Background : Verrucous carcinoma of the esophagus (VCE) is a rare tumor that is difficult to diagnose. In most cases, biopsies show nonspecific inflammatory and hyperkeratotic changes and do not show malignant findings. Most VCEs are slowly growing, locally advanced tumors with few metastases. Treatments for VCE are the same as for normal esophageal cancer, involving combined chemotherapy, surgical resection, and radiation therapy. However, it has been reported that VCE has a poor response to radiation or chemoradiotherapy (CRT). A case of VCE with complete response (CR) after CRT is presented. Case presentation : A 70-year-old man was found to have white, irregular esophageal mucosa 4 years earlier. He had been followed up as an outpatient as having candidal esophagitis. However, his tumor grew gradually, and biopsy was performed by endoscopic mucosal resection (EMR). He was finally diagnosed with VCE. He had no metastases to distant organs, but some lymph node metastases were suspected. The tumor invaded his left bronchus. The esophagostomy and gastrostomy were constructed as emergent procedures. The patient then underwent definitive CRT. 4 weeks after the end of CRT, two-stage esophagectomy was performed. First, he underwent esophagectomy with thoracic lymph node dissection. A latissimus dorsi flap was patched to the bronchus after primary suture of the hole. 6 weeks later, reconstruction of the gastric tube was performed through the antethoracic route. The pathological findings showed CR to CRT, with no proliferative cancer cells in the specimen. The patient has had no recurrence for three and half years after the resection. Conclusions : We presented a locally advanced VCE that achieved CR to CRT. In cases that have some difficulty for local resection, CRT might be an appropriate treatment for VCE.

Introduction: Verrucous carcinoma is a rare variant of squamous cell carcinoma. It generally grows exogenously and bone erosion is rare. Here we present a case of verrucous carcinoma on the foot with toe absence. Case presentation: A 64-year-old man presented with a 6-year history of a slowly growing exogenous hyperkeratotic verrucous lesion on the right fifth toe, which had been absent for 1year. Before the lesion appeared, he had undergone nail extraction on this toe. The culture of fungi and mycobacteria were negative. Three-dimensional computed tomography showed that the fifth toe of the right foot was absent. Histopathological examination showed that the lesion was exogenous and verrucous and exhibited moderate keratinocyte hyperplasia. Combined with clinical manifestation and histopathological examination, verrucous carcinoma is suggested. The lesion was surgically excised, and the patient was followed up for more than 6months without recurrence. Discussion: Verrucous carcinoma is an exophytic tumor with verrucous growth at the beginning, which can invade deep tissues and even cause limb mutilation. When encountering a patient with a wart-like plaque on the foot that responds poorly to conventional treatments, clinicians should maintain a high degree of clinical vigilance and a low threshold for biopsy. Conclusion: We observed a rare outcome of toe absence associated with verrucous carcinoma. Therefore, early diagnosis of verrucous carcinoma is very important. Introduction: Epitelioma cuniculatum is an unusual variant of epidermoid carcinoma. It is the subtype of cutaneous verrucous carcinoma that affects the foot and can be associated with the human papillomavirus (HPV). The lesion is frequently confused with plantar warts, delaying its diagnosis and treatment.Objective: To present an uncommon case of a patient with long history of epitelioma cuniculatum of the right foot associated with HPV, which spread out forming a vegetative mass affecting the last three toes.Case presentation: A sixty-nine-year-old white male patient (phototype III) that in three different moments had undergone excision of plantar warts in the right foot presented a torpid evolution with the appearance of ulcerative lesion of two centimeters in diameter. The biopsy was positive to epidermoid carcinoma associated with HPV; so, resection including safety margins around the tumor was performed.Conclusion: An unusual case of epidermoid carcinoma of the foot related to a plantar wart associated with HPV, which was initially removed by inadequate surgery, is presented., Introducción: El epitelioma cuniculatum es una variante infrecuente del carcinoma epidermoide, es el subtipo cutáneo del carcinoma verrugoso, que afecta al pie y que puede estar relacionado con el virus papiloma humano (VPH). El hecho de que la lesión inicial sea a menudo confundida con una verruga plantar dilata su diagnóstico y tratamiento.Objetivo: reportar un caso poco frecuente de un paciente con carcinoma cuniculatum del pie derecho, relacionado con VPH, de larga evolución, que se extendió formando una masa vegetante, afectando los tres últimos dedos.Presentación del caso: se presentó un paciente masculino blanco (fototipo III) de 69 años de edad, que en tres ocasiones se le habían resecado verrugas plantares en el pie derecho, presentó una evolución tórpida con aparición de lesión ulcerosa de 2 centímetros de diámetro, con biopsia positiva a carcinoma epidermoide asociado a VPH, al cual se le realizó desarticulación con bordes de seguridad de la lesión.Conclusiones: Se presentó un raro caso de carcinoma epidermoide del pie, relacionado con una verruga plantar asociada a VPH, la cual inicialmente fue extirpada quirúrgicamente de forma inadecuada.

