Purpose This paper aims to explore the purpose and outline the key features of Liverpool Football Club Foundation’s County Lines (CL) programme and how principles of collaboration and co-production can be implemented to educate children at risk of entering the youth justice system. Design/methodology/approach This paper reviews the findings from a 12-week CL intervention programme in 14 secondary schools in the Liverpool City Region between 2021 and 2022. The programme was designed in collaboration with funders, partners and participants and aimed to improve knowledge of, and change attitudes towards CL and its associated harms, including knife crime and child exploitation. Findings Knowledge and attitude changes were measured across 12 indicators, with positive changes recorded for each indicator. Perhaps of most interest to those working in the sector was the recorded success in obtaining consistent attendance from beginning to end with very little erosion of engagement. This suggests that the content and method of delivery was successful in engaging harder to reach young people to make positive change. Originality/value To the best of the authors’ knowledge, this study is the first of its kind to examine how collaboration and co-production (two of the five principles of the Serious Violence Strategy 2018) can be implemented by a football charity and its partners to educate children in a local community on the harms of CL.

Tackling complex twenty-first century global health challenges requires crossdisciplinary collaborations that extend beyond physical classrooms and across continents. The Transdisciplinary Education and Community Health Collaboratory (TEaCH CoLab) is a global teaching co-op established by health promotion and humanities faculty at three universities (Waterford Institute of Technology and the Institute of Technology, Carlow in Ireland, and the University of Washington, Bothell in the U.S.). The primary goals of TEaCH CoLab are to enhance global learning and problem-solving among the next generation of community and public health practitioners, to improve public health teaching (with a focus on digital pedagogy), and to increase empathy and community-connectedness. In this descriptive article, we present our program model and lessons learned from the first three years of collaboration to provide insights into how such capacity-building projects are established and sustained over time and across diverse geographical, cultural and temporal landscapes. Collaboration happens primarily online through academic and community partnerships, collaborative online learning, and pedagogy discussion and development. Students get to engage with course content and experiential learning that is part of a shared, global curriculum which emphasizes social justice, health equity, cultural humility and anti-racism, and advocacy. Our “lessons for the field” are collective, practice-based reflections by members of TEaCH CoLab based on their experiences and their involvement in development and facilitation. Our model for learning may help other health promotion scholars and practitioners develop meaningful global learning experiences that strengthen the interconnectedness of praxis, pedagogy, and communities.

Data Science is a relatively new requirement for business students. Historically many business students shied away from business statistics and quantitative or Operational Research (OR) modules believing them to be boring and irrelevant. The high profile use of analytics and modelling during the Covid pandemic has drawn awareness to the relevance of analytics. Greater availability of data and modelling tools afford business students an opportunity to re-engage with Operational Research and Analytics and to enjoy the satisfaction of modelling and solving real world problems, but the challenge of the mathematical modelling skills gap of business students remains. In this paper we describe a learning pathway of modules in business analytics that can enhance business students’ confidence and capabilities in performing statistical and analytical business tasks. We recommend modelling tools and incremental innovative mathematical modelling teaching approaches that are pedagogically sound and suitable for business students with varying quantitative backgrounds.

In the context of simulation-based optimisation, this paper reviews recent work related to the role of metaheuristics, matheuristics (combinations of exact optimisation methods with metaheuristics), simheuristics (hybridisation of simulation with metaheuristics), biased-randomised heuristics for ‘agile’ optimisation via parallel computing, and learnheuristics (combination of statistical/machine learning with metaheuristics) to deal with NP-hard and large-scale optimisation problems in areas such as transport and logistics, manufacturing and production, smart cities, telecommunication networks, finance and insurance, sustainable energy consumption, health care, military and defence, e-marketing, or bioinformatics. The manuscript provides the main related concepts and updated references that illustrate the applications of these hybrid optimisation–simulation–learning approaches in solving rich and real-life challenges under dynamic and uncertainty scenarios. A numerical analysis is also included to illustrate the benefits that these approaches can offer across different application fields. Finally, this work concludes by highlighting open research lines on the combination of these methodologies to extend the concept of simulation-based optimisation.

Background Obesity increases the risk of type 2 diabetes, heart disease, stroke, mobility problems and some cancers, and its prevalence is rising. Men engage less than women in existing weight loss interventions. Game of Stones builds on a successful feasibility study and aims to find out if automated text messages with or without endowment incentives are effective and cost-effective for weight loss at 12 months compared to a waiting list comparator arm in men with obesity. Methods A 3-arm, parallel group, assessor-blind superiority randomised controlled trial with process evaluation will recruit 585 adult men with body mass index of 30 kg/m2 or more living in and around three UK centres (Belfast, Bristol, Glasgow), purposively targeting disadvantaged areas. Intervention groups: (i) automated, theory-informed text messages daily for 12 months plus endowment incentives linked to verified weight loss targets at 3, 6 and 12 months; (ii) the same text messages and weight loss assessment protocol; (iii) comparator group: 12 month waiting list, then text messages for 3 months. The primary outcome is percentage weight change at 12 months from baseline. Secondary outcomes at 12 months are as follows: quality of life, wellbeing, mental health, weight stigma, behaviours, satisfaction and confidence. Follow-up includes weight at 24 months. A health economic evaluation will measure cost-effectiveness over the trial and over modelled lifetime: including health service resource-use and quality-adjusted life years. The cost-utility analysis will report incremental cost per quality-adjusted life years gained. Participant and service provider perspectives will be explored via telephone interviews, and exploratory mixed methods process evaluation analyses will focus on mental health, multiple long-term conditions, health inequalities and implementation strategies. Discussion The trial will report whether text messages (with and without cash incentives) can help men to lose weight over 1 year and maintain this for another year compared to a comparator group; the costs and benefits to the health service; and men’s experiences of the interventions. Process analyses with public involvement and service commissioner input will ensure that this open-source digital self-care intervention could be sustainable and scalable by a range of NHS or public services. Community energy projects have gained popularity in recent years, and encouraging citizens to form local energy communities (LEC) is considered an effective tool for raising awareness about renewable energy. Since no single universal method exists for operating LECs, this study investigated the impact that different business models and asset dispatch methods have on LECs’ economic and energy-related indicators. We carried out a case study, which included the development, modelling, and simulation of seven scenarios using mixed-integer linear programming (MILP). To measure and compare the prospective performance of the LECs in each scenario, six key metrics were evaluated and assessed. The authors find that simple, rule-based control systems might be well suited for LECs with a limited number of controllable assets that aim to provide increased levels of self-consumption of up to 3%. We also conclude that when the LEC utilises an energy cooperative business model, the selected asset dispatch method provides only minor differences in LEC performance, while for prosumer communities, the importance of selecting a suitable asset dispatch method is higher. We also conclude that LECs have the potential to significantly increase their economic performance by more than 10% by acting as aggregators and providing grid services directly to system operators.

