Background. Management of venous thromboembolism (VTE) in patients with haematologic malignancies and thrombocytopenia is clinically challenging due to the related risks. Abstract

Background. Following high-dose chemotherapy/bone marrow transplantation, patients are routinely, prophylactically transfused with platelet concentrates (PC) if they have a platelet count ≤10×109/L or higher in the presence of risk factors for bleeding. However, whether such transfusions are necessary in clinically stable patients with no bleeding, or whether a therapeutic transfusion strategy could be sufficient and safe, is still debated. Materials and methods. The GIMEMA Haemostasis and Thrombosis Working Party sent a questionnaire to Italian haematology departments to survey several aspects of daily platelet transfusion practice, such as the cut-off platelet count for transfusion, the evaluation of refractoriness and the type of PC administered. Results. The questionnaire was answered by 18 out of 31 centres (58%). A total of 23,162 PC were transfused in 2,396 patients in 2013. The vast majority of centres (95%) transfused PC according to Italian and international guidelines; only a few transfused always at platelet counts ≤20×109/L. The broad agreement on platelet count cut-off for transfusion (≤10×109/L) was not confirmed when the World Health Organization (WHO) bleeding score was considered: only a third of centres (33%) used transfusions as recommended when the bleeding grade was ≥2. Platelet refractoriness was poorly monitored and most centres (89%) evaluated, mostly empirically (67%), response to transfusion only 24 hours later. Thirty percent of centres transfused platelets in asymptomatic refractory patients. Discussion. Although most Italian haematology departments transfuse PC according to Italian and international guidelines, our survey shows that in routine daily practice physicians do not comply closely with the WHO recommendations on platelet transfusions and monitoring platelet refractoriness. This causes excessive platelet transfusions, with a resulting increase of costs and waste of public health resources.

Abstract

Multiple myeloma (MM) remains an incurable disease, with little information available on its management in real-world clinical practice. The results of the present prospective, noninterventional observational study revealed great diversity in the treatment regimens used to treat MM. Our results also provide data to inform health economic, pharmacoepidemiologic, and outcomes research, providing a framework for the design of protocols to improve the outcomes of patients with MM. Background: The present prospective, multinational, noninterventional study aimed to document and describe real-world treatment regimens and disease progression in multiple myeloma (MM) patients. Patients and Methods: Adult patients initiating any new MM therapy from October 2010 to October 2012 were eligible. A multistage patient/site recruitment model was applied to minimize the selection bias; enrollment was stratified by country, region, and practice type. The patient medical and disease features, treatment history, and remission status were recorded at baseline, and prospective data on treatment, efficacy, and safety were collected electronically every 3 months. Results: A total of 2358 patients were enrolled. Of these patients, 775 and 1583 did and did not undergo stem cell transplantation (SCT) at any time during treatment, respectively. Of the patients in the SCT and non-SCT groups, 49%, 21%, 14%, and 15% and 57%, 20%, 12% and 10% were enrolled at treatment line 1, 2, 3, and ≥ 4, respectively. In the SCT and non-SCT groups, 45% and 54% of the patients had received bortezomib-based therapy without thalidomide/lenalidomide, 12% and 18% had received thalidomide/lenalidomide-based therapy without bortezomib, and 30% and 4% had received bortezomib plus thalidomide/lenalidomide-based therapy as frontline treatment, respectively. The corresponding proportions of SCT and non-SCT patients in lines 2, 3, and ≥ 4 were 45% and 37%, 30% and 37%, and 12% and 3%, 33% and 27%, 35% and 32%, and 8% and 2%, and 27% and 27%, 27% and 23%, and 6% and 4%, respectively. In the SCT and non-SCT patients, the overall response rate was 86% to 97% and 64% to 85% in line 1, 74% to 78% and 59% to 68% in line 2, 55% to 83% and 48% to 60% in line 3, and 49% to 65% and 36% and 45% in line 4, respectively, for regimens that included bortezomib and/or thalidomide/lenalidomide. Conclusion: The results of our prospective study have revealed great diversity in the treatment regimens used to manage MM in real-life practice. This diversity was linked to factors such as novel agent accessibility and evolving treatment recommendations. Our results provide insight into associated clinical benefits. © 2018 The Authors

