2. Ein kombinierter Operationsansatz zur Therapie der rektovaginalen Endometriose auf der Basis histologischer Befunde

Einleitung: Eine Endometriose des Septum rectovaginale kann oberflächlich dem Darm aufliegen oder die Darmwand infiltrieren. Durch bildgebende Verfahren kann jedoch präoperativ eine Infiltration des Darms weder hinreichend bestätigt noch ausgeschlossen werden, was zu einem therapeutischen Dilemma führt, da nur die Darmwand eindeutig infiltrierende Endometrioseprozesse durch eine Darmresektion behandelt werden sollten. Wir möchten ein neues Operationsverfahren vorstellen, das eine intraoperative Differenzierung zwischen einer oberflächlichen und einer infiltrativen Darmendometriose erlaubt. Patienten und Methodik: 70 Patientinnen mit rektovaginaler Endometriose wurden nach einem neu entwickelten kombinierten vaginal-laparoskopisch-abdominalen Verfahren operiert. Diese Operationsmethode wird Schritt für Schritt vorgestellt. Die Entscheidung zur Darmresektion wurde dabei allein aufgrund der intraoperativen Befunde und nicht aufgrund der präoperativen bildgebenden Diagnostik getroffen. Alle Resektate wurden standardisiert histologisch aufgearbeitet. Ergebnisse: Ob eine Darmresektion notwendig war, ließ sich intraoperativ anhand der Präparation des rektovaginalen Septums entscheiden. Daher wurden nur bei Patientinnen mit gesichertem infiltrativem Darmbefall Darmresektionen unter Erhalt des Mesointestinums durchgeführt. Weder intraoperative noch postoperative Komplikationen wie Anastomoseninsuffizienzen oder Restharnmengen traten auf. Diskussion: Die vorgestellte Operationsmethode zur Therapie der rektovaginalen Endometriose erlaubt eine exakte Diagnosestellung und präzise Therapieentscheidung bei minimaler Morbidität. Die Resektion des Darmschlauches ohne Mesoresektion ist ausreichend, da sich die Endometrioseknoten wie histologisch bewiesen wurde nur im ventralen Darmbereich befinden. Durch die Mesoerhaltung werden die vegetativen Funktionen des kleinen Beckens nicht beeinträchtigt.

3. Permissive expansion and homing of adoptively transferred T cells in tumor-bearing hosts

Activated T cells expressing endogenous or transduced TCRs are two cell types currently used in clinical adoptive T-cell therapy. The ability of these cells to recognize their antigen, expand, and traffic to the tumor site are the initial steps necessary for successful therapy. In this study, we used in vivo bioluminescent imaging (BLI) of Renilla luciferase (RLuc) expressing T cells to evaluate the ability of adoptively transferred T cells to survive, expand and home to tumor site in vivo. Using this method, termed RT-Rack (Rluc T cell tracking), we followed T-cell response against tumors in vivo. Expansion and homing of adoptively transferred T cells were antigen dependent, but independent of the host immune status. Moreover, we successfully detected T-cell response to small and large tumors, including autochthonous liver tumors. The adoptively transferred T cells were not ignorant or excluded in a partially tolerant host, which expressed low level of the target in the periphery. Using T cell receptor-engineered T cells, we showed the ability of these cells to respond in tumor-bearing hosts by expanding and homing to the tumor site. In all these models, the host immune status, the nature of the tumor or of the antigen, the tumor size, and the presence of the targeted antigen in the periphery did not prevent the adoptively transferred T cells from responding by expanding and homing to the tumor. However, T cells had higher expression of the inhibitory receptor PD1 and reduced functional activity when a self-antigen was targeted.

4. Cellular senescence predicts treatment outcome in metastasised colorectal cancer

Background: Cellular senescence is a terminal cell-cycle arrest that occurs in response to activated oncogenes and DNA-damaging chemotherapy. Whether cancer cell senescence at diagnosis might be predictive for treatment outcome is unknown. Methods: A senescence index (SI) was developed and used to retrospectively correlate the treatment outcome of 30 UICC stage IV colorectal cancer (CRC) patients with their SI at diagnosis. Results: 5-Fluorouracil/leucovorin-treated CRC patients achieved a significantly longer progression-free survival when presenting with SI-positive tumours before therapy (median 12.0 vs 6.0 months; P=0.044). Conclusion: Cancer cell senescence predicts treatment outcome in metastasised CRC. Prospective analyses of larger patient cohorts are needed.

Activated RAS/BRAF oncogenes induce cellular senescence as a tumor-suppressive barrier in early cancer development, at least in part, via an oncogene-evoked DNA damage response (DDR). In contrast, Myc activation-although producing a DDR as well-is known to primarily elicit an apoptotic countermeasure. Using the Emu-myc transgenic mouse lymphoma model, we show here in vivo that apoptotic lymphoma cells activate macrophages to secrete transforming growth factor beta (TGF-beta) as a critical non-cell-autonomous inducer of cellular senescence. Accordingly, neutralization of TGF-beta action, like genetic inactivation of the senescence-related histone methyltransferase Suv39h1, significantly accelerates Myc-driven tumor development via cancellation of cellular senescence. These findings, recapitulated in human aggressive B cell lymphomas, demonstrate that tumor-prompted stroma-derived signals may limit tumorigenesis by feedback senescence induction.

