Values formation during childhood is crucial in the development of children which serves as the basis of their behavior and actions outside the family and in later life. 131 volunteers of UPLB Ugnayan ng Pahinungód were selected as the sample of this study to understand how values formation in childhood translates to civic participation. Using explanatory sequential design, this research gathered data in two phases. Quantitative phase used Volunteer Functions Inventory (VFI) to determine the motivations of the respondents to volunteer while the Scales of Parenting Styles (SPS) assessed the parenting styles of the respondents’ parents. Data was analyzed using descriptive quantitative analysis. Six participants were chosen for the qualitative phase and their cases were analyzed using thematic analysis. Results highlighted the importance of values formation during childhood in the learning and practice of values associated to civic participation. Parents played important roles as models of desired behaviors and socializing agents through their parenting styles and practices as well as extended childcare. Modeling and practice of behavior through family socialization contributed to their positive development. Volunteers were motivated to participate in civic activities to seek understanding of social issues and real-life situations of Filipinos anchored by their values and high regard for humanity and life. Definition of civic participation based from their experiences was also presented in the results. Main values components of civic participation are pakikipagkapwa, malasakit, and pananagutan which are recommended to be embedded in the family’s core values to contribute to the development of children’s sense of civic duty.

As a contemporary concept, heritage bridges legal discourse and ethical discourse, national affiliations and a global imaginary, colonial pasts and neocolonial presents. The term often circulates beliefs in cultural authenticity, collective memory, and historical merit under the logic of neoliberal markets and legitimate capital gain, hence the tight connections between cultural heritage, industry, and tourism. While the concept developed as early as the nineteenth century in postrevolutionary France, its expansive political, juridical, and symbolic use has matured only after the Second World War, primarily as a reaction to the mass destruction experienced in European cities. Managing “heritage” in the context of the global political imagination created after the war, UNESCO has become the key player, lead actor, and sole orchestrator of all things/sites/memories—tangible and intangible—defined as World Cultural Heritage. This article looks at recent critical engagement with heritage, and with UNESCO's role in identifying heritage sites, by exploring Refugee Heritage, a recent project of DAAR (Decolonizing Architecture Art Residency).

This article is about a recent wave of literary dystopias published in Israel, most of which center on the soon-to-come destruction of the Jewish state. Notable among these are The Third (Ha-shlishi) by Yishai Sarid (2015), Mud (Tit) by Dror Burstein (2016), and Nuntia (Kfor) by Shimon Adaf (2010). These texts draw on biblical or Rabbinic Hebrew, Jewish sources, and Jewish historical events (specifically the destruction of the First and Second Temples), making them just as much about a dystopian past as they are about a dystopian future. They are, in other words, dystopias of a circular temporality: emerging from and moving toward (Jewish) dystopia. This recent wave of Israeli dystopian narratives is primarily preoccupied with the past and future of Judaism, the Jewish people, and Israel as a secular-yet-Jewish state. Most interesting, perhaps, is the complete absence of Palestinians from these texts and from this dystopic imagination. Despite their obvious presence in Israel’s current reality, Palestinians have no role whatsoever in these texts. We are dealing therefore with exclusively Jewish dystopias. Read against some of the dystopian white South African writings under Apartheid, the complete absence of Palestinians in the recently published Israeli dystopias, appears particularly disheartening. Neither partner nor enemy, Palestinians do not even share in a future nightmare with Israeli Jews. We are left with the following questions: Does writing a Jewish Israeli dystopia require eliminating Palestinians from the narrative? Is it possible (how is it possible?) to think of a Jewish (Israeli) future, present, and past without thinking about a Palestinian past, present, and future? Following the example of South African dystopias, this article concludes that for such literary and ethical concerns to be critically explored, Israel must first be (officially) recognized as an apartheid regime.

