Abstract: Context. The ESO public survey VISTA Variables in the Vía Láctea (VVV) surveyed the inner Galactic bulge and the adjacent southern Galactic disk from 2009−2015. Upon its conclusion, the complementary VVV eXtended (VVVX) survey has expanded both the temporal as well as spatial coverage of the original VVV area, widening it from 562 to 1700 sq. deg., as well as providing additional epochs in JHKs filters from 2016−2023. Aims. With the completion of VVVX observations during the first semester of 2023, we present here the observing strategy, a description of data quality and access, and the legacy of VVVX. Methods. VVVX took ∼ 2000 hours, covering about 4% of the sky in the bulge and southern disk. VVVX covered most of the gaps left between the VVV and the VISTA Hemisphere Survey (VHS) areas and extended the VVV time baseline in the obscured regions affected by high extinction and hence hidden from optical observations. Results. VVVX provides a deep JHKs catalogue of & 1.5 × 109 point sources, as well as a Ks band catalogue of ∼ 107 variable sources. Within the existing VVV area, we produced a 5D map of the surveyed region by combining positions, distances, and proper motions of well-understood distance indicators such as red clump stars, RR Lyrae, and Cepheid variables. Conclusions. In March 2023 we successfully finished the VVVX survey observations that started in 2016, an accomplishment for ESO Paranal Observatory upon 4200 hours of observations for VVV+VVVX. The VVV+VVVX catalogues complement those from the Gaia mission at low Galactic latitudes and provide spectroscopic targets for the forthcoming ESO high-multiplex spectrographs MOONS and 4MOST.

3. Regression analysis for container ships in the early design stage

Abstract: The seaway trade market has expanded in the last years and container ship dimensions are constantly increasing for higher cargo capacity. In the early design stage, main dimensions are usually determined based on an existing ship database from which regression formulas are derived. In the present paper, a database of 260 non-sister container ships built from 1979 to 2022, representing 20% of the world fleet, has been considered to derive and compare different types of regressions. Simple regressions have been developed and compared with equivalent formulations available in literature, proving better approximations of the trends. The study has been further extended by multivariable regressions and forest tree algorithms, which allow the use of more than one independent variable and provide a better fitting compared to simple regressions. Forest tree regressions return the highest values of fitting coefficients, but the technique is not of easy application due to the absence of mathematical expressions. The main contribution is the updated set of simple and multivariable regression formulas which have a higher goodness of fit than previous works and can be easily employed by designers in the early design stage and in multi-attribute design procedures., Ship Design, Production and Operations

Abstract: COVID-19, the disease caused by SARS-CoV-2, is still afflicting thousands of people across the globe. Few studies on COVID-19 in chronic lymphocytic leukemia (CLL) are available. Here, we analyzed data from the CLL cohort of the Italian Hematology Alliance on COVID-19 (NCT04352556), which included 256 CLL patients enrolled between 25 February 2020 and 1 February 2021. Median age was 70 years (range 38–94) with male preponderance (60.1%). Approximately half of patients (n = 127) had received at least one line of therapy for CLL, including 108 (83.7%) who were on active treatment at the time of COVID-19 or received their last therapy within 12 months. Most patients (230/256, 89.9%) were symptomatic at COVID-19 diagnosis and the majority required hospitalization (n = 176). Overall, after a median follow-up of 42 days (IQR 24–96), case fatality rate was 30.1%, and it was 37.5% and 24.4% in the first (25 February 2020–22 June 2020) and second wave (23 June 2020–1 February 2021), respectively (p = 0.03). At multivariate analysis, male sex (HR 1.82, 95% CI 1.03–3.24, p = 0.04), age over than 70 years (HR 2.23, 95% CI 1.23–4.05, p = 0.01), any treatment for CLL given in the last 12 months (HR 1.72, 95% CI 1.04–2.84, p = 0.04) and COVID-19 severity (severe: HR 5.66, 95% CI 2.62–12.33, p < 0.0001; critical: HR 15.99, 95% CI 6.93–36.90, p < 0.0001) were independently associated with poor survival. In summary, we report a dismal COVID-related outcome in a significant fraction of CLL patients, that can be nicely predicted by clinical parameters.

