A database is the data collection in a well-organized manner from which its content can easily be accessed, retrieved, managed and updated as per the requirements. A web module based biomedical database management system (BM-DBMS) is developed for Voluntary Health Service (VHS) Multispecialty hospital to assist them to manage the database on their own in real-time. The developed program is based on a data set of 348 medical equipment belonging to 15 departments in the hospital. The database of each medical device having a unique assigned asset number is included in the developed database management system. The web module is built on java platform. The user information runs on My structured query language (My SQL). The facility for the management of medical devices activities from initial log-in to maintenance services is provided. In this project, 3 login credentials namely admin, biomedical and nursing has been developed. The admin can add the asset names, equipment details, medical department, equipment serial number, warranty expiring date. Also, a dashboard to view the total asset, complaints raised, planned preventive maintenance (PPM) details are provided. The nursing staff can raise complaints and the biomedical engineer can view the complaint raised through their respective login.

© The Author(s) 2020. This article draws on experiments with techniques of visual documentation and representation in circumstances of topographic intensity in Hong Kong. Centrally, the authors ask: how can approaches to visual representation reveal or reframe assemblages of material and immaterial processes in hyper-dense and hyper-topographic urban landscapes? The shaping and reshaping of the ground are essential parts of the formation of recombinant ecosystems, especially in intensively built-up environments – the constant reconfiguration of materials and spaces continually brings into being new ecological conditions. However, the visualization and representation of these transformations are often absent from the repertoire of techniques used to map and understand urban processes. This article addresses how landscape architecture can continue to develop methods to record, represent, visualize and design recombinant ecosystems in our urban environments. It considers which tools and techniques might provide ‘thickened’ understandings of hyper-dense, small-scale and intensely topographic urban environments like Hong Kong, with the objective of assisting designers and planners develop new ways to understand and see public space that can address the human and more-than-human nuances of complex urban spaces over time.

Aims To investigate the efficacy and safety of early transition from hospital to ambulatory treatment in low-risk acute PE, using the oral factor Xa inhibitor rivaroxaban. Methods and results We conducted a prospective multicentre single-arm investigator initiated and academically sponsored management trial in patients with acute low-risk PE (EudraCT Identifier 2013-001657-28). Eligibility criteria included absence of (i) haemodynamic instability, (ii) right ventricular dysfunction or intracardiac thrombi, and (iii) serious comorbidities. Up to two nights of hospital stay were permitted. Rivaroxaban was given at the approved dose for PE for >_3 months. The primary outcome was symptomatic recurrent venous thromboembolism (VTE) or PE-related death within 3 months of enrolment. An interim analysis was planned after the first 525 patients, with prespecified early termination of the study if the null hypothesis could be rejected at the level of a = 0.004 (

Employing compact quantum sensors in field or in space (e.g., small satellites) implies demanding requirements on components and integration technologies. Within our work on integrated sensors, we develop miniaturized, ultra-stable optical setups for optical cooling and trapping of cold atomic gases. Besides challenging demands on alignment precision, and thermo-mechanical durability, we specifically address ultra-high vacuum (UHV) compatibility of our integration technologies and optical components. A prototype design of an UHV-compatible, crossed beam optical dipole trap setup and its application within a cold atomic quantum sensor is described. First qualification efforts on adhesive micro-integration technologies are presented. These tests are conducted in application-relevant geometries and material combinations common for micro-integrated optical setups. Adhesive aging will be investigated by thermal cycling or gamma radiation exposure. For vacuum compatibility testing, a versatile UHV testing system is currently being set up, enabling residual gas analysis and measurement of total gas rates down to 5•10-10mbar l/s at a base pressure of 10-11 mbar, exceeding the common ASTM E595 test.

