International audience; Background: Differences in weight status by socioeconomic position occur during adolescence and is known as weight social gradient (WSG). The PRALIMAP (Promotion de l'ALIMentation et de l'Activité Physique) trial highlighted that a universal intervention was effective in reducing overweight and obesity prevalence among adolescents. However, its ability to reduce the WSG is unclear. This study aimed to evaluate the impact of the PRALIMAP intervention on the WSG among adolescents. Methods: Data were obtained from 3504 adolescents who participated in the school-based PRALIMAP trial between 2006 and 2009. The PRALIMAP study consisted of three interventional strategies: educational, environmental, screening and care. Weight status was assessed by body mass index (BMI) z-score and overweight and obesity prevalence. The "indice de position sociale des élèves" was used to measure adolescents' socioeconomic status. Linear regression and hierarchical models were used to assess the WSG and its evolution, respectively. International audience; Background: Despite social inequalities in overweight/obesity prevalence, evidence-based public health interventions to reduce them are scarce. The PRALIMAP-INÈS trial aimed to investigate whether a strengthened-care management for adolescents with low socioeconomic status has an equivalent effect in preventing and reducing overweight as a standard-care management for high socioeconomic status adolescents. Methods: PRALIMAP-INÈS was a mixed, prospective and multicenter trial including 35 state-run schools. It admitted overweight or obese adolescents, age 13-18 years old, for 3 consecutive academic years. One-year interventions were implemented. Data were collected before (T0), after (T1) and post (T2) intervention. Among 2113 eligible adolescents who completed questionnaires, 1639 were proposed for inclusion and 1419 were included (220 parental refusals). Two groups were constituted according to the Family Affluence Scale (FAS) score: the less advantaged (FAS≤5) were randomly assigned to 2 groups in a 2/1 ratio. The 3 intervention groups were: advantaged with standard-care management (A.S, n = 808), less advantaged with standard-care management (LA.S, n = 196), and less advantaged with standard and strengthened-care management (LA.S.S, n = 415). The standard-care management was based on the patient education principle and consisted of 5 collective sessions. The strengthened-care management was based on the proportionate universalism principle and consisted of activities adapted to needs. Inclusion results: The written parental refusal was less frequent among less advantaged and more overweight adolescents. A dramatic linear social gradient in overweight was evidenced. Discussion: The PRALIMAP-INÈS outcomes should inform how effectively a socially adapted public health program can avoid worsening social inequalities in overweight adolescents attending school. International audience; Background: There is an evidence of social inequalities in weight status in adolescence but the diversity of family socioeconomic status (SES) indicators can lead to discrepant findings. We aimed to identify how combination of family SES indicators can help measuring weight socioeconomic gradient (WSG) among adolescents.Methods: Cross-sectional data from 2113 adolescents (13-18 years old) of the PRALIMAP-INÈS trial were used. Multiple SES indicators and assessment of weight status including body mass index (BMI), waist circumference (WC), waist-to-height ratio (WHtR) and self-perception of overweight were used. We used principal component analysis (PCA) followed by structural equation models to identify SES dimensions. A dimension normalized score was calculated ranging from 1 to 10 (a high score corresponding to high SES). Linear regression models (linear trend test) were used to assess the WSG.Results: Three SES dimensions were identified: (i) 'Family social status', (ii) 'Family education level' and (iii) 'Family income level'. BMI was significantly lower in highly advantaged compared with highly less advantaged [-1.64 (-2.39; -0.89) for family social dimension, -0.86 (-1.37; -0.36) for family education level and -2.35 (-3.65; -1.05) for family income level]. Similar results were observed for all weight indicators excepted for self-perception of overweight status. Socially less advantaged adolescents perceived themselves less fat than they were.Conclusion: Although WSG was evident in adolescence, association between SES and weight status differed according to objective or perceived weight indicators. The proposed SES dimension can be applied in other field and future studies are needed to confirm our findings.

Background Although chronic kidney disease (CKD) and age are major risk factors for cardiovascular disease (CVD), little is known about the relative proportions of atheromatous and non-atheromatous CVD by age in CKD patients. Methods We used baseline data from the French Chronic Kidney Disease-Renal Epidemiology and Information Network (CKD-REIN) cohort of 3033 patients (65% men) with CKD Stages 3–4 to study crude and adjusted associations between age, the estimated glomerular filtration rate (eGFR), atheromatous CVD (coronary artery disease, peripheral artery disease and stroke) and non-atheromatous CVD (heart failure, cardiac arrhythmia and valvular heart disease). Results Mean age was 66.8 and mean Chronic Kidney Disease Epidemiology Collaboration (CKD-EPI) eGFR was 32.9 mL/min/1.73 m2. In the Conclusions In this large cohort of CKD patients, both atheromatous and non-atheromatous CVD were highly prevalent and more frequent in older patients. In a given age group, the prevalence of atheromatous and non-atheromatous CVD was similar (except for a greater prevalence of non-atheromatous CVD after 85).

