Hepatic sinusoidal obstruction syndrome (SOS), also known as veno-occlusive disease (VOD) can be a life-threatening complication after hematopoietic stem cell transplantation (HSCT). Diagnosis is often difficult and traditionally based on clinical parameters. Shear wave elastography (SWE) is a modern non-invasive liver stiffness measurement technique using ultrasound. In this monocentric study, we evaluated the role of SWE in diagnosing SOS/VOD in 63 adult patients undergoing HSCT from February 2020 to August 2020 in real world settings. Three patients developed SOS/VOD. This was accompanied by an increase in shear wave velocity in all three patients, indicating that this method may contribute to establishing the diagnosis SOS/VOD after HSCT.

The recent advances in myeloma treatment result in significantly better outcomes, defined as increased progression free survival (PFS) and overall survival (OS). Since there is a proven correlation between the extend of response and prolonged survival, there is an urgent need for highly sensitive assays for the detection of minimal residual disease (MRD). Next generation flow cytometry has become a valuable approach for sensitive evaluation of the depth of complete response (CR). Here, we report the diagnostic performance and validation results of a single-tube 9-color panel assay. The validation design included intra-assay analysis measuring accuracy, inter-assay analysis estimating method’s linearity and precision and inter-assay analysis evaluating repeatability. Furthermore, in inter-operator analysis assessed the comparability of the result analysis of different operators. Staining stability was evaluated in age-of-stain experiments. Our validation results show that a reliable detection of residual myeloma cells is feasible to a detection level of 10−5 with a single-tube assay for a variety of materials (peripheral blood, bone marrow and stem cell apheresis). This study establishes highly sensitive, fully standardized approach for MRD detection in myeloma that is ready for implementation in routine diagnostic laboratories.

MDS remains a disease of elderly patients; moreover, the incidence of high risk MDS is proportionally greater in elderly patients, with increased frequency of secondary acute myeloid leukemia, as well as adverse cytogenetic abnormalities. Allogeneic stem cell transplantation is a therapeutic approach with known curative potential for patients with MDS that allows the achievement of long-term disease control. Numerous controversies still exist regarding transplantation in MDS: timing of transplantation, disease status at transplantation and comorbidity, conditioning intensity, pretransplant therapy, and stem cell source. Various transplant modalities of different intensities and alternative donor sources are now in use. Current advances in transplant technology are allowing the consideration of older patients. This should result in a greater number of older patients benefiting from this potentially curative treatment modality. Despite advances in transplantation technology, there is still considerable morbidity and mortality associated with this approach. Nevertheless, with the introduction of reduced-intensity conditioning and thereby reduced early mortality, transplant numbers in MDS patients have significantly increased. Moreover, recent new developments with innovative drugs, including hypomethylating agents, have extended the therapeutic alternatives for MDS patients. Hypomethylating agents allow the delay of allogeneic stem cell transplantation by serving as an effective and well-tolerated means to reduce disease burden. Background We investigated if bortezomib during induction and maintenance improves survival in newly diagnosed Multiple Myeloma (MM).