1. Our Souls Look Back and Wonder: Reflections on Belonging and Being Invisible in Medicine

Abstract: Addressing the unequal impact of health disparities on historically marginalized communities is a top public health priority. Diversifying the work force has been lauded as key to addressing this challenge. Contributing to diversity in the workforce is the recruitment and retention of health professionals previously excluded and underrepresented in medicine. A major obstacle to retention, however, is the unequal way in which health professionals experience the learning environment. Through this perspective of 4 generations of physicians and medical students, the authors seek to highlight the similarities that have persisted over 40 years in the experiences of being underrepresented in medicine. Through a series of conversations and reflective writing, the authors reveal themes that spanned generations. Two common themes among the authors are the feeling of not belonging and feeling invisible. This is experienced in various aspects of medical education and academic careers. The lack of representation, unequal expectations, and over taxation contributes to the feeling of not belonging, leading to emotional, physical, and academic fatigue. Feeling invisible, yet paradoxically being hyper-visible, is also common. Despite the challenges, the authors conclude with a sense of hope for the future, if not for them, for the generations to come.

Abstract: Aim: Chemoresistance is a major cause of treatment failure in colorectal cancer (CRC) therapy. In this study, the impact of the IGF2BP family of RNA-binding proteins on CRC chemoresistance was investigated using in silico, in vitro, and in vivo approaches. Methods: Gene expression data from a well-characterized cohort and publicly available cross-linking immunoprecipitation sequencing (CLIP-Seq) data were collected. Resistance to chemotherapeutics was assessed in patient-derived xenografts (PDXs) and patient-derived organoids (PDOs). Functional studies were performed in 2D and 3D cell culture models, including proliferation, spheroid growth, and mitochondrial respiration analyses. Results: We identified IGF2BP2 as the most abundant IGF2BP in primary and metastastatic CRC, correlating with tumor stage in patient samples and tumor growth in PDXs. IGF2BP2 expression in primary tumor tissue was significantly associated with resistance to selumetinib, gefitinib, and regorafenib in PDOs and to 5-fluorouracil and oxaliplatin in PDX in vivo. IGF2BP2 knockout (KO) HCT116 cells were more susceptible to regorafenib in 2D and to oxaliplatin, selumitinib, and nintedanib in 3D cell culture. Further, a bioinformatic analysis using CLIP data suggested stabilization of target transcripts in primary and metastatic tumors. Measurement of oxygen consumption rate (OCR) and extracellular acidification rate (ECAR) revealed a decreased basal OCR and an increase in glycolytic ATP production rate in IGF2BP2 KO. In addition, real-time reverse transcriptase polymerase chain reaction (qPCR) analysis confirmed decreased expression of genes of the respiratory chain complex I, complex IV, and the outer mitochondrial membrane in IGF2BP2 KO cells. Conclusions: IGF2BP2 correlates with CRC tumor growth in vivo and promotes chemoresistance by altering mitochondrial respiratory chain metabolism. As a druggable target, IGF2BP2 could be used in future CRC therapy to overcome CRC chemoresistance.

Abstract: The COVID-19 pandemic has highlighted the need for FAIR (Findable, Accessible, Interoperable, and Reusable) data more than any other scientific challenge to date. We developed a flexible, multi-level, domain-agnostic FAIRification framework, providing practical guidance to improve the FAIRness for both existing and future clinical and molecular datasets. We validated the framework in collaboration with several major public-private partnership projects, demonstrating and delivering improvements across all aspects of FAIR and across a variety of datasets and their contexts. We therefore managed to establish the reproducibility and far-reaching applicability of our approach to FAIRification tasks.

Abstract: An outbreak of acute hepatitis of unknown aetiology in children was reported in Scotland 1 in April 2022 and has now been identified in 35 countries 2. Several recent studies have suggested an association with human adenovirus with this outbreak, a virus not commonly associated with hepatitis. Here we report a detailed case–control investigation and find an association between adeno-associated virus 2 (AAV2) infection and host genetics in disease susceptibility. Using next-generation sequencing, PCR with reverse transcription, serology and in situ hybridization, we detected recent infection with AAV2 in plasma and liver samples in 26 out of 32 (81%) cases of hepatitis compared with 5 out of 74 (7%) of samples from unaffected individuals. Furthermore, AAV2 was detected within ballooned hepatocytes alongside a prominent T cell infiltrate in liver biopsy samples. In keeping with a CD4+ T-cell-mediated immune pathology, the human leukocyte antigen (HLA) class II HLA-DRB1*04:01 allele was identified in 25 out of 27 cases (93%) compared with a background frequency of 10 out of 64 (16%; P = 5.49 × 10−12). In summary, we report an outbreak of acute paediatric hepatitis associated with AAV2 infection (most likely acquired as a co-infection with human adenovirus that is usually required as a ‘helper virus’ to support AAV2 replication) and disease susceptibility related to HLA class II status.

