1. Does the participation of a senior plastic surgeon improve perioperative care in craniosynostosis repair surgery?

Abstract

International audience; INTRODUCTION: The real impact of the participation of other surgical specialties together with neurosurgeons on perioperative care in craniosynostosis repair surgery has not been determined. The purpose of this study was to determine whether the participation of a second senior surgeon (plastic surgeon) during surgical repair of pediatric monosutural craniosynostosis improved perioperative medical care. MATERIAL AND METHODS: The authors retrospectively reviewed 2 cohorts of patients who had consecutively undergone primary repair surgery for trigonocephaly and unicoronal craniosynostosis. Infants were operated on by a single senior pediatric neurosurgeon before December 2017, and with the collaboration of a senior plastic surgeon after January 2018. RESULTS: Overall, 60 infants were included in the study: 29 in group 1 (single surgeon, 2011-2017), and 31 in group 2 (pair of surgeons, 2018-2021). Median surgery time was significantly shorter in group 2 than group 1: 180 vs 167 minutes; p = 0.0045. There was no significant difference between the 2 groups in blood loss or intra/postoperative packed erythrocyte transfusion. Postoperative drain output was significantly lower in group 2. Median length of hospital and intensive care stay were significantly shorter in group 2, by 1 and 2 days, respectively; p < 0.0001. Volume of infused solution, diuresis, immediate postoperative hemoglobin level, hematocrit, hemostasis (platelet count, fibrinogen, prothrombin time and activated partial thromboplastin time), and return to oral feeding did not differ from one group to the other. CONCLUSION: Results confirmed our impression of an improvement in perioperative medical care. However, the role of surgical experience and the influence of the medical/nursing staff must not be minimized in these complex surgical procedures.

Abstract

International audience; Objectives: To assess the contribution of whole-body magnetic resonance imaging (WBMRI) and bone scintigraphy (BS) in addition to skeletal survey (SS) in detecting traumatic bone lesions and soft-tissue injuries in suspected child abuse.Methods: In this prospective, multicentre, diagnostic accuracy study, children less than 3 years of age with suspected physical abuse were recruited. Each child underwent SS, BS and WBMRI. A blinded first review was performed in consensus by five paediatric radiologists and three nuclear medicine physicians. A second review investigated discrepancies reported between the modalities using a consensus result of all modalities as the reference standard. We calculated the sensitivity, specificity and corresponding 95% confidence interval for each imaging modality (SS, WBMRI and BS) and for the combinations [SS + WBMRI] and [SS + BS].Results: One hundred seventy children were included of which sixty-four had at least one lesion. In total, 146 lesions were included. The sensitivity and specificity of each examination were, respectively, as follows: 88.4% [95% CI, 82.0-93.1] and 99.7% [95% CI, 99.5-99.8] for the SS, 69.9% [95% CI, 61.7-77.2] and 99.5% [95% CI, 99.2-99.7] for WBMRI and 54.8% [95% CI, 46.4-63.0] and 99.7% [95% CI, 99.5-99.9] for BS. Sensitivity and specificity were, respectively, 95.9% [95% CI, 91.3-98.5] and 99.2% [95% CI, 98.9-99.4] for the combination SS + WBMRI and 95.2% [95% CI, 90.4-98.1] and 99.4% [95% CI, 99.2-99.6] for the combination SS + BS, with no statistically significant difference between them.Conclusion: SS was the most sensitive independent imaging modality; however, the additional combination of either WBMRI or BS examinations offered an increased accuracy.Key points: • SS in suspected infant abuse was the most sensitive independent imaging modality in this study, especially for detecting metaphyseal and rib lesions, and remains essential for evaluation. • The combination of either SS + BS or SS + WBMRI provides greater accuracy in diagnosing occult and equivocal bone injuries in the difficult setting of child abuse. • WBMRI is a free-radiation technique that allows additional diagnosis of soft-tissue and visceral injuries.