Abstract

BACKGROUND: An increased risk of thrombosis has been reported in primary immune thrombocytopenia (ITP) and with the use of thrombopoietin (TPO) receptor agonists, on the basis of population studies using administrative databases. OBJECTIVES: To evaluate if the incidence of venous and arterial thrombosis in patients with primary ITP is higher than a clinically significant cut-off set at of 3% and 6.4%. PATIENTS/METHODS: We undertook a retrospective multicenter investigation in a large cohort of patients requiring at least one treatment for ITP, enrolled from the major tertiary Italian centers treating ITP. A total of 986 patients were analyzed. RESULTS: During a 3888 patient-year follow-up, 43 first thrombotic events occurred: 28 arterial and 15 venous, resulting in a cumulative incidence of 3.2% for arterial (95% CI, 2.0-5.0) and 1.4% (95% CI, 0.8-2.5) for venous thrombosis at 5 years. The annualized rates for 100 patient-years were 1.14 (95% CI, 0.84-1.54), 0.39 (95% CI, 0.23-0.65) and 0.71 (95% CI, 0.49-1.04) for total, venous and arterial thrombosis. Splenectomy, performed in 136 patients (13.7%), increased thrombotic risk (HR = 3.5, 95% CI) compared with non-splenectomized patients. Age > 60 years, more than two risk factors for thrombosis at diagnosis and steroid use were independently associated with an increased risk of thrombosis. CONCLUSIONS: In this study, we demonstrate that the 5-year cumulative incidence of venous and arterial thrombosis in ITP is well below the predefined thresholds. Venous and arterial thromboembolism are not frequent complications in ITP, except in particular settings, such as in splenectomized and elderly patients. © 2014 International Society on Thrombosis and Haemostasis.

Abstract

The term thrombophilia describes an increased tendency to develop thrombosis and many laboratory markers with different strengths of association with thrombosis have been identified. The main causes of maternal mortality and morbidity in developed countries is venous thromboembolism (VTE) and obstetric complications. During pregnancy and puerperium the risk for VTE increases due to hemostatic imbalance towards a prothrombotic state, and it is further increased in women carriers of thrombophilia; recent studies have also demonstrated an association between thrombophilia and obstetric complications. These complications are, therefore, considered potentially preventable with the prophylactic administration of anticoagulant drugs, although their efficacy is not proven by data from randomized controlled trials. After a systematic comprehensive literature review and using a rigorous methodology, the expert panel formulated recommendations regarding the usefulness of screening for thrombophilia in pregnancy to identify high-risk women and for the management of antithrombotic prophyalxis. When evidence is lacking, consensus-based recommendations are provided.

Abstract

Background: Epidemiological and clinical information on primary plasma cell leukemia (pPCL) are rarely reported. The aims are to evaluate the clinical features, prognostic factors, and efficacy of treatments in pPCL. Patients and methods: A multicenter retrospective cohort study was carried out from January 2000 to December 2008 in 26 Italian hematology divisions. A total of 128 cases of plasma cell leukemia were collected, and 73 of them (57%) were classified as primary (male/female 43/30). Results: Sixty-four patients had at least 1 sign of end-organ damage and 10 had extramedullary localization. One patient died early; of the remaining patients, 36 (50%) received anthracycline-based regimens as first-line therapy, 17 (24%) single alkylating agents, and 30 (42%) bortezomib or thalidomide as additional (n = 11) or unique treatments (n = 19). Twenty-three patients (31%) underwent autologous and/or allogeneic hematopoietic stem cell transplantation (HSCT). The median overall survival (OS) was 12.6 months; complete or partial response was achieved in 22 (30%) and 18 patients (25%), respectively; the median duration of response (DOR) was 16.4 months. HSCT patients had a longer OS and DOR (median 38.1 and 25.8 months, respectively) compared with nontransplanted patients (9.1 and 7.3 months, respectively, P < 0.001). OS was influenced by nonresponse to treatment, hypoalbuminemia, and HSCT. DOR was favorably influenced only by HSCT. Conclusions: pPCL is an aggressive disease with a poor prognosis and a low response rate to conventional therapy. HSCT is effective, increasing OS and DOR by 69% and 88%, respectively. Abstract