7. Revisiting the need to strengthen health systems in Africa for better mental health care delivery

Abstract: Background: A substantial amount of the world's burden of disease today comes from mental, neurological, and substance use disorders (MNS). In Africa, a high percentage of people are affected by MNS and this is attributed to several factors such as poor leadership and governance for health, inadequate access to and utilisation of health services, especially for mental health, myths associated with mental health, poor knowledge of mental health disorders, stigma towards mentally ill persons, and weak health systems. The weak health system across Africa greatly affects mental health care delivery. This article discusses the state of mental health in Africa. It argues that with the Coronavirus (COVID-19) pandemic which hit in 2020 the focus of many health systems in Africa has been on addressing the pandemic with less emphasis on mental health care delivery. Method: The article, using a WHO health systems framework, discusses useful ways to strengthen mental health systems in Africa for improved mental health care delivery. Results: The various aspects of the WHO health systems framework are used to explain how to strengthen mental health care delivery in Africa. Conclusion: Strengthening health systems in Africa for improved mental health care delivery using the WHO framework is possible and has benefits for Africa.&nbsp

8. Challenges in implementing GP Clusters in Scotland:a comparison of the views of senior primary care stakeholders in 2016 and 2021

Abstract: Background: Formation of GP clusters began in Scotland in April 2016 as part of a new Scottish GP contract. They aim to improve the care quality for local populations (intrinsic role) and the integration of health and social care (extrinsic role). Aims: To compare predicted challenges of cluster implementation in 2016 with reported challenges in 2021. Design and setting: Qualitative study of senior national stakeholders in primary care in Scotland.Methods: Qualitative analysis of semi-structured interviews with 12 senior primary care national stakeholders in 2016 (n=6) and 2021 (n=6).Results: Predicted challenges in 2016 included balancing intrinsic and extrinsic roles, providing sufficient support, maintaining motivation and direction, and avoiding variation between clusters. Progress of clusters in 2021 was perceived as suboptimal, and was reported to vary significantly across the country, reflecting differences in local infrastructure. Practical facilitation (data, administrative support, training, project improvement support, funded time) as well as strategic guidance from the Scottish Government, was felt to be lacking. GP engagement with clusters was felt to be hindered by the significant time and workforce pressures facing primary care. These barriers were considered as collectively contributing to cluster lead ‘burnout’ and loss of momentum, exacerbated by inadequate opportunities for shared learning between clusters across Scotland. Such barriers preceded, but were perpetuated by, the impact of the COVID-19 pandemic. Conclusion: Apart from the COVID-19 pandemic, many of the challenges reported by stakeholders in 2021 were predicted in 2016. Abstract: The Gemini High-resolution Optical SpecTrograph (GHOST) is the newest high resolution spectrograph to be developed for a large aperture telescope, recently deployed and commissioned at the Gemini-South telescope. In this paper, we present the first science results from the GHOST spectrograph taking during its commissioning runs. We have observed the bright metal-poor benchmark star HD 122563, along with two stars in the ultra faint dwarf galaxy, Ret II, one of which was previously identified as a candidate member, but did not have a previous detailed chemical abundance analysis. This star (GDR3 0928) is found to be a bona fide member of Ret II, and from a spectral synthesis analysis, it is also revealed to be a CEMP-r star, with significant enhancements in the several light elements (C, N, O, Na, Mg, and Si), in addition to featuring an r-process enhancement like many other Ret II stars. The light-element enhancements in this star resemble the abundance patterns seen in the CEMP-no stars of other ultra faint dwarf galaxies, and are thought to have been produced by an independent source from the r-process. 12. Primary Health Care Transformation in LMICs: A Scoping Review Protocol

Abstract: Primary health care (PHC) is a desirable orientation of health systems. It is an effective way to meet the population health needs. PHC received a major boost in 1978 at the declaration of Alma-Ata. Later, in 2018 the declaration of Astana brought with it the renewed commitment to PHC focusing on health and well-being for all. Many countries of the world have adopted primary healthcare in varying degrees and in a variety of ways. The specific actions of primary care transformation vary with context. A preliminary search for existing scoping reviews showed no such studies conducted for LMICs. Additionally, it is necessary to study the methods and approaches employed for transformation which has either resulted in a success or failure along with their causes. Hence this study will add to the existing knowledge of evidence based primary care.

