BACKGROUND: Background The leucine-rich repeat kinase 2 gene (LRRK2) harbours highly penetrant mutations that are linked to familial parkinsonism. However, the extent of its polymorphic variability in relation to risk of Parkinson's disease (PD) has not been assessed systematically. We therefore assessed the frequency of LRRK2 exonic variants in individuals with and without PD, to investigate the role of the variants in PD susceptibility. METHODS: LRRK2 was genotyped in patients with PD and controls from three series (white, Asian, and Arab-Berber) from sites participating in the Genetic Epidemiology of Parkinson's Disease Consortium. Genotyping was done for exonic variants of LRRK2 that were identified through searches of literature and the personal communications of consortium members. Associations with PD were assessed by use of logistic regression models. For variants that had a minor allele frequency of 0.5% or greater, single variant associations were assessed, whereas for rarer variants information was collapsed across variants. FINDINGS: 121 exonic LRRK2 variants were assessed in 15 540 individuals: 6995 white patients with PD and 5595 controls, 1376 Asian patients and 962 controls, and 240 Arab-Berber patients and 372 controls. After exclusion of carriers of known pathogenic mutations, new independent risk associations were identified for polymorphic variants in white individuals (M1646T, odds ratio 1.43, 95% CI 1.15-1.78; p=0.0012) and Asian individuals (A419V, 2.27, 1.35-3.83; p=0.0011). A protective haplotype (N551K-R1398H-K1423K) was noted at a frequency greater than 5% in the white and Asian series, with a similar finding in the Arab-Berber series (combined odds ratio 0.82, 0.72-0.94; p=0.0043). Of the two previously reported Asian risk variants, G2385R was associated with disease (1.73, 1.20-2.49; p=0.0026), but no association was noted for R1628P (0.62, 0.36-1.07; p=0.087). In the Arab-Berber series, Y2189C showed potential evidence of risk association with PD (4.48, 1.33-15.09; p=0.012). INTERPRETATION: The results for LRRK2 show that several rare and common genetic variants in the same gene can have independent effects on disease risk. LRRK2, and the pathway in which it functions, is important in the cause and pathogenesis of PD in a greater proportion of patients with this disease than previously believed. These results will help discriminate those patients who will benefit most from therapies targeted at LRRK2 pathogenic activity. FUNDING: Michael J Fox Foundation and National Institutes of Health. Objective: We studied the independent and joint effects of the genes encoding alpha-synuclein (SNCA) and microtubule-associated protein tau (MAPT) in Parkinson disease (PD) as part of a large meta-analysis of individual data from case-control studies participating in the Genetic Epidemiology of Parkinson's Disease (GEO-PD) consortium. Methods: Participants of Caucasian ancestry were genotyped for a total of 4 SNCA (rs2583988, rs181489, rs356219, rs11931074) and 2 MAPT (rs1052553, rs242557) single nucleotide polymorphism (SNPs). Individual and joint effects of SNCA and MAPT SNPs were investigated using fixed- and random-effects logistic regression models. Interactions were studied on both a multiplicative and an additive scale, and using a case-control and case-only approach. Results: Fifteen GEO-PD sites contributed a total of 5,302 cases and 4,161 controls. All 4 SNCA SNPs and the MAPT H1-haplotype-defining SNP (rs1052553) displayed a highly significant marginal association with PD at the significance level adjusted for multiple comparisons. For SNCA, the strongest associations were observed for SNPs located at the 30 end of the gene. There was no evidence of statistical interaction between any of the 4 SNCA SNPs and rs1052553 or rs242557, neither on the multiplicative nor on the additive scale. Interpretation: This study confirms the association between PD and both SNCA SNPs and the H1 MAPT haplotype. It shows, based on a variety of approaches, that the joint action of variants in these 2 loci is consistent with independent effects of the genes without additional interacting effects. Die vorliegende Dissertationsschrift befasst sich mit der molekulargenetischen Analyse zweier Basalganglienerkrankungen. Zum einen wurden Patienten mit M. Parkinson genetisch untersucht, zum anderen Patienten mit autosomal dominanter zervikaler Torsionsdystonie. Die Aufgabe bestand in der passenden Wahl der Methode zur jeweiligen humangenetischen Fragestellung. Der erste Teil handelte von der Suche der krankheitsverursachenden Mutation für die autosomal dominante zervikale Torsionsdystonie mit Spätmanifestation auf Chromosom 18p (Kandidatenlokus DYT7). Die erkrankte Familie deutscher Herkunft zeigt dystone Symptome mit Betonung auf kraniozervikale und brachiale Körperabschnitte und ist somit die weltweit einzige