9. Inter-vendor and inter-observer reliability of diffusion tensor imaging in the musculoskeletal system:a multiscanner MR study

Background To evaluate the inter-observer and inter-vendor reliability of diffusion tensor imaging parameters in the musculoskeletal system. Methods This prospective study included six healthy volunteers three men (mean age: 42; range: 31–52 years) and three women (mean age: 36; range: 30–44 years). Each subject was scanned using different 3 Tesla magnetic resonance scanners from three different vendors at three different sites bilaterally. First, the intra-class correlation coefficient was used to determine between-observers agreement for overall measurements and clinical sites. Next, between-group comparisons were made through the nonparametric Friedman’s test. Finally, the Bland–Altman method was used to determine agreement among the three scanner measurements, comparing them two by two. Results A total of 792 measurement were calculated. ICC reported high levels of agreement between the two observers. ICC related to MD, FA, and RD measurements ranged from 0.88 (95% CI 0.85–0.90) to 0.95 (95% CI 0.94–0.96), from 0.85 (95% CI 0.81–0.88) to 0.95 (95% CI 0.93–0.96), and from 0.89 (0.85–0.90) to 0.92 (0.90–0.94). No statistically significant inter-vendor differences were observed. The Bland–Altmann method confirmed a high correlation between parameter values. Conclusion An excellent inter-observer and inter-vendor reliability was found in our study.

The identification of high-risk patients in the early stages of infected pancreatic necrosis (IPN) is critical, because it could help the clinicians to adopt more effective management strategies. We conducted a post hoc analysis of the MANCTRA-1 international study to assess the association between clinical risk factors and mortality among adult patients with IPN. Univariable and multivariable logistic regression models were used to identify prognostic factors of mortality. We identified 247 consecutive patients with IPN hospitalised between January 2019 and December 2020. History of uncontrolled arterial hypertension (p = 0.032; 95% CI 1.135–15.882; aOR 4.245), qSOFA (p = 0.005; 95% CI 1.359–5.879; aOR 2.828), renal failure (p = 0.022; 95% CI 1.138–5.442; aOR 2.489), and haemodynamic failure (p = 0.018; 95% CI 1.184–5.978; aOR 2.661), were identified as independent predictors of mortality in IPN patients. Cholangitis (p = 0.003; 95% CI 1.598–9.930; aOR 3.983), abdominal compartment syndrome (p = 0.032; 95% CI 1.090–6.967; aOR 2.735), and gastrointestinal/intra-abdominal bleeding (p = 0.009; 95% CI 1.286–5.712; aOR 2.710) were independently associated with the risk of mortality. Upfront open surgical necrosectomy was strongly associated with the risk of mortality (p p = 0.018; 95% CI 0.138–0.834; aOR 0.339) and enteral nutrition (p = 0.003; 95% CI 0.143–0.716; aOR 0.320) were found as protective factors. Organ failure, acute cholangitis, and upfront open surgical necrosectomy were the most significant predictors of mortality. Our study confirmed that, even in a subgroup of particularly ill patients such as those with IPN, upfront open surgery should be avoided as much as possible. A system for internal and voluntary reporting of abnormal events in a Nuclear Medicine Therapy Unit is described. This system is based on the Internet of Things and is composed of an application for mobile devices and a wireless network of detectors. The application is addressed to healthcare professionals and is intended to be a user-friendly tool to make the reporting procedure little laborious. The network of detectors allows for a real-time measurement of the dose distribution in the patient’s room. The staff was involved in all stages, from the design of the dosimetry system and mobile application up to their final testing. Face-to-face interviews were carried out with 24 operators in different roles in the Unit (radiation protection experts, physicians, physicists, nuclear medicine technicians and nurses). The preliminary results of the interviews and the current state of development of the application and the detection network will be described.

En la producción de plántula forestal en vivero, el sustrato es de gran importancia. Actualmente se recomienda utilizar una mezcla de peatmoss, perlita y vermiculita. Sin embargo, el precio de este sustrato es elevado. Por lo anterior, es importante encontrar alternativas más baratas y eficientes. En este sentido, en el presente trabajo se evaluó el uso de aserrín crudo como sustrato para la producción de planta en vivero. Aunque en más del noventa por ciento de la literatura disponible se reporta como tóxico o que contiene, sustancias que limitan el crecimiento de las plantas en sus diferentes etapas de crecimiento. Además, se presentan los resultados de veinticinco años de experiencia, usando aserrín crudo como sustrato. También, se presenta una propuesta de homologación entre el aserrín y la clasificación de los suelos utilizando como variable el tamaño de partícula que los divide. Basado en la textura, el aserrín es una alternativa por utilizar aserrín al 100%, es decir únicamente aserrín para producir planta de calidad, con alta sobrevivencia en campo, que es lo que se busca, en los diferentes tipos de plantaciones forestales.

