International audience; In this study we investigated whether people conceptually align when performing a language task together with a robot. In a joint picture-naming task, 24 French native speakers took turns with a robot in naming images of objects belonging to fifteen different semantic categories. For a subset of those semantic categories, the robot was programmed to produce the superordinate, semantic category name (e.g., fruit) instead of the more typical basic-level name associated with an object (e.g., pear). Importantly, while semantic categories were shared between the participant and the robot (e.g., fruits), different objects were assigned to each of them (e.g., the object of ‘a pear’ for the robot and of ‘an apple’ for the participant). Logistic regression models on participants' responses revealed that they aligned with the conceptual choices of the robot, producing over the course of the experiment more superordinate names (e.g., saying ‘fruit’ to the picture of an ‘apple’) for those objects belonging to the same semantic category as where the robot produced a superordinate name (e.g., saying ‘fruit’ to the picture of a ‘pear’). These results provide evidence for conceptual alignment affecting speakers' word choices as a result of adaptation to the partner, even when the partner is a robot.

To date, the “multiple-hit” hypothesis is largely recognized as an explanation of NAFLD pathogenesis and progression. Obesity and features of the metabolic syndrome have been closely linked to NAFLD development. Due to the increased prevalence of obesity worldwide, NAFLD has reached epidemic proportions over time. Given its unfavorable cardiometabolic burden (such as cardiovascular and metabolic consequences), it represents a worrying phenomenon needing a more comprehensive and successful management. Laboratory tests and classical imaging techniques play a pivotal role in NAFLD diagnosis, but novel noninvasive alternative methods to diagnose and monitor NAFLD have been investigated. Currently, lifestyle modifications remain the mainstay treatment, although its efficacy is poor because of the lack of compliance. Pediatric research is focusing on multiple alternative treatments targeting the main pathogenic factors such as insulin-resistance, dyslipidemia, gut-liver axis and microbiota, oxidative stress, and proinflammatory pathways. Results from these studies are promising but larger validation is needed. Innovative therapeutic approaches might add an important piece in the complex knowledge of pediatric NAFLD. We aimed to summarize recent insights into NAFLD diagnosis and treatment in children, with a focus on possible future perspectives in pediatric research.Keywords: fatty, liver, pediatric, diagnosis, management

PURPOSE As cancer surgery restarts after the first COVID-19 wave, health care providers urgently require data to determine where elective surgery is best performed. This study aimed to determine whether COVID-19–free surgical pathways were associated with lower postoperative pulmonary complication rates compared with hospitals with no defined pathway. PATIENTS AND METHODS This international, multicenter cohort study included patients who underwent elective surgery for 10 solid cancer types without preoperative suspicion of SARS-CoV-2. Participating hospitals included patients from local emergence of SARS-CoV-2 until April 19, 2020. At the time of surgery, hospitals were defined as having a COVID-19–free surgical pathway (complete segregation of the operating theater, critical care, and inpatient ward areas) or no defined pathway (incomplete or no segregation, areas shared with patients with COVID-19). The primary outcome was 30-day postoperative pulmonary complications (pneumonia, acute respiratory distress syndrome, unexpected ventilation). RESULTS Of 9,171 patients from 447 hospitals in 55 countries, 2,481 were operated on in COVID-19–free surgical pathways. Patients who underwent surgery within COVID-19–free surgical pathways were younger with fewer comorbidities than those in hospitals with no defined pathway but with similar proportions of major surgery. After adjustment, pulmonary complication rates were lower with COVID-19–free surgical pathways (2.2% v 4.9%; adjusted odds ratio [aOR], 0.62; 95% CI, 0.44 to 0.86). This was consistent in sensitivity analyses for low-risk patients (American Society of Anesthesiologists grade 1/2), propensity score–matched models, and patients with negative SARS-CoV-2 preoperative tests. The postoperative SARS-CoV-2 infection rate was also lower in COVID-19–free surgical pathways (2.1% v 3.6%; aOR, 0.53; 95% CI, 0.36 to 0.76). CONCLUSION Within available resources, dedicated COVID-19–free surgical pathways should be established to provide safe elective cancer surgery during current and before future SARS-CoV-2 outbreaks.