bekannte Familie mit Vererbung dieser ansonsten sporadisch auftretenden Erkrankung. Die PCR-Sequenzierung der Kandidatengene ZFP161, LOC390828, NDUFV2 und PTPRM auf dem DYT7 Lokus erbrachte bei den sieben erkrankten Familienmitgliedern im Vergleich zu nicht verwandten Kontrollen (Ehepartner und 96 Kontrollen der Blutbank) keinen Aminosäureaustausch, der ausschließlich bei den erkrankten Probanden zu finden war. Technisch konzentrierte sich diese Untersuchung auf die Amplifizierung und anschließende Sequenzierung jedes einzelnen Exons in den zu untersuchenden Proben, und die Bestätigung einer putativen Mutation mittels Verdau der PCR-Produkte durch Restriktionsendonukleasen. Die Auswahl der Kandidatengene erfolgte aufgrund der Annahme pathobiochemischer Mechanismen, die durch andere Formen der vererbten Torsionsdystonie oder zellbiologische Experimente als krankheitsverursachend gelten. Auch wenn keine Mutation gefunden wurde, so konnten bereits bekannte und neue single nucleotide polymorphisms (SNP) etabliert werden. Die zweite Thematik befasste sich mit der Frage, ob das bereits bekannte Parkinson-Gen UCH-L1 auf dem PARK5 Lokus krankheitsverursachend für den autosomal dominanten M. Parkinson in einer spanischen Familie ist. Diese parametrische Kopplungsanalyse wurde mithilfe der heißen Polyacrylamid-Gelelektrophorese (PAGE) durchgeführt. Dabei konnte in allen Patienten und den Verwandten ersten Grades über Analyse der Mikrosatelliten nördlich und südlich der Kandidatenregion (UCH-L1) in einem Bereich sehr niedriger Rekombinationswahrscheinlichkeit eine Haplotypisierung erfolgen. Die Haplotypisierung zeigte nicht die erforderliche Identifizierung eines Krankheitsallels bei allen betroffenen Probanden. Somit ist hier neben der einzig bekannten deutschen PARK5 Familie keine weitere Familie mit UCH-L1 Mutation bestätigt worden. Dementsprechend ist die Ätiologie dieser Erkrankung in dieser Familie noch unklar, was aber der Bedeutung des Ubiquitin-Proteasom Systems in der Parkinson-Entität keinen Abbruch getan hat. Da alle anderen autosomal dominanten Parkinson-Loci ausgeschlossen sind, muss sich die Ursache für den M. Parkinson in dieser Familie in einem heute noch unbekannten Gen befinden. Weitere Untersuchungen im Rahmen eines Genomscans sind aufgrund der geringen Fallzahl nicht möglich. Die letzte Aufgabe dieser Arbeit bestand in der Durchführung einer Assoziationsstudie mit den putativen PINK1 (PARK6) Interaktoren NME4 und MTIF3 für den mehrheitlich sporadisch auftretenden M. Parkinson. Dabei wurden in zwei unabhängigen Studiengruppen mit insgesamt 453 sporadischen Parkinsonpatienten und 370 Kontrollen jeweils zwei SNPs auf gekoppelte Vererbung mit der Erkrankung untersucht. Der Unterschied zwischen den Testgruppen bestand im Studiendesign, da zum einen mit den Patienten nicht verwandte Kontrollen und zum anderen verwandte Kontrollen verwendet wurden. Die mit beiden Studientypen normalerweise auftretenden Probleme durch Stratifikation bzw. erniedrigte statistische Power konnten durch Kombination der Studien ausgeglichen werden. Das Methodenspektrum umfasste PCR und Restriktionsverdau, was zum Auffinden eines Kopplungsungleichgewichts für das Gen MTIF3 führte. Ein heterozygoter Basenaustausch für den Polymorphismus rs7669 erhöht signifikant das Relative Risiko an M. Parkinson zu erkranken, wohingegen der homozygote Basenaustausch das Krankheitsrisiko des Trägers signifikant erniedrigt. Bezüglich des Relativen Risikos wurde der Effekt der molekularen Heterosis nachgewiesen. Bei diesem mitochondrial lokalisierten Gen handelt es sich um einen Initiator der mitochondrialen Translation. Demzufolge besteht hier Einfluss auf die Homöostase und somit Funktionalität der Atmungskettenkomplexe, die als bedeutend für die Pathogenese des M. Parkinson angesehen werden. Die Verbindung zum mitochondrial lokalisierten PINK1 besteht aufgrund seiner Kinase-Aktivität in der An- und Abschaltung des mitochondrialen Translations - Initiationsfaktors. Aber auch die Wichtigkeit von NME4 konnte in dieser Studie trotz fehlender Assoziation nicht ausgeschlossen werden, da vorangehende experimentelle Ergebnisse dieses Protein bereits in den PINK1 Signalweg zuordnen konnten. MTIF3 könnte wohlmöglich ein wichtiger genetische Risikofaktor für den idiopathischen M. Parkinson sein. Es bleibt abzuwarten, ob zukünftige genetische und zellbiologische Experimente die Wichtigkeit, die in diesem Protein zu liegen scheint, bestätigen können. This thesis delves into the molecular analysis of two basal ganglia disorders. Patients with Parkinson’s disease or with autosomal dominant cervical torsion dystonia were genetically examined. Three molecular genetic approaches were used to elucidate the genetic causes of these diseases. The first part of the dissertation deals with a candidate gene-sequencing approach to search the disease-causing mutation