Aims: Prosthetic valve endocarditis (PVE) is the most severe form of infective endocarditis associated with a high mortality rate. Whether PVE affects biological and mechanical aortic valves to the same extent remains controversial. This study aimed to compare the incidence of re-intervention because of PVE between bioprosthetic and mechanical valves. Methods: Patients undergoing isolated surgical aortic valve replacement (AVR) or combined AVR in a single cardiac surgery centre between January 1998 and December 2019 were analysed. All patients who underwent re-intervention because of PVE were identified. The primary endpoint was the rate of explants. Freedom from re-intervention and variables associated with re-intervention were analysed using Cox regression analysis including correction for competing risk. Results: During the study period, 5,983 aortic valve prostheses were implanted, including 3,620 biological (60.5%) and 2,363 mechanical (39.5%) prostheses. The overall mean follow-up period was 7.3±5.3 years (median, 6.5; IQR 2.9–11.2 years). The rate of re-intervention for PVE in the biological group was 1.5% (n=54) compared with 1.7% (n=40) in the mechanical group (p=0.541). Cox regression analysis revealed that younger age (HR 0.960, 95% CI 0.942–0.979; p<0.001), male sex (HR 2.362, 95% CI 1.384–4.033; p=0.002), higher creatinine (HR 1.002, 95% CI 0.999–1.004; p=0.057), and biological valve prosthesis (HR 2.073, 95% CI 1.258–3.414; p=0.004) were associated with re-intervention for PVE. After correction for competing risk of death, biological valve prosthesis was significantly associated with a higher rate of re-intervention for PVE (HR 2.011, 95% CI 1.177–3.437; p=0.011). Conclusions: According to this single-centre, observational, retrospective cohort study, AVR using biological prosthesis is associated with re-intervention for PVE compared to mechanical prosthesis. Further investigations are needed to verify these findings.

Introduction: Currently, anastomosis dehiscence represents one of the major complications in both elective colorectal surgery and urgent surgery. This is associated with high morbidity and mortality, increased hospital stay, and greater consumption of resources. The search for acute phase reactants as inducers for the arrest and anticipation of anastomotic dehiscence after surgery continues to be studied. Procalcitonin and C-Reactive Protein are the markers used in our center to monitor postoperative complications. The need for reoperation and anastomotic dehiscence are two closely linked concepts; every Colorectal Surgery unit should know and follow protocols to carry out a more effective, efficient and safe treatment. Methods: Observational and retrospective study nested in a cohort of 435 patients operated on for colorectal cancer at the Complejo Hospitalario Universitario de Ferrol (CHUF) between 2017-2020. Statistical analysis consisted of contrast tests and regression models. Results:The incidence of anastomotic dehiscence was 7.59%; PCT and PCR measurement on the second and fourth postoperative day showed an upward trend in cases that required reintervention, especially PCR seriation which, in terms of Odd ratio (OR), implies 3.30 times more probability of said event. Conclusions: The PCT and PCR serialization on the second and fourth postoperative days did not have a great impact on the time until surgery. However, the increasing values of both PCR and PCT suggest the need for additional tests., Introducción: Actualmente, la dehiscencia de anastomosis representa una de las grandes complicaciones tanto en la cirugía colorrectal programada como en la cirugía urgente. Esta se asocia a una gran morbimortalidad, aumento de la estancia hospitalaria y mayor consumo de recursos. Continúa en estudio la búsqueda de reactantes de fase aguda como inductores para la detención y la anticipación de la dehiscencia de anastomosis tras la cirugía. La Procalcitonina y la Proteína-C-Reactiva son los marcadores utilizados en nuestro centro para el seguimiento de complicaciones postoperatorias. La necesidad de reintervención y la dehiscencia de anastomosis son dos conceptos íntimamente ligados; toda unidad de Cirugía Colorrectal debería conocer y protocolizar para realizar un tratamiento más eficaz, eficiente y seguro. Métodos: Estudio observacional y retrospectivo anidado en una cohorte de 435 pacientes intervenidos por cáncer colorrectal del Complejo Hospitalario Universitario de Ferrol (CHUF) entre 2017-2020.El análisis estadístico consistió en test de contraste y modelos de regresión. Resultados: La incidencia de dehiscencia de anastomosis fue del 7,59%; la medición de PCT y PCR al segundo y al cuarto día postoperatorio mostró una tendencia al alza en los casos que necesitaron reintervención, sobre todo la seriación PCR que, en términos de Odd ratio (OR), implica 3,30 veces más probabilidad de dicho evento. Conclusiones: La seriación de PCT y PCR al segundo y cuarto día postoperatorio no presenta un gran impacto en el tiempo hasta la reintervención. No obstante, los valores en ascenso tanto de la PCR como de la PCT sugieren la necesidad de realización de pruebas complementarias.

