In this paper, we present the design, development, and characteristics of the novel aerosol mass spectrometer ERICA (ERC Instrument for Chemical composition of Aerosols; ERC – European Research Council) and selected results from the first airborne field deployment. The instrument combines two well-established methods of real-time in situ measurements of fine particle chemical composition. The first method is the laser desorption and ionization technique, or laser ablation technique, for single-particle mass spectrometry (here with a frequency-quadrupled Nd:YAG laser at λ = 266 nm). The second method is a combination of thermal particle desorption, also called flash vaporization, and electron impact ionization (like the Aerodyne aerosol mass spectrometer). The same aerosol sample flow is analyzed using both methods simultaneously, each using time-of-flight mass spectrometry. By means of the laser ablation, single particles are qualitatively analyzed (including the refractory components), while the flash vaporization and electron impact ionization technique provides quantitative information on the non-refractory components (i.e., particulate sulfate, nitrate, ammonia, organics, and chloride) of small particle ensembles. These techniques are implemented in two consecutive instrument stages within a common sample inlet and a common vacuum chamber. At its front end, the sample air containing the aerosol particles is continuously injected via an aerodynamic lens. All particles which are not ablated by the Nd:YAG laser in the first instrument stage continue their flight until they reach the second instrument stage and impact on the vaporizer surface (operated at 600 ∘C). The ERICA is capable of detecting single particles with vacuum aerodynamic diameters (dva) between ∼ 180 and 3170 nm (d50 cutoff). The chemical characterization of single particles is achieved by recording cations and anions with a bipolar time-of-flight mass spectrometer. For the measurement of non-refractory components, the particle size range extends from approximately 120 to 3500 nm (d50 cutoff; dva), and the cations are detected with a time-of-flight mass spectrometer. The compact dimensions of the instrument are such that the ERICA can be deployed on aircraft, at ground stations, or in mobile laboratories. To characterize the focused detection lasers, the ablation laser, and the particle beam, comprehensive laboratory experiments were conducted. During its first deployments the instrument was fully automated and operated during 11 research flights on the Russian high-altitude research aircraft M-55 Geophysica from ground pressure and temperature to 20 km altitude at 55 hPa and ambient temperatures as low as −86 ∘C. In this paper, we show that the ERICA is capable of measuring reliably under such conditions.

Purpose: This prospective clinical study was aimed at differentiating glioblastoma and cerebral metastasis, two tumor entities that often show similar radiological features, by means of combined MR oxygenation and perfusion imaging. Their distinction is highly important due to vastly differing therapy algorithms as well as patient outcomes. It was hypothesized that the infiltrative growth pattern of glioblastomas and the lack thereof in brain metastases would make it possible to distinguish the two groups based on their metabolic parameters in and around the contrast-enhancing part of the tumor. Materials and Methods: Fifteen previously untreated patients were recruited, seven of which suffered from glioblastoma (median age: 68 years, range: 54 – 84 years) with the remaining eight showing one or multiple brain metastases (median age 66 years, range: 50 – 78 years). All patients underwent preoperative MRI scans including multi-gradient echo and pseudo-continuous arterial spin labeling sequences. Three regions of interest were segmented in post-processing: contrast-enhancing tumor (CET), contralateral normal-appearing brain tissue (cNAB) and peritumoral non-enhancing T2-weighted fluid-attenuated inversion recovery hyperintense area (NET2). For these regions, oxygen extraction fraction (OEF) and cerebral blood flow (CBF) were estimated, yielding a third parameter: cerebral metabolic rate of oxygen (CMRO2). Two different machine learning-based approaches were employed to calculate OEF: an artificial neural network (ANN) and X-means clustering, both estimating the solution of the quantitative susceptibility mapping and quantitative blood-oxygen-level-dependent model (QSM + qBOLD). ANN results were used for statistical analysis and as features for training a support-vector machine algorithm for binary classification of tumor type. Classification performance was determined with receiver operating characteristic (ROC) analysis. Results: We demonstrated that OEF in CET was significantly lower (p = 0.03) in glioblastomas than metastases, all features (OEF, CBF and CMRO2) were significantly higher (p = 0.01) in CET than NET2 for metastasis patients only, and the ratios of CET/NET2 for CBF (p = 0.04) and CMRO2 (p = 0.01) were significantly higher for metastasis patients than for glioblastoma patients. For glioblastoma patients, OEF was shown to be significantly lower (p = 0.02) in CET than in cNAB. In ROC analysis, the ratios of CMRO2 and CBF in CET divided by NET2 were found to be the best single characteristics for classification with areas under the curve of 0.85 and 0.80, respectively. The best multiparametric classification model was found when training the classifier on two features: OEF in CET and the CMRO2 ratio of CET/NET2. The resulting model had an area under the ROC curve of 0.94 with 93% classification accuracy. Conclusion: The differences in oxygenation and perfusion between glioblastomas and brain metastases support the research hypothesis and allow for robust, non-invasive differential diagnosis of the tumor entity. While classification performance was found to be in line with previous MR-based publications that mainly investigated perfusion metrics such as cerebral blood flow and volume, the voxelwise estimation of CMRO2 presents a major advantage in that it may also yield an insight into the likely response to radiation, antiangiogenic and chemotherapy, especially in glioblastoma. This makes the methods employed in this study promising candidates for implementation into the clinical routine, not least to complement anatomical MRI sequences without the need for additional application of contrast agent. In the long run, they have the potential to add to or even replace brain biopsies due to the good classification accuracy and the absence of typical complications of an invasive procedure. However, further research with larger patient populations is required before the QSM + qBOLD model can find its way into clinical decision making.

Nach thermischen Verletzungen kommt es nach Abschluss der Akutversorgung in Abhängigkeit von der Tiefe der Läsionen zu großflächigen störenden Narben. Die ausgereiften, steifen Narben führen vor allem gelenknah zu Bewegungseinschränkungen. Bei ausgeschöpften konservativen Maßnahmen ist eine CO2-Lasertherapie oftmals die beste Möglichkeit, eine Verbesserung der Narbenbeschaffenheit zu erreichen. Die Lasertherapie ist zum Teil in der Verbrennungschirurgie mittlerweile zum Standardverfahren geworden. Bei der CO2-Laserung dringen die Laserstrahlen bis in die Lederhaut ein, um dort thermische Änderungen zu verursachen. Diese kleinen Gewebsschädigungen stimulieren die Bildung von neuen Kollagenfasern, welche durch Neuanordnung eine flachere, weichere Narbe erzielt. Bei der Farbstofflaserung hingegen ist die Zielkomponente das Hämoglobin. Durch die Denaturierung der Proteine bei 50-70°C und Modifizierung des Hämoglobins kommt es zum Abblassen der Narbe und Verminderung des Juckreizes. Ziel der Arbeit war es festzustellen, wie sich die Lasertherapie bei hypertrophen Narben bei Kindern auswirkt. Dies gab den Anlass eine Studie bezüglich der objektiven Effektivität und der Patientenzufriedenheit nach Lasertherapie von Verbrennungs- bzw. Verbrühungsnarben durchzuführen. In der Zeit von 2012 bis 2021 wurden die symptomatischen Narben von 78 Patienten und Patientinnen nach thermischer Verletzung mittels CO2-Laser und PDL behandelt. Eine Beurteilung und Beschreibung der Narben erfolgte durch einen Fragebogen und mehrere Scores (SF-36, VSS und POSAS). Neben der vor allem subjektiven Einschätzungen wurden auch objektive Kriterien wie die Hautelastizitätsmessung (DermaLab®, Fa. Cortex) vor und nach jeder Lasersitzung ermittelt. Die Ergebnisse zeigen, dass 96% der Patient:innen zufrieden waren und 90% eine Wiederholung der Lasertherapie wünschen. Die Beurteilung der Proband:innen zeigte im Hinblick der Dehnbarkeit nach Lasertherapie mit etwa 70% eine Besserung. 64% empfanden die Dicke der Narbe als weniger. Ebenso konnte ein positiver Einfluss auf die Lebensqualität nach Laserintervention gezeigt werden. Die Studie bestätigt eine effektive Therapie hypertropher Narben nach thermischer Verletzung mittels CO2-Laser auch bei Kindern. Mit der Lasertherapie wird bei geringem Risiko eine statistisch signifikante Besserung der Narbenbeschaffenheit, insbesondere der Elastizität, mit daraus resultierender besserer Beweglichkeit erreicht. Die Ergebnisse werden sowohl objektiv als auch subjektiv in der Arbeit dargelegt. Die Elastizitätsmessung durch das DermaLab® eignet sich dabei als objektives Messinstrument. Des Weiteren konnte gezeigt werden, dass die Lasertherapie auf eine hohe Patientenakzeptanz stößt und neben der deutlichen Zunahme der Lebensqualität der Kinder auch objektive Verbesserungen in der Narbenqualität zeigt. Eine Schmerztherapie ist quasi nicht notwendig. Besonders hervorzuheben ist, dass das Alter der Narben in dieser Studie eine sehr große Spanne, teilweise bis über 10 Jahre nach Verletzung umfasst. Eine Verbesserung der Narbenqualität nach Verbrennungen zu erreichen, die deutlich über den erwarteten Zeitrahmen des typischen Narbenumbaus hinausgeht, zeigt, dass die Lasertherapie tatsächlich eine Wirkung hat, die über den natürlichen Verlauf der Narbe hinausgeht. In zukünftigen Studien wäre es sicher sinnvoll, die Parameter der Hypo- und Hyperpigmentierung nach Behandlung als auch das Modul der sonografischen Bestimmung der Kollagenfasern mit in die Auswertung zu nehmen, um weitere objektive Untersuchungsmethoden zu validieren und so die Narbenqualität nach Laserbehandlung besser einschätzen zu können.

The emergence of new data- and algorithm-driven analysis methods is revolutionizing many areas of research and enabling solutions to problems that were previously considered intractable. But does this also apply to medicine? Improving diagnosis, fine characterization of patients for personalized medicine, monitoring disease progression and its prognosis, or predicting the outcomes of various therapies are just some of the areas that could potentially benefit from such new analysis techniques based on Deep Learning, which has enabled major advances in computer vision and related fields. Such algorithms now enable machines to better understand and interpret visual image data, which on the one hand offers promising perspectives for medical image processing, but on the other hand also poses challenges. As such, large amounts of annotated data are needed to prevent overfitting and to generate robust, generalizing and reliable models. Multicenter imaging studies could greatly improve the availability of such data and also enhance heterogeneity by obtaining training data from various sites. However, sharing data across multiple sites has proven to be difficult due to the high level of data protection associated with medical records and technical challenges such as interoperability. Consequently, this thesis attempts to avoid the necessity of data exchange by following a different approach: "Let’s share the algorithms, not the data!" The objective and central research question here is whether it is possible to shift the evaluation and training of modern image analysis methods to the clinical data owners and how this can be accomplished. Although this approach helps to circumvent the data export and the associated data protection challenges, the participating partners must still be enabled to execute the algorithms from a technical and or- ganizational perspective. For this purpose, this dissertation investigated concepts, the development and establishment of a decentralized infrastructure for clinical medical image analysis that enables standardized data access and uniform execution of analysis methods for joint data analysis in the context of multicenter imaging studies. The technical realization of this infrastructure was achieved by developing a software framework called Kaapana, which enables the building of imaging platforms. The resulting software can be hosted on dedicated servers within the clinical IT environ- ment to be operated isolated from any external connectivity and to be interconnected with other local clinical systems. By leveraging modern cloud technologies such as containers and Kubernetes, the local deployment provides a private cloud for image processing that can be accessed from any locally connected workstations via the web browser. Standardized linkage to the clinical PACS via DICOM and the integration of a research imaging archive enables redundant data management and consistent data access for the execution of analysis methods. Dashboards enable efficient data exploration and filtering by visually presenting DICOM metadata of the platform’s data and allowing it to be selected via search queries. A uniform execution environment for data processing allows algorithms to be applied to such selected data and to be uniformly packaged and shared with partners. High-performance server hardware including Graphics Processing Units enables a variety of analysis techniques such as Deep Learning-based model inference, as well as the training of new models to be shared with partners. Within the infrastructure, the standardized and widely adopted DICOM format has been prioritized so that also many analysis results can be provided in a standard-compliant way. Using formats such as DICOM SEG, SR or Encapsu- lated PDF, data annotations become compatible with clinical workflows and IT systems. With support of the Radiological Cooperative Network and the German Cancer Consor- tium, the resulting infrastructure could be deployed and evaluated within two German research consortia. To this end, all 36 German university hospitals have commissioned their own server on which the platform has been installed and tested. This involved evaluating the commissioning, integration, operation and maintenance of such a decentralized network, as well as various use cases designed to cover the typical tasks of such a system. As a result, these use cases and the corresponding varying demands could be realized with the developed concept and the implemented framework. A flexible extension mechanism also allows the integration of additional services such as137 the MITK workbench or processing algorithms such as the nnUNet into the framework. Furthermore, first analysis pipelines developed by external partners could also be integrated and delivered to the partners already. Within the scope of this work, clinics were enabled to apply up-to-date research methods to their own data through the development, distribution and support of the developed infrastructure. Since the research consortia, which already include all German university hospitals, have only just started their activities, there are great opportunities to make the high-quality data from the partner sites accessible and usable for research in the future. Because of Kaapana’s open source code, its architecture based on common industry standards, and its already broad deployment, this framework could also serve as a foundation for areas other than medical imaging, and thus offer the potential for tighter data integration for clinical computing in general.

