Acid ingestion in children is a common problem in developing countries. Immediate complications of acid ingestion are oesophageal perforation, laryngeal trauma, bleeding and fistula formation but stricture is a delayed complication. Gastric outlet obstruction is known complication of acids and surgery is the mainstay of treatment. Heineke-Mikulicz pyloroplasty was done in five cases without complications and the outcomes were satisfactory.

Lymphatic malformations are benign lesions, found in children, most common locations are neck and axillary regions. We report here a case of bilateral Peri-renal macrocystic lymphatic malformation in a 12-year-old female, masquerading as bilateral renal cysts. A 12-year-old female presented with complaints of pain in right flank for one week associated with fullness in right flank. On evaluation a provisional diagnosis of right perirenal lymphangioma was made and ultrasound guided pigtail drainage done. The pigtail output was 200-300ml daily for a week and it was removed and child was discharged. However, on follow up, CT was done and bilateral peri-renal lymphatic malformation(right>left) was diagnosed. Patient was then planned for exploratory laparotomy which revealed bilateral perirenal cyst, circumscribing both the kidneys completely. Excision of bilateral cysts was done and sent for histopathological evaluation. Cyst Fluid analysis revealed normal creatinine, LDH, total cholesterol, total protein, triglycerides, urea levels. Post-operative period was uneventful. Histopathological examination revealed cystic lesions formed by endothelial cells with focal inflammation, no signs of malignancy were seen. The child was followed up post-operatively and there was no evidence of recurrence. Perirenal lymphatic malformation is a rare entity. The entity is thought to be the result of the obstruction of the perirenal lymphatics. We are presenting this case for its rarity and unusual presentation. Macro cystic Lymphatic malformations are common diagnosis but around the kidneys, there are only very few cases reported in literature. Prognosis of cystic lymphatic malformation is excellent.

The term “Ranula” originates from the Latin Word Rana which means “underbelly of a frog.” Ranula are epithelial retention cyst or mucus extravasation pseudo cyst arising from the sublingual glands. They are rare in children. A one-year-old female was brought to paediatric surgery emergency OPD by her mother with complaints of swelling arising from oral cavity since birth, gradually increased in size to attain current size. Mother also gives History of difficulty in feeding. Clinical examination revealed a large protruded tongue, with a cystic swelling in the floor of the mouth measuring around 6×4 cm, with elevation of the tongue. Child was further evaluated; ultrasound was suggestive of ranula. Marsupialisation of the cyst was done. Intraoperatively, cyst fluid was aspirated for analysis and cyst wall was opened and excised, lateral margins of cyst were sutured to base of mouth. Cyst fluid analysis revealed elevated amylase and normal LDH levels. The post-operative period was uneventful. The histopathology report showed a pattern consistent with ranula. Child was followed up for a period of 1 year and there was no evidence of recurrence. We report a case of giant congenital ranula. The rarity of this condition in children and its atypical size make this case report unique. We treated the child with marsupialization and there was no recurrence noted. Hereby I would like to state that congenital symptomatic ranulas though rare can be managed with marsupialisation.

Sugarcane is an important cash crop of the entire world including India. It alone contributes to 75% of the worldwide sugar trade. Molecular markers are powerful tools and provide the basis for the estimation of genetic variability to start reasonable breeding program. Microsatellite markers have unique ability to determine the extent of genetic divergence among sugarcane genotypes. The objective of this study was to evaluate the genetic divergence of 12 early maturing sugarcane clones using 11 SSR markers. A total 55 alleles were found during the amplification of the primers out of which 21 alleles were found unique and 34 alleles were shared. The number of shared alleles per locus ranged from two out of five alleles in the case of primer NKS 1 and nine out of ten alleles in NKS 34. Similarly no. of unique alleles per locus ranged from one out of ten alleles in NKS 34, three out of six alleles in NKS 57. The primer pairs NKS 1 and NKS 8 generated considerably greater percentage of unique alleles. The PIC values revealing allelic diversity and frequency among the genotypes varied from 0.034 in case of NKS 48 to 0.778 in case of NKS 9 with an average of 0.549. Pair-wise combinations of CoSe15451 and CoSe15452 showed the highest similarity with the value of similarity coefficient (0.890). The lowest value of similarity coefficient was found to be 0.490 in the pair CoBln15501 and CoSe01421. The dendogram based on SSR marker analysis grouped the 12 sugarcane clones into four clusters which shows the CoSe15451 and CoSe15452 clones had maximum similarity and CoBln15501 and CoSe15452 clones had maximum diversity between each other.