The development of hybrid materials, which combines inorganic with organic materials, is receiving increasing attention by researchers. As a consequence of carbon nanostructures high chemical versatility, they exhibit enormous potential for new highly engineered multifunctional nanotherapeutic agents for cancer therapy. Whereas many groups are working on drug delivery systems for chemotherapy, the use of carbon nanohybrids for radiotherapy is rarely applied. Thus, nanotechnology offers a wide range of solutions to overcome the current obstacles of conventional chemo- and/or radiotherapies. Within this review, the structure and properties of carbon nanostructures (carbon nanotubes, nanographene oxide) functionalized preferentially with different types of polymers (synthetic, natural) are discussed. In short, synthesis approaches, toxicity investigations and anticancer efficacy of different carbon nanohybrids are described.

In recent years, isotopic studies have provided a huge contribution to archaeology. Several studies, carried out in variable localities of the world, have focused their attention on human mobility, to identify non local individuals from ancient burials and reconstruct their migrations, by measuring the strontium isotope ratios (87Sr/86Sr) of their teeth enamel. This method is based on the premise that rocks display variable 87Sr/86Sr ratios depending on several factors associated with the geological characteristics of the site, i.e., age and type of the parent rocks (e.g., Price & Gestsdóttir, 2006; Kenoyer et al., 2013), and original rubidium (Rb) content. The bioavailable Sr present in soil and groundwater is incorporated into local plants and, through the food chain, enters the human body. On this basis, 87Sr/86Sr signatures in human tissue mirror the 87Sr/86Sr values of water, plants and consumed food, which in turn reflect that of bedrock of a given region (Ericson, 1985). In this study, Sr isotope ratios were measured on commercial mineral waters from variable Italian water springs, on soils and plants from the Neapolitan area, and on teeth enamel of children. The latter were born and still live in the Neapolitan area (south Italy), so that constraints on their childhood diet are available. The aim was to discriminate among the contributions of soil, water, food etc., and identify the main one affecting the isotopic signature of teeth enamel. The results of this investigation will be important for human mobility studies because they will allow understanding to which extent Sr isotopes can be effectively used for i) discriminating non-local from local individuals, and ii) assessing the effects on the modern society of a diet based on food from different geographic areas. The Zaro lava complex, located on the western sector of the Ischia volcanic island (Gulf of Naples, southern Italy), is the product of one of the few effusive eruptions occurred in the last period of activity (< 5 ka). The complex is made up of a white trachytic main lava flow (Zaro lava s.s.), hosting rounded-shaped enclaves of both mafic (shoshonite) and felsic (trachyte) composition. Major and trace elements and isotopic (87Sr/86Sr and 143Nd/144Nd) data have been acquired on whole rocks and separated minerals. Mafic enclaves (Mg# = 60-62, CaO = 7.71-8.31 wt%, Cr =117-143 ppm, Ni = 52-61 ppm, Zr =150-164 ppm, Sr = 503-526 ppm, LREE enriched compared to HREE) have low 87Sr/86Sr (0.70495- 0.70501) and high 143Nd/144Nd (0.51268-0.51269). Trachytic lavas and felsic enclaves (SiO2 = 60.61-61.71 wt%, K2O = 6.51-7.43 wt%, Na2O = 5.00-6.83 wt%, Rb = 243-428 ppm, Sr = 90-158 ppm, Zr = 385-773 ppm) exhibit higher 87Sr/86Sr (0.70608-0.70615) and lower 143Nd/144Nd (0.51255-0.51256). The strong isotopic difference between mafic enclaves and trachytes rules out a simple crystallization process for the variable facies of the Zaro lava complex. Moreover, in mafic enclaves petrochemical and possibly isotopic disequilibria between phenocrysts and their host rocks suggest occurrence of mingling processes or crystals entrapment from a distinct magmatic phase: indeed, many clinopyroxene and feldspar crystals are in chemical equilibrium with more evolved compositions (trachytic to trachyphonolitic). Trachytic lavas and felsic enclaves are chemically similar and do not display isotopic disequilibria between phenocrysts and host rocks. The last 5 ka of volcanic activity at Ischia were characterized by emplacement of products resulting from interaction between chemically and isotopically distinct batches of magma (Cava Nocelle, Vateliero and Molara; D’Antonio et al., 2013). Although these products show features comparable to those of the Zaro mafic enclaves, the last have significantly lower Sr and higher Nd isotopic ratios. Therefore, enrichment processes, such as assimilation of continental crust or mantle contamination in different conditions, should be invoked to justify the difference in isotopic features. Additionally, although the two evolved facies of the Zaro lavas have comparable chemical compositions and “uniform” isotopic signature, field analysis shows evidence of mingling between similar magmas with different viscosities. An investigation through Sr-He-O isotopes and major oxides was carried out on Mg-rich olivine and clinopyroxene phenocrysts from a selection of volcanic rocks of the Neapolitan Volcanoes (southern Italy), which include Somma- Vesuvius, Phlegraean Fields, and the islands of Ischia and Procida. The data produced in the present work extend the Sr-He-O dataset available for the Neapolitan volcanic districts (e.g., Graham et al., 1993; Martelli et al., 2004; Di Renzo et al., 2011; D’Antonio et al., 2013; Iovine et al., 2017), improving the knowledge especially on helium isotopic compositions of the Mt. Somma rocks, never analysed before. 3He/4He ratios (2.60-5.12 R/RA) of the investigated Mgrich phenocrysts are markedly lower than those measured at most volcanic arcs worldwide, and are among the lowest measured in subduction zones. This range of 3He/4He is coupled with quite large ranges of 87Sr/86Sr (0.704882 – 0.708480) and δ18O (4.85 – 6.70‰) measured in the same mafic minerals. The 3He/4He values, and the lowest measured Sr- and O-isotope ratios, do not suggest significant crustal or fluid contamination occurred during magmatic evolution, and they should reflect the composition of the metasomatized mantle beneath the Neapolitan region. Moreover, these values display a clear geographic trend, with radiogenic 4He-87Sr, and 18O generally increasing in on-lands districts. The results of these investigations are expected to allow us to better understand the original characteristics of the mantle source(s), and to define those of other components, likely originated in the continental crust, that were involved in the genesis of the primitive magmas feeding the Neapolitan volcanoes. PNPLA3 and MTP genes have been associated with liver steatosis and chronic hepatitis C. We studied the influence of MTP and PNPLA3 polymorphisms in 114 Italian patients with chronic hepatitis C, evaluating the histological and clinical presentation of liver disease. The study confirmed the association of PNPLA3 polymorphisms with liver steatosis (p=0.041), but did not show any additive effect of MTP polymorphisms in the development of steatosis. MTP polymorphisms do not seem to influence PNPLA3 in the development of liver steatosis. Further studies with a larger number of patients are required.