Case report: A case of a verrucous carcinoma in the hallux of the right foot is presented, which was initially completely removed, evolving torpidly until a subsequent transmetatarsal amputation without evidence of tumor recurrence, during its follow-up over a period of 3 years, to evolve to another pathological entity such as the Diabetic Foot. Discussion: Verrucous carcinoma is clinically characterized by a slow growth similar in appearance to the plantar wart. The typical histology is a minimal local invasion with low incidence of metastasis and a good prognosis. Surgical biopsy is essential for correct diagnosis. Radical resection is the treatment of choice with histological confirmation of free resection margins of the tumor. Conclusions: The plantar verrucous carcinoma usually presents diagnostic difficulty. The complete surgical resection of the lesion is the treatment of choice presenting a good prognosis and prompt healing. As an exception, there are patients who present complications to this treatment., Caso Clínico: Se presenta un caso de un carcinoma verrucoso en el hallux del pie derecho, que inicialmente fue extirpado completamente, evolucionando de forma tórpida hasta una posterior amputación transmetatarsiana sin evidencia de recidiva tumoral, durante su seguimiento en un periodo de 3 años, para evolucionar a otra entidad patológica como es el Pie Diabético. Discusión: Este carcinoma se caracteriza clínicamente por un lento crecimiento de aspecto similar a la verruga plantar, siendo su histología típica la invasión local mínima con algo de displasia y baja incidencia de metástasis y presenta buen pronóstico. Para el diagnóstico correcto, es imprescindible la biopsia quirúrgica. La resección radical es el tratamiento de elección con confirmación histológica de los márgenes de resección libres del tumor. Conclusiones: El carcinoma verrucoso plantar presenta una evidente dificultad diagnóstica, constituyendo la resección quirúrgica completa de la lesión el tratamiento de elección. Posee un buen pronóstico y mejor resolución. Excepcionalmente hay pacientes que presentan complicaciones a este tratamiento.

[Resumen] Antecedentes: Los carcinomas verrucosos en pocas ocasiones se mencionan en la literatura su localización en la planta de los pies, siendo una variante bien diferenciada del carcinoma de células escamosas visto en mucosa y piel, incluyendo los pies.Objetivo: Describir las características clínicas, histopatológicas y de laboratorio y los resultados de un caso clínico inusual de un carcinoma verrucoso en la planta del pie.Métodos: Presentamos un extraño caso de un carcinoma verrucoso en la planta del pie izquierdo, a nivel del primer metatarsiano, que fue resecado completamente y se quedó sin evidencia de recidiva tumoral o metástasis, durante el seguimiento en un periodo de 4 años. Sobre esta base, se revisaron los casos publicados de carcinoma verrucoso en el pie, para definir las peculiaridades de este tumor poco común y para dar recomendaciones diagnósticas y terapéuticas.Resultados: El carcinoma verrucoso se caracteriza por una presentación clínica lenta pero inexorablemente amplia de aspecto similar a la verruga plantar, la histología típica es una invasión local mínima con baja incidencia de metástasis y buen pronóstico. Para el diagnóstico correcto, tanto el examen físico y el complementario debe ser exhaustivo y realizado mediante biopsia quirúrgica. La resección radical es el tratamiento de elección con confirmación histológica de los márgenes de resección libres del tumor. Conclusiones: El carcinoma verrucoso plantar puede presentar dificultad diagnóstica, la resección quirúrgica completa de la lesión permite el diagnóstico, presenta un buen pronóstico y pronta curación., [Abstract] background: Verrucous carcinomas rarely mentioned in the literature its location on the soles of feet, being a distinct variant of squamous cell carcinoma seen in mucosa and skin, including the feet.Objective: To describe the clinical, histopathological and laboratory and clinical results of an unusual case of a verrucous carcinoma on the sole.Methods: We present a rare case of a verrucous carcinoma in the left sole, at the first metatarsal, which was completely resected and remained without evidence of tumor recurrence or metastasis during follow-up over a period of four years. On this basis, we reviewed the published cases of verrucous carcinoma in the foot, to define the characteristics of this rare tumor and give diagnostic and therapeutic recommendations.Results: Verrucous carcinoma is characterized by a slow but inexorable clinical presentation similar in appearance to large plantar wart, histology is typical minimal local invasion with low incidence of metastases and good prognosis. For the correct diagnosis, both physical examination and must be comprehensive and complementary performed by surgical biopsy. Radical resection is the treatment of choice with histological confirmation of free resection margins of the tumor.Conclusions: The plantar verrucous carcinoma may present diagnostic difficulty, complete surgical resection of the lesion allows the diagnosis, prognosis and has good early healing.