Sulfasalazine has been widely used in treatment of rheumatoid arthritis and spondyloarthritis. This study aims to assess persistence with sulfasalazine and also frequency and severity of adverse drug reactions (ADRs) encountered with this very well-established disease-modifying anti-rheumatoid drug.This retrospective study was done in 1,114 patients from medicine and rheumatology outpatient departments of six centers across India. The inclusion criteria was patients taking sulfasalazine. Patients receiving sulfasalazine for rheumatoid arthritis or spondyloarthritis were selected and details on drugs used, duration of taking sulfasalazine, ADRs to sulfasalazine and whether sulfasalazine had to be stopped due to ADRs were analyzed.Of the total of patients included in the study, 10.1% had ADRs with sulfasalazine, and stopped the drug. Gastritis, deranged liver enzymes, hepatitis, skin rashes were the most commonly encountered ADRs. Of the total number of patients recruited for the study, 11% were lost to follow-up, as most of the centers were army hospitals and the officers and staff were posted to other places. Sulfasalazine was taken for less than 1 month by 3.8% patients while 12.5% had taken the drug for less than 6 months. Of the study patients, 28.6% had taken the drug for 24 - 60 months and 4.6% (51 patients) had taken it for more than 5 years. It was found that ADRs were most commonly encountered within the first year of using the drug, and persistence was seen in patients on long-term therapy.Sulfasalazine is a safe option in chronic therapy of rheumatoid arthritis and spondyloarthritis. Although frequency of ADRs with patients taking sulfasalazine is minimal, it did necessitate the stoppage of drug. If not well tolerated, sulfasalazine would not have been continued for more than 12 months as evidenced from this study.

Aim and objectives Aim To study the spectrum of imaging findings in patients Rhino-occulo-cerebral mucormycosis (ROCM). Objectives l To study the demographic profile of patients with ROCM l To detect the associated extrasinus complications of ROCM l To aid in clinical diagnosis of ROCM Materials & methods This retrospective descriptive study was performed in histopathologically confirmed cases of ROCM. The case records of patients with radiological, culture and histological evidence of acute invasive rhinocerebral mucormycosis were retrospectively evaluated for relevant radiological and clinical data between May 2021 to June 2022. Results The radiological evaluation included CT and MRI scans were done in 52 patients. The patient's average age was 48 years. The ethmoid sinus was involved in 46 cases (88%) and the maxillary sinus in 27 cases (52%). Biltaral (45, 86%) and pansinus (27, 52%) involvement were the most common. The orbit was involved in 39 cases (75%), the face in 25 cases (47%), and retroantral fat stranding in 24 cases (46%). Mucosal thickening (91%) was the most common pattern of involvement, followed by complete opacification (37%). Osseous involvement was seen in 17 of 44 patients who had CT scans, and the majority of patients had extrasinus extension with intact bone. MRI revealed variable T2SI, with T2 hyperintensity being the most common pattern. Heterogeneous enhancement in post contrast imaging was the most common. Conclusion Rhinocerebral mucormycosis is characterised by a variety of imaging abnormalities on CT and MRI. In determining the degree of involvement and consequences, imaging is crucial.