15. Radiomics analysis of bone marrow biopsy locations in [18F]FDG PET/CT images for measurable residual disease assessment in multiple myeloma

The combination of visual assessment of whole body [18F]FDG PET images and evaluation of bone marrow samples by Multiparameter Flow Cytometry (MFC) or Next-Generation Sequencing (NGS) is currently the most common clinical practice for the detection of Measurable Residual Disease (MRD) in Multiple Myeloma (MM) patients. In this study, radiomic features extracted from the bone marrow biopsy locations are analyzed and compared to those extracted from the whole bone marrow in order to study the representativeness of these biopsy locations in the image-based MRD assessment. Whole body [18F]FDG PET of 39 patients with newly diagnosed MM were included in the database, and visually evaluated by experts in nuclear medicine. A methodology for the segmentation of biopsy sites from PET images, including sternum and posterior iliac crest, and their subsequent quantification is proposed. First, starting from the bone marrow segmentation, a segmentation of the biopsy sites is performed. Then, segmentations are quantified extracting SUV metrics and radiomic features from the [18F]FDG PET images and are evaluated by Mann–Whitney U-tests as valuable features differentiating PET+/PET− and MFC+ /MFC− groups. Moreover, correlation between whole bone marrow and biopsy sites is studied by Spearman ρ rank. Classification performance of the radiomics features is evaluated applying seven machine learning algorithms. Statistical analyses reveal that some images features are significant in PET+/PET− differentiation, such as SUVmax, Gray Level Non-Uniformity or Entropy, especially with a balanced database where 16 of the features show a p value 18F]FDG PET images in MRD assessment in MM patients.

Nanotechnology has greatly impacted our daily life and has certainly yielded many promising benefits. Titanium dioxide nanoparticles (TiO2-NPs) are among those produced on a large industrial scale that have found many practical applications in industry and daily life. Due to their presence in products such as food, cosmetics, sunscreens, medications, paints or textiles, contact with TiO2-NPs in our daily life is inevitable. The small size, together with the corresponding large specific surface area, make nanoparticles able to penetrate through cellular barriers and reach various parts of the body through different routes of exposure, including inhalation, injection, dermal penetration, and gastrointestinal tract absorption. Furthermore, after long-term exposure, the TiO2-NPs could accumulate in tissues leading to chronic diseases. This raises serious doubts about their potentially harmful effects on human health. : In the past, TiO2-NPs have been considered inert, however, many in vitro studies have shown that they were cyto- and genotoxic, leading to the production of reactive oxygen species (ROS) and to the activation of signaling pathways involved in inflammation and cell death. Several in vivo studies have also demonstrated that TiO2-NPs, once in the bloodstream, could reach and accumulate in important organs causing toxic effects. Very recently, the International Agency for Research on Cancer (IARC) has classified these nanoparticles as possibly carcinogenic to humans. : In this survey, we summarize the latest advances in acknowledging the toxicity and safety of TiO2-NPs. Since the literature is often controversial, further studies are still needed to define the risk/benefit ratio of using these nanoparticles. Overall, the data herein reported are critical for assessing human risk after exposure to TiO2-NPs.

Background: Perioperative atrial fibrillation (AF) and myocardial injury after noncardiac surgery (MINS) are common complications after noncardiac surgery. Inflammation has been implicated in the pathogenesis of both disorders. The COP-AF trial tests the hypothesis that colchicine reduces the incidence of perioperative AF and MINS in patients undergoing major noncardiac thoracic surgery. Design: The 'COlchicine for the Prevention of Perioperative Atrial Fibrillation' (COP-AF) trial is an international, blinded, randomized trial that compares colchicine to placebo in patients aged at least 55 years and undergoing major noncardiac thoracic surgery with general anesthesia. Exclusion criteria include a history of AF and a contraindication to colchicine (e.g., severe renal dysfunction). Oral colchicine at a dose of 0.5 mg or matching placebo is given within 4 hours before surgery. Thereafter, patients receive colchicine 0.5 mg or placebo twice daily for a total of 10 days. The two independent co-primary outcomes are clinically important perioperative AF (including atrial flutter) and MINS during 14 days of follow-up. The main safety outcomes are sepsis or infection and non-infectious diarrhea. We aim to enroll 3,200 patients from approximately 40 sites across 11 countries to have at least 80% power for the independent evaluation of the two co-primary outcomes. Summary: COP-AF is a large randomized and blinded trial designed to determine whether colchicine reduces the risk of perioperative AF or MINS in patients who have major noncardiac thoracic surgery. BACKGROUND: Burnout syndrome has been extensively studied in different health science professions. It has been less studied in physiotherapy than in professions such as medicine. Moreover, it is not known how the working condition influences this syndrome. OBJECTIVE: The main objective of this study was to compare the burnout index between contract and freelance physiotherapists in the private sector in the Community of Madrid, Spain. METHODS: A cross-sectional study was performed with 174 participants divided into 2 groups; one group was composed of contract physiotherapists (n = 87) and the other group was composed of freelance physiotherapists (n = 87). A Mann–Whitney U test was performed for comparison between the groups. Spearman’s correlation coefficient was used to analyze the correlations between the burnout syndrome index and the secondary variables. RESULTS: There were statistically significant differences when comparing the groups, with a large effect size for the burnout index with a higher rate among contract physiotherapists (78 [71–84.75]) than in freelance physiotherapists (61.5 [55–72.75]).There were also significant differences in the type of patients treated, number of patients treated per day, time spent per patient, and the annual salary range between the contract and freelance physiotherapists. CONCLUSION: Contract physiotherapists who participated in this study had a significantly higher burnout syndrome index than freelance physiotherapists. Other socio-occupational variables were also found to be related to the burnout syndrome index in freelance physiotherapists and contract physiotherapists. The results of this study could be of interest for new occupational health strategies to reduce the burnout index in contract physiotherapists.