Mafic igneous rocks (pillow lavas and gabbros) embedded as olistoliths within Miocene turbiditic sequences crop out in the Cilento area at the Mount Centaurino (Campania region, Southern Italy). The concentration of major oxides, as well as trace element ratios (Nb/Yb, Nb/Ta, Th/Nb) and the chondrite-normalized Rare Earth Elements (REE) patterns suggest a tholeiitic character with Mid Oceanic Ridge Basalts (MORB) affinity. The chemical composition of pillow lavas is consistent with magmas generated by 10% degrees of non-modal fractional partial melting, of a spinel-bearing MORB-type asthenospheric mantle. Regarding gabbros, the calculated composition of parental melts in equilibrium with the clinopyroxenes show a wide compositional range, and there are very different from the pillow basalts of the Mount Centaurino, suggesting that the clinopyroxenes might have derived from more evolved melts compared to those that produced the basalts. The origin of these olistoliths is not yet understood. Here we suggest that these rocks represent fragment of a dismantled accretionary wedge embedded during the deposition of the Cilento group sedimentary successions in a thrust top basin.

Background: Bisphenol A (BPA) exposure has been associated with increased incidence of diabetes and obesity in adults. Objectives: To evaluate whether an association between BPA urinary levels and insulin resistance as well as adiponectin and resistin production and serum concentrations may occur in obese children. Methods: Clinical and biochemical features of 141 obese children were collected. Serum resistin and adiponectin were evaluated. Insulin resistance and urinary BPA levels were assessed. Moreover, the effect of BPA on adiponectin and resistin gene expression in adipocytes from eight normal weight prepubertal children was investigated by quantitative real-time RT-PCR (qPCR). Results: Direct association between BPA and homeostasis model assessment (r = 0.23; p: 0.0069) and a strong inverse association between BPA and adiponectin have been found (r = −0.48; p < 0.0001). In adipocytes, resistin expression was detected only after BPA treatment, while adiponectin expression resulted down-regulated after BPA exposure (p < 0.05 at both 10 and 100 nM BPA concentrations). Conclusions: We suggest the involvement of BPA in the development of insulin resistance in childhood obesity highlighting that urinary BPA levels are directly associated with insulin resistance regardless of BMI. This association may be explained, at least partly, by the findings that BPA affects resistin and adiponectin production in adipose tissue cultures.

