Background: There is a potential for longitudinal and horizontal transfer of the cytotoxic effects of cypermethrin, and evidence suggests that date fruit extracts can ameliorate these cytotoxic effects. Thus, the current study evaluated female Wister rats’ fertility and pup parameters after mating males treated with cypermethrin and date fruit (Phoenix dactylifera) extract. Methods: Adult Wistar rats (n = 74; 140 – 207 g) were used to conduct this experiment in two phases: The first phase was a single dose toxicity study (n = 18), while the second phase (n = 56) evaluated female Wister rats’ reproductive and pup development parameters after mating with male Wistar rats exposed to date fruit extract and cypermethrin. Male rats were distributed randomly into four groups (n = 7 per group) and administered distilled water (Group I); 60 mg/kg cypermethrin (Group II) to simulate infertility; 250 mg/kg date fruit extract alone (Group III); and date fruit extracts plus 60 mg/kg cypermethrin (Group IV). Treated males were then mated with healthy female rats at a ratio of 1:1. Successfully mated females were appraised for conception and pregnancy rates, gestation length, litter size, and weights at birth, and on days 5, 10, 15, and 21 post-partum, an anogenital distance at day-20, sex ratio, and age of pup when hair growth was evident. Results: Significant differences (p < 0.05) were observed in litter size and male anogenital distance but not in reproductive performance. Also, the study demonstrated that date fruit extract skewed the sex ratio of offspring to more female than male pups when female Wister rats were mated with males that received date fruit extracts alone or cypermethrin and date fruit extracts. The body weight gain did not differ significantly between the control and cypermethrin-treated groups Conclusion: This evidence suggested a longitudinal transfer of the protective effects of date fruit extracts following a weekly exposure of male rats to cypermethrin at 60 mg/kg.

Here, we engineer Neanderthal and Denisovan sequences into thousands of artificial genes to reconstruct the pre-mRNA processing patterns of these extinct populations. Of the 5,169 alleles tested in this massively parallel splicing reporter assay (MaPSy), we report 962 exonic splicing mutations that correspond to differences in exon recognition between extant and extinct hominins. Using MaPSy splicing variants, predicted splicing variants, and splicing quantitative trait loci, we show that splice-disrupting variants experienced greater purifying selection in anatomically modern humans than that in Neanderthals. Adaptively introgressed variants were enriched for moderate-effect splicing variants, consistent with positive selection for alternative spliced alleles following introgression. As particularly compelling examples, we characterized a unique tissue-specific alternative splicing variant at the adaptively introgressed innate immunity gene TLR1 , as well as a unique Neanderthal introgressed alternative splicing variant in the gene HSPG2 that encodes perlecan. We further identified potentially pathogenic splicing variants found only in Neanderthals and Denisovans in genes related to sperm maturation and immunity. Finally, we found splicing variants that may contribute to variation among modern humans in total bilirubin, balding, hemoglobin levels, and lung capacity. Our findings provide unique insights into natural selection acting on splicing in human evolution and demonstrate how functional assays can be used to identify candidate causal variants underlying differences in gene regulation and phenotype.

Background The COVID-19 pandemic worsened the ongoing overdose crisis in the United States (US) and caused significant mental health strain and burnout among health care workers (HCW). Harm reduction, overdose prevention, and substance use disorder (SUD) workers may be especially impacted due to underfunding, resources shortages, and chaotic working environments. Existing research on HCW burnout primarily focuses on licensed HCWs in traditional environments and fails to account for the unique experiences of harm reduction workers, community organizers, and SUD treatment clinicians. Methods We conducted a qualitative secondary analysis descriptive study of 30 Philadelphia-based harm reduction workers, community organizers, and SUD treatment clinicians about their experiences working in their roles during the COVID-19 pandemic in July–August 2020. Our analysis was guided by Shanafelt and Noseworthy’s model of key drivers of burnout and engagement. We aimed to assess the applicability of this model to the experiences of SUD and harm reduction workers in non-traditional settings. Results We deductively coded our data in alignment with Shanafelt and Noseworthy’s key drivers of burnout and engagement: (1) workload and job demands, (2) meaning in work, (3) control and flexibility, (4) work-life integration, (5) organizational culture and values, (6) efficiency and resources and (7) social support and community at work. While Shanafelt and Noseworthy’s model broadly encompassed the experiences of our participants, it did not fully account for their concerns about safety at work, lack of control over the work environment, and experiences of task-shifting. Conclusions Burnout among healthcare providers is receiving increasing attention nationally. Much of this coverage and the existing research have focused on workers in traditional healthcare spaces and often do not consider the experiences of community-based SUD treatment, overdose prevention, and harm reduction providers. Our findings indicate a gap in existing frameworks for burnout and a need for models that encompass the full range of the harm reduction, overdose prevention, and SUD treatment workforce. As the US overdose crisis continues, it is vital that we address and mitigate experiences of burnout among harm reduction workers, community organizers, and SUD treatment clinicians to protect their wellbeing and to ensure the sustainability of their invaluable work.

