Electrolysis based on renewable energies offers a promising carbon-free solution for hydrogen generation and storage. The recent developments of proton ceramic electrolysis cells operating at intermediate temperatures bear promise of superior energy efficiency compared to oxide ion conducting electrolytes. Here, a proton ceramic Single Engineering Unit (SEU) design is optimized for steam electrolysis using a computational fluid dynamics (CFD) model implemented in a COMSOL Multiphysics software. The SEU is an all-in-one tubular cell arrangement that constitutes the smallest electrolysis unit and enables efficient, adaptable pressurized hydrogen generation. The parametrical modelling study is conducted for two adiabatic operation scenarios with distinct steam conversion rates and tested for multiple key parameters, namely internal and external chamber pressures and inlet stream temperature. The modelling results show that low steam conversions enable operation at higher current densities and that the thermoneutral voltage for a fixed steam conversion is highly sensitive to the process conditions and operation modes. The increment of the pressure of the generated hydrogen implies a reduced production rate at thermoneutral voltage, although it can be compensated with an enhanced steam pressure or a reduced inlet temperature. Additionally, the introduction of a porous medium as the SEU current collector in the steam chamber enhances heat transport within this chamber. The area specific resistance of the system determines the current density, enforcing an adaption of the area of the electrolyser to satisfy the target hydrogen production and energy efficiency. The resulting proposed SEU design and adapted operational parameters allow effective delivery of pressurized dry hydrogen for a wide range of conditions and applications., The work leading to these results has received funding from Spanish Government (RTI2018-102161 grant) and from Fuel Cells and Hydrogen 2 Joint Undertaking under grant agreement 779486 (‘GAMER’). Asthma is one of the main chronic diseases in childhood, due to its high prevalence and its social and health costs. This document is a summary of a consensus guideline approved by 6 Spanish pediatric societies related to asthma and endorsed by the Spanish Pediatric Association. Asthma is easily identifiable by clinical criteria in most patients. Spirometry and other tests are helpful for diagnosis, especially in atypical cases. Asthma exacerbation is a frequent manifestation of the disease and must be identified and treated promptly. When asthma symptoms are frequent and the quality of life is affected, maintenance treatment must be instituted to achieve control of the disease. Low-dose inhaled corticosteroids are effective and safe for long-term use. Education of the patient with asthma is essential for good control. The main reason for poor asthma control is non-compliance with treatment, either due to its erratic and insufficient administration, or due to poor application technique of inhaled drugs. If control is not obtained despite adequate treatment, the diagnosis must be reconsidered, as well as the factors or comorbidities that make control difficult. Other drugs can be added to avoid high doses of inhaled corticosteroids, notably montelukast or long-acting beta 2 adrenergic agonists. Severe or difficult-to-control asthma, which does not respond to the usual treatments, should be managed in specialized units. (C) 2021 Asociacion Espanola de Pediatria. Importance:The Patient Health Questionnaire depression module (PHQ-9) is a 9-item self-administered instrument used for detecting depression and assessing severity of depression. The Patient Health Questionnaire-2 (PHQ-2) consists of the first 2 items of the PHQ-9 (which assess the frequency of depressed mood and anhedonia) and can be used as a first step to identify patients for evaluation with the full PHQ-9. Objective:To estimate PHQ-2 accuracy alone and combined with the PHQ-9 for detecting major depression. Data Sources:MEDLINE, MEDLINE In-Process & Other Non-Indexed Citations, PsycINFO, and Web of Science (January 2000-May 2018). Study Selection:Eligible data sets compared PHQ-2 scores with major depression diagnoses from a validated diagnostic interview. Data Extraction and Synthesis:Individual participant data were synthesized with bivariate random-effects meta-analysis to estimate pooled sensitivity and specificity of the PHQ-2 alone among studies using semistructured, fully structured, or Mini International Neuropsychiatric Interview (MINI) diagnostic interviews separately and in combination with the PHQ-9 vs the PHQ-9 alone for studies that used semistructured interviews. The PHQ-2 score ranges from 0 to 6, and the PHQ-9 score ranges from 0 to 27. Results:Individual participant data were obtained from 100 of 136 eligible studies (44 318 participants; 4572 with major depression [10%]; mean [SD] age, 49 [17] years; 59% female). Among studies that used semistructured interviews, PHQ-2 sensitivity and specificity (95% CI) were 0.91 (0.88-0.94) and 0.67 (0.64-0.71) for cutoff scores of 2 or greater and 0.72 (0.67-0.77) and 0.85 (0.83-0.87) for cutoff scores of 3 or greater. Sensitivity was significantly greater for semistructured vs fully structured interviews. Specificity was not significantly different across the types of interviews. The area under the receiver operating characteristic curve was 0.88 (0.86-0.89) for semistructured interviews, 0.82 (0.81-0.84) for fully structured interviews, and 0.87 (0.85-0.88) for the MINI. There were no significant subgroup differences. For semistructured interviews, sensitivity for PHQ-2 scores of 2 or greater followed by PHQ-9 scores of 10 or greater (0.82 [0.76-0.86]) was not significantly different than PHQ-9 scores of 10 or greater alone (0.86 [0.80-0.90]); specificity for the combination was significantly but minimally higher (0.87 [0.84-0.89] vs 0.85 [0.82-0.87]). The area under the curve was 0.90 (0.89-0.91). The combination was estimated to reduce the number of participants needing to complete the full PHQ-9 by 57% (56%-58%). Conclusions and Relevance:In an individual participant data meta-analysis of studies that compared PHQ scores with major depression diagnoses, the combination of PHQ-2 (with cutoff ≥2) followed by PHQ-9 (with cutoff ≥10) had similar sensitivity but higher specificity compared with PHQ-9 cutoff scores of 10 or greater alone. Further research is needed to understand the clinical and research value of this combined approach to screening.

