1. Grandmothers’ mental health is associated with grandchildren’s emotional and behavioral development:a three-generation prospective study in Brazil

Abstract

Background Maternal mental health is associated with an increased risk of emotional and behavioural problems in children, and the risk is partly explained by the negative impact of maternal depression on caregiving. The role of mental health in other family members, who in many contexts also provide substantial caregiving, has received far less attention. We examined the impact of grandmothers’ emotional symptoms, whose role in child care is increasing across the world, on internalizing and externalizing symptoms in grandchildren from a three-generation birth cohort study. Methods Prospective data from three generations in two birth cohorts 22 years apart (1982 and 2004) in Pelotas, Brazil, were used (n = 92). Mental health in grandmothers and parents was assessed using the Self-Reported Questionnaire (SRQ-20). Grandchildren were members of the 2004 birth cohort, and behavioural and emotional problems were measured using the Child-Behaviour Checklist (CBCL) at age 4 years. Results Grandmothers’ symptoms were associated with more emotional and behavioural problems in grandchildren after adjustment for confounding factors. The size of the associations between grandmothers’ and grandchildren mental health symptoms was comparable to the associations between maternal emotional symptoms and children emotional and behavioural problems. There was no evidence for associations with paternal symptoms. These effects were substantially stronger for maternal compared to paternal grandmothers. Conclusions In some contexts, grandmothers’ mental health may be as important to grandchild emotional and behavioural development as maternal mental health. Interventions to improve the mental health of grandmothers, as well as parents, may be important to child mental health. 3. Prevalence of high blood pressure in 122, 053 adolescents: A systematic review and meta-regression

Abstract

Several studies have reported high prevalence of risk factors for cardiovascular disease in adolescents. To perform: i) systematically review the literature on the prevalence of high blood pressure (HBP) in adolescents; ii) analyze the possible methodological factors associated with HBP; and iii) compare the prevalence between developed and developing countries. We revised 10 electronic databases up to August 11, 2013. Only original articles using international diagnosis of HBP were considered. The pooled prevalence's of HBP were estimated by random effects. Meta-regression analysis was used to identify the sources of heterogeneity across studies. Fifty-five studies met the inclusion criteria and total of 122,053 adolescents included. The pooled-prevalence of HBP was 11.2%, 13% for boys, and 9.6% for girls (P < 0.01). Method of measurement of BP and year in which the survey was conducted were associated with heterogeneity in the estimates of HBP among boys. The data indicate that HBP is higher among boys than girls, and that the method of measurement plays an important role in the overall heterogeneity of HBP value distributions, particularly in boys.

Abstract

INTRODUCTION: Bullying among adolescents has become the subject of extensive research due to its negative effects on attitude and behavior. However, Brazilian literature on the topic is scarce. OBJECTIVE: To assess bullying and associated factors among adolescents in a population-based sample in southern Brazil. METHOD: Population-based, cross-sectional study with adolescents aged 11 to 15 years living in the municipality of Pelotas, state of Rio Grande do Sul, Brazil. RESULTS: A total of 1,145 adolescents were included. Bullying behaviors and attitudes were found to be associated with the following factors: intentionally skipping classes, failing at school, using alcohol, smoking cigarettes, using illegal drugs, having engaged in sexual intercourse, carrying a weapon, being involved in an accident/being run over by any vehicle, and being involved in fights with physical aggression. CONCLUSION: Bullying is associated with several behaviors that pose risk to the adolescents' physical and psychological health.INTRODUÇÃO: O bullying entre adolescentes tem sido objeto de várias pesquisas devido aos prejuízos comportamentais que acarreta. Contudo, a literatura brasileira sobre o tema é escassa. OBJETIVO: Avaliar comportamentos de bullying e fatores associados em uma amostra de base populacional de adolescentes vivendo no sul do Brasil. MÉTODO: Estudo transversal, de base populacional, com adolescentes de 11 a 15 anos de idade residentes na cidade de Pelotas, RS, Brasil. RESULTADOS: Um total de 1.145 adolescentes foram selecionados. Os comportamentos e atitudes de bullying mostraram-se associados aos seguintes fatores: faltar aula intencionalmente, reprovar algum ano na escola, usar álcool, cigarro e/ou drogas ilícitas, ter tido relações sexuais, portar arma, sofrer algum acidente ou atropelamento e brigar e/ou agredir alguém. CONCLUSÃO: O bullying esteve associado com vários comportamentos considerados de risco para a saúde física e psicológica dos adolescentes.