Background The Glu167Lys (E167K) transmembrane 6 superfamily member 2 (TM6SF2) variant has been associated with liver steatosis, high alanine transaminase (ALT) levels and reduced plasma levels of liver-derived triglyceride-rich lipoproteins. Objectives The objectives of this study were to investigate in a group of obese children the association among the 167K allele of TM6SF2 gene and ALT, cholesterol and triglycerides levels, and hepatic steatosis, and to evaluate the potential interaction between this variant and the I148M patatin like phospholipase 3 gene (PNPLA3) polymorphism on liver enzymes. Methods We genotyped 1010 obese children for TM6SF2 E167K and PNPLA3 I148M polymorphisms. Anthropometrical and biochemical data were collected. Ultrasound imaging of the liver was performed. Results The 167K allele showed an association with steatosis (P < 0.0001), higher ALT levels (P < 0.001) and lower total cholesterol (P < 0.00001), low-density lipoprotein cholesterol (P < 0.0001), triglycerides (P = 0.02) and non-high-density lipoprotein cholesterol levels (P < 0.000001). The subjects homozygous for the PNPLA3 148M allele carrying the rare variant of TM6SF2 showed an odds ratio of 12.2 (confidence interval 3.8-39.6, P = 0.000001) to present hypertransaminasaemia compared with the remaining patients. Conclusion Although the TMS6SF2 E167K variant predisposes the obese children to non-alcoholic fatty liver disease, there is an association between this variant and lower levels of cardiovascular risk factors. Overall, the data suggest differential effects of TMS6SF2 E167K variant on liver and heart health.

The complexity of neuronal networks cannot only be explained by neuronal activity so neurobiological research in the last decade has focused on different components of the central nervous system: the glia. Glial cells are fundamental elements for development and maintenance of physiological brain work. New data confirm that glia significantly influences neuronal communication through specific molecules, named "gliotransmitters", and their related receptors. This new approach to the traditional model of the way synapses work is also supported by changes occurring in pathological conditions, such as neurodegenerative diseases or toxic/traumatic injury to nervous system. Experimental models have revealed that glial cells are the starting point of damage progression that subsequently involves neurons. The "bedside to bench" approach has demonstrated that clinical phenotypes are strictly related to neuronal death, however it is conceivable that the disease begins earlier, years before clinical onset. This temporal gap is necessary to determine complex changes in the neuro-glial network organization and produce a "maladaptive plasticity". We review the function of glial cells in health and disease, pointing the putative mechanisms of maladaptive plasticity, suggesting that glial cells may represent a fascinating therapeutic target to prevent irreversible neuronal cell death.