OBJECTIVE: Studies evaluating the association of metals with subclinical atherosclerosis are mostly limited to carotid arteries. We assessed individual and joint associations of nonessential metals exposure with subclinical atherosclerosis in 3 vascular territories. Approach and Results: One thousand eight hundred seventy-three Aragon Workers Health Study participants had urinary determinations of inorganic arsenic species, barium, cadmium, chromium, antimony, titanium, uranium, vanadium, and tungsten. Plaque presence in carotid and femoral arteries was determined by ultrasound. Coronary Agatston calcium score =1 was determined by computed tomography scan. Median arsenic, barium, cadmium, chromium, antimony, titanium, uranium, vanadium, and tungsten levels were 1.83, 1.98, 0.27, 1.18, 0.05, 9.8, 0.03, 0.66, and 0.23 mug/g creatinine, respectively. The adjusted odds ratio (95% CI) for subclinical atherosclerosis presence in at least one territory was 1.25 (1.03-1.51) for arsenic, 1.67 (1.22-2.29) for cadmium, and 1.26 (1.04-1.52) for titanium. These associations were driven by arsenic and cadmium in carotid, cadmium and titanium in femoral, and titanium in coronary territories and mostly remained after additional adjustment for the other relevant metals. Titanium, cadmium, and antimony also showed positive associations with alternative definitions of increased coronary calcium. Bayesian Kernel Machine Regression analysis simultaneously evaluating metal associations suggested an interaction between arsenic and the joint cadmium-titanium exposure.; CONCLUSIONS: Our results support arsenic and cadmium and identify titanium and potentially antimony as atherosclerosis risk factors. Exposure reduction and mitigation interventions of these metals may decrease cardiovascular risk in individuals without clinical disease.

In Becoming Palestine, Gil Z. Hochberg examines how contemporary Palestinian artists, filmmakers, dancers, and activists use the archive in order to radically imagine Palestine's future. She shows how artists such as Jumana Manna, Kamal Aljafari, Larissa Sansour, Farah Saleh, Basel Abbas, and Ruanne Abou-Rahme reimagine the archive, approaching it not through the desire to unearth hidden knowledge, but to sever the identification of the archive with the past. In their use of archaeology, musical traditions, and archival film and cinematic footage, these artists imagine a Palestinian future unbounded from colonial space and time. By urging readers to think about archives as a break from history rather than as history's repository, Hochberg presents a fundamental reconceptualization of the archive's liberatory potential.

Santiago Slabodsky's Decolonial Judaism: Triumphal Failures of Barbaric Thinking returns to a history of Jewish persecution, but unlike other attempts to trace the tradition of the Jewish “conscious pariah,” it does not aim to identify an “essence” of Judaism (say, “exilic consciousness”). The book's underlying goal is to think through the failures of previous Jewish projects to turn to the history of Jewish persecution, in order to form successful and meaningful political alliances with other, equally persecuted victims of Western hegemony. In this review, we position Decolonial Judaism in conversation with contemporary debates on exilic or diasporic Judaism and discuss the usefulness of a decolonial versus a postcolonial framework for evaluating the modern history of Judaism. Finally, we gauge the potential of reading decolonized or barbaric Judaism into the contemporary settler-colonial Jewish condition in Israel/Palestine.

Today, a growing number of French Jews embrace the far-right French Front National party, which has a long history of anti-Semitism – how did we get here? How is it that Jews turn to the French authorities for protection against so-called Muslim anti-Semitic violence? How is it that we see a similar pattern across Europe: in Belgium, The Netherlands, Denmark, and Britain? This essay addresses these questions by calling attention to the role played by the absent-present Christian West in creating and aggravating tensions between Jews and Muslims while seemingly fulfilling the role of mediation between the two. The essay suggests that current popular images of Jews and Muslims circulating in Western media ironically echo nineteenth-century European depictions of Semites – both Jews and Muslims as devoted monotheistic fanatics controlled by zeal and despotism and in need of external salvation. The bulk of the essay thus focuses on “Semitism” and examines the role of nineteenth-century European philology in disseminating Western perceptions of Jewish and Muslim inferiority. Finally, the essay advocates a re-appropriation of Semitism as an alternative memory, or rather a re-memory which as such is also a re-membering of Jews and Muslims along their shared historical position as radical Others within the hegemonic, Western, Euro-Christian epistemological order.

In his seven-minute film Chic Point: Fashion for Israeli Checkpoints (2003), Palestinian artist Sharif Waked introduces a slew of beautiful young men striding down a catwalk to the sound of heavy beat music, wearing “the latest in checkpoint fashion”: a tight mini black jacket that exposes a flat stomach in a sudden opening of a hidden side zipper, a white T-shirt with a large heart-shaped opening exposing most of the chest, and many more articles of designed clothing, all partially covering, but mostly exposing, the top part of the body. In her reading of this piece, Hochberg argues that in drawing attention to the body of the Palestinian who is stopped daily at Israeli checkpoints for long and humiliating searches, and resituating this body in a radically different context (fashion show, or perhaps strip show?), Waked resists common representations of the Palestinian as (always and only) a victim of military inspection, turning him instead into an object of desire well aware of his desirability. Furthermore, focusing on the most common search routine practiced by the Israeli soldiers at the checkpoint, the lifting of Palestinian shirts to ensure that they are not strapped with explosives, Waked gives this practice a new, and explicitly homoerotic, interpretation, presenting it as a means for Israeli soldiers to “check out” Palestinian men, who “dress up for the occasion.” In Waked's film, Hochberg concludes, the Israeli soldiers' treatment of the Palestinian body as a “security threat” and the Palestinian's forced cooperation function as pretexts for underlying hidden and forbidden (homoerotic) desire, here exposed as the subtext of a toxic national conflict sealed in heteronormative sexual perceptions of masculinity and its absence.