Background Physical activity (PA) interventions capable of producing health benefits cost effectively are a public health priority across the Western world. ‘Men on the Move’ (MOM), a community-based PA intervention for men, demonstrated significant health benefits up to 52-weeks (W) post-baseline. This article details the economic evaluation of MOM with a view to determining its cost-effectiveness as a public health intervention to be rolled out nationally in Ireland. Methods Cost-effectiveness was determined by comparing the costs (direct and indirect) of the programme to its benefits, which were captured as the impact on quality-adjusted life-years (QALYs). For the benefits, cost–utility analysis was conducted by retrospectively adapting various health-related measures of participants to generate health states using Brazier et al.’s (2002) short form-6D algorithm. This in turn allowed for ‘utility measures’ to be generated, from which QALYs were derived. Results Findings show MOM to be cost-effective in supporting an ‘at risk’ cohort of men achieves significant improvements in aerobic fitness, weight loss and waist reduction. The total cost per participant (€125.82 for each of the 501 intervention participants), the QALYs gained (11.98 post-12-W intervention, or 5.3% health improvement per participant) and estimated QALYs ratio costs of €3723 represents a cost-effective improvement when compared to known QALY guidelines. Conclusions The analysis shows that the cost per QALY achieved by MOM is significantly less than the existing benchmarks of £20 000 and €45 000 in the UK and Ireland respectively, demonstrating MOM to be cost-effective.

Introducción: La rinosinusitis crónica (RSC) es una de las enfermedades más prevalentes a nivel mundial. Los eosinófilos juegan un papel importante en la generación del proceso inflamatorio nasosinusal crónico secundario a la generación de una respuesta maladaptativa Th2 y cambios en la microbiota nasal. Objetivo: Determinar si la eosinofilia periférica puede usarse como factor predictor de severidad tomográfica en pacientes que cursan con rinosinusitis crónica. Métodos: Se realizó un estudio observacional de corte transversal con componente analítico en pacientes con diagnóstico de RSC quienes dispusieran en su registro de historia clínica un hemograma con recuento de eosinófilos. El tamaño de la muestra fue de 74 individuos. Resultados: La eosinofilia periférica se estableció como factor de severidad tomográfica encontrando que por cada incremento de 100 eosinófilos en sangre, aumentaría en 1 punto el puntaje total de la escala tomográfica Lund-Mackay. Este patrón fue igual de concordante en pacientes con pólipos y asma, encontrando incrementos en los puntajes totales de 4 y 5 puntos respectivamente, con respecto a los pacientes que no presentan estas patologías. Conclusión: La utilización de la eosinofilia periférica como predictor de severidad, podría ser de gran utilidad para la comunidad médica. Este biomarcador puede resultar en un ahorro potencial de costos al eliminar la necesidad de tratamientos médicos repetidos en pacientes que de entrada tienen un riesgo incrementado de enfermedad nasosinusal severa.

‘Connecting with Young Men’, Unit 6 in ENGAGE, Ireland’s National Men’s Health Training programme was developed to support service providers to engage young in mental health and related services. This study evaluated the impact of Unit 6 on front line service providers’ knowledge, skills, capacity, and practice pre and immediately post-training via questionnaire (n=206). At 1-month post-training interviews were conducted with youth workers (n=11), SPHE (social and emotional health curriculum) teachers (n=3), and sports personnel (n=3) (12-40 mins) to explore their experience of the training and its impact on practice. Background Minimally invasive esophagectomy (MIE) is performed in nearly 50% of patients worldwide. The effectiveness of the technique arises from a single randomized control trial and multiple single series cohorts. Consistent reporting of complications is varied. We describe our experience of transitioning to MIE compared with open esophagectomy (OE) with the use of Esophageal Complications Consensus Group (ECCG) standardized complication benchmark definitions. Methods Between 2007 and 2017, all patients undergoing esophagectomy were identified with the use of a prospectively curated database. Complications were defined by the ECCG and graded with the Clavien-Dindo (most severe complication) and comprehensive complication index (complexity of complications during hospital stay). Results Of 383 patients, 299 (76%) were men with a median age of 64.5 years (range, 56-72 years). MIE was performed in 49.6%. No differences were found in age, histologic finding (P = .222), pT stage (P = .136), or nodal positivity (P = .918). Stage 3 cancers accounted for 42.0% of OEs and 47.9% of MIEs. A thoracic anastomosis was more frequent in MIEs (156 of 190; 82.1%) than in OEs (113 of 193; 58.5%; P = .001). Frequency, severity (Clavien-Dindo), and complexity (comprehensive complication index) of complications were better in the MIE group, without compromising operative outcomes. No differences were identified in individual complication groupings or grade in MIEs compared with OEs (pneumonia: 19.5% versus 26.9% ([P = .09]; intensive care unit readmission: 7.4% versus 9.3% [P = .519]; atrial fibrillation: 11.1% versus 6.7% [P = .082], or grade of leak [P = .99]). Conclusions These results compare favorably to those reported by ECCG. MIE can be the standard approach for surgical management of esophageal cancer. Introduction of the approach in each surgeon's practice should be benchmarked to international standards.

