Abstract: BACKGROUND: The existing World Health Organization (WHO) pneumonia case management guidelines rely on clinical symptoms and signs for identifying, classifying, and treating pneumonia in children up to 5 years old. We aimed to collate an individual patient-level data set from large, high-quality pre-existing studies on pneumonia in children to identify a set of signs and symptoms with greater validity in the diagnosis, prognosis, and possible treatment of childhood pneumonia for the improvement of current pneumonia case management guidelines.METHODS: Using data from a published systematic review and expert knowledge, we identified studies meeting our eligibility criteria and invited investigators to share individual-level patient data. We collected data on demographic information, general medical history, and current illness episode, including history, clinical presentation, chest radiograph findings when available, treatment, and outcome. Data were gathered separately from hospital-based and community-based cases. We performed a narrative synthesis to describe the final data set.RESULTS: Forty-one separate data sets were included in the Pneumonia Research Partnership to Assess WHO Recommendations (PREPARE) database, 26 of which were hospital-based and 15 were community-based. The PREPARE database includes 285 839 children with pneumonia (244 323 in the hospital and 41 516 in the community), with detailed descriptions of clinical presentation, clinical progression, and outcome. Of 9185 pneumonia-related deaths, 6836 (74%) occurred in children CONCLUSIONS: This data set could identify an improved specific, sensitive set of criteria for diagnosing clinical pneumonia and help identify sick children in need of referral to a higher level of care or a change of therapy. Field studies could be designed based on insights from PREPARE analyses to validate a potential revised pneumonia algorithm. The PREPARE methodology can also act as a model for disease database assembly.

Abstract: Guidelines for doctors managing osteoporosis in the Asia-Pacific region vary widely. We compared 18 guidelines for similarities and differences in five key areas. We then used a structured consensus process to develop clinical standards of care for the diagnosis and management of osteoporosis and for improving the quality of care. PURPOSE: Minimum clinical standards for assessment and management of osteoporosis are needed in the Asia-Pacific (AP) region to inform clinical practice guidelines (CPGs) and to improve osteoporosis care. We present the framework of these clinical standards and describe its development. METHODS: We conducted a structured comparative analysis of existing CPGs in the AP region using a "5IQ" model (identification, investigation, information, intervention, integration, and quality). One-hundred data elements were extracted from each guideline. We then employed a four-round Delphi consensus process to structure the framework, identify key components of guidance, and develop clinical care standards. RESULTS: Eighteen guidelines were included. The 5IQ analysis demonstrated marked heterogeneity, notably in guidance on risk factors, the use of biochemical markers, self-care information for patients, indications for osteoporosis treatment, use of fracture risk assessment tools, and protocols for monitoring treatment. There was minimal guidance on long-term management plans or on strategies and systems for clinical quality improvement. Twenty-nine APCO members participated in the Delphi process, resulting in consensus on 16 clinical standards, with levels of attainment defined for those on identification and investigation of fragility fractures, vertebral fracture assessment, and inclusion of quality metrics in guidelines. CONCLUSION: The 5IQ analysis confirmed previous anecdotal observations of marked heterogeneity of osteoporosis clinical guidelines in the AP region. The Framework provides practical, clear, and feasible recommendations for osteoporosis care and can be adapted for use in other such vastly diverse regions. Implementation of the standards is expected to significantly lessen the global burden of osteoporosis.

