Objectives: The purpose is to apply a previously validated deep learning algorithm to a new thyroid nodule ultrasound image dataset and compare its performances with radiologists. Methods: Prior study presented an algorithm which is able to detect thyroid nodules and then make malignancy classifications with two ultrasound images. A multi-task deep convolutional neural network was trained from 1278 nodules and originally tested with 99 separate nodules. The results were comparable with that of radiologists. The algorithm was further tested with 378 nodules imaged with ultrasound machines from different manufacturers and product types than the training cases. Four experienced radiologists were requested to evaluate the nodules for comparison with deep learning. Results: The Area Under Curve (AUC) of the deep learning algorithm and four radiologists were calculated with parametric, binormal estimation. For the deep learning algorithm, the AUC was 0.69 (95% CI: 0.64 - 0.75). The AUC of radiologists were 0.63 (95% CI: 0.59 - 0.67), 0.66 (95% CI:0.61 - 0.71), 0.65 (95% CI: 0.60 - 0.70), and 0.63 (95%CI: 0.58 - 0.67). Conclusion: In the new testing dataset, the deep learning algorithm achieved similar performances with all four radiologists. The relative performance difference between the algorithm and the radiologists is not significantly affected by the difference of ultrasound scanner., Comment: Clinical Imaging (2023)

RESEARCH QUESTION: Is there an association between FSHR sequence variants and reproductive outcomes following IVF in predicted normoresponders? DESIGN: Multicentre prospective cohort study conducted from November 2016 to June 2019 in Vietnam, Belgium and Spain including patients aged G, c.2039A>G, c.-29G>A) and one FSHB sequence variants (c.-211G>T). Clinical pregnancy rate (CPR), live birth rate (LBR) and miscarriage rate in the first embryo transfer and cumulative live birth rate (CLBR) were compared between the different genotypes. RESULTS: A total of 351 patients underwent at least one embryo transfer. Genetic model analysis that adjusted for patient age, body mass index, ethnicity, type of embryo transfer, embryo stage and number of top-quality embryos transferred revealed a higher CPR for homozygous patients for the variant allele G of c.919A>G when compared to patients with genotype AA (60.3% versus 46.3%, adjusted odds ratio [ORadj] 1.96, 95% confidence interval [CI] 1.09-3.53). Also, c.919A>G genotypes AG and GG presented a higher CPR and LBR when compared with genotype AA (59.1% versus 46.3%, ORadj 1.80, 95% CI 1.08-3.00, and 51.3% versus 39.0%, ORadj 1.69, 95% CI 1.01-2.80, respectively). Cox regression models revealed a statistically significantly lower CLBR for c.2039A>G genotype GG in the codominant model (hazard ratio [HR] 0.66, 95% CI 0.43-0.99). CONCLUSION: These results demonstrate a previously unreported association between variant c.919A>G genotype GG and higher CPR and LBR in infertile patients and reinforce a potential role for genetic background in predicting the reproductive prognosis following IVF.

In 2019, a community-based, cross-sectional carriage survey and a seroprevalence survey of 1,216 persons 1–55 years of age were conducted in rural Vietnam to investigate the mechanism of diphtheria outbreaks. Seroprevalence was further compared with that of an urban area that had no cases reported for the past decade. Carriage prevalence was 1.4%. The highest prevalence, 4.5%, was observed for children 1–5 years of age. Twenty-seven asymptomatic Coerynebacterium diphtheriae carriers were identified; 9 carriers had tox gene–bearing strains, and 3 had nontoxigenic tox gene–bearing strains. Child malnutrition was associated with low levels of diphtheria toxoid IgG, which might have subsequently increased child carriage prevalence. Different immunity patterns in the 2 populations suggested that the low immunity among children caused by low vaccination coverage increased transmission, resulting in symptomatic infections at school-going age, when vaccine-induced immunity waned most. Recently, text-guided image manipulation has received increasing attention in the research field of multimedia processing and computer vision due to its high flexibility and controllability. Its goal is to semantically manipulate parts of an input reference image according to the text descriptions. However, most of the existing works have the following problems: (1) text-irrelevant content cannot always be maintained but randomly changed, (2) the performance of image manipulation still needs to be further improved, (3) only can manipulate descriptive attributes. To solve these problems, we propose a novel image manipulation method that interactively edits an image using complex text instructions. It allows users to not only improve the accuracy of image manipulation but also achieve complex tasks such as enlarging, dwindling, or removing objects and replacing the background with the input image. To make these tasks possible, we apply three strategies. First, the given image is divided into text-relevant content and text-irrelevant content. Only the text-relevant content is manipulated and the text-irrelevant content can be maintained. Second, a super-resolution method is used to enlarge the manipulation region to further improve the operability and to help manipulate the object itself. Third, a user interface is introduced for editing the segmentation map interactively to re-modify the generated image according to the user's desires. Extensive experiments on the Caltech-UCSD Birds-200-2011 (CUB) dataset and Microsoft Common Objects in Context (MS COCO) datasets demonstrate our proposed method can enable interactive, flexible, and accurate image manipulation in real-time. Supplemental Material for The association between intraoperative cardiopulmonary bypass power and complications after cardiac surgery by Victor Hui, Kwok M Ho, Rebecca Hahn, Robert Larbalestier, Brian Wright and Warren Pavey by Perfusion