Prostate cancer remains the most common non-skin cancer in men. Prostate-specific antigen (PSA) is recognized as a biomarker for the diagnosis, monitoring, and risk prediction of prostate cancer. However, its role in prostate cancer screening has been controversial. While some authorities have recommended its use for screening, others have stated otherwise. Some clarity is required about its precise role in clinical practice. There need to be more consistent recommendations surrounding using PSA screening in clinical practice. Serum PSA measurements show variable reliability when screening for Prostate cancer, given the dynamics of PSA physiology and the conflicting results from two large, randomized control trials that sought to determine its role in prostate cancer screening and early detection. Hence surrogate measures like PSA density, PSA velocity, free-to-complexed PSA ratio, and percentage Pro-PSA among others, have been used to improve the predictive utility of this assay for Prostate cancer diagnosis. However, the debate on screening still lingers. The current review aims to highlight the controversies and objectively outline the current recommendations. This literature review examined scholarly papers and recommendations about the use of PSA for prostate cancer screening with the aim to rationalize the pros and cons of such approaches. We concluded that although more recent guidelines from the USPSTF recommend that screening be based on individual preference and professional judgment by the healthcare provider, differences in the specific details on how to best employ a PSA screening program still exist and require further review.

Adolescents with emotional and behavioral disorders face known academic challenges and poor life outcomes. It was imperative to explore and find if the new diagnostic criterion for diagnosing autism profoundly affects educational outcomes and resilience in individuals diagnosed with co-occurring autism spectrum disorder (ASD) and attention-deficit hyperactivity disorder (ADHD). The literature is robust on the impact of adverse childhood experiences (ACEs) on educational outcomes and resilience in adolescents with no history of disability. Still, there remains a dearth of literature explaining, with no ambiguity, the complex relationships between ACEs and resilience, school engagement, and success in individuals with co-occurring ASD and ADHD. This study reviews the existing scholarships on the topic. The significance of this review is that it informs healthcare providers, rehabilitation counselors, and educators about the need for early identification of individuals with ASD and ADHD with a background in ACEs. This will enable interventions early enough to ensure they are more resilient and can obtain improved success in school-related and outside-school activities and eventually improved quality of life.

Background: Therapy-related acute myeloid leukemia (t-AML) is a well-recognized clinical syndrome occurring in a signif- icant fraction of patients who have undergone previous chemotherapy for a solid tumour. Objectives: We aim to evaluate the effect of aqueous extract of fresh Allium sativum cloves on haematological parameters, bone marrow and DNA of etoposide treated albino wistar rats. Decoction method was used to prepare plant extracts and the rats were weighed and divided into experimental and control groups. Blood and bone marrow sample were analysed and DNA fragment analysis was carried out. Results: There was progressive increase in the weight of animals that received distilled water only for the duration of the experiment while those that received etoposide only showed a sharp decrease in weight by the end of week 3. There was no significant difference in the mean of the haematological parameters in the test and control groups except for platelet count. The bone marrow smears showed no prevention of erythroblast fragmentation by the extract, in the same vein, DNA dam- age was not abated. Conclusion: Aqueous extract of fresh Allium sativum cloves may not be the option for the prevention of etoposide induced acute myeloid leukemia. Background: Length at birth is important for evaluating childhood growth and development. It is of interest in Pediatrics because of its implications for perinatal and postnatal morbidity and mortality. Predicting birth length will be useful in an- ticipating and managing possible complications associated with pregnancy and birth of babies with abnormal birth length. Objective: The aim was to identify easily accessible parental determinants of baby’s birth length in Lagos, Nigeria, using a sample of patients attending a government hospital. Methods: Parental anthropometrics and other data were obtained from 250 couples by actual measurements, oral interviews and questionnaires. Baby’s birth length was measured immediately after delivery by qualified, a well-trained obstetric nurse, and association between parental and offspring parameters were assessed. Results: Weight gain, maternal weight, parity and mid-parental height were the significant parental explanatory variables of offspring birth length. They were the most suitable variables for a generated model for predicting babies’ birth length from parental variables in the study. Conclusion: A model that might be useful for predicting babies’ birth length from easily accessible parental variables was produced. This model may complement ultrasonographic data for predicting baby’s birth length with a view to achieving better perinatal and postnatal care. Larvae of the black soldier fly (BSF), Hermetia illucens can be reared in large quantities on organic wastes. The larvae are harvested at the prepupal stage and used as feed for poultry and aquaculture. BSF farms in Nigeria make use of a combination of chicken feed and organic waste in the rearing processes. Chicken feed and organic waste can contain varying levels of heavy metals, which may accumulate in the larvae. The aim of this study was to determine bioaccumulation of heavy metals, cadmium chromium, copper, lead, and zinc, in prepupae cultured on chicken mash and various types of organic wastes (food waste, brewery waste, and fruit waste). Bioaccumulation of a particular heavy metal occurs when the concentration of the metal is higher in the harvested prepupae than in the feed substrates (Factor > 1). The results of the study showed that bioaccumulation was more likely to occur in brewery waste than in the other substrates, while bioaccumulation factors greater than one were reported for three of the five heavy metals (copper, lead and chromium). All heavy metal concentrations reported in the study were below the safe limits for use in animal feeds. The use of organic waste rather than chicken mash for rearing BSF larvae should be considered in Nigeria. However, there must be quality control measures in place to ensure that the larvae’s propensity for accumulating heavy metals are contained.