- Abstract
Cow's milk protein allergy (CMPA) is the most common in childhood, affecting 2 to 5% of children. The aim of the present study is to analyze the main scientific findings on risk factors and clinical complications associated with CMPA. a bibliographic search of articles collected in the Medline and Lilacs databases, available in the Virtual Health Library (BVS) and in Google Scholar, based on the Health Sciences Descriptors (DeCS): food allergy and cow's milk allergy. To improve the results obtained, certain inclusion and exclusion criteria were adopted. The inclusion criteria involved: national and international articles; available in full; published in the last 10 years, between 2013 and 2023From this study, some risk factors associated with CMPA were identified, among the most outstanding: maternal exposure to antibiotics during pregnancy; parental history of allergy; genetic predisposition; presence of allergic conditions caused by other allergens; intestinal microbial imbalance; lack of maturity of the immune system; and exposure of infants to antibiotics in the first year of life. The main clinical complications of CMPA involve: diarrhea, gastrointestinal bleeding, eczema, atopic dermatitis, asthma and, in severe cases, anaphylaxis.

24. New paddlefishes (Acipenseriformes, Polyodontidae) from the Late Cretaceous Tanis Site of the Hell Creek Formation in North Dakota, USA

The recently discovered mass mortality of fishes from the Tanis Site in the North Dakota portion of the Late Cretaceous Hell Creek Formation contains many well-preserved, three-dimensional skeletons. Among these are representatives of two acipenseriform families, Acipenseridae (sturgeons) and Polyodontidae (paddlefishes). This paper describes two new monotypic polyodontid genera, expanding our knowledge of polyodontid diversity. The first of the new species described here is †Parapsephurus willybemisi n. gen. n. sp. It is distinguished from all other known species by having a combination of posteriorly elongate parietals, the middle fenestra longitudinalis bordered medially by the parietal and frontal and laterally by the dermopterotic, slender and numerous dorsal caudal fulcra, an elongate hyomandibula that is not hourglass shaped, and gill rakers that are short and widely spaced. The second polyodontid species described here is †Pugiopsephurus inundatus n. gen. n. sp. It is diagnosed by a combination of having stellate bones that are exceptionally poorly developed or absent and having a dermopalatine with a medial expansion and lacking an ectopterygoid process. The two species of paddlefishes described in this paper add to the morphological and taxonomic diversity of Polyodontidae. The presence of these taxa within the Hell Creek Formation hints at substantial diversity of polyodontids at this stage of their evolutionary history. The paper explores the interplay between religion, security, and gender. By employing a broad analytical security framework ranging from warfare violence against women to violence in the domestic sphere, the paper supports the view that countries with heightened rates of gender inequalities and violence are more insecure and prone to conflict. The paper also questions the role played by religions enforcing hierarchical gender constructs in preserving women’s security and protecting them from gender-based violence. Against this background, the paper maintains that deferring the realisation of women’s security until a comprehensive reform has taken place within the religious precinct must not be an option. Such an approach would signify the capitulation of the egalitarian human rights paradigm. Instead, women’s roles as agents of change within religious institutions should be supported, and the proactive engagement of states to encourage religious institutions to reform and embrace an egalitarian ethos should be promoted. Only in this way will the interplay of religion, security, and gender truly benefit the pursuit of women’s security.

The administration of antifungals for therapeutic and, especially, prophylactic purposes is virtually a constant in patients requiring hematology-oncology treatment. Any attempt to prevent or treat Aspergillus or Mucor infections requires the administration of some drugs in the azole group, which include voriconazole, posaconazole and isavuconazole, noted for their activity against these pathogens. One very relevant aspect is the potential risk of interaction when associated with one of the antineoplastic drugs used to treat hematologic tumors, with serious complications. In this regard, acalabrutinib, bortezomib, bosutinib, carfilzomib, cyclophosphamide, cyclosporine A, dasatinib, duvelisib, gilteritinib, glasdegib, ibrutinib, imatinib, nilotinib, ponatinib, prednisone, ruxolitinib, tacrolimus, all-transretinoic acid, arsenic trioxide, venetoclax, or any of the vinca alkaloids, are very clear examples of risk, in some cases because their clearance is reduced and in others because of increased risk of QTc prolongation, which is particularly evident when the drug of choice is voriconazole or posaconazole.