Abstract: Previous research has analyzed the imprinting effect associated with the firm’s international expansion without considering the full range of differences between home and host countries. These differences are important because, depending on the development gap, and the direction of the difference, learning opportunities and the possibility of upgrading firm’s capabilities will be vastly different. For this reason, we analyze the specific influence of the exposure to a specific group of international markets, those that are more developed than the country of origin of the focal firm. Abstract: The prediction of the statutory attained subdivision index is a challenging issue for the initial design of ships due to the design freedom offered by a probabilistic damage stability assessment. To this end, optimisation techniques integrated with a parametric model of the internal layout may generate a preliminary subdivision design, fulfilling damage stability regulations and cargo volume requirements. The present study explores using a multi-objective constrained optimisation algorithm coupled with a parametric model of a single hold cargo vessel, first investigating two design goal alternatives and secondly performing a global sensitivity analysis on the design variables for the most promising solution. The adoption, in parallel, of state-of-the-art practices shows the validity of the obtained solutions and the time benefits for designers. Furthermore, the models employed and proposed solutions use simplified methods based on empirical or probabilistic concepts. One of the aims of the EC-founded project FLARE was to promote the use of first principle methods throughout the whole vessel life-cycle, from the design phase up to the onboard risk management. To this end, this work presents the challenges and potential applicability of a real-time flooding risk evaluation methodology for ship-to-ship collisions, based on first-principles calculations. The possibility to perform direct calculations for survivability allows us to define a multi-level approach to flooding risk, separating Level-1 predictions, purely based on semi-empirical models and databases, from Level-2 predictions based on the concept of Potential Loss of Life (PLL). Here, besides a description of the multi-level risk assessment based on PLL, the different tasks of design and operational phases are addressed. Such issues are then linked to the real-time flooding risk evaluation for onboard applications, potentially working for different hazard types but conceptualised for the case of ship-to-ship collisions. Abstract: The in-force probabilistic framework for passenger ship survivability assessment covers collision hazards. The framework primarily pertains to a static approach. Nonetheless, more complex dynamic analyses usually employ the same damage definitions, adding besides the breach characteristics, the environmental condition selection or, more precisely, the irregular wave environment necessary to simulate the damage scenarios. The traditional dynamic approaches to survivability consider only the significant wave height sampled from statistical formulations, with the wave period deriving from a constant steepness assumption. However, wave height and period influence ship dynamics in waves differently, especially concerning survivability after damage. Therefore, aiming at a direct assessment of ship survivability and the probability of loss of lives determination in realistic operational scenarios, it is essential to properly study the influence of combined variations of wave height and periods and their occurrence. The present study proposes a methodology for dynamic simulations in site-specific conditions derived from the Global Wave Statistics. The study documents the process in two critical collision damages for a reference passenger ship, using wave height and period combinations typical of the main sea areas of interest for passenger ships and performing a sensitivity analysis on the simulations needed to evaluate survivability. Abstract: The increasing demand for offshore operations in deep water implies the necessity to predict station-keeping ability of offshore vessels since the early stages of design. To this end, besides developing sufficiently fast and accurate methodologies for the equilibrium resolution of the forces acting on the ship, it is of utmost importance to estimate, in a reliable way, the external forces acting on the vessel. This work focuses on the current loads, aiming at developing a model for fast current load prediction based on high-fidelity Computational Fluid Dynamics (CFD) computations. Selecting the drill-ships as reference vessel-type for the study, starting from the actual fleet operating worldwide, a systematic series of hulls has been generated varying the main hull-form parameters inside the database, according to a Box-Behnken scheme. CFD calculations based on RANS equations have been performed on the whole ship set, for a set of incidence angle varying from 0 to 180 degrees considering the hull symmetric. As numerical analyses are not suitable for fast calculations the results on the systematic series have been used as input for developing a surrogate model based on Multiple Linear Regressions (MLR). The method allows for scaling the results as a function of the Reynolds number, allowing for general and flexible applicability among different vessel dimensions. Abstract: Real-time assessment of flooding risk associated with the collision between two ships, requires a fast estimation of damage dimensions and associated survivability. The state-of-the-art frameworks for risk assessment on passenger ships do not consider a direct evaluation of damages through crash simulations but refer to probabilistic considerations, modelling damage characteristics according to statutory marginal distributions of damage breaches too old to be any longer relevant. In any case, such an approach is not possible for the real-time risk assessment process developed in project FLARE, aimed at promoting the employment of first-principles tools for risk evaluation. In this spirit, the present work investigates the possibility of developing a database-oriented damage breach model, which employs direct crash analyses with the super-element code SHARP. However, the sole usage of crash simulations is not suitable for real-time applications. Therefore, starting from the collision simulation database, surrogate models have to be derived for real-time application. In this specific case, three different strategies have been used for the models creation namely: multiple linear regressions, neural networks and decision trees. Abstract: The importance of comfort during transfer and stationing becomes a key performance parameter for large yacht design, on the same level as propulsive issues. Such a matter extends questions in terms of form and service demand to the motion behaviour of the unit in waves. Relevant studies refer to outdated hull forms not specific to modern large yachts. In this study, five hull forms with different bow concepts represent the most common design solutions for yachts at constant draught and displacement. The preliminary ranking on the effect of alternative bows on comfort requires the definition of internationally accepted comfort standards. Here, the AWI 22834 guidelines for large yachts provide the service and environmental conditions and criteria for the comfort analysis, being the only reference specific to yachts. The calculations employ a strip-theory-based numerical model to provide results of easy understanding for designers during the early design stage. The obtained ranking among the design solutions on a reference large yacht favours the option nested with a bulb, contradicting the expectations in favour of a vertical bow concept. The discussion and conclusions provide a way forward for additional analyses and investigations aimed at proposing suitable multicriterial design guidelines for large yachts. 16. Digital twin for ship life-cycle: A critical systematic review