Background The incidence of allergy among mill workers has been a public health concern over the years. The human allergen-specific immunoglobulin G (IgG) and the total immunoglobulin E (IgE) are markers for allergic disorders and are important in health monitoring of individuals exposed to allergens. Nonadherence to health and safety precautions by grain workers has led to an increase in respiratory symptoms and other pathologies. Aim The aim is to assess human allergen-specific IgG and total IgE concentrations, hematological parameters, and lung functions in factory workers exposed to grain dust for effective health monitoring. Patients and methods A total of 69 patients were enrolled; 29 of them were controls whereas 40 were mill workers. Blood samples collected from the patients were analyzed using Mindray autoanalyzer for full blood count, whereas ELISA method was used for the estimation of human allergen-specific IgG and total IgE. Aim: To evaluate the interleukin 3 levels in some Hepatitis B virus and Human immunodeficiency virus (HIV) positive subjects. Study Design: Cross sectional study. Place and Duration of Study: Haematology Department, University of Nigeria Teaching Hospital Enugu, Nigeria, between June and September, 2019. Methodology: A total of 86 subjects were recruited for this study; 40 were positive for the human immunodeficiency virus, 30 were Hepatitis B positive and 16 healthy subjects that served as controls. The controls had tested negative to Hepatitis B Virus, Human immunodeficiency Virus and hepatitis C virus infections. Whole blood samples were collected from the Human immunodeficiency Virus positive and control samples. Haemoglobin concentration was analysed using the Orphěe Mythic 22 automated analyzer. Serum samples collected from all 86 subjects were used to assay Interleukin 3 using the Enzyme Linked Immunosorbent assay based Finetest human interleukin 3 kit. Results: A highly significant decrease in interleukin 3 levels was observed in Hepatitis B Virus and Human immunodeficiency virus positive subjects when compared with the apparently healthy control subjects, (P = 0.000 < 0.05). Average hemoglobin levels were also lower in the Human immunodeficiency virus subjects compared with the controls. Conclusion: A reduction of the interleukin 3 may be part of the synergistic factors responsible for the anaemia usually seen in the viral infections.

Gender and language studies in general have not been fully explored in most parts of the globe particularly in Nigeria. The objective of the study therefore was to examine the politeness and hedging strategies in the English language conversation of Igbo native speakers in Nigeria as well as establish whether men and women’s conversational styles have been gendered As a cross-sectional questionnaire and interview-based survey, the sample population was studied by means of ten-item questionnaire in the form of Discourse Completion Task and structured interview at seven Universities systematically selected from the South-East and South-South geo-political zones in Nigeria. The study instruments were completed, returned, transcribed and statistically analysed using the quantitative and qualitative tools for analysis of production and perception data respectively. Results showed that politeness and hedging are indispensable sociolinguistic elements in the conversational English of the study sample. Also, results revealed that to accomplish a communicative goal, the samples adopted different politeness/hedging strategies given the discourse situation. Also, findings showed that the female respondents were found to adhere more to linguistic politeness principles than their male counterparts considering the context hence demonstrating a difference in conversational style. The study found evidence to establish that females are more polite than males in conversations. In conclusion, findings of this study showed that respondents yielded to certain sociolinguistic factors such as age, culture, hierarchy, disposition and religion as they were believed to inform the use of a particular strategy or another.

To determine the contribution of defective splicing in Autism Spectrum Disorders (ASD), the most common neurodevelopmental disorder, a high throughput Massively Parallel Splicing Assay (MaPSY) was employed and identified 42 exonic splicing mutants out of 725 coding de novo variants discovered in the sequencing of ASD families. A redesign of the minigene constructs in MaPSY revealed that upstream exons with strong 5’ splice sites increase the magnitude of skipping phenotypes observed in downstream exons. Select hits were validated by RT-PCR and amplicon sequencing in patient cell lines. Exonic splicing mutants were enriched in probands relative to unaffected siblings -especially synonymous variants (7.5% vs 3.5%, respectively). Of the 26 genes disrupted by exonic splicing mutations, 6 were in known ASD genes and 3 were in paralogs of known ASD genes. Of particular interest was a synonymous variant in TNRC6C - an ASD gene paralog with interactions with other ASD genes. Clinical records of 3 ASD patients with TNRC6C variant revealed respiratory issues consistent with phenotypes observed in TNRC6 depleted mice. Overall, this study highlights the need for splicing analysis in determining variant pathogenicity, especially as it relates to ASD., Author summary Autism Spectrum Disorders (ASD) is the most common neurodevelopmental disorder that presents with both phenotypic and genetic heterogeneity. Before genetically defined therapeutic approaches can be applied to the treatment of ASD, it is crucial to understand the genetic basis of the disorder. Tremendous effort by consortiums, such as the Simon Foundation for Autism Research Initiative, has led to the identification of variants and, in turn, genes associated with ASD. However, the effect of ASD variants have on the process of splicing is often not fully considered when assessing ASD risk genes. Using a high-throughput splicing assay, MaPSy, ~6.3% of ASD-associated variants affected splicing, highlighting a substantial contribution of defective splicing to ASD. This analysis led to identification of genes, such as TNRC6C, that warrant further investigation with respect to ASD risk.

