The COVID-19 vaccination campaign in Italy started in December 2020, and, due to the Omicron variant's emergence, a second booster dose was recommended for high-risk individuals and healthcare workers from July 2022. The aim of the study was to evaluate the vaccination coverages for the COVID-19 second booster dose and to identify predictors of its acceptance within the population of medical residents (MRs) of the Fondazione Policlinico Universitario "A. Gemelli" IRCCS (FPG) University Hospital. The study was conducted at FPG from October 4th to December 21st, 2022, and COVID-19 second booster dose and influenza vaccines were administered. The study analyzed collected data and conducted multivariate logistic regressions to explore potential predictors of vaccination adherence. The analyses performed were compared with the sample enrolled in FPG residency programs at the start of the COVID-19 vaccination campaign. 1968 MRs were involved in the 2022-2023 vaccination campaign (mean age 28.97, SD 3.44), and the second booster dose of COVID-19 vaccination coverage was low (18.80%). Almost all participants opted for co-administration of COVID-19 and influenza vaccinations, leading to a similar rate of influenza vaccination coverage (16.26%). Being a frontline resident, meaning a direct involvement in managing COVID-19 patients and vaccination campaigns, was the main predictor of vaccination adherence (OR 1.72, 95% CI 1.25-2.17). The dropping in influenza vaccination coverage in 2022-2023 and the low adherence to COVID-19 second booster dose among young physicians is concerning, calling for tailored vaccination campaigns and interventions.

The aim of this article is to characterize responsible autonomy as a dimension of the regulation of the work process of female medical residents in a neonatal therapy unit of a public hospital in the south of Buenos Aires suburban area, which can be considered as deteriorating for their health. The responsible autonomy of the medical residents is part of a mode of subjectivation that urges them to be workers with a proactive commitment to patient care, and therefore to take responsibility for the fulfilment of their tasks and also for the supervision of the nursing staff, and to be aware of the decisions made by the medical interns. The methodological design was oriented to a case study with an explanatory descriptive purpose, with a qualitative approach through in-depth interviews. One of the most outstanding findings is that the impossibility of making a cut in the work activity and the hindrance of creative intelligence are dimensions of the regulatory process that proved to be deteriorating in the health of the medical residents. Regarding self-perceived health, fatigue and exhaustion, followed by feelings of frustration were the symptoms that appeared most frequently., El objetivo del presente artículo es caracterizar la autonomía responsable como una dimensión de la regulación del proceso de trabajo de las médicas residentes de una terapia neonatal en un hospital público del sur del conurbano bonaerense, que puede considerarse como deteriorante para su salud. La autonomía responsable de las médicas residentes es parte de un modo de subjetivación que las insta a ser trabajadoras con un compromiso proactivo hacia la atención del paciente, y por ende a responsabilizarse del cumplimiento de sus tareas y también de la supervisión del personal de enfermería, y a estar pendientes de la toma de decisiones que realizan las médicas internas. El diseño metodológico se orientó a un estudio de caso con finalidad descriptiva explicativa, con un abordaje de tipo cualitativo a través de la realización de entrevistas en profundidad. Uno de los hallazgos más destacados es que la imposibilidad de realizar un corte en la actividad laboral y la obstaculización de la inteligencia creativa son dimensiones del proceso regulatorio que mostraron ser deteriorantes de la salud en las médicas residentes. Background: Evidence suggests empathy, an essential component of holistic patientcare, may be declining amongst medical students and residents. Yet there are only a few qualitative studies, mostly from West, exploring this phenomenon. Objective: This is a qualitative study of the learning environments of a tertiary care hospital of Pakistan, aimed to understand how junior doctors perceive and develop empathy and articulate the challenges faced by them. Methods: This case study adopted constructivist theoretical perspectives and was conducted from January 2019 to June 2019. Data was gathered from three focus group discussions and analyzed by thematic analysis. Patterns were identified and reported to generate codes, basic and organizing themes. Results: The participants were cognizant of empathy and its significance in patient management. Seniors as role models, first-hand knowledge of patients' plights, active involvement in patient care and witnessing illnesses in dear ones were facets positively influencing empathy. Salient hindrances included enormous work load, gender bias, past negative experiences, language and literacy barriers. Some participants felt like devising their own strategies to cope with workload and providing empathetic care. There is pivotal role of workplace based learning; enabling junior doctors to handle multi-dimensional doctor-patient relationship. Conclusion: Clinical environments are significant for junior doctors' grooming in attainment of empathetic patient care. Exhibiting empathy may be different in eastern and western diaspora. Faculty development could promote refined understanding of empathy and strategies to convey empathetic patientcare ensuring safe medical practice.

