Abstract: One of the aims of the Work Package 2 “Exposure” of the European project RadoNorm (2020-2025) is to develop methodologies and tools applicable at European level to identify and quantify exposure of population and environment due to Naturally Occurring Radioactive Material (NORM). In particular, focus is on adapting and optimizing current approaches for identification and evaluation of exposure in light of international and national requirements for handling NORM more compelling than in the past and based on new available scientific evidence. The project is in an advanced stage of progress and several results have been obtained. However, these also let emerge research questions, which may be worth addressing in the future. For example, a methodology to establish a NORM inventory has been developed and applied to gather systematic information on NORM from European countries. Information gained, abundant for naturally occurring radionuclides (NOR), indicated that additional and more systematic information on amounts and handling approaches of other contaminants is needed. This as a basis for the establishment of a more efficient optimized and integrated approach for evaluating NORM involving situations.Impact of most recent ICRP dose coefficients for intake of radionuclides by workers and for external radiation has been analyzed for several generic NORM scenarios. Moreover, with the aim to help stakeholders for a practicable implementation of the radiation protection requirements, screening values (defined in terms of activity concentration corresponding to annual effective dose of 1 mSv/year) have been derived for NORM residues disposable in conventional landfills, and for the reuse of NORM sludge as fertilizer in agriculture. For obtaining these screening values, however, generic consideration of groundwater pathway proved difficult and a systematic analysis of types of landfills and typical hydrological characteristics of these landfills at EU level (i.e. a kind of

Abstract: Background and aim: Elevated triglyceride (TG) levels seem to identify subjects at increased cardiovascular risk, independent of LDL-C levels. We sought to evaluate the predictive role of hypertriglyceridemia, defined as TG levels ≥150 mg/dl, in very high risk (VHR) patients with chronic coronary syndromes (CCS) treated with statins. Methods and results: Using the data from the STable Coronary Artery Diseases RegisTry (START) study, an Italian nationwide registry, we assessed the association between the TG levels and baseline clinical characteristics, pharmacological treatment and major adverse cardio-cerebrovascular events (MACCE) at 1 year in a large cohort of statin-treated patients at VHR. Of the 4751 consecutive patients with CCS enrolled in the registry and classified as VHR, 2652 (55.8%) had TG values available (mean 120.6 ± 54.9) and were treated with at least a statin at baseline: 2019 (76.1%) with TG < 150 and 633 (23.9%) with TG ≥ 150 mg/dl. At 1 year from enrolment, MACCE occurred in 168 (6.3%) patients, without differences between the two groups of TG (5.9 vs 7.6%; p = 0.14). At multivariable analysis, hypertriglyceridemia did not result as independent predictor of the MACCE (hazard ratio: 1.16; 95% confidence intervals: 0.82–1.64; p = 0.42). Conclusions: In the present large, nationwide cohort of consecutive CCS patients at VHR with statin-controlled LDL-C levels, hypertriglyceridemia was present in around 24% of cases and did not result as predictor of MACCE at 1 year. Further studies with a longer follow-up and larger sample size are needed to better define the prognostic role of TG levels when intensive LDL lowering therapies are used.