International audience; BACKGROUND:The Epidemiology Network and Nephrology Information provides since 2001 the epidemiological monitoring of end stage renal disease (ESRD) in France. Two cross-sectional studies to estimate the level of quality of life in ESRD patients were performed in 2005 and 2007 and repeated in 2011. The main objective is to analyze the evolution of the quality of life of ESRD between these two surveys.METHODS:The studies were conducted on a representative sample of subjects aged 18 and over, prevalent dialysis or holders of a functioning kidney transplant last year, followed in center dialysis and/or transplantation in one of the eight French regions selected. The quality of life was measured by a generic questionnaire, and two specifics questionnaires.RESULTS:The evolution of the quality of life deteriorated slightly between 2005-2007 and 2011. There is however few dimensions affected. The variation in scores is not meaningful in general, with maximum decreases less than 4 points according to the statistical methods used. This diminution is not clinically significant.DISCUSSION:The quality of life in dialysis patients or transplant did not evolve between 2005-2007 and 2011. La qualité de vie (QDV) a été mesurée par un questionnaire générique, le MOS SF-36 et deux questionnaires spécifiques : le Kidney Disease Quality of Life (KDQOL) version courte pour les dialysés et le ReTransQol V2 (RTQ V2) pour les patients greffés. L’évolution des scores a été réalisée par des analyses multivariées, le calcul d’un score de propension et de pondération afin de prendre en compte les différences de profil des patients entre les périodes d’enquêtes.RésultatsLa QDV s’est légèrement dégradée entre 2005–2007 et 2011, et ce, quel que soit le questionnaire utilisé (générique ou spécifique). Il n’y a cependant que peu de dimensions affectées, avec 2 ou 3 domaines par outils de mesure. Pour le SF-36, ce sont les dimensions « santé générale », « douleurs physiques » ainsi que « vie et relation avec les autres » qui diminuent de manière significative (−3,2, −3,7 et −3,8 points). Pour les questionnaires spécifiques, la dimension « traitement » du RTQ V2 est la plus affectée (−2,5 points), ainsi que la dimension « symptôme/problèmes » du KDQOL (−2,4 points). La variation des scores de QDV est peu signifiante dans l’ensemble, allant de −1,6 point à −3,8 points pour la plus forte baisse, selon les méthodes statistiques utilisées. Cette diminution reste toujours inférieure au seuil des 5 points considérés comme cliniquement signifiant.DiscussionLa qualité de vie chez les patients dialysés ou greffés s’est légèrement dégradée entre 2005–2007 et 2011. Le recueil des dernières données de notre étude ayant été réalisé dès la fin du plan pour l’amélioration de la qualité de vie des personnes atteintes de maladies chroniques 2005–2011, le bénéfice du plan paraît donc difficile à évaluer dans sa globalité.

