Abstract

With stress echo (SE) 2020 study, a new standard of practice in stress imaging was developed and disseminated: the ABCDE protocol for functional testing within and beyond CAD. ABCDE protocol was the fruit of SE 2020, and is the seed of SE 2030, which is articulated in 12 projects: 1-SE in coronary artery disease (SECAD), 2-SE in diastolic heart failure (SEDIA), 3-SE in hypertrophic cardiomyopathy (SEHCA), 4-SE post-chest radiotherapy and chemotherapy (SERA), 5-Artificial intelligence SE evaluation (AI-SEE), 6-Environmental stress echocardiography and air pollution (ESTER), 7-SE in repaired Tetralogy of Fallot (SETOF), 8-SE in post-COVID-19 (SECOV), 9: Recovery by stress echo of conventionally unfit donor good hearts (RESURGE), 10-SE for mitral ischemic regurgitation (SEMIR), 11-SE in valvular heart disease (SEVA), 12-SE for coronary vasospasm (SESPASM). The study aims to recruit in the next 5 years (2021–2025) ≥10,000 patients followed for ≥5 years (up to 2030) from ≥20 quality-controlled laboratories from ≥10 countries. In this COVID-19 era of sustainable health care delivery, SE2030 will provide the evidence to finally recommend SE as the optimal and versatile imaging modality for functional testing anywhere, any time, and in any patient.

Abstract

Background The effectiveness trial “Stress echo (SE) 2020” evaluates novel applications of SE in and beyond coronary artery disease. The core protocol also includes 4-site simplified scan of B-lines by lung ultrasound, useful to assess pulmonary congestion. Purpose To provide web-based upstream quality control and harmonization of B-lines reading criteria. Methods 60 readers (all previously accredited for regional wall motion, 53 B-lines naive) from 52 centers of 16 countries of SE 2020 network read a set of 20 lung ultrasound video-clips selected by the Pisa lab serving as reference standard, after taking an obligatory web-based learning 2-h module ( http://se2020.altervista.org ). Each test clip was scored for B-lines from 0 (black lung, A-lines, no B-lines) to 10 (white lung, coalescing B-lines). The diagnostic gold standard was the concordant assessment of two experienced readers of the Pisa lab. The answer of the reader was considered correct if concordant with reference standard reading ±1 (for instance, reference standard reading of 5 B-lines; correct answer 4, 5, or 6). The a priori determined pass threshold was 18/20 (≥ 90%) with R value (intra-class correlation coefficient) between reference standard and recruiting center) > 0.90. Inter-observer agreement was assessed with intra-class correlation coefficient statistics. Results All 60 readers were successfully accredited: 26 (43%) on first, 24 (40%) on second, and 10 (17%) on third attempt. The average diagnostic accuracy of the 60 accredited readers was 95%, with R value of 0.95 compared to reference standard reading. The 53 B-lines naive scored similarly to the 7 B-lines expert on first attempt (90 versus 95%, p = NS). Abstract

Knowledge of transpiration is of fundamental importance for improving irrigation management. This study measured sap flow of the 'Tommy Atkins' mango tree using Granier?s thermal dissipation probe method under regulated deficit irrigation. The work was conducted in a 10-year-old ?Tommy Atkins? mango orchard, irrigated by micro sprinkler, located in the Irrigated Perimeter of Ceraíma, in Guanambi, Bahia, Brazil. Sap flow measurements were carried out on three consecutive days in plants under regulated deficit irrigation, with reductions of 30 and 60% of crop evapotranspiration in three phases of fruit development; beginning of flowering to early fruit growth (Phase I), fruit expansion (Phase II) and physiological maturation of fruits (Phase III). Abstract

We report the case of a 10-year-old girl with two episodes of light-headedness and chest pain during exercise. She had an unremarkable clinical record, physical examination, ECG, and echocardiogram. Noninvasive ischemia tests were positive, but coronary angiography was normal. Exercise stress echocardiogram revealed an exercise-induced intra-left-ventricular obstruction with a peak gradient of 78 mmHg and replicated her symptoms. After starting beta-blocker therapy her clinical status improved and no residual obstruction was detected. The authors review this unsuspected clinical condition, seldom reported in the adult population and, to our knowledge, never before in a child.

Abstract

O objetivo deste trabalho foi avaliar os teores foliares de nutrientes e a produtividade de bananeiras tipo Prata, submetidas a diferentes sistemas de irrigação, durante o primeiro ciclo de produção. Adotou-se o delineamento experimental em blocos ao acaso, com seis tratamentos dispostos em esquema fatorial 3x2: três sistemas de irrigação (aspersão convencional, microaspersão e gotejamento); e dois genótipos (Prata?Anã e seu híbrido PA42-44). Utilizaram-se quatro repetições, no espaçamento de 3,0x2,5 m. Procedeu-se às avaliações aos 90, 180 e 360 dias após o plantio. Os genótipos e os sistemas de irrigação apresentaram interações significativas quanto aos teores foliares de B e Cu, aos 180 dias após o plantio; e os genótipos diferiram entre si quanto aos teores de N, P, K, Ca, Mg, Cu e Mn, independentemente dos sistemas de irrigação. Os teores foliares dos nutrientes ocorreram em níveis adequados ou acima deles. Os menores teores foliares de micronutrientes ocorrem sob gotejamento. Os genótipos apresentam produtividade similar nos diferentes sistemas de irrigação.

The data bank from Santo Amaro City, Brazil, was taken to study 490 children aged 1-9 years who lived at less than 900 m from a primary lead smelter. Mean PbB was 58.9 ± 25.3 μg/dl and mean EP was 87.1 ± 68.1 μg/dl. At PbB levels lower than 60 μg/dl, the dose-response curve for EP versus PbB followed the classical pattern; children with low iron status showing greater EP "response" than those with typical iron status and those with high iron status, after controlling for the effects of age and racial group. Among children with PbB ≥ 60 μg/dl, EP response increased according to the following order: typical < low < high iron status.

Abstract

This study was designed to evaluate the right ventricular function by means of the determination of right ventricular systolic time intervals measured in pulmonary artery flow detected by Doppler.Left ventricular systolic time intervals have been used to evaluate left ventricular performance. There is, however, no information as to whether right ventricular systolic time intervals may be useful in the evaluation of right ventricular function.Simultaneous measurements of right ventricular ejection fraction and Doppler determination of right systolic time intervals were made in four pigs in 47 different hemodynamic conditions that permit the modification of the right ventricular ejection fraction from 15% to 68%. The preejection time, the ratio of right preejection time to right ventricular ejection time, with and without correction by the heart rate, were correlated simultaneously to invasively determined right ventricular ejection fraction and other hemodynamic parameters, measured with a modified pulmonary artery catheter with a rapidly responding thermistor.There was a significant linear correlation between Doppler determination of right systolic time intervals and right ventricular ejection fraction. A similar correlation was found for preejection time (r = .80, p.0001) and the ratio of right preejection time to right ventricular ejection time with correction by the heart rate (r = .83, p.0001). When we used a preejection time70 msec as a criterion to detect a ejection fraction40% we found a sensitivity of 89% and specificity of 88%. A preejection time80 msec, as criterion of more severe ventricular disfunction (ejection fraction30%), displayed a sensitivity of 76% and specificity of 100%.These results show that right ventricular systolic time intervals determined by Doppler study correlate significantly with the right ventricular ejection fraction. In view of its simplicity and its accuracy in the prediction of right ventricular ejection fraction, this method may provide useful non-invasive alternative in the prediction of right ventricular ejection fraction.
