Numerical computations and methods have become increasingly crucial in the study of spin foam models across various regimes. This paper adds to this field by introducing new algorithms based on tensor network methods for computing amplitudes, focusing on topological SU(2) BF and Lorentzian EPRL spin foam models. By reorganizing the sums and tensors involved, vertex amplitudes are recast as a sequence of matrix contractions. This reorganization significantly reduces computational complexity and memory usage, allowing for scalable and efficient computations of the amplitudes for larger representation labels on standard consumer hardware--previously infeasible due to the computational demands of high-valent tensors. We apply these tensor network algorithms to analyze the characteristics of various vertex configurations, including Regge and vector geometries for the SU(2) BF theory, demonstrating consistent scaling behavior and differing oscillation patterns. Our benchmarks reveal substantial improvements in computational time and memory allocations, especially for large representation labels. Additionally, these tensor network methods are applicable to generic 2-complexes with multiple vertices, where we introduce partial-coherent vertex amplitudes to streamline the computations. Breast cancer is the most common malignancy affecting women worldwide and is notable for its morphologic and biologic diversity, with varying risks of recurrence following treatment. The Oncotype DX Breast Recurrence Score test is an important predictive and prognostic genomic assay for estrogen receptor-positive breast cancer that guides therapeutic strategies; however, such tests can be expensive, delay care, and are not widely available. The aim of this study was to develop a multi-model approach integrating the analysis of whole slide images and clinicopathologic data to predict their associated breast cancer recurrence risks and categorize these patients into two risk groups according to the predicted score: low and high risk. The proposed novel methodology uses convolutional neural networks for feature extraction and vision transformers for contextual aggregation, complemented by a logistic regression model that analyzes clinicopathologic data for classification into two risk categories. This method was trained and tested on 993 hematoxylin and eosin-stained whole-slide images of breast cancers with corresponding clinicopathological features that had prior Oncotype DX testing. The model's performance was evaluated using an internal test set of 198 patients from Dartmouth Health and an external test set of 418 patients from the University of Chicago. The multi-model approach achieved an AUC of 0.92 (95 percent CI: 0.88-0.96) on the internal set and an AUC of 0.85 (95 percent CI: 0.79-0.90) on the external cohort. These results suggest that with further validation, the proposed methodology could provide an alternative to assist clinicians in personalizing treatment for breast cancer patients and potentially improving their outcomes., Comment: 16 pages, 4 figures and 4 tables

In 2018, the Endourology Disease Group for Excellence (EDGE) published a prospective trial comparing dusting versus basketing during ureteroscopy. One hundred fifty-nine patients were included in the original analysis, which found no difference in stone-free rate at 3 months. We report the intermediate and long-term outcomes of patients included in the original trial. Two analyses were performed. At 1-year, a retrospective chart review was performed, and data collected on stone episodes, Emergency Department (ED) visits, hospital admissions and surgical interventions. To obtain long-term outcomes, the four sites with the largest initial accrual were included in a second phase of data collection with updated analyses. The patients from those sites were contacted, re-consented, and data were collected on stone surgical interventions, stone episodes, stone recurrences on imaging, emergency department (ED) visits, and hospital admissions for stone-related care since their original procedure. One-year follow-up data were collected in 111 of the original 159 (69.8%) patients from the nine sites. There were no statistically significant differences in the number of painful episodes, ED visits, hospital admissions, or surgical interventions. 94 patients from four sites were included in the long-term analysis. There were no statistically significant differences in surgical interventions, painful stone episodes, stone recurrence on imaging, ED visits or hospitalizations for stone-related events between the two groups. Long-term outcomes of dusting versus basketing during ureteroscopy indicate that there are no significant differences in clinical outcomes between the two surgical modalities.