The application of an external 26 Tesla axial magnetic field to a D_{2} gas-filled capsule indirectly driven on the National Ignition Facility is observed to increase the ion temperature by 40% and the neutron yield by a factor of 3.2 in a hot spot with areal density and temperature approaching what is required for fusion ignition [1]. The improvements are determined from energy spectral measurements of the 2.45 MeV neutrons from the D(d,n)^{3}He reaction, and the compressed central core B field is estimated to be ∼4.9 kT using the 14.1 MeV secondary neutrons from the D(T,n)^{4}He reactions. The experiments use a 30 kV pulsed-power system to deliver a ∼3 μs current pulse to a solenoidal coil wrapped around a novel high-electrical-resistivity AuTa_{4} hohlraum. Radiation magnetohydrodynamic simulations are consistent with the experiment.

PURPOSE: ICU discharge is often delayed by a requirement for intravenous vasopressor medications to maintain normotension. We hypothesised that the administration of midodrine, an oral α(1)-adrenergic agonist, as adjunct to standard treatment shortens the duration of intravenous vasopressor requirement. METHODS: In this multicentre, randomised, controlled trial including three tertiary referral hospitals in the US and Australia, we enrolled adult patients with hypotension requiring a single-agent intravenous vasopressor for ≥24 hours. Subjects received oral midodrine (20 mg) or placebo every 8 hours in addition to standard care until cessation of intravenous vasopressors, ICU discharge, or occurrence of adverse events. The primary outcome was time to vasopressor discontinuation. Secondary outcomes included time to ICU discharge readiness, ICU and hospital lengths of stay, and ICU readmission rates. RESULTS: Between October 2012 and June 2019, 136 participants were randomised, of whom 132 received the allocated intervention and were included in the analysis (modified intention-to-treat approach). Time to vasopressor discontinuation was not different between midodrine and placebo groups (median [IQR], 23.5 [10.0–54.0] vs 22.5 [10.4–40.0] hours; difference, 1.0 hour; 95% CI, −10.4 to 12.3 hours; p=0.62). No differences in secondary endpoints were observed. Bradycardia occurred more often after midodrine administration (5 [7.6%] vs 0 [0%], p=0.02). CONCLUSION: Midodrine did not accelerate liberation from intravenous vasopressors and was not effective for the treatment of hypotension in critically ill patients. We study a mass formula for Artin--Schreier curves of genus $g$ defined over a finite field $k$ of characteristic $p$. For an odd prime $p$ and for small $g$, we determine the number of $k$-isomorphism classes of Artin-Schreier curves of genus $g$, weighted by the order of the centralizer of the Artin-Schreier involution in the automorphism group. Background : Since the earliest use of microscopes in surgery, several modifications have been made to improve the ergonomics of movement and posture, increase the resolution, and expand the visual field. The three-dimensional microscopes are latest innovation in this field. Despite the wider use of 3D microscopes in other specialities, their use in the reconstructive microsurgery in the United Kingdom is still limited. Reconstructive microsurgeons are highly skilled surgeons that are trained to operate utilising the microscope for long hours. This poses an occupational risk with a specific pattern of work-related conditions. Aims and methods : We aim in this report to demonstrate our experience utilising 3D microscopes in small cohort of patients matched to a control of patients operated utilising the traditional microscopes. Patients were matched by age, comorbidities, oncologic procedures. This was complemented by a survey completed by the operating surgeons. Results : Nine patients were included in each group. There was no significant difference in operative or ischemia time and no significant post-operative complications in both groups. The surgeons reported better ergonomics, improved staff engagement, and a better teaching experience when utilising the 3D microscopes compared to traditional microscopes. Conclusion : The utilisation of the 3D microscopes in reconstructive microsurgery has shown to provide comfort, improve ergonomics of movement and posture without significant clinical implications in this series.