Background There is scarce evidence on the fourth dose of severe acute respiratory syndrome coronavirus 2 vaccines in chronic kidney disease (CKD) patients. We evaluated the humoral response and effectivity of the fourth dose in the CKD spectrum: non-dialysis CKD (ND-CKD), haemodialysis (HD), peritoneal dialysis (PD) and kidney transplant (KT) recipients. Methods This is a prespecified analysis of the prospective, observational, multicentric SENCOVAC study. In patients with CKD who had received a complete initial vaccination and one or two boosters and had anti-Spike antibody determinations 6 and 12 months after the initial vaccination, we analysed factors associated with persistent negative humoral response and higher anti-Spike antibody titres as well as the efficacy of vaccination on coronavirus disease 2019 (COVID-19) severity. Results Of 2186 patients (18% KT, 8% PD, 69% HD and 5% ND-CKD), 30% had received a fourth dose. The fourth dose increased anti-Spike antibody titres in HD (P = .001) and ND-CKD (P = .014) patients and seroconverted 72% of previously negative patients. Higher anti-Spike antibody titres at 12 months were independently associated with repeated exposure to antigen (fourth dose, previous breakthrough infections), previous anti-Spike antibody titres and not being a KT recipient. Breakthrough COVID-19 was registered in 137 (6%) patients, 5% of whom required admission. Admitted patients had prior titres Conclusions A fourth vaccine dose increased anti-Spike antibody titres or seroconverted many CKD patients, but those with the highest need for a vaccine booster (i.e. those with lower pre-booster antibody titres or KT recipients) derived the least benefit in terms of antibody titres. Admission for breakthrough COVID-19 was associated with low anti-Spike antibody titres.

Aluminum saline slags is a waste of the metallurgical industry that presents serious environmental problems since it needs very extensive areas for its disposal, the toxicity it causes in the atmosphere and groundwater, in addition to high transportation costs. The valorization of this residue by the synthesis of alumina, a compound widely used in the chemical industry, generates a high impact and great interest. In this work, the strategies for synthesizing alumina from aluminum saline slags are reviewed in a context of growing demand for this metal and environmental crisis. The first sections present the aluminum production processes, both from natural bauxite (primary process) and from the recycling of materials with a high aluminum content (secondary process); paying attention to the waste generated and what environmental problems they produce. The main investigations that have allowed to address the recovery of the waste generated are described below, focusing on the processes of recovery/extraction of the aluminum present in its composition. The aluminum in these residues can be found as a metal or forming other compounds such as simple or mixed oxides. Chemical processes are the most relevant, especially those that deal with the acid and alkaline extraction of the metal. The most important section of the work reports on the methods of synthesis of Al2O3, highlighting the methods of precipitation, sol-gel, hydrothermal synthesis, and combustion, among others. Introducción. La plantilla de órdenes múltiples es una herramienta informática que podría producir consecuencias inadvertidas pese a sus innumerables beneficios. Nos propusimos explorar el efecto de su inactivación sobre las solicitudes de estudios complementarios y los costos asociados. Métodos. Corte transversal en la Central de Emergencias de Adultos del Hospital Italiano de Buenos Aires, que incluyó muestra consecutiva de consultas pre-intervención (Enero-Febrero 2020) y post-intervención (2021). Mediante el uso de bases secundarias, las variables incluidas fueron los débitos administrativos y sus respectivos precios de facturación. Resultados. Hubo 27.671 consultas en 2020 con una mediana de valor total de 474$, y 20.819 con 1.639$ en 2021. Tras el análisis restringido al área de consultorios de moderada complejidad (excluyendo consultas por COVID-19), se encontró: una disminución en la mediana del número de prácticas por consulta (mediana de 11 vs 10, p=0,001), una disminución en la solicitud de al menos una práctica de laboratorio (45% versus 39%, p=0,001), sin encontrar cambios significativos en costos globales (mediana 1.419$ vs 1.081$; p=0,122) ni en costos específicos de laboratorio (mediana 1.071$ vs 1.089$, p=0,710). Conclusión. Pese a la inflación interanual, se logró una reducción significativa en el número de prácticas y se mantuvieron los costos globales por consulta. Estos hallazgos demuestran la efectividad de la intervención, pero serán necesarias medidas educativas que apunten al recordatorio de los potenciales daños en la sobreutilización, y los costos sanitarios de los estudios innecesarios.

Whole wheat is an exceptional source of phenolic compounds representing a promising phytochemical class to prevent diet-related chronic diseases thanks to its antioxidant activities. The present work reports the phenolic profile, the antioxidant capacity, the antimicrobial activity and the effect on Lactobacillus brevis growth of eight whole flours obtained from four ancient and modern wheat genotypes of Italian Triticum genus. Total phenolic content (TPC) and total flavonoid content (TFC) were quantified, and antioxidant activities were assessed using oxygen radical absorbance capacity (ORAC) and 2,2-diphenyl-1-picrylhydrazyl (DPPH) in vitro tests. HPLC-DAD/FLD was used to detect the presence of phenolic compounds. Moreover, antimicrobial activity of whole flour extracts against some potentially pathogenic Gram negative and Gram positive bacteria and the effect of extracts on Lactobacillus brevis growth were assessed. Results showed quantitative differences (p L. brevis was stimulated. The principal component analysis (PCA) confirmed that the phenolic profile and the antioxidant activities were influenced by the genotypic characteristics of studied varieties, suggesting that the ancient Saragolla stand out for the most interesting phenolic profile. Overall, this research emphasizes how ancient and modern Italian Triticum spp. grains must be investigated to select the grains richer in bioactive compounds. Graphical abstract

This article aims to highlight how the sociology of memory actually corresponds to a sociology of remembering and forgetting. Indeed, if a component of forgetting is always present in memory processes, in order to fully understand the past it is not enough to focus on its visible traces, but it is also necessary to investigate those contents that have been lost, that have not been deposited or that have been prevented from having visibility. In other words, if we normally think of the sociology of memory by assuming memory as remembrance and focusing on its visible traces, we actually have to consider that it simultaneously involves remembering and forgetting; two complementary activities that are not always assumed with adequate balance in sociological analyses of memory phenomena. With this in mind, the article reflects on the different meanings of forgetting and reinterprets the traditions of memory studies in the light of a renewed focus on forgetting issues. Thus, the paths of forgetting in the main paradigms of memory will be traced and discussed: that of the social frameworks of memory, that of the critical-emancipatory paradigm, that of the places of memory, and that of memory work.

