Background: We tried to reveal the relationship between the levels of some important minerals, rare elements and heavy metals by measuring serum cobalt (Co), lead (Pb), zinc (Zn), iron (Fe), copper (Cu), cadmium (Cd), magnesium (Mg), manganese (Mn), catalase (CAT) and carbonic anhydrase (CA) levels in head and neck, CNS, esophagus, stomach and breast cancers receiving radiotherapy. Materials and Methods: Serum Cu, Fe, Pb, Zn, Co, Mg, Mn and Cd levels were measured using atomic absorption spectrophotometry. This paper presents a new regularization method to train a fully convolutional network for semantic tissue segmentation in histopathological images. This method relies on the benefit of unsupervised learning, in the form of image reconstruction, for network training. To this end, it puts forward an idea of defining a new embedding that allows uniting the main supervised task of semantic segmentation and an auxiliary unsupervised task of image reconstruction into a single one and proposes to learn this united task by a single generative model. This embedding generates an output image by superimposing an input image on its segmentation map. Then, the method learns to translate the input image to this embedded output image using a conditional generative adversarial network, which is known as quite effective for image-to-image translations. This proposal is different than the existing approach that uses image reconstruction for the same regularization purpose. The existing approach considers segmentation and image reconstruction as two separate tasks in a multi-task network, defines their losses independently, and combines them in a joint loss function. However, the definition of such a function requires externally determining right contributions of the supervised and unsupervised losses that yield balanced learning between the segmentation and image reconstruction tasks. The proposed approach provides an easier solution to this problem by uniting these two tasks into a single one, which intrinsically combines their losses. We test our approach on three datasets of histopathological images. It has been more than 3 months now since the first case of COVID-19 was reported in Turkey. Globally, the number of confirmed cases and deaths reached 9,653,048 and 491,128 respectively, as reported by 216 countries by June 27, 2020. Turkey had 1,396 new cases, 194,511 total cases, and 5,065 deaths by the same date. From the first case until today, the Turkish Thoracic Society (TTS) has been very proactive in educating doctors, increasing public awareness, undertaking academic studies, and assisting with public health policies. In the present report, social, academic, and management perspectives of the pandemic are presented under appropriate subtitles. During this critical public health crisis, TTS has once again demonstrated its readiness and constructive stance by supporting public health, healthcare workers, and the environment. Therapeutic plasma exchange (TPE) is used to treat more than 60 diseases worldwide and has drawn growing interest. Little is known about the current situation of TPE activity in Turkey, so we developed a survey to obtain information about this timely topic. We collected data on TPE from 28 apheresis units throughout Turkey. We performed a total of 24,912 TPE procedures with 3203 patients over the past decade. Twenty years ago, the majority of procedures were performed for neurological and hematological disorders, and today, most TPE procedures are done for the same reasons. The only historical change has been an increase in TPE procedures in renal conditions. Currently, renal conditions were more frequently an indication for TPE than rheumatic conditions. Fresh frozen plasma was the most frequently used replacement fluid, followed by 5% albumin, used in 57.9% and 34.6% of procedures, respectively. The most frequently used anticoagulants in TPE were ACD-A and heparin/ACD-A, used with 1671 (52.2%) and 1164 (36.4%) patients, respectively. The frequency of adverse events (AEs) was 12.6%. The most common AEs were hypocalcemia-related symptoms, hypotension, and urticaria. We encountered no severe AEs that led to severe morbidity and mortality. Overall, more than two thirds of the patients showed improvement in the underlying disease. Here, we report on a nationwide survey on TPE activity in Turkey. We conclude that there has been a great increase in apheresis science, and the number of TPE procedures conducted in Turkey has increased steadily over time. Finally, we would like to point out that our past experiences and published international guidelines were the most important tools in gaining expertise regarding TPE. © 2019 Elsevier Ltd

Paroxysmal atrial fibrillation (AF) is more common in patients with ASA than in the normal population. Neutrophil/lymphocyte ratio (NLR) has been associated with postoperative AF development in patients who have undergone cardiac surgery. This study investigated NLR in a group of ASA patients compared with a control group of healthy volunteers. Patients and methods: The study group consisted of 40 patients with ASA; the control group consisted of 30 age-, sex-, and body mass index-matched healthy volunteers. All patients and control subjects underwent echocardiographic examination. No patient had a recent history of an acute infection or an inflammatory disease. Baseline NLR was measured by dividing neutrophil count by lymphocyte count. Results: No statistically significant difference was found between the two groups in terms of basic characteristics. Mean NLR was significantly higher among persons with ASA compared with controls (3.4 ± 1.5 vs 1.6 ± 0.97, P < 0.001). Conclusion: Our results suggest that a higher NLR, an emerging marker of inflammation, has a positive correlation with ASA. The measurement of NLR may be used to indicate an increased risk of arrhythmia, such as AF, in ASA patients. Keywords: neutrophil-lymphocyte ratio, inflammation, arrhythmia