\r\n\r\nMethods 827 eligible patients with newly diagnosed symptomatic MM were randomized to receive induction therapy with VAD (vincristine, doxorubicin, dexamethasone; n=414) or PAD (bortezomib, doxorubicin, dexamethasone; n=413) followed by high-dose melphalan (HDM) and autologous stem cell transplant (ASCT). Maintenance consisted of daily thalidomide 50 mg (VAD) or 2-weekly bortezomib 1.3 mg/m2 i.v. (PAD) for 2 years. The primary analysis was progression-free survival (PFS) adjusted for ISS stage. We here report long-term results of this trial ( P. Sonneveld et al., J Clin Oncol 2012;30:2946-2955 ).\r\n\r\nResults The number of eligible patients, patient characteristics and disease characteristics are similar to those reported before. The response rates during protocol treatment have improved slightly since all patients have now completed treatment: CR+nCR 49% vs 35%, VGPR 26% vs 21% and ≥PR 91% vs 83% (PAD vs VAD).\r\n\r\nAfter a median follow-up of 67 months, 111 of patients treated with VAD and 131 of patients treated with PAD were progression-free and alive. Progression-free survival (PFS) defined as time from randomization until progression, relapse or death (censored at date of alloSCT, if applicable), was superior with PAD when adjusted for ISS, (HR=0.78, 95% CI [0.66-0.91], P=.002) and in multivariate analysis (HR=0.76 (95% CI [0.64-0.90], P=.001). For the secondary endpoint overall survival (OS) the PAD arm was superior when adjusted for ISS (HR=0.80, 95% CI [0.65-1.00], P=.047) as well as in multivariate analysis (HR=0.78, 95% CI [0.63-0.97], P=.027). Landmark analysis from start of maintenance for PFS did not show a significant difference between Thalidomide and Bortezomib maintenance, however, for OS the PAD arm was superior (P=.035) (HR=0.71, 95% CI [0.52-0.98]). Habilitationschrift, Die allogene Transplantation von hämatopoetischen Stammzellen ist eine etablierte Behandlungsmodalität bei malignen und einigen nicht malignen hämatologischen Erkrankungen. In der vorliegenden Arbeit wurden die Ergebnisse von 107 allogenen Transplantationen am Campus Benjamin Franklin der Charité analysiert und in zwei Gruppen gegenübergestellt: 55 Patienten erhielten eine Transplantation nach konventioneller Konditionierung und 52 Patienten eine Transplantation nach Konditionierung mit reduzierter Intensität. Diese Therapiemodalität wurde erfolgreich in der Klinik für Hämatologie, Onkologie und Transfusionsmedizin eingeführt und ist jetzt Behandlungsoption für einen Teil der Patienten. In der Analyse wurde gezeigt, dass trotz signifikanter Altersunterschiede beider Gruppen (53 Jahre mittleres Alter nach RIC und 40 Jahre mittleres Alter nach konventioneller Konditionierung), die Therapieergebnisse hinsichtlich des Gesamtüberlebens gleich sind. Zwar treten in der Gruppe nach RIC häufiger Rezidive auf, jedoch sind die Komplikationen, die durch die toxischen Nebenwirkungen der Konditionierung verursacht sind, weniger ausgeprägt. Hinsichtlich akuter und chronischer GvHD und dem Auftreten infektiöser Komplikationen gab es keine Differenzen. Die Wiederhestellung der Hämatopoese hinsichtlich leukozytärer und thrombozytärer Regeneration nach Transplantation läuft gleich schnell. Die seriellen Untersuchungen zum hämatopoetischen Chimärismus an den Tagen 30, 60, 100 und später charakterisierten die Transplantatfunktion und gaben Hinweise auf eine Persistenz bzw. ein Wiederauftreten des maligenen Zellklons des Patienten. Die Ergebnisse der Chimärismusanalyse der Knochenmarkzellen nach Transplantation hämatopoetischer Stammzellen hatten somit einen prädiktiven Wert für den einzelnen Patienten. Sie erlauben eine Vorhersage von Rezidivwahrscheinlichkeit und Gesamtüberleben. In der Multivarianzanalyse ist der komplette Spender-Chimärismus am Tag 60 der stärkste Faktor hinsichtlich eines verbesserten Gesamtüberlebens sowohl in den Gruppen der Patienten nach RIC als auch nach konventioneller Konditionierung. Ein Rezidiv bei Patienten mit einem gemischten Chimärismus an Tag 60 nach Transplantation tritt signifikant häufiger auf als bei Patienten mit komplettem Chimärismus an Tag 60. Die Ergebnisse der