The discovery of the 1144-phase, e.g. CaKFe$_4$As$_4$, creates opportunities to build novel intermetallics with alternative stacking of two parent compounds. Here we formalize the idea by defining a class of bulk crystalline solids with $A$-$B$ stacking (including 1144-phases and beyond), which is a generalization of hetero-structures from few-layer or thin-film semi-conductors to bulk intermetallics. Theoretically, four families of phosphides \textit{AB}(TM)$_4$P$_4$ (TM=Fe, Ru, Co, Ni) are investigated by first-principles calculations, wherein configurational, vibrational and electronic degrees of freedom are considered. It predicts a variety of stable 1144-phases (especially Ru- and Fe-phosphides). Stability rules are found and structural/electronic properties are discussed. Experimentally, we synthesize high-purity CaKRu$_4$P$_4$ as a proof of principle example. The synthetic method is simple and easily applied. Moreover, it alludes to a strategy to explore complex multi-component compounds, facilitated by a phase diagram coordinated by collective descriptors., 15 pages, 9 figures

Molecular dynamics simulations using an interatomic potential developed by artificial neural network deep machine learning are performed to study the local structural order in Al90Tb10 metallic glass. We show that more than 80% of the Tb-centered clusters in Al90Tb10 glass have short-range order (SRO) with their 17 first coordination shell atoms stacked in a '3661' or '15551' sequence. Medium-range order (MRO) in Bergman-type packing extended out to the second and third coordination shells is also clearly observed. Analysis of the network formed by the '3661' and '15551' clusters show that ~82% of such SRO units share their faces or vertexes, while only ~6% of neighboring SRO pairs are interpenetrating. Such a network topology is consistent with the Bergman-type MRO around the Tb-centers. Moreover, crystal structure searches using genetic algorithm and the neural network interatomic potential reveal several low-energy metastable crystalline structures in the composition range close to Al90Tb10. Some of these crystalline structures have the '3661' SRO while others have the '15551' SRO. While the crystalline structures with the '3661' SRO also exhibit the MRO very similar to that observed in the glass, the ones with the '15551' SRO have very different atomic packing in the second and third shells around the Tb centers from that of the Bergman-type MRO observed in the glassy phase.

We report on an ultrafast photoinduced phase transition with a strikingly long-lived Martensitic anomaly driven by above-threshold single-cycle terahertz (THz) pulses in Nb$_3$Sn. A non-thermal, THz-induced depletion of low frequency conductivity indicates increased gap splitting of high energy $\Gamma_{12}$ bands by removal of their degeneracies which enhances the Martensitic phase. In contrast, optical pumping leads to a $\Gamma_{12}$ gap melting. Such light-induced non-equilibrium Martensitic instability persists up to a critical temperature $\sim$100 K, i.e., more than twice the equilibrium temperature, and can be stabilized beyond technologically-relevant, nanosecond timescales. Together with first-principle simulations, we identify a compelling THz tuning of structural fluctuations via E$_u$ phonons to achieve a non-equilibrium ordering at high temperatures far exceeding those for equilibrium states.

Sex hormones play an important role in establishing sex‐distinctive brain structural and functional variations that could contribute to the sex differences in alcohol consumption behavior. Here, we systematically reviewed articles that studied sex hormone impacts on alcohol consumption and alcohol use disorder (AUD). An extensive literature search conducted in MEDLINE, PubMed, Scopus and CINAHL databases identified 776 articles, which were then evaluated for pre‐specified criteria for relevance and quality assurance. A total of 50 articles, including 19 human studies and 31 animal studies, were selected for this review. Existing evidence supports the association of increased testosterone level and increased risk for alcohol use and AUD in males but results are inconclusive in females. In contrast, the evidence supports the association of increased estrogen level and increased alcohol use in females, with mixed findings reported in males. Much less is known about the impact of progestins on alcohol use and misuse in human subjects. Future observational and experimental studies conducted in both sexes with a comprehensive hormone panel are needed to elucidate the impact of the interplay between various sex hormone levels during various developmental stages on alcohol use‐related phenotypes and AUD.