Die partielle Reprogrammierung von adulten Sehnenzellen nach einer Verletzung durch Etablierung eines temporären und lokal begrenzten pluripotenten Zustands ist eine neuartige zelluläre molekulare therapeutische Strategie, um die Narbenbildung nach einer Verletzung zu begrenzen und die Regeneration des betroffenen Gewebes zu fördern (Lu et al., 2020; Sarkar et al., 2020; Wang et al., 2021). Narbenbildung und fehlerhafte Heilung treten nach Verletzungen von adultem Gewebe auf und begrenzen die Regeneration des betroffenen Gewebes. Das Grundprinzip der zellulären Reprogrammierung in vivo besteht darin, die Proliferationsfähigkeit reprogrammierter Zellen durch Transformation/De-Differenzierung (Dedifferenzierung) zu fördern, was zur Bildung von Vorläuferzellen im verletzten Gewebe führt (Dedifferenzierung, Transdifferenzierung), die sich in die typischen Zielzellen dieses Gewebes differenzieren (Transdifferenzierung, Re-Differenzierung). Der Prozess der Dedifferenzierung wird in der Regel durch Yamanaka-Faktoren erreicht, und die Prozesse der Re- und Transdifferenzierung können durch Transkriptionsfaktoren erleichtert werden, die eine wichtige regulatorische Rolle bei der Entwicklung des Gewebes spielen, oder durch Transkriptionsfaktoren, die andere Zelltypen (Stammzellen oder Fibroblasten) direkt in funktionelle, reife, gewebetypische Zellen umwandeln können. Nach Aktivierung mit Doxycyclin exprimieren endogene Scx-exprimierende (mit Scarlet markierte) Achillessehnen-Vorläuferzellen in der Mauslinie OSKM-Transkriptionsfaktoren (Oct3/4, Sox2, KLF4 und c-Myc), was zu einem lokal begrenzten pluripotenten Zustand der Zellen, einer gesteigerten Proliferationsfähigkeit und einem verbesserten zellulären Status (scheinbare Verjüngung) führt, wodurch die Reparatur verbessert und die Narbenbildung reduziert wird. Gleichzeitig werden die vier wichtigsten Transkriptionsfaktoren in der embryonalen Sehnenentwicklung, Td4F (Scx, Mkx, Egr1 und Egr2), parallel zu EmGFP exprimiert, wodurch das Zellschicksal eingeleitet, die Effizienz der Redifferenzierung in Zielzellen gefördert und die funktionelle Gewebereparatur verbessert wird. Die Nachbeobachtung wird soweit fortgesetzt, bis reife Achillessehnenzellen den Marker Typ-I- Kollagen (markiert mit BFP) stark exprimierten und Typ-III-Kollagen, den wichtigsten Kollagentyp bei der Narbenheilung, ersetzen. Dementsprechend beabsichtigen wir, den Transkriptionsfaktor Td4F-OSKM in dieser Mutante nach experimenteller Sehnenverletzung zu aktivieren. Ziel ist es, OSKM-Faktoren in Sehnenläsionen zu induzieren, um die Bildung von Narbengewebe zu reduzieren und Td4F- Faktoren zu experimieren, um die Differenzierung von Zellen in der Periwunde zu neuen Tenozyten (Sehnenzellen) zu fördern. Wir werden auch untersuchen, inwieweit die OSKM- vermittelte partielle Reprogrammierung den zellulären Zustand verbessert und welche Rolle die acht Transkriptionsfaktoren bei der Verringerung der in-situ-Fibrose spielen. Damit soll eine Grundlage geschaffen werden, die eine Gewebeheilung ermöglichen kann. Zu diesem Zweck werden histologische Nachkontrollen durchgeführt, um die Pathologie des geschädigten Sehnengewebes zu beurteilen. Diese werden mit der Untersuchung des Expressionsprofils der Fibrose und der embryonalen Sehnenentwicklung analysiert. Um den oben genannten experimentellen Plan zu erreichen, ist es wichtig zu verstehen, inwieweit eine OSKM-vermittelte in situ Teilreprogrammierung die Fibrose reduzieren kann und ob temporäre und lokal begrenzte Achillessehnenzellen im pluripotenten Zustand durch Überexpression von Schlüsseltranskriptionsfaktoren in reife Achillessehnenzellen differenzieren können. Wir haben transgene Mauslinien mit induzierbarer Expression etabliert, die spezifisch zwei Klassen von (de-differenzierten und re-differenzierten) Transkriptionsfaktoren überexprimieren, sowie transgene Mauslinien mit fluoreszierenden Reportergenen, die den Status von Achillessehnenzellen durch fluoreszierende Proteine anzeigen. Damit wurden die Tiermodelle und die experimentelle Plattform für die Phase-II- Experimente etabliert.

Cancer is one of the leading causes of disease-related death worldwide. In recent years, large amounts of data on cancer genetics and molecular characteristics have become available and accumulated with increasing speed. However, the current understanding of cancer as a disease is still limited by the lack of suitable models that allow interpreting these data in proper ways. Thus, the highly interdisciplinary research field of mathematical oncology has evolved to use mathematics, modeling, and simulations to study cancer with the overall goal to improve clinical patient care. This dissertation aims at developing mathematical models and tools for different spatial scales of cancer development at the example of colorectal cancer in Lynch syndrome, the most common inherited colorectal cancer predisposition syndrome. We derive model-driven approaches for carcinogenesis at the DNA, cell, and crypt level, as well as data-driven methods for cancer-immune interactions at the DNA level and for the evaluation of diagnostic procedures at the Lynch syndrome population level. The developed models present an important step toward an improved understanding of hereditary cancer as a disease aiming at rapid implementation into clinical management guidelines and into the development of novel, innovative approaches for prevention and treatment.

The binding and elution behavior of two therapeutic bispecific monoclonal antibodies (bsAbs) on the strong cation exchange resin POROS™ XS is investigated and modeled over broad ranges of pH, salt concentrations, and column loadings. One of the two bsAbs exhibits common Langmuir elution behavior under high loading and column overloading conditions, whilst the other bsAb exhibits uncommon anti-Langmuir elution behavior as a consequence of multi-layer binding on the stationary phase surface. The frequently used Steric Mass Action (SMA) model modified with an activity coefficient for the salt in solution is used to simulate the Langmuirian elution behavior. A Self-Association Steric Mass Action (SAS-SMA) model extended with two activity coefficients for the protein and salt in solution is applied to describe the anti-Langmuir elution behavior. The SAS-SMA model is able to describe self-dimerization on the resin surface and thus can predict anti-Langmuir elution behavior. The binding models are each combined with a lumped rate model to describe mass transfer inside the chromatography column. To apply these models for describing protein elution over wide ranges of pH, the pH-dependences of all model parameters, including the linear and especially the non-linear model parameters, are investigated, described, and implemented into the binding models. Therefore, extensive data sets were generated that consist of linear gradient elution experiments comprising a pH range from pH 4.5 to 8.9 and column loadings from 0.5 to 90.0 mgbsAb/mLresin. The modeling results of both antibodies show that the pH of the mobile phase has a strong influence on the non-linear model parameters, thus valuable process insights can be gained by interpretation of these results. An increasing buffer pH leads to an increase in binding sites shielded by the antibodies, whilst self-dimerization on the resin surface becomes less with increasing pH. Empirical correlations describing the non-linear model parameters as functions of pH are established and implemented into the SMA and SAS-SMA formalisms. The functionality of these modified pH-dependent binding models is verified with linear salt, pH and dual gradient elution experiments using single-component simulations. Most of these experiments can be accurately predicted under high loading and overloading conditions, whereby especially the peak shapes are well-described. Slight discrepancies between the simulated and experimental data can be observed for some of these experiments, especially when they were performed under overloading conditions. In this dissertation, it is clearly shown that these discrepancies are not primarily a consequence of limitations of the SMA and SAS-SMA models. At lower pH values (pH ≤ 5.3), overloading phenomena such as protein breakthrough during the loading phase, additional peaks, and peak-shoulders occur. The outcomes of additional experiments in which the antibodies were loaded onto the column with different counterion concentrations and loading times show that intraparticle diffusion effects and conformational changes of the bsAbs are responsible for these overloading phenomena at low pH. The applied lumped rate mass transfer model is not adequate here since it cannot describe hindered intraparticle transport and should be extended to consider these effects. Additional peaks and peak shoulders due to bsAb conformations can only be predicted by describing multi-state binding, which is shown in this dissertation for one case by a simple extension to a multi-component simulation. Furthermore, it is shown that the description of complex peak shapes arising due to competitive binding and multi-component elution of the antibodies' charge variants cannot be adequately predicted using single-component simulations. However, an extension of the model to a simple multi-component system consisting of two charge variants enables accurate prediction of some of these complex elution profiles.

More deaths can be attributed to lung cancer, than to any other cancer type. Evidence collected over the last 10 years, from randomized trials in the USA and Europe, indicates that screening by means of low-dose computed tomography (LDCT) could reduce the number of lung cancer (LC) deaths by about 20%-24%. While these findings have led to the implementation of screening programs in the USA, South Korea and Poland, discussions on their optimal design and execution are still ongoing in various countries, including Germany. Optimizing screening means finding the right balance between mortality reduction and risks, harms, and monetary costs. LDCT-scans are expensive, expose participants to radiation and put them at risk for overdiagnosis, as well as at risk for unnecessary invasive and expensive confirmatory procedures triggered by false positive (FP) results. Minimizing the number of unnecessary screening and confirmatory examinations should be prioritized. While risk-based eligibility has been shown to best target candidates, questions regarding optimal screening frequency, accurate nodule evaluation, stop-screening criteria to reduce overdiagnosis, and the use of complementary non-invasive diagnostic methods, remain open. Statistical models and biomarkers have been developed to help answer these questions. However, there is limited evidence of their validity in data from screening contexts and populations other than those in which they were developed. The analyses presented in this thesis are based on data collected as part of the German Lung Cancer Screening Intervention (LUSI) trial in order to validate models that address the questions: 1) can candidates for biennial vs annual screening be identified on the basis of their LC risk? 2) can the number of FP test results be reduced by accurately estimating the malignancy of LDCT-detected nodules? 3) What was the extent of overdiagnosis in the LUSI trial and how does overdiagnosis risk relate to the age and remaining lifetime of participants? Additionally, blood samples from participants of the LUSI were measured to evaluate: 4) whether the well-validated diagnostic biomarker test EarlyCDT®-Lung is sensitive enough to detect tumors seen in LDCT images. The LCRAT+CT and Polynomial models predict LC risk based on subject characteristics and LDCT imaging findings. Results of this first external validation confirmed their ability to identify participants with LC detected within 1-2 years after first screening. Discrimination was higher compared to a criterion based on nodule size and, to a lesser degree, compared to a model based on smoking and subject characteristics (LCRAT). This suggested that while LDCT findings can enhance models, most of their performance can could be attributed to information on smoking. Skipping 50% of annual LDCT examinations (i.e., for participants with estimated risks

Red blood cell transfusion is routinely performed to improve tissue oxygenation in patients with decreased hemoglobin levels and oxygen-carrying capacity. Generally, blood banks process and store packed red blood cells as RCCs. During storage, RBCs undergo progressive biochemical and morphological changes which are collectively described as storage lesion. According to regulatory guidelines, the quality of RCCs is assessed by quantifying hemolysis before transfusion. However, the hemolysis level only gives an indication of the already lysed erythrocytes; it does not indicate the degree of deterioration of aged cells, which are known to compromise the post-transfusion survival. Morphological analysis, a method that has the potential to provide a simple and practical diagnosis, is suitable for indicating the degradation of RBCs and thus has considerable power to predict actual post-transfusion survival. Microfluidic systems with suspended RBCs can enable fully automated morphological diagnosis based on image analysis with large cell statistics and high sample throughput. The previous version of the flow morphometry system, which was based on a binary decision tree was able to show in a first attempt that spherocytes are a suitable candidate for such a morphological storage lesion marker. However, due to the low classification resolution (three morphology classes), possible shear-induced morphology changes of the measurement system could not be evaluated. In this study, the image classification of the flow morphometry system was substantially enhanced by using a convolutional neural network to strongly improve the resolution and accuracy of the morphology classification. The resulting CNN-based classification achieved a high overall accuracy of 92% with RBCs being classified into nine morphology classes. Through this improved classification resolution, it was possible to assess degradation-induced morphologies at high resolution simultaneously with shear-induced morphologies in RCCs. The overall goal was to provide a robust and strong marker for storage lesion that reflects post-transfusion survival of RBCs. Therefore, it was necessary to analyze the extent to which the shear in the microfluidic system affected the morphological transients between RBC classes. Indeed, it could be shown that shear-induced morphology changes appear dependent on the position of the focal plane height in the flow chamber. The proportion of stomatocytes is increased near the surfaces of the laminar flow chamber. This temporary shear-induced morphology transformation can occur only in flexible erythrocytes with intact membrane properties. Therefore, these cells should be considered a subset of healthy erythrocytes that can reversibly alter from stomatocyte to discocyte morphology. The nine RBC morphology classes of the improved classification resolution were further analyzed to determine whether they exhibit a particular pattern based on their relative proportions during storage that could be used as a storage lesion marker. All individual RBC classes, except for the spherical morphologies, undergo reversible transitions among themselves that are related to the SDE sequence and result in a low signal-to-noise ratio. The proportions of the irreversible spherical morphologies, spheroechinocytes and spherocytes, were defined as the lesion index. This lesion index showed a strong correlation to hemolysis levels. In fact, the correlation between the hemolysis level and the lesion index was so good that it persisted at an individual RCC level. A preliminary lesion index threshold of 11.1% could be established, which is equivalent to a hemolysis threshold of 0.8% established in regulatory guidelines, to assess whether an RCC is of appropriate quality for transfusion. However, the lesion index, besides predicting the hemolysis level, can also be used to generate more information about post-transfusion survival, since it consists exclusively of the RBC morphologies that are removed by the body in a very short time after transfusion in the recipient. Finally, we translated the newly established lesion index and standard biochemical parameters into a quality assessment of RCC shipped and transported repeatedly on air rescue missions to assess an eventual deterioration of the RBCs. We showed that the quality of RCCs was not inferior to control samples after repeated air rescue missions during storage. German regulations allow RCCs to be stored for 42 days in a temperature range of +2°C to +6°C. Compliance with this regulation can be secured during air rescue missions by means of suitable logistics based on a rotation system. By using efficient cooling devices, the logistics and maintenance of the thermal conditions are both safe and feasible. A well-defined rotation system for the use of RCCs during routine air rescue missions offers a resource-saving option and enables the provision of RCCs in compliance with German transfusion guidelines. This innovative concept enables life-saving prehospital transfusions directly at the incident scene. CNN-based flow morphometry and the calculated lesion index allow a reliable assessment of RCC quality. The method also decreases the demand for complex laboratory procedures. Therefore, it is highly advisable to include the lesion index as an additional marker for storage lesion in routine clinical practice. Unlike hemolysis, the lesion index may serve as a good indicator of post-transfusion survival. Thus, both measurements together could provide increased safety and efficacy of stored RCCs.