- Abstract
IMPORTANCE Short-term infusions of single vasodilators, usually given in a fixed dose, have not improved outcomes in patients with acute heart failure (AHF). OBJECTIVE To evaluate the effect of a strategy that emphasized early intensive and sustained vasodilation using individualized up-titrated doses of established vasodilators in patients with AHF. DESIGN, SETTING, AND PARTICIPANTS Randomized, open-label blinded-end-point trial enrolling 788 patients hospitalized for AHF with dyspnea, increased plasma concentrations of natriuretic peptides, systolic blood pressure of at least 100mmHg, and plan for treatment in a general ward in 10 tertiary and secondary hospitals in Switzerland, Bulgaria, Germany, Brazil, and Spain. Enrollment began in December 2007 and follow-up was completed in February 2019. INTERVENTIONS Patients were randomized 1:1 to a strategy of early intensive and sustained vasodilation throughout the hospitalization (n = 386) or usual care (n = 402). Early intensive and sustained vasodilation was a comprehensive pragmatic approach of maximal and sustained vasodilation combining individualized doses of sublingual and transdermal nitrates, low-dose oral hydralazine for 48 hours, and rapid up-titration of angiotensin-converting enzyme inhibitors, angiotensin receptor blockers, or sacubitril-valsartan. MAIN OUTCOMES AND MEASURES The primary end pointwas a composite of all-cause mortality or rehospitalization for AHF at 180 days. RESULTS Among 788 patients randomized, 781 (99.1%; median age, 78 years; 36.9% women) completed the trial and were eligible for primary end point analysis. Follow-up at 180 days was completed for 779 patients (99.7%). The primary end point, a composite of all-cause mortality or rehospitalization for AHF at 180 days, occurred in 117 patients (30.6%) in the intervention group (including 55 deaths [14.4%]) and in 111 patients (27.8%) in the usual care group (including 61 deaths [15.3%]) (absolute difference for the primary end point, 2.8% [95% CI, -3.7% to 9.3%]; adjusted hazard ratio, 1.07 [95% CI, 0.83-1.39]; P =.59). The most common clinically significant adverse events with early intensive and sustained vasodilation vs usual care were hypokalemia (23% vs 25%), worsening renal function (21% vs 20%), headache (26% vs 10%), dizziness (15% vs 10%), and hypotension (8% vs 2%). CONCLUSIONS AND RELEVANCE Among patients with AHF, a strategy of early intensive and sustained vasodilation, compared with usual care, did not significantly improve a composite outcome of all-cause mortality and AHF rehospitalization at 180 days.

- Abstract
IMPORTANCE Short-term infusions of single vasodilators, usually given in a fixed dose, have not improved outcomes in patients with acute heart failure (AHF). OBJECTIVE To evaluate the effect of a strategy that emphasized early intensive and sustained vasodilation using individualized up-titrated doses of established vasodilators in patients with AHF. DESIGN, SETTING, AND PARTICIPANTS Randomized, open-label blinded-end-point trial enrolling 788 patients hospitalized for AHF with dyspnea, increased plasma concentrations of natriuretic peptides, systolic blood pressure of at least 100mmHg, and plan for treatment in a general ward in 10 tertiary and secondary hospitals in Switzerland, Bulgaria, Germany, Brazil, and Spain. Enrollment began in December 2007 and follow-up was completed in February 2019. INTERVENTIONS Patients were randomized 1:1 to a strategy of early intensive and sustained vasodilation throughout the hospitalization (n = 386) or usual care (n = 402). Early intensive and sustained vasodilation was a comprehensive pragmatic approach of maximal and sustained vasodilation combining individualized doses of sublingual and transdermal nitrates, low-dose oral hydralazine for 48 hours, and rapid up-titration of angiotensin-converting enzyme inhibitors, angiotensin receptor blockers, or sacubitril-valsartan. MAIN OUTCOMES AND MEASURES The primary end pointwas a composite of all-cause mortality or rehospitalization for AHF at 180 days. RESULTS Among 788 patients randomized, 781 (99.1%; median age, 78 years; 36.9% women) completed the trial and were eligible for primary end point analysis. Follow-up at 180 days was completed for 779 patients (99.7%). The primary end point, a composite of all-cause mortality or rehospitalization for AHF at 180 days, occurred in 117 patients (30.6%) in the intervention group (including 55 deaths [14.4%]) and in 111 patients (27.8%) in the usual care group (including 61 deaths [15.3%]) (absolute difference for the primary end point, 2.8% [95% CI, -3.7% to 9.3%]; adjusted hazard ratio, 1.07 [95% CI, 0.83-1.39]; P =.59). The most common clinically significant adverse events with early intensive and sustained vasodilation vs usual care were hypokalemia (23% vs 25%), worsening renal function (21% vs 20%), headache (26% vs 10%), dizziness (15% vs 10%), and hypotension (8% vs 2%). CONCLUSIONS AND RELEVANCE Among patients with AHF, a strategy of early intensive and sustained vasodilation, compared with usual care, did not significantly improve a composite outcome of all-cause mortality and AHF rehospitalization at 180 days.