Introduction: Three previous individual participant data meta-analyses (IPDMAs) reported that, compared to the Structured Clinical Interview for the DSM (SCID), alternative reference standards, primarily the Composite International Diagnostic Interview (CIDI) and the Mini International Neuropsychiatric Interview (MINI), tended to misclassify major depression status, when controlling for depression symptom severity. However, there was an important lack of precision in the results. Objective: To compare the odds of the major depression classification based on the SCID, CIDI, and MINI. Methods: We included and standardized data from 3 IPDMA databases. For each IPDMA, separately, we fitted binomial generalized linear mixed models to compare the adjusted odds ratios (aORs) of major depression classification, controlling for symptom severity and characteristics of participants, and the interaction between interview and symptom severity. Next, we synthesized results using a DerSimonian-Laird random-effects meta-analysis. Results: In total, 69,405 participants (7,574 [11%] with major depression) from 212 studies were included. Controlling for symptom severity and participant characteristics, the MINI (74 studies; 25,749 participants) classified major depression more often than the SCID (108 studies; 21,953 participants; aOR 1.46; 95% confidence interval [CI] 1.11-1.92]). Classification odds for the CIDI (30 studies; 21,703 participants) and the SCID did not differ overall (aOR 1.19; 95% CI 0.79-1.75); however, as screening scores increased, the aOR increased less for the CIDI than the SCID (interaction aOR 0.64; 95% CI 0.52-0.80). Conclusions: Compared to the SCID, the MINI classified major depression more often. The odds of the depression classification with the CIDI increased less as symptom levels increased. Interpretation of research that uses diagnostic interviews to classify depression should consider the interview characteristics. © 2020

Bronchopulmonary dysplasia (BPD) is the most common complication of preterm birth, and remains a major problem in pediatric pulmonology units. The decision of discharging from the Neonatal Unit should be based on a thorough assessment of the condition of the patient and compliance with certain requirements, including respiratory and nutritional stability, and caregiver education on disease management. For proper control of the disease, a schedule of visits and complementary tests should be established prior to discharge, and guidelines for prevention of exacerbations and appropriate treatment should be applied. In this paper, the Working Group in Perinatal Respiratory Diseases of the Spanish Society of Pediatric Pulmonology proposes a protocol to serve as a reference for the follow up of patients with BPD among different centers and health care settings. Key factors to consider when planning discharge from the Neonatal Unit and during follow up are reviewed. Recommendations on treatment and prevention of complications are then discussed. The final section of this guide aims to provide a specific schedule for follow-up and diagnostic interventions to be performed in patients with BPD.