The extraction of temporal events from text and the classification of temporal relations among both temporal events and time expressions are major challenges for the interface of data mining and natural language processing. We present an ensemble method, which reconciles the outputs of multiple heterogenous classifiers of temporal expressions. We use integer programming, a constrained optimisation technique, to improve on the best result of any individual classifier by choosing consistent temporal relations from among those recommended by multiple classifiers. Our ensemble method is conceptually simple and empirically powerful. It allows us to encode knowledge about the structure of valid temporal expressions as a set of constraints. It obtains new state-of-the-art results on two recent natural language processing challenges, SemEval-2013 TempEval-3 (Temporal Annotation) and SemEval-2016 Task 12 (Clinical TempEval), with F1 scores of 0.3915 and 0.595 respectively. The success of the energy transition in Europe depends on the engagement of citizens and the sustainable replacement of conventional generation with renewable production. Highlights of the PAN European Technology Energy Research Approach (PANTERA) project, a H2020 coordination and support action, are presented in this paper. In broad terms, PANTERA offers a forum for actors in the smart grid to support the expansion of activities in smart grid research, demonstration, and innovation, especially in the below-average spending member states in the European Union (EU). The focus of this paper is on those activities of the project consortium related to the identification of gaps and barriers in regulations, standards, and network codes that hinder the sustainable engagement of energy citizens in the energy transition. The paper summarises the challenges to citizen engagement in the energy transit and considers the enablers that make the engagement of citizens viable, e.g., demand response (DR), renewable energy resources (RESs), and modern designs for local energy markets (LEMs). We focus on the barriers to the enablers that are explicitly and implicitly related to regulations, standards, and network codes and explore aspects of the relevant regulations and standards of the sample below-average spending member states. A specific case study of a research and demonstration project in Ireland for updating the network codes is also presented to demonstrate the ways in which member states are attempting to remove the barriers and enable citizen participation in the smart energy transition. Finally, the opportunities to foster smart grid research and innovation through shared knowledge and insights offered by the PANTERA European Interconnection for Research Innovation and Entrepreneurship (EIRIE) platform are highlighted.

Background Over the past decade several physical activity (PA) interventions have been shown to be efficacious in a controlled research setting, however there is a continued lack of evidence for how to successfully implement these PA interventions in real-world settings such as the community. This review aims to explore the barriers and facilitators that affect the implementation of community-based PA interventions and make recommendations to improve implementation from the included studies. Methods A systematic literature search of EBSCOhost, Scopus, PUBMED and Web of Science was conducted to identify articles that reported qualitative data on the implementation factors of community-based interventions where PA was a primary outcome. Data were extracted using the Consolidated Framework for Implementation Research (CFIR) as a guide. Implementation factors and recommendations were then mapped onto the 5 domains of the CFIR and synthesised thematically. Results From 495 articles, a total of 13 eligible studies were identified, with 6 studies using a mixed methods approach, and 7 reporting qualitative methods only. There were 82 implementation factors identified, including 37 barriers and 45 facilitators, and a further 26 recommendations from the papers across all 5 domains of the CFIR. More barriers than facilitators were identified within the CFIR domain inner setting, in contrast to all other domains where facilitator numbers outweighed barriers. Conclusions This review identified many facilitators and barriers of implementing physical activity interventions in the community. A key finding of this review was the impact of implementation strategies on successful implementation of community PA interventions. From the evidence, it was clear that many barriers to implementation could have been negated or reduced by an implementation plan in which several strategies are embedded. The findings of this review also suggest more attention to individual’ skills and involvement is needed to improve self-efficacy and knowledge. The role of individuals across all organisational levels, from providers to leaders, can impact on the implementation of an intervention and its success. This paper explores gender dimensions of the energy transition in the European Union (EU). The EU has set out its ambitions for an equitable transition to clean secure energy. It has also set out it objectives for gender equality. In this paper, I implement a systematic literature review to explore the intersection of gender issues with the energy transition in the EU. There is little peer reviewed literature in this area. Analysis of academic papers shows most focus on social science rather than technical or engineering problems. A critical review of the grey literature including EU policies and reports shows that there is a disconnect between EU gender equality and clean energy plans and that gender has yet to be mainstreamed into the EU energy transition. This review identifies opportunities to mainstream gender into EU energy policies by linking to EU gender equality objectives, and connecting to gender-energy research themes.

Highlights of a H2020 coordination and support action to increase activity in smart grid research, demonstration and innovation in the EU are presented. PANTERA (PAN European Technology Energy Research Approach) provides a forum for actors in the smart grid domain. This paper focuses on a review of regulations, codes and standards in the EU conducted by the PANTERA project and focuses on the development of Network Codes by looking at a specific case study of a research and demonstration project in Ireland. The opportunities to foster smart grid RD&I through a shared understanding of insights available on the PANTERA platform are highlighted.

In order to deal with the high development time of exact and approximation algorithms for NP-hard combinatorial optimisation problems and the high running time of exact solvers, deep learning techniques have been used in recent years as an end-to-end approach to find solutions. However, there are issues of representation, generalisation, complex architectures, interpretability of models for mathematical analysis etc. using deep learning techniques. As a compromise, machine learning can be used to improve the run time performance of exact algorithms in a matheuristics framework. In this paper, we use a pruning heuristic leveraging machine learning as a pre-processing step followed by an exact Integer Programming approach. We apply this approach to sparsify instances of the classical travelling salesman problem. Our approach learns which edges in the underlying graph are unlikely to belong to an optimal solution and removes them, thus sparsifying the graph and significantly reducing the number of decision variables. We use carefully selected features derived from linear programming relaxation, cutting planes exploration, minimum-weight spanning tree heuristics and various other local and statistical analysis of the graph. Our learning approach requires very little training data and is amenable to mathematical analysis. We demonstrate that our approach can reliably prune a large fraction of the variables in TSP instances from TSPLIB/MATILDA (>85%) while preserving most of the optimal tour edges. Our approach can successfully prune problem instances even if they lie outside the training distribution, resulting in small optimality gaps between the pruned and original problems in most cases. Using our learning technique, we discover novel heuristics for sparsifying TSP instances, that may be of independent interest for variants of the vehicle routing problem.