Abstract

OBJECTIVE Tranexamic acid (TXA) is an antifibrinolytic drug that has achieved significant reduction in perioperative blood loss and the quantity of blood transfused in many pediatric surgical procedures, without morbidity. Despite the accumulation of evidence regarding its effectiveness in craniosynostosis repair surgery, TXA is not unanimously employed by pediatric neurosurgery teams. The purpose of this study was to evaluate the impact of the routine use of TXA in a homogeneous population of children who underwent open surgery for monosutural craniosynostosis. METHODS The authors retrospectively reviewed 3 cohorts of patients that were subdivided from 2 cohorts surrounding the initiation of TXA (group 1, TXA− [2008–2011] and group 2, TXA+ [2011–2013]) and a third cohort of more recent patients (group 3, TXA+ [2016–2017]). TXA was administered using the same protocol comprising a loading dose of 10 mg/kg over 15 minutes after induction of general anesthesia followed by a 10 mg/kg per hour infusion until skin closure. Patients in all 3 groups underwent similar standardized procedures for scaphocephaly, trigonocephaly, and unicoronal craniosynostosis by the same pediatric neurosurgeon. RESULTS Overall, 102 infants were included in the study: 32 infants in group 1, 36 in group 2, and 34 in group 3. Significant reductions in transfusion of packed erythrocytes (PE) and fresh-frozen plasma (FFP) were observed between the TXA− and TXA+ time periods. The median volume of PE transfusion was reduced by > 50% with the use of TXA (42.8 mL/kg in the TXA− group vs 20.0 in the TXA+ groups, p < 0.0001). Reduction in PE transfusion was 100% postoperatively in the TXA+ groups (20.0 mL/kg in the TXA− group vs 0.0 in the TXA+ groups, p < 0.0001). The median volume of FFP transfusion was reduced by 100% with the use of TXA (12.8 mL/kg in the TXA− group vs 0.0 in the TXA+ groups, p < 0.0001). All children in group 1 received a transfusion, whereas 3 children (8%) and 7 children (20%) in groups 2 and 3, respectively, did not. Significant reductions in postoperative drain output were also noted between the TXA− and TXA+ time periods. The total hospital length of stay was significantly lower in the TXA+ groups (p < 0.0001). CONCLUSIONS Use of TXA reduced blood loss and the need for transfusions but also decreased the hospital length of stay and, thus, minimized overall medical care costs. Intraoperative administration of TXA in craniosynostosis repair surgery should be routinely used in all centers that practice these procedures.

Abstract

Analysing the surgical process, understanding the learning of the necessary gestures, how best to prepare them and carry them out, is an ambitious project. Since 2008, we have been working in close collaboration with the Innovation Center Computer Assisted Surgery(ICCAS) of Leipzig University, Germany, and using their tool, the Surgical Workflow Editor, which enables the recording of all the gestures and actions of surgeon, in the form of an action-instrument-target triple. We chose to examine standard surgical procedures, in other words those which were frequently carried out , standardized and short in duration. We therefore made live recordings in the operating theatre of the Neurosurgery Department of University Hospital of Leipzig. These concerned the gestures of surgeons performing spinal surgical procedures (removal of lumbar disc herniation) and cranial procedures (removal of pituitary tumors via transsphenoidal approach). Subsequently, in the same way, similar procedures were recorded in the Neurosurgery Department of the University Hospital of Rennes. Results illustrated the relationship in surgical practice between the surgeon's experience and the manual skills demonstrated in real-life situations. We demonstrated that there exists a real optimization of surgical gestures which develops with the seniority of the surgeons: overall reduction of operating time, reduction of the number of gestures in the most standardized phases particularly with the dominant hand, optimization of non-dominant hand use in the most complex phases., Comprendre le processus chirurgical, c'est à dire comprendre le processus d'apprentissage du geste à réaliser, sa préparation, son exécution et enfin son optimisation est un projet ambitieux. Depuis 2008, nous avons mené une collaboration étroite avec l'Innovation Center Computer Assisted Surgery de l'Université de Leipzig en Allemagne et utilisé leur outil, le "Surgical Workflow Editor" qui permet d'enregistrer en direct toute l'activité gestuelle d'un chirurgien sous la forme d'un triplet action-instrument-cible. Nous avons choisi d'étudier des procédures chirurgicales " simples", c'est-à-dire des procédures fréquentes, standardisées et de courte durée. Nous avons donc enregistré au bloc opératoire du service de Neurochirurgie de l'Hôpital Universitaire de Leipzig l'activité gestuelle des chirurgiens réalisant des procédures chirurgicales rachidienne (exérèse d'une hernie discale lombaire) et crânienne (exérèse d'un adénome hypophysaire par voie transsphénoïdale), puis de la même façon, ces procédures ont été enregistrées dans le service de Neurochirurgie de l'Hôpital Universitaire de Rennes. Les résultats ont montré les relations existant dans la pratique chirurgicale entre l'expérience des chirurgiens et la technique gestuelle dans un contexte de chirurgie réelle. Nous avons pu démontrer qu'il existe une réelle optimisation gestuelle évoluant avec la saniorisation des chirurgiens : économie globale du temps d'intervention, économie du nombre de gestes dans les phases les plus stéréotypées particulièrement avec la main dominante, optimisation de la main non dominante dans les phases les plus complexes.