Introduction and Objective Guidelines from the American Urological Association (AUA) and American College of Radiology (ACR) recommend that patients with suspected nephrolithiasis undergo low dose computerized tomography (LD CT KUB) as opposed to higher dose conventional imaging. We hypothesized that even at institutions with established LD protocols, higher dose imaging is common. Methods We identified four academic medical centers where LD CT KUB protocols were implemented to yield an Effective Dose (EDose) consistent with national guidelines. Fifty consecutive adult patients who underwent CT KUB specifically for the evaluation of nephrolithiasis were retrospectively reviewed at each site. Patient age, sex, BMI, imaging location, and EDose (millisieverts, mSv) were recorded. Results 200 patients with a mean age of 54 years were identified. 46 patients (23%) underwent CT KUB with EDose ≤ 4 mSv, accounting for 10-48% of each institution's cohort. 116 patients had a BMI < 30, and would have been expected to receive LD CTs by the AUA criteria for LD CT KUB. Within this subset, only 37 patients (32%) actually underwent LD CT KUB. The highest dose CT KUB at each institution resulted in EDose of 33.8-44.6 mSv, exceeding the recommended exposure of LD CT KUB by tenfold. Conclusions At academic institutions where LD CT KUB was implemented for the evaluation of nephrolithiasis, a minority of patients with BMI < 30 receive guideline-concordant imaging. Differences in patient BMI did not account for the variation in radiation exposure. Further research is necessary to elucidate barriers to low dose CT implementation.

Spin foams provide path integrals for quantum gravity, which employ discretizations as regulator. To obtain regulator independent predictions, we must remove these fiducial structures in a suitable refinement limit. In this chapter we present the current state of research: We begin with a discussion on the role of diffeomorphism symmetries in discrete systems, the notion of scale in background independent theories and how we can consistently improve theories via renormalization to reduce regulator dependence. We present the consistent boundary formulation, which provides a renormalization framework for background independent theories, and discuss tensor network methods and restricted spin foams, which provide concrete renormalization algorithms aiming at the construction of consistent boundary amplitudes for spin foams. Numerical methods in spin-foam models have significantly advanced in the last few years, yet challenges remain in efficiently extracting results for amplitudes with many quantum degrees of freedom. In this paper we sketch a proposal for a ``hybrid algorithm'' that would use both the full quantum amplitude and its asymptotic approximation in the relevant regimes. As a first step towards the algorithm, we derive a new representation of the partition function where each spin-foam vertex possesses its own coherent data, such that it can be individually asymptotically approximated. We do this through the implementation of gluing constraints between vertices, which we study numerically. We further derive an asymptotic expression for the constraints for arbitrary boundary data, including data for which there are no critical points. From this new representation we conjecture an intermediate quasi-geometric spin-foam regime describing a superposition of semi-classical vertices glued in a non-matching way via the gluing constraints., Comment: 36 pages, 13 figures

For the construction of the Lorentzian path integral for gravity one faces two main questions: Firstly, what configurations to include, in particular whether to allow Lorentzian metrics that violate causality conditions. And secondly, how to evaluate a highly oscillatory path integral over unbounded domains. Relying on Picard-Lefschetz theory to address the second question for discrete Regge gravity, we will illustrate that it can also answer the first question. To this end we will define the Regge action for complexified variables and study its analytical continuation. Although there have been previously two different versions defined for the Lorentzian Regge action, we will show that the complex action is unique. More precisely, starting from the different definitions for the action one arrives at equivalent analytical extensions. The difference between the two Lorentzian versions is only realized along branch cuts which arise for a certain class of causality violating configurations. As an application we discuss the path integral describing a finite evolution step of the discretized deSitter universe. Lattice actions and amplitudes that perfectly mirror continuum physics are known as perfect discretizations. Such perfect discretizations naturally preserve the symmetries of the continuum. This is a key concern for general relativity, where diffeomorphism symmetry and dynamics are deeply connected, and diffeomorphisms play a crucial role in quantization. In this work we construct for the first time a perfect discretizations for four-dimensional linearized gravity. We show how the perfect discretizations do lead to a straightforward construction of the one-loop quantum corrections for manifolds with boundary. This will also illustrate, that for manifolds with boundaries, gauge modes that affect the boundary, need to be taken into account for the computation of the one-loop correction. This work provides therefore an evaluation of the boundary action for the diffeomorphism modes for a general class of backgrounds., Comment: 22 pages