Das Ziel dieser Dissertation war es medizinische Aspekte zur photokatalytischen Wirksamkeit von Titandioxid zu untersuchen, wie die bakterizide Wirksamkeit und die Inaktivierung von komplexen Biomolekülen wie das Endotoxin Lipopolysaccharid oder Ribonuklease A. Darüber hinaus sollten das in kosmetischen Sonnenschutz-Präparaten enthaltene Titandioxid auf seine photokatalytische Wirkung hin untersucht werden. Dazu mussten entsprechende Strategien und Nachweismethoden erarbeitet werden. Eine weiteres Ziel dieser Dissertation bestand darin, einen Weg zu untersuchen, ob die photokatalytische Wirksamkeit von TiO2 neben den vorhandenen biologischen und chemischen Methoden auch durch eine praktikable physikalische Methode messbar gemacht werden kann. Der Halbleiter Titandioxid ist ein Photokatalysator, der durch ultraviolettes Licht aktiviert werden kann, wobei Elektron-Loch-Paare entstehen. In Anwesenheit von Wasser und Sauerstoff können sich daraus reaktive Intermediate wie Hydroxylradikale und Superoxid-Anionen bilden, die in der Lage sind, Redoxreaktionen an organischen und anorganischen Molekülen zu initiieren. Anhand der Untersuchungen konnte gezeigt werden, dass die Gram-negativen Testkeime Escherichia coli und die Gram-positiven Testkeime Sarcina lutea in Suspension und auf trockenen Proben durch photokatalytisch aktivierte Titandioxid-Schichten abgetötet werden können. Proben mit einer höheren Titandioxid-Schichtdicke wiesen dabei eine höhere photokatalytisch induzierte Keimabtötung auf. Unter Umständen können Zellwandfragmente der Bakterien als Endotoxine auf den Oberflächen zurück bleiben. Somit würde allein die Abtötung von Keimen zur Dekontamination einer Oberfläche nicht ausreichen. Dies führte zur Untersuchung der Inaktivierung komplexer Biomoleküle mit hochsensitiven immunologischen Nachweisverfahren, die im Rahmen dieser Dissertation erarbeitet und eingesetzt wurden. Es konnte gezeigt werden, dass Lipopolysaccharide und Ribonuklease A durch die Einwirkung von photokatalytisch aktiven Titandioxid-Schichten inaktiviert werden können. Anhand der Ergebnisse konnte berechnet werden, dass die Lipopolysaccharid-Menge, die theoretisch aus 3 x 10 exp 4 Escherichia coli Bakterien freigesetzt werden kann, innerhalb von 96 Stunden im Versuchsaufbau inaktiviert werden konnte. Bei der Inaktivierung von Ribonuklease A zeigte sich im Nachweisverfahren ein An/Aus-Effekt in Abhängigkeit von der eingesetzten Ribonuklease A Konzentration. Dies konnte damit erklärt werden, dass bei Vorhandensein selbst geringster Mengen nicht inaktivierter Ribonuklease A das Testergebnis aufgrund der hohen Sensitivität positiv ausfiel. Es konnte ermittelt werden, dass im Testaufbau 100 μg mL-1 Ribonuklease A oder 7 units mL-1 inaktiviert werden konnten. Dies entspricht in etwa der in menschlichem Speichel enthaltenen Ribonuklease A Konzentrationen, so dass zumindest durchschnittliche Kontaminationen durch Bestrahlung mit ultraviolettem Licht auf den Titandioxid-beschichteten Oberflächen inaktiviert werden können. Es wurden kommerziell erhältliche Sonnenschutz-Präparate mit Titandioxid als Inhaltsstoff untersucht. Dabei wurden per ICP-MS Analytik die Titandioxid-Konzentrationen in den Präparaten zwischen 1% und 5,6% nachgewiesen. Es konnten durch Röntgen-Pulver-Diffraktometrie die enthaltenen Titandioxid-Modifikationen ermittelt werden. Wobei in 8 Proben reines Anatas, in 11 Proben reines Rutil und in 13 Proben eine Anatas-Rutil-Mischung gefunden wurde. Die Titandioxid-Bestandteile wurden extrahiert und auf deren photokatalytische Wirksamkeit hin untersucht. Es zeigte sich, dass die Proben mit einer Rutil-Anatas-Mischung die höchsten photokatalytischen Aktivitäten aufwiesen. Die Anatas-Fraktionen wiesen etwas geringere Aktivitäten auf, während die reinen Rutil-Proben nur wenig photokatalytische Aktivität zeigten. Dies deckt sich mit den Ergebnissen aus der Literatur. Mittlerweile gibt es eine Empfehlung der EU-Kommission, dass nur noch photokatalytisch inaktives Titandioxid für Kosmetika eingesetzt werden soll, um etwaige Zellschädigungen durch Photokatalyse zu minimieren. Zur Fragestellung nach einer physikalischen Methode zur Ermittlung der photokatalytischen Aktivität wurde untersucht, ob die durch die initiierten Redoxreaktionen entstehenden elektrischen Potentialdifferenzen an den Oberflächen messbar sind. Es sollte auch untersucht werden, ob messbare Potentialdifferenzen Rückschlüsse auf die photokatalytische Wirksamkeit erlauben. In einem ersten Aufbau in Anlehnung an eine galvanische Zelle unter Verwendung von Platintiegeln, konnte zunächst die Hypothese zur Spannungsmessung bestätigt werden. Die messbaren Spannungen nahmen mit der eingestrahlten Lichtintensität entsprechend des Entfernungsgesetzes ab. Auch konnten mit steigenden Titandioxid-Konzentrationen höhere Spannungen gemessen werden, wobei der Anstieg einem asymptotischen Verlauf entsprach, was gemäß dem Lambert-Beer`schen Gesetz durch die optisch dichter werdende Suspension erklärt werden konnte. In weiteren Untersuchungen an Proben mit unterschiedlichen Titandioxid-Schichtdicken konnte gezeigt werden, dass die Spannung mit zunehmender Schichtdicke zunahm. Eine Gegenüberstellung der Ergebnisse der Spannungsmessungen mit Werten aus Versuchen zur photokatalytischen Aktivität wie Entfärbung einer Methylenblau-Lösung und Keimabtötungsversuchen, zeigte einen qualitativen Zusammenhang. Daraufhin wurde die Spannungsmessung weiter optimiert. Durch den Einsatz eines transparenten Elektrodensystems auf Basis von Indium-Zinn-Oxid konnte der Aufbau kompaktiert werden, wodurch höhere messbare Spannungen erzielt werden konnten. Die Standardabweichungen der Messwerte lagen mit bis zu 50% relativ hoch, woraufhin weitere Optimierungsmöglichkeiten untersucht wurden. So wurde eine Messapparatur zur Einstellung des Abstands zwischen Elektroden und Titandioxid-Oberfläche entwickelt und erprobt. Auch damit konnten die Messswertschwankungen nicht wesentlich verbessert werden. In einem weiteren Aufbau, bei dem das Titandioxid direkt auf einem Edelstahlsubstrate fixiert wurde, um die Abstände zwischen dem Ort der Entstehung der Ladungsträger und der Elektrode zu minimieren, sowie durch Ersatz des flüssigen Elektrolyten durch einen Agarose-Gel-Elektrolyten, konnten gut reproduzierbare Messergebnisse mit Standardabweichungen von 5% - 13% erreicht werden. Insgesamt konnte ein Zusammenhang zwischen der photokatalytischen Aktivität der Titandioxid-Proben und der messbaren Spannungen aufgezeigt werden. Damit wurden Grundlagen zur Entwicklung eines physikalischen Messinstruments zur Ermittlung der photokatalytischen Aktivität geschaffen.

Biomechanical patient modeling incorporates physical knowledge of the human anatomy into the image processing that is required for tracking anatomical deformations during adaptive radiation therapy, especially particle therapy. In contrast to standard image registration, this enforces bio-fidelic image transformation. In this thesis, the potential of a kinematic skeleton model and soft tissue motion propagation are investigated for crucial image analysis steps in adaptive radiation therapy. The first application is the integration of the kinematic model in a deformable image registration process (KinematicDIR). For monomodal CT scan pairs, the median target registration error based on skeleton landmarks, is smaller than (1.6 ± 0.2) mm. In addition, the successful transferability of this concept to otherwise challenging multimodal registration between CT and CBCT as well as CT and MRI scan pairs is shown to result in median target registration error in the order of 2 mm. This meets the accuracy requirement for adaptive radiation therapy and is especially interesting for MR-guided approaches. Another aspect, emerging in radiotherapy, is the utilization of deep-learning-based organ segmentation. As radiotherapy-specific labeled data is scarce, the training of such methods relies heavily on augmentation techniques. In this work, the generation of synthetically but realistically deformed scans used as Bionic Augmentation in the training phase improved the predicted segmentations by up to 15% in the Dice similarity coefficient, depending on the training strategy. Finally, it is shown that the biomechanical model can be built-up from automatic segmentations without deterioration of the KinematicDIR application. This is essential for use in a clinical workflow.

Es werden neue Methoden zur automatischen Bestimmung und Optimierung von Bestrahlungsparametern für die perkutane Strahlentherapie mit hochenergetischen Photonen entwickelt. Die Methoden basieren auf einer Bestrahlungstechnik mit intensitätsmodulierten Strahlenfeldern. Das wesentliche Problem besteht daher in der Bestimmung der Form der Modulationsprofile für die einzelnen Felder, und zwar auf der Grundlage der vorgegebenen Solldosisverteilung. Dieses Problem wird als das inverse Problem der Strahlentherapieplanung bezeichnet. Es wird gezeigt, daß es sich dabei um die gespiegelte Version des Problems der Rekonstruktion eines Bildes aus seinen Projektionen handelt, wie es z.B. bei der Computer-Tomographie (CT) auftritt. Ausgehend von diesem Sachverhalt werden die aus der CT bekannten Methoden zur Bildrekonstruktion konsequent auf die Optimierung der Strahlentherapie übertragen. Durch entsprechende Modifikationen der Methoden werden besondere Merkmale, die für dieses neue Anwendungsgebiet charakteristisch sind, berücksichtigt. Dazu zählt insbesondere die Tatsache, daß keine negativen Strahlungsintensitäten realisiert werden können und daß man aus praktischen Gründen auf wenige Felder beschränkt ist. Es wird gezeigt, daß in den meisten Fällen sieben oder neun Strahlenfelder ausreichend sind und daß die Verwendung von mehr Feldern nicht zu klinisch signifikanten Verbesserungen führt. Die wichtigsten Methoden der Bildrekonstruktion, nämlich die gefilterte Rückprojektion und die iterative Rekonstruktionstechnik, werden in der CT alternativ eingesetzt. In der vorliegenden Anwendung werden diese Methoden dagegen quasi "symbiotisch" verwendet. Die gefilterte Rückprojektion, die hier als gefilterte Projektion bezeichnet wird, dient dabei zur schnellen Bestimmung eines Startwertes für die Modulationsprofile. Diese Startprofile werden durch ein iteratives Verfahren, das der iterativen Rekonstruktionstechnik entspricht, weiter optimiert. Durch die Einführung von Straffunktionen ist es erstmalig möglich, medizinisch indizierte Nebenbedingungen adäquat zu berücksichtigen. Das iterative Optimierungsverfahren basiert auf einem Algorithmus zur dreidimensionalen Dosisberechnung. Ein weiterer Schwerpunkt dieser Arbeit ist daher die Entwicklung eines solchen Algorithmus für intensitätsmodulierte Strahlenfelder. Mit konventionellen Dosisberechnungsalgorithmen können Modulationen nicht angemessen berücksichtigt werden. Zur Überprüfung des neu entwickelten Verfahrens wird ein erster Vergleich der damit berechneten Dosis mit gemessenen Daten durchgeführt. Die hier vorgestellten Methoden erlauben die direkte Bestimmung der Bestrahlungsparameter ohne das heute übliche "trial and error"-Verfahren. Darüber hinaus können Dosisverteilungen erzeugt werden, die auch mit den aufwendigsten konventionellen Bestrahlungstechniken kaum realisierbar sind. Dies sind insbesondere solche mit ausgedehnten konkaven Bereichen. Einige Beispiele dieser Art werden dargestellt.

This dissertation presents two studies, in which the relationship of impairment in sensory and motor systems with frailty was investigated from a conceptual point of view (Study 1) and as a potential target for innovative treatment to reduce frailty (Study 2). The aim of Study 1 was to identify sensory and motor determinants of frailty as assessed by two common frailty instruments, the frailty phenotype (FP) and the frailty index (FI). Performance measures of sensory and motor function were assessed in 44 pre-frail and frail subjects. Separate multiple logistic regression analyses revealed that frailty as defined by the FP was associated with reduced upper extremity function, while frailty as defined by the FI was independently associated with higher hearing thresholds, reduced lower extremity performance and higher depression scores. This suggests that reduced sensory and motor function contributes to the syndrome of frailty, thereby offering a potential target for treatment, and that different frailty instruments may be differentially sensitive to capture functional impairment in frail populations. In Study 2, the effectiveness of a 90-day tablet-based sensorimotor training (n=24) targeting the reversal of age-related maladaptive neuroplasticity in the sensorimotor system to counteract frailty was evaluated, compared to a tablet-based relaxation control training (n=24). After 60 days of training, a reduction in frailty as determined by the FP was found for both groups, while the effect tended to be stronger for the sensorimotor training condition. A non-significant reduction in the FI was found irrespective of the group. No training effects were found for sensorimotor brain activity assessed by functional magnetic resonance imaging and corticomotor excitability assessed by transcranial magnetic stimulation. The results suggest that a neuroplasticity-based training may alter frailty, yet the significance of the postulated neuroplastic mechanisms and the specific training characteristics underlying the effect remain to be determined. Together, the two studies provide evidence that impairment in sensory and motor systems may represent a target mechanism to better understand pathophysiology of frailty and to develop novel, innovative treatment approaches. Longitudinal studies are needed to determine the influence of sensory and motor decline in the development of frailty. The present work may also inspire future large-scale interventional studies to validate the present preliminary, yet promising results and to examine the efficacy and mechanistic principles that approaches targeting the reversal of age-related maladaptive neuroplasticity may have in the treatment of frailty.

