What is already known about this topic? No live specimens of the snail Oncomelania hupensis (O. hupensis) and indigenous infected cases of schistosomiasis japonicum have been found in Guangdong Province since 1993, but live O. hupensis was found again in 2019. This study conducted O. hupensis identification and elimination. What is added by this report? In 2019, live O. hupensis specimens were detected by routine surveillance in areas in Qujiang of Shaoguan City and Yingde of Qingyuan City, and an emergency response was launched immediately. What are the implications for public health practice? The suspected habitat of O. hupensis in originally endemic areas of schistosomiasis in Guangdong is still complicated, so it is necessary to record suspected habitats comprehensively and carry out scientific routine surveillance for O. hupensis.

What is already known about this topic? Oncomelania hupensis(O. hupensis) and livestock are main infection sources of schistosomiasis. The schistosome infected O. hupensis and livestock’s feces are important risk factors in the transmission of schistosomiasis. What is added by this report? The potential risks of schistosomiasis transmission remain prevalent, giving an early warning to local government with information on existing transmission risks. It is expected that the effectiveness and efficiency of schistosomiasis surveillance could be improved by conducting rapid risk assessment at the beginning of transmission season. What are the implications for public health practice? Rapid risk assessment is essential in early detection and the active monitoring of indicators of the transmission risks of schistosomiasis in endemic areas. This could work synergistically with surveillance system to minimize infections and prevent rebounds of endemic schistosomiasis outbreaks.

What is already known about this topic? The endemic status of schistosomiasis appeared to continuously decrease in China from 2015 to 2019. The snail species Oncomelania hupensis is the only intermediate host involved in the transmission of Schistosoma japonicum, and this snail’s geographic distribution is strictly consistent with that of schistosomiasis. What is added by this report? The snail’s habitats did not decrease significantly in China from 2015 to 2019, and some habitats have been newly detected or recurrent in some regions. Snail habitats among nine counties in Hunan and Jiangxi covered nearly half of the areas with snails. What are the implications for public health practice? Considering the situation of snail distribution, strategies and measures on snail control should focus on key areas. In addition, study of the origin and causes of the newly-detected snail habitats and recurrent areas with snails needs to be strengthened, and comprehensive measures should be taken to prevent the spread of snails.

What is already known about this topic? Soil-transmitted helminthiasis (STH) is distributed widely in China and a large number of the population is afflicted. However, trends of STH infections are decreasing. What is added by this report? The most recent data indicates that the overall prevalence of STH was 1.29% in 2018 in China, which was based on the national sentinel surveillance and demonstrates a continuous decline pattern. What are the implications for public health practice? Considering the current prevalence in China and various endemic statuses in different regions, precision control measures should be implemented for the control and elimination of STH in China.

Additional file 4: Table S1 Primer sequences used for qPCR analysis

This scoping review analyzes the research gaps of three diseases: schistosomiasis japonica, malaria and echinococcosis. Based on available data in the P.R. China, we highlight the gaps between control capacity and prevalence levels, and between diagnostic/drug development and population need for treatment at different stages of the national control programme. After reviewing the literature from 848 original studies and consultations with experts in the field, the gaps were identified as follows. Firstly, the malaria research gaps include (i) deficiency of active testing in the public community and no appropriate technique to evaluate elimination, (ii) lack of sensitive diagnostic tools for asymptomatic patients, (iii) lack of safe drugs for mass administration. Secondly, gaps in research of schistosomiasis include (i) incongruent policy in the implementation of integrated control strategy for schistosomiasis, (ii) lack of effective tools for Oncomelania sp. snail control, (iii) lack of a more sensitive and cheaper diagnostic test for large population samples, (iv) lack of new drugs in addition to praziquantel. Thirdly, gaps in research of echinococcosis include (i) low capacity in field epidemiology studies, (ii) lack of sanitation improvement studies in epidemic areas, (iii) lack of a sensitivity test for early diagnosis, (iv) lack of more effective drugs for short-term treatment. We believe these three diseases can eventually be eliminated in mainland China if all the research gaps are abridged in a short period of time.