5. Parental Childhood Growth and Offspring Birthweight: Pooled Analyses from Four Birth Cohorts in Low and Middle Income Countries

Abstract

Objective\ud Associations between parental and offspring size at birth are well established, but the relative importance of parental growth at different ages as predictors of offspring birthweight is less certain. Here we model parental birthweight and postnatal conditional growth in specific age periods as predictors of offspring birthweight.\ud \ud Methods\ud We analyzed data from 3,392 adults participating in four prospective birth cohorts and 5,506 of their offspring.\ud \ud Results\ud There was no significant heterogeneity by study site or offspring sex. 1SD increase in maternal birthweight was associated with offspring birthweight increases of 102 g, 1SD in maternal length growth 0–2 year with 46 g, and 1SD in maternal height growth Mid-childhood (MC)-adulthood with 27 g. Maternal relative weight measures were associated with 24 g offspring birth weight increases (2 year- MC) and 49 g for MC-adulthood period but not with earlier relative weight 0–2 year. For fathers, birthweight, and linear/length growth from 0–2 year were associated with increases of 57 and 56 g in offspring birthweight, respectively but not thereafter.\ud \ud Conclusions\ud Maternal and paternal birthweight and growth from birth to 2 year each predict offspring birthweight. Maternal growth from MC-adulthood, relative weight from 2-MC and MC-adulthood also predict offspring birthweight. These findings suggest that shared genes and/or adequate nutrition during early life for both parents may confer benefits to the next generation, and highlight the importance of maternal height and weight prior to conception. The stronger matrilineal than patrilineal relationships with offspring birth weight are consistent with the hypothesis that improving the early growth conditions of young females can improve birth outcomes in the next generation.

Abstract

Background: Birth order has been associated with early growth variability and subsequent increased adiposity, but the consequent effects of increased fat mass on metabolic risk during adulthood have not been assessed. We aimed to quantify the metabolic risk in young adulthood of being first-born relative to those born second or subsequently.Methodology and Principal Findings: Body composition and metabolic risk were assessed in 2,249 men, aged 17-19 years, from a birth cohort in southern Brazil. Abstract

The aim of this study was to determine the levels of certain factors of cardiovascular risk in children and adolescents in rural areas of Gallicia.One thousand eight children between the ages of 6 and 15 years selected from three primary schools were studied. Weight, height, blood pressure, serum levels of cholesterol and self admitted consumption of tobacco were determined in each participant.The patterns of body mass, blood pressure and total cholesterol selected to age and sex are very similar to those observed in a great deal of the Spanish and foreign studies. However, the absolute values do not necessarily coincide. The children studied were more obese than those reported in other studies carried out in the rest of Spain with similar values of systolic blood pressure and lower cholesterol levels. The levels of cholesterol in this study were within the range observed in other Western countries, lower than those found in other parts of Spain but much higher than the ideal quantities recommended by the WHO. Blood pressure was similar to that of a recent meta-analysis of Spanish studies but much higher than that observed in the Second American Task Force Study. With regard to the use of tobacco, this population demonstrated lower prevalences to those of other Spanish populations. From the age of 14 the consumption of cigarettes greatly increases, especially in boys.Obesity and blood pressure appear to be a more important potential problem than cholesterol among the cardiovascular risk factors considered in the children studied in comparison with populations of a similar age. If prevention plans on the use of tobacco are foreseen in Spain, they should be made before the age of 14 when an important number of children already smoke or have tried cigarettes.

