Objectives: To differentiate the primary small-cell lung cancer (SCLC) and non-small-cell lung cancer (NSCLC) for patients with brain metastases (BMs) based on a deep learning (DL) model using contrast-enhanced magnetic resonance imaging (MRI) T1 weighted (T1CE) images. Methods: Out of 711 patients with BMs of lung cancer origin (SCLC 232, NSCLC 479), the MRI datasets of 192 patients (lesions’ widths and heights > 30 pixels) with BMs from lung cancer (73 SCLC and 119 NSCLC) confirmed pathologically were enrolled, retrospectively. A typical convolutional neural network ResNet18 was applied for the automatic classification of BMs lesions from lung cancer based on T1CE images, with training and testing groups randomized per patient to eliminate learning bias. A 5-fold cross-validation was performed to evaluate the classification of the model. The receiver operating characteristic (ROC) curve, accuracy, precision, recall and f1 score were calculated. Results: For a 5-fold cross-validation test, the DL model achieved AUCs of 0.8019 and 0.8024 for SCLC and NSCLC patients with BMs, respectively, and a mean overall accuracy of 0.7515±0.04. The DL model performed well in differentiating the primary SCLC and NSCLC with BMs. Conclusion: The proposed DL model is feasible and effective in differentiating the pathological subtypes of SCLC and NSCLC causing BMs, which may be used as a new tool for oncologists to diagnose noninvasively BMs and guide therapy based on the imaging structure of tumors.

Background: Talaromyces marneffei (T. marneffei) is a heat-dimorphic fungus that commonly causes fatal opportunistic infections in immunocompromised patients, such as those with human immunodeficiency virus (HIV) infection. In this case report, Case Presentation: In this case report we describe a case of intracranial infection of T. marneffei in a 42-year-old AIDS patient. Contrast enhanced MRI showed the left occipital lobe mass with ring enhancement, MRS showed elevated AAs and Lip waves in the mass. Surgical resection of the occipital lobe confirmed the lesion to be T. marneffei infection and possibly with tuberculosis after a pathological examination. Patients with intracranial ring enhancing space-occupying lesions on MRI should be considered for intracranial T. marneffei infection. Intracranial T. marneffei infection is relatively rarely reported and recently studied. Conclusion: The MRI in this case suggests that ring enhancement mass and elevated AAs and Lip waves are helpful in the diagnosis of T.marneffei infection.

Adeno-associated virus (AAV) gene therapy vectors, which contain a DNA transgene packaged into a protein capsid, have shown tremendous therapeutic potential in recent years. Methods traditionally used in quality control labs, such as high-performance liquid chromatography (HPLC) and capillary electrophoresis (CE), do not provide a complete understanding of capsid viral protein (VP) charge heterogeneity. In the present study, we developed simple, one-step sample preparation and charge-based VP separation using imaged capillary isoelectric focusing (icIEF) for monitoring AAV products. The robustness of the method was confirmed through a design of experiments (DoE) exercise. An orthogonal reverse-phase (RP) HPLC method coupled with mass spectrometry was developed to separate and identify charge species. Additionally, capsid point mutants demonstrate the capability of the method to resolve deamidation at a single site on the viral proteins. Finally, case studies using two different AAV serotype vectors establish the icIEF method as stability indicating and demonstrate that increases in acidic species measured by icIEF correlate with increased deamidation, which, we show, results in decreased transduction efficiency. The addition of a rapid and robust icIEF method to the AAV capsid analytical toolkit enables development and consistent manufacturing of well-characterized gene therapy products.

BACKGROUND: The automotive industry is labor-intensive, and workers are involved in highly repetitive tasks, long hours, and medium to low workloads, resulting in work-related musculoskeletal disorders (WMSDs), which have become a major health concern for workers in this industry. China is a major automotive country with tens of thousands of auto repair workers, but their conditions of WMSDs have been poorly reported. OBJECTIVE: This work aimed to analyze the current prevalence of WMSDs and their associated risk factors among automobile maintenance workers. METHODS: A total of 539 respondents from 50 automotive repair and maintenance companies in China were selected to investigate the prevalence of WMSDs via the modified Nordic Musculoskeletal Disorders Questionnaire. Chi-square test and logistic regression models were applied to analyze their associated risk factors. RESULTS: The total prevalence of WMSDs among 539 workers was 32.8%. The top three body parts for prevalence were the lower back (17.1%), neck (16.3%), and shoulders (14.5%). The highest prevalence of WMSDs was found in the neck (23.6%) and lower back (14.3%) for sheet metal workers and mechanics, respectively, whereas painters had the highest prevalence of WMSDs in the lower back and feet (both at 15.4%). This research addresses the finite-time control problem for nonaffine stochastic nonlinear systems with actuator faults and input saturation. Specifically, a new finite-time control scheme is constructed based on the adaptive backstepping framework, with the usage of a state observer and taking advantage of the universal approximation capability of the fuzzy-logic system (FLS). The novelty of this work is that it considers the output feedback problem of a completely nonaffine stochastic system and incorporates the idea of the dynamic surface control (DSC) design. By using the Lyapunov stability theory, all the signals of the controlled system can be semiglobal finite-time stable in probability (SGFSP) while the system is imposed with multiple actuator constraints. In the meantime, the problem of "complexity explosion" is avoided. Two simulation examples are given to demonstrate the validity of the presented strategy.

Based on the finite element software, a calculation model of cold working forming (pier thickness) of rivet type electrical contact materials was established, and the effects of different material properties (elongation, elastic modulus and hardness) on the cold working of AgSnO2 electrical contact materials were analyzed. The proposed simulation technology fills the gap in the force analysis method of silver alloy rivets, and can provide certain technical theory and process parameter design basis for rivet deformation control.

Data on the frequency of colorectal neoplasia in sporadic ampullary tumors remains scarce.We retrospectively reviewed 135 patients undergoing endoscopic ampullectomy from January 2018 to July 2021, and identified 95 patients with sporadic ampullary adenoma who underwent total colonoscopy. Colonoscopy findings were compared with 380 asymptomatic controls using the chi-squared test. Whole-exome sequencing (WES) was performed on one patient with synchronous adenomas of the ampulla of Vater and ascending colon.Colorectal polyps were present in 60% of Cases vs. 34.7% of Controls (P = 0.001), advanced adenoma in 20% vs. 5.5%, and adenocarcinoma in 4.2% vs. 0.8%. Cases tended to have larger polyps than Controls (P0.001), while there was no difference in polyp location and histology between the two groups. The odds ratio of all the colorectal lesions, advanced colorectal adenoma and adenocarcinoma in Cases was 1.7, 4.2, and 4, respectively. WES in one patient revealed that both of ampullary adenoma and colonic adenoma shared somatic ABCB1 mutation.The frequency of colorectal polyps or neoplasia was significantly higher in Cases than Controls. We proposed that ampullary neoplasia is analogous to colon lesions and warrants total colonoscopy screening in patients diagnosed with ampullary tumors.