36. Case study of advanced processed OFHC copper by dry sliding wear test

The wear behaviour of copper material processed by ECAP (Equal Channel Angular Pressing) and orbital forging (OF) is presented in this study. Dry sliding wear tests were carried out for the wear behaviour of the investigated system. Oxygen-free high thermal conductivity (OFHC) copper was used for testing. The new combination of metal forming processes was used because of ease of fabrication. Additionally, wear rate, friction coefficient and wears mechanisms were observed. The friction resistance is caused by the destruction of the adhesion between surface asperities in metal friction. Moreover, increased asperity interactions connected with wear particle entrapment gradually increase the friction coefficient. These results show the metal forming process's positive influence in reducing interfacial adhesion and asperity deformation. Finally, the combinations of newly used advanced processing demonstrated excellent wear characteristics of copper.

Several surgical treatments are available for managing lower urinary tract symptoms secondary to benign prostatic hyperplasia (LUTS/BPH). Water vapor thermal therapy (WVTT) is a new minimally invasive therapy. This study estimates the budget impact of introducing WVTT for LUTS/BPH into the Spanish health care system. A model simulated the evolution of men over 45 years of age with moderate-severe LUTS/BPH after surgical treatment, over a 4-year time horizon, from the Spanish public health care service´s perspective. The technologies in scope included those most used in Spain: WVTT, transurethral resection (TURP), photoselective laser vapourization (PVP) and holmium laser enucleation (HoLEP). Transition probabilities, adverse events and costs were identified from the scientific literature and validated by a panel of experts. Sensitivity analyses were performed by varying the most uncertain parameters. Per intervention, WVTT resulted in savings of €3,317, €1,933 and €2,661 compared to TURP, PVP and HoLEP. Over a 4-year time horizon, when performed in 10% of the cohort of 109,603 Spanish males with LUTS/BPH, WVTT saved €28,770,125 against the scenario without WVTT availability. WVTT could reduce the cost of managing LUTS/BPH, increase the quality of health care and reduce the length of procedure and hospital stay.

Antecedentes: los hemangiomas infantiles son los tumores vasculares benignos más frecuentes en la infancia, siendo su localización más habitual la cabeza y el cuello, representando el 50 % de las masas que afectan a la glándula parótida. Debido al rápido crecimiento expansivo de estas lesiones, pueden aparecer complicaciones significativas, como pérdida de la función del órgano o desfiguración permanente, por lo que la intervención terapéutica precoz es de gran importancia para minimizar estos efectos indeseables. Reporte de caso: presentamos dos casos de pacientes con hemangioma parotídeo tratados con propranolol, con resultados muy favorables, y con escasas o nulas reacciones adversas. Conclusiones: actualmente, el propranolol oral está considerado como el fármaco de primera elección en el tratamiento de los hemangiomas parotídeos, mostrando un adecuado perfil de seguridad y una excelente eficacia.

40. Stage III Non-Small-Cell Lung Cancer: An Overview of Treatment Options

Lung cancer is the second-most commonly diagnosed cancer and the leading cause of cancer death worldwide. The most common histological type is non-small-cell lung cancer, accounting for 85% of all lung cancer cases. About one out of three new cases of non-small-cell lung cancer are diagnosed at a locally advanced stage—mainly stage III—consisting of a widely heterogeneous group of patients presenting significant differences in terms of tumor volume, local diffusion, and lymph nodal involvement. Stage III NSCLC therapy is based on the pivotal role of multimodal treatment, including surgery, radiotherapy, and a wide-ranging option of systemic treatments. Radical surgery is indicated in the case of hilar lymphnodal involvement or single station mediastinal ipsilateral involvement, possibly after neoadjuvant chemotherapy; the best appropriate treatment for multistation mediastinal lymph node involvement still represents a matter of debate. Although the main scope of treatments in this setting is potentially curative, the overall survival rates are still poor, ranging from 36% to 26% and 13% in stages IIIA, IIIB, and IIIC, respectively. The aim of this article is to provide an up-to-date, comprehensive overview of the state-of-the-art treatments for stage III non-small-cell lung cancer.