Objective: To evaluate fetal aortic Doppler for the prenatal diagnosis of hemoglobinopathies in the first trimester of pregnancy. Materials and Methods: Between January and November 2014, a total of 108 patients were enrolled in the study. The couples were carriers of either alpha/beta thalassemia, sickle cell disease or combined carriers of these and were admitted to Çukurova University Faculty of Medicine, Department of Obstetrics and Gynecology Prenatal Diagnosis Center. One hour before the chorionic villus sampling (CVS), patients were evaluated using fetal aortic Doppler. Pulsatility index, peak systolic velocity, and heart rate were noted. Results: There were no statistically significant differences in Doppler indices between different groups of CVS results when compared with the healthy controls. Conclusion: Fetal aortic Doppler investigation was found to be ineffective for the prenatal diagnosis of hemoglobinopathies. © 2016, Turkish Society of Obstetrics and Gynecology. Pectus excavatum (PE) is the depression of the lower part of manubrium sterni and xiphoid process. The main problem of PE depends on the cardiopulmonary morbidity caused by the narrowing of the thoracic space. To date, prenatal diagnosis of this deformity has been reported only once and was associated with Down syndrome. We present another case which we diagnosed as PE during a second-trimester fetal anatomic scan. The pectus severity index is used for these patients in postnatal life; however, prenatal adaption of this index is reported for the first time in our case. © Turkish Journal of Obstetrics and Gynecology, Published by Galenos Publishing House.

Aim: In this study, we aimed to determine the mothers' knowledge levels about burns and first aid. When they encounter burns, what will they do in the first instance and emergency application. At the end of this research correct information given to mothers. Aimed to supply the deficiency information.Material and Method: 25-49 age group of mothers who have 1-6 years aged childrens' in the study were choisen ramdomly. After being informed about the study, only volunteries for questionnaires were used. There were a thousand mothers. Results: Children' 21.6 % have burns. 81.4% burns were in their house, 18.6 % burns were out of their house. Often burns case were 2-4 years aged group children. 89.6 % mothers found themselves have no true knowledge about first aid for burns. Discussion: Mothers' knowledge more deficient about the first aid for the burns. In this regard mothers' lack of knowledge must be correct with the training activities to be applied. Prime necessity of the mothers were informed about the right applications.