for autosomal dominant cervical torsion dystonia with late onset on chromosome 18p (locus DYT7). The medical examination of a large family with German origin shows dystonia in the cranio-cervical region and the upper limbs. These symptoms usually appear in sporadic patients, but were observed with autosomal dominant inheritance in this family, which was the first pedigree worldwide with a large enough size to permit linkage analysis. The sequencing of the candidate genes ZFP161, LOC390828, NDUFV2 and PTPRM (within the DYT7 locus) in affected family members showed no disease causing mutation compared to unaffected controls of the same geographic origin. Technically, in each sample the candidate genes were amplified, followed by PCR sequencing. In case of a putative amino acid exchange, the frequency of such a variant was confirmed by restriction digest in further family members and unaffected controls from the blood bank. The selection of each candidate gene based on pathological and biochemical mechanisms known from other forms of inherited torsion dystonia or from cell biology experiments. Although no mutation was found, new single nucleotide polymorphisms (SNP) could be established and old ones could be verified. The second part of this thesis uses the linkage approach to deal with the question if a Spanish family with autosomal dominant Parkinson´s disease shows disease association with the candidate locus PARK5. To establish a parametric linkage analysis, the polyacrylamid gel eletrophoresis of microsatellites neighbouring the candidate gene UCH-L1 was applied as an appropriate method. In this region of very low recombination probability, chromosomal haplotypes in each patient and in unaffected but related controls could be established. This haplotype analysis could not verify the candidate alleles to be linked to disease inheritance. Besides the only known German family with mutation in the gene UCH-L1 there is currently no other specified family who shows linkage to PARK5. All other currently known autosomal inherited genes for familial Parkinon´s disease had been previously excluded. Consequently, the genetic etiology of this disease in the Spanish family stays unidentified until there are more members affected, which will give the opportunity to perform a genome-wide independent linkage analysis. In the last part of this thesis, a statistical approach via case-control association studies was used to test candidate interactor proteins of PINK1 for involvement in sporadic Parkinson´s disease pathogenesis. In a hypothesis-testing sample consisting of 192 Parkinson´s disease patients and 192 random blood bank controls two single nucleotide polymorphisms in the gene encoding for the mitochondrial initiation factor 3 (MTIF3) and the gene for the kinase protein NME4 were tested with allele association studies. The results showed a significantly increased frequency of heterozygotes among the patients and a significantly decreased frequency of homozygotes among the control group for one MTIF3 genotype. These results were corroborated in a hypothesis-confirming sample of Central European origin, containing 262 sporadic Parkinson patients and 182 related but uneffected controls. Analysing the distribution of the relative risk, the phenomenon of molecular heterosis can be shown. Here the study design differed from conventional association studies due to the fact that patients and unrelated random controls as well as patients and related but unaffected controls were tested. The first group maximizes statistical power, while the second group reduces stratification bias and environmental influences. The methodical range included polyacrylamide gel electrophoresis (PCR) following a restriction digest analysis. In conclusion, the relative risk for Parkinson´s disease may be significantly increased when the single nucleotide polymorphism (SNP) rs7669 shows a heterozygote nucleotide exchange and significantly decreased when the SNP shows a homozygote nucleotide exchange in contrast to the wildtype. The allele association found here marks a chromosomal area of 100kB distance centromeric and telomeric from MTIF3 as a candidate region. Within this chromosomal area there is only one other gene (GTF3A), but the protein encoded influences cellular protein synthesis rather than having influence on mitochondrial function. The putative PINK1 interactor MTIF3 may have influence on the homeostasis and functionality of the mitochondrial respiratory chain, emphasizing its putative role in Parkinson pathogenesis. For the polygenetic, non-Mendelian forms of Parkinson´s disease, MTIF3 might have an important role next to other, not yet identified genetic risk factors. This result should be validated by further genetic and biochemical studies to understand in detail the role of PINK1 and mitochondria in the Parkinson pathogenesis.