BackgroundIn 2016, 26% of UK men were estimated to be obese. Systematic reviews suggest that few men engage in formal weight loss interventions that support weight reduction and improve health.ObjectiveTo co-produce, with patient and public involvement, an acceptable and feasible randomised controlled trial design to test a men-only weight management intervention.DesignThis was a two-phase feasibility study. Phase 1 was the development of intervention components, study procedures and materials including a discrete choice experiment with survey questions. Phase 2 was an individually randomised three-arm feasibility trial over 12 months. Qualitative interviews were conducted at 3 and 12 months.SettingThe setting was two sites in Scotland that had disadvantaged urban and rural areas and differed in employment levels and ethnic groups.ParticipantsIn phase 1, 1045 men with obesity were recruited by Ipsos MORI (London, UK;www.ipsos.com/ipsos-mori/en-uk) to represent the UK population. In phase 2, 105 men with obesity were recruited in the community or through general practice obesity registers. Qualitative interviews were conducted with 50 men at 3 months and with 33 men at 12 months.InterventionsThe trial arms were narrative short message service (SMS) for 12 months (SMS only), financial endowment incentive informed by loss aversion and linked to achievement of weight loss targets plus narrative SMS for 12 months (SMS + I), and waiting list control group for 12 months followed by 3 months of an alternative SMS style developed based on feedback from men who had received the narrative SMS (control).Main outcome measuresThe main outcome measures were acceptability and feasibility of recruitment, retention, engagement, intervention components and trial procedures. Outcomes were assessed by examining procedural, quantitative and qualitative data at 3, 6 and 12 months.ResultsThe most acceptable incentive strategy, based on the discrete choice experiment results, was to verify weight loss of 5% at 3 months, verify weight loss of 10% at 6 months and maintain weight loss of 10% at 12 months. Overall, 105 men with obesity from across the socioeconomic spectrum were successfully recruited to target, 59% of whom lived in more disadvantaged areas. Retention at 12 months was acceptable (74%) and was higher among individuals from disadvantaged areas. Narrative SMS were acceptable to many men, with a minority reporting negative reactions. Incentives were acceptable but were not the primary motivation for behaviour change. Twelve men in the incentive arm (33%) secured at least some money and three (8%) secured the full amount. Both intervention arms lost some weight, with greater weight loss in the arm that received SMS and incentives. The alternative SMS based on men’s feedback received no strong negative reactions.LimitationsFewer participants from the SMS + I arm (64%) completed the study at 12 months than did those in the SMS-only (79%) and control (83%) arms. The reasons for this difference were complex.ConclusionsThe men-only weight management intervention consisting of narrative SMS and financial incentives was acceptable and feasible, meeting the progression criteria for a full trial. The 10th International Conference on Sustainable Energy Information Technology (SEIT 2020), Leuven, Belgium, 9-12 August 2020 The Irish government is implementing policies to transition Ireland to a low carbon and environmentally sustainable economy by 2050. Ireland has sectoral targets of 600,000 installed heat pumps by 2030, currently roughly 28,000 are installed. Such a high target of heat pumps will not only have a significant effect on electricity demand but also on the management and operation of the grid. In this paper we explore the demand from homes heated by air source heat pumps using an innovative dataset from a field trial in Ireland. To assess the impact of large-scale adoption of heat pumps, this paper estimates the after diversity maximum demand per heat pump heated home. In particular we explore statistical distributions to best model coincident demand, and estimate after diversity maximum demand per home. We use the software package RStudio to model several different distributions. Based on goodness-of-fit statistics and criteria, a Gamma distribution is the best fit. We apply our methodology to data from a similar heat pump trial in the UK to complement our results. This paper describes a study in Smart Grid Tariff Design. We use historic Smart Meter electricity usage data and Representative Load Profiles (RLPs) to explore Time-of-Use (ToU) tariffs. We consider the problem from the perspective of a supplier who wishes to set tariffs to maximise revenue, and has to anticipate responses from consumers who wish to minimise their costs. This is a typical bilevel optimisation problem, but with additional challenges as consumers have uncertain electricity usage. We create test RLP instances using historical Smart Meter data and demonstrate the use of a robust optimisation framework to address uncertain usage patterns. Our modelling and optimisation approach offers a framework for suppliers, system operators and innovators to explore and evaluate potential ToU market offerings.

