Abstract

Objective: To determine the time required to perform a complete skin examination (CSE) as a means of opportunistic screening for skin cancer both without and with dermoscopy. Design: Randomized, prospective multicenter study. Setting: Eight referral pigmented lesion clinics. Patients: From June 2006 to January 2007, 1359 patients with at least 1 melanocytic or nonmelanocytic skin lesion were randomly selected to receive a CSE without dermoscopy or CSE with dermoscopy. For each patient, the total number of lesions and the duration of the CSE were recorded. A total of 1328 patients were eligible for analysis (31 were excluded because of missing data). Main Outcome Measures: The median time (measured in seconds) needed for CSE with and withoutdermoscopy and according to total cutaneous lesion count. Results: The median time needed for CSE without dermoscopy was 70 seconds and with dermoscopy was 142 seconds, a significant difference of 72 seconds (P

Abstract

The increasing use of dermoscopy in preoperative diagnosis of melanocytic skin neoplasms is impacting on routine histopathology to a relevant extent. We herein present the dermoscopic-pathologic features of 6 cases of histopathologically controversial melanocytic skin neoplasms. By illustrating these cases, we emphasize at least 3 different fields of interest for a combined (clinico-)dermoscopic-pathologic diagnostic approach, namely, information about the evolution of lesions; detection of gross sampling errors; definition of peculiar clinicopathologic entities. We describe 3 cases of desmoplastic nevus with special emphasis on some repetitive dermoscopic and histopathologic features, which-if confirmed on larger series-could allow to identify desmoplastic nevus as a specific clinicopathologic entity within the spectrum of acquired melanocytic nevi.

Abstract

Background: We tested the relevance of clinical information in the histopathologic evaluation of melanocytic skin neoplasm (MSN). Methods: Histopathologic specimens from 99 clinically atypical MSN were circulated among ten histopathologists; each case had clinical information available in a database with a five-step procedure (no information; age/sex/location; clinical diagnosis; clinical image; dermoscopic image); each step had a histopathologic diagnosis (D1 through D5); each diagnostic step had a level of diagnostic confidence (LDC) ranging from 1 (no diagnostic certainty) to 5 (absolute diagnostic certainty). The comparison of the LDC was employed with an analysis of variance (ANOVA) for repeated measures. Abstract

Background: Sclerosing nevi with pseudomelanomatous features or, else, nevi with regression-like fibrosis (NRLF) are histopathologic simulators of regressing melanoma. Objective: We aimed at evaluating the clinical features in a series of NRLF. Methods: Dermoscopic images of NRLF were re-evaluated according to the amount of regression, the presence of white/blue areas and the 7-point checklist. Results: Forty-six lesions from 44 patients (M:F = 3.4:1; mean age: 42 years) were evaluated. Thirty-seven lesions were excised from the back, mostly from the scapular area. All the lesions were dermoscopically atypical, with large amounts of regression (>10% in 43 cases) and with coexistence of white and blue areas (41 cases). According to the 7-point checklist, 25 lesions were labeled as benign, probably because regression obscured other dermoscopic features of atypia. Conclusion: NRLF are mostly found in the convex area of the back. A better recognition of their clinicopathologic features can allow a more conservative management.

Abstract

A DNEXAL TUMORS BELONG TO A HETEROGEneous group of mostly benign neoplasms with appendageal (eccrine, apocrine, follicular, and sebaceous) differentiation. The lesions shown inFigures1,2, and3were located on the chest, neck, and abdomen of a 39-year-old woman, a71-year-oldman, andanother39-yearoldwoman, respectively. In lesions 1 and 2, a clinical diagnosis of basal cell carcinoma (Figure 1A and Figure 2A) was strengthened by dermoscopy, which revealed leaflike areas (Figure 1B) and arborizing vessels (Figure 1B and Figure 2B). In lesion 3 a diagnosis of collision tumor (seborrheic keratosis with basal cell carcinoma) was made for the presence of multiple comedolike openings and arborizing vessels (Figure 3). Histopathologic examination revealed an angiohistiocytoma, a hydradenoma, and an intraepidermal poroma (with features of acrosyringeal nevus) in lesions 1, 2 and 3, respectively. Clinically and dermoscopically, adnexal tumors may often mimic basal cell carcinoma; thus, histopathologic evaluation remains the gold standard for diagnosis.

Abstract

Eccrine poroma (EP) may clinically mimic a number of benign and malignant skin tumors. Dermoscopy improves the clinical diagnosis of many pigmented and nonpigmented skin tumors, but to date little is known about the impact of dermoscopy in the diagnosis of EP. Abstract

BACKGROUND: The starburst pattern is the dermoscopic hallmark of pigmented Spitz nevus, although it has been rarely observed in melanoma as well. OBJECTIVE: To describe a case of melanoacanthoma simulating pigmented Spitz nevus. MATERIAL AND METHODS: Clinical, dermoscopic, and histopathologic examinations were performed for the occurrence of a 4-mm pigmented skin lesion on the hip of a 38-year-old Caucasian woman. RESULTS: Dermoscopy examination of the lesion disclosed a stereotypical starburst pattern characterized by pigmented streaks symmetrically distributed at the periphery. A preoperative diagnosis of pigmented Spitz nevus was made, and the lesion was excised. However, subsequent histopathologic examination revealed a melanoacanthoma. CONCLUSION: The starburst pattern, although diagnostic for pigmented Spitz nevus, can be rarely observed in other benign or malignant pigmented skin lesions. Accordingly, all lesions in adults exhibiting a starburst pattern or other spitzoid features should be excised for histopathologic evaluation.