Chimärismus-Analyse an Tag 100 korrelierten analog zu den Ergebnissen an Tag 60 mit Rezidivwahrscheinlichkeit und Überleben. Ein früher kompletter Chimärismus an Tag 30 korreliert nicht mit der Häufigkeit eines Rezidivs der Grundkrankheit oder mit der Überlebenswahrscheinlichkeit der Patienten, ebenso nicht ein gemischter Chimärismus an Tag 30. Damit ist die Chimarismusanalyse an Tag 30 ohne prediktiven Wert für den weiteren Verlauf nach der Transplantion. Ein früher Spender-Chimärismus ist aber bedeutend für die Beurteilung der Transplantatfunktion bei fehlendem klinischen Engraftment. Nach den erhobenen Daten ist der Chimärismus zwei Monate nach Transplantation von entscheidender prognostischer Bedeutung und damit für Therapieentscheidungen heranzuziehen. Differenziert werden kann mittels Analyse der minimalen Resterkrankung (MRD) zwischen einer Persistenz und einem Rezidiv der Grundkrankheit oder einer Regeneration der normalen Empfängerhämatopoese. Ohne Chimärismusanalyse ist das Engraftment nach Transplantation mit RIC nicht beurteilbar. Die Wahl der Therapie beinhaltet die Entscheidung zur Induktion einer Graft-versus-leukemia-Reaktion mittels Reduktion der immunsuppressiven Therapie, einer Immunstimulation und der DLI oder die Entscheidung für eine Tumortherapie. Es erwies sich, dass auch Patienten höheren Alters, die sich einer konventionellen komplexen Polychemotherapie eines hämatologischen Malignoms unterziehen, mit einer allogenen Transplantation nach RIC geheilt werden können. Eine Altersgrenze kann somit nicht mehr empfohlen werden. Da Patienten mit einem Alter über 50 Jahre nur in Ausnahmen eine konventionelle Konditionierung mit anschließender Transplantation allogener Stammzellen tolerieren, ist für sie ist eine Konditionierung mit reduzierter Intensität die Therapie der Wahl. Gemäß dieser Analyse und der aktuellen Literatur wird die Transplantation nach Konditionierung mit reduzierter Intensität (RIC) nunmehr als Standardtherapieverfahren genutzt. Die Analyse des hämatopoetischen Chimärismus ist dabei eine wesentliche Voraussetzung für die rechtzeitige Prognostizierung eines Rezidivs der Grunderkrankung und folglich die Basis für einen dauerhaften Erfolg der Transplantation allogener Stammzellen und damit der Heilung der Patienten., Allogeneic stem cell transplantation (SCT) is a potentially curative treatment for various hematological disorders. Antimalignancy effects result from cytoreduction by the pretransplantation preparative regimen and the immune- mediated graft-versus-leukemia (GvL) effect. The latter effect plays the predominant role for a variety of hematological malignancies and was the basis for introducing reduced-intensity conditioning (RIC) regimens in the allogeneic SCT setting. This made the approach applicable for patients previously ineligible due to advanced age or comorbidity. It has been suggested that, compared to conventional conditioning, RIC might be associated with delayed engraftment and mixed donor hematopoietic chimerism. Mixed chimerism was found to result in a higher relapse risk than complete chimerism in patients with severe aplastic anemia and chronic myeloid leukemia. However, the prognostic value of the degree of chimerism remains controversial in patients with acute leukemia. The nucleoside analogue fludarabine was established in RIC protocols in 1997. Its favorable toxicity profile and immunosuppressive effects, particularly in relation to T-cell activity, makes the substance suitable as part of a RIC regimen prior to allogeneic SCT. The alkylating agent treosulfan offers due to its low non-hematological toxicity a promising alternative to conventional conditioning agents in the autologous and allogeneic SCT setting. Therefore, this conditioning regimen might be considered as “reduced-toxicity” and discriminated from truly “reduced- intensity” regimens. Despite previous evidence substantiating the feasibility of conditioning with fludarabine and