1. Differences in Attitudes and Practices of Cancer Pain Management between Medical Oncologists and Palliative Care Physicians

Abstract

This study aimed to evaluate whether there are differences in the attitudes and practices of cancer pain manage-ment between medical oncologists and palliative care physicians. An online nationwide survey was used to collect responses from board-certified medical oncologists and palliative care physicians in Japan. The survey questionnaire comprised 30 questions. The differences in responses between medical oncologists and palliative care physicians were examined. Out of the 1,227 questionnaires sent, 522 (42.5%) were returned. After apply-ing the exclusion criteria, 445 questionnaires (medical oncologists: n = 283; palliative care physicians: n = 162) were retained for analysis. Among the questions about potential barriers to optimal cancer pain man-agement, both medical oncologists and palliative care physicians considered the reluctance of patients to take opioids due to fear of adverse effects as the greatest barrier. Significantly different ratings between medical oncologists and palliative care physicians were observed on 5 of the 8 questions in this area. Significantly differ-ent ratings were observed for all questions concerning pain specialists and their knowledge. For effective cancer pain management, it is important to account for differences in attitudes and practice between medical oncolo-gists and palliative care physicians.

Abstract

International audience; We report on a Hitomi observation of IGR J16318−4848, a high-mass X-ray binary system with an extremely strong absorption of N_H ∼ 10^24 cm^−2. Previous X-ray studies revealed that its spectrum is dominated by strong fluorescence lines of Fe as well as continuum emission lines. For physical and geometrical insight into the nature of the reprocessing material, we utilized the high spectroscopic resolving power of the X-ray microcalorimeter (the soft X-ray spectrometer: SXS) and the wide-band sensitivity by the soft and hard X-ray imagers (SXI and HXI) aboard Hitomi. Even though the photon counts are limited due to unintended off-axis pointing, the SXS spectrum resolves Fe Kα_1 and Kα_2 lines and puts strong constraints on the line centroid and line width. The line width corresponds to a velocity of 160$$^{+300}_{-70}$$ km s^−1. This represents the most accurate, and smallest, width measurement of this line made so far from the any X-ray binary, much less than the Doppler broadening and Doppler shift expected from speeds that are characteristic of similar systems. Combined with the K-shell edge energy measured by the SXI and HXI spectra, the ionization state of Fe is estimated to be in the range of Fe i–iv. Considering the estimated ionization parameter and the distance between the X-ray source and the absorber, the density and thickness of the materials are estimated. The extraordinarily strong absorption and the absence of a Compton shoulder component have been confirmed. These characteristics suggest reprocessing materials that are distributed in a narrow solid angle or scattering, primarily by warm free electrons or neutral hydrogen. This measurement was achieved using the SXS detection of 19 photons. It provides strong motivation for follow-up observations of this and other X-ray binaries using the X-ray Astrophysics Recovery Mission and other comparable future instruments.

Abstract

The Crab nebula originated from a core-collapse supernova (SN) explosion observed in 1054 A.D. When viewed as a supernova remnant (SNR), it has an anomalously low observed ejecta mass and kinetic energy for an Fe-core collapse SN. Intensive searches were made for a massive shell that solves this discrepancy, but none has been detected. An alternative idea is that the SN1054 is an electron-capture (EC) explosion with a lower explosion energy by an order of magnitude than Fe-core collapse SNe. In the X-rays, imaging searches were performed for the plasma emission from the shell in the Crab outskirts to set a stringent upper limit to the X-ray emitting mass. However, the extreme brightness of the source hampers access to its vicinity. We thus employed spectroscopic technique using the X-ray micro-calorimeter onboard the Hitomi satellite. By exploiting its superb energy resolution, we set an upper limit for emission or absorption features from yet undetected thermal plasma in the 2-12 keV range. Abstract

The origin of the narrow Fe-K�� fluorescence line at 6.4 keV from active galactic nuclei has long been under debate; some of the possible sites are the outer accretion disk, the broad line region, a molecular torus, or interstellar/intracluster media. In February-March 2016, we performed the first X-ray microcalorimeter spectroscopy with the Soft X-ray Spectrometer (SXS) onboard the Hitomi satellite of the Fanaroff-Riley type I radio galaxy NGC 1275 at the center of the Perseus cluster of galaxies. With the high energy resolution of ~5 eV at 6 keV achieved by Hitomi/SXS, we detected the Fe-K�� line with ~5.4 �� significance. The velocity width is constrained to be 500-1600 km s$^{-1}$ (FWHM for Gaussian models) at 90% confidence. The SXS also constrains the continuum level from the NGC 1275 nucleus up to ~20 keV, giving an equivalent width ~20 eV of the 6.4 keV line. Because the velocity width is narrower than that of broad H�� line of ~2750 km s$^{-1}$, we can exclude a large contribution to the line flux from the accretion disk and the broad line region. Furthermore, we performed pixel map analyses on the Hitomi/SXS data and image analyses on the Chandra archival data, and revealed that the Fe-K�� line comes from a region within ~1.6 kpc from the NGC 1275 core, where an active galactic nucleus emission dominates, rather than that from intracluster media. Therefore, we suggest that the source of the Fe-K�� line from NGC 1275 is likely a low-covering fraction molecular torus or a rotating molecular disk which probably extends from a pc to hundreds pc scale in the active galactic nucleus system., 20 pages, 8 figures, 6 tables, accepted for publication in PASJ

