The COVID-19 pandemic caused by infection with Severe Acute Respiratory Syndrome Coronavirus 2 (SARS-CoV-2) has led to more than 100,000 deaths in the United States. Several studies have revealed that the hyper-inflammatory response induced by SARS-CoV-2 is a major cause of disease severity and death in infected patients. However, predictive biomarkers of pathogenic inflammation to help guide targetable immune pathways are critically lacking. We implemented a rapid multiplex cytokine assay to measure serum IL-6, IL-8, TNF-α, and IL-1β in hospitalized COVID-19 patients upon admission to the Mount Sinai Health System in New York. Patients (n=1484) were followed up to 41 days (median 8 days) and clinical information, laboratory test results and patient outcomes were collected. In 244 patients, cytokine measurements were repeated over time, and effect of drugs could be assessed. Kaplan-Meier methods were used to compare survival by cytokine strata, followed by Cox regression models to evaluate the independent predictive value of baseline cytokines. We found that high serum IL-6, IL-8, and TNF-α levels at the time of hospitalization were strong and independent predictors of patient survival. Importantly, when adjusting for disease severity score, common laboratory inflammation markers, hypoxia and other vitals, demographics, and a range of comorbidities, IL-6 and TNF-α serum levels remained independent and significant predictors of disease severity and death. We propose that serum IL-6 and TNF-α levels should be considered in the management and treatment of COVID-19 patients to stratify prospective clinical trials, guide resource allocation and inform therapeutic options. We also propose that patients with high IL-6 and TNF-α levels should be assessed for combinatorial blockade of pathogenic inflammation in this disease.

Simultaneous measurements of THz wave and hard X-ray emission from thin and flat water flow when irradiated by double femtosecond laser pulses (800 nm, 35 fs, transform-limited, 0.5 kHz, delay times up to 15 ns) were carried out at the transmission and the reflection sides of the flow with THz time-domain spectroscopy and Geiger counters. THz wave emission spectra show their dynamic peak-shifts toward the low frequency with the highest intensity enhancements more than 1.5 x 10^3 times in |E|^2 at the delay time of 4.7 ns between the two pulses, while X-ray intensity enhancements are limited to about 20 times at 0 ns under the same experimental conditions. The mechanisms for the spectral changes and the intensity enhancements in THz wave emission are discussed from the viewpoint of laser ablation on the water flow induced by the pre-pulse irradiation., Comment: 11 pages, 4 figures

Cu or Cu/Au (80 nm thick Cu, 50 nm thick Au) sputtered on Si were kept at 25 degrees C for a week or annealed at a temperature from 80 to 300 degrees C, then tested for THz emission under femtosecond laser irradiation (35 fs-800 nm). THz radiation was detected from samples annealed from 80 to 170 degrees C, which had a Cu2O/Cu interface as the THz source. Cu/Au/Si annealed at 80 degrees C emitted the highest THz radiation because of high laser absorption by the porous Cu2O layer formed at low temperature and the Au film reflected THz radiation and/or increased the laser absorption by the Fabry-Perot effect.

