormal embryonic development of the ventricular wall and can be isolated or associated to other cardiac anomalies. In most of the cases, these pathologies are not symptomatic and in some patients can be associated to ventricular arrhythmia, cardiac rupture with tamponade and sudden death. Authors are presenting the case of a sudden death in an 8-weeks-old newborn due to rupture of a cardiac congenital diverticulum of the left ventricle, discovered only at the moment of the autopsic examination. The parents of the victim pressed charges against the medical staff that was appointed to the cares, blaming them with malpractice. The missed diagnosis of a cardiac congenital diverticulum of the left ventricle, a rare pathology, reflects the trickiness of the medical management that can lead to medico-legal controversies and, even though such rare conditions must be always taken into consideration when investigating possible dysfunction causing the death, diagnostic difficulties, in the case in exam, justify the missed diagnosis intra-vitam of cardiac ventricular diverticulum.

Abstract: Abdominal apoplexy, also known as idiopathic spontaneous hemoperitoneum, is a rare event related to sudden death, whose diagnosis and treatment is extremely challenging and whose pathologic mechanisms are still mainly unknown. It is frequently associated with vascular disorders (alterations in the vascular structures) and mainly linked to liver cirrhosis. However, the presence of hemoperitoneum may in such cases pose many challenges to forensic post-mortem examinations since the source of bleeding remains often unknown even after a careful and thorough dissection. The Authors present two cases of sudden death related to massive hemoperitoneum (4,650 ml in case 1 and 5,100 ml in case 2) occurred in two cirrhotic males aged 49 and 51: no traumatic injuries were detected and the source of bleeding was not identified although a careful dissection of organs and vascular structures was performed. Rare cases have been already described in the literature, only as case reports, and no systematic studies have been performed on this issue. Nevertheless, this event ought to be taken into account as a cause of sudden death in people with advanced liver disease. A general glimpse is provided among the different causal mechanisms and the challenges within forensic pathology.

Abstract: The effective value of microbiological post-mortem examinations stands as fundamental in forensic cases involving microbiology. We ran these analyses on five victims, who suddenly died after showing persistent fever. The examinations were conducted between 48 hours and 10 days after death, and adrenal gland apoplexy was detected in all the cases. Microbiological examinations identified Neisseria meningitidis, which was accountable for Waterhouse-Friderichsen syndrome. Diplococci were isolated from three cadavers that underwent forensic dissection between 2 and 3 days after death. The remaining two cadavers showed polymicrobial contamination, and a polymerase chain reaction technique was necessary to identify the pathogen. We assumed that the microbial overlap could lead to diagnostic mistakes and conceal the identification of the lethal pathogen. Therefore, we suggest using molecular techniques for a postmortem interval (PMI) longer than 72 hours. Classical microbiological examination should be performed for PMI within 72 hours.

Abstract: Pulmonary thromboembolism (PTE) is one of the major complications in oncologic patients. The incidence of PTE in these cases is 4 to 7 times higher than in non-oncologic patients. Ovarian tumors, specifically those of large sizes, may impair the blood flow through the pelvic veins as tumor pressure over the pelvic vessels increases the incidence of thrombosis. The authors report the case of the unexpected death of a 74-year-old female due to massive pulmonary thromboembolism, associated with an ovarian tumor almost of 15 kg of weight that filled the abdominal and pelvic cavities. The compressive effect on the walls of the pudendal and periuterine veins somehow facilitated the local thrombosis. According to the histological characterization on post-mortem samples, the mass was identified as an ''atypical proliferative (borderline) mucinous tumor." The case emphasizes the important association between pulmonary thromboembolism and ovarian tumors.