13. 'Deeply Human, Fundamentally Social': Fascism and Internal Colonization in Badajoz Province during the Early Franco Dictatorship

Abstract: This article explores the ambition of the Franco regimes National Institute of Colonization (INC) to remake the countryside through irrigation and settlement. It focuses on the human side of this project, arguing that Spains colonization effort was fundamentally fascist in its aim to ensure the success of a nationalized and politically loyal segment of the rural population. It argues that the agricultural engineers of the INC inserted themselves as paternalistic authorities in the countryside, managing a carefully chosen portion of the peasantry. As the INC worked to begin its large-scale colonization schemes in the 1950s, it sought to determine which towns suffered from a social problem caused by the unemployment of tenants and small-property owners rather than landless laborers. In spite of the technical rhetoric of the engineers, with their stress on productivity and rationalizing land use, this article will demonstrate that certain political and moral considerations remained paramount throughout the process., Este artigo explora a ambição do Instituto Nacional de Colonização (INC) do regime franquista de refazer o campo através da irrigação e colonização. Centra-se no lado humano deste projecto, argumentando que o esforço de colonização de Espanha foi fundamentalmente fascista no seu objectivo de assegurar o sucesso de um segmento nacionalizado e politicamente leal da população rural. Argumenta que os engenheiros agrícolas do INC se inseriram como autoridades paternalistas no meio rural, gerindo uma porção cuidadosamente escolhida do campesinato. Enquanto o INC trabalhava para iniciar os seus esquemas de colonização em grande escala nos anos 50, procurou determinar que cidades sofriam de um "problema social" causado pelo desemprego dos inquilinos e dos pequenos donos de propriedades, em vez dos trabalhadores sem terra. The wizard not only brings the FAIRplus outcomes together and makes the Squads FAIRification learning experience more reusable. Abstract: OH/OD product state distributions arising from the reaction of gas-phase O(³P) atoms at the surface of the liquid hydrocarbon squalane C₃₀H₆₂/C₃₀D₆₂ have been measured. The O(³P) atoms were generated by 355 nm laser photolysis of NO₂ at a low pressure above the continually refreshed liquid. It has been shown unambiguously that the hydroxyl radicals detected by laser-induced fluorescence originate from the squalane surface. The gas-phase OH/OD rotational populations are found to be partially sensitive to the liquid temperature, but do not adapt to it completely. In addition, rotational temperatures for OH/OD(v′=1) are consistently colder (by 34±5 K) than those for OH/OD(v′=0). This is reminiscent of, but less pronounced than, a similar effect in the well-studied homogeneous gas-phase reaction of O(³P) with smaller hydrocarbons. We conclude that the rotational distributions are composed of two different components. One originates from a direct abstraction mechanism with product characteristics similar to those in the gas phase. The other is a trapping-desorption process yielding a thermal, Boltzmann-like distribution close to the surface temperature. This conclusion is consistent with that reached previously from independent measurements of OH product velocity distributions in complementary molecular-beam scattering experiments. It is further supported by the temporal profiles of OH/OD laser-induced fluorescence signals as a function of distance from the surface observed in the current experiments. The vibrational branching ratios for (v′=1)/(v′=0) for OH and OD have been found to be (0.07±0.02) and (0.30±0.10), respectively. The detection of vibrationally excited hydroxyl radicals suggests that secondary and/or tertiary hydrogen atoms may be accessible to the attacking oxygen atoms.

Abstract: We describe an experimental approach to the determination of the nascent internal state distribution of gas-phase products of a gas–liquid interfacial reaction. The system chosen for study is O(³P) atoms with the surface of liquid deuterated squalane, a partially branched long-chain saturated hydrocarbon, C₃₀D₆₂. The nascent OD products are detected by laser-induced fluorescence. Both OD (v′=0) and (v′=1) were observed in significant yield. The rotational distributions in both vibrational levels are essentially the same, and are characteristic of a Boltzmann distribution at a temperature close to that of the liquid surface. This contrasts with the distributions in the corresponding homogeneous gas-phase reactions. We propose a preliminary interpretation in terms of a dominant trapping-desorption mechanism, in which the OD molecules are retained at the surface sufficiently long to cause rotational equilibration but not complete vibrational relaxation. The significant yield of vibrationally excited OD also suggests that the surface is not composed entirely of –CD₃ endgroups, but that secondary and/or tertiary units along the backbone are exposed.