According to the World Health Organization (WHO), anemia is the most common disease, affecting > 1.5 billion people worldwide. Furthermore, iron deficiency anemia (IDA) accounts for 50% of cases of anemia. IDA is common during pregnancy and the postpartum period, and can lead to serious maternal and fetal complications. The aim of this report was to present the experiences of a multidisciplinary expert group, and to establish reference guidelines for the optimal diagnosis and treatment of IDA during pregnancy and the postpartum period. Studies and guidelines on the diagnosis and treatment of IDA published in Turkish and international journals were reviewed. Conclusive recommendations were made by an expert panel aiming for a scientific consensus. Measurement of serum ferritin has the highest sensitivity and specificity for diagnosis of IDA unless there is a concurrent inflammatory condition. Computer-based imaging systems are becoming important tools for quantitative assessment of peripheral blood and bone marrow samples to help experts diagnose blood disorders such as acute leukemia. These systems generally initiate a segmentation stage where white blood cells are separated from the background and other nonsalient objects. As the success of such imaging systems mainly depends on the accuracy of this stage, studies attach great importance for developing accurate segmentation algorithms. Although previous studies give promising results for segmentation of sparsely distributed normal white blood cells, only a few of them focus on segmenting touching and overlapping cell clusters, which is usually the case when leukemic cells are present. In this article, we present a new algorithm for segmentation of both normal and leukemic cells in peripheral blood and bone marrow images. In this algorithm, we propose to model color and shape characteristics of white blood cells by defining two transformations and introduce an efficient use of these transformations in a marker-controlled watershed algorithm. Particularly, these domain specific characteristics are used to identify markers and define the marking function of the watershed algorithm as well as to eliminate false white blood cells in a postprocessing step. Working on 650 white blood cells in peripheral blood and bone marrow images, our experiments reveal that the proposed algorithm improves the segmentation performance compared with its counterparts, leading to high accuracies for both sparsely distributed normal white blood cells and dense leukemic cell clusters. Objective: In our study we included pregnant patients diagnosed with thrombophilia in whom thromboprophylaxis was initiated and evaluated their perinatal outcomes (pregnancy loss, preeclampsia, low birth weight, preterm labor, and venous thrombosis). We aimed to compare perinatal outcomes of the study group with those of control group with negative thrombophilia screening results. Material and Methods: Pregnant subjects that applied to Obstetrics and Gynecology Department of Çukurova University Faculty of Medicine Hospital between June 2010 and February 2012 were recruited in the study. Patients with a positive thrombophilia screening for primary and secondary recurrent pregnancy loss were initiated a thromboprophylaxia protocol in the first trimester and their demographic backgrounds and past obstetric histories were also recorded. In addition to this, the type of thrombophilia and number of thrombophilia defects were also noted and the route of birth and perinatal outcomes were evaluated prospectively. The control group included patients who had a history of recurrent unexplained miscarriages, with a negative thrombophilia screening. Hyperemesis gravidarum, is the most severe form of pregnancy induced nausea-vomiting. More than 5% weight loss, electrolyte imbalance, acetonuria and oliguria can be seen in the patients. Up to date, infrequent complications like Wernicke encephalopathy, eyeball subluxation, esophageal ruptures and pneumothorax have been reported. Although the frequency of the temporomandibular joint disorders does not change during pregnancy, in the presented case, a temporomandibular dislocation has been developed due to hyperemesis gravidarum. The disorder, known as displacement of the mandibular condyle outside the glenoid fossa, mainly occurs during movements which force the joint for hyperextension. Objective: To assess the working conditions and current challenges of the gynecology assistants in Turkey and to propose solutions within this context. Materials and methods: A survey on the problems of assistant doctors and the solution proposals they offered was executed with gynecology assistants who serve at the teaching, research and university hospitals in different regions of Turkey. Results: 60 gynecology assistants from 7 training institutions (university and teaching research hospitals) were included in the study. 75% of the participants (n:45) prefered gynecology as their first choice in the medical specialty exam. 51% of the survey participants(n:31)were satisfied with their existing conditions. The main problem was determined as severe working conditions by 43% of the participants. (n:26) Based on the answers of 42% of the participants(n:25), malpractice was observed as the leading concern following the specialty. Furthermore, setting rules on duty hours and daily leave after duty as well as arranging working hours were the common opinion of assistant doctors with regard to the solution proposals to the working conditions. Additionally, issuing assistant report cards and boosting participation in conferences and scientific research activities were other prevailing opinions of the assistant doctors when it comes to reducing training problems. Discussion: It is striking that under the current circumstances, assistant gynecologists are discontent with strenuous working environment and inadequate training. The common proposed solution is to initiate standardization in all training agencies across Turkey.

In this study, we extend the catalog-based single-channel speech-music separation method such that it incorporates prior speech information to enhance the separation performance of the method. We develop an inference method that enables us to use a speech prior model. The method uses a complex Gaussian observation model and an inverse-gamma prior model. We compare the separation performance of the catalog-based method with and without the prior speech model in both complex Gaussian and Poisson observation models. It is shown that for both observation models incorporating prior speech information improves the separation performance of the catalog-based method.

In this study, we analyze the effect of the catalog-based single-channel speech-music separation method, which we proposed previously, on speech recognition performance. In the proposed method, assuming that we know a catalog of the background music, we developed a generative model for the superposed speech and music spectrograms. We represent the speech spectrogram by a Non-negative Matrix Factorization (NMF) model and the music spectrogram by a conditional Poisson Mixture Model (PMM). In this paper, we propose to recover the speech signals from the mixed signal in time-domain by detecting the active catalog frames using the catalog-based method. We compare the performances of 3 different signal reconstruction techniques; Expectation Based, Posterior-Based and Time-Domain reconstruction. Moreover, we compare the performance of our system with the performance of the traditional NMF model. Our method outperforms the NMF method in ASR performance and separation performance in most experimental conditions.