Der akute ischämische Schlaganfall ist aktuell weltweit eine der häufigsten Todesursachen von Menschen. Zu seiner Behandlung hat sich inzwischen die Thrombektomie als Standardtherapie etabliert. Bei ihr handelt es sich um eine minimal-invasive, endovaskuläre Interventionstechnik, bei der Führungsdrähte, Katheter und Stentretriever intraarteriell unter Fluoroskopieführung und Kontrastmittelgaben zum Thrombus vorgeschoben werden, um diesen auf mechanische Weise zu bergen. Zur Kontrolle, ob die Thrombusbergung erfolgreich war, werden während der Intervention digitale Subtraktionsangiographie-Bildsequenzen erstellt. Sie visualisieren die aktuellen Flussverhältnisse in den cerebralen Arterien und dienen der Erkennung etwaiger noch vorhandener Verschlüsse. Thrombektomiepatienten und die behandelnden Neuroradiologen sind einer hohen Strahlendosis ausgesetzt. Die Kontrastmittelgaben sind insbesondere für Patienten mit Niereninsuffizienz als kritisch zu betrachten. Bei eher proximal gelegenen cerebralen Gefäßverschlüssen ermöglicht die Thrombektomie eine Revaskularisation in bis zu 90% der Fälle. Ursächlich für das Ausbleiben des Behandlungserfolgs kann eine nicht optimale Platzierung der Bergungsinstrumente im direkten Umfeld des Thrombus sein. Eine weitere Ursache können intraoperativ neu aufgetretene, thromboembolische Verschlüsse sein, die aber während der Intervention in den digitalen Subtraktionsangiographie-Bildsequenzen übersehen werden. Um Neuroradiologen künftig bei der endovaskulären Thrombektomie Hilfestellungen zu geben und die Rate erfolgreicher Revaskularisierungen potentiell zu steigern, wurden in dieser Arbeit verschiedene computergestützte Assistenzmethoden für die Thrombektomie entwickelt und evaluiert. Zur Minimierung des Risikos, neue Thrombosierungen während der Thrombektomie zu übersehen, wurde im Rahmen einer retrospektiven Studie an 260 cerebralen Thrombektomien eine neue Deep-Learning-basierte Klassifikationsmethode entwickelt. Sie diente der automatischen Unterscheidung von thrombenfreien und nicht-thrombenfreien digitalen Subtraktionsangiographie-Bildsequenzen. Die Klassifikation war mit einem Matthews-Korrelationskoeffizienten von 0,77 und einer Area-Under-the-Curve von 0,94 möglich. Aus medizinischer Sicht mag die erreichte Klassifikationsleistung durchaus annehmbar sein, aber erst eine prospektive Studie kann analysieren, wie sich die Klassifikationsmethode im klinischen Alltag bewährt. Ferner konnte an realen Fällen, bei denen Thromben während der Intervention übersehen wurden, demonstriert werden, dass dies mit der vorgestellten Methode hätte vermieden werden können. Als ein weiterer Bestandteil dieser Arbeit wurde ein Gefäßphantom konzipiert und hergestellt, das die realitätsnahe Simulation thrombektomietypischer Arbeitsabläufe wie etwa die Gefäßsondierung oder die intraarterielle Instrumentenführung ermöglicht. Um zu untersuchen, wie die Führung der Bergungsinstrumente erleichtert und die Strahlenexposition und die Kontrastmittelgaben künftig reduziert werden können, wurde zusätzlich eine Methode entwickelt, mit der die Position eines elektromagnetisch räumlich lokalisierten Bergungsinstruments in Relation zum cerebrovaskulären Gefäßbaum angezeigt werden kann. Die quantitative Evaluation dieser registrierungsgestützten Instrumentennavigationsmethode erfolgte in einer in-vitro-Benutzerstudie am Gefäßphantom und ergab, dass ein Aspirationskatheter rein registrierungsgestützt mit einem durchschnittlichen (n = 20) Gesamtfehler von 2,3 ± 1,3 mm an fünf vordefinierten Sondierungszielen platziert werden konnte. Dabei zeigte sich, dass vermutlich die Lokalisierungsungenauigkeit des Katheters maßgeblich zum Gesamtfehler beigetragen hat. Aus Sicht der klinisch tätigen Neuroradiologen fiel die Beurteilung der registrierungsgestützten Navigationsmethode insgesamt positiv aus. Als vorteilhaft wurde empfunden, dass insbesondere die Darstellung der Sondierungsszene als 3D-Ansicht einen Beitrag dazu leisten kann, die Navigation der Bergungsinstrumente in Zukunft zu erleichtern und die Einsparung von Fluoroskopieaufnahmen und Kontrastmittelgaben zu ermöglichen. Die durchgeführten Studien dieser Arbeit haben gezeigt, dass die entwickelten computergestützten Assistenzmethoden das Potential haben, Neuroradiologen bei Thrombektomieinterventionen zu unterstützen. Darüber hinaus kann durch diese Assistenzmethoden die Rate erfolgreich durchgeführter Thrombektomien gesteigert werden. Bis zum in-vivo-Einsatz der Methoden an Thrombektomiepatienten ist weitere prospektive Forschungs- und Entwicklungsarbeit notwendig. Zu diesem Zweck kann das Gefäßphantom seinen Beitrag leisten, indem es die Durchführung weiterer präklinischer in-vitro-Studien ermöglicht. Sicher ist die Integration dieser Assistenzsysteme in den klinischen Alltag auch von der Akzeptanz innovativer Techniken abhängig. Dabei wird die Vorteilsbildung für die Patienten und die Behandler in der ärztlichen Wahrnehmung die entscheidende Rolle spielen.

Cognitive-behavioral models of chronic pain assume that fear of pain and subsequent avoidance behavior contribute to pain chronicity and the maintenance of chronic pain. In chronic back pain (CBP), avoidance of movements often plays a major role in pain perseverance and interference with daily life activities. In treatment, avoidance is often addressed by teaching patients to reduce pain behaviors and increase healthy behaviors. The current project explored the use of personalized virtual characters (doppelganger avatars) in virtual reality (VR), to influence motor imitation and avoidance, fear of pain and experienced pain in CBP. We developed a method to create virtual doppelgangers, to animate them with movements captured from real-world models, and to present them to participants in an immersive cave virtual environment (CAVE) as autonomous movement models for imitation. Study 1 investigated interactions between model and observer characteristics in imitation behavior of healthy participants. We tested the hypothesis that perceived affiliative characteristics of a virtual model, such as similarity to the observer and likeability, would facilitate observers’ engagement in voluntary motor imitation. In a within-subject design (N=33), participants were exposed to four virtual characters of different degrees of realism and observer similarity, ranging from an abstract stickperson to a personalized doppelganger avatar designed from 3d scans of the observer. The characters performed different trunk movements and participants were asked to imitate these. We defined functional ranges of motion (ROM) for spinal extension (bending backward, BB), lateral flexion (bending sideward, BS) and rotation in the horizontal plane (RH) based on shoulder marker trajectories as behavioral indicators of imitation. Participants’ ratings on perceived avatar appearance were recorded in an Autonomous Avatar Questionnaire (AAQ), based on an explorative factor analysis. Linear mixed effects models revealed that for lateral flexion (BS), a facilitating influence of avatar type on ROM was mediated by perceived identification with the avatar including avatar likeability, avatar-observer-similarity and other affiliative characteristics. These findings suggest that maximizing model-observer similarity may indeed be useful to stimulate observational modeling. Study 2 employed the techniques developed in study 1 with participants who suffered from CBP and extended the setup with real-world elements, creating an immersive mixed reality. The research question was whether virtual doppelgangers could modify motor behaviors, pain expectancy and pain. In a randomized controlled between-subject design, participants observed and imitated an avatar (AVA, N=17) or a videotaped model (VID, N=16) over three sessions, during which the movements BS and RH as well as a new movement (moving a beverage crate) were shown. Again, self-reports and ROMs were used as measures. The AVA group reported reduced avoidance with no significant group differences in ROM. Pain expectancy increased in AVA but not VID over the sessions. Pain and limitations did not significantly differ. We observed a moderation effect of group, with prior pain expectancy predicting pain and avoidance in the VID but not in the AVA group. This can be interpreted as an effect of personalized movement models decoupling pain behavior from movement-related fear and pain expectancy by increasing pain tolerance and task persistence. Our findings suggest that personalized virtual movement models can stimulate observational modeling in general, and that they can increase pain tolerance and persistence in chronic pain conditions. Thus, they may provide a tool for exposure and exercise treatments in cognitive behavioral treatment approaches to CBP.

Da Knorpelgewebe eine limitierte intrinsische Regenerationskapazität hat, sind operative Interventionen zur Therapie von Knorpelschäden oftmals unerlässlich. Aktuelle Therapieoptionen bieten jedoch häufig keine langfristig zufriedenstellenden Ergebnisse, so dass der Einsatz von Gelenkprothesen meist unvermeidbar ist. Diese bieten, anders als vitales Gewebe, jedoch keine Anpassungsmöglichkeiten an ihre mechanische Umgebung. Einen vielversprechenden Ansatz zur Herstellung von vitalem Knorpelersatzgewebe stellt das Tissue Engineering dar. Allerdings muss sichergestellt werden, dass das in vitro gezüchtete Knorpelersatzgewebe auch eine knorpeltypische Mechanokompetenz erwirbt, um die Homöostase des Gewebes gewährleisten zu können. Das bedeutet, dass die Aspekte der Mechanokopplung im Gewebe, der Mechanotransduktion der Reize ins Zellinnere und ihre Übersetzung in intrazelluläre Signale, sowie der Mechanoadaptierung denen von nativem Knorpelgewebe entsprechen sollten. Ob mesenchymale Stromazellen (MSC), die aufgrund ihrer leichten Isolierbarkeit, hohen Proliferationskapazität und chondrogenen Differenzierbarkeit eine vielversprechende Zellquelle für die Züchtung von Knorpelersatzgewebe darstellen, die gleiche Mechanokompetenz entwickeln wie artikuläre Chondrozyten (AC), war nicht bekannt. Daher verfolgte diese Arbeit das Ziel, die Mechanokompetenz von AC- und MSC-basiertem Knorpelersatzgewebe zu vergleichen. In dieser Studie konnte qualitativ hochwertiges AC- und MSC-basiertes Knorpelersatzgewebe mit vergleichbaren Mechanokopplungsparametern, basierend auf einem vergleichbaren Glykosaminoglykan (GAG)-Gehalt und gleicher Härte, die der von nativem Knorpel nahekam, hergestellt werden. Dies erlaubte eine vergleichende Untersuchung der Rolle der Zellherkunft für die Mechanotransduktion und Mechanoadaptierung in Knorpelersatzgewebe. AC und MSC-basierte Chondrozyten reagierten auf eine dreistündige, für AC zuvor als physiologisch bewertete zyklische Kompressionsepisode, die dreistündiges Gehen in 10-min Intervallen imitierte, mit einer gleichartigen Stimulation der Mechanotransduktionsparameter pERK1/2, der Expression mechanosensitiver Gene, mit Ausnahme von SOX9, und von microRNAs. Dabei konnte erstmals eine Mechanoinduktion gängiger mechanosensitiver Gene auch für humanes MSC-basiertes Knorpelersatzgewebe gezeigt werden. Ferner wurde erstmals eine Mechanostimulation ausgewählter microRNAs in MSC-basierten Chondrozyten belegt. Zum ersten Mal wurden die GAG- und Kollagensynthese in MSC-basierten Chondrozyten nach zyklischer Kompression bestimmt und ein Vergleich zu AC zeigte, dass MSC-basierte Chondrozyten empfindlicher auf das gewählte physiologische Belastungsprotokoll reagierten. Somit ergaben sich für MSC-basierte Chondrozyten bei ähnlicher Mechanotransduktion maßgebliche Defizite in der Mechanoadaptierung. Um die Ursachen für die differenzielle Mechanoadaptierung zu identifizieren, wurde die Rolle von Signalkaskaden mit anaboler, kataboler und nicht eindeutig zugeordneter Wirkung untersucht. Der anabole BMP-Signalweg war kein limitierender Faktor für die Matrixsyntheseleistung der Zellen. Erstmals wurden eine Mechanoinduktion von PGE2 für humanes MSC-basiertes Knorpelersatzgewebe sowie eine signifikant höhere PGE2-Freisetzung aus MSC- vs. AC-basiertem Knorpelersatzgewebe unter gleichen Kulturbedingungen belegt. Eine für MSC-basierte Chondrozyten bereits bekannte Freisetzung von NO nach zyklischer Kompression wurde in dieser Studie erstmalig auch für humane AC nachgewiesen. Nachweislich katabole Einflüsse von NFκB trugen zur gestörten Matrixsynthese in MSC-basierten Chondrozyten nach Belastung bei. Erstmals wurde auch eine PTHLH-Stimulation in AC und MSC-basierten Chondrozyten nach dynamischer Kompression belegt. Katabole Einflüsse des PTHrP/cAMP/PKA-Signalwegs deuteten einen Beitrag zur reduzierten Matrixsynthese nach Belastung sowie zur Mechanoinduktion einzelner mechanosensitiver Gene an. Die Belastung stoppte die Differenzierung MSC-basierter Chondrozyten und reduzierte begleitend dazu die Hypertrophie. So hatte die Belastung zwar keinen spezifischen anti-hypertrophen Effekt, eine spezifische PTHLH-Stimulation legte allerdings, gemeinsam mit einer Reduktion der Matrixsynthese durch PTHrP(1-34), nahe, dass der anti-chondrogene Effekt der Belastung auch einen Beitrag aus dem PTHrP-Signalweg enthalten könnte. Diese Studie liefert damit neben der Erweiterung von aus AC bekannten Mechanoadaptierungsparametern um vier Signalkaskaden mit kataboler oder variabler Wirkung auf den Zellmetabolismus (NFκB, NO, PGE2 und PTHrP) einen bedeutenden Beitrag zur Charakterisierung der Mechanokompetenz von AC- und MSC-basiertem Knorpelersatzgewebe. Durch Inhibition kataboler Einflüsse von NFκB und dem PTHrP/cAMP/PKA-Signalweg wurden wichtige Stellschrauben für die Installation einer Mechanoresistenz von MSC-basierten Chondrozyten hin zu Verhältnissen wie in nativen AC erkannt. Diese Erkenntnisse können nun dazu beitragen, funktionelle Tissue Engineering Ansätze zu verbessern und erweiterte pharmakologische Behandlungsoptionen nach zellbasierten Knorpeltherapieverfahren zu erwägen. Für zukünftige Studien gilt es, den Einfluss der Matrixzusammensetzung auf die Mechanokompetenz von Knorpelersatzgewebe zu untersuchen und die molekularen Ursachen der variableren SOX9-Regulation durch Belastung in MSC-basierten Chondrozyten zu entschlüsseln.

