Purpose: The Focused Assessment with Sonography for Trauma (FAST) is a tool to rapidly detect intraabdominal and intrapericardial fluid with point-of-care ultrasound. Previous studies have questioned the role of FAST in patients with pelvic fractures. The aim of the present study was to assess the accuracy of FAST to detect clinically significant intraabdominal hemorrhage in patients with pelvic fractures. Methods: We included all consecutive patients with pelvic and/or acetabular fractures treated our Level 1 trauma center from 2009–2020. We registered patient and fracture characteristics, FAST investigations and CT descriptions, explorative laparotomy findings, and transfusion needs. We compared FAST to CT and laparotomy findings, and calculated true positive and negative findings, sensitivity, specificity, positive predictive value (PPV) and negative predictive value (NPV). Results: We included 389 patients. FAST had a sensitivity of 75%, a specificity of 98%, a PPV of 84%, and a NPV of 96% for clinically significant intraabdominal bleeding. Patients with retroperitoneal hematomas were at increased risk for laparotomy both because of True-negative FAST and False-positive FAST. Conclusion: FAST is accurate to identify clinically significant intraabdominal blood in patients with severe pelvic fractures and should be a standard asset in these patients. Retroperitoneal hematomas challenge the FAST interpretation and thus the decision making when applying FAST in patients with pelvic fractures.

Pelvic fractures are among the severe injuries of the musculoskeletal system, which are not always diagnosed in a timely manner, they are accompanied by a significant number of complications and adverse effects.Objective: to inform the readers with the clinical case of total hip arthroplasty and pelvic osteosynthesis in a patient with transacetabular fracture with bone fragments displacement and post-traumatic hip joint arthritis.Results: the patient had serious vehicle accident, fracture of the hip and anterior column was not diagnosed in time. After urgent care and stabilizing of the patient, she was discharged into outpatient treatment with severe pain syndrome. Hip joint X-rays were made in 3 months after injury. At preoperative planning we made X-rays of the pelvic and multispiral computed tomography. According to the results of the computed tomography, a plastic model of the pelvis was printed to assess the fracture and degree of displacement. A model was created and an individual implant of titanium powder for pelvic bone osteosynthesis was printed. Direction of screwing and their size are calculated individually. We made hip replacement and the osteosynthesis of the pelvic bones with the individual trabecular structured plate. Defects of the acetabular bottom are filled with autograft from the resected femoral head. In the postoperative period, the weight bearing on the operated limb was limited to the consolidation of the fracture and the restructuring of the bone material. 3 months later consolidation and function restoration were recorded.Conclusions: computed tomography is necessarily in patients with pelvic fractures at preoperative examination. If necessary, additive technologies can be used to create spatial reconstruction of the damaged segment.

Background. Fractures of the pelvic girdle and lower extremities as a result of a road traffic accident are the most frequent localization of injuries in victims, however, the structure of pedestrian and intra-salon injuries has not been sufficiently studied. Objective - to study the nature of injuries in victims of a car injury to the pelvic girdle and lower extremities. Methods. Research material - 170 acts of primary forensic medical examinations of victims with fractures of the pelvic bones and long bones of the lower extremities, received as a result of a non-lethal automobile injury. An expert assessment of the nature of bodily injuries was carried out at the Kharkiv Regional Bureau of Forensic Medicine (KRBSFM) within a period of no more than 1 month after injury. Acts of primary forensic medical examinations were selected at random for the period February - June 2018. Research methods - retrospective analysis, descriptive statistics. Results. Among the injured as a result of a car injury - 129 (75.9%) pedestrians, 23 (13.5%) drivers, 14 (8.2%) front seat passengers and 4 (2.4%) rear seat passengers. Thus, pedestrians and car drivers are the most vulnerable to road traffic injuries; the most protected rear seat passengers. Men predominated among the drivers; in the remaining groups of victims, the gender distribution was generally even. Among the participants in the accident were mainly people of working age. Injuries to the chest (26.5%), head (25.9%), abdomen (16.5%) prevailed among the associated injuries. Conclusion. A distinctive feature of injuries sustained as a result of a collision of a pedestrian or a car with another car with a relatively low speed of movement was an isolated injury to the bones of the pelvic girdle or lower extremities, both during pedestrian and intra-salon trauma. All the victims had injuries of the integumentary system of various localization in the form of subcutaneous hematomas and abrasions (158; 92.9%), bruised lacerations

__Abstract__ High energy pelvic fractures are life-threatening injuries and are among the most challenging injuries to treat. Complete evaluation of the patient with a high energy pelvic fracture is essential because this is rarely an isolated injury. Most deaths in patients with pelvic fractures are not caused by the pelvic fracture itself but are linked to associated injuries. The same forces that lead to disruption of the pelvic ring are frequently associated with abdominal, head, and thoracic injury. Bleeding remains the leading cause of death in patients with pelvic fractures but is rarely the only cause of blood loss in the patient with multiple injuries. In addition to bleeding from the fracture surfaces (i.e., cancellous bone) bleeding from the venous plexus and arterial lesions in a patient with a pelvic ring fracture potentially causes serious complications. These anatomical structures that are at risk are discussed into more detail in the pelvic anatomy section below.

Our aim was to evaluate survival rate and get patient-reported outcome in patients operated for a pelvic or acetabular fracture sustained when jumping from a height as a suicide attempt. Patients and Methods: During the period 2003-2004, 12 patients (11 women) of whom eight were below 30 years of age, were prospectively included. At two years HRQoL (Health-Related Quality of Life) questionnaires (SF-36 and LiSat-11) were used to describe outcome, and at four years a structured psychiatric interview SCID-I (Structured Clinical Interview for DSM-IV Axis I Disorders) was done. Results: At four years all patients were alive. One patient had made a new suicide attempt. Eight patients gave adequate reply on SF-36 and LiSat-11 at two years. In all domains patients scored lower than a norm group with the relatively lowest values in physical domains. Younger patients assessed life as better when compared with middle aged patients. Conclusions: This study showed a very low recurrence rate into suicidal behaviour in a group of jumpers and all patients were alive at four years after a suicidal attempt by jumping. The high proportion of psychiatric disorder in these patients highlights the need for a combined treatment effort between orthopaedic and psychiatric expertise.