Background: Homozygous mutations in acid-labile subunit (IGFALS) gene result in short stature, very low circulating levels of acid-labile subunit (ALS), insulin growth factor 1 (IGF1) and insulin growth factor binding protein 3 (IGFBP3) and a poor response to growth hormone (GH). The impact of IGFALS mutations heterozygosity on growth is unknown. Patient and Methods: We describe a 10-year-old girl with severe short stature (height -3.2 SDS), heterozygous for a new IGFALS mutation. Results: The girl showed low circulating IGF1, IGFBP3 and ALS levels and normal GH secretion. We found a novel heterozygous frameshift IGFALS mutation (c.1283delA, p.Gln428Argfs*14). Size-exclusion chromatography showed a reduction of the IGF1, IGFBP3 and ALS 150-kDa ternary complex (by about 55%) compared to a control. An IGF-1 generation test, with two different GH dosages, showed a good response in term of increase in IGF1 and in formation of the ternary complex at size-exclusion chromatography. Clinical response after 6 months of therapy with GH was satisfactory (height velocity increased from 3 to 8 cm/year). Conclusion: We suggest that (1) heterozygous IGFALS mutations can be responsible for a subset of patients with severe short stature (below -2.5 SDS), low IGF1 (below -2 SDS) and normal GH secretion, and (2) the identification by IGFALS molecular screening of this subset of patients could help in the administration of the appropriate therapy. Background: Homozygous mutations in acid-labile subunit (IGFALS) gene result in short stature, very low circulating levels of acid-labile subunit (ALS), insulin growth factor 1 (IGF1) and insulin growth factor binding protein 3 (IGFBP3) and a poor response to growth hormone (GH). The impact of IGFALS mutations heterozygosity on growth is unknown. Patient and Methods: We describe a 10-year-old girl with severe short stature (height -3.2 SDS), heterozygous for a new IGFALS mutation. Results: The girl showed low circulating IGF1, IGFBP3 and ALS levels and normal GH secretion. We found a novel heterozygous frameshift IGFALS mutation (c.1283delA, p.Gln428Argfs*14). Size-exclusion chromatography showed a reduction of the IGF1, IGFBP3 and ALS 150-kDa ternary complex (by about 55%) compared to a control. An IGF-1 generation test, with two different GH dosages, showed a good response in term of increase in IGF1 and in formation of the ternary complex at size-exclusion chromatography. Clinical response after 6 months of therapy with GH was satisfactory (height velocity increased from 3 to 8 cm/year). Conclusion: We suggest that (1) heterozygous IGFALS mutations can be responsible for a subset of patients with severe short stature (below -2.5 SDS), low IGF1 (below -2 SDS) and normal GH secretion, and (2) the identification by IGFALS molecular screening of this subset of patients could help in the administration of the appropriate therapy. © 2013 S. Il volume nasce dall’iniziativa di un gruppo di persone confluite in un’associazione culturale Knowledge Integration for Health il cui obiettivo è superare la parcellizzazione del sapere medico in una pluralità di iperspecializzazioni formate da professionisti che spesso non comunicano tra loro. Tutto ciò si riflette non solo sulla cura dei pazienti, costretti a peregrinare tra diversi specialisti, non di rado in disaccordo tra loro, ma anche sulla ricerca scientifica, sulla formazione, sull'organizzazione dei servizi sanitari e sulla spesa pubblica. Sebbene tale esigenza sia avvertita dai cittadini, dai profes-sionisti della salute e dai ricercatori, tuttora non è emerso un modello e/o una teoria coerente dell'integrazione dei saperi per la salute a causa di barriere culturali, economiche ed organizzative. Il volume, espressione del progetto ambizioso di abbattere tali barriere, raccoglie le riflessioni scaturite da una serie di inziative (convegni, workshop, corsi di perfezionamento, sperimentazioni didattiche, ricerche sul campo, conversazioni telematiche e in-formali, e così via) realizzatesi a partire dall'anno 2010. Esso è concepito come uno strumento di lavoro e aperto alla discussione volto a creare una rete comunicativa ed informativa tra esperti di "discipline" diverse, e tra questi e gli stessi pazienti che sono portatori di conoscenze e saperi poco rappresentati nell’attuale scenario scientifico e organizzativo. Su questa falsariga è stata data voce anche a saperi "minoritari" che si configurano a volte come ipotesi di lavoro, più o meno eterodosse, ancora da provare scientificamente ma che sono un sintomo dell’esigenza sempre più sentita di una visione unitaria della salute, della malattia e della terapia. I contributi sono stati scritti da professionisti diversi appartenenti non solo al mondo accademico, ma anche istanze extrauniver-sitarie, istituzionali e non: medici di medicina generale,, ostetriche, fisioterapisti riabilitatori, pediatri, infermieri, logopedisti, tecnici di radiolo-gia, tecnici di laboratorio, epidemiologi, pedagogisti, antropo-logi, psichiatri, chimici, veterinari, statistici, urbanisti, igienisti, informatici. E'stato altresì dato spazio anche ai lavori condotti da studenti e discussi sotto forma di tesi di laurea. La struttura del volume si articola su tre livelli interconnessi: clinico, sociale ed ecologico. Il livello clinico, centrato sull'individuo e la sua malattia, ha co-me paradigma di riferimento il cosiddetto modello bio-medico, noto anche come modello dell'acuzie, fondato sul processo diagnostico-terapeutico e sulla diade terapeuta-paziente. Al secondo livello, quello sociale, il malessere del paziente è iscritto in una serie di dinamiche interpersonali, espressione di comportamenti individuali e collettivi, fattori economici, cultu-rali, e così via. Mentre il modello dell'acuzie concettualizza la malattia come un evento che ha un inizio ed una fine (almeno momentanea), il modello sociale si incentra su cronicità e disabilità che richiedono la partecipazione di familiari, badanti, servizi di riabilitazione e di supporto sociale per un tempo indefinito. I due modelli non sono mutuamente incompatibili perché spesso le condizioni di cronicità e disabilità sono costel-late da esacerbazioni o nuovi eventi che richiedono il ricorso ad interventi clinici. Il livello ecologico riguarda il modo in cui le popolazioni umane si modificano a seguito dell'interazione con altre popolazioni umane, con il mondo animale e vegetale e con l'ambiente chimi-co-fisico, formando ecosistemi la cui dinamica si dipana su scale temporali e geografiche differenti e si riflette sui profili delle malattie di ciascuna popolazione. Così, ad esempio, le grandi trasformazioni demografiche dovute ai processi migratori e alla creazione di barriere efficienti contro alcuni micro-organismi (attraverso il miglioramento delle condizioni igieniche e della conservazione degli alimenti) determinano modificazioni della composizione per età delle popolazioni che a loro volta com-portano transizioni epidemiologiche caratterizzate da cambia-menti più o meno profondi della prevalenza e l'incidenza dei diversi tipi di patologia.