One of the most important contributions of this special issue of GLQ is the forum for three queer Palestinian activists to speak about the complexity of their identities and politics. In this roundtable conversation, these activists make clear that while they do not speak for all Palestinian LGBTQs, and that they do not necessarily agree among themselves on strategies or details (the women's rights approach of Aswat is different from the queer activism of Al-Qaws), they do succeed in laying out the major challenges facing their work and the strategies they employ to address them. Most central among these challenges is the need to forge a queer agenda in the context of military occupation and state-governed racism.

This essay is dedicated to the writings of Sayed Kashua, the young Israeli Palestinian novelist, journalist, and screenwriter who has become a central, if controversial, figure within the Israeli public domain: a target of both political and literary praise and blame. Specifically the essay examines Kashua's active participation in the Israeli (Hebrew) public sphere and the way in which his writings take part in the construction of a critical minority discourse. By this I do not simply mean the obvious: that, as a Palestinian citizen of Israel, Kashua occupies a minority position, which is therefore “reflected” in his writing. To the contrary, I argue that what is significant about Kashua's writings is that they seem to engage in the destruction of the very perception of coherent and natural “national affiliations” as they call attention to the discursive processes through which the Israeli Arab/Palestinian is created as a national minority who as such is also (and necessarily so) a traitor, an inauthentic Palestinian, a wannabe Jew, in short, a failed identity. Indeed, Kashua's insistence on his “Israeliness” (by writing in Hebrew, referring to himself as an Israeli Arab, and rejecting the demand that he be more “authentically Arab”), while repeatedly documenting the impossibility of actually being an Israeli-Arab or Israeli-Palestinian, draws attention to the limits of any national discourse as such to successfully account for the question of minorities. Regarding the experience of the Israeli Palestinian as a site of productive ironic performativity, Kashua's texts, I conclude, break open the monolithic image of the nation, exposing the inherent violence and contradictions involved in positioning Israel as a “Jewish State” and in situating Israeli Palestinians as incomplete members of either Israeli or Palestinian (national) collectives.

Objectives: The goals of our study were to review all cases of urethral and bladder trauma that presented to the University of California, Los Angeles (UCLA) Medical Center between January 1998 and August 2005 and determine (1) the clinical characteristics of patients with urethral and/or bladder injuries as well as the sensitivities of those clinical characteristics; (2) whether or not a blind attempt to insert a urethral catheter was performed; and (3) whether there is any evidence that a blind attempt to insert a urethral catheter worsened the initial urinary tract injury. Methods: This is a retrospective chart review. Results: The study cohort comprised 46 patients with a mean age of 30 years, including 36 men (78.2%) and 10 women (21.8%). Bladder tears were found in 33 patients, 10 patients had urethral lacerations, and 3 patients had combined bladder and urethral lacerations. The most sensitive finding for urinary bladder or urethral injury was the presence of gross hematuria in the urethral catheter (100%, 95% confidence interval [CI] 0.63–0.89). Blinded insertion of a urethral catheter was attempted in 30 (90.9%, 95% CI 0.75–0.98) patients who suffered from urinary bladder injury, 6 (50%, 95% CI 0.26–0.87) patients who suffered from urethral injury and 1 (33%, 95% CI 0.0–0.9) patient who suffered from a combined urinary bladder and urethral injuries. We did not find evidence that a blind attempt to insert a urethral catheter worsened the initial urinary injury. Conclusion: Gross hematuria in the urethral catheter was the most sensitive sign for the presence of a urethral or urinary bladder injury in our study cohort, and often the only sign of such an injury. We found no evidence that a blind attempt to insert a urethral catheter in patients suffering from urethral and or urinary bladder injuries worsened the initial injury. Larger studies will be needed to determine the safety of blind urethral catheterization in patients that are suspected to suffer from a lower urological trauma. It is our opinion that the current guidelines should be revised to better reflect the current knowledge, technologies, and clinical practice.

