We investigated BSI in patients undergoing HSCT to provide an appropriate clinical anti-infection experience and improve the prognosis of recipients with BSI after HSCT.Methods: A total of 105 patients with BSI after HSCT at our center from January 2015 to June 2020 were included in this retrospective study. We analyzed the clinical and microbiological data, and the risk factors for mortality at 3 months after BSI.Results: Of the 1141 HSCT recipients, 105 (9.2%) patients presented with 122 episodes of BSI, of which we isolated 85 (65.9%) gram-negative bacteria, 32 (24.8%) gram-positive bacteria and 12 (9.3%) fungi. Multidrug-resistant bacteria (MDR) were more than 70% of all pathogens and carbapenem-resistant organisms (CRO) were 25.6%. There were 55 episodes of BSI in the pre-engraftment phase and 67 episodes in the post-engraftment phase. The mortality of post-engraftment BSI was significantly higher than that of pre-engraftment (56.7% vs 32.7%, p = 0.005). Through multivariate analysis, the independent risk factors for all-cause mortality at 3 months after BSI were higher levels of procalcitonin (PCT), failure to cover appropriate antibiotics timely, and CRO BSI in pre-engraftment period or multidrug-resistant gram-negative bacteria (MDRGNB) BSI in post-engraftment period.Conclusion: Although the incidence of BSI was lower after HSCT, MDR-dominated BSI had a high mortality rate. Rapid identification of infection or pathogens’ classification with various testing methods and the more sensible and timely antibiotic cover are critical to the outcome of BSI after HSCT.Keywords: bloodstream infection, hematopoietic stem cell transplantation, multidrug-resistant bacteria, all-cause mortality

Recent advances in end-to-end continuous-flow synthesis are rapidly expanding the capabilities of automated customized syntheses of small-molecule pharmacophores, resulting in considerable industrial and societal impacts; however, many hurdles persist that limit the number of sequential steps that can be achieved in such systems, including solvent and reagent incompatibility between individual steps, cumulated by-product formation, risk of clogging and mismatch of timescales between steps in a processing chain. To address these limitations, herein we report a strategy that merges solid-phase synthesis and continuous-flow operation, enabling push-button automated multistep syntheses of active pharmaceutical ingredients. We demonstrate our platform with a six-step synthesis of prexasertib in 65% isolated yield after 32 h of continuous execution. As there are no interactions between individual synthetic steps in the sequence, the established chemical recipe file was directly adopted or slightly modified for the synthesis of twenty-three prexasertib derivatives, enabling both automated early and late-stage diversification., Although strategies for the automated assembly of compounds of pharmaceutical relevance is a growing field of research, the synthesis of small-molecule pharmacophores remains a predominantly manual process. Now, an automated six-step synthesis of prexasertib is achieved by multistep solid-phase chemistry in a continuous-flow fashion using a chemical recipe file that enables automated scaffold modification through both early and late-stage diversification.

By using the methods such as literature review, expert interview, video analysis and mathematical statistics, this paper analyzes the application of VAR in 240 matches of 30 rounds of China League One in 2019 season. Through the analysis of factors such as the number of reviews, time of review, classification, accuracy rate and the influence on the teams’ results of VAR technology, it is found that the accuracy rate of key decisions is significantly improved, and VAR intervention boundaries are not so clear. The wrong judgment of goals and penalty kicks was corrected in time, and fouls involving red cards could receive effective penalties. VAR technology has no great impact on the flow of the competition. However, for the use of VAR technology in China League One, intervention boundaries are not accurately got hold of, VAR is not operated well, referees are not very meticulous and comprehensive in the VAR viewing process, and communication between VAR and referees is not standardized, which needs to be further improved.