Phosphorus (31P) magnetic resonance spectroscopic imaging (MRSI) enables non-invasive imaging of pH value and magnesium ion concentration, [Mg2+], in living tissue. Typically, the chemical shifts of 31P metabolites are translated to pH and [Mg2+] via calibration equations, which, however, are well-defined only for physiological conditions, thus limiting their reliability in pathologies. In this thesis, a novel approach for the in vivo determination of pH and [Mg2+] applicable for various chemical conditions, utilizing multiple 31P spectral properties in form of a look-up table, is proposed. To this end, the variability of spectral properties in vivo was investigated in high-quality human 31P MRSI data acquired at B0 = 7T, and reproduced in measurements of 114 model solutions prepared with physiologically relevant pH, [Mg2+] and ionic strengths, at B0 = 9.4T. The identified chemical shift changes of the adenosine-5’-triphosphate (ATP) resonances were translated into a multi-dimensional model, which was used for the implementation of a look-up algorithm, accounting also for measurement uncertainties. Application to in vivo data resulted in successful assignments of plausible pH and [Mg2+] values in healthy and tumorous tissue. The proposed approach is extendable by including further spectral properties, potentially allowing for more profound tissue characterizations, being particularly important for the application in pathologies.

Owing to high stakes in the field of healthcare, medical machine learning (ML) applications have to adhere to strict safety standards. In particular, their performance needs to be robust toward volatile clinical inputs. The aim of the work presented in this thesis was to develop a framework for uncertainty handling in medical ML applications as a way to increase their robustness and trustworthiness. In particular, it addresses three root causes for lack of robustness that can be deemed central to the successful clinical translation of ML methods: First, many tasks in medical imaging can be phrased in the language of inverse problems. Most common ML methods aimed at solving such inverse problems implicitly assume that they are well-posed, especially that the problem has a unique solution. However, the solution might be ambiguous. In this thesis, we introduce a data-driven method for analyzing the well-posedness of inverse problems. In addition, we propose a framework to validate the suggested method in a problem-aware manner. Second, simulation is an important tool for the development of medical ML systems due to small in vivo data sets and/or a lack of annotated references (e. g. spatially resolved blood oxygenation (sO 2 )). However, simulation introduces a new uncertainty to the ML pipeline as ML performance guarantees generally rely on the testing data being sufficiently similar to the training data. This thesis addresses the uncertainty by quantifying the domain gap between training and testing data via an out-of-distribution (OoD) detection approach. Third, we introduce a new paradigm for medical ML based on personalized models. In a data-scarce regime with high inter-patient variability, classical ML models cannot be assumed to generalize well to new patients. To overcome this problem, we propose to train ML models on a per-patient basis. This approach circumvents the inter-patient variability, but it requires training without a supervision signal. We address this issue via OoD detection, where the current status quo is encoded as in-distribution (ID) using a personalized ML model. Changes to the status quo are then detected as OoD. While these three facets might seem distinct, the suggested framework provides a unified view of them. The enabling technology is the so-called invertible neural network (INN), which can be used as a flexible and expressive (conditional) density estimator. In this way, they can encode solutions to inverse problems as a probability distribution as well as tackle OoD detection tasks via density-based scores, like the widely applicable information criterion (WAIC). The present work validates our framework on the example of biophotonic imaging. Biophotonic imaging promises the estimation of tissue parameters such as sO 2 in a non-invasive way by evaluating the “fingerprint” of the tissue in the light spectrum. We apply our framework to analyze the well-posedness of the tissue parameter estimation problem at varying spectral and spatial resolutions. We find that with sufficient spectral and/or spatial context, the sO 2 estimation problem is well-posed. Furthermore, we examine the realism of simulated biophotonic data using the proposed OoD approach to gauge the generalization capabilities of our ML models to in vivo data. Our analysis shows a considerable remaining domain gap between the in silico and in vivo spectra. Lastly, we validate the personalized ML approach on the example of non-invasive ischemia monitoring in minimally invasive kidney surgery, for which we developed the first-in-human laparoscopic multispectral imaging system. In our study, we find a strong OoD signal between perfused and ischemic kidney spectra. Furthermore, the proposed approach is video-rate capable. In conclusion, we successfully developed a framework for uncertainty handling in medical ML and validated it using a diverse set of medical ML tasks, highlighting the flexibility and potential impact of our approach. The framework opens the door to robust solutions to applications like (recording) device design, quality control for simulation pipelines, and personalized video-rate tissue parameter monitoring. In this way, this thesis facilitates the development of the next generation of trustworthy ML systems in medicine.

It is tempting to assume that FAIR data principles effectively apply globally. In practice, digital research platforms play a central role in ensuring the applicability of these principles to research exchange, where General Data Protection Regulation (EU) and Multi Level Protection Scheme 2.0 (PRC) provide the overarching legal frameworks. For this article, we conduct a systematic review of research into Chinese Republican newspapers as it appears in Chinese academic journal databases. We experimentally compare the results of repeated search runs using different interfaces and with different points of origin. We then analyze our results regarding the practical and technical accessibility conditions. Concluding with an analysis of conceptual mismatches surrounding the classification of items as “full-text“, and of a case of total data loss that is nevertheless symptomatic of the limited degree of data re-usability. Our results show structural challenges preventing Findability, Accessibility, Interoperability, and Re-usability from being put into practice. Since these experiments draw upon our Digital Humanities (DH) research, we include a state-of-the-field overview of historical Periodicals and digitization research in the PRC. Our research on the one hand addresses DH practitioners interested in digital collections, and technical aspects of document processing with a focus on historical Chinese sources. On the other hand, our experience is helpful to researchers irrespective of the topic. Our article is accompanied by a data publication containing sources and results of our experiments, as well as an online bibliography of the research articles we collected.

Die zirkulierende Low-Density-Lipoprotein-Konzentration im Blut kann durch die Verabreichung von Statinen gesenkt werden. Hierfür wird häufig Simvastatin (SV) verschrieben. Aufgrund der berichteten pleiotropen Wirkungen von SV war es das Ziel dieser Studie, die Mineralisierungseffekte auf Stromazellen aus menschlichem Fettgewebe nach Verabreichung von SV zu untersuchen. Nach Zustimmung der Probanden (14) wurden Stromazellen aus menschlichem Fettgewebe welches aus Gewebsüberschüssen von rouitinemäßiger Behandlungen gewonnen. Nach etablierten Protokollen wurden nach Zugabe verschiedener SV-Konzentrationen (0,01 µM, 0,1 µM, 1,0 µM, 2,0 µM) alkalische Phosphatase (Osteoblastenmarker), Mineralisierungsfähigkeit und Lebensfähigkeit an Tag 18, 21 und 28 bestimmt. Für die statistische Analyse wurde der Kruskal-Wallis-Test durchgeführt. Targeted therapies improve the prognosis of advanced anaplastic lymphoma kinase-driven non-small cell lung cancer (ALK+ NSCLC) patients. However, clinical courses vary considerably due to acquired drug resistance. Thus, timely detection of treatment failure is crucial to guide subsequent therapies and optimize patient outcome. The analysis of tumor alterations in cell-free DNA (cfDNA) represents a novel approach to monitor cancer dynamics during therapy in longitudinal plasma samples. Besides mutations and copy number alterations, cancer-specific epigenomic changes have emerged as promising biomarkers for cfDNA-based tumor assessment. This thesis aimed to identify tumor-derived methylation (5mC) and hydroxymethylation (5hmC) alterations in cfDNA of metastatic ALK+ NSCLC patients, associate these epigenetic biomarkers to gene expression in lung cancer and assess their suitability for monitoring of tyrosine kinase inhibitor therapy in serial plasma samples. To this end, 79 longitudinal plasma samples from 31 patients were collected alongside plasma of 14 healthy individuals. Genome-wide 5mC and 5hmC profiles were generated by cell-free methylation immunoprecipitation and 5hmC selective chemical labeling, followed by sequencing. Additionally, 5(h)mC profiles of primary monocytes, neutrophils and erythroid progenitor cells were prepared using the same methods. These hematopoietic cells constitute the major non-tumor contributors (72.2%) to cfDNA of cancer patients. A technical novelty of this study was the enrichment for tumor-derived 5(h)mC signals in cfDNA by excluding genomic regions highly (hydroxy-)methylated in the reference blood cell types. Of 9,603,454 300-bp genomic loci, 577,701 (5mC; 6.0%) and 499,681 (5hmC; 5.2%) exhibited low (or no) signal in the profiled blood cells. The blood cell signal-reduced 5mC regions demonstrated an increased correlation to lung cancer tissue methylation (Spearman, r = 0.26), compared to the entire dataset (r = 0.11), and revealed cancer- as well as tissue-specific 5mC signals. Cancer versus control analysis at the remaining genomic regions identified 5,499 differentially methylated (DMRs) and 495 differentially hydroxymethylated regions (DhMRs). Hierarchical clustering analysis based on the D(h)MRs cleanly separated patient from control samples and clustered patient cfDNA according to the inferred tumor burden within the samples. This suggests that sample separation is primarily driven by tumor-derived signals and confirms that the identified D(h)MRs are enriched for cancer 5(h)mC alterations. DMRs proximal to transcription start sites were enriched at genes downregulated in lung cancer tissue, demonstrating that cancer-specific gene regulatory 5mC marks can be retrieved from cfDNA. Many of these genes (e.g. GATA4 and HOXA9) were previously described to confer tumor suppressive functions in NSCLC. 5(h)mC levels in cfDNA correlated with tumor-derived genomic alterations (e.g. EML4-ALK fusion and global chromosomal instability [t-MAD score]) determined in matched plasma samples. The highest correlation was observed between PTGER4 methylation and t-MAD scores (Pearson, r = 0.86). Four 5mC (SOX9-AS1, HOXA10-AS, PRAC1, and PTGER4) and three 5hmC biomarkers (IL1RAP, KPNA7 and KIF25) were employed for therapy monitoring in ten patients with available longitudinal samples (³2). In particular 5mC biomarkers mirrored cancer dynamics found by radiologic imaging and genomic tumor alterations in cfDNA. At four instances, cfDNA 5mC levels anticipated therapy relapse in advance of imaging with a maximum lead time of 481 days. In one patient, both 5mC and 5hmC biomarkers detected disease progression ahead of imaging and genomic alterations in cfDNA, highlighting the sensitivity of 5(h)mC-based tumor assessment. In conclusion, 5mC and 5hmC profiling from cfDNA provides an opportunity for sensitive cancer detection and therapy monitoring. The tissue-specificity and the regulatory functions of these DNA modifications provide data about the tumors that currently cannot be obtained by copy number or single nucleotide variation profiling.

Multimodal imaging, the combination of various optical imaging techniques, is widely used for biomedical applications because of the complementary nature of the information content. In this context, a promising technology is Raman spectroscopy. Raman spectroscopy is a label-free, highly molecular specific technique, which enables the detection of biomolecules such as lipids and proteins. This study presents the design of a novel multimodal imaging system that combines Raman spectroscopy with visible (VIS) and near-infrared (NIR) spectroscopy as well as light microscopy (bright-field, dark-field, and polarisation microscopy) in the incident light mode. In addition, the imaging system is equipped with a light path for transmitted light microscopy. In clinical and pathological diagnostics, bright-field microscopy of stained tissue sections in the transmitted light mode is a well-established imaging technique. Combined with images of an unstained tissue section obtained from other imaging modalities, the stained tissue section serves as a reference for tissue classification models. The first application of the imaging system is realised to distinguish between grey matter (GM) and white matter (WM) of mouse brain tissue. All applied modalities are able to differentiate between the two main brain areas. The best results for the unstained tissue section are achieved for Raman spectroscopy in the high-wavenumber region and polarisation microscopy. In addition to the distinction between GM and WM, both modalities emphasise differences in the density of myelinated axons. Further tissue experiments investigate the design‘s capability to detect haemorrhage with VIS and Raman spectroscopy and to detect water with NIR spectroscopy. Moreover, experiments for the specification of several setup parameters like the lateral resolutions of the spectroscopic modalities, the spectral characteristics of the light sources, the microscope stage’s accuracy, and the various signal stabilities are carried out. Additionally, a workflow to perform imaging of biological samples is elaborated. In the end, potential setup modifications to increase the measurement efficiency are proposed. Anyhow, the designed multimodal imaging technique is an all-in-one solution, which facilitates the measurement of unstained tissue samples with different modalities and the results‘ complementation by images of stained tissue sections.