Abstract

Early life stunting may have long-term effects on body composition, resulting in obesity-related comorbidities. We tested the hypothesis that individuals stunted in early childhood may be at higher cardiometabolic risk later in adulthood. 1753 men and 1781 women participating in the 1982 Pelotas (Brazil) birth cohort study had measurements of anthropometry, body composition, lipids, glucose, blood pressure, and other cardiometabolic traits at age 30 years. Early stunting was defined as height-for-age Z-score at age 2 years below -2 against the World Health Organization growth standards. Linear regression models were performed controlling for sex, maternal race/ethnicity, family income at birth, and birthweight. Analyses were stratified by sex when p-interaction, The last phase of the 1982 Pelotas (Brazil) birth cohort study was supported by the Wellcome Trust and the Fundação de Aparo à Pesquisa do Estado do Rio Grande do Sul; Brazil (Edital 04/2012 – PQG; Processo 12/2185-9). Abstract

Age at first sexual intercourse and age at first birth have implications for health and evolutionary fitness. In this genome-wide association study (age at first sexual intercourse, N = 387,338; age at first birth, N = 542,901), we identify 371 single-nucleotide polymorphisms, 11 sex-specific, with a 5–6% polygenic score prediction. Heritability of age at first birth shifted from 9% [CI = 4–14%] for women born in 1940 to 22% [CI = 19–25%] for those born in 1965. Signals are driven by the genetics of reproductive biology and externalising behaviour, with key genes related to follicle stimulating hormone (FSHB), implantation (ESR1), infertility and spermatid differentiation. Our findings suggest that polycystic ovarian syndrome may lead to later age at first birth, linking with infertility. Late age at first birth is associated with parental longevity and reduced incidence of type 2 diabetes and cardiovascular disease. Higher childhood socioeconomic circumstances and those in the highest polygenic score decile (90%+) experience markedly later reproductive onset. Results are relevant for improving teenage and late-life health, understanding longevity and guiding experimentation into mechanisms of infertility.

Abstract

Educational attainment is widely used as a surrogate for socioeconomic status (SES). Low SES is a risk factor for hypertension and high blood pressure (BP). To identify novel BP loci, we performed multi-ancestry meta-analyses accounting for gene-educational attainment interactions using two variables, "Some College" (yes/no) and "Graduated College" (yes/no). Interactions were evaluated using both a 1 degree of freedom (DF) interaction term and a 2DF joint test of genetic and interaction effects. Analyses were performed for systolic BP, diastolic BP, mean arterial pressure, and pulse pressure. We pursued genome-wide interrogation in Stage 1 studies (N = 117 438) and follow-up on promising variants in Stage 2 studies (N = 293 787) in five ancestry groups. Through combined meta-analyses of Stages 1 and 2, we identified 84 known and 18 novel BP loci at genome-wide significance level (P < 5 × 10 -8 ). Two novel loci were identified based on the 1DF test of interaction with educational attainment, while the remaining 16 loci were identified through the 2DF joint test of genetic and interaction effects. Ten novel loci were identified in individuals of African ancestry. Several novel loci show strong biological plausibility since they involve physiologic systems implicated in BP regulation. They include genes involved in the central nervous system-adrenal signaling axis (ZDHHC17, CADPS, PIK3C2G), vascular structure and function (GNB3, CDON), and renal function (HAS2 and HAS2-AS1, SLIT3). Collectively, these findings suggest a role of educational attainment or SES in further dissection of the genetic architecture of BP.