Making the Lorentzian path integral for quantum gravity well-defined and computable has been a long standing challenge. In this work we adopt the recently proposed effective spin foam models to the Lorentzian case. This defines a path integral over discrete Lorentzian quantum geometric configurations, which include metric and torsion degrees of freedom. The torsion degrees of freedom arise due to an anomaly, which is parametrized by the Barbero--Immirzi parameter. Requiring a semi-classical regime constrains this parameter, but the precise bound has to be determined by probing the dynamics. The effective models provide the computationally most efficient spin foam models yet, which allows us to perform first tests for determining the semi-classical regime. The purpose of this paper is to present the findings of the users’ perception of library facilities on quality of service at Sam Jonah Library University of Cape Coast, Ghana using Library Quality (LibQUAL) model approach. The survey used the LibQUAL+ model to evaluate quality of service at Sam Jonah Library University of Cape Coast, Ghana. The data were collected from library users by administering the LibQUAL+ questionnaire in a printed format. Using the survey design, both open-ended and close-ended questions were used to obtain data from 400 respondents, comprising of 310 and 50 undergraduate and postgraduate students, respectively and 40 faculty members. Statistical Package for the Social Sciences (SPSS) version 21 was used to analyze data from the questionnaire and descriptive statistics in the form of frequencies and percentages. Inferential statistics was also used to determine the relationship between gender and adequacy of library resources. Findings from the study showed that majority of the respondents agreed that the library had adequate number of computers to meet their needs. The respondents also affirmed that the library had comfortable tables and chairs for effective learning and a space that is conducive for learning. However, postgraduate students did not consider the library environment conducive for learning. The respondents indicated that the printing materials in the library were inadequate. Students’ expectations were higher than their perceptions on what the library provided. Therefore, the researchers recommended that the library should establish knowledge commons for undergraduate students and research commons for postgraduate students.

The Facility for Antiproton and Ion Research (FAIR) in Darmstadt, Germany, provides unique possibilities for a new generation of hadron-, nuclear- and atomic physics experiments. The future antiProton ANnihilations at DArmstadt (PANDA or $\overline{\rm P}$ANDA) experiment at FAIR will offer a broad physics programme, covering different aspects of the strong interaction. Understanding the latter in the non-perturbative regime remains one of the greatest challenges in contemporary physics. The antiproton-nucleon interaction studied with PANDA provides crucial tests in this area. Furthermore, the high-intensity, low-energy domain of PANDA allows for searches for physics beyond the Standard Model, e.g. through high precision symmetry tests. This paper takes into account a staged approach for the detector setup and for the delivered luminosity from the accelerator. The available detector setup at the time of the delivery of the first antiproton beams in the HESR storage ring is referred to as the \textit{Phase One} setup. Diatoms are ubiquitous microalgae that exhibit a characteristic golden-brown color due to the high quantity of fucoxanthin. Fucoxanthin is a brown pigment molecule that belongs to the xanthophyll class of carotenoids. Owing to unique chemical properties, this pigment is known for a variety of biological functions having important medicinal properties like anti-inflammatory, anti-oxidant, anti-obesity, anti-diabetic, anti-cancer, anti-osteoporotic, anti-hypertensive, and anti-microbial activities. On account of its versatile applications and growing demands, it is one of the exorbitant pigments globally and plays a crucial role in the circular bioeconomy. Extraction of fucoxanthin from a natural source is considered most efficient, eco-friendly, and cost-effective. Therefore, selecting a suitable natural source for efficient extraction, quantification, purification of this economically important biomolecule is of utmost importance. This paper presents the current understanding of the fucoxanthin biosynthetic pathway in diatoms, characterizations of essential genes involved in its biosynthesis, and various efforts to enhance production using the biorefinery approach reviewed. This review also focuses on current knowledge of extraction methods and biological functions of fucoxanthin with its potential health benefits.

