Goals of JeffBeWell Expand and enhance Jefferson's Primary Care Integrated Behavioral Health Program (PCIBH) by enhancing integrated behavioral health and education for pharmacy, primary care (FM, IM-PC, Nursing) residents, med students Enhance health professional and student education and training and increase primary care team capacity to provide substance use screening and treatment, including MAT (MOUD) Create a Jefferson Health Primary Care Wellness Council, which will asses needs, implement, and assess effectiveness of evidence-based programs that promote wellness for primary care trainees, providers, and practice teams across Jefferson Health

Objectives Briefly review the "upstreamist" landscape and opportunities to improve health Describe community health workers, their identity and training-- and contrast with other members oft he health care team Review evidence that confirms the roles & effectiveness of community health workers in health care Appreciate how a JFMA CHW contributes to interdisciplinary care that closes gaps and builds health

Objectives By end of session, participants will be able to: Articulate the drivers of health outcomes and the impact of social "influencers" of health on clinical care in a primary care setting Describe the features of the Community Health Needs Assessment & Improvement Plan, including Jefferson's priority areas for 2019-2021 Participate in generating ideas for one "tactic" that could be implemented in our clinical setting(s) and would address a priority area in the Jefferson CNHA
