Abstract

The objective of this study is to develop and validate a screening instrument for the recognition of child maltreatment in the emergency department (ED). Existing data on screening questions and outcomes (diagnosis of child maltreatment) from three large observational screening studies at eight different EDs in the Netherlands were harmonized. A multivariate logistic regression was performed to develop the Screening instrument for Child Abuse and Neglect (SCAN). The SCAN was validated by performing a cross-validation and calculating the discriminative ability. A total of 24,963 patients were included. Out of the potential screening questions the following questions were included in the final instrument: is the injury compatible with the history, and does it correspond to the child’s developmental level? (aOR 10.40, 95% CI 5.69–19.02), was there an unnecessary delay in seeking medical help? (aOR 3.45, 95% CI 1.73–6.88) and is the behaviour/interaction of the child and parents (carers) appropriate? (aOR 14.67, 95% CI 7.93–27.13). The SCAN had a pooled AUC of 0.75 (95% CI 0.63–0.87) in the cross-validation. The question “Are there other signals that make you doubt the safety of the child and/or family?” (available in only one of the original datasets, OR 182.9; 95% CI 102.3–327.4) was by consensus added to the final SCAN. Conclusion: This validated and brief Screening instrument for Child Abuse and Neglect (SCAN) is designed to improve early recognition of child maltreatment in the ED. A positive screening result of the SCAN warrants a thorough work-up for child maltreatment, including a top-toe examination, if necessary additional diagnostics and adequate safety measures.What is Known:• Screening instruments increase the detection of possible cases of child maltreatment in the emergency department and support health care professionals to identify these cases.• The application of different screening instruments led to the need for one brief validated instrument.What

11. Influence of baseline severity on antidepressant efficacy for anxiety disorders: meta-analysis and meta-regression

Abstract

Background Antidepressants are established first-line treatments for anxiety disorders, but it is not clear whether they are equally effective across the severity range. Aims To examine the influence of baseline severity of anxiety on antidepressant efficacy for generalised anxiety disorder (GAD), social anxiety disorder (SAD), obsessive-compulsive disorder (OCD), post-traumatic stress disorder (PTSD) and panic disorder. Method Fifty-six trials of second-generation antidepressants for the short-term treatment of an anxiety disorder were included. Baseline and change scores were extracted for placebo and treatment groups in each trial. Mixed effects meta-regression was used to investigate the effects of treatment group, baseline severity and their interaction. Results Increased baseline severity did not predict greater improvement in drug groups compared with placebo groups. Standardised regression coefficients of the interaction term between baseline severity and treatment group were 0.04 (95% CI 70.13 to 0.20, P = 0.65) for GAD, 70.06 (95% CI 70.20 to 0.09, P = 0.43) for SAD, 0.04 (95% CI 70.07 to 0.16, P = 0.46) for OCD, 0.16 (95% CI 70.22 to 0.53, P = 0.37) for PTSD and 0.002 (95% CI 70.10 to 0.10, P = 0.96) for panic disorder. For OCD, baseline severity did predict improvement in both placebo and drug groups equally (b = 0.11, 95% CI 0.05 to 0.17, P = 0.001). Conclusions No relationship between baseline severity and drug-placebo difference was found for anxiety disorders. These results suggest that if the efficacy of antidepressants is considered clinically relevant, they may be prescribed to patients with anxiety regardless of symptom severity.

Abstract

Zn absorption is hampered by phytate (PA), which is present in large amounts in plant-based foods. We aimed to assess in vitro Zn accessibility at various PA/Zn molar ratios, as well as fractional absorption of Zn from partially polished intrinsically labelled rice with and without the addition of milk in humans. Triplicate samples of steamed white rice were blended with water, raw milk or UHT milk, and incubated for 4 hrs in the absence or presence of PA, and Zn content in the supernatant was assessed. Female subjects (n=18) were given 90 g of steamed rice that was intrinsically labelled with 67Zn (75%), either with water or milk in a cross-over design with a 1-month washout period. Both meals contained the same amount of Zn. An intravenous infusion dose of 0.02 mg 70Zn in saline was given half-way each meal. FAZ was determined from isotope ratios measured in spot urine samples collected 4 days after consumption of the rice meal. UHT milk and raw milk showed a significantly higher extraction of Zn from white rice in comparison to water at PA/Zn molar ratios of 15 and 30. Abstract

Intralesional use of cidofovir (Vistide(®)) has been one of the mainstays of adjuvant therapy in patients with recurrent respiratory papillomatosis (RRP) since 1998. In 2011, a communication provided by the producer of cidofovir addressed very serious side effects concerning its off-label use. As this was a general warning, it was inconclusive whether this would account for its use in RRP. The aim of this study is to determine whether nephrotoxic, neutropenic, or oncogenic side effects have occurred after intralesional use of cidofovir in patients with RRP. Update of recent developments in RRP, a multicentre questionnaire and a multicentre retrospective chart review. Sixteen hospitals from eleven countries worldwide submitted records of 635 RRP patients, of whom 275 were treated with cidofovir. RRP patients received a median of three intralesional injections (interquartile range 2-6). There were no statistical differences in occurrence of neutropenia or renal dysfunction before and after cidofovir. There was no statistical difference in occurrence of upper airway and tracheal malignancies between the cidofovir and the non-cidofovir group. In this retrospective patient chart review, no clinical evidence was found for more long-term nephrotoxicity, neutropenia or laryngeal malignancies after the administration of intralesional cidofovir in RRP patients.