Abstract

High-resolution X-ray spectroscopy with Hitomi was expected to resolve the origin of the faint unidentified E=3.5 keV emission line reported in several low-resolution studies of various massive systems, such as galaxies and clusters, including the Perseus cluster. We have analyzed the Hitomi first-light observation of the Perseus cluster. The emission line expected for Perseus based on the XMM-Newton signal from the large cluster sample under the dark matter decay scenario is too faint to be detectable in the Hitomi data. However, the previously reported 3.5 keV flux from Perseus was anomalously high compared to the sample-based prediction. We find no unidentified line at the reported high flux level. Taking into account the XMM measurement uncertainties for this region, the inconsistency with Hitomi is at a 99% significance for a broad dark-matter line and at 99.7% for a narrow line from the gas. We do not find anomalously high fluxes of the nearby faint K line or the Ar satellite line that were proposed as explanations for the earlier 3.5 keV detections. We do find a hint of a broad excess near the energies of high-n transitions of Sxvi (E=3.44 keV rest-frame) -- a possible signature of charge exchange in the molecular nebula and another proposed explanation for the unidentified line. While its energy is consistent with XMM pn detections, it is unlikely to explain the MOS signal. Abstract

Here we present a new approach for constraining luminous blazars, incorporating fully time-dependent and self-consistent modeling of bright gamma-ray flares of PKS1510-089 resolved with Fermi-LAT, in the framework of the internal shock scenario. The results of our modeling imply the location of the gamma-ray flaring zone outside of the broad-line region, namely around 0.3pc from the core for a free-expanding jet with the opening angle Gamma, \theta_\mathrm{jet} \simeq 1 (where Gamma is the jet bulk Lorentz factor), up to \simeq 3pc for a collimated outflow with Gamma, \theta_\mathrm{jet} \simeq 0.1. Moreover, under the Gamma, \theta_\mathrm{jet} \simeq 1 condition, our modeling indicates the maximum efficiency of the jet production during the flares, with the total jet energy flux strongly dominated by protons and exceeding the available accretion power in the source. This is in contrast to the quiescence states of the blazar, characterized by lower jet kinetic power and an approximate energy equipartition between different plasma constituents. We demostrate how strictly simultaneous observations of flaring PKS1510-089 at optical, X-ray, and GeV photon energies on hourly timescales, augmented by extensive simulations as presented in this paper, may help to impose further precise constraints on the magnetization and opening angle of the emitting region. Our detailed modeling implies in addition that a non-uniformity of the Doppler factor across the jet, caused by the radial expansion of the outflow, may lead to a pronounced time distortion in the observed gamma-ray light curves, resulting in particular in asymmetric flux profiles with substantially extended decay phases., Comment: 11 pages, 7 figures, accepted for publication in ApJ

Abstract

Mastication has been suggested to increase neuronal activities in various regions of the human brain. However, because of technical difficulties, the fine anatomical and physiological regions linked to mastication have not been fully elucidated. Using functional magnetic resonance imaging during cycles of rhythmic gum-chewing and no chewing, we therefore examined the interaction between chewing and brain regional activity in 17 subjects (aged 20-31 years). In all subjects, chewing resulted in a bilateral increase in blood oxygenation level-dependent (BOLD) signals in the sensorimotor cortex, supplementary motor area, insula, thalamus, and cerebellum. In addition, in the first three regions, chewing of moderately hard gum produced stronger BOLD signals than the chewing of hard gum. However, the signal was higher in the cerebellum and not significant in the thalamus, respectively. These results suggest that chewing causes regional increases in brain neuronal activities which are related to biting force.