No. of bitstreams: 1 tese_5993_.pdf: 1741106 bytes, checksum: 63f365d95eee6b62c4767decb3be9723 (MD5) Previous issue date: 2012-10-15 A doença renal crônica (DRC) já é considerada uma pandemia e indivíduos por ela acometidos possuem um sistema imunológico debilitado e são mais suscetíveis a doenças infecciosas e entre estas a tuberculose (TB) que também é reconhecidamente um problema global de saúde pública. Temos então que indivíduos portadores de DRC possuem um risco reconhecidamente aumentado para a TB e com evidências de que por conta de seu estado de imunossupressão estes indivíduos também possuem um pior desfecho durante o tratamento. Dessa forma, este estudo pretendeu investigar aspectos epidemiológicos e clínicos da convergência da TB e DRC. RESULTADOS: Através de uma revisão sistemática e meta-análise encontramos uma prevalência agrupada de TB em indivíduos com DRC submetidos ao transplante renal de 2,51% (95% IC = 2,17 2,85). No Espírito Santo esta prevalência foi de 1,54% (95% IC 0,71% - 2,38%) e apresentou associação com história prévia de TB, número de episódios infecciosos e uso de sirolimus na imunossupressão inicial. Ainda nesta população o tempo médio para o desenvolvimento de TB após o transplante renal foi de quatro anos e entre os treze indivíduos identificados com TB oito apresentaram doença pulmonar, sete necessitaram de hospitalização e quatro morreram em consequência da TB. Já os fatores que diferenciam as chances de abandono, óbito por TB e óbito por outra causa em relação cura no tratamento da TB, entre indivíduos com DRC no Brasil, foram os estratos etários; alcoolismo; AIDS; estar institucionalizado num presídio; estar sob a terapia de substituição renal transplante; retornar para o tratamento após abandono prévio; possuir um raio X de tórax suspeito para TB; e ter indicação ou ter realizado tratamento diretamente observado. CONCLUSÃO: Os desafios apresentados pela relação entre a TB e os indivíduos com DRC, que vão desde a alta prevalência até um pior desfecho para o tratamento, são imensos e nossos dados ajudam a reforçar a necessidade da implementação de estratégias para controle da TB direcionadas a determinados grupos populacionais, priorizando especialmente os portadores de doenças crônicas não transmissíveis como a DRC. Abstract

The social determination of health outcomes is well established in literature. These studies usually show how health outcomes vary on the basis of current socioeconomic position (SEP) However, recent evidence suggests that current health out-comes depend on SEP in different periods in the life course.1 Also, past SEP may modify the effect of the individual’s current socioeconomic situation on health. Several studies have shown that adverse socioeconomic conditions in childhood determine adult mortality rates, even when present socioeconomic situation is controlled for. Mortality rates from stroke and stomach cancer were 1.87 and 2.03 times higher in men whose fathers were manual workers compared with those whose fathers were not manual workers, while the all-cause mortality rate ratio was 1.22, in a Scottish cohort study of working men.2 In a census-based Finnish study, manual social class in childhood increased all-cause mortality by 20% among non-manual workers in adulthood, while having no effect among manual workers, whose mortality rate was three times higher than for subjects who always belonged to non-manual class. For cardiovascular mortality, childhood manual class was associated with increased rates for both manual and non-manual adult workers.3 Height has been shown to be related to childhood social class and financial hardship in infancy,4 and despite the increase in mean population height, differences between the richest and the poorest have not changed significantly.5 Nutrition and diseases in childhood are believed to be the main environmental factors determining height, and, thus, the effect of childhood social class is probably more important then adulthood social class. However, a study from the UK showed that upward social class mobility (from the age of 7–33 years) was associated with increased height for children originally from lower classes. On the other hand, children from the upper classes who moved down the social ladder were not different from those who remained wealthy.6 This study did not try to establish the causal direction—it could be either the improvement of social position leading to taller young adults or selective social mobility of taller individuals. Obesity and overweight are increasing in most parts of the world,7 and their relation with SEP has been the focus of many studies. In Brazil, overweight and obesity have shown, overall, a steep increase. From 1975–97, obesity increased from 2.4 to 6.9% among men and from 7 to 12.5% among women.8 This change, however, was not consistent for all socioeconomic groups. Among men, all groups showed an increase in obesity over time, but poorer men present less obesity than the richer. Among women, the poorer show an increase of obesity more than 2-fold. The richer, on the other hand, peak in 1989 (12.8%) and then show a decrease to 9.2%. A similar trend was found, analysing the same surveys, comparing the trends in the poorer North-east of the country with the richer South-east.9 A recent review showed that in developing countries obesity tends to either increase or not to vary with wealth in men, while among women the association tends to be inverse.10 In the present study we explore how past and current SEP affects the height and weight of 19-year-old adults in a Brazilian birth cohort. This analysis may bring new light to the understanding of how these characteristics depend on SEP, not only at one point in time, as most studies have approached, but in terms of SEP change over time.