Objective: During the first months of 2020, the coronavirus disease of 2019 (COVID-19) spread rapidly and soon reached a pandemic level. With the increasing number of hospitalizations, medical and nursing personnel resources were soon inadequate. As a consequence, medical volunteers became a key human resource and young medical residents in any specialty were hired on a voluntary basis to contribute to take care of patients with COVID-19. This study reports on the lived experience of residents in child neuropsychiatry who volunteered in Italian hotspot COVID-19-designated hospitals during the epidemic outbreak. Methods: A phenomenological, qualitative approach using semi-structured interviews with open-ended questions was used to obtain in-depth narratives of the experience of residents in child neuropsychiatry volunteering in North Italy COVID-19-designated hospitals. All residents (n = 8) participated in the study. Interviews were conducted by an expert researcher trained in qualitative methods. Data analysis was performed by independent coders. Results: Five core themes could be identified from the interviews: acting as mediators on two fronts, facing the shock of COVID-19 reality, capitalizing from specialty education, growing as persons and professionals, and humanizing medical care. Conclusions: This study is unique in providing an in-depth understanding of the experience of young residents in child neuropsychiatry volunteering in general hospitals during the COVID-19 pandemic in Northern Italy. The findings suggest that this experience may be highly beneficial for both the residents and the hospital quality of care. Insights for an accurate planning of residents’ engagement in future healthcare emergencies are provided.

We analysed the impact of several strategies aimed at increasing influenza immunisation rates among Medical Residents (MRs) of an Italian teaching Hospital. During the 2015–16 and 2016–17 influenza seasons we carried out several interventions: ambulatory doubling where vaccination was offered, ambulatory opening hour extension, email invitations, informative materials, forum theatre and vaccination campaign conference. In addition, during the 2016–17 the OSV was carried out: MRs who go to specific wards where they perform influenza immunisation counselling and eventually vaccinate the personnel. 99/1041 (9.5%) and 184/1013 (18.2%) MRs received the vaccine in 2015–16 and in 2016–17 respectively (p < 0.0001). Significant difference was found among three specialisation areas (medicine, surgery, “services”; p < 0.001) and among years of residency (p < 0.001). The highest coverage was found among Infectious Diseases, Paediatrics, Haematology and Hygiene MRs (80%, 67%, 52% and 52% respectively) in 2016–2017 season. The highest coverage increase was found among MRs in Pneumology and Geriatrics (566% and 268%, respectively). The coverage rate has increased even though important differences among specialties persist. The study shows a significant increase in immunisation rate among MRs after the implementation of these strategies.

