Background Individuals with Borderline Personality Disorder (BPD) feel rejected even when socially included. The pathophysiological mechanisms of this rejection bias are still unknown. Using the Cyberball paradigm, we investigated whether patients with BPD, display altered physiological responses to social inclusion and ostracism, as assessed by changes in Respiratory Sinus Arrhythmia (RSA). Methods The sample comprised 30 patients with BPD, 30 with remitted Major Depressive Disorder (rMDD) and 30 Healthy Controls (HC). Self-report ratings of threats toward one’s fundamental need to belong and RSA reactivity were measured immediately after each Cyberball condition. Results Participants with BPD showed lower RSA at rest than HC. Only patients with BPD, reported higher threats to fundamental needs and exhibited a further decline in RSA after the Inclusion condition. Conclusions Individuals with BPD experience a biased appraisal of social inclusion both at the subjective and physiological level, showing higher feelings of ostracism and a breakdown of autonomic regulation to including social scenarios.

Replication efforts in psychological science sometimes fail to replicate prior findings. If replications use methods that are unfaithful to the original study or ineffective in eliciting the phenomenon of interest, then a failure to replicate may be a failure of the replication protocol rather than a challenge to the original finding. Formal pre-data collection peer review by experts may address shortcomings and increase replicability rates. We selected 10 replications from the Reproducibility Project: Psychology (RP:P; Open Science Collaboration, 2015) in which the original authors had expressed concerns about the replication designs before data collection and only one of which was “statistically significant” (p < .05). Commenters on RP:P suggested that lack of adherence to expert review and low-powered tests were the reasons that most of these failed to replicate (Gilbert et al., 2016). We revised the replication protocols and received formal peer review prior to conducting new replications. We administered the RP:P and Revised replication protocols in multiple laboratories (Median number of laboratories per original study = XX; Range XX to YY; Median total sample = XX; Range XX to YY) for high-powered tests of each original finding with both protocols. Overall, XX of 10 RP:P protocols and XX of 10 Revised protocols showed significant evidence in the same direction as the original finding (p < .05), compared to an expected XX. The median effect size was [larger/smaller/similar] for Revised protocols (ES = .XX) compared to RP:P protocols (ES = .XX), and [larger/smaller/similar] compared to the original studies (ES = .XX) and [larger/smaller/similar] compared to the original RP:P replications (ES = .XX). Overall, Revised protocols produced [much larger/somewhat larger/similar] effect sizes compared to RP:P protocols (ES = .XX). We also elicited peer beliefs about the replications through prediction markets and surveys of a group of researchers in psychology. The peer researchers predicted that the Revised protocols would [decrease/not affect/increase] the replication rate, [consistent with/not consistent with] the observed replication results. The results suggest that the lack of replicability of these findings observed in RP:P was [partly/completely/not] due to discrepancies in the RP:P protocols that could be resolved with expert peer review.

Recent reasoning in psychopathology sees mental disorders as phenomena that emerge, at least in part, from complex patterns of symptom-symptom interactions. Borderline Personality Disorder (BPD) is characterized by instability of self-image, interpersonal relationships, and affects, as well as by marked impulsivity. In this study, we investigated the dynamical interplay of BPD symptoms over time. We employed an Ecological Momentary Assessment (EMA) protocol assessing BPD symptoms five times a day for 31 days in a sample of 156 participants. Participants also completed a comprehensive assessment of personality, including BPD, before the EMA, after the EMA, and one month later. We examined BPD symptoms at two timescales: The short scale of EMA and the long scale offered by the three questionnaires. At the short timescale, we employed a multilevel vector auto-regressive network model, which provided insights into the contemporaneous and cross-lagged relationships among symptoms. At the long timescale, we observed a decrease in all BPD symptoms immediately after EMA and after one month, suggesting beneficial effects of the self-reflection induced by EMA. We discuss how each analysis gave unique insights into the dynamics of BPD.