The coherence of collective modes, such as phonons, and their modulation of the electronic states are long sought in complex systems, which is a cross-cutting issue in photovoltaics and quantum electronics. In photovoltaic cells and lasers based on metal halide perovskites, the presence of polaronic coupling, i.e., photocarriers dressed by the macroscopic motion of charged lattice, assisted by terahertz (THz) longitudinal optical (LO) phonons, has been intensely studied yet still debated. This may be key for explaining the remarkable properties of the perovskite materials, e.g., defect tolerance, long charge lifetimes and diffusion length. Here we use the intense single-cycle THz pulse with the peak electric field up to $E_{THz}=$1000\,kV/cm to drive coherent band-edge oscillations at room temperature in CH$_3$NH$_3$PbI$_3$. We reveal the oscillatory behavior dominantly to a specific quantized lattice vibration mode at $\omega_{\mathrm{LO}}\sim$4 THz, being both dipole and momentum forbidden. THz-driven coherent dynamics exhibits distinguishing features: the room temperature coherent oscillations at $\omega_{\mathrm{LO}}$ longer than 1 ps in both single crystals and thin films; the {\em mode-selective} modulation of different band edge states assisted by electron-phonon ($e$-$ph$) interaction; {\em dynamic mode splitting} controlled by temperature due to entropy and anharmonicity of organic cations. Our results demonstrate intense THz-driven coherent band-edge modulation as a powerful probe of electron-lattice coupling phenomena and provide compelling implications for polaron correlations in perovskites.

Purpose of this paper is to identify the tendency of shrimp value chain development and impact of its governance on the small scale shrimp farmers in Vietnam. Design/methodology/approach - Data from the shrimp farmers surveys in Mekong delta, Vietnam from 2008-2010 with the update information in 2014 were taken to analyse by the value chain analysis method. Findings - Traditional governance type of the shrimp value chain in the early state (before 2004) showed the different levels of coordination of farmers with collectors, among collectors, and collectors with processing plants. In this type of governance, trust and linkages are inextricably linked. However, they are not strong. The processing plants determine shrimp prices and quality requirement in the market while many collectors do not seem to be highly responsible for the quality of their products. To avoid this limitation, with the governmental support policy to improve farmers' income, the processing plants set up a direct buying from farmers under contracts. These contracts led to a new governance type with an expectation of improving farmers' position. However, this model was broken due to several reasons including un-controlled shrimp raw material from small scale and individual farmers. Consequently, processors now tend to establish their own raw material zone to comply shrimp quality assurance, and eject the existence of farmers. This will lead small scale farmers to very difficult problems in finding the market. Poverty and social problems of small scale farmers might appear. The result recommends a greater strengthening and tightening of the value chain. Re-organizing shrimp farmers into legal teams or groups that help farmers to re-participate in the game with others actor in the chain is crucial. Research limitations/implications - The research mainly follows inductive approach in which qualitative methods are dominant due to the difficulties of data collection for the quantitative analysis.

A grand challenge underlies the entire field of topology-enabled quantum logic and information science: how to establish topological control principles driven by quantum coherence and understand the time-dependence of such periodic driving? Here we demonstrate a THz pulse-induced phase transition in Dirac materials that is periodically driven by vibrational coherence due to excitation of the lowest Raman-active mode. Above a critical field threshold, there emerges a long-lived metastable phase with unique Raman coherent phonon-assisted switching dynamics, absent for optical pumping. The switching also manifest itself by non-thermal spectral shape, relaxation slowing down near the Lifshitz transition where the critical Dirac point (DP) occurs, and diminishing signals at the same temperature that the Berry curvature induced Anomalous Hall Effect varnishes. These results, together with first-principles modeling, identify a mode-selective Raman coupling that drives the system from strong to weak topological insulators, STI to WTI, with a Dirac semimetal phase established at a critical atomic displacement controlled by the phonon pumping. Harnessing of vibrational coherence can be extended to steer symmetry-breaking transitions, i.e., Dirac to Weyl ones, with implications on THz topological quantum gate and error correction applications.