Abstract: The focus on digitalisation in manufacturing is spreading to other industry fields, including large and complex objects like ships. Such interest introduces the concept of Digital Twins in supporting designers and operators through the whole ship-life cycle. However, the term Digital Twin is typically abused in the shipping industry, many times erroneously referring to any virtual version of a model-based system as a Digital Twin of the ship. The mutual data exchange between the physical and virtual environment, which is the basis of a true Digital Twin, is mostly missing, confusing a virtual model with a sophisticated living virtual environment. Few reviews are available in the literature for Digital Twins on ships. This systematic review proposes the identification of weaknesses and correlations between current Digital Twin applications in the maritime industry and other industry fields. Furthermore, the methodology applied here may be repeated in future studies to provide a fair and objective overview of the research advancements in the topic. Abstract: Theory and application of damage stability followed over the years two dissociated paths: static assessments and dynamic simulations. The first approach, being easy to apply and understand, has been preferred by ship designers and regulators; the second, more advanced and first-principle oriented, has been mainly reserved for research or high-level consultancy, especially for passenger ships. Nowadays, the availability of numerical flooding simulation tools across the scientific community and calculation power in the industry allows for a possible definitive transition of damage stability assessment towards direct numerical analyses. However, research should softly drive designers towards more advanced processes via a suitable didascalic calculation framework. The multi-level approach pursued in project FLARE is an example of such a transition from static to dynamic damage stability assessment. The present work initially carefully reviews the probabilistic concept of damage stability, critically comparing the prescriptive statistical methods with direct ones and providing insights and guidance on how researchers and designers can reconcile with the original implicit assumption of the probabilistic approach. Secondly, the development of the multi-level framework highlights incongruences concerning modelling of damages between static and dynamic assessments, disfavouring the comprehension of dynamic results to designers. Two detailed examples highlight the differences in dynamic simulation results between different damage breach modelling, leading to completely different flooding paths for the same damage case. 18. A Monte Carlo approach for the fully probabilistic evaluation of operability in ship dynamic positioning scenarios

Abstract: The Dynamic Positioning system allows a vessel to keep a precise position and heading during stationing operations in a rough sea by using onboard actuators only. During the design phase, it is mandatory to identify the capability of the system actuators to counteract the environmental forces. Conventional predictions are limited to the estimation of a maximum sustainable wind speed on predefined encounter angles by estimating the corresponding wave parameters with questionable standard deterministic correlations. The proposed approach aims at determining the dynamic positioning performances by using site-specific long-term environmental conditions which are modelled with joint distributions of wind and wave parameters. To this end, the operability of the dynamic positioning system is evaluated as a non-deterministic multidimensional Monte Carlo integration process, based on the sampling of environmental joint distributions. For each environmental condition, a quasi-static dynamic positioning analysis is performed solving the equilibrium between external forces and the vessel's actuators through a non-linear thrust allocation algorithm. Abstract: Functional reserve of organs and systems is known to be relevant in predicting immunochemotherapy tolerance. Age and comorbidities, assessed by the cumulative illness rating scale (CIRS), have been used to address chemotherapy intensity. In the ibrutinib era, it is still unclear whether age, CIRS, and Eastern Cooperative Oncology Group performance status (ECOG-PS) retain their predictive role on treatment vulnerability. In this series of 712 patients with chronic lymphocytic leukemia (CLL) treated with ibrutinib outside clinical trials, baseline ECOG-PS and neutropenia resulted as the most accurate predictors of treatment feasibility and outcomes. Age did not independently influence survival and ibrutinib tolerance, indicating that not age per se, but age-related conditions, may affect drug management. We confirmed the role of CIRS . 6 as a predictor of a poorer progression- and event-free survival (PFS, EFS). The presence of a severe comorbidity was significantly associated with permanent dose reductions (PDRs), not translating into worse outcomes. As expected, del(17p) and/or TP53mut and previous therapies affected PFS, EFS, and overall survival. No study so far has analyzed the influence of concomitant medications and CYP3A inhibitors with ibrutinib. In our series, these factors had no impact, although CYP3A4 inhibitors use correlated with Cox regression analysis, with an increased risk of PDR. Despite the limitation of its retrospective nature, this large study confirmed the role of ECOG-PS as the most accurate predictor of ibrutinib feasibility and outcomes, and importantly, neutropenia emerged as a relevant tool influencing patients’ vulnerability. Although CIRS > 6 retained a significant impact on PFS and EFS, its value should be confirmed by prospective studies.

Abstract: Objectives: Few studies have analyzed factors associated with delirium subtypes. In this study, we investigate factors associated with subtypes of delirium only in patients with dementia to provide insights on the possible prevention and treatments. Design: This is a cross-sectional study nested in the “Delirium Day” study, a nationwide Italian point-prevalence study. Setting and Participants: Older patients admitted to 205 acute and 92 rehabilitation hospital wards. Measures: Delirium was evaluated with the 4-AT and the motor subtypes with the Delirium Motor Subtype Scale. Dementia was defined by the presence of a documented diagnosis in the medical records and/or prescription of acetylcholinesterase inhibitors or memantine prior to admission. Results: Of the 1057 patients with dementia, 35% had delirium, with 25.6% hyperactive, 33.1% hypoactive, 34.5% mixed, and 6.7% nonmotor subtype. There were higher odds of having venous catheters in the hypoactive (OR 1.82, 95% CI 1.18-2.81) and mixed type of delirium (OR 2.23, CI 1.43-3.46), whereas higher odds of urinary catheters in the hypoactive (OR 2.91, CI 1.92-4.39), hyperactive (OR 1.99, CI 1.23-3.21), and mixed types of delirium (OR 2.05, CI 1.36-3.07). We found higher odds of antipsychotics both in the hyperactive (OR 2.87, CI 1.81-4.54) and mixed subtype (OR 1.84, CI 1.24-2.75), whereas higher odds of antibiotics was present only in the mixed subtype (OR 1.91, CI 1.26-2.87). Conclusions and Implications: In patients with dementia, the mixed delirium subtype is the most prevalent followed by the hypoactive, hyperactive, and nonmotor subtype. Motor subtypes of delirium may be triggered by clinical factors, including the use of venous and urinary catheters, and the use of antipsychotics. Future studies are necessary to provide further insights on the possible pathophysiology of delirium in patients with dementia and to address the optimization of the management of potential risk factors.