The present work aims at discussing some clinical and methodological issues connected to our work in a Counselling Centre for University Students in Italy. We will initially give a short account of the history, development and characteristics of the counselling intervention. We will subsequently focus on the Italian context, describing the development of University Counselling services in our country. Then we will present the work that we do within the Counselling Centre of Milan University "Bicocca". The aspects that we will consider are the following: - the institutional context; - the theoretical framework, methodology, and theory; - some data on the clients who came to the Centre (e.g. age, Faculty, provenance, drop-out, main areas of distress, etc.) in the first 10 months of activity. Objective. This work aims at assessing the psychometric properties of the Italian versions of the Inventory of Personality Organization (IPO; Lenzenweger et al. 2001), a self-report instrument for the assessment of personality organization, according to Otto Kernberg’s model, and the Structured Interview of Personality Organization (STIPO), the interview-version of the IPO. Study 1 and Study 2 (Chapter 2) evaluated the psychometric properties of the IPO. Methods. 696 university students and people from the community (Study 1; males 37%; mean age=36.51 ys; SD=±14.08; range: 18-74) and 121 psychiatric patients (Study 2; males 43.8%; mean age=37.22 ys; SD=±10.54; range: 18-66) completed a set of questionnaires including the IPO, the Severity Indices of Personality Problems (SIPP-118; Verheul et al., 2008), the Borderline Personality Disorder Checklist (Giesen-Bloo et al., unpublished manuscript), the Response Evaluation Measure 71 (Steiner et al., 2001), and the Symptom Checklist 90-R (Derogatis, 1977). Results. Confirmatory factor-analyses conveyed results similar to those obtained by Ellison and Levy (2011) and supporting a 4-factor solution (instability of sense of self and others, instability of goals, instability of behaviors, and psychosis). The factor structure proved to be invariant across samples (non clinical and clinical) and gender. The four IPO subscales showed good levels of internal coherence (Cronbach’s alphas between .72 and .91), and good test-retest reliability in the non clinical sample (time frame: one month; ICC ranging between .80 and .96). Patterns of associations with the criterion measures, assessed through path analysis, emerged in line with theoretical expectations. The first factor, Instability of sense of self and others, showed associations with measures of identity integration and defensive functioning, as well as associations with borderline symptoms and psychopathological distress. The second factor, Instability of goals, showed associations with coherent measures of the capacity to set realistic goals and to achieve them. The third factor, Psychosis, showed the expected associations with paranoid/dissociative symptomatology only in the clinical sample. Finally, the fourth factor, Instability of behaviors, was found to be associated with personality functions and symptom clusters connected with difficulties in the regulation of anger, impulses, and aggression. The four factors showed the expected criterion relations: all the dimensions, in fact, discriminate between clinical and non clinical subjects, whereas only Instability of sense of self and others and Instabilty of goals discriminate between borderline patients and non borderline patients. The study presented in Chapter 3 evaluated the psychometric properties of the STIPO. Methods 30 university students and people from the community (males 23%; mean age=40.97 ys; SD=±14.35; range: 22-63) and 49 psychiatric patients (males 46.8%; mean age=36.60 ys; SD=±9.45; range: 20-53) were interviewed through the STIPO and completed a set of questionnaires including the IPO, REM 71 (Steiner et al., 2001), and the SIPP-118 (Verheul et al., 2008); psychiatric patients were also interviewed through the Structured Clinical Interview for Axis II Disorders (SCID II; First et al., 2001). Results Confirmatory factor-analyses confirmed the theoretical 7-factor dimensionality of the interview (Identity, Object relations, Defenses, Coping, Aggression, Moral values, and Reality testing). The STIPO subscales showed good levels of internal coherence (Cronbach’s alphas between .78 and .91), and good inter-rater reliability (ICC ranging between .82 and .97). For what concerns the three primary scales of the STIPO (Identity, Defenses, and Reality testing), the patterns of associations with the criterion measures, assessed through path analysis and regressions, were in line with theoretical expectations. The Identity scale showed associations with measures of the stability of self image and the capacity of pursuing goals. The Defenses scale resulted to be associated with the external measure of primitive defenses and with the domains of the IPO concerning the Instability of the sense of self and others and the Instability of goals, as well as with measures of lack of self-control and emotional instability. Finally, the Reality testing scale showed coherent associations with the Psychosis factor of the IPO. The three primary scales of the STIPO also showed the expected criterion relations: all the dimensions, in fact, discriminate between clinical and non clinical subjects, whereas only Identity and Defenses, and not Reality testing, discriminate between borderline patients and non borderline patients. Conclusions. Our results suggest that the Italian versions of the IPO and the STIPO are reliable and valid tools for the assessment of personality organization according to Kernberg’s model. Results are discussed in terms of their implications regarding research and clinical practice, as well as in the context of the new conception of personality disorders proposed for the DSM V.

Objectives This work compares the efficacy of four treatment models for Borderline Personality Disorder (BPD) that have been tested in empirical research. The models considered are Mentalization- Based treatment (MBT) by Bateman and Fonagy, Transference-Focused Psychotherapy (TFP) by Clarkin, Yeomans and Kernberg, Dialectical Behaviour Therapy (DBT) by Linehan, and Schema-Focused Therapy (SFT) by Young. For all four models, a specific treatment manual is available that allows reliable trials on efficacy to be carried out. Methods A critical review of literature from 1991 to date has been conducted, reviewing all published empirical studies on the efficacy of the four models. Pubmed and Psychinfo databases were searched using as keywords the terms “Mentalization-Based Treatment”, “Transference-Focused Psychotherapy”, “Dialectical Behaviour Therapy” and “Schema-Focused Therapy”. The efficacy of treatments was compared considering three different parameters: dysregulation, mentalization, and duration/stability of changes. Results The review of literature identified 5 studies on MBT, 7 on TFP, 20 on DBT and its modified forms, and 7 on SFT. The four models showed efficacy in reducing self-mutilating, suicidal, and parasuicidal behaviours. TFP is the most effective treatment in reducing anger and impulsivity, while DBT was effective in treating eating behaviour disorders, one of the symptoms of severe dysregulation. Considering substance abuse, data are unclear. As for mentalization, only TFP demonstrated efficacy in increasing the level of reflective functioning. The changes obtained with treatments showed long-term stability for MBT, TFP, and DBT, while further follow-up studies are required for SFT. Conclusions All four models can be considered effective in treating BPD (or, at least, some aspects) with a level of efficacy that varies depending on the parameter considered. According to criteria of the American Psychiatric Association 5 for empirically-validated treatments, TFP, DBT and SFT can be considered well established treatments for BPD, while MBT meets the criteria for probably-efficacious treatment.