It was investigated how COIS (crude oil impacted sand) affected the compressive strength of concrete. All specimens with a w/c of 0.5 were intended to use the concrete mix, 1:1.8:2.7. Sand was contaminated with crude oil (2.5%, 5%, 10%, 15%, 20%, and 25%) by weight in order to create COIS concrete. Concrete cubes measuring 147 100 mm were made (with 21 control samples and 126 contaminated samples). The compressive strengths of the cubes were assessed at ages 3, 7, 14, 28, 56, 84, and 168 days after curing. In comparison to the control samples, the COIS concrete samples exhibited a slow rate of strength gain and a drop in strength. A loss of 18–90% compressive strength was experienced as a result of 2.5–25% crude oil contamination.

Literacy is an indicator of social development and quality of human life. It is a metaphor of social development which accelerates the pace of economic development along with social development in an area. Literacy has an important role for the progress of backward society. In the view of population geographers, literacy, despite being the social aspect of the population, is such a qualitative fact that indirectly indicates the changing socio-economic trends on a regional basis. The intent of the present article is to analyze the regional distribution and spatio-temporal trends of literacy in a developing region. Objective The purpose of this study was to find out the incidence of typical and atypical radiological imaging findings of hepatocellular carcinoma (HCC) on multiphase multidetector computed tomography (MDCT) scans in histologically proven cases. Materials and Methods A multiphase computed tomography study of 73 patients with histologically proven HCC was evaluated by a radiologist. Our multiphasic protocol was composed of precontrast, arterial, portal, and delayed venous phases. The reviewers analyzed the CT images for tumor size, enhancement patterns of HCC in different phases, relative timing of washout, internal cystic changes, and presence of dysmorphic intratumoral vessel aneurysms or arteriovenous shunt. Results Most of the cases (95.9%) showed typical enhancement patterns in MDCT, i.e., enhancements in the arterial phase with the portal or delayed venous phase contrast washout. Three cases (4.9%) out of 73 HCC showed enhancements in the portal phase and washout in the delayed venous phase. Seven cases out of total 73 HCC (9.5%) patients showed heterogeneous enhancements in both arterial and portal phases and contrast washout in the delayed venous phase. Venous thrombosis was noted in about 44 cases (60%) out of total 73 patients, in which portal vein thrombosis was more common than hepatic veins/inferior vena cava. Conclusion Most of the HCC show typical enhancement patterns, i.e., heterogeneous enhancements in the arterial phase and washout in the portal venous phase when we use MDCT. These findings are higher than those described previously. However, in our study, the main difference with MDCT was lower frequency of intratumoral pseudoaneurysm and bile duct invasion, and intratumoral fat and calcification (atypical character of HCC) were not found which were rare findings in previous study.

Mermaid syndrome is a very rare, lethal congenital anomaly. It is associated with other congenital anomalies and is incompatible with life. Typical clinical features are fused lower limb from perineum to ankle and single umbilical artery. Etiology is multifactorial and is extreme form of caudal regression syndrome. When diagnosed in early gestation, termination of pregnancy may be considered.

Congenital bilateral scrotal agenesis is very rare anomaly and characterized by the absence of scrotal rugae. It may be associated with other extra-genital anomalies as a part of a syndrome. Etiology is unknown but may be due to primary failure of labioscrotal folds development or secondary to localized 5-alpha-reductase type 2 deficiency. Various surgical techniques used for neoscrotum construction with different types of complications. In index case, we performed bilateral orchidopexy without creating neoscrotum.

Cloud storage provides a potential solution replacing physical disk drives in terms of prominent outsourcing services. A threaten from an untrusted server affects the security and integrity of the data. However, the major problem between the data integrity and cost of communication and computation is directly proportional to each other. It is hence necessary to develop a model that provides the trade-off between the data integrity and cost metrics in cloud environment. In this paper, we develop an integrity verification mechanism that enables the utilisation of cryptographic solution with algebraic signature. The model utilises elliptic curve digital signature algorithm (ECDSA) to verify the data outsources. The study further resists the malicious attacks including forgery attacks, replacing attacks and replay attacks. The symmetric encryption guarantees the privacy of the data. The simulation is conducted to test the efficacy of the algorithm in maintaining the data integrity with reduced cost. The performance of the entire model is tested against the existing methods in terms of their communication cost, computation cost, and overhead cost. The results of simulation show that the proposed method obtains reduced computational of 0.25% and communication cost of 0.21% than other public auditing schemes.