Background The present study aimed to classify lymphoid neoplasms according to the latest World Health Organization (WHO) classification and outlining the distribution in Nigeria of different entities. Additionally, the study describes the prevalence of lymphoid neoplasms associated with Epstein-Barr virus (EBV) infection in the Nigerian population. Methods We collected 152 formalin-fixed paraffin-embedded (FFPE) tissues diagnosed as lymphoma from 2008 to 2018, coming from three different institutions located within three geopolitical zone in Nigeria. These institutions included the University College Hospital (UCH), Ibadan, Oyo State, the Enugu State University of Science and Technology Teaching Hospital (ESUTH), Enugu, Enugu State, and the Meena Histopathology and Cytology Laboratory (MHCL), Jos, Plateau State. Results From the total 152 cases retrieved, 50 were excluded due to insufficient tissue materials or inconclusive antigen reactivity. We confirmed 66 (64.7%) cases as lymphomas out of the remaining 102 FFPE with a male to female ratio of 2:1 and a mean age of 44.4 years. Ten entities were identified, and of these, chronic lymphocytic leukemia (CLL) was the most prevalent category (34.8%). For the diffuse large B-cell lymphomas not otherwise specified (DLBCL, NOS), the germinal centre B–cell type was the most common (71.4%). Ten lymphoma cases (15.2%) were positive for Epstein-Barr virus (EBV), most of which were Hodgkin lymphoma (HL). CLL was common in the Hausa ethnic group, HL in the Yoruba ethnic group, while the Igbo ethnic group had an equal distribution of CLL, HL, and DLBCL diagnosis. Conclusion Although the distribution of lymphomas in Nigeria shares some similarities with those of other countries, we described distinct features of some subtypes of lymphomas. Also, the study underscores the need for a more precise diagnosis and classification of lymphoid neoplasms in Nigeria using the latest WHO classification.

Striga hermonthica is a major stress of maize in sub-Saharan Africa (SSA). The International Institute of Tropical Agriculture (IITA), in collaboration with national scientists, have used team approach to investigate how best to solve the problem. Emphasis was on (i) establishing a reliable infestation technique for selecting resistant/tolerant genotypes, (ii) availability of appropriate germplasm and good sources of Striga resistance, (iii) use of appropriate breeding methods for incorporating resistance genes into adapted germplasm, and (iv) extensive multilocational evaluation to identify genotypes with stable performance. Host plant resistance, with additive-dominance model, has been the major control option for S. hermonthica infestation on maize. Recurrent selection, followed by hybridization of inbred lines developed from its products, have been used to identify high-yielding, stable hybrids for commercialization in SSA. In a study involving early-maturing tropical maize inbred lines, 24 single nucleotide polymorphism (SNP) markers significantly associated with grain yield, Striga damage, ears per plant, and ear aspect under Striga infestation were detected. In a quantitative trait loci (QTL) mapping study involving extra-early white BC 1 S 1 families obtained from TZEEI 29 ( Striga- resistant) and TZEEI 23 ( Striga susceptible) inbreds, 14 QTLs were identified for Striga resistance/tolerance traits. In a second QTL study involving the extra-early yellow F 2:3 families derived from the Striga- resistant parent (TZEEI 79) and the susceptible parent (TZdEEI 11), 12 QTL were identified for 4 Striga resistance/tolerance traits. QTL identified in the studies would be invaluable for rapid introgression of Striga resistance genes into maize genotypes using marker-assisted selection approaches after validation of QTL in inbreds.