Inertialsensoren spielen eine zentrale Rolle bei der Umsetzung von Anwendungen für die Interaktion zwischen Nutzer:Innen und eingebetteten Systemen. Dazu zählen Gestensteuerung, virtuelle Realität sowie Indoor-Navigation. Diese Dissertationsschrift stellt Arbeiten vor, mit denen es auf einfache Weise und ohne externe Hilfsmittel möglich ist, die Qualität der Inertialsensoren und der Sensordatenfusion zu bewerten. Die Arbeiten umfassen Bewertungsmethoden für Smartphone-Sensoren, Untersuchungen von Sensorfusionsalgorithmen sowie Methoden zur Augmentation von Sensordaten und Debugging., Inertial sensors play an important role in the implementation of applications for interaction between users and embedded systems. These include methods such as gesture control, virtual reality and indoor navigation. This dissertation paper presents work that makes it possible to evaluate the quality of inertial sensors and fusion of sensor data in a simple way and without external tools. The works include evaluation methods for smartphone sensors, investigations of sensor fusion algorithms, and methods for sensor data augmentation and debugging.

Die Prävalenz von Alkohol in der heutigen Gesellschaft ist unumstritten, wodurch sich vielfältige Fragestellungen ergeben, welche oftmals nicht alleinig durch die Messung einer Blutalkoholkonzentration beantwortet werden können. Aufgrund dessen nahm die Bedeutung von spezifischen Abbauprodukten des Ethanols, auch Alkoholbiomarker genannt, in den letzten Jahren stetig zu, da ein Alkoholkonsum durch deren Messung deutlich länger nachweisbar ist. Dafür ist eine verlässliche und robuste Analytik der verschiedenen Biomarker zur Beantwortung diverser Fragestellungen, z.B. im Rahmen der Abstinenzkontrolle, unerlässlich. Aus diesem Grund war das Ziel dieser Arbeit die Entwicklung, Etablierung, Validierung und Anwendung einer neuartigen und spezifischen Multimethode zum Nachweis diverser Alkoholbiomarker wie Ethylglucuronid, Ethylsulfat, N-Acetyltaurin und 16:0/18:1 Phosphatidylethanol. Nach einer erfolgreichen Methodenentwicklung und -validierung entsprechend der Richtlinien der GTFCh konnten alle vier Biomarker mit einer sehr guten Intensität und Reproduzierbarkeit qualitativ nachgewiesen und quantitativ bestimmt werden. Anschließend wurde die Methode im Rahmen verschiedener Studien angewendet, um forensisch und analytisch verwertbare Ergebnisse zu gewinnen. Es ergaben sich Anwendungsgebiete aus der Leichentoxikologie, Pharmakokinetik sowie der klinischen Diagnostik. Zusätzlich wurde geklärt, inwieweit eine Anwendung von NAcT als Alkoholbiomarker sinnvoll ist, wobei die erhaltenen Ergebnisse eine Anwendung nicht unterstützen. Um die Eignung der Methode zur Untersuchung von postmortalem Blut zu testen, wurden 31 Proben gemessen, in denen zuvor hohe Blutalkoholkonzentrationen nachgewiesen werden konnten. Die hierdurch ermittelten Ergebnisse bestätigten die Anwendbarkeit der Methode für postmortales Probenmaterial. Aufgrund der Bestimmung mehrerer Alkoholbiomarker kann eine Alkoholisierung vor dem Tod von einer durch postmortal gebildetes EtOH verursachten Verfälschung der BAK unterschieden werden. Durch die parallele Bestimmung der drei Alkoholbiomarker Ethylglucuronid, Ethylsulfat und 16:0/18:1 Phosphatidylethanol steigt die Aussagekraft der erhobenen Ergebnisse. Zusätzlich wurden zwei Trinkversuche am Institut für Rechts- und Verkehrsmedizin in Heidelberg durchgeführt, in welchem Probanden eine individuelle Alkoholmenge aufgenommen haben, um eine vorgegebene Blutalkoholkonzentration zu erreichen. Über den ersten Versuchstag und einem Zeitfenster von sieben Tagen wurden in definierten Abständen Blut- und Urinproben erhoben, um die Verlässlichkeit der entwickelten Multimethode und die Pharmakokinetiken der einzelnen Biomarker zu untersuchen. Ein weiteres Ziel dieses Studiendesigns war die Ermittlung eines physiologischen Grundwertes für N-Acetyltaurin, da diese Substanz im menschlichen Körper endogen gebildet wird. Es konnte belegt werden, dass N-Acetyltaurin den Ansprüchen eines Biomarkers zum Nachweis eines Alkoholkonsums nicht gerecht werden kann, da die individuellen Schwankungen zu stark sind und keine nachvollziehbare Pharmakokinetik aufgezeigt werden konnte. Im Gegensatz hierzu konnte für die bereits etablierten Biomarker Ethylglucuronid und Ethylsulfat erneut bestätigt werden, dass diese in der Matrix Urin eine entsprechend gute Möglichkeit bieten, in Abhängigkeit von der aufgenommenen Dosis, einen vorangegangen Alkoholkonsum verlässlich nachzuweisen. Der Biomarker 16:0/18:1 Phosphatidylethanol, welcher nur in Vollblut nachgewiesen werden kann, deckt ein noch größeres Nachweisfenster ab und kann auch ein einmaliges, länger zurückliegendes Trinkgeschehen belegen. Dieser Biomarker ist bisher in der forensischtoxikologischen Analytik in Deutschland zum Nachweis einer Alkoholabstinenz nicht etabliert. Mittels der im Rahmen dieser Arbeit validierten Methode ist es nun möglich, den Biomarker 16:0/18:1 Phosphatidylethanol im Rahmen des Abstinenzprogrammes am IRVM analytisch zu bestimmen und in die Beurteilung mit einfließen zu lassen. In einem weiteren Projekt wurde die neu entwickelte Methode zum Nachweis der drei anwendbaren Biomarker im Entzugskontext verwendet. Das Kollektiv bestand aus Blutproben, die zu Beginn und Ende eines Entzugsaufenthaltes im Krankenhaus Salem abgenommen und auf ihre Biomarkerkonzentrationen untersucht wurden. Dabei konnte gezeigt werden, dass Ethylglucuronid und Ethylsulfat innerhalb der Klinikaufenthalte unabhängig von dem Parameter der Lebersteifigkeit im Blut komplett abgebaut wurden. Da Phosphatidylethanol in den Erythrozytenmembranen akkumuliert und nur sehr langsam abgebaut wird, war dieser Biomarker auch zum Ende des Entzugs bei nahezu allen Patienten nachweisbar. Im Falle dieses Alkoholbiomarkers konnte durch einen Kruskal-Wallis-Test gezeigt werden, dass es keinen Zusammenhang zwischen dem Abbau und der Lebersteifigkeit gab. Zusammenfassend lässt sich feststellen, dass die in dieser Arbeit entwickelte analytische Multimethode vielfältige Möglichkeiten bietet, um ein sehr breites Spektrum von Fragestellungen im Bereich der Forensischen Toxikologie beantworten zu können.

Using human cell lines for the production of therapeutic proteins can be beneficial due to their ability to generate fully human post-translational modifications. HEK 293 cells are an efficient expression system with a record of approved therapeutic protein products. However, the presence of N-acetylgalactosamine, the low sialylation on N-glycans of recombinant HEK 293 cell-derived glycoproteins and the resulting efficient clearance via glycan receptors in patients—namely the asialoglycoprotein receptor and the mannose receptor—currently limits its use in therapeutic protein production. To be able to produce recombinant proteins without N-acetylgalactosamine on their N-glycans, HEK 293-F clones featuring a functional KO of both the B4GALNT3 and B4GALNT4 genes were generated. Subsequently, to increase the sialylation, sialyltransferases ST6GAL1 and ST3GAL6 were overexpressed by gene knock-in. The model protein factor VII-albumin was expressed in these new host cell lines to evaluate the phenotypic changes induced by the described glyco-engineering. Furthermore, medium supplementation with N-acetylmannosamine and cytidine was tested to improve sialylation. HEK 293-F cells with B4GALNT3 and B4GALNT4 knock-out produced factor VII-albumin devoid of N-acetylgalactosamine and sulfated N-acetylgalactosamine, with reduced antenna fucosylation and increased antennarity, bisecting N-acetylglucosamine as well as sialylation. Overexpression of ST6GAL1 or ST3GAL6 further increased sialylation and reduced antenna fucosylation, while sialylation was not affected by medium supplementation of sialic acid metabolism precursors N-acetylmannosamine and cytidine. Factor VII-albumin produced in the double N-acetylgalactosamine transferase knock-out cell line with simultaneous ST6GAL1 knock-in showed a level of sialylation similar to that of plasma-derived factor VII and higher than that of factor VII from CHO or BHK cells. As a result, asialoglycoprotein and mannose receptor binding of glyco-engineered factor VII-albumin variants was abolished in vitro and pharmacokinetic properties were improved in vivo. To determine whether the detected changes in N-glycosylation were specific for factor VII-albumin or whether the results are applicable to other proteins, B domain-deleted factor VIII and factor IX were expressed in the wild-type HEK 293-F cell line, the double N-acetylgalactosamine transferase knockout clone and the double N-acetylgalactosamine transferase knock-out clone with ST6GAL1 knock-in. Despite considerable differences in the N-glycosylation profiles of the three proteins, the effects of the applied glyco-engineering were largely comparable. Thus, these new glyco-engineered cell lines are suitable for the expression of fully human therapeutic proteins with favorable N-glycosylation and improved pharmacokinetic properties.

Magnetic resonance imaging (MRI) employs time-varying magnetic gradient fields, which induce electric fields (E-fields) in the patient that can potentially stimulate the heart. The performance of novel gradient systems is increasingly restricted by regulatory safety limits, thus motivating a deeper understanding of cardiac stimulation (CS) in MRI. This thesis investigates the thresholds and mechanisms underlying CS using an integrative approach of modeling and measurements. First, a numerical modeling framework was developed that combines E-field simulations in computational body models with electrophysiological cardiac fiber models to predict stimulation thresholds and sites in the heart. The CS thresholds predicted for two commercial gradient systems were >10-fold higher than the regulatory limits. Second, cardiac magnetostimulation thresholds were measured in ten healthy pigs using magnetic field pulses created by capacitor discharges into a coil. The average threshold E-field in the porcine heart was 92.9 pm 13.5 V/m. Die Versorgung der Mitralklappeninsuffizienz mithilfe der minimalinvasiven Mitralklappenreparatur ist von erheblicher Bedeutung für die Erhaltung bzw. Wiederherstellung der gesundheitsbezogenen Lebensqualität von Patientinnen und Patienten weltweit. Im Rahmen der vorliegenden Arbeit sind sowohl fluidmechanische Prüfmethoden als auch numerische Fluid-Struktur-Interaktionsmodelle für die Beurteilung aktueller und die Konzeptionierung neuer Clipimplantatdesigns und Implantationsstrategien entwickelt worden.

An den Antriebskomponenten auftretende Kavitationserosionserscheinungen setzen Wirkungsgrad, Sicherheit und Effizienz schnell fahrender Schiffe herab und führen zum Abtrag aus der Oberfläche. Repairing such damage represents a tremendous time and cost factor. Arc spraying potentially presents an opportunity to coat and restore eroded contours. In order to achieve good mechanical properties and durability, defects in the coatings must be minimized and residual stresses reduced and, respectively, their influence on the functional properties clarified.

Optrodes usually represent a combination of optical fiber or waveguide and single or multiple microelectrodes. This thesis explaines fabrication process of fiber-based optrode consisting of tapered optical fiber with two deposited gold electrodes on its surface. Furthermore, design and construction of custom mechanical grinding setup, that enables fabrication of tapered fiber tips of good optical quality with various cone angles, represents an important part of this thesis. Detailed optical and electrical characterization of fabricated optrodes is also presented.

