Summary of recommendations and levels of evidence: Chapter 2: Screening and assessment for unhealthy alcohol use Screening Screening for unhealthy alcohol use and appropriate interventions should be implemented in general practice (Level A), hospitals (Level B), emergency departments and community health and welfare settings (Level C). Quantity–frequency measures can detect consumption that exceeds levels in the current Australian guidelines (Level B). The Alcohol Use Disorders Identification Test (AUDIT) is the most effective screening tool and is recommended for use in primary care and hospital settings. For screening in the general community, the AUDIT-C is a suitable alternative (Level A). Indirect biological markers should be used as an adjunct to screening (Level A), and direct measures of alcohol in breath and/or blood can be useful markers of recent use (Level B). Sustained abstinence is the optimal outcome for most patients with alcohol dependence (Level C). Structural brain markers are studied extensively in the field of neurodegeneration, but are thought to occur rather late in the process. Functional measures such as functional connectivity are gaining interest as potentially more subtle markers of neurodegeneration. However, brain structure and function are also affected by ‘normal’ brain ageing. More information is needed on how functional connectivity relates to aging, particularly in the absence of overt neurodegenerative disease. We investigated the association of age with resting-state functional connectivity in 2878 non-demented persons between 50 and 95 years of age (54.1% women) from the population-based Rotterdam Study. We obtained nine well-known resting state networks using data-driven methodology. Within the anterior default mode network, ventral attention network, and sensorimotor network, functional connectivity was significantly lower with older age. In contrast, functional connectivity was higher with older age within the visual network. Between resting state networks, we found patterns of both increases and decreases in connectivity in approximate equal proportions. Our results reinforce the notion that the aging brain undergoes a reorganization process, and serves as a solid basis for exploring functional connectivity as a preclinical marker of neurodegenerative disease.

Democratic decision-making surrounding large infrastructure projects is often prone to conflict and impasse hampering the effectiveness of especially environmental policy. How impasses in decision-making form and how they should be interpreted in the broader context of democratic decisionmaking is still a matter of debate. In order to explore a mechanistic approach to analyzing policy impasse a single case study was conducted of the impasse situation surrounding wind energy infrastructure project Drentse Monden & Oostermoer in the Netherlands. Through media analysis and interviews with stakeholders, institutional, cognitive and relational mechanisms leading to impasse were uncovered and a reconstruction of the impasse through time was made. Different interpretations of the application of state coordination, poor cooperation between various governmental layers and inconsistent information fed distrust and conflict. To fill the void in impasse literature in dealing with the role of the impasse in decision-making, the results of the case study were compared to definitions in the literature. In this case a lack of proper deliberation formed the basis of the impasse.

Neonatal Brachial Plexus Palsy (NBPP) may lead to permanent impairment of arm function. As NBPP occurs when central motor programs develop, these may be ill-formed. We studied elbow flexion and motor imagery with fMRI to search for abnormal motor programming. We compared the cortical activity of adults with conservatively treated NBPP to that of healthy individuals stratified for hand dominance, using fMRI BOLD tasks of elbow flexion and motor imagery of flexion. Additionally, resting-state networks and regional gray matter volume were studied. Sixteen adult NBPP patients (seven men; median age 29 years) and sixteen healthy subjects (seven men, median age 27 years) participated. Cortical activation was significantly higher in patients during flexion imagery compared to healthy individuals and it increased with lesion extent and muscle weakness. The contralateral and ipsilateral premotor cortex, and the contralateral motor cortex showed stronger activity during imagined flexion in the right-handed NBPP subjects compared to healthy individuals. Activity patterns during actual flexion did not differ between groups. No differences in resting-state network connectivity or gray matter amount were found between the groups. NBPP affected imagined but not actual elbow flexion, suggesting an impairment of motor planning which would indicate abnormal motor programming in NBPP.

We introduce a generalized Gaudin Lie algebra and a complete set of mutually commuting quantum invariants allowing the derivation of several families of exactly solvable Hamiltonians. Different Hamiltonians correspond to different representations of the generators of the algebra. The derived exactly-solvable generalized Gaudin models include the Bardeen-Cooper-Schrieffer, Suhl-Matthias-Walker, the Lipkin-Meshkov-Glick, generalized Dicke, the Nuclear Interacting Boson Model, a new exactly-solvable Kondo-like impurity model, and many more that have not been exploited in the physics literature yet.