Abstract: One of the main issues in the treatment of patients with chronic lymphocytic leukemia (CLL) deals with the choice between continuous or fixed-duration therapy. Continuous ibrutinib (IB), the first-in-class BTK inhibitor, and obinutuzumab-chlorambucil (G-CHL) are commonly used therapies for elderly and/or comorbid patients. No head-to-head comparison has been carried out. Within the Italian campus CLL network, we performed a retrospective study on CLL patients without TP53 disruption treated with IB or G-CHL as first-line therapy. Patients in the G-CHL arm had a higher CIRS score and the worst renal function. The overall response rates between the G-CHL and IB arms were similar, but more complete remissions (CRs) were achieved with G-CHL (p = 0.0029). After a median follow-up of 30 months, the progression-free survival (PFS, p = 0.0061) and time to next treatment (TTNT, p = 0.0043), but not overall survival (OS, p = 0.6642), were better with IB than with G-CHL. Similar results were found after propensity score matching and multivariate analysis. While PFS and TTNT were longer with IB than with G-CHL in IGHV unmutated patients (p = 0.0190 and 0.0137), they were superimposable for IGHV mutated patients (p = 0.1900 and 0.1380). In the G-CHL arm, the depth of response (79% vs. 68% vs. 38% for CR, PR and SD/PD; p < 0.0001) and measurable residual disease (MRD) influenced PFS (78% vs. 53% for undetectable MRD vs. detectable MRD, p = 0.0203). Hematological toxicities were common in the G-CHL arm, while IB was associated with higher costs. Although continuous IB provides better disease control in CLL, IGHV mutated patients and those achieving an undetectable MRD show a marked clinical and economic benefit from a fixed-duration obinutuzumab-based treatment.

Abstract: Globular clusters (GCs) are excellent tracers of the formation and early evolution of the Milky Way. The bulge GCs (BGCs) are particularly important because they can reveal vital information about the oldest, in-situ component of the Milky Way. We aim at deriving mean metallicities and radial velocities for 13 GCs that lie towards the bulge and are generally associated with this component. We use near infrared low resolution spectroscopy with the FORS2 instrument on the VLT to measure the wavelengths and equivalent widths of the CaII triplet (CaT) lines for a number of stars per cluster. We derive radial velocities, ascertain membership and apply known calibrations to determine metallicities for cluster members, for a mean of 11 members per cluster. We derive mean cluster RV values to 3 km/s, and mean metallicities to 0.05 dex. Our sample has metallicities lying between -0.21 and -1.64 and is distributed between the traditional metal-rich BGC peak near [Fe/H] aprox. -0.5 and a more metal-poor peak around [Fe/H] aprox. -1.1, which has recently been identified. These latter are candidates for the oldest GCs in the Galaxy, if blue horizontal branches are present, and include BH 261, NGC 6401, NGC 6540, NGC 6642, and Terzan 9. Finally, Terzan 10 is even more metal-poor. However, dynamically, Terzan 10 is likely an intruder from the halo, possibly associated with the Gaia-Enceladus or Kraken accretion events. Terzan 10 is also confirmed as an Oosterhotype II GC based on our results. The lone halo intruder in our sample, Terzan 10, is conspicuous for also having by far the lowest metallicity, and casts doubt on the possibility of any bonafide BGCs at metallicities below about aprox. -1.5., Comment: 13 pages, 10 figures. Abstract: We derived radial velocities and CaT metallicity of more than 150 red giants stars in six SMC star clusters and their surrounding fields, with the instrument GMOS on GEMINI-S. The mean cluster radial velocity and metallicity were obtained with mean errors of 2.2 km\,s$^{-1}$ and 0.03 dex, while the mean field metallicities have a mean error of 0.13 dex. We add this information to that available for another 51 clusters and 30 fields with CaT metallicities on the same scale. Using this expanded sample we analize the chemical properties of the SMC Main Body, defined as the inner 3.4 degrees in semimajor axis. We found a high probability that the metallicity distribution of the Main Body clusters is bimodal with a metal-rich and a metal-poor cluster group, having mean metallicities with a dispersion of $\mu = -0.80$, $\sigma = 0.06$ and $\mu = -1.15$, $\sigma = 0.10$ dex, respectively. On the other hand, Main Body field stars show a unimodal metallicity distribution peaking at $[Fe/H] \sim -1$ and dispersion of $0.3$. Neither metal-rich nor metal-poor clusters present a metallicity gradient. However the full Main Body cluster sample and field stars have a negative metallicity gradient consistent with each other, but the one corresponding to clusters has a large error due to the large metallicity dispersion present in the clusters studied in that region. Metal-rich clusters present a clear age-metallicity relation, while metal-poor clusters present no chemical enrichment throughout the life of the galaxy. We present observational evidence that the chemical enrichment is complex in the SMC Main Body. Two cluster groups with potential different origins could be coexisting in the Main Body. More data with precise and homogeneous metallicities and distances are needed and dynamical simulations are required to understand possible different origins for the two possible cluster groups.