Abstract: Introduction Physical restraint is an extraordinary nursing act that poses ethical, nursing, deontological and legal issues. Literature shows weak evidence to support it, despite its frequent use in clinical practice. Prevalence monitoring is recommended, even if the evaluations are uneven due to different characteristics of the settings of care and whether bedrails are included among physical restraints or not. The objective of this study is to assess the prevalence of physical restraint in the Italian health system, through its evaluation in the Papa Giovanni XXIII hospital, Bergamo. Methods Observational, prospective study. All in-patients undergoing physical restraint were assessed along an observation period lasting 51 consecutive days and involving 21 continuous-cycle hospital care units. The critical area, the paediatric area and the mental health area were excluded. Daily surveys, direct observations of the patients contained and analysis of the clinical documentation have been carried out. Results The different methods of investigation used indicate a prevalence of containment of 2.13% (daily surveys) and 1.5% (direct observations).

44. On Comparing Software Quality Metrics of Traditional vs Blockchain-Oriented Software: An Empirical Study

Abstract: Driven by the surge of interest generated around blockchain technologies over the last years, a new category of systems, called Blockchain-Oriented Software (BOS), which are strictly tied to Blockchain distributed environment, has become increasingly popular. Yet, there is not a thorough understanding of their structure and behaviour and if and to which extent they differ from traditional software systems. The present work provide a first statistical characterisation of BOS. We analysed and compared 5 C++ open source Blockchain-Oriented and 5 Traditional Java software systems, aiming at detecting potential differences between the two categories of projects, and specifically in the statistical distribution of 10 software metrics. Although, in general, the statistical distributions for Traditional software and Blockchain software show similarities, the distribution of Average Cyclomatic and Ration Comment To Code metrics reveal significant differences in their queue, whereas the Number of Statements metric shows meaningful differences on the double Pareto distribution.

45. Characterisation of the weapon used in a patricide by SEM/EDS analysis of a microscopic trace from the object

Abstract: This article presents a case of patricide. The murder was characterised by multiple blunt traumas and asphyxia. A mass of contused wounds was localised to the head and neck, and included the complete avulsion of the left eye (by an unknown tool), which was recovered near the cadaver. This case is of interest due to the possibility of identifying microscopic traces of the object that was used for the homicide by examining the skin margins around the ocular injury. The analysis was conducted using scanning electron microscopy combined with energy dispersive X-ray analysis (SEM/EDX). Analysis of the skin margins allowed microscopic inorganic traces to be detected, which were identified as ceramic material. This result focused the attention of the investigation on a small fish-shaped statue that had been previously found by the police when examining the crime scene. The use of SEM/EDX was therefore essential in determining a match between the microscopic traces detected on the perilesional skin and the composition of the statue. This led to the suspicion that the statue was the murder weapon.

46. The medico-legal observation of an aggressive urogenital fibromatosis with isolated development not related to any traumatic event

Abstract: Desmoid tumor is a fibroproliferative neoplasm with an intermediate malignancy and it can be localized in every bodily district: some locations are considered exceptional, like the urogenital localization. The Author point out a rare case of giant idiopathic scrotal fibromatosis that was found during an autopsy.A widower, that lived alone in poor hygienic conditions, was found dead in his house. The Judicial Authority ordered the autopsy, that was performed two days later at the Medico-Legal Section of Milan University.External examinations revealed only the considerable dimension of the scrotum (cm 24 × 41). The cause of death was fixed in a cardiac tamponade due to a natural heart laceration localized in correspondence of a transmural infarction. The toxicological exam resulted negative, while the histopathological and immunohistochemical analysis qualify the scrotal mass as a desmoids tumor.Due to the absence of predisposing conditions and of fibroproliferative infiltration in bladder and retroperitoneal space, the neoplasm was configured as an idiopathic desmoid tumor. The presented case gives the reason for the discussion concerning medico-legal aspects that are typical of rare neoplasms.

