Current European Commission (EC) surveillance regulations require discriminatory testing of all transmissible spongiform encephalopathy (TSE)-positive small ruminant (SR) samples in order to classify them as bovine spongiform encephalopathy (BSE) or non-BSE. This requires a range of tests, including characterization by bioassay in mouse models. Since 2005, naturally occurring BSE has been identified in two goats. It has also been demonstrated that more than one distinct TSE strain can coinfect a single animal in natural field situations. This study assesses the ability of the statutory methods as listed in the regulation to identify BSE in a blinded series of brain samples, in which ovine BSE and distinct isolates of scrapie are mixed at various ratios ranging from 99% to 1%. Additionally, these current statutory tests were compared with a new in vitro discriminatory method, which uses serial protein misfolding cyclic amplification (sPMCA). Western blotting consistently detected 50% BSE within a mixture, but at higher dilutions it had variable success. The enzyme-linked immunosorbent assay (ELISA) method consistently detected BSE only when it was present as 99% of the mixture, with variable success at higher dilutions. Bioassay and sPMCA reported BSE in all samples where it was present, down to 1%. sPMCA also consistently detected the presence of BSE in mixtures at 0.1%. While bioassay is the only validated method that allows comprehensive phenotypic characterization of an unknown TSE isolate, the sPMCA assay appears to offer a fast and cost-effective alternative for the screening of unknown isolates when the purpose of the investigation was solely to determine the presence or absence of BSE.

Asthma and rhinitis frequently coexist in the same subjects. Allergic Rhinitis and its Impact on Asthma (ARIA) was initiated during a World Health Organization workshop in 1999 (published in 2001). ARIA has reclassified ARas mild/moderate-severe and intermittent/persistent. This classification closely reflects patients' needs and underlines the close relationship between rhinitis and asthma. Patients, clinicians, and other health care professionals are confronted with various treatment choices for the management of AR. This contributes to considerable variation in clinical practice, and worldwide, patients, clinicians, and other health care professionals are faced with uncertainty about the relative merits and downsides of the various treatment options. in its 2010 Revision, ARIA developed clinical practice guidelines for the management of AR and asthma comorbidities based on the Grading of Recommendation, Assessment, Development and Evaluation (GRADE) system. ARIA is disseminated and implemented in more than 50 countries of the world. The links between asthma and rhinitis are well characterized. The Allergic Rhinitis and its Impact on Asthma (ARIA) guidelines stress the importance of these links and provide guidance for their prevention and treatment. Despite effective treatments being available, too few patients receive appropriate medical care for both diseases. Most patients with rhinitis and asthma consult primary care physicians and therefore these physicians are encouraged to understand and use ARIA guidelines. Patients should also be informed about these guidelines to raise their awareness of optimal care and increase control of the two related diseases. To apply these guidelines, clinicians and patients need to understand how and why the recommendations were made. The goal of the ARIA guidelines is to provide recommendations about the best management options for most patients in most situations. These recommendations should be based on the best available evidence. Making recommendations requires the assessment of the quality of available evidence, deciding on the balance between benefits and downsides, consideration of patients’ values and preferences, and, if applicable, resource implications. Guidelines must be updated as new management options become available or important new evidence emerges. Transparent reporting of guidelines facilitates understanding and acceptance, but implementation strategies need to be improved.

Allergic rhinitis is a symptomatic disorder of the nose induced after allergen exposure by an IgE-mediated inflammation of the membranes lining the nose. It is a global health problem that causes major illness and disability worldwide. Over 600 million patients from all countries, all ethnic groups and of all ages suffer from allergic rhinitis. It affects social life, sleep, school and work and its economic impact is substantial. Risk factors for allergic rhinitis are well identified. Indoor and outdoor allergens as well as occupational agents cause rhinitis and other allergic diseases. The role of indoor and outdoor pollution is probably very important, but has yet to be fully understood both for the occurrence of the disease and its manifestations. In 1999, during the Allergic Rhinitis and its Impact on Asthma (ARIA) WHO workshop, the expert panel proposed a new classification for allergic rhinitis which was subdivided into 'intermittent' or 'persistent' disease. This classification is now validated. The diagnosis of allergic rhinitis is often quite easy, but in some cases it may cause problems and many patients are still under-diagnosed, often because they do not perceive the symptoms of rhinitis as a disease impairing their social life, school and work. The management of allergic rhinitis is well established and the ARIA expert panel based its recommendations on evidence using an extensive review of the literature available up to December 1999. The statements of evidence for the development of these guidelines followed WHO rules and were based on those of Shekelle et al. A large number of papers have been published since 2000 and are extensively reviewed in the 2008 Update using the same evidence-based system. Recommendations for the management of allergic rhinitis are similar in both the ARIA workshop report and the 2008 Update. In the future, the GRADE approach will be used, but is not yet available. Another important aspect of the ARIA guidelines was to consider co-morbidities. Both allergic rhinitis and asthma are systemic inflammatory conditions and often co-exist in the same patients. In the 2008 Update, these links have been confirmed. The ARIA document is not intended to be a standard-of-care document for individual countries. It is provided as a basis for physicians, health care professionals and organizations involved in the treatment of allergic rhinitis and asthma in various countries to facilitate the development of relevant local standard-of-care documents for patients.