Abstract: Within the frame of the RadoNorm project (https://www.radonorm.eu/), the goal is to better understand the importance of hydrogeochemical and chemical processes for radionuclides’ transfer in the environment with a focus on Naturally Occurring Radioactive Material (NORM). In that context, a wetland in a downstream area of a former uranium mine tailing, i. e. the Rophin site, has been selected and the following water flows and solute transport models, in 1, 2 or 3 D could be used: HYTEC [1], HYDRUS [2], CIEMAT model [3], and MELODIE [4]. This application should allow to understand the fate of uranium and radium observed in the wetland area, the link between radionuclides’ concentration in the wetland and in the crossing stream and also to highlight a possible transfer of radionuclides to the groundwater. The Rophin site located in the department of Puy-de-Dôme of the region Auvergne-Rhone-Alpes (France) where measured dose rates are 20 to 30 times higher than the background along the Gourgeat stream and in a so-called “wetland area”. A source of contamination is present due to successive discharges from settling ponds during the operational phase of the mine and located in the whitish layer of the wetland, where three different soil horizons are identified (an organic-rich surface layer, a whitish layer, and a paleosol layer). The watershed has been equipped since 2019 with piezometers and surface sensors to monitor the water (electrical conductivity, temperature and water flow). Metereological data is available to identify patterns for precipitation regimes for the wet and dry seasons. First results from measuring campaigns in 2021 and 2022 confirm the presence of a groundwater table in the wetland, which is rather close to the surface and varies depending on meterological conditions. Abstract: Objectives: Survival and death prognostic factors of SSc patients varied during the past decades. We aimed to update the 5- and 10-year survival rates and identify prognostic factors in a multicentre cohort of Italian SSc patients diagnosed after 2009. Material and methods: Patients who received a diagnosis of SSc after 1 January 2009 and were longitudinally followed up in four Italian rheumatologic centres were retrospectively assessed up to 31 December 2020. Overall survival of SSc patients was described using the Kaplan–Meier method. Predictors of mortality at 10-year follow-up were assessed by the Cox regression model. A comparison of our cohort with the Italian general population was performed by determining the standardized mortality ratio (SMR). Results: A total of 912 patients (91.6% females, 20% dcSSc) were included. Overall survival rates at 5 and 10 years were 94.4% and 89.4%, respectively. The SMR was 0.96 (95% CI 0.81, 1.13), like that expected in the Italian general population. Pulmonary arterial hypertension (PAH) and interstitial lung disease (ILD) associated with pulmonary hypertension (PH) significantly reduced survival (P < 0.0001). Main death predictors were male gender (HR 1⁄4 2.76), diffuse cutaneous involvement (HR 1⁄4 3.14), older age at diagnosis (HR 1⁄4 1.08), PAH (HR 1⁄4 3.21), ILD-associated PH (HR 1⁄4 4.11), comorbidities (HR 1⁄4 3.53) and glucocorticoid treatment (HR1⁄4 2.02). Conclusions: In the past decade, SSc patients have reached similar mortality of that expected in the Italian general population. Male gender, diffuse cutaneous involvement, comorbidities and PAH with or without ILD represent the main poor prognostic factors.

Abstract: Purpose: to evaluate the use, effectiveness, and adverse events of intravenous brivaracetam (BRV) in status epilepticus (SE). Methods: a retrospective multicentric study involving 24 Italian neurology units was performed from March 2018 to June 2020. A shared case report form was used across participating centres to limit biases of retrospective data collection. Diagnosis and classification of SE followed the 2015 ILAE proposal. We considered a trial with BRV a success when it was the last administered drug prior the clinical and/or EEG resolution of seizures, and the SE did not recur during hospital observation. In addition, we considered cases with early response, defined as SE resolved within 6 h after BRV administration. Results: 56 patients were included (mean age 62 years; 57 % male). A previous diagnosis of epilepsy was present in 21 (38 %). Regarding SE etiology classification 46 % were acute symptomatic, 18 % remote and 16 % progressive symptomatic. SE episodes with prominent motor features were the majority (80 %). BRV was administered as first drug after benzodiazepine failure in 21 % episodes, while it was used as the second or the third (or more) drug in the 38 % and 38 % of episodes respectively. The median loading dose was 100 mg (range 50−300 mg). BRV was effective in 32 cases (57 %). An early response was documented in 22 patients (39 % of the whole sample). The use of the BRV within 6 h from SE onset was independently associated to an early SE resolution (OR 32; 95 % CI 3.39–202; p = 0.002). No severe treatment emergent adverse events were observed. Conclusion: BRV proved to be useful and safe for the treatment of SE. Time to seizures resolution appears shorter when it is administered in the early phases of SE.

Abstract: Aims Recently, the cardiovascular outcomes for people using anticoagulation strategies (COMPASS) trial demonstrated that dual therapy reduced cardiovascular outcomes compared with aspirin alone in patients with stable atherosclerotic disease. Methods and We sought to assess the proportion of patients eligible for the COMPASS trial and to compare the epidemiology results and outcome of these patients with those without COMPASS inclusion or with any exclusion criteria in a contemporary, nationwide cohort of patients with stable coronary artery disease. Among the 4068 patients with detailed information allowing evaluation of eligibility, 1416 (34.8%) did not fulfil the inclusion criteria (COMPASS-Not-Included), 841 (20.7%) had exclusion criteria (COMPASS-Excluded), and the remaining 1811 (44.5%) were classified as COMPASS-Like. At 1 year, the incidence of major adverse cardiovascular event (MACE), a composite of cardiovascular death, myocardial infarction, and stroke, was 0.9% in the COMPASS-Not-Included and 2.0% in the COMPASS-Like (P = 0.01), and 5.0% in the COMPASS-Excluded group (P < 0.0001 for all comparisons). Among the COMPASS-Like population, patients with multiple COMPASS enrichment criteria presented a significant increase in the risk of MACE (from 1.0% to 3.3% in those with 1 and >_3 criteria, respectively; P = 0.012), and a modest absolute increase in major bleeding risk (from 0.2% to 0.4%, respectively; P = 0.46). Conclusion In a contemporary real-world cohort registry of stable coronary artery disease, most patients resulted as eligible for the COMPASS. These patients presented a considerable annual risk of MACE that consistently increases in the presence of multiple risk factors.