treosulfan, limited data are available on engraftment kinetics and the significance of hematopoietic chimerism in terms of relapse, acute graft-versus-host disease (GvHD) and survival. Multiple myeloma (MM) is a B-cell malignancy characterized by accumulation of malignant plasma cells (PC) within the bone marrow. The bone marrow microenvironment such as bone marrow stroma cells (BMSC) supports MM disease progression, resistance to chemotherapy, protects the tumor cells against apoptosis and causes osteolytic bone disease and angiogenesis. The aim of this study was to identify constitutive genetic alterations in BMSC derived from patients with MM (MM-BMSC) in comparison to BMSC from healthy donors. For BMSC selection, mononuclear cells were isolated from fresh bone marrow aspirates and were further expanded in cell culture. We studied 25 MM patients and 5 healthy donors. Senescence status was determined in passage 1 of cell cultures. MM-BSMC displayed a considerably higher percentage of senescence cells (p A downregulation of CCNE1 (p=0,05), CDKN1B (p=0,29), and an upregulation of CCND1 (p=0,05), VCAM-1 (p=0,33), ICAM-1 (p=0,33), and IKK-alpha (p=0,05) was demonstrated. Furthermore, the expression profile of miRNAs, targeting the analyzed mRNA genes or correlating with senescence, was studied (miR-16, miR-221, miR-126, miR-223, miR-485-5p and miR-519d). For miRNA detection treatment with Poly(A)-Polymerase and cDNA-Synthesis with a Poly(T)VN-Adaptor primer were carried out following an amplification with an universal reverse primer corresponding to the adaptor sequence and a miRNA-specific primer. miR-16, miR-223, miR-485-5p and miR-519d were significantly upregulated, (p=0,02; p=0,004; p=0,02; and p=0,002, respectively), whereas miR-221 and miR-126 showed no considerable differences to BMSC obtained from healthy donors. Next we investigated incubation of immunmodulatory drug Lenalidomid in BMSC cultures. Cells were treated with 10 µM Lenalidomid over 72 hours and expression was normalized to a 0,01 % DMSO control. Significant difference in gene expression were visible for ICAM-1 (p=0,01). For CDKN1B (p=0,16) and VCAM-1 (p=0,12) we demonstrated changes in gene expression. Our results indicate aberrant expression of cell cycle and adhesion-related genes, such as CCNE1, CCND1 and CDKN1B VCAM-1, ICAM-1 and IKK-alpha in MM-BMSC as compared with healthy donors. Furthermore, we found significant upregulation of miR-16, miR-223, miR-485-5p and miR-519d. Further investigation are needed to determine molecular mechanisms in MM-BMSC and PC interaction that lead to constitutive changes in BMSC characteristics and gene expression patterns. Multiple myeloma (MM) is a B-cell malignancy characterized by accumu- lation of malignant plasma cells (PC) within the bone marrow. Bone marrow mesenchymal stromal cells (BMMSCs) represent a crucial component of multiple myeloma (MM) microenvironment supporting its progression and proliferation. Alterations in bone marrow mesenchymal stromal cells of multiple myeloma patients (MM-BMMSCs) have become an important research focus. However, the role of alterations of MM-BMMSCs in the pathophysiology of MM is not clear. In this study, aberrations in MM-BMMSCs and their modification through interaction with MM cells should be analysed. MM-BMMSCs displayed a senescence-like state that was accompanied by an increased senescence associated ß-galactosidase activity (SAßGalA), a reduced number of colony-forming units, an accumulation of cells in S phase of the cell cycle and the overexpression of different microRNAs (miR-16, miR-223, miR-485-5p, miR-519d) and p21. MM-BMMSCs showed reduced expression of mitochondrial stress response protein SIRT3 and an increased mitochondrial DNA mass that was associated with a higher amount of reactive oxygen species compared to healthy donor BMMSCs (HD-BMMSCs). The overexpressed microRNAs miR-485-5p and miR-519d are located on DLK1-DIO3 and