Background Men, particularly those living in disadvantaged areas, are less likely to participate in weight management programmes than women despite similar levels of excess weight. Little is known about how best to recruit men to weight management interventions. This paper describes patient and public involvement in pre-trial decisions relevant to recruitment and aims to report on recruitment to the subsequent men-only weight management feasibility trial, including the: i) acceptability and feasibility of recruitment; and ii) baseline sample characteristics by recruitment strategy. Methods Men with BMI ≥30 kg/m2 and/or waist circumference ≥ 40 in. were recruited to the feasibility trial via two strategies; community outreach (venue information stands and word of mouth) and GP letters, targeting disadvantaged areas. Recruitment activities (e.g. letters sent, researcher venue hours) were recorded systematically, and baseline characteristics questionnaire data collated. Qualitative interviews (n = 50) were conducted three months post-recruitment. Analyses and reporting followed a complementary mixed methods approach. Results 105 men were recruited within four months (community n = 60, GP letter n = 45). Community outreach took 2.3 recruiter hours per participant and GP letters had an opt-in rate of 10.2% (n = 90/879). More men were interested than could be accommodated. Most participants (60%) lived in more disadvantaged areas. Compared to community outreach, men recruited via GP letters were older (mean = 57 vs 48 years); more likely to report an obesity-related co-morbidity (87% vs 44%); and less educated (no formal qualifications, 32% vs 10%, degree educated 11% vs 41%). Recruitment strategies were acceptable, a sensitive approach and trusting relationships with recruiters valued, and the ‘catchy’ study name drew attention. Conclusions Targeted community outreach and GP letters were acceptable strategies that successfully recruited participants to a men-only weight management feasibility trial. Both strategies engaged men from disadvantaged areas, a typically underserved population. Using two recruitment strategies produced samples with different health risk profiles, which could add value to research where either primary or secondary prevention is of interest. Further work is required to examine how these strategies could be implemented and sustained in practice. Trial registration ClinicalTrials.gov: NCT03040518, 2nd February 2017.

ObjectivesTo examine the acceptability and feasibility of narrative text messages with or without financial incentives to support weight loss for men.DesignIndividually randomised three-arm feasibility trial with 12 months’ follow-up.SettingTwo sites in Scotland with high levels of disadvantage according to Scottish Index for Multiple Deprivation (SIMD).ParticipantsMen with obesity (n=105) recruited through community outreach and general practitioner registers.InterventionsParticipants randomised to: (A) narrative text messages plus financial incentive for 12 months (short message service (SMS)+I), (B) narrative text messages for 12 months (SMS only), or (C) waiting list control.OutcomesAcceptability and feasibility of recruitment, retention, intervention components and trial procedures assessed by analysing quantitative and qualitative data at 3, 6 and 12 months.Results105 men were recruited, 60% from more disadvantaged areas (SIMD quintiles 1 or 2). Retention at 12 months was 74%. Fewer SMS+I participants (64%) completed 12-month assessments compared with SMS only (79%) and control (83%). Narrative texts were acceptable to many men, but some reported negative reactions. No evidence emerged that level of disadvantage was related to acceptability of narrative texts. Eleven SMS+I participants (31%) successfully met or partially met weight loss targets. The cost of the incentive per participant was £81.94 (95% CI £34.59 to £129.30). Incentives were acceptable, but improving health was reported as the key motivator for weight loss. All groups lost weight (SMS+I: −2.51 kg (SD=4.94); SMS only: −1.29 kg (SD=5.03); control: −0.86 kg (SD=5.64) at 12 months).ConclusionsThis three-arm weight management feasibility trial recruited and retained men from across the socioeconomic spectrum, with the majority from areas of disadvantage, was broadly acceptable to most participants and feasible to deliver.Trial registration numberNCT03040518.

Background There is a need to compare the proportions, risk factors, and natural histories of postesophagectomy paraconduit hernias in minimally invasive and open esophagectomies. Methods This is a single-center, retrospective cohort study of esophageal cancer surgery performed between 2007 and 2017. Postesophagectomy paraconduit hernias were identified on cross-sectional imaging. Patient charts were reviewed to describe the management and natural history. Results Between 2007 and 2017, 391 esophagectomies were performed. After exclusions, 347 patients remained, 135 of whom were total minimally invasive esophagectomies (MIEs) (39%). Postoperative paraconduit hernias developed in 10% of patients. Median time to diagnosis was 258 days. Of 135 MIEs, 20 had a paraconduit hernia (15%) compared with 16 of 212 open or hybrid esophagectomies (8%; P = .03). Hernias were symptomatic in 13 patients (36%) and asymptomatic in 23 (64%), which were detected radiographically. Repair was performed in 11 of 13 symptomatic patients (85%), compared with 3 of 23 asymptomatic patients (13%). In the asymptomatic group, only 1 required emergency repair (4.3%). There was a trend toward a greater proportion of symptomatic paraconduit hernias compared with asymptomatic patients (77% versus 43%; P = .08) in MIE patients. Factors associated with the development of paraconduit hernias on univariate analysis were younger age (P = .02) and not receiving neoadjuvant chemotherapy (P = .01) or neoadjuvant radiation (P = .03). Conclusions Postesophagectomy paraconduit hernia is more common after totally minimally invasive esophagectomy compared with open or hybrid techniques. One third are symptomatic and the remainder are detected only radiographically. Repair of asymptomatic hernias consider the patient’s cancer prognosis.

Men’s health and life expectancy, particularly for those men from lower socioeconomic groups, remains an issue of concern in Ireland. This concern is reflected in the recent National Men’s Health Action Plan where important priority has been placed on finding appropriate ways to garner sustained involvement in health promotion interventions for men. Physical activity (PA) has been shown to be a useful ‘hook’ to assist with such engagement. ‘Men on the Move’ (MOM) is a 12 week, community-based, gender-sensitised, PA programme established as a pragmatic controlled trial and aimed at improving the health and well-being of inactive men. The programme was co-created with Local Sports Partnerships (LSP), delivered by experienced PA Coordinators (PACs), and often supported by local community champions. This paper reports on the process evaluation of the MOM programme using data collected from focus groups with the LSP’s and those involved in delivering MOM from all eight counties that took part. Findings highlight the importance of negotiated partnerships at and between national and local levels in terms of providing support, consistent guidance and appropriately branded materials to the LSP’s. The underpinning inclusive ethos of MOM, embodied by the PAC’s, led to the creation of a fun, inclusive and comfortable atmosphere that helped sustain men’s involvement. This was aided by the use of male familiar settings through which to deliver the programme. While PA focused, findings here suggest a much wider impact on mental well-being and social connection and that this was achieved in a very cost-effective way. Importantly, men’s health training (ENGAGE) was a key factor in programme design and implementation assisting in building capacity among service providers to work with men. Joined up service provision and drawing on existing, trusted, local community networks were vital to recruiting men into the programme. Finally, the potential for MOM to signpost and offer an aftercare plan to community support for the men beyond the 12-week programme was noted as important particularly where there is increased need of these among more marginalised groups of men. This process evaluation provides a good example of how health promotion interventions need to recognise and exploit the fact that health and well-being are integrally linked to the communities where people live out and experience their daily lives. Ensuring that MOM was embedded within existing community structures, and supported by community champions with the requisite skills and local knowledge, underpinned programme success and sustainability.