To achieve this (i) the effect of microorganisms on NOR mobility in uranium (U) mine waters is studied, (ii) new datasets of NOR sorption and desorption parameters are acquired, (iii) a methodology for the determination of site-specific Kd values is evolved and (iv) models able to predict Kd (NOR) in relevant scenarios are developed. The impact of microbes on the speciation of U in U mine waters is characterized by a multidisciplinary approach providing insights into the microbe/U interaction mechanisms needed to predict the effect of microbial processes on the mobility of this radionuclide. Laboratory studies are performed to identify the soil properties that govern NOR interactions in soils. Sorption and desorption Kds for representative soils are determined, also considering soil aging effects. Chemical analogy between NOR and stable elements (e.g., Ba vs. Ra) is also examined, with new data and additional values gathered from literature. Considering the dynamics of sorption-desorption reactions, studies are carried out at the Zatu site (France) to develop a method to determine site-specific Kds. Experiments with soil core samples are performed to determine the amount of desorbed U, Ra and Pb and to derive apparent Kd values. The validity of this approach will be confirmed combining these results with in situ studies (Zatu site). Two approaches are followed to derive models for Kd (NOR) prediction. Abstract: The results of X-ray photoelectron spectra (XPS) characterization of the surface of Ag-Au colloidal nanoparticles (Ag-Au NPs), prepared by laser ablation in water before and after interaction with linear carbon chains (LCC), are presented. No additional features appear in high-energy resolved XPS core level spectra of Ag-Au NPs which indicates that surface is not oxidized. The measurements of XPS Ag 3d-spectrum of (Ag-Au)@LCC manifests the additional low-energy structure that is associated with the formation of Ag–C bonds. The charge transfer between Au atoms on the NPs surface and LCC was established. Additionally, some oxidation of the Ag atoms on the surface of (Ag-Au)@LCC is observed which arises during laser ablation in water. We assume that oxidative species will preferably interact with the areas outside the LCC instead of oxidizing the carbon chains which was confirmed by XPS C 1s spectra. © 2021 by the authors. Abstract: Radiological environmental impact assessment models are important tools to ensure protection of the public and the environment. The IAEA has an ongoing programme to improve capabilities in this area by model testing and comparison, reaching consensus on modelling philosophies, approaches and parameter values. This publication describes and summarizes the findings of Working Group 9 carried out during the IAEA EMRAS II (Environmental Modelling for Radiation Safety) programme. The results presented draw on international model validation exercises conducted to test and improve the predictive ability of models used for the assessment of radioactive contamination in urban settings, including dispersion and deposition events, short and long term contaminant redistribution following deposition events, and the effectiveness of potential countermeasures (protective actions, including remedial actions) for reducing human exposures.

Our findings suggest that a simple training in chunking may be useful as part of a cognitive strategy training for improving working memory performance in patients with Korsakoff's syndrome.

Abstract: In this work, we report X-ray photoelectron (XPS) and valence band (VB) spectroscopy measurements of surfactant-free silver nanoparticles and silver/linear carbon chains (Ag@LCC) structures prepared by pulse laser ablation (PLA) in water. Our measurements demonstrate significant oxidation only on the surfaces of the silver nanoparticles with many covalent carbon-silver bonds but only negligible traces of carbon-oxygen bonds. Theoretical modeling also provides evidence of the formation of robust carbon-silver bonds between linear carbon chains and pure and partially oxidized silver surfaces. A comparison of theoretical and experimental electronic structures also provides evidence of the presence of non-oxidized linear carbon chains on silver surfaces. To evaluate the chemical stability, we investigated the energetics of the physical adsorption of oxidative species (water and oxygen) and found that this adsorption is much preferrable on oxidized or pristine silver surfaces than the adsorption of linear carbon chains, which makes the initial step in the oxidation of LCC energetically unfavorable. © 2017 Elsevier Ltd