La valutazione del benessere del personale sanitario che opera nelle strutture ospedaliere è di estrema rilevanza per individuare e controllare eventuali situazioni di disagio psicologico, le cui conseguenze si riflettono sulla qualità delle prestazioni professionali e delle relazioni sia con i colleghi che con l'utenza. Le variabili che influiscono sull'esaurimento fisico e psichico, prodromi della sindrome di burn-out, sono riconducibili a fattori sia individuali che ambientali. Il presente contributo ha analizzato un campione di 944 tra medici (N totale = 271, età media 45 anni -DS = 9.56 anni) e infermieri (N totale = 673, età media 36.58 anni - DS = 6.85 anni) ospedalieri allo scopo di valutare le correlazioni esistenti tra clima organizzativo, burn-out, stress e strategie di coping e le differenze in alcuni di questi costrutti ascrivibili alla professione e al genere e all’anzianità di servizio. Gli strumenti utilizzati sono stati: Il Maslach Burn-out Inventory (Maslach, Jackson,1981), un questionario sul clima lavorativo creato ad hoc, che valuta tre dimensioni: la Leadership, la Qualità della relazione tra i colleghi e l'Organizzazione della struttura e l'Health Professions Stress and Coping Scale (Ripamonti, Steca, Prunas, 2007), un nuovo questionario self-report che valuta il livello di stress soggettivamente percepito rispetto a una serie di situazioni stressanti relative all'ambito ospedaliero e le strategie di coping adottate per fronteggiarle. I risultati hanno evidenziato l'esistenza di interessanti differenze tra medici e infermieri nelle diverse variabili considerate e di significative relazioni tra burn-out, aspetti del clima organizzativo e strategie di coping.

Il presente lavoro esamina il tema delle credenze irrazionali. L'Irrational Beliefs Scale (IBS) è un questionario, ideato per valutare le credenze irrazionali, dato che non ci risultano strumenti analoghi validati in lingua italiana. I dati relativi a 798 studenti universitari sono stati analizzati attraverso un'analisi fattoriale, adottando una soluzione fattoriale a 8 dimensioni. Sono emerse differenze significative rispetto al genere e alla facoltà frequentata: le femmine hanno ottenuto punteggi di credenza significativamente più elevati rispetto ai maschi, così come gli studenti di materie umanistiche rispetto a quelli di materie scientifiche. Rispetto alla religiosità, il quadro risulta composito, anche se in generale, i soggetti religiosi sono risultati più credenti. Abbiamo inoltre indagato, tramite la somministrazione del questionario “Symptom Checklist-90-Revised”, le associazioni tra credenze irrazionali e tratti psicopatologici. I risultati mostrano associazioni tra credenze irrazionali e disturbi d’ansia, in particolare, disturbo ossessivo-compulsivo, e somatizzazione. Per quanto riguarda la personalità, sono state somministrate le scale relative a Energia e Stabilità emotiva del “Big Five Questionnaire” e sono emerse correlazioni inverse con la Stabilità Emotiva, soprattutto nelle femmine.

Health profession has been traditionally considered as one of the most at risk for psychological distress; in spite of a remarkable interest for the burn-out syndrome, only recently, scholars and polities devoted their attention to the factors apt to promote health professionals’ satisfaction and gratification. Aim of the present study was to investigate general and domain job satisfaction of physicians and nurses working in various Italian hospitals. The influence of four variables on job satisfaction was investigated: type of profession (physicians vs. nurses), job seniority, the desire of changing job, and the desire of changing work place. Participants’ suggestions in order to better their ward and to increase their personal satisfaction were collected through open questions. Results showed that physician were more satisfied with their job than nurses and that high seniority professionals were more satisfied than low seniority professionals. General satisfaction for the ward are mostly associated with the satisfaction for the leadership and the satisfaction for the relationships with colleagues and superiors; negative and modest correlations were found between job satisfaction and burn-out. A material improvement of the ward, more personnel and an improvement of the relationships among colleagues and superiors are reported more frequently as the most important factors in order to improve the ward functioning; a higher salary and more adequate turns and work loadings are reported more frequently as the most important factors in order to increase personal job satisfaction.