The advantages of carbon-ion pencil beam radiotherapy imply an increased sensitivity of the dose distribution in the patient to any changes in the patient geometry, such as internal anatomical changes or patient misalignment. This can lead to a deterioration of the dose distribution within the patient. Monitoring methods of the internal patient’s dose distribution for carbon-ion beam radiotherapy are therefore of great importance to early detect possible under- or over-dosage in the patient, eventually, reduce the tumor safety margins applied around targeted tumor volumes and thus decrease the delivered dose in healthy tissues. Up to now, several non-invasive in-vivo ion-beam monitoring methods have been developed. These are mostly based on the detection of different kinds of secondary radiation, such as annihilation-photons from β+ emitters, prompt photons, or prompt charged nuclear fragments, emitted from a patient during the treatment delivery. These secondary radiations are the results of nuclear interactions of the primary treatment beam with the irradiated tissue. They potentially carry valuable information about the primary treatment beam range, position, or intensity in the patient. However, so far none of the monitoring methods has reached sufficient maturity for a wide application in clinical routine. This thesis aimed to develop methods for detection and localization of therapy-relevant geometry variations of 2 mm in head models, mimicking possible inter-fractional changes on the surface or inside patients’ heads. In contrast to previous research which concentrated on single stationary pencil beams, this thesis was focused on entire therapy-like treatment plans composed of thousands of single pencil beams with low numbers of primary-ions and irradiated under clinic-like conditions in terms of dose, dose rate, and tumor volume. In this thesis, methods were based on the detection and tracking of charged secondary nuclear fragments (secondary ions) emitted from the patient during carbon ion radiotherapy delivery. Subsequently, methods for analysis and interpretation of the measured secondary-ion paths (tracks) were developed. The developed radiation detection methods exploited the capabilities of a novel mini-tracker, based on the Timepix3 technology developed at CERN and positioned behind the patient. The deadtime-free data acquisition enabled a gapless recording of all impacting secondary ion tracks. Moreover, it enabled synchronization of the data with the beam application monitoring system, and thus assign each measured secondary ion with its respective pencil beam, opening entirely new research possibilities. The experiments were performed at the Heidelberg Ion-Beam Therapy Center (HIT), closely mimicking clinic-like conditions. Single fields of carbon-ion treatment plans with a prescribed fraction dose of 3 Gy (RBE) were used to simulate treatments of spherical tumor volumes in the used head models. Two types of head models were used: a homogeneous plastic cylinder and an anthropomorphic head phantom composed of real bones and tissue-equivalent materials. Secondary ions exiting the head models during irradiation were detected with a mini-tracker composed of two small (2cm²) parallel Timepix3 detectors placed downstream of the head with a certain angle with respect to the beam axis. Inter-fractional changes were modeled by adding or removing 2-mm-thick slabs positioned in front or inside the targeted head models. Within the thesis, it was demonstrated that the developed method for the analysis of the measured track distributions, taking into account the actual time-dependent position of the pencil beam, approximated the measured position of the secondary ion creation in the head model significantly better than the methods developed up to now. By using this method, surface changes down to 1 mm were found to be detectable even for the anatomical head phantom. Internal changes of 2-mm-thickness extending over the whole lateral tumor dimension (wide changes) were found to be detectable for all investigated positions between the dose plateau and the distal end of the tumor. The significance was at least 3 standard deviations for a single mini-tracker and of at least 9 standard deviations when using 8 mini-trackers at 30°, as it is planned for the future. Correct localization of all the studied changes was achieved within 6.3 mm of their actual position. This is sufficient to provide information to the clinicians about the part of the dose distribution which is affected. The detection of 2-mm-thick changes affecting only a part of the tumor (narrow changes), required the development of a new method based on the additional information on the lateral pencil beam positions. With this technique, internal 2-mm-thick changes as small as 10 mm in diameter placed in front of the tumor, were demonstrated to be detectable with a significance of almost 2 standard deviations. This technique makes the developed monitoring method sensitive to the lateral position of the cavity and thus reaches the third dimension. Positions of the mini-tracker closer to the beam axis were found to provide higher detection efficiencies due to the larger amount of data, but also lead to larger geometrical uncertainties and lower localization accuracies. At larger angles, the accuracy of the change localization was found to be better. For future measurements, multi-angle detection systems are recommended to maximize both detectability and localization accuracy. Finally, the applicability of the monitoring of carbon-ion pencil beam delivery in a real patient treatment was demonstrated by designing a patient-friendly measurement system that was shown to be safely used in a clinical environment. After investigating the influence of the developed system on the beam delivery, and with the fulfillment of all clinical and safety requirements, the integration of this system into the clinical workflow of the HIT facility was achieved. With this detection system, the first measurement of a real patient irradiation fraction was performed. The amount of measured data was sufficient to determine a secondary-ion emission profile along the depth of the patient’s head. And a differentiation between pencil beams with a 1 cm range difference was demonstrated. In conclusion, this thesis presents novel methods for carbon ion treatment monitoring of external and internal patient geometry changes in the head based on secondary ion tracking, allowing detection changes down to the clinically desired 2 mm. The designed monitoring system was proven to be well incorporable into a clinical workflow. Thus, the presented work paves the way towards monitoring inter-fractional changes along the beam direction during carbon-ion beam therapy and builds the basis for the upcoming clinical trial at the HIT facility.

Bei der angeborene Zwerchfellhernie (CDH) handelt es sich um eine schwere Erkrankung, die mit einer Inzidenz von 1:2000–1:5000 auftritt. Die Prognose wird maßgeblich vom Ausmaß des Defektes und der pulmonalen Hypertonie bestimmt. Trotz Versorgung in Schwerpunktzentren werden Mortalitätsraten von bis zu 30 % angegeben. Eine wesentliche Säule der Behandlung ist, neben der chirurgischen Versorgung, die Therapie der pulmonalen Hypertonie. Je nach respiratorischer und zirkulatorischer Einschränkung kann eine extrakorporale Membranoxygenierung (ECMO) notwendig sein. Etablierte pränatale klinische Prognoseparameter wie die Position der Leber oder das Lungen-zu-Kopf-Verhältnis (LHR) helfen, den klinischen Verlauf und die Notwendigkeit einer invasiven Therapie abzuschätzen. Nichtsdestotrotz fehlen insbesondere leicht zu erhebende und objektiv zu bewertende postnatale Prognoseparameter. In diesem Zusammenhang sind die kardialen Marker Pro-BNP und Troponin in den Fokus der Wissenschaft gerückt. Kleinere retrospektive Studien sowie eine prospektive Arbeit lassen einen potenziell prädiktiven Wert dieser Proteine bei Patienten mit CDH vermuten. Ziel dieser Arbeit war es folglich, die Bedeutung postnatal gemessener Pro-BNP- und Troponin-I-Werte an einer großen Kohorte eines ausgewiesenen CDH-Zentrums zu untersuchen. Dazu wurden Patienten mit CDH, die zwischen 2007 und 2012 an der Universitätsmedizin Mannheim behandelt wurden, retrospektiv ausgewertet. Die Erfassung der klinischen Basisparameter erfolgte in einer prospektiven Datenbank. Eingeschlossen wurden Patienten mit linksseitigem Zwerchfelldefekt, die in der Frauenklinik des Universitätsklinikums Mannheim geboren wurden („inborn“) und die 34+0 SSW erreicht hatten. Ausgeschlossen wurden Patienten ohne verfügbaren Pro-BNP-Wert inner-halb der ersten 4 Lebensstunden. Als Endpunkte wurden das Versterben, die Notwendigkeit einer ECMO-Therapie, einer iNO-Therapie sowie das Auftreten einer chronischen Lungenerkrankung (CLD) Stadium II-III festgelegt. Es erfolgten Analysen zu Gruppenvergleichen in Bezug zu klinischen Prognoseparametern und End-punkten sowie Korrelationsanalysen. Pro-BNP und Troponin-I wurden durch univariante und multivariate Regressionsmodelle als Prädiktoren für die Endpunkte geprüft. Für unabhängige Prädiktoren wurden zusätzlich ROC-Analysen durchgeführt. Im untersuchten Zeitraum wurden insgesamt 335 Patienten mit CDH am Universitätsklinikum Mannheim behandelt, von denen insgesamt 119 Patienten in die Auswertung eingeschlossen wurden. 62,1 %, der Patienten waren männlich und kamen im Median in der 37,4 SSW (34,0–39,7) mit einem medianen Geburtsgewicht von 2990 g (1750–4050) zur Welt. Insgesamt wiesen 57,1 % (n = 68) der Patienten eine Hernierung der Leber auf. Patienten mit Leberhernierung hatten keine signifikant unterschiedlichen Pro-BNP-Werte, jedoch erhöhte Troponin-I-Werte (p = 0,007). Es zeigte sich keine signifikante Korrelation zwischen Pro-BNP und Troponin-I (r = 0,16, p = 0,17). Insgesamt verstarben 10,1 % (n = 12) der Patienten. Eine ECMO-Therapie wurde bei 35,2 % (n = 42) der Patienten initiiert. Verstorbene Patienten und Patienten mit ECMO-Bedarf wiesen signifikant höhere Troponin-I-Werte auf (p = 0,003 und p = 0,01), wobei sich kein Unterschied in den bestimmten Pro-BNP-Leveln zeigte (p = 0,27 und p = 0,22). In der multivariaten Regression wurde Troponin-I mit einem Grenzwert von 0,04 ng/ml als unabhängiger Prädiktor für das Ver-sterben (OR = 14,8, p < 0,001) und für die ECMO-Notwendigkeit (OR = 3,0, p = 0,02) identifiziert. Ein Einfluss auf die iNO-Therapie wurde nicht beobachtet (p = 0,16). Das Pro-BNP zeigte keinen signifikanten Zusammenhang mit den End-punkten. Die ROC-Analysen brachten eine AUC für das Troponin-I zur Vorhersage des Überlebens von 0,8 und der ECMO-Notwendigkeit von 0,72. Eine explorative Subgruppenanalyse zeigte, dass bei Patienten mit hohem Pro-BNP-Anstieg im Ver-lauf die Wahrscheinlichkeit der ECMO-Notwendigkeit signifikant höher war (p = 0,008). Die Daten der vorliegenden Arbeit bekräftigen die insgesamt wenigen, meist retrospektiven Arbeiten zur eingeschränkten Bedeutung des Pro-BNP als Prognoseparameter bei der CDH. Die Dynamik der Pro-BNP-Veränderung kann die ECMO-Notwendigkeit möglicherweise vorhersagen, was in weiteren Arbeiten geprüft wer-den sollte. Troponin-I kann als postnataler, einfacher und ubiquitär verfügbarer Marker den Krankheitsverlauf und die ECMO-Notwendigkeit möglicherweise vorhersagen. Dies muss in weiteren Arbeiten bestätigt werden, um sinnvolle Grenzwerte zu etablieren.

Assessment of T cell responses towards disease-associated antigens is of key interest in order to develop and monitor immunotherapeutic interventions. Patient sample material, however, is usually highly limited, which hinders comprehensive screening of putative novel antigens by conventional methods such as the enzyme-linked immunospot (ELISPOT) or peptide major histocompatibility complex (pMHC) multimer staining. To overcome this issue, several alternative assays have recently been proposed that allow simultaneous measurement (i.e. multiplex) of a larger pool of T cell specificities within a single assay reaction. However, these technological approaches are currently not well suited for a broad and routine laboratory usage. Therefore, we have developed a novel sensitive method called T-Plex Assay for the multiplex detection of antigen-specific T cell responses using flow cytometry, which can be implemented easily into routine laboratory practice. The elaborated T-Plex Assay concept is based on cell-sized, fluorescently color-coded magnetic polystyrene beads such as the Luminex xMAP MagPlex® microspheres, which we have conjugated to defined recombinant pMHC-I or pMHC-II molecules. This pMHC conjugation in turn converts the beads into barcoded, artificial antigen-presenting cells (aAPCs) with the capacity to drive antigen-specific T cell activation. In addition, on the same pMHC-coated bead surface we co-immobilized monoclonal antibodies specific for various T cell effector cytokines such as interferon-γ that are released upon T cell activation. These assembled barcoded aAPCs with cytokine capture capacity serve ultimately as cell contact-dependent biosensors for cognate T cell populations. Executing the T-Plex Assay concept, we developed an easy-to-use and profoundly optimized T-Plex Assay protocol facilitating highly reproducible as well as simultaneous measurement of theoretically up to 80 antigen-specific T cell responses within a single reaction, while allowing for complete recovery of the viable T cell sample post analysis. As a proof of concept, we demonstrated the T-Plex Assay-based detection of various HLA-A*02:01-restricted viral antigen-specific T cell populations within healthy donor peripheral bloodderived T cells. Here, the T-Plex Assay displayed a similar sensitivity compared to conventional standard assays such as the pMHC multimer staining. Moreover, we used the T-Plex Assay in a small pilot study of four cancer patients to validate the immunogenicity of several in silico predicted HLA-A* 02:01-restricted tumor neoantigens. Unfortunately, analysis of cancer patient-derived peripheral blood by the T-Plex Assay or corresponding pMHC multimer staining did not reveal any T cell population specifically responding to the tested putative neoantigens. In an interlinked study aspect, we established a novel manufacturing strategy for efficient and easily parallelized small-scale production of correctly folded and ready-to-use soluble pMHC-I and pMHC-II molecules based on various mammalian cell transient gene expression systems. Here, we elaborated several expression constructs encoding for covalently assembled pMHC-I as well as pMHC-II Fc-fusion proteins. These pMHC-Fc proteins were successfully produced and notably bound in an antigenspecific manner to their cognate T cell populations. Although the true potential of the T-Plex Assay remains to be further analyzed, we think that the T-Plex Assay including our soluble pMHC production strategy holds great promise for clinical applications aiming for the routine discovery and validation of novel disease-associated T cell antigens particularly in cases where cell material is limited.

We report on the developments that enabled the field deployment of a fully-automated aerosol mass spectrometer, specially designed for high-altitude measurements on unpressurised aircraft. The merits of the two main categories of real-time aerosol mass spectrometry, i.e. (a) single particle laser desorption and ionization, and (b) continuous thermal desorption / electron impact ionization of aerosols, have been integrated into one compact apparatus with the aim to perform in-situ real-time analysis of aerosol chemical composition. The demonstrated instrument, named ERICA (European Research council Instrument for the Chemical composition of Aerosols), operated successfully aboard the high-altitude research aircraft M-55 Geophysica at altitudes up to 20 km, while being exposed to ambient conditions of very low atmospheric pressure and temperature. A primary goal of those field deployments was the in-situ study of the Asian Tropopause Aerosol Layer (ATAL). During 11 research flights, the instrument operated for more than 49 hours and collected chemical composition information of more than 150,000 single particles combined with quantitative chemical composition analysis of aerosol particle ensembles. This paper presents in detail the technical characteristics of the main constituent parts of the instrument, as well as the design considerations for its integration into the aircraft and its autonomous operation in the upper troposphere and lower stratosphere (UT/LS). Additionally, system performance data from the first field deployments of the instrument are presented and discussed, together with exemplary mass spectrometry data collected during those flights.

