Local inflammation is a strong risk factor for the development of gastrointestinal adenocarcinomas. Mucosal regulatory T cells and IgA-secreting cells both contribute to reduce inflammatory responses, and their recruitment to tissues is dependent on local production of chemokines. More specifically, IgA-secreting cells are recruited to mucosal tissues by CCL28 signalling through CCR10. Here, we examined the recruitment of IgA-secreting plasma cells to tumor-associated mucosa in patients suffering from colon adenocarcinoma. Flow cytometric analyses of single cell suspensions from tumor-associated and unaffected colon mucosa showed a marked decrease in CD19(+)CD38(high)IgA(+) plasmablasts in the tumor-associated mucosa, while the total frequencies of B and T cells were similar. This finding was confirmed in ELISPOT assays, demonstrating a 64 % reduction in the frequencies of IgA-secreting cells among cells from the tumor-associated mucosa. The few IgA(+) plasmablasts present in the tumor did not express CCR10, and functional migration assays demonstrated that IgA-secreting cells from tumor-associated mucosa did not migrate in response to CCL28. Taken together, our results show an impaired migration of IgA-secreting cells to colon tumors, presumably caused by a decreased production of CCL28 in the tumor. The lack of local IgA antibodies may lead to impaired barrier function and increased bacterial colonization, driving further inflammatory responses and promoting tumor growth.

Purpose. Latanoprost has become one of the most widely prescribed topical antihypertensive medications in recent years. Yet there have been few reports of secondary iris pigment epithelial (IPE) and ciliary body (CB) cyst formation to date and none, to our knowledge, reported in eyes predisposed to primary angle closure. Methods. We report the first documented case of bilateral IPE and CB cysts in a hypermetropic patient with prior laser peripheral iridotomies (LPIs) as a rare, delayed side effect of topical Latanoprost treatment. The cysts subsided with discontinuation of Latanoprost, thereby demonstrating a causal relationship. We discuss the pathogenesis of such cysts and advocate using of serial ultrasound biomicroscopic (UBM) images to monitor them. Results and Conclusions. Latanoprost may cause iris pigment epithelial and ciliary body cysts that remain clinically undetected. In eyes predisposed to angle closure, such cysts may become clinically detectable and masquerade as iris tumours. Prior laser peripheral iridotomies in these eyes may delay or prevent the detection of these cysts. Ultrasound biomicroscopy (UBM) imaging is therefore a valuable tool in diagnosing and monitoring these cysts.

Background: The objectives of this study were to evaluate the prevalence of edentulousness, patient's perception on dietary changes resulting from tooth loss and to identify the disparity between actual and patient perceived need to replace missing teeth in an elderly rural population in south India. Methods: A cross sectional study using the systematic cluster sampling method was used to select the study sample of 150 elderly men and women. Data were collected using questionnaires and oral examination. The data were statistically analyzed using chi square test and pearson correlation. Results: 15.6% of the rural elderly were completely edentulous and 54.7% were partially edentulous. Observed differences in distribution between the sampled elderly age groups were found to be statistically significant. Although 70.3% of the evaluated elderly actually required prosthodontic treatment, only 14.4% perceived the need to replace missing teeth. A small percentage of the elderly (18%) perceived a severe change in their diets due to tooth loss. Thirty three percent of them perceived a moderate change and 28% felt that there were no dietary changes because of tooth loss. Conclusions: It is essential to identify feasible strategies to provide primary dental health education and treatment to all rural elderly in the future. We suggest community dental health services as a general health need of the elderly rather than a special health need of the community.

Diagnostic imaging plays a pivotal role in cancer management. Imaging has made rapid strides in the past few years with new modalities and new indications getting added all the time. Molecular and metabolic imaging with 18F Flurodeoxyglucose Positron Emission Tomography (FDG PET) and other non-FDG isotopes have revolutionized oncologic imaging and changed our approach towards cancer diagnosis, staging and imaging surveillance. The evidence for the use of FDG PET is mounting every day with newer indications getting added to the literature. The main focus of our discussion will be to understand the basic principles of FDG PET imaging and to study a few of its clinical applications in oncology.

Integrated PET/CT has emerged as an integral component of oncology management because of its unique potential of providing both functional and morphological images in a single imaging session. In this work, performance of the ′bismuth germinate (BGO) crystal′-based PET of a newly installed Discovery ST PET/CT was evaluated in 2D and 3D mode for whole-body scanning using National Electrical Manufacturers Association (NEMA) NU 2-2001 protocol and the recommended phantoms. During the entire measurements, the system operates with an energy window of 375-650 keV and 11.7 ns coincidence time window. The set of tests performed were spatial resolution, sensitivity, scatter fraction (SF) and counting rate performance. The average transaxial and axial spatial resolution measured as full width at half maximum (FWHM) of the point spread function at 1 cm (and 10 cm) off-axis was 0.632 (0.691) and 0.491 (0.653) cm in 2D and 0.646 (0.682) and 0.54 (0.601) cm in 3D respectively. The average sensitivity for the two radial positions ( R = 0 cm and R = 10 cm) was 2.56 (2.63) cps/kBq in 2D and 11.85 (12.14) cps/kBq in 3D. The average scatter fraction was 19.79% in 2D and 46.19% in 3D. The peak noise equivalent counting rate (NECR) evaluated with single random subtraction was 89.41 kcps at 49 kBq/cc in 2D and 60 kcps at 12 kBq/cc in 3D acquisition mode. The NECR with delayed random subtraction was 61.47 kcps at 40.67 kBq/cc in 2D and 45.57 kcps at 16.45 kBq/cc in 3D. The performance of the PET scanner was satisfactory within the manufacturer-specified limits. The test result of PET shows excellent system sensitivity with relatively uniform resolution throughout the FOV, making this scanner highly suitable for whole-body studies.

With improvement in the early diagnosis of breast cancer, breast conserving therapy (BCT) is being increasingly used. Precise preoperative evaluation of the incision margin is, therefore, very important. Utilizing three dimentional (3D) images in a preoperative evaluation for breast conserving surgery has considerable significance, but the currently 3D CT scan reconstruction commonly used has problems in accurately displaying breast cancer. Thin slice 3D reconstruction is also widely used now to delineate organs and tissues of breast cancers. This study was aimed to compare 3D CT with thin slice 3D reconstruction in breast cancer patients to find a better technique for accurate evaluation of breast cancer.A total of 16-slice spiral CT scans and 3D reconstructions were performed on 15 breast cancer patients. All patients had been treated with modified radical mastectomy; 2D and 3D images of breast and tumours were obtained. The specimens were fixed and sliced at 2 mm thickness to obtain serial thin slice images, and reconstructed using 3D DOCTOR software to gain 3D images.Compared with 2D CT images, thin slice images showed more clearly the morphological characteristics of tumour, breast tissues and the margins of different tissues in each slice. After 3D reconstruction, the tumour shapes obtained by the two reconstruction methods were basically the same, but the thin slice 3D reconstruction showed the tumour margins more clearly.Compared with 3D CT reconstruction, thin slice 3D reconstruction of breast tumour gave clearer images, which could provide guidance for the observation and application of CT 3D reconstructed images and contribute to the accurate evaluation of tumours using CT imaging technology.