Background Amid increasing concerns about rising obesity rates and unhealthy lifestyle behaviours, physical activity (PA) is seen as a prophylactic to many chronic conditions affecting men. Men respond best to community-based PA programmes, using gender-specific promotional and delivery strategies. ‘Men on the Move’ (MOM) was developed on this basis and targeted inactive adult men in Ireland. Methods Sedentary men (n = 927; age = 50.7 ± 10.9 years; weight = 92.7 ± 16.0 kg; METS = 6.06 ± 2.13) were recruited across eight counties: four ‘intervention group’ (IG; n = 501) and four ‘comparison-in-waiting group’ (CG; n = 426). The MOM programme involved structured group exercise twice weekly for 12 weeks (W), along with health-related workshops with the groups maintained up to 52 W. Primary outcome measures [aerobic fitness, bodyweight and waist circumference (WC)] together with self-administered questionnaires were used to gather participant data at baseline, 12, 26 and 52 W. Results Results show a net positive effect on aerobic fitness, bodyweight and WC, with significant (P < 0.05) net change scores observed in the IG compared to the CG (METS: 12 W = +2.20, 26 W = +1.89, 52 W = +0.92; weight: 12 W = −1.72 kg, 26 W = −1.95 kg, 52 W = −1.89 kg; WC: 12 W = −4.54 cm, 26 W = −2.69 cm, 52 W = −3.16 cm). The corresponding reduction in cardiovascular disease risk is particularly significant in the context of a previously inactive and overweight cohort. The high ‘dropout’ (42.7% presenting at 52 W), however, is of particular concern, with ‘dropouts’ having lower levels of aerobic fitness and higher bodyweight/WC at baseline. Conclusions Notwithstanding dropout issues, findings address an important gap in public health practice by informing the translational scale-up of a small controllable gender-specific PA intervention, MOM, to a national population-based PA intervention targeting inactive men.

This article describes a project conducted in conjunction with the Central Statistics Office of Ireland in response to a planned national rollout of smart electricity metering in Ireland. We investigate how this new data source might be used for the purpose of official statistics production. This study specifically looks at the question of determining household composition from electricity smart meter data using both Neural Networks (a supervised machine learning approach) and Elastic Net Logistic regression. An overview of both classification techniques is given. Results for both approaches are presented with analysis. We find that the smart meter data alone is limited in its capability to distinguish between household categories but that it does provide some useful insights.

This paper focuses on high resolution wind power statistical models fitted to meteorological data for the island of Ireland. A discrete Burr model efficiently represents the number of consecutive hours of wind power availability. The models developed in this study may be most useful at time resolutions less than 6 hours to capture zero power and short bursts of wind power potential. Smart metering in electricity markets offers an opportunity to explore more diverse tariff structures. In this article residential electricity demand and the System Marginal Price of Ireland’s Single Electricity Market are simulated to estimate the wholesale risk associated with possible tariffs. A genetic algorithm (GA) with a stochastic fitness function is proposed to search for time-of-use tariffs that minimise wholesale risk to the supplier in residential markets. Alternative search algorithms and fitness functions are investigated in detail, as well as trade-offs in GA and simulation parameter settings.

Air Source Heat Pumps (ASHPs) are part of the solution to decarbonise the residential heating sector. The coefficient of performance (COP) is a measure of the instantaneous efficiency of a heat pump. The heat energy produced by an ASHP is deemed renewable if it meets a specified sustained COP over a period of time, e.g., a seasonal performance factor (SPF). Heat pump performance in situ often differs from laboratory test conditions. This paper explores the performance of ASHPs in a field trial of deeply retrofitted Irish houses. Analysis shows that all houses in the trial qualify as producing renewable heat but vary from the manufacturer’s laboratory test performance. ASHPs with ratings of 8.5 kW (11.2 kW) underperformed against the manufacturers COP values on average by 16 (24%) at outside temperatures of 7 °C, and 3 (11%) at outside temperatures of 2 °C. The paper evaluates statistical and machine learning models, including a random forest (RF) model, to predict COPs and compare the difference between the manufacturer’s laboratory COPs and the trial results.

Compared to other occupational groups, farmers in Ireland experience a disproportionate burden of health problems, which impact farmers’ livelihoods and farming sustainability. Internationally, farmers’ poor health outcomes are associated with intersecting economic, environmental, socio-cultural, and occupation-specific factors linked to changes in agricultural governance. This qualitative study explored the challenges and stressors facing farmers in Ireland and how changes in farming governance have impacted farmers’ identities, masculinities and health. Eleven focus groups ( n = 26 female, n = 35 male, age-range 20s–70s) were conducted with both male farmers ( n = 3 focus groups; n = 13) and key informants ( n = 8 focus groups; n = 48, 22 male, 26 female). Utilizing Thematic Content Analysis, transcripts were coded independently by the first and second author using open and comparative coding techniques, with emerging themes grouped into primary and subthemes. Theme memos and conceptual maps tracked evolving relationships between themes. The analysis identified three broad themes. “Wrestling with challenges to autonomy and control within farming” examines the impact of tighter regulatory frameworks associated with changes to farming governance and unpacks other challenges associated with scale and succession. “Farming masculinities and health” explores how farming masculinities were closely aligned with farming practices and health practices and were framed relationally. “Isolation and the demise of rural communities” considers the impact of reduced social interaction on loneliness among farmers, particularly among more “at risk” single and older farmers. Findings provide unique insights into contemporary challenges and stressors facing farmers and have important implications for informing the design and roll-out of a national farmers’ health training program.