This paper sets out the results of a research project carried out by the University of Palermo and financed by the Sicilian Region, which aims to establish the impact of the Fischler Reform on Sicilian agriculture, and to project future scenarios that take into account some of the changes that the production process may undergo in the Region, following both the application of the Reform itself (now in force) and the eventual application of indications contained in the Health check. The impacts of the Fischler Reform, and especially the application of the Single Payment scheme to companies, were examined in cereal cropping companies, especially considering the significant amount of durum wheat cultivation in vast areas of the Region’s hilly and its strategic importance for many areas where there is a risk of farming being abandoned, with grave consequences for the territory and its farmland, for employment and for the encouragement of food and processing industries. The chosen means for assessing the effects of the reform was that of direct interviews at a statistically representative sample made up of 400 agricultural companies, determining possible earnings within the current situation, but also in the eventuality of some of the indications in the Health check being applied. The results indicate that Community aid plays a crucial role in the companies looked at, allowing them to remain on the market. Indeed, the elimination of the aid planned within the framework of the Common Agricultural Policy in the hypothetical scenarios showed a negative impact on earnings for the companies and could cause their exclusion from the market.

Abstract

Summary Background Since 1960, oral melphalan and prednisone (MP) has been regarded as the standard of care in elderly multiple myeloma patients. This multicentre randomised trial compared oral MP plus thalidomide (MPT) with MP alone in patients aged 60–85 years. Methods Patients with newly diagnosed multiple myeloma were randomly assigned to receive oral MP for six 4-week cycles plus thalidomide (n=129; 100 mg per day continuously until any sign of relapse or progressive disease) or MP alone (n=126). Analysis was intention-to-treat. This study is registered at ClinicalTrial.gov, number NCT00232934. Results Patients treated with thalidomide had higher response rates and longer event-free survival (primary endpoints) than patients who were not. Combined complete or partial response rates were 76·0% for MPT and 47·6% for MP alone (absolute difference 28·3%, 95% CI 16·5–39·1), and the near-complete or complete response rates were 27·9% and 7·2%, respectively. 2-year event-free survival rates were 54% for MPT and 27% for MP (hazard ratio [HR] for MPT 0·51, 95% CI 0·35–0·75, p=0·0006). 3-year survival rates were 80% for MPT and 64% for MP (HR for MPT 0·68, 95% CI 0·38–1·22, p=0·19). Rates of grade 3 or 4 adverse events were 48% in MPT patients and 25% in MP patients (p=0·0002). Introduction of enoxaparin prophylaxis reduced rate of thromboembolism from 20% to 3% (p=0·005). Conclusion Oral MPT is an effective first-line treatment for elderly patients with multiple myeloma. Anticoagulant prophylaxis reduces frequency of thrombosis. Longer follow-up is needed to assess effect on overall survival.

Thrombophilia is the term now used to describe predisposition to increased risk of venous and occasionally arterial thromboembolism due to hematological abnormalities. It can be a multifactorial disorder where congenital defects of anticoagulant or procoagulant factors may be combined with acquired hematological abnormalities. It should be considered in patients with a documented unexplained thrombotic episode or a positive family history. The aim of this document is to provide guidelines for investigation and management of patients with thrombophilia in the presence or absence of venous thromboembolism (VTE).

Abstract

Background Splanchnic vein thrombosis (SVT) is a challenging disease. The aim of this international registry was to describe the characteristics of a large cohort of patients with SVT and their management in clinical practice. Patients and Methods Consecutive patients with objectively diagnosed SVT were eligible. Information on clinical presentations, diagnostic approaches, risk factors, and therapeutic strategies was collected. Results We included 613 patients from 12 countries. Mean age was 53.1 (SD±14.8) years (range 16-85); 62.6% were males, 74.4% Caucasians and 22.4% Asians. Overall, 470 patients had portal vein thrombosis, 266 had mesenteric vein thrombosis, 139 had splenic vein thrombosis, and 56 had supra-hepatic vein thrombosis. SVT occurred in multiple vein segments in 38.8% of patients. Abdominal pain was the most common symptom occurring in 55.0% of the patients; 10.4% of patients had ascites or increasing abdominal girth, 8.8% gastrointestinal bleeding at the time of diagnosis; in 29.8% of patients, SVT diagnosis was incidental. Objective diagnosis was obtained with abdominal CT in 70.7% of patients and with ultrasonography in 22.6%. Most common risk factors included cirrhosis (27.8%), solid cancer (22.3%), intra-abdominal inflammation/infection (11.5%), surgery (8.9%), and myeloproliferative neoplasm (MPN)(8.2%); in 27.6% of patients SVT was idiopathic. Patients with SVT secondary to solid cancer and cirrhosis were significantly older than other patient groups (mean age 60.8 and 58.5 vs. 48.5 years); female sex was more represented in MPN patients only (55.3%). During the acute phase, 77.5% of patients were treated with anticoagulant drugs; 21.2% were left untreated. Surgery was performed in 3.1%, thrombolysis in 1.5%. A 2-year follow up is currently ongoing to assess recurrent thrombosis, bleeding, and mortality. Comment SVT patients represent a heterogeneous population with variable baseline characteristics according to thrombosis site or major underlying risk factors. These differences have obvious prognostic implications and may drive different management strategies.

