震災BCPの策定支援などを目的に，建物基本情報から構造耐力を推定し，1自由度系にモデル化した建物の限界耐力計算によって建築構造・非構造部材・建築設備の被害予測を行う簡易動的耐震評価方法を提案して，その被害予測精度を検証した．近年，2011年の東日本大震災に関する各種報告書の発刊や建物の地震観測記録の公開によって，施設情報，地震観測記録，被害情報などを複合して利用できる環境が整ってきたことから，28棟の建物を対象として簡易動的耐震評価方法による被害予測精度の検証を行った．この結果，提案した簡易動的耐震評価方法は応答をやや大きめに評価する場合があるものの，被害判定ではほぼ妥当な結果が得られており，複数の既往建物の耐震性調査や地震対策の計画段階などにおけるスクリーニング評価としては有効な方法となることが確認できた., In order to support earthquake disaster BCP, the authors proposed a simple dynamic earthquake resistance evaluation method using basic information of the building, which consists of a method of estimating the load capacity coefficient and a method of predicting the damage to the building structure from the capacity spectrum method for the one-degree-of-freedom building model. However, problems such as verification of the damage prediction accuracy of a simple dynamic seismic evaluation method remained unsolved. In recent years, environments for verification analysis that include, for example, facility information, earthquake observation records, and damage information have been prepared by publishing various building damage reports and earthquake observation records for buildings affected by the 2011 Great East Japan Earthquake. For this reason, the accuracy of damage prediction by a simple dynamic earthquake resistance evaluation method was verified using public information on 28 buildings. Although the simple dynamic earthquake resistance evaluation method sometimes gives a slightly higher response value, the resultant damage assessment was for the most part reasonable. It was therefore confirmed that this method is effective as a screening evaluation method in seismic resistance surveys of multiple existing buildings and at the planning stage for earthquake countermeasures.

Herein is reported a case of the nutcracker phenomenon with severe gross hematuria which was surgically treated. A 14-year-old boy had been suffering from left flank pain and recurrent gross hematuria causing urinary retention with blood clots. Radiological images including three-dimensional computed tomography (CT) and angiography disclosed the nutcracker phenomenon with left renal venous hypertension. Since the aforementioned symptoms could not be managed conservatively, he underwent transposition of the left renal vein at 5.5 cm caudal from the original position. Postoperative course was uneventful and the patient has had no further hematuria after the surgery.

79歳男.約10年前に膀胱癌に対して膀胱全摘術, 舟状窩以外の尿道摘除術と回腸導管増設術を施行し, 追加治療は行わず経過観察となっていたが, CTで左鼠径部リンパ節腫大を指摘された.経過観察となったが, 増大傾向を認めたため膀胱癌術後左鼠径部リンパ節転移と診断し, 鼠径部リンパ節摘除術を施行した.術後は全身化学療法を希望せず経過観察となったが, 胸部X-P上で肺転移が指摘された.更に, 左鼠径部のリンパ節腫大が再発し, 摘出術を行ったが, 肺転移の増悪とリンパ節腫大の再発を認め, 化学療法は効果なく, 亀頭部先端の乳頭状腫瘤も指摘された.その後, 全身状態が徐々に悪化し, 呼吸不全で死亡した, A 79-year-old man had undergone radical cystourethrectomy for bladder carcinoma in January, 1989. Pathological report was Small cell carcinoma (SCC) >> transitional cell carcinoma (TCC), G2 > G3, pT4 (prostate), ew (-). Ten years later, follow-up computed tomography (CT) revealed swollen left inguinal lymph node in October 1998 and lymph node dissection was performed in January, 2000. The pathological report showed TCC, G2. Left inguinal lymph node metastases appeared again in January, 2001. Chest X-ray films showed multiple lung metastases in March, 2001. Three couses of MVAC (methotrexate, vinblastine, doxorubicin cisplatinum) chemotherapy had been performed since September, 2001 but were in effective. Papillary tumor was observed at external urethral meatus in September, 2002 and the biopsied specimens showed TCC, G1 > G2, pathologically. Finally he died of respiratory insufficiency in January, 2003. It is suggested that the recurrent TCC tumor in the urethral remnants might metastasize into the inguinal lymph nodes.

The usefulness of a chemical prosthesis was studied for the diagnosis and treatment of 20 patients with erectile impotence (14 due to diabetes, 2 due to pelvic surgery and 4 due to psychogenic factors) using papaverine hydrochloride and phentolamine mesilate. Absolute erection was observed in 16 patients (80%) and coital penetration was possible in all 9 patients who actually tried sexual intercourse. Penile arterial blood flow was measured in 15 patients using a real-time mechanical sector duplex scanner (Diasonics DRF/400V) just before and after the chemical prosthesis, and a marked increase in blood flow was observed in all patients after administration. From these results, it is considered that measurement of the penile blood flow using a mechanical sector duplex scanner is useful in the diagnosis and monitoring of impotence. Furthermore, a chemical prosthesis is suitable mainly for the treatment of impotence due to diabetes, by adjusting the dosage and administration.

A case of retroperitoneal germ cell tumors in a 32-year-old male is reported. He was admitted to our hospital with complaints of lumbago and abdominal mass. He was diagnosed with primary retroperitoneal malignant tumor and underwent resection of the tumor with retroperitoneal lymphadenectomy. The final pathological diagnosis revealed germ cell tumors of more than one histological type in the retroperitoneal space. Serum human chorionic gonadotropin (hCG) and hCG-beta levels were very high preoperatively (4, 8000 mIU/ml, 57 ng/ml, respectively) and they decreased to normal after vinblastine-bleomycin cisplatin combination chemotherapy three times postoperatively. The patient later developed a solitary lung metastasis and died in another hospital 7 months after the operation. Retroperitoneal germ cell tumors in adults are rare. One question continually arises: are they extragonadal occurrences or germ cell tumors or do they represent metastatic lesions from occult testicular tumors?