10. Biographies of a reformation: religious change and confessional coexistence in Upper Lusatia, c. 1520-1635

- Abstract
This thesis investigates how religious coexistence functioned in the multiconfessional region of Upper Lusatia in Western Bohemia. Lutherans and Catholics found a feasible modus vivendi through written agreements and regular negotiations. This meant that the Habsburg king of Bohemia ruled over a Lutheran region. He knew of the situation in Lusatia but was not willing to intervene decisively to reintroduce Catholicism. Lutherans and Catholics in Upper Lusatia shared spaces, objects and rituals. Catholics adopted elements previously seen as a firm part of a Lutheran confessional culture. Lutherans, too, were willing to incorporate Catholic elements into their religiosity. Some of these overlaps were subconscious, while others were a conscious choice. Later generations of historians fashioned these complex processes of change into a neater Reformation narrative. This research provides a narrative of the Reformation in Upper Lusatia and engages with three historiographical paradigms. Firstly, the results show that the concept of the 'urban Reformation', where towns are seen as centres of Lutheranism has to be reassessed, particularly in towns in former East Germany, where much work remains to be done. Secondly, it shows that in a region like Upper Lusatia which did not have a political centre, undergoing a complex Reformation, there was no clear confessionalization process. As other studies have found similar tendencies elsewhere in the Holy Roman Empire, the usefulness of the confessionalization paradigm is increasingly questionable. Thirdly, the example of dissenting religious groups cautions us that it is not helpful to push the idea of toleration in early modern Europe too far. In the case of Upper Lusatia, the coexistence of two confessions, Lutherans and Catholics, meant that others were excluded.

- Abstract
In acute heart failure (AHF) syndromes significant respiratory failure (RF) is essentially seen in patients with acute cardiogenic pulmonary oedema (ACPE) or cardiogenic shock (CS). Non-invasive ventilation (NIV), the application of positive intrathoracic pressure through an interface, has shown to be useful in the treatment of moderate to severe RF in several scenarios. There are two main modalities of NIV: continuous positive airway pressure (CPAP) and pressure support ventilation (NIPSV) with positive end expiratory pressure. Appropriate equipment and experience is needed for NIPSV, whereas CPAP may be administered without a ventilator, not requiring special training. Both modalities have shown to be effective in ACPE, by a reduction of respiratory distress and the endotracheal intubation rate compared to conventional oxygen therapy, but the impact on mortality is less conclusive. Non-invasive ventilation is also indicated in patients with AHF associated to pulmonary disease and may be considered, after haemodynamic stabilization, in some patients with CS. There are no differences in the outcomes in the studies comparing both techniques, but CPAP is a simpler technique that may be preferred in low-equipped areas like the pre-hospital setting, while NIPSV may be preferable in patients with significant hypercapnia. The new modality â €high-flow nasal cannula' seems promising in cases of AHF with less severe RF. The correct selection of patients and interfaces, early application of the technique, the achievement of a good synchrony between patients and the ventilator avoiding excessive leakage, close monitoring, proactive management, and in some cases mild sedation, may warrant the success of the technique. © The Author 2017.

Little is known about treatments provided by advanced life support (ALS) ambulance teams to patients with acute heart failure (AHF) during the prehospital phase, and their influence on short-term outcome. We evaluated the effect of prehospital care in consecutive patients diagnosed with AHF in Spanish emergency departments (EDs). We selected patients from the EAHFE registry arriving at the ED by ALS ambulances with available follow-up data. We recorded specific prehospital ALS treatments (supplemental oxygen, diuretics, nitroglycerin, non-invasive ventilation) and patients were grouped according to whether they received low- (LIPHT; 0/1 treatments) or high-intensity prehospital therapy (HIPHT; > 1 treatment) for AHF. We also recorded 46 covariates. The primary endpoint was all-cause 7-day mortality, and secondary endpoints were prolonged hospitalisation (> 10 days) and in-hospital and 30-day mortality. Unadjusted and adjusted odds ratios were calculated to compare the groups. We included 1493 patients [mean age 80.7 (10) years; women 54.8%]. Prehospital treatment included supplemental oxygen in 71.2%, diuretics in 27.9%, nitroglycerin in 13.5%, and non-invasive ventilation in 5.3%. The LIPHT group included 1041 patients (70.0%) with an unadjusted OR for 7-day mortality of 1.770 (95% CI 1.115-2.811; p = 0.016), and 1.939 (95% CI 1.114-3.287, p = 0.014) after adjustment for 16 discordant covariables. The adjusted ORs for all secondary endpoints were always > 1 in the LIPHT group, but none reached statistical significance. Patients finally diagnosed with AHF at then ED that have received LIPHT by the ALS ambulance teams have a poorer short-term outcome, especially during the first 7 days.