Abstract

International audience; Despite progress in the initial management of glioblastoma (GB), the vast majority of patients will experience recurrence within 2-3 years. The medical treatment of these recurrences is being modified by the use of antiangiogenic therapies. Twenty-four patients, who relapsed from GB after chemoradiation followed by adjuvant temozolomide in Rennes, were treated by conventional chemotherapy (nitrosourea) or by the combination of irinotecan and bevacizumab. In this retrospective analysis, overall survival from diagnosis of recurrence was significantly longer in patients treated with the combination of bevacizumab and irinotecan than with nitrosourea (5 months versus 11.5 months). The combination of irinotecan and bevacizumab appeared to provide clinical benefit to patients with recurrent GB.

Abstract

Toward the creation of new computer-assisted intervention systems, Surgical Process Models (SPMs) is an emerging concept used for analyzing and assessing surgical interventions. SPMs represent Surgical Process (SPs) which are formalized as symbolic structured descriptions of surgical interventions, using a pre-defined level of granularity and a dedicated terminology. In this context, an important challenge is the creation of new metrics for the comparison and the evaluation of SPs. Thus, correlations between these metrics and pre-operative data allow to classify surgeries and highlight specific information on the surgery itself and on the surgeon, such as his/her level of expertise. In this paper, we explore the automatic classification of a set of SPs based on the Dynamic Time Warping (DTW) algorithm. DTW allows to compute a distance between two SPs that focuses on the different types of activities performed during surgery and their sequencing, by minimizing time differences. Indeed, it turns out to be a complementary approach to classical methods focusing on the time and the number of activities differences only. Experiments were carried out on 24 lumbar disc herniation surgeries to discriminate the level of expertise of surgeons according to prior classification of SPs. Unsupervised classification experiments have shown that this approach was able to automatically identify groups of surgeons according to their level of expertise (senior and junior), and opens many perspectives for the creation of new metrics for surgeries comparison and evaluation.

Abstract

In image-guided surgery, the automatic extraction of information from the Operating Room (OR) has recently gained much interest. In particular, the automatic recognition of surgical phases, or more generally surgical events, allows bringing an additional help to surgeons, for intra or post-operative use. Clinical applications include the creation of post-operative reports, teaching, learning, surgical assessment, optimisation of OR management and of surgeries. In this article, we present a novel approach that focused on the automatic recognition of phases by microscope image analysis, which has never been done before. We used a hybrid method that combines supervised classification to extract binary visual cues and a discrete Hidden Markov Model to take into account the temporal aspect. Our framework was tested on two various datasets, including one specific type of neurosurgical intervention and one type of ophthalmological surgery. Cross-validation studies were carried out to find recognition rates of 90% for the first dataset and 93% for the second one. This will enable the system to be used in clinical applications such as post-operative surgical video indexation., Dans le vaste domaine de la chirurgie guidée par l'image, l'extraction automatique d'informations émanant d'une salle d'opération a récemment été source de nombreuses études. Plus particulièrement, la reconnaissance automatique des étapes, ou plus globalement des événements, d'une procédure chirurgicale pourrait permettre d'apporter une aide intra ou post-opératoire non-négligeable aux chirurgiens. Les applications visées par ces approches sont la création automatique de rapports post-opératoires, l'enseignement, l'évaluation, l'optimisation de la gestion des salles d'opérations ou l'aide au geste chirurgical. Dans cet article, nous proposons une approche qui tend vers la reconnaissance automatique des étapes d'une chirurgie via les vidéos issues des microscopes chirurgicaux, ce qui n'a encore jamais été expérimenté. Notre méthode est basée sur la combinaison d'une classification supervisée permettant d'extraire des attributs propres à chaque chirurgie, et d'une modélisation de séries temporelles par chaine de Markov cachée. Des validations croisées ont été menées sur deux types distincts de chirurgie : une chirurgie du cerveau et une chirurgie ophtalmologique. Des taux de reconnaissance respectifs de 90% et 93% ont ainsi été trouvés, ouvrant la possibilité à de nombreuses applications dans le domaine de la chirurgie guidée par l'image, comme l'indexation de vidéos chirurgicales.