Objective To compare cesarean delivery rates and indications by race/ethnicity among nulliparous women with term, singleton, vertex presentation deliveries. Methods This is a retrospective cohort study of nulliparous women delivering term, singleton, vertex neonates at Kaiser Permanente Northern California from 1/1/2016 to 6/30/2017. Women with cesarean for elective, malpresentation, or previa were excluded. Multivariable logistic regression models adjusting for maternal, neonatal, and facility factors were used to assess the likelihood of cesarean by race/ethnicity. Further modeling was performed to examine odds of cesarean for the indications of failure to progress and fetal intolerance by race/ethnicity. Results The cohort of 16,587 racially/ethnically diverse women meeting inclusion and exclusion criteria consisted of 41.62% White, 27.73% Asian, 22.11% Hispanic, 5.32% Black, and 3.21% multiple race/other women. In adjusted logistic regression models, all race and ethnic categories had higher odds of cesarean deliveries in comparison to White women. Black women had the highest odds of cesarean delivery (adjusted OR [aOR] = 1.73, 95% CI: 1.45–2.06), followed by Asian (aOR = 1.59, 95% CI: 1.45–2.06), multiple race/other (aOR = 1.45, 95% CI: 1.17–1.80), and Hispanic (aOR = 1.43, 95% CI: 1.28–1.59) women. Compared with White women, Asian (aOR = 1.46, 95% CI: 1.22–1.74) and Hispanic (aOR = 1.25, 95% CI: 1.03–1.52) women had higher odds of failure to progress as the indication. Among women with failure to progress, Black (aOR = 0.50, 95% CI: 0.30–0.81), Hispanic (aOR = 0.68, 95% CI: 0.53–0.87), and Asian (aOR = 0.77, 95% CI: 0.61–0.96) women were less likely than White women to reach 10 cm dilation. Compared with White women, Black women were more likely to have cesarean delivery for fetal intolerance (aOR = 1.51, 95% CI: 1.10–2.07). Among women with fetal intolerance of labor, there were no significant differences by race/ethnicity for Apgar score or neonatal intensive care unit admission. Conclusions Race/ethnicity was significantly associated with the odds of cesarean and indication. All other race/ethnicity groups had higher odds of cesarean compared with White women. Compared with White women, Black women had greater odds of fetal intolerance as an indication, while Hispanic and Asian women had greater odds of failure to progress. Maternal, neonate, and facility factors for cesarean delivery did not explain the observed disparities in cesarean delivery rates.

Cardiovascular disease remains the leading cause of morbidity and mortality for women in the USA. Recent multi-society guideline updates endorse using a history of premature menopause, defined as occurring before age 40 years, as a risk-enhancing factor for atherosclerotic cardiovascular disease risk assessment in women. Here, we review recent literature on cardiovascular disease risk in women with premature menopause, possible mechanisms linking premature menopause to cardiovascular disease, and the role of menopausal hormone therapy in postmenopausal women. Large cohort studies and meta-analyses have demonstrated significant associations between premature menopause and development of diverse cardiovascular diseases. In addition to atherosclerotic cardiovascular disease, recent studies have noted accelerated development of valvular heart disease and venous thromboembolism. Surgical premature menopause may be associated with greater risk of cardiovascular disease compared with natural premature menopause; further research to clarify differential risk and relevant underlying mechanisms is needed. Although the cardiovascular risks of menopausal hormone therapy appear largely restricted to women over age 60 years and more than 10 years post-menopause, data do not demonstrate convincing cardiovascular protective benefit in any age group. Premature menopause is associated with elevated future risk of diverse cardiovascular diseases. Further studies are needed to further elucidate mechanisms and identify novel preventive strategies.

IntroductionAs childhood mortality from infectious diseases falls across sub-Saharan Africa (SSA), the burden of disease attributed to surgical conditions is increasing. However, limited data exist on paediatric surgical outcomes in SSA. We compared the outcomes of five common paediatric surgical conditions in SSA with published benchmark data from high-income countries (HICs).MethodsA multicentre, international, prospective cohort study was undertaken in hospitals providing paediatric surgical care across SSA. Data were collected on consecutive children (birth to 16 years), presenting with gastroschisis, anorectal malformation, intussusception, appendicitis or inguinal hernia, over a minimum of 1 month, between October 2016 and April 2017. Participating hospitals completed a survey on their resources available for paediatric surgery.The primary outcome was all-cause in-hospital mortality. Mortality in SSA was compared with published benchmark mortality in HICs using χ2 analysis. Generalised linear mixed models were used to identify patient-level and hospital-level factors affecting mortality. A pResults1407 children from 51 hospitals in 19 countries across SSA were studied: 111 with gastroschisis, 188 anorectal malformation, 225 intussusception, 250 appendicitis and 633 inguinal hernia. Mortality was significantly higher in SSA compared with HICs for all conditions: gastroschisis (75.5% vs 2.0%), anorectal malformation (11.2% vs 2.9%), intussusception (9.4% vs 0.2%), appendicitis (0.4% vs 0.0%) and inguinal hernia (0.2% vs 0.0%), respectively. Mortality was 41.9% (112/267) among neonates, 5.0% (20/403) in infants and 1.0% (7/720) in children. Paediatric surgical condition, higher American Society of Anesthesiologists score at primary intervention, and needing/receiving a blood transfusion were significantly associated with mortality on multivariable analysis.ConclusionMortality from common paediatric surgical conditions is unacceptably high in SSA compared with HICs, particularly for neonates. Interventions to reduce mortality should focus on improving resuscitation and timely transfer at the district level, and preoperative resuscitation and perioperative care at paediatric surgical centres.