Objective Diagnostic errors in the emergency department (ED) are harmful and costly. We reviewed a selected high-risk cohort of patients presenting to the ED with abdominal pain to evaluate for possible diagnostic errors and associated process breakdowns. Design We conducted a retrospective chart review of ED patients >18 years at an urban academic hospital. A computerised ‘trigger’ algorithm identified patients possibly at high risk for diagnostic errors to facilitate selective record reviews. The trigger determined patients to be at high risk because they: (1) presented to the ED with abdominal pain, and were discharged home and (2) had a return ED visit within 10 days that led to a hospitalisation. Diagnostic errors were defined as missed opportunities to make a correct or timely diagnosis based on the evidence available during the first ED visit, regardless of patient harm, and included errors that involved both ED and non-ED providers. Errors were determined by two independent record reviewers followed by team consensus in cases of disagreement. Results Diagnostic errors occurred in 35 of 100 high-risk cases. Over two-thirds had breakdowns involving the patient–provider encounter (most commonly history-taking or ordering additional tests) and/or follow-up and tracking of diagnostic information (most commonly follow-up of abnormal test results). The most frequently missed diagnoses were gallbladder pathology (n=10) and urinary infections (n=5). Conclusions Diagnostic process breakdowns in ED patients with abdominal pain most commonly involved history-taking, ordering insufficient tests in the patient–provider encounter and problems with follow-up of abnormal test results.

Background Palliative sedation to treat severely distressing symptoms in those with a poor prognosis is well-accepted. Objective We discuss palliative sedation in the Emergency Department and the use of ketamine. Case Report We present the case of a patient with angioedema of the tongue and severe respiratory distress. The patient's nursing home was unable to control her symptoms and she was transferred to the Emergency Department. The patient received fentanyl 50 μg i.v. and ketamine 50 mg i.v. every 5 min until adequate palliative sedation was achieved. Conclusion Ketamine can be considered for Emergency Department palliative sedation in selected patients. Identifying and caring for unmet palliative care needs is an important skill for Emergency Medicine.

Background Evans' syndrome is an uncommon condition defined by the combination (either simultaneously or sequentially) of immune thrombocytopenia purpura and autoimmune hemolytic anemia with a positive direct antiglobulin test in the absence of known underlying etiology. Objectives We present a case of Evans' syndrome following influenza vaccination. Case Report A 50-year-old man with no prior medical history developed Evans' syndrome 4 days after receiving influenza immunization. The patient improved following treatment with oral prednisone and intravenous immunoglobulin. Conclusion Influenza vaccine is one of the most commonly used vaccines worldwide, with millions of people being vaccinated annually. Despite its wide use, only sparse information has been published concerning any hematological effects of this vaccine. The rarity of such effects supports the safety of using this vaccine.

We present 2 cases of endotracheal tube obstruction as a result of previously aspirated foreign bodies that moved from the bronchial tree into the endotracheal tube. The signs of endotracheal tube obstruction were demonstrated in both cases: (1) activation of the high-pressure alarm; (2) difficulty ventilating by using a bag-valve-mask device; (3) rapid decrease of end-tidal CO2; (4) oxygen desaturation that may have lagged behind the decrease of end-tidal CO2; and (5) the inability to advance a catheter down the endotracheal tube. The differential diagnosis of endotracheal tube obstruction is discussed, as are possible interventions to remove the obstruction and secure an open airway.

We present the case of a 12-year-old boy admitted with a complaint of recurrent syncopal episodes. A careful history taking revealed the cause of the syncopal episodes to be a dangerous game played by adolescents called "suffocation roulette." We believe that recognition of this game as a possible cause of syncopal events, together with prompt educative intervention, might prevent adolescent morbidity and mortality and also might eliminate the need for unnecessary medical investigations.

The mechanism of ultrasound augmentation of pharmacological thrombolysis is yet unknown. The goal of this study is to find the best timing regimen for in-vitro ultrasound augmented clot dissolution by streptokinase, heparin and their combination. Blood clots from 4 donors were cut into 200-400 mg sections and randomized to no treatment with ultrasound; pre-treatment with ultrasound (before immersion); early treatment with ultrasound; or late treatment with ultrasound. Clots were placed in tubes containing either saline; heparin; streptokinase or streptokinase +heparin. All groups showed significant weight reduction (p < 0.001). Using the one way ANOVA test, we showed that ultrasound application resulted in a significantly higher rate of clots dissolution (p < 0.05) than without ultrasound in all of the solutions tested. We found no statistically significant difference between the three ultrasound regimens tested. In conclusion, in our in-vitro model, no single ultrasound timing schedule was found to provide better clot dissolution than the other schedules. This finding may suggest an additive effect between the ultrasound and the different solutions rather than a synergistic effect.