The data recorded were general parameters (operating time for the whole procedure and for each step), and general and specific parameters of the surgeon's activities (number of manual gestures, number and duration of actions performed, use of the instruments, and use of interventions on anatomic structures). The Mann-Whitney U test was used for comparison between the 2 groups of neurosurgeons. RESULTS: The operating time was statistically lower for the group of senior surgeons. The seniors statistically demonstrated greater economy in time and in gestures during the closure step, for sewing and for the use of scissors, needle holders, and forceps. The senior surgeons statistically worked for a shorter time on the skin and used fewer manual gestures on the thoracolumbalis fascia. The number of changes in microscope position was also statistically lower for this group. CONCLUSION: There is a relationship between surgical practice, as determined by a method of objective measurement using observation software, and surgical experience: gesture economy evolves with seniority.

Abstract

National audience; Dans cet article, nous appliquons une méthode d'analyse sur des descriptions de procédures de neurochirurgie dans le but d'en améliorer la compréhension. La base de données XML utilisée dans cette étude est constituée de la description de 157 chirurgies de tumeurs. Trois cent vingt deux variables ont été identifiées et décomposées en variables prédictives (connues avant l'opération) et variables à prédire (décrivant des gestes chirurgicaux). Une analyse factorielle des correspondances (AFC) a été réalisée sur les variables prédictives, ainsi qu'un arbre de décision basé sur un dendrogramme préalablement établi. Six classes principales de variables prédictives ont ainsi été identifiées. Puis, pour chacune de ces classes, une analyse AFC a été réalisée sur les variables à prédire, ainsi qu'un arbre de décision. Bien que le nombre de cas et le choix des variables constituent une limite à cette étude, nous avons réussi à prédire certaines caractéristiques liées aux procédures en partant de données prédictives.

Abstract

article clinique; The authors describe the case of a 72-year-old woman with dural plasmacytoma revealing an immunoglobulin (Ig) G-kappa multiple myeloma (MM). She presented with headaches and left hemiparesis. Magnetic resonance imaging demonstrated a right frontal extraaxial lesion arising from the dura mater, and biological studies revealed hypercalcemia, hyperproteinemia, and a serum gamma globulin peak. A diagnosis of IgG-kappa MM was based on microscopic examination and immunohistochemical analysis of the dural plasmacytoma as well as on signs of systemic myeloma after surgery. The patient died 3 years after the first symptoms of MM despite systemic chemotherapy and no recurrence of the dural plasmacytoma. Myelomatous involvement of the dura mater is a rare occurrence given that only three cases have been reported to date. Nevertheless, this pathological entity should be differentiated from solitary dural plasmacytoma (SDP) because the prognosis is radically different. Progression seems to be correlated with systemic disease in contrast to the long-term survival associated with SDP. Careful systemic evaluation should be made in such a presentation to rule out MM, which would require different management and has a different prognosis.

Abstract

BACKGROUND: Endoscopic third ventriculostomy is an effective procedure for treating obstructive (noncommunicating) hydrocephalus as an alternative to ventricular shunt placement. CASE: Five pregnant women undergoing endoscopic third ventriculostomy as treatment for newly diagnosed acute obstructive hydrocephalus (n=2) or in cases of malfunction of a preexisting ventriculoperitoneal shunt (n=3) are presented. CONCLUSION: Endoscopic third ventriculostomy may be preferred to ventricular shunt placement as the initial mode of treatment for pregnant patients with newly diagnosed obstructive hydrocephalus as well as in cases of malfunction of a preexisting shunt.

Abstract

Hydrocephalus may rarely be the revealing symptom of intraspinal tumours. This form of presentation is more common in malignant spinal cord tumours due to subarachnoid dissemination and meningeal tumoural infiltration up to the intracranial basal cisterns. In contrast, in benign intraspinal tumours the mechanism of hydrocephalus has been a subject of controversy and various explanations have been proposed. We discuss different pathophysiological hypotheses and propose a mechanical explanation based on current knowledge of cerebrospinal fluid (CSF) dynamics. We hypothesize that hydrocephalus in benign intraspinal tumours is the result of alteration of craniospinal compliance distribution. Intraspinal tumours may decrease the compliance of the caudal spinal portion, thereby reducing overall CSF space compliance. In this condition, minimal changes to other implicated factors such as CSF protein concentration, intracranial CSF flow resistance due to arachnoiditis or intracranial vascular compliance may have a greater effect on CSF dynamics. After tumour removal, hydrocephalus disappears rapidly due to spinal compliance recovery.