The interactions of early stage business students with learning resources over the duration of an introductory statistics module were analysed using latent class analysis. Four distinct behavioural groups were identified. While differing levels of face-to-face attendance and online interaction existed, all four groups failed to engage with online material in a timely manner. The four groups were found to demonstrate significantly different levels of attainment of the module learning outcomes. The patterns of behaviour of the different groups of students give insights as to which analytics education learning resources students use and how their use patterns relate to their level of attainment of the module learning outcomes.

This chapter provides an introduction to exact methods and their application to solving vehicle routing problems in sustainable transportation. We provide an overview of existing methods and focus on providing an intuition as to how these approaches work, rather than providing rigorous mathematical proofs. We demonstrate the use of the vehicle flow formulation on a sample of benchmark instances, and on some new problem instances that incorporate geospatial information to aid vehicle routing decision making in sustainable transportation. We outline the use of emissions models to estimate the CO2 emissions on solution routes.

Data science is a relatively new requirement in business curricula. Historically many business students have shied away from business statistics. We describe a project to create learning objects to enhance business students confidence and capabilities in performing statistical and analytics business tasks. In this paper we focus on the content development process, rather than the impact of the learning objects on student learning outcomes.We reflect on the steps in the learning object design and implementation project and conclude that the Plan, Act, Observe and Reflect iterative cycle worked well for the project team. We include recommendations on how this framework could be augmented to improve the sustainability of learning objects.

The electrification of home heating is proposed as a low carbon solution in climate change action plans. It is therefore important to understand the energy efficiency and capability of heat pumps when deployed in the homes of end users. This paper presents a systematic literature review of Air Source Heat Pumps (ASHP) field studies. The papers are reviewed with respect to the field study focus, and analysed in terms of the field study design, and the analysis methodologies applied to the gathered data. The review finds three main areas of focus: defrosting management, ASHP system management, and ASHPs as smart grid demand response components. The review also identifies what ASHP data is publicly available to assist other researchers in building, testing and analysing ASHP efficiency models. Finally, future research topics on the use of ASHPs in home heating are identified.

Biopharmaceutical manufacturers are required to collect extensive observational data sets in order to meet regulatory and process quality monitoring requirements. These datasets contain information that may improve the performance of the production process. Analytics provides a means of extracting this information while Six Sigma provides a means for the insights to be incorporated into production practices. We present a novel framework which combines Six Sigma and Business Analytics. This approach mines large volumes of inline and offline biopharmaceutical production data, allowing the entire production process to be analysed and modeled. The recommendations of the model are represented as manufacturing rules which give actionable insights to improve the performance of the process. The integrated approach delivers promising results from synthetic experiments as well as being applied in practice to a cell culture process. Gastrocardiac fistulae are an exceedingly rare complication following esophagectomy with gastric conduit pull up. Unsurprisingly, there is a significant ascribed mortality with the disease process, often only identified postmortem. We report a case of transdiaphragmatic fistula between the intraabdominal gastric conduit and the right ventricle and surgical management thereof.

Historically, men, as a population group, have been conspicuous by their absence at a global and national health policy level. Moreover, most gender-focused health policy initiatives and gender-mainstreaming approaches to health have tended to be synonymous with women’s health. This places Ireland’s National Men’s Health Policy (NMHP) and recent external 5-year review in the collector’s item category within the wider health policy landscape. This paper will review the impetus and background to men’s health policy development in Ireland against a backdrop of the invisibility of men more generally from health policy. Reflecting on the key milestones and challenges associated with transitioning from policy development to implementation, the paper will seek to inform a wider public health debate on the case for targeting men as a specific population group for the strategic planning of health. The case for a NMHP on the grounds of a gender inequity will also be explored in the context of contributing more broadly to gender equality. There will be a particular focus on exploring how strategies associated with governance and accountability, advocacy, research and evaluation, partnerships and capacity-building, have acted as a catalyst and framework for action in the rollout of a broad range of men’s health initiatives. With the central challenge being the translation of cross-departmental and inter-sectoral recommendations into sustainable actions, the role of NMHP in applying a gender lens to other policy areas will also be discussed. Ireland’s NMHP has raised the visibility of men’s health in Ireland; the lessons learned during its implementation provide a strong rationale and blueprint for NMHP development elsewhere.

The excess burden of ill-health, mortality and premature death experienced by many men, and poorer men in particular, across the developed world has prompted calls for the development of gender sensitised health related services for men. An emergent body of evidence indicates that successful public health work with men can be accomplished when it utilises elements with which men are familiar and secure. In particular, physical activity (PA) is proven here to be a useful ‘hook’ to engage men. ‘Men on the Move’ (MoM) is a community-based PA programme designed to engage inactive men to improve their overall health and well-being. The MoM programme was delivered by practitioner partnerships in diverse communities and among diverse groups of men under ‘real world’ conditions to assess both its efficacy and replicability with a view to scaling-up the programme nationally for population wide impact. Establishing appropriate protocols is critical when conducting research that translates into practice, is replicable in practice and can be disseminated at a population level. The purpose of this paper is to detail the protocols used in the design, implementation and evaluation of the MoM programme. Specifically, the process of engaging men in a community based PA intervention and sustaining that engagement over the 12 weeks and the protocols used to evaluate the impact of participation in MoM on biopsychosocial health up to 52 weeks will be outlined. If the intervention proves successful, gender-sensitive community based PA interventions for men could be a promising avenue to address their health needs. These findings may be of support to both practitioners endeavouring to engage men and others engaged in translational research to ensure their research translates to meaningful action in practice.