Objective: Evaluate the effect of sotagliflozin, a dual inhibitor of sodium glucose cotransporter (SGLT) 1 and 2, on arterial stiffness in patients with type 1 diabetes (T1D) treated with sotagliflozin as adjunct to optimized insulin therapy. Methods: In this post hoc analysis, indirect markers of arterial stiffness, including pulse pressure, mean arterial pressure (MAP), and double product, were calculated using observed systolic blood pressure (SBP), diastolic blood pressure (DBP), or pulse rate at 24 weeks using data from a pooled patient population from the inTandem1 and inTandem2 randomized controlled trials (n = 1575). Results: Baseline characteristics were similar among groups. Relative to placebo at Week 24, sotagliflozin 200 mg and 400 mg reduced SBP by 2.03 mm Hg (95% CI −3.30 to −0.75; p = 0.0019) and 2.85 mm Hg (−4.12 to −1.57; p < 0.0001), respectively. DBP decreased by 1.1 and 0.9 mm Hg, MAP by 1.4 and 1.6 mm Hg, and double product by 202.5 and 221.1 bpm × mm Hg, respectively (p < 0.05 for all). No increases in heart rate were observed. Conclusion: In adults with T1D, adding sotagliflozin to insulin significantly reduced blood pressure and other markers of arterial stiffness and vascular resistance.

The first computation of a spin foam dynamics that provides a test of the quantum equations of motions of gravity is presented. Specifically, a triangulation that includes an inner edge is treated. The computation leverages the recently introduced effective spin foam models, which are particularly numerically efficient. Previous work has raised the concern of a flatness problem in spin foam dynamics, identifying the potential for the dynamics to lead to flat geometries in the small $\hbar$ semiclassical limit. The numerical results presented here expose a rich semiclassical regime, but one that must be understood as an interplay between the various parameters of the spin foam model. In particular, the scale of the triangulation, fixed by the areas of its boundary triangles, the discreteness of the area spectrum, input from Loop Quantum Gravity, and the curvature scales around the bulk triangles, all enter the characterization of the semiclassical regime identified here. In addition to these results on the dynamics, we show that the subtle nature of the semiclassical regime is a generic feature of the path integral quantization of systems with second class constraints., Comment: 39 pages, 19 figures

We report measurements of the photon beam asymmetry $\Sigma$ for the reaction $\vec{\gamma} p\to K^+\Sigma^0$(1193) using the GlueX spectrometer in Hall D at Jefferson Lab. Data were collected using a linearly polarized photon beam in the energy range of 8.2-8.8 GeV incident on a liquid hydrogen target. The beam asymmetry $\Sigma$ was measured as a function of the Mandelstam variable $t$, and a single value of $\Sigma$ was extracted for events produced in the $u$-channel. These are the first exclusive measurements of the photon beam asymmetry $\Sigma$ for the reaction in this energy range. For the $t$-channel, the measured beam asymmetry is close to unity over the $t$-range studied, $-t=(0.1-1.4)~$(GeV/$c$)$^{2}$, with an average value of $\Sigma = 1.00\pm 0.05$. This agrees with theoretical models that describe the reaction via the natural-parity exchange of the $K^{*}$(892) Regge trajectory. A value of $\Sigma = 0.41 \pm 0.09$ is obtained for the $u$-channel integrated up to $-u=2.0$~(GeV/$c$)$^{2}$.

Background: Individuals with sickle cell disease (SCD) are susceptible to infective conditions that predispose them to hemolysis and anemia. Folic acid is recommended as a preventative measure against anemia in SCD patients; however, there is scarce literature on the implications of this practice.Patients and Methods: Plasma concentrations of folate were measured in acutely ill pediatric SCD patients presenting with malaria or bacteremia and compared with those of SCD patients in steady state, or acutely ill non-SCD patients with confirmed malaria.Results: The proportion of individuals with high (>45.3 nmol/L) folate concentrations was 29.5% (13/44), 18.2% (8/44), 33.3% (6/18), and 0% in the SCD-malaria, SCD steady state, SCD bacteremia, and the non-SCD malaria groups, respectively. The proportion of SCD patients with high folate levels did not vary significantly at steady state and during confirmed malaria (p = 0.216), and during acute bacteremia (p = 0.20). The median (interquartile range) plasma folate levels were 34.50 (24.40-52.00 nmol/L), 33.40 (15.83-60.85 nmol/L), 30.85 (24.68-39.65 nmol/L), and 13.30 (10.03-17.18 nmol/L), respectively, in the SCD malaria, SCD bacteremia, SCD steady state, and the non-SCD malaria sub-groups. The median folate levels of SCD steady state, SCD malaria, and SCD bacteremia sub-groups differed significantly (p < 0.0001) when compared with non-SCD patients, but the levels in the SCD bacteremia and malaria groups were not significantly different from the SCD steady state group.Conclusion: Elevated levels of plasma folate were found in a high proportion of pediatric SCD patients. The implications of such elevated folate levels in pediatric SCD patients are unknown but may suggest a need for review of current recommendations for prophylactic doses of folic acid in SCD patients.