Abstract: Large cruise ships can carry 10 000 persons onboard, and consequently, survivability of the ship in the event of a flooding accident is essential. Many designers are already conducting advanced damage stability analyses beyond the regulatory requirements. With increased computing capacity, survivability analyses, by using time-domain simulation tools, are already commonly applied in the design of new cruise ships. Consequently, it is essential that such tools are properly validated, in terms of ship response and detailed flooding behavior, to assess the capability and applicability of the tools. For this purpose, an international benchmark study on simulation of flooding and motions of damaged cruise ships was conducted within the EU Horizon 2020 project FLARE, using experimental data from new dedicated model tests as a reference. The test cases include transient and progressive flooding, both in calm water and in irregular beam seas. Abstract: The damaged stability assessment for a passenger ship is a process requiring the simulation of multiple damage scenarios. Nevertheless, the stochastic nature of the damage stability framework requires the analysis of a statistically significant number of cases. On the other hand, the probability density functions used to estimate the possible damage dimensions and locations along the ship generate many scenarios that are not critical for the ship's survivability, especially for large passenger ships. It is standard to apply empirical rules to restrict the number of damage scenarios, such as critical damages is only above two compartments, considering that damage stability regulations currently in force ensure survivability levels beyond this extent of breaches. However, a rigorous approach is lacking. To this end, in the present work, it is proposed to use more scientific-based methods to identify critical damages. This paper presents three original approaches developed in the context of a multi-level damage stability assessment. 30. A probabilistic approach for Dynamic Positioning capability and operability predictions

Abstract: Determining Dynamic Positioning capability for an offshore vessel is mandatory to identify the environmental forces the system can counteract, together with the operability in a specific operational area of interest. Conventional predictions evaluate the capability as a maximum sustainable wind speed at a predefined encounter angle for a given wind–wave correlation, not reflecting the effective wind and waves occurrence at the site. In this respect, a step forward is provided by the scatter diagram approach, allowing the evaluation of operability in a specific sea area, using a simplified method to predict wind speed from wave parameters. Here, using known wind–waves joint distributions for the long-term environmental conditions further improves the scatter diagram approach, assessing the operability of a Dynamic Positioning system through a Quasi-Monte Carlo sampling of the joint distribution. Analysing the results of the Quasi-Monte Carlo process, it is possible to obtain a site-specific capability plot, allowing the identification of critical wind speeds in a way that is familiar to operators in the offshore industry. The application of this novel method in the case of quasi-static calculations both to a reference supply vessel and a pipe-lay vessel shows the flexibility of the proposed approach for site-specific Dynamic Positioning capability predictions., Ship Design, Production and Operations

Abstract: Over the last few years, several studies have investigated the possible effects of mindfulness meditation on time perception. Mindfulness meditation has been linked to both longer time production (TP) and increased bodily perception, and bodily processes are in turn thought to lie at the core of human time perception. Nevertheless, the connection between mindful movements (MMs; i.e., specific types of mind–body coordination-demanding physical activity) and time perception has been ignored. Consequently, in the current study we examined the effect of MMs on TP. A mixed observational study design was adopted to investigate whether (1) the extensive practice of MMs is associated with longer TP and interlimb coordination efficiency, and (2) there is a relationship between these two variables. Thirty-four healthy adults volunteered, including 11 Aikido and nine advanced Quadrato Motor Training (aQMT) practitioners, and 14 physically non-active controls. Practitioners of aQMT had significantly longer produced durations in contrast to the Aikido and control groups. In addition, longer produced duration was associated with increased coordination efficiency. In conclusion, the current results are in line with previous studies linking embodied time and mindful meditation, and suggest the importance of incorporating movement meditation, especially in the context of time estimation and the self.

Abstract: Absorption, the ability to highly focus attention, as well as openness to self-altering experiences, is an important psychological construct, closely related to deep-meditation states and other altered states of consciousness. Yet, little is known about the electrophysiological profile of states of absorption, possibly due to the difficulty to induce this state in the lab. While most studies have used a visual Ganzfeld (homogeneous perceptual field), here we use a novel technique of full immersion—the OVO Whole-Body Perceptual Deprivation (OVO-WBPD) chamber, which is an altered sensory environment in the form of a human-sized egg. Consequently, the aims of the current study were to investigate whether the OVO-WBPD chamber induces state absorption, using first-person reports, as well as to examine electrophysiological change following immersion in this altered sensory environment. Fourteen participants volunteered to participate in the study. Trait absorption was measured using the Tellegen Absorption Scale (TAS). State absorption was assessed by analyzing the content of the subjective reports, using sub-categories of the absorption construct (e.g., synesthesia). EEG was measured before and during a 20-min OVO-WBPD experience. Using exact low-resolution brain electromagnetic tomography (eLORETA), we analyzed change in oscillatory EEG activity and localized the generators of the scalp EEG power spectra following the OVO-WBPD. Our results show that OVO-WBPD immersion leads to a state of absorption in all participants. We also report significant increased oscillatory activity in the delta and beta bands, in the left inferior frontal cortex, with a peak in the sub-lobar of the left insula. In addition, a positive correlation was found between change in delta activity and trait absorption. The results are discussed in light of other meditative practices and altered states of consciousness.