Logistics operators have become important agents in supply chains, due to the current trend of outsourcing logistical activities by companies. In this way, the level of quality of the services provided is an important aspect for the competitiveness of companies. In addition, the sustainability aspect has become relevant, given the environmental impact caused by logistics activities. Studies have shown that there are points of convergence between quality management approaches and sustainable practices. The objective of this work was to analyze aspects of quality management and sustainability in Brazilian logistics operators, from the perspective of the company contracting these services. To this end, a case study was conducted at a large company that outsources its logistics activities. The five main logistics operators providing services to the company were considered. The analysis was carried out on the basis of a model that integrates sustainability and quality management criteria. Quality assets, processes and dimensions were considered. It was found that the contracted logistics operators have a wide portfolio of services offered, quality assets and well-defined processes. OBJECTIVE Timing of mobilization after chronic subdural hematoma (cSDH) surgery is highly heterogeneous among neurosurgical centers. Past studies have suggested that early mobilization may reduce medical complications without increasing recurrence, but evidence remains scarce. The purpose of this study was to compare an early mobilization protocol with a 48-hour bed rest practice, with a focus on the occurrence of medical complications. METHODS The GET-UP Trial is a prospective, randomized, unicentric, open-label study with an intention-to-treat primary analysis designed to evaluate the impact of an early mobilization protocol after burr hole craniostomy for cSDH on the occurrence of medical complications and functional outcomes. A total of 208 patients were recruited and randomly assigned to either an early mobilization group where they began head-of-bed elevation within the first 12 hours after surgery and proceeded to sedestation, orthostatism, and/or walking as rapidly as tolerated, or to a bed rest group where they remained recumbent with a head-of-bed angle inferior to 30° for 48 hours after surgery. The primary outcome was the occurrence of a medical complication (defined as either an infection, seizure, or thrombotic event) after surgery and until clinical discharge. Secondary outcomes included length of stay measured from randomization to clinical discharge, surgical hematoma recurrence at clinical discharge and 1 month after surgery, and Glasgow Outcome Scale–Extended (GOSE) assessment at clinical discharge and 1 month after surgery. RESULTS A total of 104 patients were randomly assigned to each group. No significant baseline clinical differences were observed before randomization. The primary outcome occurred in 36 (34.6%) patients included in the bed rest group and 20 (19.2%) in the early mobilization group (p = 0.012). At 1 month after surgery, a favorable functional outcome (defined as GOSE score ≥ 5) was observed in 75 (72.1%) patients in the bed rest group and 85 (81.7%) in the early mobilization group (p = 0.100). Surgical recurrence occurred in 5 (4.8%) patients in the bed rest group and 8 (7.7%) in the early mobilization group (p = 0.390). CONCLUSIONS The GET-UP Trial is the first randomized clinical trial to assess the impact of mobilization strategies on medical complications after burr hole craniostomy for cSDH. Early mobilization was associated with a reduction in medical complications without a significant effect on surgical recurrence, compared with a 48-hour bed rest protocol.

45. Evidence-Based vs Informal Suicide Training: Nurse Confidence and Comfort With Suicidal Patient Care

Emergency nurses are on the front line of patient care for suicidal persons, yet many nurses report feeling unprepared to effectively manage suicidal patients owing to a lack of suicide-specific training. The purpose of this study was to examine the suicide-specific training experiences of emergency nurses and evaluate how training relates to burnout, confidence, and comfort working with suicidal patients.Emergency nurses at critical access and community hospitals completed an anonymous online survey during work hours. The survey included questions about training experiences, burnout, confidence, and comfort working with suicidal patients, perceptions of the quality and interactive nature of training, and desires for future suicide-specific intervention training.Group comparisons among the 117 emergency nurses revealed that those who received evidence-based/expert-delivered training reported greater confidence, comfort, and perceived ability to treat suicidal patients and lower burnout than those who received informal or no training. Those with informal training reported greater confidence and ability to treat suicidal patients, but similar levels of comfort and burnout as those with no training. Mediation analyses showed that training was associated with greater comfort working with suicidal patients through its effect on increased confidence. A majority desired additional suicide-specific training.Evidence-based/expert-delivered professional training in suicide intervention is associated with improved confidence, comfort, and perceived ability to care for suicidal patients and lower burnout. Providing evidence-based suicide intervention training may improve quality of care for suicidal patients by improving emergency nurse confidence and comfort for treating these high-risk patients.

ImportanceDelays in screening programs and the reluctance of patients to seek medical attention because of the outbreak of SARS-CoV-2 could be associated with the risk of more advanced colorectal cancers at diagnosis.ObjectiveTo evaluate whether the SARS-CoV-2 pandemic was associated with more advanced oncologic stage and change in clinical presentation for patients with colorectal cancer.Design, Setting, and ParticipantsThis retrospective, multicenter cohort study included all 17 938 adult patients who underwent surgery for colorectal cancer from March 1, 2020, to December 31, 2021 (pandemic period), and from January 1, 2018, to February 29, 2020 (prepandemic period), in 81 participating centers in Italy, including tertiary centers and community hospitals. Follow-up was 30 days from surgery.ExposuresAny type of surgical procedure for colorectal cancer, including explorative surgery, palliative procedures, and atypical or segmental resections.Main Outcomes and MeasuresThe primary outcome was advanced stage of colorectal cancer at diagnosis. Secondary outcomes were distant metastasis, T4 stage, aggressive biology (defined as cancer with at least 1 of the following characteristics: signet ring cells, mucinous tumor, budding, lymphovascular invasion, perineural invasion, and lymphangitis), stenotic lesion, emergency surgery, and palliative surgery. The independent association between the pandemic period and the outcomes was assessed using multivariate random-effects logistic regression, with hospital as the cluster variable.ResultsA total of 17 938 patients (10 007 men [55.8%]; mean [SD] age, 70.6 [12.2] years) underwent surgery for colorectal cancer: 7796 (43.5%) during the pandemic period and 10 142 (56.5%) during the prepandemic period. Logistic regression indicated that the pandemic period was significantly associated with an increased rate of advanced-stage colorectal cancer (odds ratio [OR], 1.07; 95% CI, 1.01-1.13; P = .03), aggressive biology (OR, 1.32; 95% CI, 1.15-1.53; P P = .03).Conclusions and RelevanceThis cohort study suggests a significant association between the SARS-CoV-2 pandemic and the risk of a more advanced oncologic stage at diagnosis among patients undergoing surgery for colorectal cancer and might indicate a potential reduction of survival for these patients.