Background Issues surrounding gender and men’s health have become an increasing focus of public health globally. Unhealthy lifestyles and lower engagement in health promotion initiatives contributed to lower life expectancy and higher mortality rates among men. This study presents the pre-adoption characteristics of men who registered for ‘Men on the Move’—a community-based physical activity (CBPA) programme, to ascertain whether the programme reached its intended target group, i.e. ‘at-risk’ adult men who did not meet physical activity (PA) guidelines and were likely to have multiple risk factors for cardiovascular disease (CVD). Methods Multiple recruitment strategies were adopted to engage the target group and baseline data collection included a range of demographic, self-report and outcome measures. Results The recruitment strategy succeeded in reaching the target group, with the majority (n = 927) presenting being previously inactive (89.0%), overweight/obese (89.7%) and having multiple CVD risk factors (53.1% ≥ 2 risk factors). However, the strategy was less successful in engaging ‘hard-to-reach’ groups, with the majority being middle-aged, white, married/cohabiting, educated and employed. Conclusions A gender-sensitized, partnership and community outreach recruitment strategy can maximize the reach and recruitment of an ‘at-risk’ cohort for CBPA initiatives, but more targeted approaches are needed to recruit marginalized groups of men.

There have been increasing calls for more gender-specific service provision to support young men’s (20-29 years) mental health and well-being. In Ireland, young men are the demographic group that are most likely to die by suicide but among the least likely to seek help. This study sought to investigate service providers’ perspectives on the factors that support or inhibit young men from engaging in services targeted at supporting their mental/emotional well-being. Qualitative methodologies (focus groups, n = 9; interviews, n = 7) were used for this study. Disconnection from family and community was identified as a key indicator of “at-risk” groups of young men who, more typically, had experienced significant disruption in their lives. The discord between demands and expectations facing young men on one hand, and insufficient life-management and coping skills on the other, left many young men vulnerable and bereft. The desire to save face and preserve one’s masculine identity was linked to young men’s reluctance to seek help when feeling down. There was a strong consensus that there could be no shortcuts to [re]connecting with young men. While sport, technology, and social media were cited as appropriate media in which to engage young men, the essence of sustained connection revolved around creating safety, trust, rapport, and meaningful relationships. The findings from this study have informed the development of a Train the Trainer program (“Connecting with Young Men”), which is currently being delivered to service providers in Ireland and which may have implications for service provision elsewhere.

We present high resolution wind power statistical models fitted to meteorological data for the island of Ireland. We find that a discrete Burr model efficiently represents the number of consecutive hours of wind power availability. Burr models are also useful to model the complement of the wind power availability events, wind droughts, when wind speed is insufficient to produce wind power. The models developed in this study may be most useful at time resolutions less than six hours to capture zero power and short bursts of wind power potential. They could serve as a useful complement to other wind power modelling approaches such as MERRA reanalysis models. Wind power duration models, and their wind drought complements, also provide insights for investors on potential wind power availability at geographic locations. We propose a Genetic Algorithm (GA) to address a Green Vehicle Routing Problem (G-VRP). Unlike classic formulations of the VRP, this study aims to minimise the CO2 emissions per route. The G-VRP is of interest to policy makers who wish to reduce greenhouse gas emissions. The GA is tested on a suite of benchmark, and real-world instances which include road speed and gradient data. Our solution approach incorporates elements of local and population search heuristics. Solutions are compared with routes currently used by drivers in a courier company. Reductions in emissions are achieved without incurring additional operational costs.

We describe a set of nonparametric and machine learning models to forecast the proportion of planned end products (PEP) that can be extracted from a forest compartment. We determine which forest crop attributes are significant in predicting the product proportions (of sawlog, pallet, stake, and pulp) based on an Irish data set supplied by Coillte, the Irish state forestry company. Dirichlet regression and neural networks are applied to predict the product proportions and evaluated against a multivariate multiple regression benchmark model. Based on predictive performance, the neural network performs slightly better in comparison to Dirichlet regression. However, assessing the model logic and taking account of user interpretation, the Dirichlet regression outperforms the neural network. Both models are also compared to an existing rule-based model used by Coillte. The nonparametric and machine learning techniques provided consistent reliable models to accurately predict the PEP proportions. The two proposed models extend the versatility of nonparametric and machine learning techniques to areas such as forestry.

Non-homologous end joining (NHEJ) is a key cellular process ensuring genome integrity. Mutations in several components of the NHEJ pathway have been identified, often associated with severe combined immunodeficiency (SCID), consistent with the requirement for NHEJ during V(D)J recombination to ensure diversity of the adaptive immune system. In contrast, we have recently found that biallelic mutations in LIG4 are a common cause of microcephalic primordial dwarfism (MPD), a phenotype characterized by prenatal-onset extreme global growth failure. Here we provide definitive molecular genetic evidence supported by biochemical, cellular, and immunological data for mutations in XRCC4, encoding the obligate binding partner of LIG4, causing MPD. We report the identification of biallelic mutations in XRCC4 in five families. Biochemical and cellular studies demonstrate that these alterations substantially decrease XRCC4 protein levels leading to reduced cellular ligase IV activity. Consequently, NHEJ-dependent repair of ionizing-radiation-induced DNA double-strand breaks is compromised in XRCC4 cells. Similarly, immunoglobulin junctional diversification is impaired in cells. However, immunoglobulin levels are normal, and individuals lack overt signs of immunodeficiency. Additionally, in contrast to individuals with LIG4 mutations, pancytopenia leading to bone marrow failure has not been observed. Hence, alterations that alter different NHEJ proteins give rise to a phenotypic spectrum, from SCID to extreme growth failure, with deficiencies in certain key components of this repair pathway predominantly exhibiting growth deficits, reflecting differential developmental requirements for NHEJ proteins to support growth and immune maturation.