Abstract: This review examines the factors that affect the decision-making process of parental couples evaluating prenatal screening and diagnostic tests. A systematic search was performed using PubMed and PsycInfo databases. The 46 included studies had to: investigate the decision-making process about prenatal testing; focus on tests detecting trisomy 21, 18, 13, and abnormalities of sex chromosomes; be published in English peer-reviewed journals. The decision-making process seems composed of different levels: an individual level with demographic, clinical, and psychological aspects; a contextual level related to the technical features of the test and the information received; a relational level involving family and society.

Abstract: Recent studies show that the inner Galactic regions host genuine bulge globular clusters, but also halo intruders, complex remnants of primordial building blocks, and objects likely accreted during major merging events. In this study we focus on the properties of M 28, a very old and massive cluster currently located in the Galactic bulge. We analysed wide-field infrared photometry collected by the VVV survey, VVV proper motions, and intermediate-resolution spectra in the calcium triplet range for 113 targets in the cluster area. Our results in general confirm previous estimates of the cluster properties available in the literature. We find no evidence of differences in metallicity between cluster stars, setting an upper limit of Delta[Fe/H]<0.08 dex to any internal inhomogeneity. We confirm that M 28 is one of the oldest objects in the Galactic bulge (13-14 Gyr). From this result and the literature data, we find evidence of a weak age-metallicity relation among bulge globular clusters that suggests formation and chemical enrichment. In addition, wide-field density maps show that M 28 is tidally stressed and that it is losing mass into the general bulge field. Our study indicates that M 28 is a genuine bulge globular cluster, but its very old age and its mass loss suggest that this cluster could be the remnant of a larger structure, possibly a primeval bulge building block., Comment: Accepted for publication in Astronomy & Astrophysics

Abstract: We have investigated the stellar and interstellar content of the distant star formation region IRAS 17591-2228 (WISE HII region GAL 007.47+0.06). It is associated to a water maser, whose parallax distance is d=20.4^{+2.8} {-2.2} kpc, supported by independent measurements of proper motion and radial velocity. It is projected in the same direction as an extremely red (J-Ks ~ 6 mag) group of stars, and a shell of mid-infrared emission. We qualify the group of stars as a cluster candidate, VVV CL177. Its radius spans between 0.45' and 1' and contains at least two young stellar objects with an extreme extinction near Av ~ 40 mag. Yet more analysis will be required to determine is it is a real single cluster associated with the water maser. Abstract: This review examines the factors that affect the decision-making process of parental couples evaluating prenatal screening and diagnostic tests. A systematic search was performed using PubMed and PsycInfo databases. The 46 included studies had to: investigate the decision-making process about prenatal testing; focus on tests detecting trisomy 21, 18, 13, and abnormalities of sex chromosomes; be published in English peer-reviewed journals. The decision-making process seems composed of different levels: an individual level with demographic, clinical, and psychological aspects; a contextual level related to the technical features of the test and the information received; a relational level involving family and society.

Abstract: Because the efficacy of new drugs reported in trials may not translate into similar results when used in the real-life, we analyzed the efficacy of idelalisib and rituximab (IR) in 149 patients with relapsed/refractory chronic lymphocytic leukemia treated at 34 GIMEMA centers. Median progression-free survival (PFS) and overall survival were 22.9 and 44.5 months, respectively; performance status (PS) ≥2 and ≥3 previous lines of therapy were associated with shorter PFS and overall survival (OS). 48% of patients were on treatment at 12 months; the experience of the centers (≥5 treated patients) and PS 0-1 were associated with a significantly longer treatment duration (p = 0.015 and p = 0.002, respectively). TP53 disruption had no prognostic significance. The overall response rate to subsequent treatment was 49.2%, with median OS of 15.5 months and not reached in patients who discontinued, respectively, for progression and for toxicity (p < 0.01). Treatment breaks ≥14 days were recorded in 96% of patients and adverse events mirrored those reported in trials. In conclusion, this real-life analysis showed that IR treatment duration was longer at experienced centers, that the ECOG PS and ≥3 lines of previous therapy are strong prognostic factor and that the overall outcome with this regimen was superimposable to that reported in a randomized trial.