Abstract: Objectives: The aim of this study was to evaluate the hemodynamic performance of a sutureless bioprosthesis under high workload at mid‐term follow‐up. Methods: Thirty‐two patients who underwent isolated aortic valve replacement with a Perceval sutureless bioprosthesis with a minimum follow‐up of 1 year were enrolled in this study. S size prosthesis was deployed in 10 patients (31.3%), M size in 9 (28.1%), L size in 8 (25%) and XL size in 5 (15.6%). Effective orifice area (EOA), EOA index (EOAi), and transvalvular gradients were assessed at rest and during dobutamine stress echocardiography (DSE) a median of 19.5 months after surgery. Results: Dobutamine stress echocardiography (DSE) significantly increased heart rate, stroke volume, ejection fraction, and transvalvular gradients (peak gradient, 24.0 ± 7.6 vs 38.7 ± 13.6 mm Hg, P < .001; mean gradient, 12.6 ± 4.2 vs 19.8 ± 8.3, P < .001). When compared to baseline, estimated valve areas significantly increased at follow‐up (EOA, 1.48 ± 0.46 vs 2.06 ± 0.67, P < .001; EOAi, 0.84 ± 0.26 vs 1.17 ± 0.37, P < .001). Mean percentage increase in EOAi was 40.3% ± 28.0%. S size prostheses had the highest increase in EOA1, but the difference was not significant (S 46.0% ± 27.5% vs M 45.4% ± 34.5% vs L 32.7% ± 26.4% vs XL 32.1% ± 20.5%, P = .66). Severe patient‐prosthesis mismatch (EOAi ≤ 0.65 cm²/m²) was present at rest in 8 patients (25%), but only in one patient (3.1%) during DSE. Conclusions: The Perceval sutureless bioprosthesis demonstrated good hemodynamics at rest and under high workload. The significant increase in EOAi during DSE suggests the potential advantages of Perceval sutureless bioprostheses in case of small aortic annulus or when patient‐prosthesis mismatch is anticipated.

In a subsequent recognition test, they had to indicate to which celebrity they had previously told the proverbs. Participants also completed a neuropsychological battery including a binding task in which they were required to associate letters with their correspondent locations to assess context memory. RESULTS: Results showed worse binding and destination memory in patients with Korsakoff's syndrome than in controls. In the Korsakoff group, destination memory was significantly correlated with and predicted by performances on the binding task. CONCLUSIONS: The binding process seems to be impaired in Korsakoff's syndrome, a deficit that may account for the destination memory compromise in the syndrome, and probably, for the difficulty to retrieve the "where and when" of an encountered event.

STUDY DESIGN: This study aims to identify the prognostic significance of FoxP3(+) Tregs in pancreatic neuroendocrine tumors (PNETs). Patients diagnosed with PNETs between 1992 and 2014 (n = 101) were included in this retrospective analysis. Clinical data, histopathology, and expression of FoxP3(+) Tregs and Ki-67 by immunohistochemistry were assessed. The association of these factors with survival was tested by log-rank test and in additional multivariable analysis. RESULTS: A total of 101 patients were included in this study. Mean age was 58.0 years (range 18 to 87 years) and median tumor size was 25 mm (range 8 to 160 mm). The degree of infiltration of tumor by FoxP3(+) Tregs was graded as 0 (n = 75), 1 (n = 15), or 2 (n = 11). Median follow-up was 50 months (interquartile range 123 months; Q1 = 20 months and Q3 = 123 months). In univariate analyses, patient age older than 57 years, TNM stage III or IV, tumor size >25 mm, Ki-67 labeling index >20, and a high number of FoxP3(+) tumor-infiltrating lymphocytes were significantly associated with poorer overall survival. In multivariable analyses, FoxP3(+) expression score of 2 (hazard ratio = 6.9; 95% CI 1.4-34.4) was the only statistically significant predictor for overall mortality. CONCLUSIONS: FoxP3(+) Treg expression is an independent prognostic factor in patients with PNETs, associated with statistically significant shorter overall survival. There is a role for additional research into the immune-mediated role of FoxP3(+) Tregs in PNETs.