A research project has been initiated to help diagnose, design, and test interventions for better inclusion of the rural poor, women, and youth in the tilapia value chain. This report describes the baseline data on 603 small-scale tilapia farmers in Ghana. The data collected during two-hour face-to-face interviews during May–July 2019 are disaggregated by socioeconomic indicators, gender, and age group. Baseline data show that 9 percent of farm managers and owners were women, and an additional 9 percent of farms engaged women in some decision-making. Moreover, women contributed 16 percent of family labor and 5 percent of hired labor on farms. Youth represented 14 and 24 percent of owners and managers, respectively, but contributed 68 percent of the total family and hired labor on farms. A large majority of managers and owners had at least high school education, with a third of owners and a quarter of managers attaining at least a college degree. In Brong Ahafo and Ashanti regions especially, most farmers engaged mainly in crop farming and non-farm businesses as their main livelihood, with fish farming as a small contributor to overall household income and livelihood. Farmers in all regions had poor record-keeping and management practices and low compliance with sanitation, fish health, and food safety standards. A wide variety of input usage, management practices, and performance was observed among fish farms. As a result, the profitability of fish farms was also wide-ranging, between –12.00 and 46.00 cedi per m2, with an average of 8.82 cedi per m2. Despite wide variability in production and profits, the majority of farmers experienced positive profits. On average, a farmer received a profit of 2.4 cedi per kilogram of tilapia produced or a 27 percent profit margin. The

We report measurements of the photon beam asymmetry $\Sigma$ for the reaction $\vec{\gamma} p\to K^+\Sigma^0$(1193) using the GlueX spectrometer in Hall D at Jefferson Lab. Data were collected using a linearly polarized photon beam in the energy range of 8.2-8.8 GeV incident on a liquid hydrogen target. The beam asymmetry $\Sigma$ was measured as a function of the Mandelstam variable $t$, and a single value of $\Sigma$ was extracted for events produced in the $u$-channel. These are the first exclusive measurements of the photon beam asymmetry $\Sigma$ for the reaction in this energy range. For the $t$-channel, the measured beam asymmetry is close to unity over the $t$-range studied, $-t=(0.1-1.4)~$(GeV/$c$)$^{2}$, with an average value of $\Sigma = 1.00\pm 0.05$. This agrees with theoretical models that describe the reaction via the natural-parity exchange of the $K^{*}$(892) Regge trajectory. A value of $\Sigma = 0.41 \pm 0.09$ is obtained for the $u$-channel integrated up to $-u=2.0$~(GeV/$c$)$^{2}$.

It remains unclear how this growth is and can be made more inclusive of poor and young women and men. This study was conducted to analyze different inclusive business models along the fish seed value chain that can potentially be implemented in Ghana. Based on literature review, field interviews, analysis of survey data, and stakeholder workshops, this study develops four business model prototypes for seed multiplication and distribution to increase farmers’ access to and use of quality tilapia seed: (1) Nursery , which buy fish fry from a reliable hatchery, transport them to locations near other farmers, and grow it to a larger size; (2) Local feed mill, with pelleting machine and technical knowledge to advise on feed formulation; (3) Agents, technical experts who supply fingerlings, handle transport and marketing, and provide technical advice; and (4) Local hatchery, which obtains brood stock from a reliable source, produces local fingerlings to sell to nearby farmers, and provides technical support. Initial ex ante financial and profitability analyses were undertaken and will be refined according to the actual context in the particular district where the sensitization and pilot-testing will take place. These business models have the potential not only to increase farmers’ access to and use of quality tilapia seed but also to provide livelihood and income generation along the fish seed value chain.