International audience; Background: In Great Britain, sheep scrapie cases have been detected through active and passive surveillance programmes which do not conform to the characteristics usually associated with classical scrapie. These cases, designated as ‘atypical scrapie’, demonstrate a weak positive PrPres reaction in Bio-Rad ELISA and in histopathology produce a weak immunohistochemistry signal and demonstrate a different PrPSc distribution pattern compared to classical scrapie. Other features, such as an absence of vacuolation at the obex and a characteristic PrPres Western blotting profile are also apparent. Aim: The aim of this Defra-funded study was to determine the transmissibility of atypical scrapie cases from Britain to transgenic PrP and conventional mice, and to examine the biochemical and histopathological characteristics of the transmitted agent(s). Methods: Atypical scrapie isolates were selected from sheep carrying a range of PrP genotypes and inoculated into tg338, C57BL/6 and VM mice. Inoculated mice were clinically monitored and at termination brain samples were examined by Western blotting, lesion profiling and immunohistochemistry. Results: Eighteen atypical scrapie isolates derived from sheep of different genotypes transmitted successfully to tg338 mice at VLA and INRA. Incubation periods, lesion profiles, PrPSc distribution and Western blot profiles of the examined cases were similar, suggesting the isolation of a single strain. Furthermore, the biochemical and histopathological characteristics produced were indistinguishable from those of Nor 98 in the same transgenic mouse line. Bioassay in wild-type mice is ongoing with no TSE positive mice identified up to 431 days post-inoculation. Conclusion: Our findings suggest that the atypical scrapie cases from GB represent infection resulting from a single strain of TSE agent that is indistinguishable from Nor98.

The pharmacologic treatment of allergic rhinitis proposed by ARIA is an evidence-based and step-wise approach based on the classification of the symptoms. The ARIA workshop, held in December 1999, published a report in 2001 and new information has subsequently been published. The initial ARIA document lacked some important information on several issues. This document updates the ARIA sections on the pharmacologic and anti-IgE treatments of allergic rhinitis. Literature published between January 2000 and December 2004 has been included. This thesis describes the investigation of time-division multiplexing for use in mUlticomponent laser Doppler anemometry (LDA). To make two- or three- dimensional velocity measurements of a flow, two or three separate velocity measurements in different, non-collinear or non-coplanar directions respectively must be taken from which the flow vector can be calculated. It is necessary to separate each velocity channel. Conventionally, the separation is carried out by optical techniques, such as separating the signal from each channel by wavelength or polarisation, with one detector per channel. In time division multiplexing, each channel is switched On and off in turn. A single detector records a stream of interleaved pulses, and each channel is extracted by taking every other or every third pulse. The envelope of the amplitudes of the pulses varies at the Doppler frequency. A novel system has been demonstrated which uses a single laser diode source and a single detector. The distribution of the pulses to each channel and the delay between each channel is carried out using a optical fibre network with fibre delay lines, with a consequent reduction in 'electronic complexity. The characteristics of the optoelectronic components used in the system are tested, and two signal processing schemes are investigated. A two-dimensional Doppler difference fibre-linked probe was constructed to demonstrate the technique. Hybrid instruments using two Doppler difference channels and a reference beam channel to measure orthogonal velocity components offer the possibility of avoiding the errors introduced by non-orthogonal systems. A pulsed-diode reference beam anemometer based on optical fibres and suitable for inclusion in a time-division multiplexed hybrid instrument was tested.