Abstract: The Urban Areas Working Group is part of the International Atomic Energy Agency’s EMRAS II (Environmental Modelling for Radiation Safety) Programme. The goal of this Working Group is to test and improve the capabilities of models used in assessment of radioactive contamination in urban settings, including dispersion and deposition events, short- and long-term contaminant redistribution following deposition events, and potential countermeasures or remediation efforts for reducing human exposures and doses. The Working Group has developed three modeling exercises, which are designed to permit intercomparison of model predictions and, in one case, comparison of model predictions with measurements. This paper describes the scenarios and provides comparisons of initial modeling results. Reasons for similarities and discrepancies among model predictions are discussed in terms of the modeling approaches, models, and parameter values used by different assessors. Preliminary conclusions emphasize the value of explaining individual approaches and the importance of understanding the effects of different assumptions and parameter values on the modeling results.

Abstract: The interaction between cancer cells and microenvironment has a critical role in tumor development and progression. Although microRNAs regulate all the major biological mechanisms, their influence on tumor microenvironment is largely unexplored. Here, we investigate the role of microRNAs in the tumor-supportive capacity of stromal cells. We demonstrated that miR-15 and miR-16 are downregulated in fibroblasts surrounding the prostate tumors of the majority of 23 patients analyzed. Such downregulation of miR-15 and miR-16 in cancer-associated fibroblasts (CAFs) promoted tumor growth and progression through the reduced post-transcriptional repression of Fgf-2 and its receptor Fgfr1, which act on both stromal and tumor cells to enhance cancer cell survival, proliferation and migration. Moreover, reconstitution of miR-15 and miR-16 impaired considerably the tumor-supportive capability of stromal cells in vitro and in vivo. Our data suggest a molecular circuitry in which miR-15 and miR-16 and their correlated targets cooperate to promote tumor expansion and invasiveness through the concurrent activity on stromal and cancer cells, thus providing further support to the development of therapies aimed at reconstituting miR-15 and miR-16 in advanced prostate cancer. Abstract: The ASDEX Upgrade programme is directed towards physics input to critical elements of the ITER design and the preparation of ITER operation, as well as addressing physics issues for a future DEMO design. After the finalization of the tungsten coating of the plasma facing components, the re-availability of all flywheel-generators allowed high-power operation with up to 20 MW heating power at Ip up to 1.2 MA. Implementation of alternative ECRH schemes (140 GHz O2- and X3-mode) facilitated central heating above ne = 1.2 × 1020 m−3 and low q95 operation at Bt = 1.8 T. Central O2-mode heating was successfully used in high P/R discharges with 20 MW total heating power and divertor load control with nitrogen seeding. Improved energy confinement is obtained with nitrogen seeding both for type-I and type-III ELMy conditions. The main contributor is increased plasma temperature, no significant changes in the density profile have been observed. This behaviour may be explained by higher pedestal temperatures caused by ion dilution in combination with a pressure limited pedestal and hollow nitrogen profiles. Core particle transport simulations with gyrokinetic calculations have been benchmarked by dedicated discharges using variations of the ECRH deposition location. The reaction of normalized electron density gradients to variations of temperature gradients and the Te/Ti ratio could be well reproduced. Doppler reflectometry studies at the L–H transition allowed the disentanglement of the interplay between the oscillatory geodesic acoustic modes, turbulent fluctuations and the mean equilibrium E × B flow in the edge negative Er well region just inside the separatrix. Improved pedestal diagnostics revealed also a refined picture of the pedestal transport in the fully developed H-mode type-I ELM cycle. Abstract: Current efforts to identify protein biomarkers of disease use mainly mass spectrometry (MS) to analyze tissue and blood specimens. The low-molecular-weight "peptidome" is an attractive information archive because of the facile nature by which the low-molecular-weight information freely crosses the endothelial cell barrier of the vasculature, which provides opportunity to measure disease microenvironment-associated protein analytes secreted or shed into the extracellular interstitium and from there into the circulation. However, identifying useful protein biomarkers (peptidomic or not) which could be useful to detect early detection/monitoring of disease, toxicity, doping, or drug abuse has been severely hampered because even the most sophisticated, high-resolution MS technologies have lower sensitivities than those of the immunoassays technologies now routinely used in clinical practice. Identification of novel low abundance biomarkers that are indicative of early-stage events that likely exist in the sub-nanogram per milliliter concentration range of known markers, such as prostate-specific antigen, cannot be readily detected by current MS technologies. We have developed a new nanoparticle technology that can, in one step, capture, concentrate, and separate the peptidome from high-abundance blood proteins. Herein, we describe an initial pilot study whereby the peptidome content of ovarian and prostate cancer patients is investigated with this method. Differentially abundant candidate peptidome biomarkers that appear to be specific for early-stage ovarian and prostate cancer have been identified and reveal the potential utility for this new methodology