Lead zirconate titanate (PZT) thin films have received much attention due to their excellent ferroelectric, pyroelectric and piezoelectric properties, making them excellent candidates for next generation high-precision micro/nano electromechanical systems, integrated circuit resonators and actuators. In these applications, the functional property is determined by ferroelectric and ferroelastic domain wall movement (i.e., electromechanical response). It has been shown that electromechanical response enhancement can be achieved by fabricating multilayered heterostructures. Moreover, new polar phases can arise from interfacial effects in these multilayers. In this dissertation, tetragonal (T) PbZr0.30Ti0.70O3 / rhombohedral (R) PbZr0.55Ti0.45O3 / La0.67Sr0.33MnO3 (LSMO) heterostructures were grown by pulsed laser deposition. A (001) oriented SrTiO3 (STO) substrate was used to achieve high quality epitaxial growth. The T and R films were both ~100 nm thick and the LSMO layer (bottom electrode) was ~20 nm thick. Ferroelastic herringbone c/a1/c/a2 domains were found in the T layer. In contrast, reducing the T layer thickness to ~20 nm, a-axis oriented domains were observed. Additionally, local regions (~50 nm wide) were observed exhibiting fine c/a non-equilibrium domains. Cross-sectional transmission electron microscopy revealed crystallographic relationships between the domains and domain walls. Systematic piezoresponse force microscopy studies revealed complex interactions between the electrical and mechanical fields. These bilayered samples exhibited enhanced electromechanical properties due to the presence of extremely mobile ferroelastic domains in the T layer, where the local symmetry within the domain walls differed from the bulk. These domains were formed by elastic interactions between T and R layers. Atomic-resolution imaging was used to further investigate how domains interacted near, or at, the interfaces. The location of interface between adjacent layers was determined by electron energy loss spectroscopy. Aberration-corrected scanning transmission electron microscopy imaging, followed by post processing, was used to map the polar displacements. Unconventional polarization rotation was found near the T/R interfaces and the domain wall boundaries. Moreover, a new phase was identified, presumably formed to accommodate electric fields for local and interfacial structures. The results presented provide a fundamental insight into the relationships between the ferroelectric/ferroelastic domain structures and physical mechanisms that govern ferroelastic domain behaviours.

Since the 2008 inception of universal childhood influenza vaccination, national rates have risen more dramatically among younger children than older children and reported rates across racial/ethnic groups are inconsistent. Interventions may be needed to address age and racial disparities to achieve the recommended childhood influenza vaccination target of 70%.To evaluate an intervention to increase childhood influenza vaccination across age and racial groups.In 2011-2012, a total of 20 primary care practices treating children were randomly assigned to the intervention and control arms of a cluster randomized controlled trial to increase childhood influenza vaccination uptake using a toolkit and other strategies including early delivery of donated vaccine, in-service staff meetings, and publicity.The average vaccination differences from pre-intervention to the intervention year were significantly larger in the intervention arm (n=10 practices) than the control arm (n=10 practices); for children aged 9-18 years (11.1 pct pts intervention vs 4.3 pct pts control, p0.05); for non-white children (16.7 pct pts intervention vs 4.6 pct pts control, p0.001); and overall (9.9 pct pts intervention vs 4.2 pct pts control, p0.01). In multi-level modeling that accounted for person- and practice-level variables and the interactions among age, race, and intervention, the likelihood of vaccination increased with younger age group (6-23 months); white race; commercial insurance; the practice's pre-intervention vaccination rate; and being in the intervention arm. Estimates of the interaction terms indicated that the intervention increased the likelihood of vaccination for non-white children in all age groups and white children aged 9-18 years.A multi-strategy intervention that includes a practice improvement toolkit can significantly improve influenza vaccination uptake across age and racial groups without targeting specific groups, especially in practices with large percentages of minority children.

The advantages of adopting a new software system for Supply Chain Management (SCM) enable the organization to enhance work efficiency as well as customer satisfaction in the supply chain network. These advantages make enterprises update their SCM system through project as a method of realize their strategic change. This research explores how the project manager leads the project team, a temporarily formed unit with multi-functional members, by looking into three focus concepts of the practice perspective: Practitioner, Praxis, and Practices. The empirical study utilized two projects in the electronics firms as the case study. It was decided that a qualitative approach was the most appropriate method to gather the data based on the practice perspective. The findings of this thesis support this primacy of communication. To successfully execute the short duration project for system change, intra-organizational communication has an important part to play in the success of change initiatives. Analysis revealed that the project managers’ soft skills are more important than their knowledge of using project-specific tools or techniques in managing short-duration projects. In the empirical study, the requirements of all project practitioners were not met due to tight schedule and limited resource. However, the project objectives were still achieved on time. Some key success factors were identified, such as top management commitment to the project, correct prioritization of different stakeholders’ power, and involvement of employees as early as possible within the change process. The study concludes that the three focus concepts of the practice perspective interplay during the project process and they contribute to the general framework in the short duration project for realizing strategic change, which can be a reference for industry participants in managing similar projects.