The incidence of obesity and its most feared comorbidity, diabetes mellitus type 2, is increasing and there would not seem to be any medical treatment to help control these pandemics. However, there is a bariatric surgery technique, the Roux-en-Y Gastric Bypass (RYGB), which is safe and not only helps control excess weight, but produces encouraging results in the control and remission of diabetes.We present 15 selected patients with a BMI between 30 and 35 kg/mt² and diabetes type 2 who underwent a laparoscopic RYGB with of one-year follow-up.A total of 14 women and one man were operated with the following average values: age: 37 years, weight: 88.3 kg, BMI: 32.8 kg/mt², blood glucose: 120 ± 38.8 mg%, HbA1c: 7.6 ± 0.73. Forty percent (40%) suffered from high blood pressure and 33.3% were dyslipidemic. Average surgical time was 75 minutes, hospital length of stay was two days, and there was a low rate of complications and no mortality. Diabetes remission was achieved in 93% of cases with significant drops in blood glucose and HbA1c (p ≤ 0.05 and p ≤ 0.001 respectively), dyslipidemia was 100% controlled and hypertension was 83.3% controlled.RYGB in selected patients with obesity type 1 and diabetes mellitus type 2 is a safe and effective technique for metabolic control and obesity control.

Fifty-six bulk isolates of Trichoderma spp. from avocado (Persea americana Mill.), carnation (Dianthus caryophyllus L.), litchi (Litchi chinensis Sonn), rice (Oryza sativa L.) and sugar beet (Beta vulgaris L.) crops located in southern Spain were evaluated for antagonism against one isolate of Rosellinia necatrix Prill. Isolates of both types of fungi were tested in dual and cellophane culture. The origin, cultural characteristics, overgrowth sporulation and staining of growth medium were recorded. As a result, 21 Trichoderma bulk isolates were selected and their corresponding monoconidial isolates were evaluated as above. Next eight monoconidial Trichoderma isolates with the largest in vitro antagonism were selected, and they were additionally tested against nine representative isolates of R. necatrix from nine virulence groups. These were established after pathogenicity tests on 57 isolates of R. necatrix from diseased avocado orchards in southern Spain. These monoconidial Trichoderma isolates were considered as potential biological control agents with a high potential for effective control of white root rot of avocado., Se analizó el antagonismo de cincuenta y seis aislados masales de Trichoderma spp. procedentes de cultivos de aguacate (Persea americana Mill.), clavel (Dianthus caryophyllus L.), litchi (Litchi chinensis Sonn), arroz (Oryza sativa L.) y remolacha (Beta vulgaris L.) del Sur de España frente a un aislado de Rosellinia necatrix Prill. Los aislados de ambos hongos se analizaron in vitro en cultivos duales y de celofán, valorándose la procedencia, características culturales, sobrecrecimiento, esporulación y tinción del medio de cultivo. Se seleccionaron 21 aislados masales y se evaluaron sus correspondientes monoconídicos mediante los análisis in vitro citados. Posteriormente se seleccionaron los 8 aislados monoconídicos de Trichoderma que presentaron el mayor antagonismo in vitro. Estos 8 aislados se evaluaron adicionalmente mediante análisis frente a nueve aislados representativos de R. necatrix, procedentes de nueve grupos de virulencia. Estos se obtuvieron mediante análisis de patogenicidad de 57 aislados de R. necatrix procedentes de fincas de aguacate enfermas del Sur de España. Aquellos aislados monoconídicos de Trichoderma podrían considerarse como agentes de control biológico con un alto potencial para un efectivo control de la podredumbre blanca del aguacate.