13. The town chronicle of Johannes Hass: history writing and divine intervention in the early sixteenth century

The council annals of Johannes Hass, the last Catholic mayor of the West Bohemian town of Görlitz, are a fascinating document. In a single source we can see the author’s changing interpretation of divine intervention between c.1509 and 1542. After the introduction of the Reformation, Hass steadily decreased the importance of God, whilst the Devil became increasingly important. Regardless of Hass’s staunch Catholicism, Martin Luther’s impact can be felt in his vision of the divine and the demonic as he subconsciously incorporated minor elements of Lutheranism and commented positively on small changes brought about by the Reformation. Hass shows how receptive Catholics were towards Lutheran theology, without necessarily acknowledging it, as long as the new religion did not challenge the urban order. In rich and colourful language, Hass changed the very nature of God, the Devil and the saints. In this way he was responding to the slow but steady introduction of the Reformation in Görlitz. Only by gradually turning God into a passive observer and giving greater agency to human actors could Hass make sense of the absence of a clear sign in favour of the Catholics in Görlitz.

Material and methods: In-hospital cardiac arrest teams at our hospital were instructed to complete a questionnaire following every emergency call between July 2011 and June 2013. Data on all resuscitation attempts were collected and analyzed. Results: A total of 65 in-hospital resuscitations were recorded in 42 males (64.6%) and 23 females (35.4%) (mean age 72.0±14.3 years). A total of 54 (83.1%) cardiac arrests were witnessed; seven (10.8%) showed a shockable rhythm at the time of the first ECG. Resuscitation attempts lasted 29.3±41.3 minutes, and 4.1±3.1 mg epinephrine was given. Return of spontaneous circulation could be achieved in 38 patients (58.5%); 29 (44.6%) survived the first day, 23 (35.4%) the seventh day, and 15 patients (23.1%) were discharged alive. After intragluteal injection of a drug mixture in a 39-year-old woman, circumscribed necroses developed at the site of injection. This is a rare complication of intramuscular administration of certain drugs, most of them antirheumatic agents. A haemorrhagic dermatitis develops, which may progress to necrosis. Healing occurs after spontaneous or surgical (deep excision) exclusion of the necrotic area with atrophic scarring. The complication is so rare that there is no requirement for the doctor to inform the patient of this possibility before injection. Nor is there liability by the doctor or the nurse who undertake the injections.

We study the production of Sigma^+-pi^+-pK^+ particle quartets in p+p reactions at 3.5 GeV kinetic beam energy. The data were taken with the HADES experiment at GSI. This report evaluates the contribution of resonances like Lambda(1405$, Sigma(1385)^0, Lambda(1520), Delta(1232), N^* and K^*0 to the Sigma^+- pi^-+ p K+ final state. The resulting simulation model is compared to the experimental data in several angular distributions and it shows itself as suitable to evaluate the acceptance corrections properly., Comment: 15 pages, 5 figures

Exhaustive exercise causes PaCO2 elevation, intense acidosis, and a mobilization of catecholamines into the blood of trout. A radioisotopic assay (Wood and Perry, J. Exp. Biol. 157: 349-366, 1991) was used to examine changes in HCO3- dehydration rate in blood sampled by dorsal aortic catheter from exercised trout. The rate increased about 50% during and throughout the 30 min period following exercise when the blood was assayed immediately at the acid-base status prevailing in vivo. However when the blood from exercised fish was rapidly equilibrated to resting levels of PCO2 prior to assay, the rate was decreased by 15-35% relative to resting values. This effect was seen only in whole blood, not in plasma, indicating an inhibition at the level of the red cells. Conversely, when blood from resting trout was equilibrated to the elevated PaCO2 seen in vivo after exercise, the measured dehydration rate was greater than twice that in samples from exercised fish. Thus a relative inhibition of HCO3- dehydration through the red cells occurs after exercise, an effect likely associated with adrenergic activation of Na+/H+ exchange.