Verrucous carcinoma is a rare, low-grade, well-differentiated form of squamous cell carcinoma seen on skin and mucosa. It is a slow-growing and locally aggressive tumor whose standard treatment is surgery. A case of strongly invasive oral verrucous carcinoma is presented with the medical history. The differential diagnosis of this neoplasm is difficult and requires clinic and pathologic data confrontation. In this paper, the importance of knowing the malignant potential of this lesion is stressed. Indeed, in 20% of verrucous carcinoma, foci of squamous cell carcinoma can be found. Verrucous carcinoma is a rare subtype of well-differentiated squamous cell carcinoma that may occur anywhere on the skin. It is slow growing, enlarging relentlessly and invading locally. Some cases were reported to occur on chronic scar tissue but demonstrated the same clinical course than in other locations. Herein, we report on a very unusual case of cutaneous verrucous carcinoma arisen from a long-standing (40 years old) burn scar and which involved the pleura five years after the initial surgical resection. Die Arbeit umfaßt erstens einen Literaturüberblick über die charakteristischen Merkmale und Besonderheiten des verrukösen Karzinoms (VK) und die Prognose bei unterschiedlichen therapeutischen Herangehensweisen. Von besonderem Interesse ist die LK-Metastasierung von VK. In der Literaturauswertung konnten dabei 18 Fälle ermittelt werden; dabei waren nur in einem Fall Fernmetastasen vorhanden. Die Prognose von VK ist gut bei einer adäquaten chirurgischen Therapie; die Mitbehandlung des Lymphabstromgebietes sollte nicht routinemäßig erfolgen. Zweiter Schwerpunkt der Arbeit ist eine Auswertung des Patientengutes der Klinik und Poliklinik für Mund-, Kiefer- und Gesichtschirurgie der Charité (24 Patienten) und der Hals-Nasen-Ohrenklinik der Charité (13 Patienten) unter der besonderen Fragestellung des Metastasierungsverhaltens und der Prognose von VK. In 2 Fällen wurden LK-Metastasen eines VK festgestellt. In keinem Fall ist das VK als Todesursache eines Patienten bekannt. Bei 8 Patienten traten Rezidive auf. Es wurde festgestellt, daß größere Tumoren schneller und häufiger rezidivieren. Bei der Hälfte der Patienten trat das Rezidiv 4 bis 9 Monate post operationem auf. Two patients had lymph node metastases caused by a VC. In no case VC is known as cause of death. Eight patients had relapses. Half of these patients had relapse 4 to 9 monthes post operationem. It was established that larger tumors relapse more often and faster. Early detection of VC is necessary, because the prognosis is the better the smaller the tumor is.

Verrucous carcinoma is a rare form of squamous cell carcinoma, It behaves as a slow-growing, locally aggressive and rarely metastasizing tumour. Three clinical cases are presented, with emphasis on clinical presentation, pathological diagnosis and therapeutic approach.