The open abdomen (OA) is defined as intentional decision to leave the fascial edges of the abdomen un-approximated after laparotomy (laparostomy). The abdominal contents are potentially exposed and therefore must be protected with a temporary coverage, which is referred to as temporal abdominal closure (TAC). OA use remains widely debated with many specific details deserving detailed assessment and clarification. To date, in patients with intra-abdominal emergencies, the OA has not been formally endorsed for routine utilization; although, utilization is seemingly increasing. Therefore, the World Society of Emergency Surgery (WSES), Abdominal Compartment Society (WSACS) and the Donegal Research Academy united a worldwide group of experts in an international consensus conference to review and thereafter propose the basis for evidence-directed utilization of OA management in non-trauma emergency surgery and critically ill patients. In addition to utilization recommendations, questions with insufficient evidence urgently requiring future study were identified.

Adequate pre- and intraoperative imaging is important for optimal results in endovascular procedures. However, the standard use of CT and angiography may not always be sufficient to provide necessary information required for treatment, and complementary techniques are warranted in selected cases. TEVAR in acute complicated type B aortic dissections is proven effective in several reports, but long-term clinical outcome and aortic remodelling are still not fully evaluated. Intravascular phased array imaging (IPAI) was used in patients undergoing EVAR and TEVAR for aortic aneurysm and dissection. The combined information from IPAI and fluoroscopy allowed exact positioning of the stent graft. The colour Doppler function facilitated detection of blood-flow in relevant arteries during and after the procedures, and it also facilitated control of ceased flow in excluded false lumens or aneurysms. Clinical early and long-term results after TEVAR for acute complicated type B aortic dissection were investigated in all patients treated between 1999 and 2009 at UppsalaUniversityHospital. Results were favourable regarding survival and permanent neurological complications. Long-term follow-up of aortic morphological changes in the same patient group showed overall significant reduction of aortic and false lumen diameters, and an increase of true lumen diameter. Total thrombosis of the false lumen occured more often in patients with DeBakey IIIa aortic dissection, than in IIIb. In conclusion, IPAI may be a complementary tool to traditional imaging modalities in EVAR and TEVAR in selected cases. Long-term clinical outcome is excellent with favourable aortic remodeling after TEVAR in patients

OBJECTIVES: The study aimed to investigate early and long-term outcome of thoracic endovascular aortic repair (TEVAR) for acute complicated type B dissection. DESIGN: This was a retrospective, single-centre, consecutive case series. MATERIALS AND METHODS: During the period 1999-2009, TEVAR was carried out in 50 patients with non-traumatic acute complicated type B dissection, and in another 10 patients with acute complications, including rupture, end-organ ischaemia and acute dilatation during the primary hospitalisation, but >14 days after onset of symptoms. Thus, in total, 60 patients were included; 22 with a DeBakey type IIIa dissection and 38 with a type IIIb; median age was 67 years. Early (30-day) and long-term (5-year) survival, re-intervention rate and complications were recorded until 1 July 2010. RESULTS: Within 30 days, two (3%) deaths, one (2%) paraplegia and three (5%) strokes were observed. Five-year survival was 87% and freedom from re-intervention at 5 years was 65%. CONCLUSIONS: In patients with acute complicated type B aortic dissection, TEVAR can be performed with excellent early and long-term survival, whereas morbidity and long-term durability must be further elucidated.
