Amaç: Yüksek kan basıncının karotis arterler üzerinde istenmeyen etkilere yol açtığı iyi bilinmesine ragmen bu etkinin hangi değerden itibaren başladığı net değildir. Bu çalışmanın amacı bunu değerlendirmektir. Yöntem: Çalışma kesitsel planlandı ve toplam 308 birey dahil edildi. Sistolik kan basıncı (SKB) 0.05) ancak grup 3 ile 1 arasında anlamlı fark vardı (p135,5 mmHg olmasının%78,2 sensitivite, %74,6 spesifite, DKB>86,5 mmHg olmasının %79,3 sensitivite ve %71,6 spesifite ile karotis distensibilitesini etkilemeye başladığı bulundu. Benzer olarak SKB>137,5 mmHg olmasının %80,4 sensitivite, %73,1 spesifite, DKB>88,5 mmHg olmasının ise %79,1 sensitivite ve %73,8spesifite ile karotis elastisitesini etkilemeye başladığı gösterildi. Sonuç: Karotis distensibilitesi ve elastisitesi kan basıncının artışı ile azalmaktadır. Yüksek-normal kan basıncına sahip bireylerde belirli bir kan basıncı değerinden sonra karotis hasarı başlıyor olabilir.

Amaç: Bu çalışmanın amacı supraaortik çoklu stenotik lezyonlarda yapılan, eş zamanlı endovasküler tedavinin başarısı ile komplikasyonlarını değerlendirmektir.Gereç ve Yöntem: 1 Ocak-2004 ile 31 Aralık 2009 tarihler arasında supraaortik damar yatağında çoklu darlık nedeni ile perkütan girişimsel işlem uygulanan hastaların kayıtları geriye dönük incelendi. Doppler ultrasonografi, manyetik rezonans inceleme, bilgisayarlı tomografi veya dijital subtraksiyon anjiyografi ile supraaortik damarlarda %50 nin üzerinde darlık saptanması koşuluyla perkütan endovasküler girişim uygulanması kararı alındı. Klinik olarak en az 2 tane ciddi darlığı olan ve aynı seansta çoklu girişimsel işlem kararı verilen hastalar çalışmaya dahil edildi.Bulgular: Çalışmaya 6’sı kadın (%17,4) toplam 34 hasta (ortalama yaş 60,4±12,5) dahil edildi. Otuzdört hastada toplam 74 lezyona girişim uygulandı. Girişimlerin tamamında stent kullanıldı. Mortalite veya ciddi morbidite görülmedi. İşlemin teknik olarak akut başarısı %100’dü. Ortalama 29,5±18,3 aylık takipte, 4 hastada (%11,7) restenoz geliştiği görüldü. Bu hastalardan 3’üne tekrar girişim uygulandı.Sonuç: Çalışmamızın sonuçlarına göre supraaortik dallarda tıkayıcı aterosklerotik damar hastalığı gelişmesi durumunda endovasküler girişim güvenle uygulanabilir. Bu tedavinin mortalitesive morbiditesi oldukça düşüktür. Takiplerinde restenoz gelişen hastalara tekrar girişim güvenle uygulanabilir. Aynı hasta grubunda çoklu lezyon var ise aynı seansta çoklu girişim güvenle uygulanabilen bir tedavi seçeneğidir., Purpose: The aim of this study was to evaluate the success and complications of endovascular treatment in supraaortic multiple stenotic lesionsMaterials and Methods: The study was planned as a retrospectively. The records of patients who underwent percutaneous interventional treatment for multiple stenosis in the supraaortic vessels between January 1, 2004 and December 31, 2009 were reviewed. Percutaneous endovascular intervention was decided with doppler ultrasonography, magnetic resonance imaging, computed tomography or digital subtraction angiography provided that more than 50% stenosis was detected in the supraaortic vessels. Patients with at least 2 severe stenosis who underwent multiple interventional procedures in one session were included in the study.Results: A total of 34 patients (mean age 60.4 ± 12.5 years and 6 female (17.4%)) were included in the study. A total of 74 lesions were treated in 34 patients. Stents were used in all procedures. No mortality or serious morbidity was observed during follow-up. The technical success of the procedure was 100%. Restenosis was observed in 4 patients (11.7%) at a mean follow-up of 29.5 ± 18.3 months. Three of these patients were re-treated.Conclusion: According to the results of our study, endovascular intervention can be safely performed in occlusive atherosclerotic vascular disease of the supraaortic branches. Mortality and morbidity of this treatment is very low. In patients with restenosis during follow-up, re-intervention can be performed safely. If multiple lesions are present in the same patient group, multiple interventions are a safe treatment option.