PubMedID: 21303759 Objective: To establish and compare the frequency of intrauterine congenital heart defects in high-risk and low-risk pregnancies for congenital heart diseases. Methods: Records of 3782 patients who underwent fetal echocardiography at the Pediatric Cardiology Unit were reviewed for reasons of refer- ral and results of echocardiography retrospectively. The categorical variables between the groups were analyzed using a Chi-square test. Results: Structural heart defects were found in 213 (5.6%) fetuses in both high and low risk groups. Most common defects were ventricular septal defect (36.2%) and atrioventricular septal defect (14.1%). Frequency of congenital heart diseases was 7.8% (169 fetuses) in high-risk group: 4.1% were complex, 2.3% significant and 1.4% were minor. In low-risk group, the frequency was 2.7% (44 fetuses): 0.6% were complex, 0.8% significant and 1.3% were minor. In this group, rates of congenital defects were high regarding particular reasons: intrauterine fetal death in previous pregnancy (6.3%), abnormal first or second trimester screening tests (4.3%), and multiple gestations (3.4%). The sensitivity and specificity of fetal echocardiography according to transthoracic echocardiography were found to be 86% and 99%, respectively. Conclusion: Congenital heart diseases rate, as expected, was found to be higher in high-risk group. However, regarding intrauterine fetal death in previous pregnancy, abnormal first or second trimester screening tests and multiple gestation, no statistically significant difference was observed between low-risk and high-risk groups. Therefore, we suggest the routine use of fetal echocardiography in cases we have mentioned above if the staff and equipment of the pediatric cardiology clinic are eligible. © 2011 by AVES Yayi{dotless}nci{dotless}li{dotless}k Ltd.

PubMedID: 21077522 Objective: This study aimed to evaluate the usefulness and safety of a new method for taking a placental biopsy. Methods: The procedures were performed using the traditional single needle technique (group 1) or the new method (group 2). In group 2, the piston was fixed in a simple metallic clip and the negative pressure was maintained in a continuous manner which was controlled with a three-way stopcock. Results: Multiple uterine insertion was necessary in 14 cases (32.6%) in group 1 and five (11.9%) in group 2 (p < 0.05). The amount of chorionic tissue obtained was significantly higher in group 2 (19.1 ± 15.0 mg vs 33.9 ± 17.4 mg p < 0.05). The abortion rates did not differ in either group. Conclusion: While using this technique, the operator is capable of performing the procedure without any assistance and of applying constant negative pressure only in the placenta. The advantageous outcomes are probably related to the size as well as the incessant fashion of the vacuum force.

Introduction: Uterine arteriovenous malformations are classified as acquired or congenital. Caesarean section which is performed more frequently on nowadays, uterine curettage and other uterine surgeries are the most common causes of the acquired arteriovenous malformations. Case: Twenty-two years old woman delivered her second child by caesarean section, due to history of caesarean section, was admitted with complaint of profuse vaginal bleeding 15 days after the surgery. She demonstrated the clinical features of the hypovolemic shock. At first she treated with four units of red blood suspension, two units of fresh frozen plasma and rapid infusion of the fluids. On endometrial curettage only blood and coagulum were obtained. The bleeding could only be controlled by the internal pressure of the 18 F Foley catheter. Doppler analyze of the uterus revealed a vascular malformation signifying a turbulent flow pattern with low pressure and high flow rate. A unilateral uterine artery embolization was performed to her and her menstruel cycles are restarted at the fifth month of the puerperium. Discussion: The clinical presentation of the uterine arteriovenous malformations depends on the localization and the dimensions of the malformation. The uterine arteriovenous malformation should be kept in mind in cases of late occurrence vaginal bleeding unresponsive to the uterotonic medications, especially in a woman with previous history of uterine damage, like curettage or caesarean section.

In some malignant disorders, it was reported that urinary albumin excretion (UAE) was correlated with the prognosis and the extent of the disease. In this study, 24-h UAE was determined in 34 Hodgkin's disease patients without prior treatment and 19 healthy controls. Microalbuminuria (MAU) was defined as UAE >= 20 mu g/min. In patients with MAU, UAE was determined again after the treatment. Mean UAE was 31.2 mu g/min in the patient group and 5.6 mu g/min in the controls (p = 0.005). Whereas MAU frequency was 47% in the patients, there was no MAU in the controls. Mean UAE tended to be higher in advanced stage patients compared to early stage patients (p = 0.051). Also, MAU frequency tended to be higher in the advanced stage group compared to the early stage group (p = 0.196). In four patients in whom remission could not have been achieved, although UAE was reduced, MAU did not disappear. In conclusion, UAE was increased in Hodgkin's disease. However, there is no significant correlation between UAE and the disease extent.