Objective: Identify the perceptions and knowledge that the resident doctors have about Hand Hygiene (HM) in the Hospital son Llàtzer in 2011 and 2017. Material and methods: Descriptive cross-sectional study through self-administered questionnaire, 23 questions of opinion and 25 of knowledge. The information was collected among the resident physicians of the Hospital are Llàtzer in 2011 and 2017. Results: 94 participants. 20 and 40 women (46.5% and 78.4%, respectively, 2011-2017) (p = 0.001). Average age 28 years. They said they had received training in HM 73 and 31 (53.4% and 60.7%) and used alcohol-based products (PBA) 40 and 42 (93% and 82.3%). On the priority of HM in their center they think that it was High 62.7% and 83.6% (27 and 41) (p = 0.02) and that the managers, heads of service, colleagues and patients promote it (p = 0.001). They knew the main route of transmission of microorganisms 41 and 47 (95% and 92%). The most frequent source of microorganisms was identified by 19 and 19 (44% and 37%). The actions that prevent the transmission of microorganisms to the patient were known moderately but in those that protect health personnel 71.4% and 37.5% (p = 0.002). Friction of hands (FM) parched the skin more than hand washing (LM) 24 and 13 (40% and 27%) (p = 0.002). They know the correct FM time 41% and 47%. They do not know that the use of creams does not favor colonization of the hands 5 and 37 (12% and 40%) (p = 0.002). Conclusions: They claim to have training on HM but does not match the results. The importance of using FM or LM is not known. Training on HM should be emphasized., Obejtivo: Identificar percepciones y conocimientos de los médicos residentes sobre Higiene de Manos (HM) en el Hospital Son Llàtzer en 2011 y 2017. Material y Métodos: Estudio descriptivo transversal mediante cuestionario autoadministrado, 23 preguntas de opinión y 25 de conocimientos. Resultados: 94 participantes, 43 médicos en el 2011 con una media de edad 20.67 (DE:4.2) y 51 en 2017 con una media de edad 27.47 (DE:3.5). 20 y 40 mujeres (46.5% y 78.4% respectivamente 2011-2017) (p=0.001). Dijeron haber recibido formación en HM 23 y 31 (53.4% y 60.7%) y utilizar productos de base alcohólica (PBA) 40 y 42 (93% y 82.3%). Sobre la prioridad de HM en su centro opinan que era Alta 62.7% y 83.6% (27 y 41) (p=0.02). Que los directivos, jefes de servicio, colegas y pacientes promuevan la HM hubo diferencias en los dos periodos (p=0.001). Conocían la principal vía de transmisión de microorganismos 41 y 47 (95% y 92%). La fuente más frecuente de microorganismos la identificaron 19 y 19 (44% y 37%). En las acciones que previenen la transmisión de microorganismos al pacienteno hubo diferencias. Pero en las que protegen al personal sanitario si las hubo 71.4% y 37.5% (p=0.002). La fricción de manos (FM) reseca más la piel que el lavado de manos (LM) 24 y 13 (40% y 27%) (p=0.002). Conocen el tiempo correcto de FM 41% y 47%. Dicen que el uso de cremas no favorece la colonización de las manos 5 y 37 (12% y 40%) (p=0.002). Conclusiones: Refieren tener formación sobre HM pero no se corresponde con los resultados. La importancia para utilizar FM o LM no se conoce. Debería insistirse en la formación pregrado y postgrado en HM.

Introduction. A few number of literature specifically addresses vaccination uptake among Public Health Residents (PHRs). Influ- enza vaccine attitudes and risk perceptions of PHRs across Italy were studied, contributing to literature on influenza vaccination uptake predictors, in particular among young physicians. Methods. An online survey was conducted in 25 Schools of Public Health in Italy in 2011-2012. Results were analysed using preva- lence and logistic regression methods. Results. A total of 365 Italian public health residents were included in the study. Vaccination uptake was confirmed by 22.2 and 33.2% of PHRs in 2010-2011 and 2011-2012, respectively. For the 2010-2011 influenza season, vaccination was associated with male sex (adj-OR 3.43; 95%CI = 1.5-7.84) and vaccination history (adj-OR 29.44; 95%CI = 6.4-135.04). For the 2011-2012 season, vaccination was significantly associated with having had between one and three influenza vaccinations in the previ- ous five years (adj-OR 11.56; 95%CI = 6.44-20.75) or more than three (adj-OR 136.43; 95%CI = 30.8-604.7) and with individual participation in general population vaccination campaigns (adj- OR 1.85; 95%CI = 1.01-3.41). Discussion. Italian residents in public health have no confidence and a low personal risk perception about vaccinations therefore taking no measures to protect patients, general population and themselves. Annual influenza vaccination acceptance is associ- ated with influenza vaccine uptake in the previous years and per- sonal involvement in general population vaccination campaigns. These factors should be considered for the design of future cam- paigns targeting public health residents.