Reasons for this include strict regulatory frameworks, limited financial resources, lack of appropriate training amongst the medical profession and fear of addiction in both general practitioners and the wider population. Consequently, the weak opioid tramadol has become the analgesic most frequently used in the region to treat moderate to severe pain.Methods: To obtain a clearer picture of the current role and clinical use of tramadol in Southeast Asia, pain specialists from 7 countries in the region were invited to participate in a survey, using a questionnaire to gather information about their individual use and experience of this analgesic.Results: Fifteen completed questionnaires were returned and the responses analyzed. Tramadol is used to manage acute and chronic pain caused by a wide range of conditions. Almost all the specialists treat moderate cancer pain with tramadol, and every one considers it to be significant or highly significant in the treatment of moderate to severe non-cancer pain. The reasons for choosing tramadol include efficacy, safety and tolerability, ready availability, reasonable cost, multiple formulations and patient compliance. Its safety profile makes tramadol particularly appropriate for use in elderly patients, outpatients, and for long-term treatment. The respondents strongly agreed that tighter regulation of tramadol would reduce its medical availability and adversely affect the quality of pain management. In some countries, there would no longer be any appropriate medication for cancer pain or the long-term treatment of chronic pain.Conclusions: In Southeast Asia, tramadol plays an important part in the pharmacological management of moderate to severe pain, and may be the only available treatment option. Abstract: BACKGROUND: Obesity is a chronic disease with serious health consequences, but weight loss is difficult to maintain through lifestyle intervention alone. Liraglutide, a glucagon-like peptide-1 analogue, has been shown to have potential benefit for weight management at a once-daily dose of 3.0 mg, injected subcutaneously. METHODS: We conducted a 56-week, double-blind trial involving 3731 patients who did not have type 2 diabetes and who had a body-mass index (BMI; the weight in kilograms divided by the square of the height in meters) of at least 30 or a BMI of at least 27 if they had treated or untreated dyslipidemia or hypertension. We randomly assigned patients in a 2:1 ratio to receive once-daily subcutaneous injections of liraglutide at a dose of 3.0 mg (2487 patients) or placebo (1244 patients); both groups received counseling on lifestyle modification. The coprimary end points were the change in body weight and the proportions of patients losing at least 5% and more than 10% of their initial body weight. RESULTS: At baseline, the mean (±SD) age of the patients was 45.1±12.0 years, the mean weight was 106.2±21.4 kg, and the mean BMI was 38.3±6.4; a total of 78.5% of the patients were women and 61.2% had prediabetes. At week 56, patients in the liraglutide group had lost a mean of 8.4±7.3 kg of body weight, and those in the placebo group had lost a mean of 2.8±6.5 kg (a difference of -5.6 kg; 95% confidence interval, -6.0 to -5.1; P

6. Observation of the radiative decay D*+-> D+gamma

Abstract: We have observed a signal for the decay D*+ → D+ γ at a significance of 4 standard deviations. From the measured branching ratio B(D*+ → D+ γ)/B(D*+ → D+ π0) = 0.055 ± 0.014 ± 0.010 we find B(D*+ → D+ γ) = 0.017 ± 0.004 ± 0.003, where the first uncertainty is statistical and the second is systematic. We also report the highest precision determination of the remaining D*+ branching fractions.

Abstract: We have searched for lepton flavor violating decays of the τ lepton with one or two KS 0 mesons in the final state. The data used in the search were collected with the CLEO II and II.V detectors at the Cornell Electron Storage Ring (CESR) and correspond to an integrated luminosity of 13.9 fb-1 at the Y (4S) resonance. No evidence for signals were found; therefore, we have set 90% confidence level (C.L.) upper limits on the branching fractions Β(τ- → e- K S 0)

We present a restrictive upper limit on the product of the two-photon partial width and the KsKs branching fraction, (Γγγ BKsks)fJ(2220). We use this limit to calculate a lower limit on the stickiness, which is a measure of the two-gluon coupling relative to the two-photon coupling. This limit on stickiness indicates that the fJ(2220) has substantial glueball content.

Abstract: Using data collected in the region of the Y(4S) resonance with the CLEO II detector operating at the Cornell Electron Storage Ring (CESR), we present the first observation of B mesons decaying into the charmed strange baryons Ξc0 and Ξc+. We find 79 ± 27 Ξc0 and 125 ± 28 Ξc+ candidates from B decays, leading to product branching fractions of ℬ(B̄ → Ξc0X)ℬ(Ξc0 → Ξ-π+ = (0.144 ± 0.048 ± 0.021) × 10-3 and ℬ(B̄ → Ξc+X)ℬ(ΞC+ → Ξ-π+π+) = (0.453 ± 0.096- 0.065+ 0.085) × 10-3.

Abstract: Using a data sample with an integrated luminosity of 3.9 fb collected in e e annihilation with the CLEO-II detector at the Cornell Electron Storage Ring, we have measured the branching ratios for the decay modes D → (η,η′)π and D →(η,η′)ρ relative to D → φπ . These decay modes are among the most common hadronic decays of the D , and can be related by factorization to the semileptonic decays D →(η,η′)ℓ ν . The results obtained are compared with previous CLEO results and with the branching ratios measured for the related semileptonic decays. We also report results on the Cabibbo-suppressed decays of the D to the same final states.

Abstract: We report the first observation of B → η′X transitions with high momentum η′ mesons. We observe 39.0 ± 11.6 B decay events with 2.0 < pη′ < 2.7 GeV/c, the high momentum region where background from b → c processes is suppressed. We discuss the physical interpretation of the signal, including the possibility that it is due to b → sg* transitions. Given that interpretation, we find B(B → η′Xs) = [6.2 ± 1.6(stat) ±1.3(syst)1.50.0(bkg)] × 10-4 for 2.0 < pη′ < 2.7 GeV/c.