Abstract

Background Splanchnic vein thrombosis (SVT) is often diagnosed incidentally. Whether demographic characteristics, underlying risk factors and therapeutic management of patients with incidentally detected SVT (IDSVT) differ from symptomatic patients is unknown. Materials and Methods Consecutive patients with objectively diagnosed SVT were eligible for a multicenter, international registry, between May 2008 and January 2012. Information on clinical presentations, risk factors and therapeutic strategies was collected and analyzed separately for asymptomatic patients. Results Out of 613 episodes of SVT 180 were incidentally detected. IDSVT patients were significantly older than symptomatic patients (56.2 and 52.1 years, respectively, p=0.0007). Gender distribution (66.7% and 60.7% males, p=0.196) and ethnicity (73.9% and 74.4% Caucasians, p=1) were similar between the two groups. Computed tomography was more commonly used as diagnostic imaging in symptomatic patients (63.7% and 73.8%, p=0.016), while ultrasonography was more frequent in IDSVT (29.1% and 19.8%, p=0.018). IDSVT patients were more likely to have single vein involvement (60.6% and 47.5%, p=0.004). Portal vein was the most frequently involved vessel in both groups (77.8% and 76.5%, p=0.806), while mesenteric (32.2% and 47.8%, p=0.0006) and splenic vein thrombosis (17.2% and 25.2%, p=0.043) was more common in symptomatic patients. Abstract

This paper sets out the results of a research project carried out by the University of Palermo and financed by the Sicilian Region, which aims to establish the impact of the Fischler Reform on Sicilian agriculture, and to project future scenarios that take into account some of the changes that the production process may undergo in the Region, following both the application of the Reform itself (now in force) and the eventual application of indications contained in the Health check. The impacts of the Fischler Reform, and especially the application of the Single Payment scheme to companies, were examined in cereal cropping companies, especially considering the significant amount of durum wheat cultivation in vast areas of the Region’s hilly and its strategic importance for many areas where there is a risk of farming being abandoned, with grave consequences for the territory and its farmland, for employment and for the encouragement of food and processing industries. The chosen means for assessing the effects of the reform was that of direct interviews at a statistically representative sample made up of 400 agricultural companies, determining possible earnings within the current situation, but also in the eventuality of some of the indications in the Health check being applied. The results indicate that Community aid plays a crucial role in the companies looked at, allowing them to remain on the market. Indeed, the elimination of the aid planned within the framework of the Common Agricultural Policy in the hypothetical scenarios showed a negative impact on earnings for the companies and could cause their exclusion from the market.

Abstract

The fish processing industry and consumers in Japan are willing to pay high (and sometimes outrageous) prices for fresh bluefin tuna with fat content and flesh colour just right to be served raw as sashimi or sushi. This willingness was and still is the economic incentive for a high fishing pressure and for the development of tuna fattening in floating cages. After seeing the economic success of tuna fattening in Australia companies in the Mediterranean region also started with the fattening of bluefin tuna (Thunnus thynnus). So bluefin tuna fishing and ranching in the Mediterranean has grown out of proportion. Such an expansion is very difficult to justify when taking into account the total dependence of tuna ranching on the bluefin tuna fishery, a fishery that has been facing serious difficulties for more than a decade now and for which both scientists and NGOs have been consistently warning that catch levels are too high