Taxus wallichiana Zucc. is a high valued medicinal plant and has been mainly studied for its anti-cancer properties. However, research on its other important biological activities, such as its antimicrobial potential, still needs attention. The focus of the present study is to investigate the antimicrobial activity of secondary metabolites of T. wallichiana needles against 3 different groups of microorganisms, i. e., bacteria, actinobacteria, and fungi. Bioactive compounds from T. wallichiana needles were separated through column chromatography, and, TLC-bioautography. Mobile phases were optimized using Snyder's selectivity triangle. Antimicrobial spots were fractionated and compounds were identified by gas chromatography-mass spectroscopy (GC/MS) and liquid chromatography-mass spectrometry (LC/MS). Functional groups were characterized using Fourier transform infrared spectrometry (FTIR) and nuclear magnetic resonance (NMR) was used to identify the molecular structures. GC/MS and LC/MS data analysis confirm the presence of fatty acids (arachidic acid, behenic acid, palmitic acid, and stearic acid), vitamins (nicotinamide), and alkaloids (cinchonine, timolol), aminobenzamides (procainamide), carbocyclic sugar (myoinositol), and alkane hydrocarbon (hexadecane), having antimicrobial activity in the needles of T. wallichiana. To the best of our knowledge, this is the first report on the isolation and characterization of antimicrobial compounds from the needles of Taxus wallichiana (Himalayan yew). The data obtained from the present study will be supportive to the new drug discoveries in modern medicine with various combinations of medicinal plant's active constituents that can be used for curing many diseases.

Foreign body aspiration is a life threatening emergency in children. Different types of foreign bodies are aspirated and reported in literature but light emitting diode (LED) bulb is an unusual cause of foreign body aspiration. Rigid bronchoscopy is gold standard for treatment of foreign body aspiration. We wish to present a case of 9 years old male child with LED bulb aspiration which was retrieved with rigid bronchoscopy.

Punctually and appropriate maintenance of pavement black top surface using suitable material and method is significant for the preservation of road assets and to serve the intended purpose. An adequate maintenance management system that would be useful to highway agencies in regularly planning pavement maintenance strategies to ensure the minimal maintenance fund is used rationally. The target of this paper is to develop a hypothetic overall pavement condition index (OPCI) for the maintenance strategy selection for Indian Highways, exclusively for flexible pavements. This index incorporates salient indicators as distress, structural capacity, roughness and skid resistance. The distress index has been computed considering the maximum allowable extent principle. Multiplicative Index Approach was applied to develop OPCI. An expert opinion survey was conducted to evaluate the weightage for each indicator using the relative impact on pavement condition. The results reveal that the weight factor is 0.6 for structure capacity, 0.5 for roughness and 0.15 for skid resistance which is lower than the distress weight factor. The relative importance that should be given to each indicator are calculated to be 80% for distresses, 10% for structural capacity, 8% for roughness and 2% for skid resistance. The combined distress pavement condition index infests in almost one higher rating scale than PCI whereas OPCI is 21% lower than the PCI. This indicates the requirement of a conservative maintenance alternative. However, a condition indicator that consists of multiple indices is much more important in identifying suitable maintenance alternative approaches to fully restore the structural integrity and riding quality of the pavement.