International audience; Objective –To estimate the quality of life (QOL) of patients with end-stage renal disease (dialysis or transplant).Methods –Cross-sectional survey by self-administered questionnaire conducted in 2011 in patients over 18 years old followed in one of the 21 regions participating in the network REIN (Renal Epidemiology and Information Network) in 2009. Patients were selected randomly after stratification on their age and region. QOL was measured using the MOS SF-36 generic questionnaire and specific questionnaires, the KDQOL for dialysis patients and the RETRANSQOL for transplanted patients.Results –1,251 dialysis patients and 1,658 transplanted patients participated in the survey. QOL scores were higher (from 10 to 30.6 points) in transplant patients than in dialysis patients, and in all dimensions of the MOS SF36, with regional differences. In transplanted patients, these scores tended to be closer to those of the general population. Dialysis patients registered on the waiting list for transplantation had a better QOL than dialysis patients not enrolled for all dimensions, with differences ranging from 3.9 (general vitality dimension) to 7.5 (physical functioning). Similarly, preemptive transplanted patients reported better QOL than non-preemptive patients, with differences ranging from 4.2 points (mental health) to 8.5 (limitations due to physical condition).Conclusion –The results of these studies on QOL give clinicians arguments in favor of transplantation. Clinicians can inform their patients that when transplantation is feasible and when associated pathologies are comparable, life after transplantation is of better quality, mainly because of an increase in their physical abilities, a reduction of physical limitations and an improvement of their social relations.; Objectif –Estimer le niveau de qualité de vie (QDV) des patients atteints d’insuffisance rénale chronique terminale (dialysés ou greffés).Méthodes –Enquête transversale par auto-questionnaire menée en 2011 auprès de patients de plus de 18 ans suivis dans l’une des 21 régions participant au Réseau épidémiologie et information en néphrologie (REIN) en 2009. Les patients ont été tirés au sort après stratification sur l’âge et la région. La QDV a été mesurée avec le questionnaire générique MOS SF36, et des questionnaires spécifiques, le KDQoL pour les patients dialysés et le ReTransQoL pour les greffés.Résultats –1 251 patients dialysés et 1 658 patients transplantés ont participé à l’enquête. Les scores de QDV étaient plus élevés (de 10 à 30,6 points) chez les greffés que chez les dialysés, et ce sur toutes les dimensions du MOS SF36, avec des différences régionales. Chez les greffés, ces scores tendaient à se rapprocher de ceux de la population générale. Les dialysés inscrits sur liste d’attente de greffe présentaient une meilleure QDV que les dialysés non inscrits sur toutes les dimensions, avec des différences variant de 3,9 (dimension vitalité générale) à 7,5 (fonctionnement physique). De même, les greffés préemptifs déclaraient une meilleure QDV que les non préemptifs, avec des différences variant de 4,2 points (santé mentale) à 8,5 (limitations dues à l’état physique).Conclusion –Les résultats de ces études de QDV donnent des arguments en faveur de la greffe aux cliniciens. International audience; The AVENIR study is a pharmaco-epidemiological study, lead in Lorraine region (France) between 1st January, 2005 and 31st December, 2006, which aim at: evaluating the quality of therapeutic practices, delivered by nephrologists, for chronic kidney disease patients during the year preceding dialysis onset, assessing the association between quality of predialysis therapeutic practices and survival and hospitalization during the first year of dialysis, and health-related quality of life at dialysis onset. Several data were collected for the AVENIR study: demographic, clinical, biological and therapeutic data before dialysis, morbidity and mortality during dialysis treatment. These data were used for secondary analyses investigating the decline in glomerular filtration rate over the year preceding dialysis, the management of hypertension and proteinuria before dialysis, and characteristics and outcomes of patients with delayed dialysis initiation. Results from the AVENIR study have been published in various international journals. The aim of this manuscript is to present a summary of these results and the lessons we can learn for the nephrological practice.; L’étude AVENIR est une étude de pharmaco-épidémiologie, conduite en 2005 et 2006 en région Lorraine, et dont les objectifs principaux étaient : d’évaluer la qualité des pratiques néphrologiques de prise en charge médicamenteuse chez les insuffisants rénaux chroniques au cours de l’année précédant le démarrage de la dialyse, de mesurer l’impact de la qualité de ces pratiques sur la morbimortalité pendant la première année de dialyse et la qualité de vie à l’initiation de la suppléance rénale. De nombreuses données démographiques, cliniques, biologiques et thérapeutiques avant dialyse et de morbidité et mortalité pendant la dialyse, ont été recueillies dans le cadre de cette étude. Ces données ont permis de réaliser des analyses répondant à des objectifs secondaires de l’étude ; ces objectifs portaient sur déclin au cours du temps du débit de filtration glomérulaire dans l’année précédant la suppléance rénale, la prise en charge de l’hypertension artérielle (HTA) et de la protéinurie avant dialyse, le retard à l’initiation de la dialyse et ses conséquences sur la morbidité et la mortalité. Les résultats, correspondant aux objectifs principaux et secondaires de l’étude AVENIR, ont été publiés dans différentes revues internationales. L’objectif de cet article est de présenter une synthèse de ses résultats et les leçons à en tirer pour la pratique néphrologique.

Background Some studies have investigated the association between body mass index (BMI) and health-related quality of life (HRQoL) among adolescents, but their results have been discrepant and few paid attention to the role of gender. The present investigation aimed to assess the relationship between weight status and HRQoL in adolescents and to verify whether it was similar in boys and girls. Methods Five thousand two hundred and twenty six adolescents aged 14 to 18 years were included in the PRomotion de l’ALIMentation et de l’Activité Physique (PRALIMAP) trial, a 2x2x2 factorial cluster randomized trial performed in 24 high schools in France. Sociodemographic, anthropometric and HRQoL data were collected. BMI was categorized in four classes (thin, normal-weight, overweight, obese). Linear regression models were used to estimate the association between weight status and HRQoL, adjusting for confounders. Results The mean age of adolescents was 15.7±0.6 years and their mean BMI was 21.6 ±3.5 kg/m2; 55% were girls. Boys were more often overweight and obese than were girls (overweight: 15.6% vs 14.2%, obese: 4.8% vs 3.3%), and girls were more likely to be thin (5.5% vs 4.5%, p=0.0042). All HRQoL scores were higher for boys (p=β=+6.17, p=0.0010), and overweight and obese girls had lower mental HRQoL (β=−3.89 and β=−5.90, respectively, p Conclusions Gender can modify the association between weight status and HRQoL in adolescents. Body image could be an important target of public health programs to improve subjective health during adolescence.