Background: We investigated whether alterations of transient receptor potential canonical (TRPC) channel expression may be observed in tissues from Munich Wistar Frömter (MWF) rats showing proteinuria compared to control Wistar rats. Methods: TRPC expression was investigated in tissue from MWF and Wistar rats using quantitative real time PCR, immunoblotting, and immunohistochemistry. Results: Compared to Wistar rats MWF rats showed significantly increased systolic blood pressure and significantly higher left ventricle weight (each p < 0.01). Quantitative real time PCR revealed that TRPC3 transcripts were significantly higher in kidney cortex from MWF rats compared to Wistar rats (p < 0.01). TRPC3 transcripts were not significantly different in kidney medulla nor in aorta from both groups (p = n.s.). Furthermore, TRPC6 transcripts were significantly lower in kidney cortex from MWF rats compared to Wistar rats (p < 0.001). Immunoblotting showed that TRPC3 channel protein expression was also significantly higher in kidney cortex from MWF rats compared to Wistar rats (p < 0.01). There was a significant correlation of TRPC3 mRNA and a specific marker for endothelium, von Willebrand factor (vWF; Spearman r = 0.564; p < 0.01). We observed a significant correlation between the TRPC3 transcripts to TRPC6 transcripts ratio in kidney cortex and urinary albumin excretion (Spearman r = 0.785, p < 0.001). Conclusion: Altered TRPC expression pattern in kidney cortex is associated with kidney damage in MWF rats showing hypertension and albuminuria.

9. Untypische Primärmanifestationen einer Syphiliserkrankung im Hals-Nasen-Ohrengebiet

In unserer Klinik stellten sich zwei Männer (32 und 48 Jahre) vor, die über rezidivierendes Fieber, Abgeschlagenheit sowie über eine indolente linkszervikale Schwellung mit zunehmender Dysphagie und Odynophagie seit 3-4 Wochen klagten. Bei einem Patienten bestand eine HIV-Infektion. Bei der Untersuchung zeigten sich bei dem einen Patienten Ulzerationen im Bereich der Tonsille und bei dem anderen Patienten Ulzerationen im Bereich der Taschenfalte und Epiglottis. Eine histologische Untersuchung, der im Rahmen eine Panendoskopie entnommen Proben, zeigte eine spezifische Entzündung mit Spirochäten. Eine daraufhin erfolgte serologische Untersuchung bestätigte die Diagnose einer primären Syphilis.Eine Ulzeration im Mund bzw. Rachen mit begleitender Lymphadenitis ist eine relativ untypische Primärmanifestation einer Syphilis. Dabei ist es zunächst wichtig ein malignes Geschehen histologisch auszuschließen. Orale Primärmanifestationen befinden sich normalerweise an den Lippen, an der Zunge, am Gaumen oder in der Mund- und Wangenschleimhaut. Die steigende Syphilisrate unter Männern mit gleichgeschlechtlichem Sexualverkehr in Kombination mit ungeschützten Orogenital-Praktiken wird die Inzidenz von untypischen Primärmanifestationen, wie sie in unserem Fall beschrieben werden, erhöhen. Dies ist vom klinisch tätigen Arzt zu beachten.

Enhanced NF-kappaB activity is involved in the pathology of both forms of inflammatory bowel disease (IBD), Crohn disease (CD) and ulcerative colitis (UC). Here we analyzed the mechanism of proteasome-mediated NF-kappaB activation in CD and UC. Our studies demonstrate that the subunit composition and the proteolytic function of proteasomes differ between UC and CD. High expression of the immunoproteasome subunits beta1i and beta2i is characteristic of the inflamed mucosa of CD. In line with this, we found enhanced processing of NF-kappaB precursor p105 and degradation of inhibitor of NF-kappaB, IkappaBalpha, by immunoproteasomes isolated from the mucosa of CD patients. In comparison with healthy controls and CD patients, UC patients exhibited an intermediate phenotype regarding the proteasome-mediated processing/degradation of NF-kappaB components. Finally, increased expression of the NF-kappaB family member c-Rel in the inflamed mucosa of CD patients suggests that p50/c-Rel is important for IFN-gamma-mediated induction of immunoproteasomes via IL-12-driven Th1 responses. These findings suggest that distinct proteasome subunits influence the intensity of NF-kappaB-mediated inflammation in IBD patients.

Mantle cell lymphoma (MCL) has a heterogeneous clinical course and is mainly an aggressive B cell non-Hodgkin lymphoma; however, there are some indolent cases The Ki-67 index, defined by the percentage of Ki-67-positive lymphoma cells on histopathological slides, has been shown to be a very powerful prognostic biomarker. The pathology panel of the European MCL Network evaluated methods to assess the Ki-67 index including stringent counting, digital image analysis, and estimation by eyeballing. Counting of 2 × 500 lymphoma cells is the gold standard to assess the Ki-67 index since this value has been shown to predict survival in prospective randomized trials of the European MCL Network. Estimation by eyeballing and digital image analysis showed a poor concordance with the gold standard (concordance correlation coefficients [CCC] between 0.29 and 0.61 for eyeballing and CCC of 0.24 and 0.37 for two methods of digital image analysis, respectively). Counting a reduced number of lymphoma cells (2 × 100 cells) showed high interobserver agreement (CCC = 0.74). Pitfalls of the Ki-67 index are discussed and guidelines and recommendations for assessing the Ki-67 index in MCL are given.