BackgroundCoronaviruses are a group of viruses that belong to the Family Coronaviridae, GenusBetacoronavirus. In December 2019, a new coronavirus disease (COVID-19) characterized by severe respiratory symptoms was discovered. The causative pathogen was a novel coronavirus known as 2019-nCoV and later as SARS-CoV-2. Within two months of its discovery, COVID-19 became a pandemic causing widespread morbidity and mortality.MethodologyWhole genome sequence data of SARS-CoV-2 isolated from Nigerian COVID-19 cases were retrieved by downloading from GISAID database. A total of 18 sequences that satisfied quality assurance (length ≥ 29700 nts and number of unknown bases denoted as ‘N’ ≤ 5%) were used for the study. Multiple sequence alignment (MSA) was done in MAFFT (Version 7.471) while SNP calling was implemented in DnaSP (Version 6.12.03) respectively and then visualized in Jalview (Version 2.11.1.0). Phylogenetic analysis was with MEGA X software.ResultsNigerian SARS-CoV-2 had 99.9% genomic similarity with four large conserved genomic regions. A total of 66 SNPs were identified out of which 31 were informative. Nucleotide diversity assessment gave Pi = 0.00048 and average SNP frequency of 2.22 SNPs per 1000 nts. Non-coding genomic regions particularly 5’UTR and 3’UTR had a SNP density of 3.77 and 35.4 respectively. The region with the highest SNP density was ORF10 with a frequency of 8.55 SNPs/1000 nts). Majority (72.2%) of viruses in Nigeria are of L lineage with preponderance of D614G mutation which accounted for 11 (61.1%) out of the 18 viral sequences. Nigeria SARS-CoV-2 revealed 3 major clades namely Oyo, Ekiti and Osun on a maximum likelihood phylogenetic tree.Conclusion and RecommendationNigerian SARS-CoV-2 reveals high mutation rate together with preponderance of L lineage and D614G mutants. Implication of these mutations for SARS-CoV-2 virulence and the need for more aggressive testing and treatment of COVID-19 in Nigeria is discussed. Additionally, attempt to produce testing kits for COVID-19 in Nigeria should consider the conserved regions identified in this study. Strict adherence to COVID-19 preventive measure is recommended in view of Nigerian SARS-CoV-2 phylogenetic clustering pattern, which suggests intensive community transmission possibly rooted in communal culture characteristic of many ethnicities in Nigeria.

INTRODUCTION: The objective of this study was to assess the differences in HPV-related knowledge, attitudes, and beliefs among African American and non-Hispanic white women and to determine their communication preferences for cancer-related information. METHODS: Data was obtained from the National Cancer Institute's (NCI) 2014 Health Interview National Trends Survey (HINTS), a cross-sectional survey of U.S. adults 18 years of age or older. Descriptive statistics, bivariate and multivariate logistic regression were used to identify differences in awareness and knowledge. Data was collected in 2014 and analyzed in 2016. RESULTS: HPV awareness (71% vs. 77%) and knowledge that HPV causes cervical cancer (64% vs. 81%) was significantly lower among blacks. Additionally, there were significant disparities in awareness of the HPV vaccine (66% vs. 79%), with only 25% of Black women indicating that they or a family member was recommended the HPV vaccine by a health care professional. There were also differences in cancer communication preferences. Blacks were more likely than Whites to trust cancer information from family (OR 2.7, confidence interval [CI] 0.725-10.048), television (OR 3.0, 95% [CI] 0.733-12.296), government health agencies (OR 5.8, [CI] 0.639-52.818), and religious organizations (OR 6.4, 95% [CI] 1.718-23.932). CONCLUSION: Study results indicate that racial/ethnic differences exist in HPV knowledge/awareness and cancer communication preferences. These results highlight the need to increase HPV prevention and education efforts using methods that are tailored to Black women. To address HPV/cervical cancer disparities, future interventions should utilize preferred communication outlets to effectively increase HPV knowledge and vaccine awareness.

Iresine herbstii is fed to livestock in South-eastern Nigeria in the belief that it boosts their blood supply. The aim of this work is to study the effect of the methanolic extract of I. herbstii Hook on some blood parameters of experimentally induced anaemic albino rats. For acute toxicity study, graded doses of the methanolic leaf extract of I. herbstii at 10, 100, 1000, 1600, 2900 and 5000 mg/kg were administered orally to rats randomly allotted to six groups of three animals each and signs of toxicity were observed for 24 h. To evaluate its haematological effects, 100, 200, and 400 mg/kg of extract was administered orally to three out of five groups of six rats each for 14 days after the induction of acute blood loss anaemia. Parameters monitored were packed cell volume (PCV), haemoglobin concentration (Hb), weight gain and red blood cell counts (RBC). A sample of sixty students living with dyslexia was purposively drawn from 3 public secondary schools used for the study. Quasi experimental design by Solomon Four-group design was used in the study. To guide this study five research questions and five null hypotheses were formulated. The research questions were answered using mean, standard deviation of the pretest and post-test scores, while dependent and independent samples t-test, one way and two way analysis of variance (ANOVA) were used to analyse the null hypothesis. The major result of the study is that the Brainfeed alternative intervention programme is effective in enhancing working memory of students living with dyslexia. Based on these findings, recommendations were made.