BackgroundTo compare head to head two end-engaging nitinol stone retrieval devices available to urologists, in terms of durability, versatility and efficacy.MethodsFor durability testing, 30 NGage and Dakota baskets were cycled 20 times between grasping and releasing synthetic stone models and evaluated for damage or device failure. For versatility and efficacy testing, baskets were assessed in their ability to capture and release stone models from 1 to 11 mm. Each stone was raised above the capture site and the basket was opened to passively release the stone. If the stone did not release, the basket handle was shaken and the OpenSure feature employed if needed. Manual release was used as a last resort.ResultsDurability-the Cook NGage demonstrated a statistically significant increased rate of visible device breakdown (P=0.0046) in 8 of 30 (26.7%) devices vs. 0 of 30 Dakota devices, with mean damage at 13.5 cycles. Versatility and efficacy-both 8 mm baskets successfully captured stones from 1-8 mm. The Dakota more effectively released 7-8 mm stones (P<0.0001). NGage required manual release of 8 mm stones in 13 cases compared to none with Dakota. For 11 mm baskets, the Dakota released all stones up to 10 mm with simple opening, while the NGage released 10 of 15 (67%) of 9 mm stones and 1 of 15 (7%) of 10 mm stones by simple opening. For 11 mm stones, the Dakota captured 100% whereas NGage could not capture any.ConclusionsBoth baskets showed similar durability characteristics. The Dakota basket more effectively captured and released stones over 7 mm, as compared to the NGage basket. The OpenSure aspect conferred an advantage in handling and release of larger stones. These in vitro results demonstrate potential versatility, durability and efficacy of the Dakota basket.

Higher gauge theories play a prominent role in the construction of 4d topological invariants and have been long ago proposed as a tool for 4d quantum gravity. The Yetter lattice model and its continuum counterpart, the BFCG theory, generalize BF theory to 2-gauge groups and -- when specialized to 4d and the Poincar\'e 2-group -- they provide an exactly solvable topologically-flat version of 4d general relativity. The 2-Poincar\'e Yetter model was conjectured to be equivalent to a state sum model of quantum flat spacetime developed by Baratin and Freidel after work by Korepanov (KBF model). This conjecture was motivated by the origin of the KBF model in the theory of 2-representations of the Poincar\'e 2-group. Its proof, however, has remained elusive due to the lack of a generalized Peter-Weyl theorem for 2-groups. In this work we prove this conjecture. Our proof avoids the Peter-Weyl theorem and rather leverages the geometrical content of the Yetter model. Key for the proof is the introduction of a kinematical boundary Hilbert space on which 1- and 2-Lorentz invariance is imposed. Geometrically this allows the identification of (quantum) tetrad variables and of the associated (quantum) Levi-Civita connection. States in this Hilbert space are labelled by quantum numbers that match the 2-group representation labels. Our results open exciting opportunities for the construction of new representations of quantum geometries. Compared to loop quantum gravity, the higher gauge theory framework provides a quantum representation of the ADM-Regge initial data, including an identification of the intrinsic and extrinsic curvature. Furthermore, it leads to a version of the diffeomorphism and Hamiltonian constraints that acts on the vertices of the discretization, thus providing a prospect for a quantum realization of the hypersurface deformation algebra in 4d., Comment: 53 pages, 6 figures

In this work we construct holographic boundary theories for linearized 3D gravity, for a general family of finite or quasi-local boundaries. These boundary theories are directly derived from the dynamics of 3D gravity by computing the effective action for a geometric boundary observable, which measures the geodesic length from a given boundary point to some centre in the bulk manifold. We identify the general form for these boundary theories and find that these are Liouville like with a coupling to the boundary Ricci scalar. This is illustrated with various examples, which each offer interesting insights into the structure of holographic boundary theories., Comment: 47 pages