Abstract: Background: A better understanding of attitudes and beliefs held by traditional healers and utilizers of traditional medicine concerning mental health conditions in Liberia is important as Liberia seeks to improve its delivery of mental healthcare in the context of scarce resources and recovery from civil war. Methods: A qualitative research design was used to collect data from 24 Liberian traditional healers, and 11 utilizers of Liberian traditional medicine. Participants were queried about mental health problems in Liberia, treatments, and attitudes towards modern healthcare. Qualitative data were probed and aggregated using content analysis. Results: Mental health problems described by study participants included: Open Mole, African Science, Epilepsy, Depres- sion and Mental Illness (trauma/substance use). Mental health problems were often associated with socioeconomic distress, and participants described their attitudes and beliefs concerning mental healthcare, traditional medicine, and modern health- care. Conclusion: Traditional medicine is an important part of mental healthcare in Africa. Mental illness, social factors, and healthcare access were important problems in Liberia. Mental health problems blended local cultural beliefs with Western- ized nosology and social factors. Traditional healer’s attitudes towards Western medicine reflected ambivalence. There is a desire for collaboration with ‘modern’ health care providers, but this will require reciprocal trust-building. Abstract: FAIRplus seeks to establish ‘FAIRification’ processes that can be used at scale to ensure FAIRness of IMI data.The goal of the FAIRplus Maturity Framework is to help the IMI office, IMI projects, and EFPIA partners analyse and plan their advancement in creation and maintenance of FAIR data. To this end, the work package aims to deliver two maturity models. One is a Data Management Maturity Model which focuses on ensuring that processes in place support the creation of new FAIR datasets, the second is the Dataset Maturity Model which is regarding the FAIRness of existing datasets. These two maturity models fit into the updated FAIRplus FAIRification process; the Dataset Maturity Model supports the data requirements task while the Data Management Maturity Model supports the task to identify FAIRification capabilities and resources. These models will be aligned with and based on the RDA FAIR indicators. However, before including them, an analysis was done on their suitability for use with human data as well as their overall clarity. This led to the creation of FAIR Assessment Indicators on which the Maturity Models will be built.

Abstract: Aims:\ud The safety and efficacy of the novel selective cardiac myosin activator, omecamtiv mecarbil, in patients with heart failure with reduced ejection fraction (HFrEF) is tested in the Global Approach to Lowering Adverse Cardiac outcomes Through Improving Contractility in Heart Failure (GALACTIC‐HF) trial. Here we describe the baseline characteristics of participants in GALACTIC‐HF and how these compare with other contemporary trials.\ud \ud Methods and Results:\ud Adults with established HFrEF, New York Heart Association functional class (NYHA) ≥ II, EF ≤35%, elevated natriuretic peptides and either current hospitalization for HF or history of hospitalization/ emergency department visit for HF within a year were randomized to either placebo or omecamtiv mecarbil (pharmacokinetic‐guided dosing: 25, 37.5 or 50 mg bid). 8256 patients [male (79%), non‐white (22%), mean age 65 years] were enrolled with a mean EF 27%, ischemic etiology in 54%, NYHA II 53% and III/IV 47%, and median NT‐proBNP 1971 pg/mL. HF therapies at baseline were among the most effectively employed in contemporary HF trials. Abstract: Recognizing a need for more guidance on the SARS-CoV-2 (COVID-19) pandemic, members of the Archives of Physical Medicine and Rehabilitation Editorial Board invited several clinicians with early experience managing the disease to collaborate on a document to help guide rehabilitation clinicians in the community. This consensus document is written in a “question and answer” format and contains information on the following items: common manifestations of the disease; rehabilitation recommendations in the acute hospital setting, recommendations for inpatient rehabilitation and special considerations. These suggestions are intended for use by rehabilitation clinicians in the inpatient setting caring for patients with confirmed or suspected COVID-19. The text represents the authors’ best judgment at the time it was written. However, our knowledge of COVID-19 is growing rapidly. The reader should take advantage of the most up to date information when making clinical decisions.