Herein, we aim to examine the order-order transition (OOT) of self-assembled block copolymer (BCP) under spatial confinement using double gyroid (DG) structured polystyrene-block-polydimethylsiloxane (PS-PDMS) thin film as an exemplary system for three-dimensional (3D) visualization of phase transitions. An interesting OOT from DG to hexagonally perforated lamellae (HPL) can be found after thermal annealing, and the morphological evolution and corresponding mechanism for the OOT was systematically investigated by 3D transmission electron microscopy (3D TEM). Our results revealed that the phase transition of DG nanostructure in thin film was different from that in bulk, which is attributed into the surficial/interfacial effects and the effect of confinement on BCP self-assembly. This work provides a feasible method to prepare DG-nanostructured thin film and presents a model system for the examination of morphological evolution from metastable to phase with higher thermodynamic stability in the thin-film state through the OOT.

We discover hidden Rashba fine structure in CH$_3$NH$_3$PbI$_3$ and demonstrate its quantum control by vibrational coherence through symmetry-selective vibronic (electron-phonon) coupling. Above a critical threshold of a single-cycle terahertz pump field, a Raman phonon mode distinctly modulates the middle excitonic states with {\em persistent} coherence for more than ten times longer than the ones on two sides that predominately couple to infrared phonons. These vibronic quantum beats, together with first-principles modeling of phonon periodically modulated Rashba parameters, identify a {\em three-fold} excitonic fine structure splitting, i.e., optically-forbidden, degenerate dark states in between two bright ones. Harnessing of vibronic quantum coherence and symmetry inspires light-perovskite quantum control and sub-THz-cycle "Rashba engineering" of spin-split bands for ultimate multi-function device., Comment: Phys. Feedback control theory has been extensively implemented to theoretically model human sensorimotor control. However, experimental platforms capable of manipulating important components of multiple feedback loops lack development. This paper describes the WheelCon, which is an open source platform aimed at resolving such insufficiencies. WheelCon enables safely simulation of the canonical sensorimotor task such as riding a mountain bike down a steep, twisting, bumpy trail etc., with provided only a computer, standard display, and an inexpensive gaming steering wheel with a force feedback motor. The platform provides flexibility, as will be demonstrated in the demos provided, so that researchers may manipulate the disturbances, delay, and quantization (data rate) in the layered feedback loops, including a high-level advanced plan layer and a low-level delayed reflex layer. In this paper, we illustrate WheelCon's graphical user interface (GUI), the input and output of existing demos, and how to design new games. In addition, we present the basic feedback model, and we show the testing results from our demo games which align well with prediction from the model. In short, the platform is featured as cheap, simple to use, and flexible to program for effective sensorimotor neuroscience research and control engineering education., 20 pages, 6 figures

We report the low-energy electrodynamics of a moderately clean A15 superconductor (SC) following ultrafast excitation to understand and manipulate terahertz (THz) quasi--particle (QP) transport by tuning pump photoexcitation of from competing orders. Using 35-fs optical pulses, we observe a non-thermal enhancement in the low frequency conductivity, opposite to that observed for THz pump, which persists up to an additional critical temperature, above the SC one, from an electronic order in the Martensitic normal state. In the SC state, the fluence dependence of pair breaking kinetics together with an analytic model provides evidence for a `one photon-to-one pair' non-resonant energy transfer during the laser pulse. Such initial transfer of photon energy $\hbar\omega$ to QPs at the {\em quantum} limit, set by $2\Delta_{SC}/\hbar\omega$=0.33$\%$, is more than one order of magnitude smaller than in previously studied BCS SCs, which we attribute to strong electron--phonon coupling and possible influence of phonon condensation in A15 SCs., Comment: 5 pages 4 figures

Protein engineering is a powerful tool to modify e.g. protein stability, activity and substrate selectivity. Heterologous expression of the enzyme α-ketoisovalerate decarboxylase (Kivd) in the unicellular cyanobacterium Synechocystis PCC 6803 results in cells producing isobutanol and 3-methyl-1-butanol, with Kivd identified as a potential bottleneck. In the present study, we used protein engineering of Kivd to improve isobutanol production in Synechocystis PCC 6803. Isobutanol is a flammable compound that can be used as a biofuel due to its high energy density and suitable physical and chemical properties. Single replacement, either Val461 to isoleucine or Ser286 to threonine, increased the Kivd activity significantly, both in vivo and in vitro resulting in increased overall production while isobutanol production was increased more than 3-methyl-1-butanol production. Moreover, among all the engineered strains examined, the strain with the combined modification V461I/S286T showed the highest (2.4 times) improvement of isobutanol-to-3M1B molar ratio, which was due to a decrease of the activity towards 3M1B production. Protein engineering of Kivd resulted in both enhanced total catalytic activity and preferential shift towards isobutanol production in Synechocystis PCC 6803.