Purpose: Kidney stone patients routinely have CT scans during diagnostic work-up before being referred to a tertiary center. How often these patients exceed the recommended dose limits for occupational radiation exposure of >100 mSv for 5 years and >50 mSv in a single year from CT alone remains unknown. This study aimed to quantify radiation doses from CTs received by stone patients before their evaluation at a tertiary care stone clinic. Methods: From November 2015 to March 2017, consecutive new patients enrolled into the Registry for Stones of the Kidney and Ureter (ReSKU™) had the dose-length product of every available CT abdomen/pelvis within 5 years of their initial visit recorded, allowing for an effective dose (EDose) calculation. Multivariate logistic regression analysis identified factors associated with exceeding recommended dose limits. Models were created to test radiation reducing effects of low-dose and phase-reduction CT protocols. Results: Of 343 noncontrast CTs performed, only 29 (8%) were low-dose CTs (calculated EDose <4 mSv). Among 389 total patients, 101 (26%) and 25 (6%) had an EDose >20 mSv and >50 mSv/year, respectively. Increased body mass index, number of scans, and multiphase scans were associated with exceeding exposure thresholds (p < 0.01). The implementation of a low-dose CT protocol decreased the estimated number of scans contributing to overexposure by >50%. Conclusions: Stone patients referred to a tertiary stone center may receive excessive radiation from CT scans alone. Unnecessary phases and underutilization of low-dose CT protocols continue to take place. Enacting new approaches to CT protocols may spare stone patients from exceeding recommended dose limits.

Background: There is limited information on the safety or efficacy of currently recommended antimalarial drugs in patients with sickle cell disease (SCD), a population predisposed to worse outcomes if affected by acute malaria. Artesunate-amodiaquine (ASAQ) is the first-line treatment for uncomplicated malaria (UM) in many malaria-endemic countries and is also used for treatment of UM in SCD patients. There is, however, no information to date, on the pharmacokinetics (PK) of amodiaquine or artesunate or the metabolites of these drugs in SCD patients.Objectives: This study sought to determine the PK of desethylamodiaquine (DEAQ), the main active metabolite of amodiaquine, among paediatric SCD patients with UM treated with artesunate-amodiaquine (ASAQ).Methods: Plasma concentration-time data (median DEAQ levels) of SCD children (n = 16) was initially compared with those of concurrently recruited non-SCD paediatric patients with acute UM (n = 13). A population PK modelling approach was then used to analyze plasma DEAQ concentrations obtained between 64 and 169 hours after oral administration of ASAQ in paediatric SCD patients with acute UM (n = 16). To improve PK modeling, DEAQ concentration-time data (n = 21) from SCD was merged with DEAQ concentration-time data (n = 169) of a historical paediatric population treated with ASAQ (n = 103) from the same study setting.Results: The median DEAQ concentrations on days 3 and 7 were comparatively lower in the SCD patients compared to the non-SCD patients. A two-compartment model best described the plasma DEAQ concentration-time data of the merged data (current SCD data and historical data). The estimated population clearance of DEAQ was higher in the SCD patients (67 L/h, 21% relative standard error (RSE) compared with the non-SCD population (15.5 L/h, 32% RSE). The central volume of distribution was larger in the SCD patients compared with the non-SCD patients (4400 L, 43% RSE vs. 368 L, 34% RSE).