- Abstract
É com grande entusiasmo que lhe apresento este livro, uma obra que mergulha no campo da educação, da tecnologia e da inclusão. Para tanto, exploram-se temas relevantes e instigantes que refletem o panorama dos estudos investigados na contemporaneidade. Por meio de uma abordagem abrangente e fundamentada, cada capítulo oferece uma análise sobre diferentes aspectos a partir de olhares multifacetados. No capítulo, "A Importância da Inclusão Escolar do Indivíduo Autista desde a Educação Infantil", exploramos o que é o autismo bem como o comportamento dos acometidos pela síndrome. Em seguida, no capítulo "Inclusão da Criança com Transtorno do Espectro Autista na Educação Infantil: O que Indicam as Pesquisas Disponíveis no Banco de Teses e Dissertações da CAPES (2010-2020)?", nos aprofundamos acerca da inclusão da criança com TEA na Educação Infantil. No capítulo, "Direitos Humanos em e para a Educação: As Tecnologias Assistivas como Instrumento de Autonomia", discutimos as tecnologias assistivas no âmbito da educação a partir da concepção de direitos humanos. Prosseguindo, adentramos no capítulo "Ensino e Aprendizagem na Disciplina de Teoria Geral de Administração II para o Desenvolvimento de Competências Empreendedoras". Nessa seção, exploramos o desenvolvimento de capacidades empreendedoras dos acadêmicos por meio de atividades que permitiram o uso dos conceitos atrelados às habilidades individuais/ grupais aos saberes atitudinais. No capítulo seguinte, "Educação Básica e a Pandemia de COVID-19: Reflexos do Ensino Remoto, Desafios e Possibilidades", analisamos compreender o que representou o período de suspensão das atividades presenciais durante a deflagração da pandemia de Covid-19. Avançando na obra, chegaremos ao capítulo "A Teoria dos Jogos como Eixo Norteador para a Tomada de Decisões Racionais no Âmbito da Vida Pessoal e Profissional". Nessa seção, exploramos o princípio da racionalidade a partir da teoria dos jogos, no contexto de um jogo de soma zero, mais especificamente focando-se no princípio do problema do Dilema do Prisioneiro. Logo depois, no capítulo intitulado "A Docência na Educação Superior – Análise Prospectiva", refletimos sobre o papel do docente na educação superior. Refletiremos sobre as perspectivas e desafios que envolvem a docência nesse contexto, considerando as transformações e demandas do docente na contemporaneidade. Na sequência, apresentamos o texto "Find Me a partir da educação profissional e tecnológica: uma proposta de aplicativo para busca de emprego". Nesse capítulo, apresentamos uma proposta de criação de um aplicativo de busca de emprego, nomeado como Find Me, que tem como intuito facilitar a comunicação entre profissionais e contratantes. Já no capítulo, “Projeto Anúbis: a educação profissional e tecnológica como instrumento de combate ao crime contra a mulher”, propomos um plano de negócios que visa localizar e monitorar regiões das cidades, categorizando cada tipo de crime e sua potencialidade, por meio da criação de uma ferramenta que permita acesso rápido às informações sobre crimes, informando em tempo real os riscos daquela região. Na redação do capítulo, “A educação profissional e tecnológica a favor da valorização do hortifrúti na agricultura familiar: reflexões sobre uma proposta de criação de site e aplicativo” temos uma proposta de criação de um aplicativo e um site que viabilizem esse comércio, na expectativa de superar esse que pode ser um desafio e também de incentivar a valorização desse tipo de produção. À medida que avançamos em nossa jornada, explorando os capítulos deste livro, nos deparamos com temas igualmente relevantes e instigantes. Desde a busca por soluções inovadoras na educação profissional e tecnológica até o enfrentamento do crime contra a mulher. Cada capítulo proporciona uma visão aprofundada e reflexiva sobre questões cruciais da atualidade. Esperamos que esta obra desperte seu interesse, promova debates construtivos e amplie sua compreensão sobre os desafios e possibilidades que permeiam os campos aqui aludidos. Convido-o, assim, a embarcar nessa jornada intelectual e enriquecedora, explorando as reflexões e propostas apresentadas ao longo dessas páginas.