Abstract: Objectives: To compare the capacity of ibrutinib (IB) and idelalisib-rituximab (IDELA-R) of prolonging overall survival (OS) as in CLL patients, previously treated with chemotherapy only. Methods: A real-life cohort of 675 cases has been identified and investigated in the database of the groups participating in the study. Results: At an unadjusted univariate analysis, a significant death risk reduction was observed favoring IB (IDELA-R vs IB HR = 0.5, 95% CI = 0.36-0.71) although with some limitations due to the non-randomized and retrospective nature of the study and to the lower number of patients in the IDELA-R group (112 cases) related to the current prescribing practice. To overcome the potential problem of confounding by indication, we adjusted the association between the type of therapy and mortality for all variables significantly associated with OS at Cox univariate analysis. Furthermore, those variables, differently distributed between the two study groups, were introduced into the multivariate Cox model to improve the effectiveness of the analysis. By introducing all these variables into the multiple Cox regression model, we confirmed the protective effect of IB vs IDELA-R (HR = 0.67, 95% CI = 0.45-0.98, P =.04) independent of potential confounders. Conclusions: Although our analysis presents some constraints, that is, the unavailability of additional potential confounders, and the retrospective nature of the study, this observation may be of help for the daily clinical practice, particularly in the absence of randomized trials comparing the two schedules.

Abstract: Ibrutinib represents extraordinary progress in the treatment of chronic lymphocytic leukemia (CLL). However, treatment-related adverse events limit the benefit of this agent. This obser-vational, multicenter study focused on the incidence, risk factors, and prognostic impact of infections in 494 patients with CLL treated with an ibrutinib-based treatment. Ibrutinib was given to 89 (18%) previously untreated patients (combined with rituximab, 24) and 405 (82%) relapsed/refractory patients. Pneumonia (PN), grade ≥3 non-opportunistic infections (NOI), and opportunistic infections (OI) were recorded in 32% of patients with an overall incidence rate per 100 person-year of 15.3% (PN, 10%; NOI, 3.3%; OI, 2%). Infections were the reason for the permanent discontinuation of ibrutinib in 9% of patients. Patients who experienced pneumonia or a severe infection showed a significantly inferior survival than those who were infection-free (p < 0.0001). A scoring system based on the three factors associated with a significant and independent impact on infections—PN or severe infection in the year before starting ibrutinib, chronic obstructive pulmonary disease, ≥2 prior treatments—identified patients with a two-to threefold increase in the rate of infections. In conclusion, the results of this study highlight the adverse impact of infectious events on the outcomes of CLL patients treated with ibrutinib.

Abstract: Autoimmune cytopenias (AICs) affect 5% to 9% of patients with chronic lymphocytic leukemia (CLL). Targeted drugs—ibrutinib, idelalisib, and venetoclax—have a prominent role in the treatment of CLL, but their impact on CLL-associated AICs is largely unknown. In this study, we evaluated the characteristics and outcome of preexisting AICs and described the incidence, quality, and management of treatment-emergent AICs during therapy with targeted drugs in patients with CLL. We collected data from 572 patients treated with ibrutinib (9% in combination with an anti-CD20 monoclonal antibody), 143 treated with idelalisib-rituximab, and 100 treated with venetoclax (12% in combination with an anti-CD20 monoclonal antibody). A history of preexisting AICs was reported in 104 (13%) of 815 patients. Interestingly, 80% of patients whose AICs had not resolved when treatment with a targeted drug was started experienced an improvement or a resolution during therapy. Treatment-emergent AICs occurred in 1% of patients during ibrutinib therapy, in 0.9% during idelalisib therapy, and in 7% during venetoclax therapy, with an estimated incidence rate of 5, 6, and 69 episodes per 1000 patients per year of exposure in the 3 treatment groups, respectively. The vast majority of patients who developed treatment-emergent AICs had unfavorable biological features such as an unmutated IGHV and a del(17p) and/or TP53 mutation. Notably, despite AICs, 83% of patients were able to continue the targeted drug, in some cases in combination with additional immunosuppressive agents. Overall, treatment with ibrutinib, idelalisib, or venetoclax seems to have a beneficial impact on CLL-associated AICs, inducing an improvement or even a resolution of preexisting AICs in most cases and eliciting treatment-emergent AICs in a negligible portion of patients.

Abstract: The formation and evolutionary processes of galaxy bulges are still unclear, and the presence of young stars in the bulge of the Milky Way is largely debated. We recently demonstrated that Terzan 5, in the Galactic bulge, is a complex stellar system hosting stars with very different ages and a striking chemical similarity to the field population. This indicates that its progenitor was likely one of the giant structures that are thought to generate bulges through coalescence. Here we show that another globular cluster-like system in the bulge (Liller 1) hosts two distinct stellar populations with remarkably different ages: only 1-3 Gyr for the youngest, 12 Gyr for the oldest, which is impressively similar to the old component of Terzan 5. Abstract: Milky Way globular clusters (MW GCs) are difficult to identify at low Galactic latitudes because of high differential extinction and heavy star crowding. The new deep near-IR images and photometry from the VISTA Variables in the Via L\'actea Extended Survey (VVVX) allow us to chart previously unexplored regions. Our long term aim is to complete the census of MW GCs. The immediate goals are to estimate the astrophysical parameters, measuring their reddenings, extinctions, distances, total luminosities, proper motions, sizes, metallicities and ages. We use the near-IR VVVX survey database, in combination with Gaia DR2 optical photometry, and with the Two Micron All Sky Survey (2MASS) photometry. We report the detection of a heretofore unknown Galactic Globular Cluster at $RA =$ 14:09:00.0; $DEC=-$65:37:12 (J2000). We calculate a reddening of $E(J-K_s)=(0.3\pm 0.03)$ mag and an extinction of $A_{K_s}=(0.15\pm 0.01)$ mag for this new GC. Its distance modulus and corresponding distance were measured as $(m-M)=(15.93\pm0.03)$ mag and $D=(15.5\pm1.0)$ kpc, respectively. We estimate the metallicity and age by comparison with known GCs and by fitting PARSEC and Dartmouth isochrones, finding $[Fe/H]=(-0.70\pm0.2)$ dex and $t=(11.0\pm1.0)$ Gyr. The mean GC PMs from Gaia are $\mu_{\alpha^\ast}=(-4.68 \pm 0.47 )$ mas $yr^{-1}$ and $\mu_{\delta}=(-1.34 \pm 0.45)$ mas $yr^{-1}$. The total luminosity of our cluster is estimated to be $M_{Ks}=(-7.76\pm 0.5)$ mag. We have found a new low-luminosity, old and metal-rich globular cluster, situated in the far side of the Galactic disk, at $R_{G}=11.2$ kpc from the Galactic centre, and at $z=1.0$ kpc below the plane. Interestingly, the location, metallicity and age of this globular cluster are coincident with the Monoceros Ring (MRi) structure.