C19MC, respectively. Analyses revealed copy number accumulation and hypomethylation of both clusters. In vitro interaction with MM cells decreased the SAßGalA of MM-BMMSCs in correlation with the reduction of p21 and cells in S phase of the cell cycle. MiR-485-5p was significantly decreased in co-cultured MM-BMMSCs in connection with an increased methylation of DLK1-DIO3. Modification of miR-485-5p levels using microRNA mimic or inhibitor altered senescence and cell cycle characteristics of MM-BMMSCs. Cell contact to MM cells induced increased expression of SIRT3 associated with reduced levels of reactive oxygen species in MM-BMMSCs. Co-cultured MM-BMMSCs displayed an increased level of monocarboxylate transporter MCT1/CD147, whereas co-cultured MM cells showed a higher level of MCT4/CD147. In this context, interaction between MM-BMMSCs and MM cells led to the exchange of lactate, which was inhibited by treatment of co-cultures with MCT inhibitor alpha-cyano-4-hydroxycinnamic acid (alpha-CN). This induced apoptosis in MM cells and suppressed the increase of SIRT3 as well as the reinforced activation of NF-kappaB in MM-BMMSCs. Inhibition of SIRT3 in HD-BMMSCs using siRNA induced increased SAßGalA and a higher level of reactive oxygen species. Furthermore, knock-down of SIRT3 led to the accumulation of HD-BMMSCs in S phase of the cell cycle. MiR-223 expression in MM-BMMSCs was reduced by the presence of MM cells in vitro in a cell-contact dependent manner. Co-cultivation of MM cells and MM-BMMSCs induced activation of notch amongst others via jagged-2/notch-2 leading to increased expression of Hes1, Hey2 or Hes5 in both cell types. Cultivation of MM-BMMSCs with increasing levels of recombinant jagged-2 reduced miR-223 and increased Hes1 levels in a concentration-dependent manner. Transient reduction of miR-223 levels increased VEGF and IL-6 expression and secretion by MM-BMMSCs and impaired their osteogenic differentiation potential. Inhibition of notch signaling induced apoptosis in both MM cells and MM-BMMSCs. Furthermore, it increased miR-223 levels and reduced the expression of VEGF and IL-6 by both cell types. MM-BMMSCs show constitutive alterations in vitro resulting in a senescence-like state. MM cells modulate multiple pathways in senescence-like MM-BMMSCs leading to a better viability. SIRT3 and the DLK1-DIO3 cluster seem to influence the senescence-like state of MM-BMMSCs. Modulation of this phenotpye by MM cells can be reduced by treatment of co-cultures with MCT inhibitor alpha-CN. It is questionable whether the senescence-like state of MM-BMMSCs plays a pathological role in active MM or is more important for the promotion of a relapse. MiR-223 seems to participate in different MM supporting pathways in MM-BMMSCs including regulation of cytokine secretion as well as osteogenic differentiation. Further in vivo analysis regarding the role of the senescence-like state of MM-BMMSCs and of miR-223 for the support of MM could provide starting points for a more efficient anti-myeloma treatment. Das Multiple Myelom (MM) ist eine B-Zellneoplasie, welche sich durch die Akkumulation maligner Plasmazellen im Knochenmark klinisch manifestiert. Mesenchymale Knochenmarkstromazellen sind eine essentielle Komponente der Mikroumgebung des MM und können die Tumorproliferation und -progression fördern. Die Untersuchung von Alterationen in Knochenmarkstromazellen von MM-Patienten (MM-BMMSCs) stellt einen Schwerpunkt in der Erforschung des MM dar. Allerdings ist der Einfluss dieser Alterationen auf die Pathogenese des MM nicht vollständig geklärt. In dieser Arbeit sollten Aberrationen in MM-BMMSCs und deren Beeinflussung durch die Interaktion mit MM-Zellen untersucht werden. MM-BMMSCs zeigten einen seneszenzartigen Zustand, welcher sich durch die erhöhte Aktivität der Seneszenz-assoziierten ß-Galactosidase (SAßGal), die verringerte Anzahl von „colony-forming units“, die Akkumulation der Zellen in der S-Phase des Zellzyklus