La lipoaspiración, técnica tradicional usada para el tratamiento de las lipodistrofias con cicatrices mínimas, ha sido a través de los años mejorada con los avances anestésicos, farmacológicos y de instrumental. A su vez han aparecido otras técnicas que han publicitado ventajas respecto al tiempo de recuperación, sintomatología y resultados estéticos. Una de estas técnicas es la laserlipolisis a la que se atribuye menos dolor, menos equimosis y rápido postoperatorio. No se encuentran trabajos que acrediten estas ventajas, por lo que decidimos estudiar si existen ventajas de laserlipolisis en comparación con la lipoaspiración tradicional respecto del dolor, síntoma importante a considerar en el postoperatorio. Para esto se diseñó un trabajo prospectivo randomizado con una muestra de 60 pacientes a los cuales se aplicaron las dos técnicas en diferentes áreas y al mismo paciente. Se evaluó el dolor con escala numérica analógica a las 4 horas, 48 horas y 5 días de postoperatorio. Los resultados mostraron que la laserlipolisis presenta menos dolor en los muslos medido a las 48 horas y 5 días de la intervención. No hay diferencias entre estas dos técnicas en las otras zonas medidas como abdomen, áreas pretrocantéreas ("alforjas"), zona lumbar y pectoral. Considerando todos los pacientes sin diferenciar la técnica usada, las "alforjas" a las 4 horas duelen menos que el abdomen y a los 5 días duelen menos los muslos que el abdomen. No encontramos relación entre volumen aspirado e intensidad del dolor. The traditional suction-assisted lipoplasty, is a well established and commonly performed technique used for the treatment of lipodystrophias, which has undergone improvements with advances in anesthetics, pharmacology and introduction of new instruments. New techniques have appeared, promising advantages in recovery periods, symptomathology and aesthetic results when compared to the traditional suction-assisted lipoplasty. One of these newly introduced techniques is the laser-assisted lipoplasty (laserlipolysis), which claims less pain, less bruising and faster postoperative recovery. No evidence has been published to support these advantages when comparing laser-assisted lipoplasty to the traditional method, specifically when refering to pain, an important symptom to be taken into consideration in the postoperative period. For this reason we designed a prospective, randomized clinical study comprising 60 patients to which the two techniques were applied to different body areas. Pain was assessed using the numeric analog scale at 4 and 48 hours, and at 5 days after surgery. Evidence showed that laserlipolysis results in less pain of the thighs measured at 48 hours and 5 days after surgery. No difference in pain was found between both techniques on the other body areas evaluated (abdomen, trochanteric bulge, lumbar and pectoral areas). Assesing all patients, independent of the technique used, trochanteric bulge at 4 hours was found to be less painful than the abdominal area, and at 5 day evaluation the thighs were less painful than the abdominal area. No relationshipwas found between pain intensity and lipoaspirated volume.

Gestational diabetes mellitus (GDM) constitutes a risk factor for the development of non insulin-dependent diabetes mellitus (NIDDM). The search for parameters to provide discrimination between a high risk and a low risk for future development of NIDDM is today the aim of many investigations. The absence or presence of several factors such as glycemia during pregnancy and post partum, the need for insulin treatment, disorders of the pancreatic insulin secretion, the number of pregnancies, maternal obesity, the early diagnosis of GDM, the family history of diabetes mellitus, the race and immune disorders give rise to a very high relative risk (RR) of developing NIDDM. To know the degree of risk will allow a future appropriate clinical intervention to reduce the incidence of NIDDM and its economic cost.

Lung cancer screening (LCS) with low-dose computed tomography (LDCT) was demonstrated in the National Lung Screening Trial (NLST) to reduce mortality from the disease. European mortality data has recently become available from the Nelson randomised controlled trial, which confirmed lung cancer mortality reductions by 26% in men and 39–61% in women. Recent studies in Europe and the USA also showed positive results in screening workers exposed to asbestos. All European experts attending the “Initiative for European Lung Screening (IELS)”—a large international group of physicians and other experts concerned with lung cancer—agreed that LDCT-LCS should be implemented in Europe. However, the economic impact of LDCT-LCS and guidelines for its effective and safe implementation still need to be formulated. To this purpose, the IELS was asked to prepare recommendations to implement LCS and examine outstanding issues. A subgroup carried out a comprehensive literature review on LDCT-LCS and presented findings at a meeting held in Milan in November 2018. The present recommendations reflect that consensus was reached.