Abstract: We have studied semileptonic B meson decays with a p-wave charm meson in the final state using 3.29 × 106 BB̄ events collected with the CLEO II detector at the Cornell Electron-Positron Storage Ring. We find a value for the exclusive semileptonic product branching fraction ℬ(B- → D10ℓ ν̄ℓ)ℬ(D10 → D*+π-) = (0.373 ± 0.085 ± 0.052 ± 0.024)% and an upper limit for ℬ(ℬ- → D2*0ℓ-ν̄ ℓ)ℬ(D2*0 → D*+π) < 0.16% (90% C.L.). Furthermore, we present the first measurement of the q2 spectrum for B- → D10ℓ-ν̄ℓ.

Abstract: We have observed a signal for the decay D*+ → D+ γ at a significance of 4 standard deviations. From the measured branching ratio B(D*+ → D+ γ)/B(D*+ → D+ π0) = 0.055 ± 0.014 ± 0.010 we find B(D*+ → D+ γ) = 0.017 ± 0.004 ± 0.003, where the first uncertainty is statistical and the second is systematic. We also report the highest precision determination of the remaining D*+ branching fractions.

Abstract: Using 3.1 fb-1 of data accumulated at the Y (4S) by the CLEO-II detector, corresponding to 3.3×106 BB̄ pairs, we have searched for the color-suppressed B hadronic decay processes B̄0→D0(D*0)X0, where X0 is a light neutral meson π0, ρ0, η, η′ or ω. The D*0 mesons are reconstructed in D*0→D0π0 and the D0 mesons in D0→K- π+,K- π+ π0 and K- π+ π+ π- decay modes. No obvious signal is observed. We set 90% C.L. upper limits on these modes, varying from 1.2×10-4 for B̄0→D0π0 to 1.9×10-3 for B̄0→D*0η′.

22. Two-Body B Meson Decays to η and η': Observation of B → η'K

Abstract: In a sample of 6.6×10^6 produced B mesons we have observed decays B→η′K, with branching fractions B(B^+→η′K^+) = (6.5_(-1.4)^(+1.5)±0.9)×10^(-5) and B(B^0→η′K^0) = (4.7_(-2.0)^(+2.7)±0.9)×10^(-5). We have searched with comparable sensitivity for 17 related decays to final states containing an η or η′ meson accompanied by a single particle or low-lying resonance. Our upper limits for these constrain theoretical interpretations of the B→η′K signal.

Abstract: We present a measurement of the absolute branching fraction for D^0→K^-π^+ using the reconstruction of the decay chain B̅ →D^(*+)Xℓ^-ν̅ , D^(*+)→D^0π^+ where only the lepton and the low-momentum pion from the D^(*+) are detected. With data collected by the CLEO II detector at the Cornell Electron Storage Ring, we have determined B(D^0→K^-π^+) = [3.81±0.15(stat)±0.16(syst)]%.

Abstract: Using data recorded by the CLEO-II detector at CESR, we report new measurements of the branching fractions for the decays of the charmed baryon Λ_c^+ into pK^-π^+π^0, pK^0, pK^0π^+π^-, and pK^0π^0, all measured relative to pK^-π^+. The relative branching fractions are 0.67±0.04±0.11,0.46±0.02±0.04,0.52±0.04±0.05, and 0.66±0.05±0.07, respectively.

Abstract: The decays Y(4S) → ̄BB, followed by B → D* π and D* → Dπ, permit reconstruction of all kinematic quantities that describe the sequence without reconstruction of the D. with reasonably low backgrounds. Using an integrated e+ e- luminosity of 3.1 fb-1 accumulated at the Y(4S) by the CLEO-II detector, we report measurements of B(̄0B → D*+ π-) = (2.81 ± 0.11 ± 0.21 ± 0.05) × 10-3 and B(B → D*0 π) = (4.34 ± 0.33 ± 0.34 ± 0.18) × 10-3.

Abstract: Using a data sample with an integrated luminosity of 3.9 fb-1 collected in e+e- annihilation with the CLEO-II detector at the Cornell Electron Storage Ring, we have measured the branching ratios for the decay modes Ds+→(η,η′)π+ and Ds+→(η,η′)ρ+ relative to Ds+→φ+. These decay modes are among the most common hadronic decays of the Ds+ , and can be related by factorization to the semileptonic decays Ds+ →(η,η′)ℓ+νl. The results obtained are compared with previous CLEO results and with the branching ratios measured for the related semileptonic decays. We also report results on the Cabibbo-suppressed decays of the D+ to the same final states. © 1998 The American Physical Society.

Abstract: In a sample of 6.6 × 106 produced B mesons we have observed decays B → η′K, with branching fractions B(B+ → η′K+) = (6.5-1.4+1.5 ± 0.9) × 10-5 and B(B0 → η′K0) = (4.7-2.0+2.7 ± 0.9) × 10-5. We have searched with comparable sensitivity for 17 related decays to final states containing an η or η′ meson accompanied by a single particle or low-lying resonance. Our upper limits for these constrain theoretical interpretations of the B → η′K signal.