Background The ability to accurately predict operative duration has the potential to optimise theatre efficiency and utilisation, thus reducing costs and increasing staff and patient satisfaction. With laparoscopic cholecystectomy being one of the most commonly performed procedures worldwide, a tool to predict operative duration could be extremely beneficial to healthcare organisations. Methods Data collected from the CholeS study on patients undergoing cholecystectomy in UK and Irish hospitals between 04/2014 and 05/2014 were used to study operative duration. A multivariable binary logistic regression model was produced in order to identify significant independent predictors of long (> 90 min) operations. The resulting model was converted to a risk score, which was subsequently validated on second cohort of patients using ROC curves. Results After exclusions, data were available for 7227 patients in the derivation (CholeS) cohort. The median operative duration was 60 min (interquartile range 45–85), with 17.7% of operations lasting longer than 90 min. Ten factors were found to be significant independent predictors of operative durations > 90 min, including ASA, age, previous surgical admissions, BMI, gallbladder wall thickness and CBD diameter. A risk score was then produced from these factors, and applied to a cohort of 2405 patients from a tertiary centre for external validation. This returned an area under the ROC curve of 0.708 (SE = 0.013, p 90 min increasing more than eightfold from 5.1 to 41.8% in the extremes of the score. Conclusion The scoring tool produced in this study was found to be significantly predictive of long operative durations on validation in an external cohort. As such, the tool may have the potential to enable organisations to better organise theatre lists and deliver greater efficiencies in care.

Using an effective genetic algorithm, we uncover the structure of a metastable Al41Sm5 phase that supplements its family sharing similar short-range orders. The phase evolves upon heating an amorphous Al-9.7at.%Sm ribbon, produced by melt-spinning. The dynamical phase selection is discussed with respect to the structural connections between the short-range packing motifs in the amorphous precursor and those observed in the selected phases. The phase elucidated here is one of several newly discovered large-unit-cell phases found to form during devitrification from the glass in this binary system, further illustrating the power and efficiency of our approach, the important role of structural hierarchy in phase selection, and the richness of the metastable phase landscape accessible from the glassy structure.

Differential evolution (DE) has been applied extensively in drug combination optimization studies in the past decade. It allows for identification of desired drug combinations with minimal experimental effort. This article proposes an adaptive population-sizing method for the DE algorithm. Our new method presents improvements in terms of efficiency and convergence over the original DE algorithm and constant stepwise population reduction-based DE algorithm, which would lead to a reduced number of cells and animals required to identify an optimal drug combination. The method continuously adjusts the reduction of the population size in accordance with the stage of the optimization process. Our adaptive scheme limits the population reduction to occur only at the exploitation stage. We believe that continuously adjusting for a more effective population size during the evolutionary process is the major reason for the significant improvement in the convergence speed of the DE algorithm. The performance of the method is evaluated through a set of unimodal and multimodal benchmark functions. In combining with self-adaptive schemes for mutation and crossover constants, this adaptive population reduction method can help shed light on the future direction of a completely parameter tune-free self-adaptive DE algorithm.

The 1144 iron arsenide (e.g. CaKFe4As4) has recently been discovered and inspired a tide of search for superconductors. Such far, the discovered compounds are confined to iron arsenides (ABFe4As4), where A and B are either alkali metals or alkaline earth elements. In this work, we propose two directions in searching 1144 structures: (i) using tri-valence cations for A; (ii) substituting the transition metal, e.g. replacing Fe by Co. Following the two directions, we employ density functional theory to study stability and electronic structures of 1144 pnictides of various tri-valence cations (La, Y, In, Tl, Sm and Gd), as well as cobalt arsenides. For LaAFe4As4, the 1144 phase can be stabilized in three systems: LaKFe4As4, LaRbFe4As4 and LaCsFe4As4, which show quasi-two-dimensional semi-metal features similar to the iron pnictide superconductors: hole-type Fermi surface at Gama point and electron-type Fermi surface at M point in B.Z. In addition, LaKFe4As4 feature an extra bubble shaped Fermi surface sheets, distinct from the other two peers. Y does not support any 1144 phase within our search. For In and Tl, substitute Fe by Co and two unknown compounds of the 122 phase are stabilized: InCo2As2 and TlCo2As2. The two cobalt arsenides have Fermi surfaces of similar topology as iron arsenides, but the Fermi surfaces are all electron-type, showing potentials to be undiscovered superconductors. Stable 1144 phases are also found in InKCo4As4 and InRbCo4As4. For Sm and Gd, most 1144 and 122 iron arsenides are found unstable., Comment: 11 pages, 8 figures