Repeated exposure to psychostimulant drugs induces complex molecular and structural modifications in discrete brain regions of the meso-cortico-limbic system. This structural remodeling is thought to underlie neurobehavioral adaptive responses. Administration to adolescent rats of methylphenidate (MPH), commonly used in attention deficit and hyperactivity disorder (ADHD), triggers alterations of reward-based behavior paralleled by persistent and plastic synaptic changes of neuronal and glial markers within key areas of the reward circuits. By immunohistochemistry, we observe a marked increase of glial fibrillary acidic protein (GFAP) and neuronal nitric oxide synthase (nNOS) expression and a down-regulation of glial glutamate transporter GLAST in dorso-lateral and ventro-medial striatum. Using electron microscopy, we find in the prefrontal cortex a significant reduction of the synaptic active zone length, paralleled by an increase of dendritic spines. We demonstrate that in limbic areas the MPH-induced reactive astrocytosis affects the glial glutamatergic uptake system that in turn could determine glutamate receptor sensitization. These processes could be sustained by NO production and synaptic rearrangement and contribute to MPH neuroglial induced rewiring.

Co-infection with HBV and HCV seems to be associated with more severe liver disease in retrospective and cross-sectional studies in adults, but no data are available when co-infection is acquired in youth. The long-term outcome of infection acquired in youth was assessed in patients co-infected with HBV and HCV and in patients with HBV infection only. Twenty-seven patients with HBV and HCV co-infection and 27 patients infected with HBV only were enrolled. Seventy-six per cent of the patients were treated with alpha-interferon for 1 year. After a median follow-up of 23 years, the annual progression rate of fibrosis was 0.07 in patients co-infected with HBV and HCV, and in those infected with HBV it was 0.07 and 0.11 (P < 0.004) for HBe and anti-HBe-positive patients, respectively. In co-infected patients, the development of cirrhosis was observed in 2 (7.4%) and of hepatocellular carcinoma (HCC) in 1 (3.7%), while in those with HBV, cirrhosis appeared in one patient (3.7%). Alcohol intake (OR = 9.5 +/- 1.2; 95% CI = 6.6-13.9; P < 0.0001) was independently associated with cirrhosis and HCC. alpha-interferon showed no efficacy during treatment, but the treated group showed higher HCV RNA clearance during post-treatment follow-up. Co-infection with HBV and HCV and single HBV infection acquired in youth showed a low rate of progression to liver fibrosis, no liver failure, and low development of HCC during a median follow-up of 23 years (range 17-40).