Biohydrogen is one of the cleanest renewable energies with a high calorific value. Algal biomass can be utilized as a sustainable feedstock for biohydrogen production via dark fermentation. However, the recovery of fermentable sugar from algal biomass is challenging because of the diversity and complex cell wall composition and therefore, requires an additional pretreatment step. However, most of the conventional pretreatment strategies suffer from limited technological feasibility and poor economic viability. In this context, this review aims to present the structural complexities of the cell wall of algae and highlight the innovative approaches such as the use of hybrid technologies, biosurfactants, nanoparticles, and genetic engineering approaches for the hydrolysis of algal biomass and improved biohydrogen production. Additionally, a comprehensive discussion of the comparative evaluation of various pretreatment methods, and the techno-economic and life cycle assessment of algal biohydrogen production is also presented in this review.

The Hibiscus Rosa-Sinensis is a flower that has been used in traditional Chinese medicine for centuries. Recently, there has been renewed interest in the potential health benefits of this flower, particularly with regards to its Antidiabetic effects. A number of studies have shown that the extract from Hibiscus Rosa-Sinensis can help to lower blood sugar levels in people with diabetes mellitus, and it is thought that this may be due to the presence of certain compounds within the plant that have hypoglycemic activity. Polycyclic aromatic hydrocarbons (PAHs) are widely distributed in the ecosystem and are of significant concern due to their toxicity and mutagenicity. Bioremediation of PAHs is a popular and benign approach that ameliorates the environment. This study investigated the biodegradation and proteome response of Pseudomonas aeruginosa IIPIS-8 for two-ringed PAH: naphthalene (NAP) to understand proteome alteration during its bioremediation. Rapid biodegradation was observed up to 98 ± 1.26% and 84 ± 1.03%, respectively, for initial concentrations of 100 mg L

Cold bitumen emulsion mix (CBEM) is an emerging sustainable material for constructing the base or surface layer of flexible pavements, reducing energy consumption and global warming effects compared to hot mix asphalt. The main challenges associated with CBEM are low initial strength, moisture susceptibility, etc. This is primarily due to water encapsulation. The curing process allows the water to evaporate gradually over time, improving mechanical properties. This study investigates CBEM’s volumetric and mechanical properties under different curing conditions. CBEMs were prepared using low (50 blows) and high (75 blows) compaction efforts and cured at 25, 40, and 55 °C for 1 to 28 days. In volumetric characteristics, the air voids and voids in mineral aggregates decreased slightly with curing, while total binder volume decreased. Effective binder volume and voids filled with binder increased slightly, then decreased. The mechanical properties, i.e., Marshal Stability, Marshal Quotient, Indirect Tensile Strength (ITS), and Moisture Susceptibility parameters like the Indirect Tensile Strength Ratio (ITSR) and Retained Marshal Stability (RMS), were found to improve over time at each curing temperature. Using a high curing temperature further improved the mechanical characteristics of CBEM. The curing process was decelerated in case of CBEM prepared by high compactive efforts compared to CBEM prepared by low compactive efforts. The Michaelis-Menten model can be used to analyze moisture loss growth and the development of mechanical properties over time. The strength-maturity model can be developed by using ITS as strength parameters. There was a strong correlation between all mechanical characteristics and moisture loss.

A whole-body 1.5 T superconducting MRI magnet is operated in 4.2 K inside a zero-boil-off helium cryogenic vessel, i.e., cryostat. The cryostat has horizontal bore of 850 mm used as the patient bore. The cryostat houses the 1.5 T superconducting magnet in a coaxial position maintaining a high degree of alignment along with the gradient coil, the birdcage RF coil necessary for any MRI scanner. A set of support link having low thermal conductivity and higher mechanical strength would provide structural support to the cold mass at 4.2 K, which includes the superconducting magnet, helium vessel. A two stage 4.2 K G-M cryocooler is used to recondense helium to achieve a “zero boil-off” condition. The helium vessel and the vacuum vessel of the cryostat experience stresses during the normal operation, transportation etc. The ASME B&PV Code and the Finite Element Analysis has been used for the engineering design and stress analysis of cryostat. The modal analysis of the cryostat has been done during the transportation of the cryostat. The seismic behavior of the cryostat has also been analyzed extensively for a 1.5 T MRI cryostat.