Context: The benefit of antenatal care is no longer in doubt, but the ideal number of encounters to achieve those benefits has been the subject of discussion among maternity care stakeholders. Objective: To study the effect of frequency of antenatal visits on pregnancy outcome. Materials and Methods: This is a prospective study of 228 pregnant women attending the antenatal clinic of the Ahmadu Bello University Teaching Hospital, Zaria, Nigeria, over a 3-month period. Maternal and fetal outcome were compared against the frequency of antenatal visits. Results: The mean frequency of visits was 6.77. Majority of the women 124 (54.4%) attended less than seven times. Compliance with antenatal drugs and mean hemoglobin level at delivery increased with increasing frequency of visits reaching 93.7% and 11.44 g/dl, respectively, in the moderate attendees (4–6 visits). Prevalence of anemia was high (56.7%) among the unbooked but lowest (9.5%) with moderate attendees. Thereafter, there was no significant improvement. Delivery mode, malaria parasitemia, and gestational age at delivery were fairly uniform in all the groups. The prevalence of low birth weight was 20% in the unregistered and decreased from 22% in the low frequency (1–3 visits) group, to 4.8% in the moderate attendees, and to 0% in the very high-frequency (ten and above visits) group. Mean Apgar scores at 1 and 5 min were also best (7.1/8.1) in the moderate attendees and worst in the unregistered. Conclusion: More than six visits conferred no significant advantage on fetomaternal outcome. Therefore, the new WHO recommendation on reduced antenatal visits can be satisfactorily implemented in Kaduna.

Background Hypertrophic cardiomyopathy (HCM), the prototypical cardiovascular genetic disease, is caused by dominantly inherited mutations in genes encoding sarcomeric proteins. Genetic testing is recommended for individuals with HCM, the results of which promote prevention of morbidity and mortality through detection of disease in at-risk relatives. However, utilization of genetic testing in clinical medicine has not been widely adopted. Prior studies of inherited cancer syndromes have noted racial inequities in access to genetic testing. The aim of this study was to investigate for structural inequities in referral to a cardiovascular genetics clinic for genetic testing and counseling among Black, Latinx, and female patients with HCM. Methods We performed a case-control study (January 2012-December 2017) of 280 patients with HCM within a large academic health system. HCM patients were identified by ICD-9 diagnostic codes, and then verified by manual chart review. Of these 280 patients, 20% were referred to cardiovascular genetics clinic for genetic testing (cases) and 80% were not (controls). Demographic factors including race and gender were extracted for both cases and controls. Fischer exact, χ2tests, and t tests were used to assess the statistical significance of difference in the characteristics of cases and controls as indicated. Results HCM patients who were referred to cardiovascular genetics clinic for genetic testing (cases) were less likely to be black (5.4% of cases vs. 8.0% of controls) and less likely to be female (46.4% of cases vs 56.7% controls), but this did not achieve statistical significance (p=0.82 for race; p=0.17 for gender). English language was not associated with referral (p=0.30). Older patients, however, were significantly less likely to be referred for genetic testing (odds ratio 0.94 [95% CI 0.92, 0.96][p Conclusions Overall referral rates to a cardiovascular genetics clinic for genetic testing were low among patients with HCM. Additional ascertainment is important to understand if the demonstrated trends are significant on a larger scale.

The study explored the potential impact of emerging trends in the formal written English language of youths which hitherto is uninvestigated in South-east Nigeria. Through a random sampling process, 2000 copies of pretested and validated questionnaire written in English were administered to undergraduates at five federal universities in south-east Nigeria. In addition, print-out of a couple of group conversations on Facebook and WhatsApp and Key Person Interview (KPI) was used to supplement the questionnaire data. In all, 1940 copies of questionnaire representing 97.00% were completed and returned for analysis. Results showed that emerging trends in written English conversation as social media convention is common among students in tertiary institutions. Findings revealed that 98.87% of participants used emerging trends during classes which indicates a massive infiltration of formal written English with social media trivialities while a statistically significant 34.54% have had to use these emerging trends though unconsciously during exams which reflects their deep and deliberate learning of these most potentially contentious tools of informal written conversation. Ultimately, the study confirmed that activities on social media was taking its toll on the students’ performance in written English as they can no longer tell the appropriateness of one variety or another. In conclusion, the study established that emerging trends have profound negative impact on written English and recommends that teachers/instructors must make purposeful efforts to unteach that which has been wrongly learned by insisting that students begin to imbibe the culture of standard and formal writing regardless of the purpose of the writing.

This study examines the effect of oil price on the volatility of food price in Nigeria. It specifically considers the long-run, short-run, and causal relationship between these variables. Annual data on oil price and individual prices of maize, rice, sorghum, soya beans, and wheat spanning from 2000 to 2013 were used. The price volatility for each crop was obtained using Generalized Autoregressive Conditional Heteroskedascity (GARCH (1, 1)) model. Our measure of oil price is the Refiner acquisition cost of imported crude oil. The Augmented Dickey–Fuller and Phillip–Perron unit root tests show that all the variables are integrated of order one, I (1). Therefore, we use the Johansen co-integration test to examine the long-run relationship. Our results show that there is no long-run relationship between oil price and any of the individual food price volatility. Thus, we implement a VAR instead of a VECM to investigate the short-run relationship. The VAR model result revealed a positive and significant short-run relationship between oil price and each of the selected food price volatility with exception of that of rice and wheat price volatility. These results were further confirmed by the impulse response functions. The Granger causality test result indicates a unidirectional causality from oil price to maize, soya bean, and sorghum price volatilities but does not show such relationship for rice and wheat price volatilities. We draw some policy implications of these findings.