Context: Variants on the nucleotide pyrophosphatase/phosphodiesterase-1 (ENPP-1) gene have been associated with obesity and insulin resistance. Because insulin resistance is a pivotal factor in the development of metabolic syndrome (MS) and impaired glucose tolerance (IGT), we aimed to test the association between the K121Q and rs997509 ENPP-1 variants with obesity, MS and IGT in obese children and adolescents. Methods: We screened 809 children, 409 obese and 400 lean controls. Obese subjects underwent a standard oral glucose tolerance test, whole body insulin sensitivity index (WBISI) and homeostasis model assessment (HOMA) were calculated. Results: No difference in prevalence for K121Q and rs997509 polymorphisms between obese and controls (P > 0.05) were observed. Obese children carrying the rs997509 rare allele showed higher insulin (P = 0.001), HOMA (P < .001) and lower WBISI values (P = 0.04) compared with common allele homozygous. A similar observation was done for K121Q variant, with 121Q allele carriers showing higher insulin (P = 0.03) and HOMA(P = 0.04) values than 121K homozygotes. Moreover, subjects carrying the rs997509 rare allele had higher risk of MS (odds ratio 2.4, 95% confidence interval: 1.3– 4.3) and IGT (odds ratio 4.7, 95% confidence interval: 1.9 –11.4) than common allele homozygotes. Evaluating combined effects of both polymorphisms, which are in strong linkage disequilibrium, we showed that the effect on insulin sensitivity was due to the rs997509 T variant. Conclusion:We conclude that the ENPP1 rs997509T allele can predispose obese children to MS and IGT and that this variant might drive the association between the ENPP1 121Q allele and insulin resistance.

Objective The aim of this paper was to assess the efficacy of process-based cognitive training (pb-CT) combined with reminiscence therapy (RT) in patients with mild Alzheimer's disease (mAD) and mild cognitive impairment (MCI) and in healthy elderly (HE) subjects. Methods This multicenter, randomized, controlled trial involved 348 participants with mAD, MCI, and HE from four European countries. Participants were randomly assigned to two arms of a crossover design: those in arm A underwent 3 months of computerized pb-CT for memory and executive functions combined with RT and 3 months of rest; those in arm B underwent the reverse. The primary outcome was the effect of the training on memory and executive functions performance. The secondary outcome was the effect of the training on functional abilities in mAD assessed with the instrumental activities of daily living. Results We found a significant effect of the training for memory in all three groups on delayed recall of the Rey Auditory Verbal Learning Test and for executive functions in HE on the phonological fluency test. MCI and HE participants maintained these effects at follow-up. MCI and mAD participants also showed a significant effect of the training on the Mini-mental state examination scale. Participants with mAD showed more stable instrumental activities of daily living during the training versus the rest period. Conclusions Our results corroborate the positive effect of pb-CT and its maintenance primarily on memory in HE and MCI participants that did not seem to be potentiated by RT. Although linkage studies strongly suggest that proopiomelanocortin (POMC) alterations could play a role in the genetic predisposition to obesity, systematic POMC mutational analysis did not completely confirm this hypothesis.To verify the presence of mutations of the POMC coding region in Italian children with very early onset obesity.Eighty seven unrelated Italian obese children and adolescents were studied. Mean age at obesity onset was 4.7+/-2.5 y. The POMC gene coding region was screened using single-strand conformation polymorphism (SSCP) analysis. Bi-directional automatic sequencing of PCR products was performed for all individuals who showed an aberrant SSCP pattern.Three new mutations have been identified in the heterozygous state in three patients: (a) G3834C, resulting in the substitution of Ser with Thr at codon 7 within the POMC signal peptide; (b) C3840T, resulting in the substitution of Ser with Leu at codon 9 of the pre-proopiomelanocortin signal peptide; and (c) C7406G, producing the substitution of Arg with Gly at codon 236 within the beta-endorphin peptide. A polymorphism consisting of a 9 bp insertion, AGC AGC CGC, between position 6997 and 6998 has been found at the heterozygous state in nine patients. They showed leptin levels adjusted for BMI, gender and pubertal stage significantly higher than obese subjects homozyous for the POMC wild-type allele.Mutations in codons 7 and 9 of the signal peptide may alter the translocation of the pre-proopiomelanocortin into the endoplasmic reticulum and, therefore, can be implicated in obesity. Although further studies are required, the polymorphism between position 6997 and 6998 may represent one of the genetic variations that explain the linkage between obesity and POMC.
