A systematic review to identify risk factors associated with shoulder instability

Burnout is a psychological-psychiatric term defined as ''the extinction of motivation or incentive, especially where one's devotion to a cause or relationship fails to produce the desired results”. Burnout syndrome is usually observed in jobs that involve human interaction. The effects of burnout have been well documented in the organizational literature. Factors such as prolonged heavy workloads, insufficient time for personal life, or a paucity of human or material resources can increase the risks of burnout. Pre-pandemic research shows, in academia, burnout has been associated with a decline in faculty members’ ability to teach lack of concentration and creativity, deterioration in mental and physical health and report high turnover intentions. A study in 2019 by Alves and colleagues found that burnout had a direct negative effect on faculty members’ quality of life, regardless of their field of expertise/study. However, this has not been examined in specifically medical and health science educators specifically in the US.

The aim of the current scoping review was to identify if the presence of irritable bowel syndrome was included as eligibility criteria of participants included in clinical trials investigating the effects of physical therapy in individuals with temporomandibular pain disorders (TMDs). A systematic electronic literature search in the Web of Science database was conducted. Scientifically relevant, randomized clinical trials (those cited in other studies at least 5 times, or clinical trials published in high-impact journals, i.e., first and second quartiles (Q1-Q2) of any category of the Journal Citation Report (JCR)) evaluating the effects of any physical therapy intervention in patients with TMDs were included. The Physiotherapy Evidence Database (PEDro) scale was used to evaluate the methodological quality of the selected trials. Authors affiliated to a clinical or non-clinical institution, total number of citations, objective, sex/gender, age, and eligibility criteria in each article were extracted and analyzed independently by two authors. From a total of 98 identified articles, 12 and 19 clinical trials were included according to the journal citation criterion or JCR criterion, respectively. After removing duplicates, a total of 23 trials were included. The PEDro score ranged from 4 to 8 (mean: 6.26, SD: 1.48). Based on the eligibility criteria of the trials systematically reviewed, none considered the presence of comorbid irritable bowel syndrome in patients with TMDs. The comorbidity between TMDs and irritable bowel syndrome is not considered within the eligibility criteria of participants in highly cited clinical trials, or published in a high-impact journal, investigating the effects of physical therapy in TMDs. Office-based Exercise Therapy as a Non-pharmacological Treatment for Discogenic Low Back Pain among Army Staff

Neck pain is a significant problem and many treatment options exist. While some studies suggest exercise is beneficial for individuals with non-specific neck pain clinicians have few tools to assist in the decision making process. Therefore, the purpose of this study was to derive a preliminary clinical prediction rule (CPR) for identifying patients with neck pain (NP) who may respond to an exercise-based treatment program. Exercise-based interventions have demonstrated positive outcomes in patients with NP, however it is unclear which patients are more likely to respond to this treatment approach.Consecutive patients with a primary report of nonspecific NP with or without arm pain were recruited. All patients participated in a standardized exercise program and then were classified as having a successful or non-successful outcome at 6 weeks. Potential predictor variables were entered into a stepwise regression analysis. Variables retained in the regression model were used to develop a multivariate CPR that can be used to classify patients with NP that may benefit from exercise-based treatment. A 6-month follow up of the patients was used to evaluate the long-term effects.Ninety-one patients were enrolled in the study of which 50 had a successful outcome. A CPR with 5 variables was identified (Neck Disability Index score18/50, presence of shoulder protraction during static postural assessment, patient does not bicycle for exercise, cervical side bending32°, and Fear Avoidance Belief Questionnaire-Physical Activity Score15). If 4 of the 5 variables were present, the probability of a successful outcome shifted from 56% to 78% (+LR 2.97). At 6 months no significant difference existed in self-reported outcomes between those considered positive on the rule for a successful outcome and those negative on the rule for a successful outcome.The proposed CPR may identify patients with NP likely to benefit from exercise-based treatment in the short term. However, long-term follow up did not demonstrate a significant difference between groups.2b.

OBJECTIVE: To evaluate the relative effectiveness of standing and supine hamstring stretching in increasing hamstring flexibility as measured by increasing range of motion at the knee. DESIGN AND SETTING: The trial was randomized, and the setting was local academic physical therapy and physical therapist assistant programs. SUBJECTS: Twenty-nine healthy subjects who exhibited limited hamstring muscle flexibility bilaterally (22 women, 7 men, 25.9 +/- 6.13 years of age) volunteered to participate in this study. Subjects were randomly assigned a different stretch for each leg. Each leg was stretched 3 days per week for 3 weeks (3 x 30 seconds). Stretching sessions were supervised. MEASUREMENTS: We measured supine active knee extension. Measurements were taken before and after the 3-week stretching phase by the same investigator, who was blind to limb assignment. We calculated a 2-way mixed-design analysis of variance and Tukey Honestly Significant Difference post hoc tests to analyze data. An independent t test was performed to determine whether the change scores in the stretching groups differed by sex. RESULTS: Prestretching and poststretching measurements were significantly different for both the standing and supine stretch (0.05). No significant difference (P.05) in change score existed between the 2 stretches or between the sexes. CONCLUSIONS: The standing and supine hamstring stretches were comparably effective in improving flexibility.