Background The patterns of regional metastasis in adenoid cystic carcinoma (ACC) of the head and neck and its association with outcome is not established. Methods We conducted a retrospective multicentered multivariate analysis of 270 patients who underwent neck dissection. Results The incidence rate of neck metastases was 29%. The rate observed in the oral cavity is 37%, and in the major salivary glands is 19% (p =.001). The rate of occult nodal metastases was 17%. Overall 5-year survival rates were 44% in patients undergoing therapeutic neck dissections, and 65% and 73% among those undergoing elective neck dissections, with and without nodal metastases, respectively (p =.017). Multivariate analysis revealed that the primary site, nodal classification, and margin status were independent predictors of survival. Conclusion Our findings support the consideration of elective neck treatment in patients with ACC of the oral cavity.

There has been a great deal of ferment in ‘Complexity Science’ in recent years, as chronicled in the proceedings of the New England Complex Systems Institute’s International Conference on Complex Systems [Minai & Bar-Yam 2006, 2008] and those of the Santa Fe Institute [Nadel & Stein 1995, Cowan 1994]. We have been primarily focused on developing metrics of complexity relevant to chemistry, especially synthetic chemistry [Bertz 2003a–c]. Our approach involves abstracting a molecule or a plan for its synthesis as a graph and then using the tools of graph theory to characterize its complexity and diversity.

Objectives: Our goal was to evaluate the utility of the pelvic ring stability examination for detection of mechanically unstable pelvic fractures in blunt trauma patients. Methods: Retrospective chart review. Results: We enrolled 1,502 consecutive blunt trauma patients and found 115 patients with pelvic fractures including 34 patients with unstable pelvic fractures (Tile classification B and C). Unstable pelvic ring on physical examination had a sensitivity and specificity of 8% (95% CI 4-14) and 99% (95% CI 99-100), respectively, for detection of any pelvic fracture and 26% (95% CI 15-43) and 99.9% (95% 99-100), respectively, for detection of mechanically unstable pelvic fractures. The sensitivity and specificity of pelvic pain or tenderness in patients with Glasgow Coma Scale >13 were 74% (95% CI 64-82) and 97% (95% CI 96-98), respectively for diagnosing any pelvic fractures, and 100% (95% CI 85-100) and 93% (95% CI 92-95), respectively for diagnosing of mechanically unstable pelvic fractures. The sensitivity and specificity of the presence of pelvic deformity were 30% (95% CI 22-39) and 98% (95% CI 98-99), respectively for detection of any pelvic fracture and 55% (95% CI 38-70) and 97% (95% CI 96-98), respectively for detection of mechanically unstable pelvic fractures. Conclusions: The presence of either pelvic deformity or unstable pelvic ring on physical examination has poor sensitivity for detection of mechanically unstable pelvic fractures in blunt trauma patients. Our study suggests that blunt trauma patients with Glasgow Coma Scale >13 and without pelvic pain or tenderness are unlikely to suffer an unstable pelvic fracture. A prospective study is needed to determine whether a set of clinical criteria can safely detect or exclude the presence of an unstable pelvic fracture.

Study objective We externally validate the ability of the San Francisco Syncope Rule to accurately identify syncope patients who will experience a 7-day serious clinical event. Methods Patients who presented to a single academic emergency department (ED) between 8 am and 10 pm with syncope or near-syncope were prospectively enrolled. Treating physicians recorded the presence or absence of all San Francisco Syncope Rule risk factors. Patients were contacted by telephone at 14 days for a structured interview. A 3-physician panel, blinded to the San Francisco Syncope Rule score, reviewed ED medical records, hospital records, and telephone interview forms to identify predefined serious clinical events. The primary outcome was the ability of the San Francisco Syncope Rule to predict any 7-day serious clinical event. A secondary outcome was the ability of the San Francisco Syncope Rule to predict 7-day serious clinical events that were not identified during the initial ED evaluation. Results Of 592 eligible patients, 477 (81%) provided informed consent. Direct telephone contact or admission/outpatient records were successfully obtained for 463 (97%) patients. There were 56 (12%) patients who had a serious 7-day clinical event, including 16 (3%) who received a diagnosis after the initial ED evaluation. Sensitivity and specificity of the San Francisco Syncope Rule for the primary outcome were 89% (95% confidence interval [CI] 81% to 97%) and 42% (95% CI 37% to 48%), respectively, and 69% (95% CI 46% to 92%) and 42% (95% CI 37% to 48%), respectively, for the secondary outcome. Estimates of sensitivity were minimally affected by missing data and most optimistic assumptions for missing follow-up information. Conclusion In this external validation cohort, the San Francisco Syncope Rule had a lower sensitivity and specificity than in previous reports.