The Marmara Sea region is deforming alone, the branches of the North Anatolian Fault (NAF), which is the boundary between the Anatolian and the Eurasian plates. We evaluate Global Positioning System (GPS) measurements performed from 1992 to 1999, up to the 17 August 1999 Izmit earthquake, to quantify the interseismic strain accumulation pattern in the Marmara Sea region. We compute interseismic velocities in a frame fixed to the Eurasian plate at 136 GPS points. To compute the frame-independent strain rates, we first smooth the north-south and the east-west velocity components separately to obtain the values at grid nodes, and then we compute directional derivatives and strain rates. Shear-strain rates reveal that the Marmara Sea region is within the plate boundary deformation zone having a width of about I 10 kin. The largest shear-strain rate accumulation is along the northern branch of the NAF, with maximum shear-strain rate reaching 220 nstrain/yr in the Marmara Sea. We calculate clockwise rigid body rotation rates with a maximum of 10 deg/m.y. along the nor-them branch while the Anatolian plate rotates anticlockwise. Dilatation rates display adjacent local tensional and compressional areas in the east-west direction within the Marmara Sea. Assuming, the uniform simple shear, we compute the NAF slip rates vary between I I and 26 mm/yr, with a minimum around Izmit, and increase east and west of Izmit.

Paroxysmal shooting or stabbing pain in the dermatomes of the nervus occipitalis major or nervus occipitalis minor is defined as occipital neuralgia. The initial cause of the neuralgia appears to be from inflammation, damage or irritation of these nerves. In this article, we present a patient with occipital neuralgia followed by thoracic herpes lesion.

We present and interpret Global Positioning System (GPS) measurements of crustal motions for the period 1988-1997 at 189 sites extending east-west from the Caucasus mountains to the Adriatic Sea and north-south from the southern edge of the Eurasian plate to the northern edge of the African plate. Sites on the northern Arabian platform move 18 +/- 2 mm/yr at N25 degrees +/- 5 degrees W relative to Eurasia, less than the NUVEL-1A circuit closure rate (25 +/- 1 mm/yr at N21 degrees +/- 7 degrees W). Preliminary motion estimates (1994-1997) for stations located in Egypt on the northeastern part of Africa show northward motion at 5-6 +/- 2 mm/yr, also slower than NUVEL-1A estimates (10 +/- 1 mm/yr at N2 degrees +/- 4 degrees E). A study was performed on 176 patients with benign cervical lesions. Human papillomavirus (HPV) structural antigens which were stained with the immunoperoxidase staining were sought for in cervical smears and in cervical tissues excised by the loop electrosurgical excision procedure (LEEP). HPV infection was found in 16.4% of the patients. Furthermore, cytologic screening on Papanicolaou (PAP) smears, colposcopy, and histologic examinations of whole transformation zones excised by the LEEP were performed. The diagnosis of HPV was based on the positive immunoperoxidase staining of either the exfoliated cells or the cervical tissue. HPV was detected on 5.6% of the PAP smears. The reliability of cytologic colposcopic, and histologic diagnosis of HPV was investigated.

Dysgenetic testes and remnants of the mullerian and wolffian ducts were identified at exploratory laparotomy. Mild mental retardation was also present in both cases, These data may suggest the specific role of genetic factors in the etiology of testicular dysgenesis.

In this study, we introduce a catalog-based single-channel speech-music separation method with the Itakura-Saito (IS) divergence measure. Previously, we have developed the catalog-based separation method with the Kullback-Leibler (KL)divergence. In the probabilistic point of view, IS divergence corresponds to a complex Gaussian observation models in speech-music separation task is carried out with both of catalog-based and traditional Non-Negative matrix Factorization (NMF) methods. The separation performance is compared using Speech-to-Music Ratio (SMR), Speech-to-Artifact Ratio (SAR) and speech recognition performance measure via the word Error Rate (WER). We showed that, using IS divergence in both of catalog-based or NMF based speech-music separation methods yields better separation performance than KL divergence. Moreover, in this study, it is shown that catalog-based approaches with both divergence measure outperform traditional NMF based approaches in speech recognition experiments.

The purpose of this study was to determine the level of job satisfaction of nurses working at Turkish military hospitals. A questionnaire was prepared as a data-gathering tool. This study measured the job satisfaction level at two stages. At the first stage, nurses' expectations for their jobs were measured with a Likert-type scale; at the second stage, the current level of satisfaction was determined. The differences between expectations and current level of satisfaction was calculated as the level of job satisfaction.