Abstract: Aim of this paper is the description of a new tool to support institutions in the implementation of targeted countermeasures, based on quantitative and multi-scale elements, for the fight and prevention of emergencies, such as the current COVID-19 pandemic. The tool is a centralized system (web application), single multi-user platform, which relies on Artificial Intelligence (AI) algorithms for the processing of heterogeneous data, and which can produce an output level of risk. The model includes a specific neural network which will be first trained to learn the correlation between selected inputs, related to the case of interest: environmental variables (chemical-physical, such as meteorological), human activity (such as traffic and crowding), level of pollution (in particular the concentration of particulate matter), and epidemiological variables related to the evolution of the contagion. The tool realized in the first phase of the project will serve later both as a decision support system (DSS) with predictive capacity, when fed by the actual measured data, and as a simulation bench performing the tuning of certain input values, to identify which of them lead to a decrease in the degree of risk. Abstract: In recent years, considerable progress has been made in frontline therapy for elderly/physically unfit patients with CLL. The combination of obinutuzumab and chlorambucil (O-Clb) has been shown to prolong progression free survival (PFS, median PFS-31.5 months) and overall survival (OS) compared to chlorambucil alone. More recently, obinutuzumab given in combination with either ibrutinib or venetoclax improved PFS but not OS when compared to O-Clb. In this retrospective multinational, multicenter co-operative study, we evaluated the efficacy and safety of frontline treatment with O ± Clb in unfit patients with CLL, in a “real-world” setting. Patients with documented del (17p13.1)/TP53 mutation were excluded. A total of 437 patients (median age, 75.9 years; median CIRS score, 8; median creatinine clearance, 61.1 mL/min) were included. The clinical overall response rate was 80.3% (clinical complete and partial responses in 38.7% and 41.6% of patients, respectively). Median observation time was 14.1 months and estimated median PFS was 27.6 months (95% CI, 24.2-31.0). In a multivariate analysis, high-risk disease [del (11q22.3) and/or IGHV-unmutated], lymph nodes of diameter > 5 cm, obinutuzumab monotherapy and reduced cumulative dose of obinutuzumab, were all independently associated with shorter PFS. The median OS has not yet been reached and estimated 2-year OS is 88%. In conclusion, in a “real-world” setting, frontline treatment with O-Clb achieves PFS comparable to that reported in clinical trials. Inferior outcomes were noted in patients with del (11q22.3) and/or unmutated IGHV and those treated with obinutuzumab-monotherapy. Thus, O-Clb can be still considered as legitimate frontline therapy for unfit CLL patients with low-risk disease.

Abstract: Chronic lymphocytic leukemia (CLL) is a disease of the elderly, characterized by immunodeficiency. Hence, patients with CLL might be considered more susceptible to severe complications from COVID-19. We undertook this retrospective international multicenter study to characterize the course of COVID-19 in patients with CLL and identify potential predictors of outcome. Of 190 patients with CLL and confirmed COVID-19 diagnosed between 28/03/2020 and 22/05/2020, 151 (79%) presented with severe COVID-19 (need of oxygen and/or intensive care admission). Severe COVID-19 was associated with more advanced age (≥65 years) (odds ratio 3.72 [95% CI 1.79–7.71]). Only 60 patients (39.7%) with severe COVID-19 were receiving or had recent (≤12 months) treatment for CLL at the time of COVID-19 versus 30/39 (76.9%) patients with mild disease. Hospitalization rate for severe COVID-19 was lower (p < 0.05) for patients on ibrutinib versus those on other regimens or off treatment. Of 151 patients with severe disease, 55 (36.4%) succumbed versus only 1/38 (2.6%) with mild disease; age and comorbidities did not impact on mortality. In CLL, (1) COVID-19 severity increases with age; (2) antileukemic treatment (particularly BTK inhibitors) appears to exert a protective effect; (3) age and comorbidities did not impact on mortality, alluding to a relevant role of CLL and immunodeficiency.