The FAIRplus KPI Dashboard enables tracking the progress of the FAIRplus project consortium in the following areas: (1) FAIRification of the datasets from selected IMI projects, (2) Development of the FAIR cookbook, (3) Content and usage of the IMI FAIR Data Catalogue, and (4) Impact of the FAIR SME and Innovation Forums. Abstract: Exposure to hepatitis viruses is a recognized occupational risk for health care personnel (HCP). This report establishes new CDC guidance that includes recommendations for a testing algorithm and clinical management for HCP with potential occupational exposure to hepatitis C virus (HCV). Baseline testing of the source patient and HCP should be performed as soon as possible (preferably within 48 hours) after the exposure. A source patient refers to any person receiving health care services whose blood or other potentially infectious material is the source of the HCPs exposure. Two options are recommended for testing the source patient. The first option is to test the source patient with a nucleic acid test (NAT) for HCV RNA. This option is preferred, particularly if the source patient is known or suspected to have recent behaviors that increase risk for HCV acquisition (e.g., injection drug use within the previous 4 months) or if risk cannot be reliably assessed. The second option is to test the source patient for antibodies to hepatitis C virus (anti-HCV), then if positive, test for HCV RNA. For HCP, baseline testing for anti-HCV with reflex to a NAT for HCV RNA if positive should be conducted as soon as possible (preferably within 48 hours) after the exposure and may be simultaneous with source-patient testing. If follow-up testing is recommended based on the source patients status (e.g., HCV RNA positive or anti-HCV positive with unavailable HCV RNA or if the HCV infection status is unknown), HCP should be tested with a NAT for HCV RNA at 3-6 weeks postexposure. If HCV RNA is negative at 3-6 weeks postexposure, a final test for anti-HCV at 4-6 months postexposure is recommended. A source patient or HCP found to be positive for HCV RNA should be referred to care. Postexposure prophylaxis of hepatitis C is not recommended for HCP who have occupational exposure to blood and other body fluids. This guidance was developed based on expert opinion (CDC. Updated U.S. Public Health Service guidelines for the management of occupational exposures to HBV, HCV, and HIV and recommendations for postexposure prophylaxis. Health care providers can use this guidance to update their procedures for postexposure testing and clinical management of HCP potentially exposed to hepatitis C virus.

Abstract: Key points - As with other parts of the UK, COVID-19 has caused a significant increase in deaths in Scotland, particularly amongst older individuals. - Scotland’s care home sector has not expanded in response to demographic change: rather the focus of care provision has moved to care at home. - Many of the characteristics of the care home sector in Scotland are similar to those in the rest of the UK. - The COVID-19 epidemic has spread to the majority of Scotland’s care homes. - The impact of COVID-19 on deaths in care homes lagged those in hospitals but have now surpassed deaths in all other settings. - Although the total number of deaths is now declining, the share of care home deaths in the total continues to increase. - Excess mortality during the pandemic has been high in all settings in Scotland, but has been particularly high in care homes. - Non-COVID deaths in hospital settings have declined during the pandemic, which may be the result of re-orienting hospital activity towards dealing with the immediate crisis. Increased deaths in other settings, including care homes, may have been the consequence. - Whereas care homes have been particularly affected by COVID-19, there has also been significant excess deaths attributed to causes other than COVID-19 outside hospitals and care homes. Specifically, there have been 616 non-COVID “excess deaths” in care homes and 1,320 such deaths outside care homes and hospitals. Given the age profile of deaths, these are likely to have been concentrated among the oldest old. - Scotland, unlike England, does not report the number of deaths of care home residents who die in hospital and elsewhere. If the shares of such deaths are similar across both jurisdictions, then the number of care home resident deaths in Scotland attributable to COVID-19-would be significantly larger.