Although influenza vaccination is recognized to be safe and effective, recent studies have confirmed that immunization coverage among health care workers remain generally low, especially among medical residents (MRs). Aim of the present multicenter study was to investigate attitudes and determinants associated with acceptance of influenza vaccination among Italian MRs. A survey was performed in 2012 on MRs attending post-graduate schools of 18 Italian Universities. Each participant was interviewed via an anonymous, self-administered, web-based questionnaire including questions on attitudes regarding influenza vaccination. A total of 2506 MRs were recruited in the survey and 299 (11.9%) of these stated they had accepted influenza vaccination in 2011-2012 season. Vaccinated MRs were older (P = 0.006), working in clinical settings (P = 0.048), and vaccinated in the 2 previous seasons (P<0.001 in both seasons). Moreover, MRs who had recommended influenza vaccination to their patients were significantly more compliant with influenza vaccination uptake in 2011-2012 season (P<0.001). "To avoid spreading influenza among patients" was recognized as the main reason for accepting vaccination by less than 15% of vaccinated MRs. Italian MRs seem to have a very low compliance with influenza vaccination and they seem to accept influenza vaccination as a habit that is unrelated to professional and ethical responsibility. Otherwise, residents who refuse vaccination in the previous seasons usually maintain their behaviors. Promoting correct attitudes and good practice in order to improve the influenza immunization rates of MRs could represent a decisive goal for increasing immunization coverage among health care workers of the future.

Annual influenza vaccination acceptance is associ- ated with influenza vaccine uptake in the previous years and per- sonal involvement in general population vaccination campaigns. These factors should be considered for the design of future cam- paigns targeting public health residents.

The opportunity to educate obese patients on healthy lifestyle practices and address habits related to chronic disease development is present among many physician office visits, though this opportunity is often overlooked (Flocke, Stange, & Goodwin, 1998). Understanding ways to improve the medical education and enhance the counseling skills of future physicians are of practical and personal relevance to current research. By improving the ways in which physicians counsel obese patients on weight management practices, the healthcare paradigm is poised to create an indelible mark on the wellbeing of our nation. Based on the need to address patient education and counseling, the purpose of this study was to investigate the relationship between physician attitudes and health behaviors on their overall communication and communication skills. The study surveyed 38 second-year medical residents at the New York University Bellevue School of Medicine using the Weight Management Survey developed by NYU researchers. Communication and counseling skills were measured using scores from Objective Structured Clinical Exams (OSCEs) administered on the same day as the Weight Management Counseling survey. Results of the survey and the OSCEs were analyzed to investigate relationships between each survey item of three categories of questions (attitudes toward weight management counseling, attitudes toward obese patients, physician health habits) and each of two sets of OSCE scores (obesity-related communication skills and overall counseling skills). Results of the data analysis suggest significant relationships between physicians' personal health habits–specifically dietary habits–and obesity counseling–related communication skills. Results also suggest a significant relationship between physicians' attitudes toward obesity counseling-related communication skills and overall communication skills. Each July thousands begin medical residencies and acquire increased responsibility for patient care. Many have suggested that these new medical residents may produce errors and worsen patient outcomes—the so-called “July Effect;” however, we have found no U.S. evidence documenting this effect. Determine whether fatal medication errors spike in July. We examined all U.S. death certificates, 1979–2006 (n = 62,338,584), focusing on medication errors (n = 244,388). We compared the observed number of deaths in July with the number expected, determined by least-squares regression techniques. We compared the July Effect inside versus outside medical institutions. We also compared the July Effect in counties with versus without teaching hospitals. JR = Observed number of July deaths / Expected number of July deaths. Inside medical institutions, in counties containing teaching hospitals, fatal medication errors spiked by 10% in July and in no other month [JR = 1.10 (1.06–1.14)]. In contrast, there was no July spike in counties without teaching hospitals. The greater the concentration of teaching hospitals in a region, the greater the July spike (r = .80; P = .005). These findings held only for medication errors, not for other causes of death. We found a significant July spike in fatal medication errors inside medical institutions. After assessing competing explanations, we concluded that the July mortality spike results at least partly from changes associated with the arrival of new medical residents.