Nile tilapia (Oreochromis niloticus) is a globally significant aquaculture species rapidly gaining status as a farmed commodity. In West Africa, wild Nile tilapia genetic resources are abundant yet knowledge of fine-scale population structure and patterns of natural genetic variation are limited. Coinciding with this is a burgeoning growth in tilapia aquaculture in Ghana and other countries within the region underpinned by locally available genetic resources. Using 192 single nucleotide polymorphism (SNP) markers this study conducted a genetic survey of Nile tilapia throughout West Africa, sampling 23 wild populations across eight countries (Benin, Burkina Faso, Côte d’Ivoire, Ghana, Togo, Mali, Gambia and Senegal), representing the major catchments of the Volta, Niger, Senegal and Gambia River basins. A pattern of isolation-by-distance and significant spatial genetic structure was identified throughout West Africa (Global FST = 0.144), which largely corresponds to major river basins and, to a lesser extent, sub-basins. Two populations from the Gambia River (Kudang and Walekounda), one from the western Niger River (Lake Sélingué) and one from the upper Red Volta River (Kongoussi) showed markedly lower levels of diversity and high genetic differentiation compared to all other populations, suggesting genetically isolated populations occurring across the region. Genetic structure within the Volta Basin did not always follow the pattern expected for sub-river basins. This study identifies clear genetic structuring and differentiation amongst West African Nile tilapia populations, which concur with broad patterns found in previous studies. In addition, we provide new evidence for fine-scale genetic structuring within the Volta Basin and previously unidentified genetic differences of populations in Gambia. The estimated population clearance of DEAQ was higher in the SCD patients (67 L/h, 21% relative standard error (RSE) compared with the non-SCD population (15.5 L/h, 32% RSE). The central volume of distribution was larger in the SCD patients compared with the non-SCD patients (4400 L, 43% RSE vs. 368 L, 34% RSE).

Gravity is uniquely situated in between classical topological field theories and standard local field theories. This can be seen in the the quasi-local nature of gravitational observables, but is nowhere more apparent than in gravity's holographic formulation. Holography holds promise for simplifying computations in quantum gravity. While holographic descriptions of three-dimensional spacetimes and of spacetimes with a negative cosmological constant are well-developed, a complete boundary description of zero curvature, four-dimensional spacetime is not currently available. Building on previous work in three-dimensions, we provide a new route to four-dimensional holography and its boundary gravitons. Using Regge calculus linearized around a flat Euclidean background with the topology of a solid hyper-torus, we obtain the effective action for a dual boundary theory which describes the dynamics of the boundary gravitons. Remarkably, in the continuum limit and at large radii this boundary theory is local and closely analogous to the corresponding result in three-dimensions. The boundary effective action has a degenerate kinetic term that leads to singularities in the one-loop partition function that are independent of the discretization. Discretization of general relativity is a promising route towards quantum gravity. Discrete geometries have a finite number of degrees of freedom and can mimic aspects of quantum geometry. However, selection of the correct discrete freedoms and description of their dynamics has remained a challenging problem. We explore classical area Regge calculus, an alternative to standard Regge calculus where instead of lengths, the areas of a simplicial discretization are fundamental. There are a number of surprises: though the equations of motion impose flatness we show that diffeomorphism symmetry is broken for a large class of area Regge geometries. This is due to degrees of freedom not available in the length calculus. In particular, an area discretization only imposes that the areas of glued simplicial faces agrees; their shapes need not be the same. We enumerate and characterize these non-metric, or `twisted', degrees of freedom and provide tools for understanding their dynamics. The non-metric degrees of freedom also lead to fewer invariances of the area Regge action---in comparison to the length action---under local changes of the triangulation (Pachner moves). This means that invariance properties can be used to classify the dynamics of spin foam models. The dynamics of phosphorus (P) in the environment is important for regulating nutrient cycles in natural and managed ecosystems and an integral part in assessing biological resilience against environmental change. Organic P (P-o) compounds play key roles in biological and ecosystems function in the terrestrial environment being critical to cell function, growth and reproduction. We asked a group of experts to consider the global issues associated with P-o in the terrestrial environment, methodological strengths and weaknesses, benefits to be gained from understanding the P-o cycle, and to set priorities for P-o research. We identified seven key opportunities for P-o research including: the need for integrated, quality controlled and functionally based methodologies; assessment of stoichiometry with other elements in organic matter; understanding the dynamics of P-o in natural and managed systems; the role of microorganisms in controlling P-o cycles; the implications of nanoparticles in the environment and the need for better modelling and communication of the research. Each priority is discussed and a statement of intent for the P-o research community is made that highlights there are key contributions to be made toward understanding biogeochemical cycles, dynamics and function of natural ecosystems and the management of agricultural systems., A Correction to this article was published on 02 December 2017.