- Abstract
The solubility of CO2, CH4, N2, and CO was measured in a broad number of ionic liquids using a pressure drop set-up. A successful correlation of the Henry's law constants at 60°C was made to two empirical models already described in literature (Camper et al. [13] and Moganty and Baltus [25]). Subsequently, a new empirical model was proposed, which describes the Henry's law constant as a function of the molar mass of the ionic liquid, as long as physical interactions between gas and ionic liquid are dominant. Additionally, the entropy and enthalpy of solvation was studied by means of the temperature influence to the Henry's law constant.

Among their biological properties, several oxysterols display a stronger toxicity towards tumor cells than towards normal cells. Water-soluble phosphodiesters of 7 beta-hydroxycholesterol (JB69 and XA29), that were proved to retain a specific antitumoral activity in vitro, have been recently developed, allowing in vivo studies. They have been assayed on transgenic mice expressing the Large T antigen of SV40 in their liver and developing systematically hepatocarcinoma within 8 months. We show that JB69 and XA29 administered intraperitoneally into transgenic mice, before the onset of adenoma, may prevent or delay the tumor development. Consequently, oxysterol derivatives might constitute new efficient prodrugs against naturally occurring tumors.

Epidemiologic data is however unavailable. We aimed to provide an international picture of the extent of pressure injuries and factors associated with ICU-acquired pressure injuries in adult ICU patients. Methods: International 1-day point-prevalence study; follow-up for outcome assessment until hospital discharge (maximum 12 weeks). Factors associated with ICU-acquired pressure injury and hospital mortality were assessed by generalised linear mixed-effects regression analysis. Results: Data from 13,254 patients in 1117 ICUs (90 countries) revealed 6747 pressure injuries; 3997 (59.2%) were ICU-acquired. Overall prevalence was 26.6% (95% confidence interval [CI] 25.9–27.3). ICU-acquired prevalence was 16.2% (95% CI 15.6–16.8). Sacrum (37%) and heels (19.5%) were most affected. Factors independently associated with ICU-acquired pressure injuries were older age, male sex, being underweight, emergency surgery, higher Simplified Acute Physiology Score II, Braden score < 19, ICU stay > 3 days, comorbidities (chronic obstructive pulmonary disease, immunodeficiency), organ support (renal replacement, mechanical ventilation on ICU admission), and being in a low or lower-middle income-economy. Gradually increasing associations with mortality were identified for increasing severity of pressure injury: stage I (odds ratio [OR] 1.5; 95% CI 1.2–1.8), stage II (OR 1.6; 95% CI 1.4–1.9), and stage III or worse (OR 2.8; 95% CI 2.3–3.3). Conclusion: Pressure injuries are common in adult ICU patients. ICU-acquired pressure injuries are associated with mainly intrinsic factors and mortality. Optimal care standards, increased awareness, appropriate resource allocation, and further research into optimal prevention are pivotal to tackle this important patient safety threat.

WOS: 000368722000002, PubMed ID: 26370690, Purpose: Acute heart failure (AHF) causes high burden of mortality, morbidity, and repeated hospitalizations worldwide. This guidance paper describes the tailored treatment approaches of different clinical scenarios of AHF and CS, focusing on the needs of professionals working in intensive care settings. Results: Tissue congestion and hypoperfusion are the two leading mechanisms of end-organ injury and dysfunction, which are associated with worse outcome in AHF. Diagnosis of AHF is based on clinical assessment, measurement of natriuretic peptides, and imaging modalities. Simultaneously, emphasis should be given in rapidly identifying the underlying trigger of AHF and assessing severity of AHF, as well as in recognizing end-organ injuries. Early initiation of effective treatment is associated with superior outcomes. Oxygen, diuretics, and vasodilators are the key therapies for the initial treatment of AHF. In case of respiratory distress, non-invasive ventilation with pressure support should be promptly started. In patients with severe forms of AHF with cardiogenic shock (CS), inotropes are recommended to achieve hemodynamic stability and restore tissue perfusion. In refractory CS, when hemodynamic stabilization is not achieved, the use of mechanical support with assist devices should be considered early, before the development of irreversible end-organ injuries. Conclusion: A multidisciplinary approach along the entire patient journey from pre-hospital care to hospital discharge is needed to ensure early recognition, risk stratification, and the benefit of available therapies. The complete T1 theory of such forms in the perfect dyadic model is developed. We also mention some applications to problems on the convergence of multiple ergodic averages.