Two thirds of projects to be addressed by FAIRplus will cover societal priorities of H2020, namely; 1) promoting healthy ageing[1]; 2) addressing chronic diseases; 3) neurodegenerative diseases and 4) emergence of antibiotic resistance. In addition, we have identified cross-cutting projects “Cross” which provide toolssuch as cell lines, biomarkers and animal models, which enable research progression in the primary priority areas. Such patients are considered at very high risk (VHR) based on a history of >1 major atherosclerotic cardiovascular disease (ASCVD) event or a single ASCVD event and multiple high-risk conditions. We investigated the association of US guideline-defined risk categories with the occurrence of ischemic events after acute coronary syndrome and reduction of those events by alirocumab, a PCSK9 (proprotein convertase subtilisin/kexin type 9) inhibitor. Methods: in the ODYSSEY OUTCOMES trial (Evaluation of Cardiovascular Outcomes After an Acute Coronary Syndrome During Treatment With Alirocumab), patients with recent acute coronary syndrome and residual dyslipidemia despite optimal statin therapy were randomly assigned to alirocumab or placebo. the primary trial outcome (major adverse cardiovascular events, ie, coronary heart disease death, nonfatal myocardial infarction, ischemic stroke, or hospitalization for unstable angina) was examined according to American College of Cardiology/American Heart Association risk category. Results: of 18 924 participants followed for a median of 2.8 years, 11 935 (63.1%) were classified as VHR: 4450 (37.3%) had multiple prior ASCVD events and 7485 (62.7%) had 1 major ASCVD event and multiple high-risk conditions. Major adverse cardiovascular events occurred in 14.4% of placebo-treated patients at VHR versus 5.6% of those not at VHR. in the VHR category, major adverse cardiovascular events occurred in 20.4% with multiple prior ASCVD events versus 10.7% with 1 ASCVD event and multiple high-risk conditions. Alirocumab was associated with consistent relative risk reductions in both risk categories (hazard ratio=0.84 for VHR; hazard ratio=0.86 for not VHR; P-interaction=0.820) and by stratification within the VHR group (hazard ratio=0.86 for multiple prior ASCVD events; hazard ratio=0.82 for 1 major ASCVD event and multiple high-risk conditions; P-interaction=0.672). the absolute risk reduction for major adverse cardiovascular events with alirocumab was numerically greater (but not statistically different) in the VHR group versus those not at VHR (2.1% versus 0.8%; P-interaction=0.095) and among patients at VHR with multiple prior ASCVD events versus a single prior ASCVD event (2.4% versus 1.8%; P-interaction=0.661). Abstract: Meticulous study design, exacting scientific review, scrupulous data management, rigorous human subjects' protection, and effective recruitment strategies are essential components of all clinical research studies that must be in place prior to the conduct of the study. Equally important to the success of the study and to the well-being and protection of study participants is the assurance that safe clinical environments are provided for the care of research participants. This chapter describes approaches to managing patient care and clinical research quality, as well as the application of established health care-based clinical quality methods and tools to the complex processes of clinical research. Techniques for identifying clinical risks, tools for collecting data about the quality of clinical care processes, strategies for mitigating risk and patient harm, and techniques for measuring the participants' perceptions of the research process are discussed in detail.

