Background Oesophageal pressure (Poes) is used to approximate pleural pressure (Ppl) and therefore to estimate transpulmonary pressure (PL). We aimed to compare oesophageal and regional pleural pressures and to calculate transpulmonary pressures in a prospective physiological study on lung transplant recipients during their stay in the intensive care unit of a tertiary university hospital. Methods Lung transplant recipients receiving invasive mechanical ventilation and monitored by oesophageal manometry and dependent and nondependent pleural catheters were investigated during the post-operative period. We performed simultaneous short-time measurements and recordings of oesophageal manometry and pleural pressures. Expiratory and inspiratory PL were computed by subtracting regional Ppl or Poes from airway pressure; inspiratory PL was also calculated with the elastance ratio method. Results 16 patients were included. Among them, 14 were analysed. Oesophageal pressures correlated with dependent and nondependent pleural pressures during expiration (R2=0.71, p=0.005 and R2=0.77, p=0.001, respectively) and during inspiration (R2=0.66 for both, p=0.01 and p=0.014, respectively). PL values calculated using Poes were close to those obtained from the dependent pleural catheter but higher than those obtained from the nondependent pleural catheter both during expiration and inspiration. Conclusions In ventilated lung transplant recipients, oesophageal manometry is well correlated with pleural pressure. The absolute value of Poes is higher than Ppl of nondependent lung regions and could therefore underestimate the highest level of lung stress in those at high risk of overinflation., During controlled ventilation without respiratory muscle activity, absolute oesophageal pressure is higher than the pleural pressure of the nondependent lung regions and could therefore underestimate the highest level of lung stress in that lung https://bit.ly/3a95CUh

Background: Rapid Response Systems (RRS) are now commonly implemented throughout hospital health systems to manage in-hospital emergencies (IHE). There is limited data on characteristics and outcomes of such patients admitted to an intensive care unit (ICU). The goal was to determine whether the hospital mortality of ICU patients was different depending on their admission pathway: in-hospital via rapid response teams (RRT), or out-of-hospital emergencies (OHE) via prehospital emergency medical systems. Results: Out of 422 ICU admissions (Timone University Hospital ICU), 241 patients were retrospectively (2019-2020) included: 74 IHE versus 167 OHE. In-hospital mortality rates did not differ between both cohorts (n = 31(42%) vs. 63(39%) respectively, NS). IHE patients were older and had more comorbidities (immunosuppression and ongoing malignancy). OHE patients had more severe organ failures at presentation with more frequent mechanical ventilation support. Independent global hospital mortality risk factors were ongoing malignancy (OR = 10.4 [2.7-40], p < 0.001), SAPS II (OR = 1.05 [1.03-1.08], p < 0.0001) and SOFA scores (OR = 1.14 [1.01-1.3], p < 0.05), hemorrhagic stroke as admission diagnosis (OR = 8.4 [2.7-26], p < 0.001), and arterial lactate on arrival (OR = 1.11 [1.03-1.2], p < 0.01). Conclusion: This study provides a thorough and comprehensive analysis of characteristics and outcomes of ICU admissions following a mature rapid response activation system, compared to the “conventional” out-of-hospital admission pathway. Despite the more vulnerable background of IHE patients, hospital mortality does not differ, supporting the use of early RRS to identify deteriorating ward patients. Take-home message: Hospital mortality does not differ between in-hospital emergencies admitted to intensive care unit and conventional out-of-hospital admissions, supporting the use of early rapid response systems and the importance of early intensive care unit admission.

Introduction : l’Arrêt Cardiaque (AC) est une pathologie fréquente. Lorsqu’un retour à la circulation spontanée est obtenu, un patient sur dix au moins va évoluer vers la Mort Encéphalique (ME). En France, 70% des prélèvements d’organes sont réalisés à partir de patients en ME. Une détection plus précise et précoce des potentielles évolutions vers la mort encéphalique pourrait aider les médecins à fournir des soins adaptés et anticiper la question du don d’organes. L’objectif de notre étude était d’identifier de potentiels facteurs prédictifs de passage en mort encéphalique à 24 heures après un arrêt cardiaque réanimé avec succès. Méthodologie : nous avons analysé les données de tous les patients admis entre 2014 et 2019, dans notre réanimation après un AC, ayant survécu au moins 24h, mais dont l’évolution fut fatale. Dans notre étude un EEG très malin est défini par une activité de fond absente sans activité paroxystique ou une absence d’activité de fond avec activité paroxystique continue ou une activité de fond en burst suppression avec ou sans activité paroxystique. Un scanner anormal est défini dans notre étude par la présence d’une hémorragie cérébrale ou d’une ischémie cérébrale ou d’œdème cérébral, quel que soit leurs localisations et leurs volumes. Une régression logistique a été réalisée afin d’identifier deux facteurs de risques indépendants. Résultats : 138 patients ont été inclus (âge médian 65 ans, sexe ratio 1.5, étiologie neurologique à l’arrêt cardiaque 13,8%, étiologie cardiaque 32,6%). 32 (23,2%) patients ont évolués vers la ME. Nous avons trouvé deux facteurs prédictifs indépendants de progression vers la mort encéphalique : un EEG très malin (OR 8,7 [2,1-35,8]) et un scanner cérébral anormal (OR 6,6 [1,5-30,2]). Conclusion : un EEG très malin et un scanner anormal réalisés à 24 heures après un arrêt cardiaque récupéré, sont deux facteurs de risques indépendants de progression vers la mort encéphalique. En plus des éléments anamnestiques, cliniques et biologiques, ces facteurs peuvent être utiles pour une détection précoce de l’évolution vers la mort encéphalique.