und die Überexpression verschiedener microRNAs (miR-16, miR-223, miR-485-5p, miR-519d) und p21 äußerte. Weiterhin war dieser Phänotyp von einer reduzierten Expression des mitochondrialen Stressproteins SIRT3, einer erhöhten Masse mitochondrialer DNA und einer erhöhten Menge an reaktiver Sauerstoffspezies im Vergleich zu BMMSCs von gesunden Spendern (HD-BMMSCs) begleitet. MiR-485-5p und miR-519d sind auf den zwei genomischen Clustern DLK1-DIO3 und C19MC lokalisiert. Analysen zeigten die Hypomethylierung und Kopieanzahlakkumulation von beiden Clustern in MM-BMMSCs. Die in vitro Interaktion mit MM-Zellen führte zur Erniedrigung der SAßGal-Aktivität, welche mit der reduzierten Expression von p21 und der Reduktion der Zellen in der S-Phase des Zellzyklus assoziiert war. MiR-485-5p war signifikant reduziert in co-kultivierten MM-BMMSCs in Verbindung mit einer gesteigerten Methylierung der regulatorischen Region von DLK1-DIO3. Die Modifizierung von miR-485-5p über microRNA Mimikry und Inhibitoren beeinflusste die Zellzykluseigenschaften und den seneszenzartigen Phänotyp von MM-BMMSCs. Des Weiteren verursachte der Zellkontakt zu MM-Zellen die erhöhte Expression von SIRT3 und die Reduktion der reaktiven Sauerstoffspezies in MM-BMMSCs. Zudem konnte in co-kultivierten MM-BMMSCs ein gesteigertes Level des Monocarboxylat-Transporters MCT1/CD147 detektiert werden, wogegen in co-kultivierten MM-Zellen MCT4/CD147 erhöht wurde. In diesem Zusammenhang zeigten MM-BMMSCs und MM-Zellen einen Austausch von Laktat, welcher durch die Behandlung von Co-Kulturen mit dem MCT-Inhibitor alpha-Cyano-4-hydroxyzimtsäure (alpha-CN) inhibiert wurde. Dies führte zur Apoptose der MM-Zellen, verhinderte die Steigerung von SIRT3 und die erhöhte Aktivierung des NF-kappaB Signalweges in MM-BMMSCs. Die Inhibierung der SIRT3 Expression in HD-BMMSCs mittels siRNA verursachte eine Steigerung der SAßGal-Aktivität, sowie des Gehalts an reaktiver Sauerstoffspezies, und induzierte die Akkumulation der HD-BMMSCs in der S-Phase des Zellzyklus. Zusätzlich zeigte sich bei Co-Kultivierung mit MM-Zellen eine zellkontakt- abhängige Reduktion von miR-223 in MM-BMMSCs. Die Co-Kultivierung verursachte die Aktivierung des Notch-Signalweges in beiden Zelltypen unter anderem über eine Jagged2-Notch2-Interaktion. Diese führte zur erhöhten Expression der Notch-Targetgene Hes1, Hey2 oder Hes5 in beiden Zellfraktionen. Die Inkubation von MM-BMMSCs mit rekombinantem Jagged2 verursachte die konzentrationsabhängige Steigerung von Hes1 und gleichzeitig die Reduktion von miR-223. Die transiente Inhibierung von miR-223 erhöhte die Sekretion und Expression von IL-6 und VEGF und verschlechterte das osteogene Differenzierungspotential von MM-BMMSCs. Die Inhibierung der Notch-Signalisierung verursachte Apoptose in MM-Zellen und MM-BMMSCs und reduzierte die Expression und Sekretion von VEGF und IL-6 beider Zelltypen. Gleichzeitig wurde der Gehalt von miR-223 in MM-BMMSCs gesteigert. MM-BMMSCs zeigen konstitutive Veränderungen in vitro, welche in einem seneszenzartigen Zustand resultieren. MM-Zellen modulieren verschiedene Signalwege in seneszenzartigen MM-BMMSCs, welche die Vitalität der Zellen zu verbessern scheinen. SIRT3 und der DLK1-DIO3 Cluster könnten einen Einfluss auf den seneszenzartigen Phänotyp von MM-BMMSCs haben. Die Beeinflussung dieses Phänotyps durch MM-Zellen kann durch den MCT-Inhibitor alpha-CN reduziert werden. Es ist fraglich ob dieser Phänotyp für die Pathogenese des aktiven MM von Bedeutung ist oder vielmehr ein Rezidiv des MM fördern könnte. MiR-223 scheint eine wichtige Rolle für multiple, tumorfördernde Signalwege in MM-BMMSCs zu spielen. Hierzu zählt die Modifikation der Zytokinsekretion und der osteogenen Differenzierung von MM-BMMSCs.Weiterführende in vivo Analysen bezüglich der Rolle des Seneszenz-artigen Phänotyps von MM-BMMSCs und von miR-223 für die Unterstützung des MM könnten neue Ansatzpunkte für eine effizientere Therapie des MM aufzeigen.