Abstract: Plastic Bag Suffocation is a rare cause of death in developed countries and almost unknown in the rest of the world. This study aims to retrospectively evaluate cases of PBS _asphyxia in Milan's Department of Legal Medicine from 1993 to 2013. Cases were selected from the database of 21,472 autopsies performed in the considered timeframe. One hundred and one cases were considered to be cases of Plastic Bag Suffocation, comprised of 100 suicides, no accidental events and 1 homicide. The most relevant elements pertaining to this type of death were evaluated for both the corpses and the crime scenes. From this study the typical PBS victim is an adult male, aged 52.3 years on average, depressed or afflicted by chronic or terminal diseases, and found at home. In 42.6% of cases, the victims were found with the plastic bag still positioned over their head, fastened by tying. Also common among these cases are drugs, alcohol and chemical abuses. According to the collected data, it is impossible to outline a pathognomonic detrimental background that leads to this cause of death. Crime scene investigation is, therefore, the one and only technical resource for evaluating these cases.

48. Technical note: Improvement of cadaveric skin samples (with severe morphological alteration connected to putrefaction or injury) by an extended histological processing

Abstract: Introduction: The microscopic study and the interpretation of skin samples with advanced post-mortal phenomena or with particular destructive injuries is problematic for the forensic pathologist. In an attempt of restoring the histological architecture of cadaveric skin and overcoming these types of problem, the Authors performed a histological processing that was longer than the standard: it was extended until 62 days to evaluate the improvement of the microscopic morphological aspect. Materials and methods: Cutaneous samples were taken from 25 cadavers (5 typologies of skin: charred, putrified, corifed, mummified and partially skeletonized), fixed with a 10%-buffered formalin and then processed in two different ways: one half of the samples was routinely addressed to the standard-time automatic technique, while the other half was manually processed with prolonged times. All the slides were then stained in Hematoxylin-Eosin. Results: The standard-processed slides demonstrated marked morphological alterations and artefacts at the microscopic observation; conversely, those processed with the prolonged manual technique showed an improvement in the morphological structure, sometimes permitting the identification of the anatomical components. Conclusion: Though it is characterized by the inconvenience of protracted times, the application of a long-term manual histological processing to cadaveric skin samples with advanced post-mortal alteration permits to better observe the anatomical architecture of skin and it could be useful and helpful in the evaluation of such cases.

49. Fatal air embolism during endoscopic retrograde cholangiopancreatography (ERCP): An ‘impossible’ diagnosis for the forensic pathologist

Abstract: Fatal air embolism related to endoscopic retrograde cholangiopancreatography is a very rare phenomenon. The authors describe the case of a 51-year-old female patient who developed this mortal complication; a computed tomography (CT) examination was performed in articulo mortis by the physicians. Autopsy was unreliable because of bizarre post-mortem changes (reabsorption of intra-cardiac gas vs. conservation of intra-cranial gas) and a lack of strong diagnostic value of histological findings. The right diagnosis was possible thanks only to the CT examination that permitted the assumption of this possible cause of death before the autopsy and to prepare the necessary procedures to recognise and probe air embolism. This case exemplifies how early post-mortem imaging can be crucial to avoid a wrong diagnosis.

50. Two decades of adolescent suicides assessed at Milan University's medicolegal unit: Epidemiology, forensic pathology and psychopathology

Abstract: Aim to analyze the cases of suicide committed by adolescents in and around the city of Milan over a 20-year period (1993-2012). Materials and methods cases of suicide involving individuals between 10 and 19 years of age were drawn from 20,757 autopsies performed by the Medico-legal Unit at Milan University. Seventy-eight cases were considered (20 females and 58 males, with the ratio of 1:2.9), and their clinical and circumstantial histories, epidemiology, forensic pathology and psychopathological issues were analyzed. Result Adolescents were involved in 2.23% of all suicides committed in Milan during the period examined. The "mean" victim is a male without psychiatric disease, aged between 16 and 19, that commits suicide outdoors. Conclusions In the majority of cases, there were no premonitory signs, nor any particular contingent or remote reason that might explain these violent deaths, which therefore remain "impulsive".
