Closed-loop insulin delivery systems have the potential to address suboptimal glucose control in children and adolescents with type 1 diabetes. We compared safety and efficacy of the Cambridge hybrid closed-loop algorithm with usual care over 6 months in this population.In a multicentre, multinational, parallel randomised controlled trial, participants aged 6-18 years using insulin pump therapy were recruited at seven UK and five US paediatric diabetes centres. Key inclusion criteria were diagnosis of type 1 diabetes for at least 12 months, insulin pump therapy for at least 3 months, and screening HbAOf 147 people initially screened, 133 participants (mean age 13·0 years [SD 2·8]; 57% female, 43% male) were randomly assigned to either the closed-loop group (n=65) or the control group (n=68). Mean baseline HbAThe Cambridge hybrid closed-loop algorithm had an acceptable safety profile, and improved glycaemic control in children and adolescents with type 1 diabetes. To ensure optimal efficacy of the closed-loop system, usage needs to be consistently high, as demonstrated with CamAPS FX.National Institute of Diabetes and Digestive and Kidney Diseases.

This study investigated the corrosion of aluminum in various ethanol/water mixtures. It has been shown that a critical corrosion temperature exists at which a sudden exothermic corrosion occurs. This critical corrosion temperature is dependent on presence of water and increases with increasing water concentration. This corrosion is independent of the atmosphere. Furthermore, it was demonstrated that denaturants in ethanol with a carbonyl group will further increase the critical corrosion temperature. This increase is due to a reaction of water and carbonyl to carbonyl-hydrate which reacts with the aluminum oxide surface forming a passivation layer. The critical corrosion temperature has been defined by the reaction of aluminum with ethanol. Thereby, the same energy is always released, independent of the water concentration. However, the presence of water influences the activation energy of this reaction. The observed corrosion of aluminum depends on the exposed surface area. Consequently, with increasing surface area, a higher statistical failure rate is observed. This is most likely due to defects in the passivation layer of aluminum.

Objective: To assess performance of the 2016 macrophage activation syndrome (MAS) classification criteria for patients with systemic juvenile idiopathic arthritis (JIA) who develop MAS while treated with biologic medications. Methods: A systematic literature review was performed to identify patients with MAS while being treated with interleukin (IL)-1 and IL-6 blocking agents. Clinical and laboratory information was compared to a large previously compiled historical cohort. Results: Eighteen publications were identified, and after removing duplicates, 35 patients treated with canakinumab and 49 patients with tocilizumab were available for analysis; 5 anakinra-treated patients were excluded due to limited numbers. MAS classification criteria were less likely to classify tocilizumab-treated patients as having MAS compared to the historical cohort or canakinumab-treated patients (56.7%, 78.5%, and 84%, respectively; P < 0.01). Patients who developed MAS while treated with canakinumab trended towards lower ferritin at MAS onset than the historical cohort (4,050 versus 5,353 ng/ml; P = 0.18) but had no differences in other cardinal clinical or laboratory features. In comparison, patients who developed MAS while treated with tocilizumab were less likely febrile and had notably lower ferritin levels (1,152 versus 5,353 ng/ml; P < 0.001). Other features of MAS were more pronounced in patients treated with tocilizumab, including lower platelet counts, lower fibrinogen, and higher aspartate aminotransferase levels. Mortality rates for patients with MAS treated with tocilizumab or canakinumab were not significantly different from the historical cohort. Conclusion: These findings show substantial alterations in MAS features that may limit utility of defined criteria for diagnosis of systemic JIA patients treated with biologic agents. © 2017, American College of Rheumatology

The ActivAge project is currently exploring heterogeneous IoT solutions for AHA over 9 different European Deployment Sites (DS), in an interoperable network. In this work, we present main features of the DS-RER experimentation, being implemented in the Emilia-Romagna Region (RER, Italy) and mostly aimed at effectively embedding IoT-enabled solutions within the actual organization and practices of regional social and health-care services, to support users over 65 suffering post-stroke conditions.

These factors lead to undue delay in the establishment of the correct diagnosis and the delivery of acceptable therapy, including surgery, radiation therapy, and chemotherapy. There is, however, evidence of improved results of treatment of these tumors since the addition of multiple drug chemotherapy to surgery and radiotherapy.

Psychogenic polydipsia can occur in infants and may be associated with urinary tract dilation. It is not known whether this dilation can lead to a decline in renal function, as has been reported in patients with diabetes insipidus and hydronephrosis. The structural changes may be reversed by treatment of polydipsia through fluid restriction and counseling.

The mechanical resistance of D₂O crystals, approxi-mately 12 cm long and 1.5 cm in diameter, was measured at frequeneies from 0.2 to 4 kHz and at temperatures from -90 to + 2 deg C. The results showed that the resistance of D₂O crystals has the same dependence on temperature and frequency, oscillation type, and crystallographic orientation as light ice. The activation energy, determined from the temperature dependence of the resistance maxima, has the same value as that of H₂O crystals. The frequency displacement was calculated. (J. S.R.)