We fabricate high-mobility p-type few-layer WSe2 field-effect transistors and surprisingly observe a series of quantum Hall (QH) states following an unconventional sequence predominated by odd-integer states under a moderate strength magnetic field. By tilting the magnetic field, we discover Landau level (LL) crossing effects at ultra-low coincident angles, revealing that the Zeeman energy is about three times as large as the cyclotron energy near the valence band top at {\Gamma} valley. This result implies the significant roles played by the exchange interactions in p-type few-layer WSe2, in which itinerant or QH ferromagnetism likely occurs. Evidently, the {\Gamma} valley of few-layer WSe2 offers a unique platform with unusually heavy hole-carriers and a substantially enhanced g-factor for exploring strongly correlated phenomena., Comment: To appear in Physical Review Letters, including the Supplemental Material

Background: Cleft lip and/or palate (CL/P) is one of the most common birth conditions in the world, but little is known about its causes. Professional opinion remains divided as to which treatments may be the most beneficial for patients with CL/P, and the factors that contribute to psychological adjustment are poorly understood. The use of different methodological approaches and tools plays a key role in hampering efforts to address discrepancies within the evidence base. A new UK-wide program of research, The Cleft Collective, was established to combat many of these methodological challenges and to address some of the key research questions important to all CL/P stakeholders. Objective: To describe the establishment of CL/P cohort studies in the United Kingdom and to consider the many opportunities this resource will generate. Results: To date, protocols have been developed and implemented within most UK cleft teams. Biological samples, environmental information, and data pertaining to parental psychological well-being and child development are being collected successfully. Recruitment is currently on track to meet the ambitious target of approximately 9800 individuals from just more than 3000 families. Conclusions: The Cleft Collective cohort studies represent a significant step forward for research in the field of CL/P. The data collected will form a comprehensive resource of information about individuals with CL/P and their families. This resource will provide the basis for many future projects and collaborations, both in the United Kingdom and around the world.

Venous thromboembolism (VTE) is the commonest cause of preventable death in hospitalized patients. Elderly patients have higher risk of VTE because of the high prevalence of predisposing co-morbidities and acute illnesses. Clinical diagnosis of VTE in the elderly patient is particularly difficult and, as such, adequate VTE prophylaxis is of pivotal importance in reducing the mortality and morbidities of VTE. Omission of VTE prophylaxis is, however, very common despite continuous education. A simple way to overcome this problem is to implement universal VTE prophylaxis for all hospitalized elderly patients instead of selective prophylaxis for some patients only according to individual's risk of VTE. Although pharmacological VTE prophylaxis is effective for most patients, a high prevalence of renal impairment and drug interactions in the hospitalized elderly patients suggests that a multimodality approach may be more appropriate. Mechanical VTE prophylaxis, including calf and thigh compression devices and/or an inferior vena cava filter, are often underutilized in hospitalized elderly patients who are at high-risk of bleeding and VTE. Because pneumatic compression devices and thigh length stockings are virtually risk free, mechanical VTE prophylaxis may allow early or immediate implementation of VTE prophylaxis for all hospitalized elderly patients, regardless of their bleeding and VTE risk. Although the cost-effectiveness of this Multimodality Universal STat ('MUST') VTE prophylaxis approach for hospitalized elderly patients remains uncertain, this strategy appears to offer some advantages over the traditional 'selective and single-modal' VTE prophylaxis approach, which often becomes 'hit or miss' or not implemented promptly in many hospitalized elderly patients. A large clustered randomized controlled trial is, however, needed to assess whether early, multimodality, universal VTE prophylaxis can improve important clinical outcomes of hospitalized elderly patients.