Background Although the outcomes of health promotion and prevention programmes may depend on the level of intervention, studies and trials often fail to take it into account. The objective of this work was to develop a framework within which to consider the implementation of interventions, and to propose a tool with which to measure the quantity and the quality of activities, whether planned or not, relevant to the intervention under investigation. The framework and the tool were applied to data from the diet and physical activity promotion PRALIMAP trial. Methods A framework allowing for calculation of an intervention dose in any health promotion programme was developed. A literature reviews revealed several relevant concepts that were considered in greater detail by a multidisciplinary working group. A method was devised with which to calculate the dose of intervention planned and that is actually received (programme-driven activities dose), as well as the amount of non-planned intervention (non-programme-driven activities dose). Results Indicators cover the roles of all those involved (supervisors, anchor personnel as receivers and providers, targets), in each intervention-related groups (IRG: basic setting in which a given intervention is planned by the programme and may differ in implementation level) and for every intervention period. All indicators are described according to two domains (delivery, participation) in two declensions (quantity and quality). Application to PRALIMAP data revealed important inter- and intra-IRG variability in intervention dose. Conclusions A literature analysis shows that the terminology in this area is not yet consolidated and that research is ongoing. The present work provides a methodological framework by specifying concepts, by defining new constructs and by developing multiple information synthesis methods which must be introduced from the programme's conception. Application to PRALIMAP underlined the feasibility of measuring the implementation level. The framework and the tool can be used in any complex programme evaluation. The intervention doses obtained could be particularly useful in comparative trials. Trial registration PRALIMAP is registered at ClinicalTrials.gov under NCT00814554

Introduction: Distance learning (e-learning) can facilitate access to training. Yet few public health E-learning experiments have been reported; institutes in developing countries experience difficulties in establishing on-line curricula, while developed countries struggle with adapting existing curricula to realities on the ground. In 2005, two schools of public health, one in France and one in Benin, began collaborating through contact sessions organised for Nancy University distance-learning students. This experience gave rise to a partnership aimed at developing training materials for e-Learning for African students. The distance-learning public health course at Nancy teaches public health professionals through a module entitled “Health and Development.” The module is specifically tailored for professionals from developing countries. To promote student-teacher exchanges, clarify content and supervise dissertations, contact sessions are organized in centres proximate and accessible to African students. The Benin Institute’s main feature is residential team learning; distance-learning courses are currently being prepared. Outcome: The two collaborating institutions have developed a joint distance-learning module geared toward developing countries. The collaboration provides for the development, diffusion, and joint delivery of teaching modules featuring issues that are familiar to African staff, gives the French Institute credibility in assessing research work produced, and enables modules on specific African issues and approaches to be put online. Lessons learned: While E-learning is a viable educational option for public health professionals, periodic contact can be advantageous. Our analysis showed that the benefit of the collaboration between the two institutions is mutual; the French Institute extends its geographical, cultural and contextual reach and expands its pool of teaching staff. The Benin Institute benefits from the technical partnership and expertise, which allow it to offer distance learning for Africa-specific contexts and applications.

Background Investigations focusing and implementing on the impact of cancer on health-related quality of life (HRQoL) by the way of a mean comparison between cancer patients and subjects from the general population, are scarce and usually cross-sectional. Longitudinal application of HRQoL instruments to a general, initially healthy population allows for change to be assessed as an event occurs, rather than afterwards. The objective of the present study was to investigate the impact of new cancer on HRQoL. Methods The 36-item Short Form (SF-36) and 12-item General Health Questionnaire (GHQ-12) were applied to the French SU.VI.MAX cohort in 1996 and 1998. A controlled longitudinal study was used to determine the impact on HRQoL of newly diagnosed cancer: 84 patients with cancer that occurred between the 2 HRQoL measures were compared with 420 age- and sex-matched cancer-free controls. Results Initial HRQoL level was similar in the two groups. A new cancer had a particularly marked effect on the SF-36 Physical functioning, Role-physical and General health dimensions (more than 6.6-point difference in change in HRQoL evolution on a 0–100 scale). The Bodily pain and Vitality dimensions were less severely affected (difference in change varying from 4.4 to 6.3 points), and there was no effect on either the GHQ-12 score or the SF-36 Mental health, Role-emotional and Social functioning dimensions. Conclusions The negative impact of cancer on the lives of patients was assessed in terms of HRQoL. The aspects most likely to be affected were those with a physical component, and general health perceptions. These results can thus help quantify the impact of a new cancer on HRQoL evolution and potentially facilitate early intervention by identifying the most affected HRQoL domains.