Hauptaugenmerk der Arbeit lag ausschließlich auf der elektrisch evozierten Stapediusreflexschwelle (ESRT), welche bisher lediglich während einer Cochlea Implantat (CI) Operation zum Einsatz kommt. Weiter bestand das primäre Ziel dieser Arbeit darin, einen standardisierten CI Anpassungsleitfaden zu entwickeln sowie in der Schaffung von Grundlagenwissen, auf welchem aufbauend weiterführende Forschungen betrieben werden können. Bei der ESRT-Schwellenbestimmung wird durch einen elektrischen Reiz die hervorgerufene Kontraktion des M. stapedius gemessen. Durch die Kontraktion des Muskels kommt es zu einer Versteifung der Gehörknöchelchenkette, welche am Trommelfell gemessen werden kann. Die ESRT-Messungen korrelieren gut mit dem subjektiven Lautheitsempfinden der CI Träger. Bisher wurde diese sehr aussagekräftige objektive Messmethode im Sinne einer Lautstärkenermittlung postoperativ auf Grund der fehlenden Praxiserfahrung nicht eingesetzt. Die CI-Einstellung erfolgt meist nach subjektiven Angaben des Patienten. Die ESRT-Messung wird aktuell häufig nur während der Operation in Narkose visuell durch mikroskopische Beobachtung von Kontraktionen der Sehne des M stapedius bestimmt. Bei der intraoperativen ESRT Bestimmung galt bisher auch nicht, den eben noch visuell erkennbaren Stapediusreflex mit der niedrigsten Stromeinheit zu ermitteln. Die notwendige Stromstärkenschwelle, die den Reflex intraoperativ auslöst, soll allerdings dem Audiologen einen Anhalt dafür geben, die Stimulationsgrenzen bei der Anpassung des CI Sprachprozessors nicht zu überschreiten. Weiter können Anästhetika zu einer veränderten Reflexschwelle führen, sodass eine postoperative Ermittlung der ESRT am nicht sedierten Patienten zu einer noch genaueren Bestimmung der Stimulationsgrenzen führen kann. Im Rahmen dieser Arbeit konnte gezeigt werden, dass die ESRT-Schwellenwertermittlung mittels mikroskopischer Beobachtung (subjektive Methode) und die objektive Messmethode mittels funktionsdiagnostischer Verifikationstechnik sehr stark miteinander korrelieren. Darüber hinaus konnte gezeigt werden, dass die intra- sowie postoperativ erhobenen ESRT-Schwellenwertdaten nur noch moderat miteinander korrelieren. Deutlich zu Tage trat in den erhobenen Daten der Zusammenhang zwischen der postoperativen ESRT- und C-Level-Schwelle. Hier zeigte sich statistisch eine sehr starke Korrelation. Vergleicht man die Resultate der qualitativ und quantitativ ermittelten ESRT- und C-Level-Linienprofile miteinander, so belegen die postoperativ erhobenen ESRT-Schwellenwertdaten eine Orientierungshilfe für die CI-Anpassung. Aus der vorliegenden Arbeit ergeben sich für die praktische CI-Anpassung Vorteile gegenüber dem bisherigen Vorgehen im Sinne einer langfristigen Standardisierung und somit einer Qualitätssicherung. Die CI-Anpassung wird dahingehend vereinfacht, dass ausgehend von der ESRT-Schwelle das subjektive Lautheitsempfinden bestimmt werden kann, ohne dass jede Elektrode einzeln nach angenehmer Lautheit geprüft werden muss. Hieraus folgt, dass aus der postoperativ erhobenen ESRT-Kurve eine C-Level Kontur antizipiert werden kann, wenn eine Bestimmung dieser z.B. bei Incompliance, Sprachbarriere, bei Kindern oder aus zeitlichen Gründen nicht möglich ist. Eine Limitation der vorliegenden Arbeit besteht allerdings darin, dass die CI-Anpassung nach der dargestellten Methode anschließend nicht durch eine Sprachaudiometrie in Ruhe und im Störschall, einen Logatomtest sowie eine Lautheitsskalierung oder eine Messung der Hörermüdung ergänzend analysiert wurde. Somit wäre für potenzielle Folgestudien von Interesse, wenn sprachaudiometrische Ergebnisse nach beiden CI-Anpassungsmethoden verglichen würden, also, wenn die CI-Grundeinstellung einerseits auf der Grundlage der subjektiven Lautheitsakzeptanz und andererseits auf Basis der objektiven ESRT-Schwellenwerte vorgenommen würde. Die objektive Messmethode des postoperativ elektrisch ausgelösten Stapediusreflexes wird ein wichtiger Bestandteil dennoch zukünftig mit hoher Wahrscheinlichkeit in der CI-Anpassung werden, da mit ihrer Hilfe eine praktische und reproduzierbare Messmethodik für einen standardisierten CI-Anpassungsleitfaden entwickelt werden kann.

Eine ermüdungsfeste Auslegung von Bauteilen und Strukturen ist von essenzieller Bedeutung, sollen deren Ausfälle durch Ermüdungsbrüche sowie die finanziellen, wirtschaftlichen und sozialen Folgen verhindert werden. Viele Erscheinungen und Einflüsse im Very High Cycle Fatigue (VHCF) Bereich sind zum aktuellen Stand der Forschung nicht vollumfänglich geklärt. Um die Phänomene und Einflussgrößen der Rissausbreitung und -initiierung im VHCF-Bereich zu untersuchen, wurden innerhalb dieser Arbeit systematische Experimente, Bruchflächenanalysen und numerische Simulationen durchgeführt.

Biological systems are first of all physical systems whose function (or malfunction) is also determined by their response to external and internal forces as well as their physical properties. Therefore, measuring mechanical properties is not only essential for a complete understanding of cell function and organism development but also invaluable in disease diagnosis and treatment. Several techniques are available in the field of mechanobiology (an emerging field that focuses on the role of mechanical forces and properties in biology) to measure mechanical properties but they all virtually rely on analyzing the response of the sample to an external perturbation and have limited three-dimensional capabilities. Instead, Brillouin microscopy is a purely optical technique that requires no contact and no external mechanical perturbation of the sample and can provide 3D maps of viscoelastic properties with (sub)micron resolution. Its first demonstration for 3D imaging of biological samples was accomplished in 2007 and, since then, it has gained increasing interest among biologists for its potentialities. In this context, one of the aims of my PhD project was to facilitate the use of Brillouin microscopy for addressing biological questions. I did this by equipping a state-of-the-art Brillouin microscope with the add-ons necessary for studying living biological samples (confocal fluorescence and environment control) and by writing an intuitive software interface that would allow non-experts to use the microscope with minimal training. In collaboration with biologists, we investigated several biological questions, some of which produced interesting findings (that led to scientific publications) while others are still being explored. The second aim of my PhD was to tackle two issues that limit current implementations of Brillouin microscopy: limited speed (leading to imaging times in the excess of hours when whole organisms are imaged in 3D) and potential photodamage, especially in light-sensitive samples. I overcame this limitation by designing a line-scanning version of a Brillouin microscope, with particular attention to ensuring physiological mounting of the sample, low photodamage and high spatial resolution. The speed advantage is given by the parallel acquisition of about 100 Brillouin spectra in a single camera acquisition. This allowed us to image fast processes (gastrulation in Drosophila), large volumes (Phallusia embryos) and follow development of light-sensitive samples (mouse embryos) over two days.

Chemical exchange saturation transfer (CEST) magnetic resonance imaging (MRI) allows to non-invasively obtain information about the micromolecular environment of living tissues, particularly, intracellular pH (pHi). In this thesis, a method for quantitative CEST-based pHi mapping was developed enabling, for the first time, compensation of all concomitant effects on CEST signals in tumor tissues, such as unknown concentration changes and superimposing contributions. To this end, a novel model was established enabling direct translation of magnetization exchange rates between protons in proteins and water into pH. This model utilizes (i) the reliable extraction of exchange rates, accomplished by an optimized isolation of measured CEST signals at B0 = 9.4 T, and (ii) a calibration procedure exploiting a newly identified, distinct functional behavior of CEST signals. Porcine brain lysates mimicking in vivo conditions were employed for calibration and to demonstrate reliable pH mapping in the physiologically relevant range of 6.2–8.0. Application in vivo to lesions of tumorbearing mice showed a median pHi of approximately 7.2, further validating the developed method. This novel approach, now, allows to robustly assess the potential of pHi as imaging biomarker for cancer diagnosis or treatment monitoring in preclinical settings, representing an important milestone towards application in humans.

Die vorliegende Dissertation war Teil des vom Bundesministerium für Bildung und Forschung geförderten Projekts „Informationstechnologie für eine patientenorientierte Gesundheitsver-sorgung in der Metropolregion Rhein-Neckar“. Sie verfolgt die Zielsetzung, die Entwicklung eines Patientenportals einer persönlichen, einrichtungsübergreifenden elektronischen Patien-tenakte (PEPA) an den Anforderungen von Tumorpatienten und deren Angehörigen auszu-richten. Darüber hinaus soll die Handhabbarkeit eines PEPA-Prototyps evaluiert und mögliche Herausforderungen für die Implementierung einer solchen technischen Lösung identifiziert werden. Zunächst nimmt die Forschungsarbeit die Prototypenentwicklung in den Blick, die mithilfe einer neuen Methode, sogenannten ‚Action Sheets‘, umgesetzt wurde. Hierbei werden in ei-nem ersten Schritt Herausforderungen aufgezeigt, die sich in der nutzerzentrierten Entwick-lung von digitalen Lösungen für die Gesundheitsversorgung ergeben können. In einem nächs-ten Schritt wird dargestellt, wie man diesen durch neue Herangehensweisen des Require-ments Engineerings begegnen kann. Im vorliegenden Forschungsprojekt wurden Action Sheets als „Kommunikationsbrücke“ zwischen verschiedenen methodischen Ansätzen einge-setzt und auf diese Weise das interprofessionelle Vorgehen bei der Entwicklung der persönli-chen, einrichtungsübergreifenden elektronischen Patientenakte in eine strukturiertere und klarere Form gebracht. Zur weiteren Beantwortung der Forschungsfragen wird im Rahmen dieser Studie ein Mixed-Methods-Ansatz verfolgt. In einer Prä-Implementierungsstudie wurde der Prototyp des PEPA-Patientenportals zunächst mithilfe von Krebspatienten und Patientenangehörigen in Usability Tests erprobt. Anschließend wurden die Teilnehmer in teilstandardisierten Interviews zu ihren Erfahrungen mit dem Patientenportal befragt. Darüber hinaus wurden Patienteninterviews innerhalb einer dreimonatigen Machbarkeitsstudie zur Erprobung des Patientenportals unter realen Versorgungsbedingungen durchgeführt. Auf Basis dieser Vorgehensweise konnten Erkenntnisse zu Optimierungspotentialen des PEPA-Prototyps und des zugrundeliegenden Konzepts generiert werden. Alle Interviews wurden digital aufgezeichnet, transkribiert und mittels qualitativer Inhaltsanalyse ausgewertet. Die Nutzerfreundlichkeit des Prototyps wurde mithilfe eines standardisierten Fragebogens erhoben und in Bezug auf verschiedene sozio-demographische Faktoren ausgewertet. Diese Forschungsarbeit zeigt auf, in welcher Art und Weise das PEPA-Konzept weiterentwi-ckelt werden sollte, um eine nachhaltigere Verankerung einer solchen Akte im Versorgungs-alltag zu erreichen. Obwohl die Zufriedenheit mit der Usability des PEPA-Patientenportals hoch war, konnten generelle Erkenntnisse bezüglich patientenseitig wahrgenommener Hür-den für die Implementierung einer persönlichen, einrichtungsübergreifenden elektronischen Patientenakte in der Routineversorgung generiert werden. Eine dieser Hürden ergibt sich aus der zentralen Rolle, die der Patient im PEPA-Konzept ein-nimmt. Die Meinungen hinsichtlich der aktiven Patientenrolle im Versorgungsprozess, die bei-spielsweise durch die Vergabe von Zugriffsrechten auf Inhalte der Akte durch den Patienten angestrebt wird, variierten zwischen den verschiedenen Befragten. Ausgehend davon er-scheint es bedeutsam, Patientenportale in ihrer technischen Umsetzung möglichst flexibel zu gestalten, um sie individuell an die jeweiligen Nutzer und ihre Vorstellungen anpassen zu kön-nen. Auf der anderen Seite zeigt sich gerade am Beispiel von Tumorerkrankungen, dass ne-ben dem Patienten auch dessen engeres Umfeld in den Blick genommen werden muss. Die gezielte Einbindung von Angehörigen kann dazu beitragen, die Nutzerakzeptanz im Hinblick auf digitale Anwendungen zu erhöhen. Gleiches gilt für die Wissensvermittlung im Umgang mit innovativen Technologien in der Gesundheitsversorgung sowie eine niedrigschwellige Hil-festellung bei Anwendungsproblemen. Gerade der Einbezug von Patienten sowie deren Angehörigen als potenzielle Endnutzer eines Patientenportals sollte künftig ein selbstverständlicher Bestandteil bei der Entwicklung und Implementierung einer digitalen Lösung für die Gesundheitsversorgung werden. Dabei ist es wichtig, ihren wertvollen Input bereits bei der Konzeption von digitalen Anwendungen, wie z.B. einer persönlichen, einrichtungsübergreifenden elektronischen Patientenakte, einfließen zu lassen. Um die Öffentlichkeit mit einzubeziehen, muss auch der Herausforderung begegnet werden, mögliche Unterstützungs- und Schulungsbedarfe seitens dieser sehr heterogenen Anwendergruppe frühzeitig zu identifizieren und entsprechend zu adressieren. Demzufolge bedarf es einer transparenten und verständlichen Herangehensweise, um das Vertrauen der künftigen Nutzer zu gewinnen und die Akzeptanz digitaler Technologien zu befördern. Insgesamt bedarf das komplexe Versorgungsgefüge, in das eine persönliche, einrichtungs-übergreifende elektronische Patientenakte einzubetten ist, innovative Herangehensweisen bei der frühzeitigen und fortlaufenden Einbindung aller beteiligten Akteure. Außerdem sind auch die gesellschaftlichen Rahmenbedingungen zu berücksichtigen und entsprechend in einem umfassenden PEPA-Konzept miteinzubinden, um eine nachhaltige Nutzerakzeptanz sowie eine gewinnbringende Anwendung einer PEPA zu erreichen.

Process mining is a Business Process Management (BPM) technique that uses execution data of business processes for their analysis. By transforming the data to so-called event logs, process mining tools generate process models that describe the executions as close as possible. Process discovery can result either in graph-based notations (e.g., Petri nets or BPMN) or declarative ones like Declare. One hypothesis in this work is that declarative constraint templates can support model understanding in case process mining results in large, confusing "spaghetti" diagrams. Overall, this work contributes an approach including a prototypical implementation for applying association rule and sequential pattern mining to event logs for discovering declarative process models. Preprocessing steps and the transformation of rules and patterns to constraints in Declare are addressed explicitly.