Amaç: Son dönem böbrek yetmezliği olan kronik böbrek hastalarında kardiyovasküler olayların sıklığının artığı bilinmektedir ancak böbrek yetmezliğinin karotis arterler üzerinde nasıl bir etki gösterdiği net değildir. Bu çalışmanın amacı bunu değerlendirmektir. Bu amaçla subklinik karotis aterosklerozu ve hasarını gösteren elastisite ve distensibilite son dönem böbrek yetmezliği olan hastalar ve sağlıklı populasyonda ölçülerek karşılaştırılmıştır.Gereç ve Yöntem: Çalışma kesitsel olarak planlandı. Son dönem böbrek yetmezliği olan 61 hasta ve 58 sağlıklı gönüllünün karotis elastisitesi ve distensibilitesi ölçülerek aralarında istatistiksel anmalı fark olup olmadığı değerlendirildi. Ayrıca hasta grubundaki bireylerin diyalize girme süreleri ile karotis elastisitesi ve distensibilitesi arasında anlamlı korelasyon olup olmadığı araştırıldı.Bulgular: Hasta grubundahem karotis elastisitesi hem de distensibilitesi istatistiksel anlamlı olarak daha düşüktü (0,12±0,02 vs. 0,23±0,04, p < 0,001, ve 7,47±0,51 vs. 9,42±0,73, p < 0,001, sırasıyla). Diyaliz süresi ile hem karotis elastisitesi hem de karotis distensibilitesi arasında istatistiksel anlamlı negatif yönlü korelasyon bulundu (r = -0,783, p < 0,001 ve r = -0,385, p = 0,002, sırasıyla).Sonuç: Son dönem böbrek yetmezliği olan hastalarda karotis elastisitesi ve distensibilitesi azalmıştır. Bu sonuçları bize bu hasta grubunda karotis arterlerde subklinik ateroskleroz riskinin arttığını düşündürebilir, ayrıca serebrovasküler olay riskindeki artışın sebebinin karotis arterlerdeki ateroskleroz olduğunu da indirekt olarak düşündürebilir.

Although it is well known that obesity increases the risk of atherosclerosis in carotid arteries, it is not clear whether this risk changes after obesity surgery. The aim of this study was to investigate whether weight reduction with a laparoscopic sleeve gastrectomy (LSG) has any effect on distensibility and elasticity which show subclinical atherosclerosis in the carotid arteries of obese individuals. This prospective study included 130 patients (body mass index (BMI) 48.21 ± 6.97 kg/m2) who underwent LSG. The patients were followed up for 1 year. Comparisons were made of the distensibiliy and elasticity values calculated preoperatively and at 1, 3, 6, and 12 months, postoperatively. ÖzAmaç: Bu çalışmanın amacı serebral venöz trombozlu (SVT) hastalarda radyolojik görüntüleme ile klinik şiddetin tahmin edilip edilemeyeceğini araştırmaktır. Bu amaçla SVT tanısı konulan hastaların manyetik rezonans görüntüleme (MRG) skoru ile Glasgow koma skorları (GKS) arasında ilişki olup olmadığı araştırılmıştır.Gereç ve Yöntem: Çalışma geriye dönük olarak planlandı. 1 Ocak 2013-31 Aralık 2019 tarihleri arasında SVT tanısı konulan hastaların kayıtları incelendi. Semptom başlangıcından sonraki ilk 5 gün akut, 5-15 gün subakut ve 15 gün sonrası kronik SVT olarak kabul edildi. Tüm hastaların MRG skoru ve GKS’leri kaydedildi ve aralarında istatistiksel anlamlı korelasyon olup olmadığı araştırıldı.Bulgular: Çalışmaya 47 akut, 26 subakut ve 19 kronik olmak üzere toplam 92 SVT’li hasta alındı. Akut gruptaki hastalarda median GKS 13 (çeyreklikler arası değişim (ÇAD)=3), subakut grupta 15 (ÇAD=3) ve kronik grupta ise 15 (ÇAD=1) olarak bulundu. Aim of the study: The aim of this study was to evaluate the long-term effects of access to the femoral artery for the purposes of coronary angiography through the measurement of femoral artery distensibility and elasticity on the accessed and non-accessed sides. Material and methods: This cross-sectional study included patients who underwent femoral angiography at least 1 year previously. Those whose femoral artery was accessed once formed Group 1 (n = 59), those who were accessed twice formed Group 2 (n = 57), those accessed 3 times formed Group 3 (n = 55), and those with more than 3 accesses, Group 4 (n = 60). The groups were compared in respect of femoral artery elasticity and distensibility in the accessed and non-accessed sides. Results: No statistically significant difference was determined in respect of femoral distensibility and elasticity in Group 1 (9.40 ± 0.84 vs 9.48 ± 0.75, p = 0.107 and 0.23 ± 0.03 vs 0.23 ± 0.03, p = 0.433, respectively). AIM To evaluate the relationship between trigeminal neuralgia (TN) and potential magnetic resonance imaging (MRI)-related measurements in patients with TN. MATERIAL AND METHODS Retrospective analysis of 104 patients with TN was performed. MRI studies of 98 healthy controls were included in the study to compare the parameters with TN patients’ measurements. MRI measurements of cerebellopontine cistern (CPC) cross-sectional area, trigeminal-pontine angle (TPA) width, and trigeminal nerve cisternal segment length and thickness were assessed on both symptomatic and asymptomatic sides using 1.5T MRI with constructive interference in steady-state sequences. The images were interpreted by two radiologists blinded to the affected sides of the patients. RESULTS There were significant differences between the symptomatic and asymptomatic sides in terms of mean trigeminal nerve length (8.8 ± 2.34 mm vs. 9.39 ± 2.29 mm; respectively, p=0.001) and thickness (20.9 ± 9.6 mm2 vs. 25 ± 9.98 mm2, respectively; p < 0.001). The median cerebellopontine cistern cross-sectional area was considerably lower on the symptomatic side compared with the asymptomatic side [201 mm2 (interquartile range=93) vs. 224.5 mm2 (interquartile range=77), respectively; p < 0.001]. There were no significant differences between the trigeminal-pontine angle width on either side (38.32 ± 10.38 vs. 38.78 ± 10.9, respectively; p=0.679). There were no statistically significant differences between the right and left sides regarding these parameters in the control group. CONCLUSION Smaller CPC cross-sectional area, trigeminal nerve length, and trigeminal nerve thickness on MRI were demonstrated to commonly exist on the symptomatic side in patients with TN. We suggest that this narrow space may increase the risk of vascular compression on the nerve.

