Edward Gillis,Charles Rampersaud,Emmanuelle Pease,Paul Buscemi ReVENT Medical, Inc., Newark, CA,USA Background: In obstructive sleep apnea (OSA), occlusion of the upper airway by soft tissue causes intermittent hypoxemia and can have serious sequelae. A novel implantable medical device for OSA is composed of a linear silicone elastic element held in an extended state by a bioabsorbable external sheath. The implant is delivered to the tongue base or soft palate via a minimally invasive approach. Normal tissue healing anchors the device at the attachment points before the bioabsorbable material dissolves and the elastic element contracts to stabilize the surrounding tissue.Methods: Device prototypes were evaluated in multiple investigations: 1) a finite elements analysis model simulated the movement of the tongue base during sleep with and without the implant; 2) dynamic mechanical testing simulated 10years’ normal use; 3) cadaveric implantations were conducted; 4) an ovine study in which implants of varying design were evaluated via gross pathology and histological assessment; and 5) a canine study in which implants of varying design in the tongue base and soft palate were evaluated via gross pathology and histological assessment.Results: 1) The implant was capable of reducing ~95% of tongue base movement during simulated sleep; 2) implants remained intact throughout the testing with no evidence of creep fatigue or change in dynamic modulus; 3) the device could be reliably deployed in the desired placement locations and was appropriate for various anatomies; and 4) all implants were well tolerated through 1year, with minimal inflammatory responses.Conclusion: This new minimally invasive device for OSA has been demonstrated, through various bench and animal testing, to be safe, well tolerated, suitable for long-term use, and to function as intended. No adverse health consequences were observed in the animals, and histological evaluation indicated good healing. This study establishes proof of concept and supports human trials. A 28-year-old female presented to the Emergency Department (ED) with a chief complaint of strings protruding from her vagina. The patient also complained of recurrent symptoms of cystitis and occasional hematuria over the past five months without resolution after treatment. The patient underwent ED evaluation and was noted to have strings coated in calculus protruding from her urethral meatus. On AP abdominal film a T-shaped intrauterine device (IUD) with calculus was noted in the pelvis. By computed tomography (CT) scan the object was shown to be extruding from the vagina into the bladder. Of note the patient had a history of IUD use with supposed removal five years prior to presentation. The diagnosis of IUD perforation of the bladder with calculus formation was confirmed by cystoscopy, and the IUD and calculi were successfully removed without complication.

Fragestellung: Standardeingriffe zur Therapie der retrolingualen Obstruktion bei OSA bedeuten ob ihrer Invasivität für den Patienetn eine erhebliche postoperative Morbidität. Das ReVent System besteht aus sich selbst verkürzenden Implantaten, die in ITN über einen kleinen submentalen Hautschnitt in Seldinger-Technik in die Zunge eingebracht werden. Nach 4 Wochen verkürzen sich diese Implantate nach Resorption der äußeren Hülle. Dadurch soll ein Kollaps im Schlaf verhindert werden Der gering operative Aufwand bedeutet eine stark reduzierte, postoperative Morbidität, wie eine erste Untersuchung an 40 Patienten mit je 2 Zungenimplantaten ergab. Ziel dieser Studie war die Feststellung der Morbidität nach Implantation von jetzt 4 ReVent Implantaten in die Zunge. Methoden: 7 OSA-Patienten (AHI= 38,8±26,6) mit persistierender retrolingualer Obstruktion nach verschiedenen Voroperationen wurden mit je 4 Implantaten versorgt. Am ersten Tag postoperativ erfolgte eine flexible Endoskopie. 10 Tage postop. erfolgte die Lagekontrolle der Implantate mittels MRT. Die Schmerzhaftigkeit wurde mittels visueller Analogskala (VAS 0–10) über 10 Tage erfasst. Ergebnisse: Die OP-Dauer lag zwischen 20 und 43 Minuten. Schwere oder bleibende Komplikationen wurden nicht beobachtet. 2 Implantate mussten wegen Luxation in den Hypopharynx am ersten postoperativen Tag entfernt werden. Die Schmerzhaftigkeit lag am OP-Tag bei 4,1 und sank über 2,0 (Tag 6) auf 1,2 (Tag 10). Alle Patienten waren mit dem postoperativen Ergebnis zufrieden. Schlussfolgerungen: Die Implantation von 4 ReVent Implantaten in Zungengrund hat sich als sicher und einfach durchführbar erwiesen. Die postoperative Morbidität ist geringer als die bei anderen Verfahren zur Behandlung der retrolingualen Obstruktion. Objektive Nachuntersuchungen im Schlaflabor zur Bestimmung der Effektivität sind terminiert.