- Abstract
É com grande entusiasmo que lhe apresento este livro, uma obra que mergulha no campo da educação, da tecnologia e da inclusão. Para tanto, exploram-se temas relevantes e instigantes que refletem o panorama dos estudos investigados na contemporaneidade. Por meio de uma abordagem abrangente e fundamentada, cada capítulo oferece uma análise sobre diferentes aspectos a partir de olhares multifacetados. No capítulo, "A Importância da Inclusão Escolar do Indivíduo Autista desde a Educação Infantil", exploramos o que é o autismo bem como o comportamento dos acometidos pela síndrome. Em seguida, no capítulo "Inclusão da Criança com Transtorno do Espectro Autista na Educação Infantil: O que Indicam as Pesquisas Disponíveis no Banco de Teses e Dissertações da CAPES (2010-2020)?", nos aprofundamos acerca da inclusão da criança com TEA na Educação Infantil. No capítulo, "Direitos Humanos em e para a Educação: As Tecnologias Assistivas como Instrumento de Autonomia", discutimos as tecnologias assistivas no âmbito da educação a partir da concepção de direitos humanos. Prosseguindo, adentramos no capítulo "Ensino e Aprendizagem na Disciplina de Teoria Geral de Administração II para o Desenvolvimento de Competências Empreendedoras". Nessa seção, exploramos o desenvolvimento de capacidades empreendedoras dos acadêmicos por meio de atividades que permitiram o uso dos conceitos atrelados às habilidades individuais/ grupais aos saberes atitudinais. No capítulo seguinte, "Educação Básica e a Pandemia de COVID-19: Reflexos do Ensino Remoto, Desafios e Possibilidades", analisamos compreender o que representou o período de suspensão das atividades presenciais durante a deflagração da pandemia de Covid-19. Avançando na obra, chegaremos ao capítulo "A Teoria dos Jogos como Eixo Norteador para a Tomada de Decisões Racionais no Âmbito da Vida Pessoal e Profissional". Nessa seção, exploramos o princípio da racionalidade a partir da teoria dos jogos, no contexto de um jogo de soma zero, mais especificamente focando-se no princípio do problema do Dilema do Prisioneiro. Logo depois, no capítulo intitulado "A Docência na Educação Superior – Análise Prospectiva", refletimos sobre o papel do docente na educação superior. Refletiremos sobre as perspectivas e desafios que envolvem a docência nesse contexto, considerando as transformações e demandas do docente na contemporaneidade. Na sequência, apresentamos o texto "Find Me a partir da educação profissional e tecnológica: uma proposta de aplicativo para busca de emprego". Nesse capítulo, apresentamos uma proposta de criação de um aplicativo de busca de emprego, nomeado como Find Me, que tem como intuito facilitar a comunicação entre profissionais e contratantes. Já no capítulo, “Projeto Anúbis: a educação profissional e tecnológica como instrumento de combate ao crime contra a mulher”, propomos um plano de negócios que visa localizar e monitorar regiões das cidades, categorizando cada tipo de crime e sua potencialidade, por meio da criação de uma ferramenta que permita acesso rápido às informações sobre crimes, informando em tempo real os riscos daquela região. Na redação do capítulo, “A educação profissional e tecnológica a favor da valorização do hortifrúti na agricultura familiar: reflexões sobre uma proposta de criação de site e aplicativo” temos uma proposta de criação de um aplicativo e um site que viabilizem esse comércio, na expectativa de superar esse que pode ser um desafio e também de incentivar a valorização desse tipo de produção. À medida que avançamos em nossa jornada, explorando os capítulos deste livro, nos deparamos com temas igualmente relevantes e instigantes. Desde a busca por soluções inovadoras na educação profissional e tecnológica até o enfrentamento do crime contra a mulher. Cada capítulo proporciona uma visão aprofundada e reflexiva sobre questões cruciais da atualidade. Esperamos que esta obra desperte seu interesse, promova debates construtivos e amplie sua compreensão sobre os desafios e possibilidades que permeiam os campos aqui aludidos. Convido-o, assim, a embarcar nessa jornada intelectual e enriquecedora, explorando as reflexões e propostas apresentadas ao longo dessas páginas.

Background: Immuno-oncology combinations have achieved survival benefits in patients with metastatic renal cell carcinoma (mRCC). Objective: The ARON-1 study (NCT05287464) was designed to globally collect real-world data on the use of immuno-combinations as first-line therapy for mRCC patients. Patients and methods: Patients aged ≥ 18 years with a cytologically and/or histologically confirmed diagnosis of mRCC treated with first-line immuno-combination therapies were retrospectively included from 47 International Institutions from 16 countries. Patients were assessed for overall survival (OS), progression-free survival (PFS), and overall clinical benefit (OCB). Results: A total of 729 patients were included; tumor histology was clear-cell RCC in 86% of cases; 313 patients received dual immuno-oncology (IO + IO) therapy while 416 were treated with IO-tyrosine kinase inhibitor (IO + TKI) combinations. In the overall study population, the median OS and PFS were 36.5 and 15.0 months, respectively. The median OS was longer with IO+TKI compared with IO+IO therapy in the 616 patients with intermediate/poor International mRCC Database Consortium (IMDC) risk criteria (55.7 vs 29.7 months; p=0.045).