Abstract: Using data taken with the CLEO II detector at the Cornell Electron Storage Ring, we present the first full angular analysis in the color-suppressed modes B0 → J/ψ K*0 and B+ → J/ψ K*+. This leads to a complete determination of the decay amplitudes of these modes including the longitudinal polarization TL/Γ = 0.52 ± 0.07 ± 0.04 and the P wave component |P|2 = 0.16 ± 0.08 ± 0.04. In addition, we update the branching fractions for B → J/ψ K and B → J/ψ K*.

Abstract: We use the CLEO detector at the Cornell e+ e- storage ring, CESR, to search for the two-photon production of the glueball candidate fJ(2220) in its decay to KsKs. We present a restrictive upper limit on the product of the two-photon partial width and the KsKs branching fraction, (Γγγ BKsks)fJ(2220). We use this limit to calculate a lower limit on the stickiness, which is a measure of the two-gluon coupling relative to the two-photon coupling. This limit on stickiness indicates that the fJ(2220) has substantial glueball content.

Abstract: Using data collected in the region of the Y(4S) resonance with the CLEO II detector operating at the Cornell Electron Storage Ring (CESR), we present the first observation of B mesons decaying into the charmed strange baryons Ξc0 and Ξc+. We find 79 ± 27 Ξc0 and 125 ± 28 Ξc+ candidates from B decays, leading to product branching fractions of ℬ(B̄ → Ξc0X)ℬ(Ξc0 → Ξ-π+ = (0.144 ± 0.048 ± 0.021) × 10-3 and ℬ(B̄ → Ξc+X)ℬ(ΞC+ → Ξ-π+π+) = (0.453 ± 0.096- 0.065+ 0.085) × 10-3.

Abstract: We have searched for the decay of the τ lepton into seven charged particles and zero ot one π0. The data used in the search were collected with the CLEO II detector at the Cornell Electron Storage Ring (CESR) and correspond to an integrated luminosity of 4.61 fb-1. No evidence for a signal is found. Assuming all the charged particles are pions, we set an upper limit on the branching fraction B(τ-→4π-3π+(π 0)vτ)

Abstract: We report new measurements of the differential and total branching ratios for inclusive B decay to D0, D+, and D*+ and the first measurement of the same quantities for inclusive B decay to D*0. Here B is the mixture of Bd and Ba from Y(4S) decay. Furthermore, since more than one charm particle (or antiparticle) of the same kind can be produced in B decay, here "inclusive B branching ratio" is used to mean the average number of charm particles and their antiparticles of a certain species produced in B decay. We obtain the following results (the first error is statistical, the second systematic of this analysis, the third propagated from other measurements): B(B→D0X) = (0.636±0.014±0.019±0.018), B(B→D+X) = (0.235±0.009±0.009 ±0.024), B(B→D*0X) = (0.247±0.012±0.018±0.018), B(B→D*+X) = (0.239±0.011±0.014±0.009). The following ratio of branching ratios is not affected by most of the systematic errors: B(B→D*0X)/B(B→D*+X) = (1.03±0.07±0.09±0.08). We also report the first measurement of the momentum-dependent D*0 polarization and a new measurement of the D*+ polarization in inclusive B decay. Using these measurements and other CLEO results and making some additional assumptions, we calculate the average number of c and c̄ quarks produced in B decay to be 〈nc〉= 1.10±0.05.

Abstract: Using the CLEO detector at the Cornell e+e- storage ring CESR we study the two-photon production of ΛΛ̄, making the first observation of γγ→ΛΛ̄. We present the cross section for γγ→ΛΛ̄ as a function of the γγ center of mass energy and compare it to that predicted by the quark-diquark model.

Abstract: Using e+e- annihilation data collected by the CLEO II detector at CESR, we have observed the decay Ds+ → ωπ+. This final state may be produced through the annihilation decay of the Ds+, or through final state interactions. We find a branching ratio of Γ(Ds+ → ωπ+)/Γ(Ds+ → ηπ+) = 0.16 ± 0.04 ± 0.03, where the first error is statistical and the second is systematic.

Abstract: We have searched for lepton flavor violating decays of the τ lepton with one or two KS 0 mesons in the final state. The data used in the search were collected with the CLEO II and II.V detectors at the Cornell Electron Storage Ring (CESR) and correspond to an integrated luminosity of 13.9 fb-1 at the Y (4S) resonance. No evidence for signals were found; therefore, we have set 90% confidence level (C.L.) upper limits on the branching fractions Β(τ- → e- K S 0)