Abstract: BACKGROUND: Patients with acute coronary syndrome (ACS) and concomitant noncoronary atherosclerosis have a high risk of major adverse cardiovascular events (MACEs) and death. The impact of lipid lowering by proprotein convertase subtilisin-kexin type 9 inhibition in such patients is undetermined. OBJECTIVES: This pre-specified analysis from ODYSSEY OUTCOMES (Evaluation of Cardiovascular Outcomes After an Acute Coronary Syndrome During Treatment With Alirocumab) determined whether polyvascular disease influenced risks of MACEs and death and their modification by alirocumab in patients with recent ACS and dyslipidemia despite intensive statin therapy. METHODS: Patients were randomized to alirocumab or placebo 1 to 12 months after ACS. The primary MACEs endpoint was the composite of coronary heart disease death, nonfatal myocardial infarction, fatal or nonfatal ischemic stroke, or unstable angina requiring hospitalization. All-cause death was a secondary endpoint. RESULTS: Median follow-up was 2.8 years. Of 18,924 patients, 17,370 had monovascular (coronary) disease, 1,405 had polyvascular disease in 2 beds (coronary and peripheral artery or cerebrovascular), and 149 had polyvascular disease in 3 beds (coronary, peripheral artery, cerebrovascular). With placebo, the incidence of MACEs by respective vascular categories was 10.0%, 22.2%, and 39.7%. With alirocumab, the corresponding absolute risk reduction was 1.4% (95% confidence interval [CI]: 0.6% to 2.3%), 1.9% (95% CI: -2.4% to 6.2%), and 13.0% (95% CI: -2.0% to 28.0%). With placebo, the incidence of death by respective vascular categories was 3.5%, 10.0%, and 21.8%; the absolute risk reduction with alirocumab was 0.4% (95% CI: -0.1% to 1.0%), 1.3% (95% CI: -1.8% to 4.3%), and 16.2% (95% CI: 5.5% to 26.8%). CONCLUSIONS: In patients with recent ACS and dyslipidemia despite intensive statin therapy, polyvascular disease is associated with high risks of MACEs and death. The large absolute reductions in those risks with alirocumab are a potential benefit for these patients. Abstract: Introduction:\ud Linked health care datasets have been used effectively in Scotland for some time. Use of social care data has been much more limited, partly because responsibility for these services is distributed across multiple local authorities. However, there are substantial interactions between health and social care (also known internationally as long-term care) services, and keen policy interest in better understanding these. We introduce two social care resources that can now be linked to health datasets at a population level across Scotland to study these interdependencies. These data emerge from the Scottish Government’s centralised collation of data from mandatory returns provided by local authorities and care homes.\ud \ud Methods:\ud Deterministic and Probabilistic methods were used to match the Social Care Survey (SCS) and Scottish Care Home Census (SCHC) to the Community Health Index (CHI) number via the National Records of Scotland (NRS) Research Indexing Spine.\ud \ud Results:\ud For the years 2010/11 to 2015/16, an overall match rate of 91.2% was achieved for the SCS to CHI from 31 of Scotland’s 32 local authority areas. This rate varied from 76.7% to 98.5% for local authority areas. A match rate of 89.8% to CHI was achieved for the SCHC in years 2012/13 to 2015/16 but only 52.5% for the years 2010/11 to 2011/12.\ud \ud Conclusion:\ud Indexing of the SCS and SCHC to CHI offers a new and rich resource of data for health and social care research.

Abstract: This chapter describes the assurance view of testimonial knowledge and warrant and consider a significant challenge to the view. An important but infrequently discussed variant of the assurance view holds that assurance is closely tied to assertion itself and the norms governing assertion. The assurance view is an epistemological position regarding testimonial knowledge and warrant. Krista Lawlor develops an account of assurance that is helpful in understanding what its particular character might be, as contrasted with ordinary assertion. For the assurance view, a role must be given to the assurance itself—as Lawlor understands it, the claim to knowledge—in the practice of testimony, and this role must be sufficient, under the right conditions, to provide knowledge in the audience. A central goal of the assurance view is to provide an account of what is distinctive about testimonial warrant and knowledge. Testimonial assurance is given to a specific person or persons, the audience, who must recognize it as being given to them.

Abstract: OBJECTIVE: Individuals with schizophrenia experience increased lung cancer mortality and decreased access to cancer screening and tobacco cessation. Screening promotes early lung cancer detection and can decrease mortality among high-risk populations, yet this screening remains underutilized. To promote screening among individuals with schizophrenia we need to investigate the proportion who meet screening criteria, and examine smoking behaviors, cancer risk perceptions, receipt of primary care and tobacco cessation interventions. METHODS: A cross-sectional survey and medical record review of 112 adults with schizophrenia treated with clozapine in a CMHC. RESULTS: Among older participants (aged 55–77) with schizophrenia, 34% met criteria for screening based on smoking history (heavy current and former smokers); over half believed they had a low risk of developing lung cancer. 88% of all participants visited their PCP in the past year; PCPs represented 35 different practices. Among all current smokers, only 1 in 3 reported that their PCPs and/or psychiatrists assisted with medications for tobacco cessation. CONCLUSIONS: Older adults with schizophrenia are at increased risk for lung cancer and eligible for lung screening, and yet most participants underestimated their lung cancer risk. While participants regularly accessed care, PCP and psychiatric visits may be missed opportunities to engage patients with schizophrenia in tobacco cessation and decrease preventable premature mortality. Embedding interventions in a CMHC, a centralized access point of care delivery for patients with schizophrenia, may have unique potential to increase uptake of screening and tobacco cessation, and thereby contribute to closing the cancer mortality gap for patients with schizophrenia.