15. MAXI J1659-152: The shortest orbital period black-hole transient in outburst

Abstract

MAXI J1659−152 is a bright X-ray transient black-hole candidate binary system discovered in September 2010. We report here on MAXI, RXTE, Swift, and XMM-Newton observations during its 2010/2011 outburst. We find that during the first one and a half week of the outburst the X-ray light curves display drops in intensity at regular intervals, which we interpret as absorption dips. About three weeks into the outbursts, again drops in intensity are seen. These dips have, however, a spectral behaviour opposite to that of the absorption dips, and are related to fast spectral state changes (hence referred to as transition dips). The absorption dips recur with a period of 2.414 ± 0.005 h, which we interpret as the orbital period of the system. This implies that MAXI J1659−152 is the shortest period black-hole candidate binary known to date. The inclination of the accretion disk with respect to the line of sight is estimated to be 65–80°. We propose the companion to the black-hole candidate to be close to an M5 dwarf star, with a mass and radius of about 0.15–0.25 M⊙ and 0.2–0.25 R⊙, respectively. We derive that the companion had an initial mass of about 1.5 M⊙, which evolved to its current mass in about 5–6 billion years. The system is rather compact (orbital separation of ≳1.33 R⊙), and is located at a distance of 8.6 ± 3.7 kpc, with a height above the Galactic plane of 2.4 ± 1.0 kpc. The characteristics of short orbital period and high Galactic scale height are shared with two other transient black-hole candidate X-ray binaries, i.e., XTE J1118+480 and Swift J1735.5−0127. We suggest that all three are kicked out of the Galactic plane into the halo, rather than being formed in a globular cluster.

We analytically find that the estimated ranges of mass-loss rate and orbital-period derivative for Cyg X-3 are consistent with two models: i) the system is detached and the mass loss from the system comes from the stellar wind of a massive helium star, of which only a fraction that allows for the observed X-ray luminosity is accreted, or ii) the system is semidetached and a Roche-lobe-overflowing low- or moderate-mass helium donor transfers mass to the compact object, followed by ejection of its excess over the Eddington rate from the system. These analytical results appear to be consistent with evolutionary calculations. By means of population synthesis we find that currently in the Galaxy there may exist ~1 X-ray binary with a black hole that accretes from a ⪆7~M_⊙ Wolf-Rayet star and ~1 X-ray binary in which a neutron star accretes matter from a Roche-lobe-overflowing helium star with mass ⪅1.5~M_⊙. Cyg X-3 is probably one of these systems.

These analytical results appear to be consistent with evolutionary calculations. By means of population synthesis we find that currently in the Galaxy there may exist ~1 X-ray binary with a black hole that accretes from a ⪆7~M_⊙ Wolf-Rayet star and ~1 X-ray binary in which a neutron star accretes matter from a Roche-lobe-overflowing helium star with mass ⪅1.5~M_⊙. Cyg X-3 is probably one of these systems.

Abstract

We present multiwavelength observations of a gamma-ray burst detected by INTEGRAL (GRB 030227) between 5.3 hours and ∼ 1.7 days after the event. Here we report the discovery of a dim optical afterglow (OA) that would not have been detected by many previous searches due to its faintess (R ∼ 23). This OA was seen to decline following a power law decay with index α R = -0.95 ± 0.16. The spectral index βopt/NIR yielded -1.25 ± 0.14. These values may be explained by a relativistic expansion of a fireball (with p = 2.0) in the cooling regime. We also find evidence for inverse Compton scattering in X-rays.

Abstract

We present UBVRIZJsHKs broad band photometry of the host galaxy of the dark gamma-ray burst (GRB) of February 10, 2000. These observations represent the most exhaustive photometry given to date of any GRB host galaxy. A grid of spectral templates have been fitted to the Spectral Energy Distribution (SED) of the host. The derived photometric redshift is z = 0.842(-0.042)(+0.014), which is in excellent agreement with the spectroscopic redshift (z = 0.8463 +/- 0.0002) proposed by Piro et al. (2002) based on a single emission line. Furthermore, we have determined the photometric redshift of all the galaxies in an area of 6' x 6' around the host galaxy, in order to check for their overdensity in the environment of the host. We find that the GRB 000210 host galaxy is a subluminous galaxy (L similar to 0.5 +/- 0.2 L*), with no companions above our detection threshold of 0.18 +/- 0.06 L*. Based on the restframe ultraviolet flux a star formation rate of 2.1 +/- 0.2 M, yr(-1) is estimated. The best fit to the SED is obtained for a starburst template with an age of 0.181(-0.026)(+0.037) Gyr and a very low extinction (A(v) similar to 0). We discuss the implications of the inferred low value of Av and the age of the dominant stellar population for the non-detection of the GRB 000210 optical afterglow.

Abstract

We report on multi-band (UBVRIZJ(s)K(s)) observations of the host galaxy of the April 18, 2000 gamma-ray burst. The Spectral Energy Distribution (SED) is analysed by fitting empirical and synthetic spectral templates. We find that: (i) the best SED fit is obtained with a starburst template, (ii) the photometric redshift is consistent with the spectroscopic redshift, (iii) the colours of the host are inconsistent with an old stellar population, and (iv) the global extinction is constrained to be in the range A(V) = 0.12-0.61 mag. The derived global extinction agrees with the one reported for the afterglow (A(V) = 0.4-0.9 mag), suggesting a homogeneous distribution of the interstellar medium (ISM) in the host galaxy. These findings are supplemented by morphological information from Hubble Space Telescope (HST) imaging: the surface brightness profile is smooth, symmetric and compact with no underlying structures (like dust lanes, spiral arms or disks). A natural scenario which accounts of all the above results is a nuclear starburst that harbours a young population of stars from which the GRB originated.