Seckel syndrome and Moyamoya diseases are different entities that rarely associated with each other. Several cases presenting with both these diseases were reported. Intracerebral artery aneurysms and collateral vessels can be seen with Moyamoya. They are commonly treated with medical treatment. We present a 12-years old patient with both Seckel syndrome and Moyamoya disease presented with middle cerebral artery aneurysm which was treated with endovascular modalities.

Purpose: To establish the normal limits of portal vein diameter according to age, height and weight. Materials and methods: One hundred and sixty-eight healthy children, ages ranging from 1 month to 15 years were examined by ultrasound. Portal vein diameters at hepatic port, weight and height were recorded. Results: A range of normal limits for portal veneous diameter according to age, weight and height are obtained and presented in tables. Conclusion: Knowing the normals for portal venous diameter in every age group in children is mandatory in differentiating disease. The tables according to age, weight and height will definitely be helpful in the work-up process. © 2009 Elsevier Ireland Ltd.

PubMedID: 16243175 Purpose: The purposes of the study presented are to identify the microbial content of the collected bone debris and to determine the antibacterial efficiency of chlorhexidine mouth rinse in reducing the microbial content of the collected bone debris. Patients and Methods: Twenty-five patients who had asymptomatic fully impacted mandibular third molars bilaterally underwent surgical removal for prophylactic and orthodontic considerations. Immediately before surgery all patients rinsed with 10 mL of their assigned solution (chlorhexidine or sterile saline) for 2 minutes. Before surgical removal of the teeth, bone debris was collected with a stringent aspiration protocol from the ramus by bone filter. The microbial content of the bone debris was assessed and the bacterial levels between the 2 groups were compared statistically. Results: All samples from both groups (chlorhexidine or sterile saline) yielded viable microorganisms. There was no significant difference between the mean/median colony-forming units (CFU)/g values of both group samples, for aerobes (Streptococcus salivarius) and anaerobes (Bacteroides, Peptococcus, Peptostreptococcus, and Veillonella species). With regard to total microorganisms, the mean CFU/g derived from the chlorhexidine group samples were 1.5 × 10 8 CFU/g per bone sample compared with 1.5 × 10 9 CFU/g for the sterile saline control group (P < .05). Conclusion: Although chlorhexidine rinsing immediately before surgery reduced the levels of total microorganisms when compared with stringent aspiration protocol alone, it has not been found effective on aerobic Streptococcus salivarius and, importantly, on anaerobes. The reduced bacterial levels may still carry high infectious risk and may lead to failure in autogenous grafting procedures in oral surgery. © 2005 American Association of Oral and Maxillofacial Surgeons.