Abstract: Candidate Pilot IMI projects (eTOX, ND4BB-TRANSLOCATION, ReSOLUTE, Aetionomy, OncoTrack and OPENPhacts) were initially identified based upon a long list highlighted in the DoA and through the FAIRplus partner network. Summaries of the activities and data sets available within these candidate Pilot IMI projects were collected via a survey tool, and further information was presented at the FAIRplus kick-off event by project owners. Following review, WP1 prioritized 4 IMI projects as Pilots, (eTOX, ND4BB-TRANSLOCATION, ReSOLUTE and OncoTrack). The prioritized Pilots were ratified by the General Assembly and formally selected. Each project was then assigned a key FAIRplus contact person, who worked with the data owners to identify, prioritise and release suitable data sets to the FAIRplus consortium, Squads and WP2 / 3 members. As of 30.06.2019, data sets have been released from eTOX, ND4BB-TRANSLOCATION and ReSOLUTE. OncoTrack is on course to release data in July 2019.

Abstract: BACKGROUND: The ODYSSEY OUTCOMES (Evaluation of Cardiovascular Outcomes After an Acute Coronary Syndrome During Treatment With Alirocumab) trial compared alirocumab with placebo, added to high-intensity or maximum-tolerated statin treatment, after acute coronary syndrome (ACS) in 18,924 patients. Alirocumab reduced the first occurrence of the primary composite endpoint and was associated with fewer all-cause deaths. OBJECTIVES: This pre-specified analysis determined the extent to which alirocumab reduced total (first and subsequent) nonfatal cardiovascular events and all-cause deaths in ODYSSEY OUTCOMES. METHODS: Hazard functions for total nonfatal cardiovascular events (myocardial infarction, stroke, ischemia-driven coronary revascularization, and hospitalization for unstable angina or heart failure) and death were jointly estimated, linked by a shared frailty accounting for patient risk heterogeneity and correlated within-patient nonfatal events. An association parameter also quantified the strength of the linkage between risk of nonfatal events and death. The model provides accurate relative estimates of nonfatal event risk if nonfatal events are associated with increased risk for death. RESULTS: With 3,064 first and 5,425 total events, 190 fewer first and 385 fewer total nonfatal cardiovascular events or deaths were observed with alirocumab compared with placebo. Alirocumab reduced total nonfatal cardiovascular events (hazard ratio: 0.87; 95% confidence interval: 0.82 to 0.93) and death (hazard ratio: 0.83; 95% confidence interval: 0.71 to 0.97) in the presence of a strong association between nonfatal and fatal event risk. CONCLUSIONS: In patients with ACS, the total number of nonfatal cardiovascular events and deaths prevented with alirocumab was twice the number of first events prevented. 50. The African Global Mental Health Institute: Increasing capacity, inclusivity and translation of psychiatric research and care

Abstract: The largest treatment gap for mental, neurological and substance use (MNS) disorders to date exists in Sub-Saharan Africa (e.g., South Africa, Malawi, Nigeria). While efforts have been made to reduce the global burden of MNS disorders, there remain significant barriers to delivery of equitable mental health care in Africa and the diaspora. These barriers are deeply rooted in a lack of research structures to inform care delivery, few outlets to train in-country mental health professionals, and limited involvement of policymakers in translation of research findings. Given the demonstrated need across domains of research, education, service delivery and policy in Africa, it is in the purview of global leaders and mental health professionals to build the infrastructure necessary to make systematic, targeted strides to develop each of these areas. With this conceptual framework in mind, over fifty global leaders, psychiatrists, mental health professionals and advocates with expertise in cultural psychiatry convened at a global conference in Cape Town, South Africa in 2016 to establish the African Global Mental Health Institute (AGMHI). The AGMHI is an organizational effort that aims to reduce the global burden of mental illness through research, education and training, service delivery, and policy. Aligned with mental health initiatives posited by the World Health Organization (WHO) and the World Bank, the AGMHI represents an opportunity to explore and understand cultural nuance among populations in Africa and the diaspora. This article focuses on the AGMHI’s targeted strategies in the following actions: 1) bolstering training and education programs for clinicians and/or researchers in Africa and the diaspora to increase human capacity for mental health; 2) conducting and disseminating inclusive, culturally-relevant research (e.g., research that involves community stakeholders in its design and conduct) to meet the urgent need for a culturally-relevant evidence base; 3) identifying key stakeholders to promote sustainable mental health care and inform policy; and 4) fostering local and global collaborations (e.g., engaging local and national governments, non-governmental organizations (NGO), academic institutions, etc.) to share methods of improving access to mental health services. In concert with empirical data, these strategies were devised by members of the AGMHI, which is comprised of international leaders with decades of expertise in global psychiatry.