This study examines the long and short run relationships between oil price and food price volatility as well as the causal link between them. The study used annual food price volatility index from FAO from 2000 to 2013 and crude oil price from U.S. Energy Information and Administration (EIA) from 2000 to 2013. The Johansen and Jesulius co-integration test revealed that there is a long run relationship between oil price and domestic food price volatility. The vector error correction model indicated a positive and significant short run relationship between oil price and food price volatility. The Granger causality test revealed a unidirectional causality with causality running from oil price to food price volatility but not vice versa. It is recommended that policies and interventions that will help reduce uncertainty about food prices such as improved market information, trade policies and investment in research and development among others should be encouraged. Also to reduce the effect of oil price shock, it is recommended that government should subsidise pump price of refined oil, seek alternative sources of energy and there should be less dependence on oil for fertilizer production.

Background: Anaemia has been associated with poor materno-foetal performance among pregnant women in Nigeria. The objective, therefore, of this study is to determine the prevalence of anaemia at the time of antenatal booking and the associated factors in the hospital. Methods: Four hundred and seventy-seven women were studied prospectively over 3 months in the antenatal booking clinic, and their characteristics compared with their haemoglobin (Hb) levels. Results: Age range was 15–46 years, and mean Hb concentration was 10.76 g/dl. Nearly 53.05% of the study population had Hb Conclusion: Anaemia is still highly prevalent in the society, and the existing mechanisms for its correction should be reassessed for effectiveness and adequate utilisation in the antenatal clinic. Most anaemic women booked early, therefore, allowing enough time for correction before delivery.

Background Although obstetrician/gynecologists (OB/GYNs) play an important role in sickle cell disease (SCD) screening and patient care, there is little information on knowledge of SCD or sickle cell trait (SCT) or related practices in this provider group. Our objective was to assess SCD screening and prenatal management practices among OB/GYNs. Methods Twelve hundred Fellows and Junior Fellows of the American College of Obstetricians and Gynecologists (the College)a were invited to complete a mailed survey, of which half (n = 600) belonged to the Collaborative Ambulatory Research Network.b Participants answered questions regarding appropriate target patient groups for prenatal SCD screening, folic acid requirements, practice behaviors and adequacy of their medical school and residency training. Results A total of 338 CARN members (56.3%) and 165 non-CARN members (27.5%) returned a survey. Of the 503 responders, 382 provided obstetric services and were included in the analyses. Forty percent of these respondents (n = 153) reported seeing at least 1 patient with SCD in the last year. Of these, 97.4% reported regularly screening people of African descent for SCD or SCT, whereas 52.9% reported regularly screening people of Mediterranean descent and 30.1% reported regularly screening people of Asian descent. Only 56.2% knew the correct recommended daily dose of folic acid for pregnant women with SCD. The proportion of respondents that rated training on SCD screening, assessment and treatment as barely adequate or inadequate ranged from 19.7% to 39.3%. Conclusions The practice of many OB/GYNs who care for patients with SCD are not consistent with the College Practice Guidelines on the screening of certain target groups and on folic acid supplementation. There may be an opportunity to improve this knowledge gap through enhanced medical education.

Background: Nigerian adolescents constitute about 20% of the total population. Problems associated with menstruation affect 75% of adolescent females in both the developing and the developed world. Objectives: The objective of this study is to establish the age at menarche and menstrual abnormalities among adolescents in secondary schools in Zaria. Materials and Methods: A descriptive cross sectional study in which a self-administered pretested, semi-structured questionnaire in english was administered to 535 female secondary school students in Zaria, northern Nigeria. The students were selected from five schools that were chosen by simple random sampling from a list of schools in Zaria metropolis. Information obtained included age at menarche and menstruation related issues. Results: Of the 535 female secondary school students interviewed, 448 (83.7%) were in senior secondary school. The mean age of the respondents was 15.35 ± 1.48 years. The average age at menarche was 12.53 ± 1.33 years. Most respondents, 369 (69.0%) experienced regular menses. The average duration of menstruation was 5.45 ± 1.81 days in 354 (66.2%) of the respondents. Three hundred and forty-four (64.3%) of the respondents had dysmenorrhea, and it was severe in 229 (66.6%) of them. Only, 137 (25.6%) experienced premenstrual symptoms, the most common symptom being breast tenderness in 62 respondents (45.2%). Conclusion: The age at menarche and the menstrual pattern amongst adolescents in Zaria, northern Nigeria is similar to findings from other parts of Nigeria as well as from other parts of the world. Further studies are however needed to explore the full range of menstrual abnormalities that can occur in the adolescent age group.