In order to solve the problem of detecting the cone barrel targets on both sides of the unmanned formula racing car, the traditional method is to extract features based on artificial design and select appropriate classifiers for detection and identification. This method is not ideal for detecting small targets of cone buckets. It is difficult to meet the requirements in terms of accuracy and speed of detection, and there are very large limitations. A new method based on improved Faster R-CNN for cone bucket target detection is proposed. This method can effectively avoid the problem of relying on artificial design features and classifier selection, and the speed and accuracy of detection are significantly improved. Method The method is based on a deep convolutional neural network. First, we have to determine the target object to be detected: the cones with the same shape and size on both sides of the track but different colors can be divided into red, yellow and blue cones according to the color. Next, the data set of the cone bucket target is created, and the corresponding configuration file is generated according to the format of the VOC 2007 data set. The sample data in the training set is sent to the network to obtain a convolution feature map. In order to fully extract the characteristics of the sample data, the method uses a deep residual network as the feature extraction network. After obtaining the convolution feature map, it is sent to the Region Proposal Network (RPN) for generating anchor boxes. Distinguish whether the target object belongs to the foreground or the background and perform a rough position correction on the region proposals. In order to enhance the detection effect on the small target cone, the finer division of the anchor boxes in the RPN. The traditional image processing method is used to realize the Region of Interest pooling (ROI pooling), and the output region proposals is fixed to a unified scale. The region proposals are extracted by optimizing the non-maximum suppression (NMS) algorithm to reduce the missed detection rate of the adjacent cone bucket target. Finally, the Softmax classifier is used to judge the category to which the target object belongs and to use the bounding box regression to achieve accurate position correction. Result The network model of target detection was obtained by using four-step training on the self-made cone bucket dataset. We compare the original Faster R-CNN model with the improved Faster R-CNN model to test the detection effect under different weather and environment. The improved model improves the detection of mutual occlusion between small targets and adjacent targets. The average accuracy on the test set is as high as 88.1%, and the detection frame rate reaches 10FPS. The improved method for cone bucket target detection proposed in this paper avoids the problem that traditional target detection methods need to manually select features and time-consuming. There is an obvious improvement in the accuracy and speed of detection, and it has strong robustness. It provides a new method for detecting the target of cone bucket of formula racing car.

Highly selective cross-hydroalkenylation of endocyclic-1,3-dienes at the least substituted site with a-olefins was achieved by set of neutral NHC-Ni(II)H(OTf), and by cationic Ni(II) catalysts with novel NHC structure. Under the heteroatom assistance, skipped dienes were obtained in good yield by using as low as equal molar amount of substrate pairs and 2-5 mol% catalyst. A rare 4,3- product selectivity (i.e., H at C4 and alkenyl group at C3 of diene) was observed, which is different from known dimerizations of a-olefins with both acyclic Co and Fe systems. The substituent effects of NHC, 1,3-diene and a-olefins on chemo-, regio- and diastereo-selectivity were studied. High level of chirality transfer was observed by using chiral cyclohexadiene derivatives.

Background and Objectives In urology, potassium-titanyl-phosphate (KTP) laser is mainly used in the treatment of benign prostatic hyperplasia with a low rate of intraoperative and postoperative complications. A prospective, randomized study was undertaken to investigate the treatment outcomes of KTP laser vaporization for primary non-muscle-invasive bladder tumors (NMIBTs) as compared with conventional monopolar transurethral resection of bladder tumors (TURBT). Materials and Methods This study was designed as a prospective, randomized trial. After institutional review board approval, 229 consecutive patients with NMIBTs were randomized to 2 groups. Among them, 116 patients underwent KTP laser vaporization of a bladder tumor (laser group) and 113 patients underwent standard transurethral electroresection of the bladder tumors using monopolar loop electrode (TURBT group). According to the prognostic factors for recurrence, all patients were divided into low, intermediate or high risk subgroups. The clinical data were recorded and compared between the two groups. Results Eighty-nine patients in laser group and 94 in TURBT group were evaluable for the study end points. The preoperative characteristics of the patients were comparable in the two groups. There was no statistical difference in operation time between the two groups. Patients in the laser group had fewer perioperative complications and more patients needed bladder irrigation in the TURBT group. Compared with laser group, patients in the TURBT group had longer catheterization time and hospitalization duration. There were no statistical differences in the oncologic results in term of 2-year recurrence rates as compared between the two groups. Conclusions Our study demonstrated that using KTP laser, transurethral vaporization is an effective and safe treatment for the patients with primary NMIBT. The present study was aimed to examine whether icariin, a traditional Chinese medicine, could improve therapeutic effects of adipose derived mesenchymal stem cells (ADSCs) for diabetes-associated erectile dysfunction (DMED). DMED were induced in rats by intraperitoneal injection of streptozotocin and confirmed by erectile function measurement. Then, rats of diabetic ED were randomly divided to receive the treatment of saline, ADSCs, icariin or ADSCs combined with icariin respectively.