Abstract

Rigid-body-spring networks are used to model brittle fracture in homogeneous, isotropic materials. Discretization is based on Voronoi diagrams with random geometry. Methods are given for ensuring elastic uniformity and maximizing the degree of isotropy with respect to potential crack direction. A method for rendering ordinary beam-spring networks elastically uniform is also given. Model abilities to predict crack patterns are demonstrated through analyses of mixed mode fracture in cement mortar panels and shear failure in reinforced concrete beams. The latter example utilizes an effective method for introducing continuous reinforcement into random geometry beam-spring networks.

Abstract

A retrospective study was carried out in 110 cadaveric kidney transplant recipients to compare the effects of low doses of cyclosporine (CsA), azathioprine (AZP) and steroids (triple-drug therapy) with those of higher doses of steroids plus AZP (conventional immunosuppression). Graft survival rate in the triple-drug therapy was 77%, 69%, and 69% at 1, 3, and 5 years, respectively. This was significantly better than 48%, 34%, and 29% in conventional immunosuppression. The incidence of acute rejection episodes was significantly lower in the triple-drug therapy than in conventional immunosuppression (25% vs 58%). In conclusion, our study shows that triple-drug therapy using low-dose cyclosporine is the safest of the immunosuppressive regimens and provides a beneficial effect on the long-term survival of cadaveric kidney transplants.

Abstract

Age-related changes in mastication-induced brain neuronal activity have been suggested. However, in humans, little is known about the anatomical regions involved. Using fMRI during cycles of rhythmic gum-chewing and no chewing, we have examined the effect of aging on brain regional activity during chewing in young adult (19–26 yrs), middle-aged (42–55 yrs), and aged (65–73 yrs) healthy humans. In all subjects, chewing resulted in a bilateral increase in the BOLD signals in the sensorimotor cortex, cerebellum, thalamus, supplementary motor area, and insula, and a unilateral increase in the right prefrontal area. In the first three regions, the signal increases were attenuated in an age-dependent manner, whereas, in the right prefrontal area, the converse was seen. The remaining two regions showed no significant differences with ages. These results indicate that chewing causes regional increases in neuronal activity in the brain, some of which are age-dependent.

First, we divide the time-domain data into several subbands and select the sub band to embed the watermarking. Then, we spread the signal included in the selected sub band using Direct Spectrum Spreading (DSS), one of the spectrum spreading methods, and embed the watermarking in the frequency-domain data. Finally, we reconstruct the signal by synthesizing whole subbands. By this method, we can reduce the noise and improve security and robustness against MPEG1 Audio Layer3 compression. Experimental results show good robustness of the approach against some signal processing manipulations. Quality and robustness are evaluated by computer simulation.

Abstract

Epidemiological studies have demonstrated that nonsteroidal anti-inflammatory drugs (NSAIDs), known to inhibit cyclooxygenase (COX), reduce the risk of colorectal cancer. COX is a key enzyme in prostaglandin biosynthesis, and two isoforms of COX, COX-1 and COX-2, have been identified. Recently COX-2 has been reported to frequently overexpress in colorectal neoplasms and to play a role in colorectal tumorigenesis and tumour progression. In this study, using immunohistochemistry, we examined COX-2 expression in advanced human colorectal cancer and its correlation with clinicopathological features. COX-2 expression was observed mainly in the cytoplasm of cancer cells in all the specimens examined, but some stromal cells and endothelial cells were also stained. According to the grade of COX-2 expression of the cancer cells, patients were divided into high- and low-COX-2 expression groups. High-COX-2 expression significantly correlated with tumour recurrence, especially haematogenous metastasis. These results suggest that a selective COX-2 inhibitor can be a novel class of therapeutic agents not only for tumorigenesis but also for haematogenous metastasis of cololectal cancer. To our knowledge, this is the first report on the correlation between COX-2 overexpression and recurrence of colorectal cancer. © 2000 Cancer Research Campaign

Abstract

Plasma 5-hydroxytryptamine (serotonin), tryptophan, neopterin and cortisol levels were measured in patients with depressive cancer cachexia and in healthy controls during the same time period. Patients with advanced cancers had significantly raised neopterin, a marker of endogenous gamma-interferon (IFN-γ) production, and cortisol values, but decreased serotonin and tryptophan levels. Much work has been done to elucidate the possible role of serotonin in depressive states. IFN-γ induces a high level of indoleamine dioxygenase (IDO), a tryptophan degrading enzyme, and high cortisol levels induce high tryptophan oxygenase activity, which in turn increases metabolism along the tryptophannicotinic acid pathway. These results suggest that persistent immune activation and intense adrenal activity occur in patients with cancer cachexia, resulting in disorders involving tryptophan metabolism followed by depression in cancer cahexia.

Abstract

Rho protein (rho p21), a p21ras-related small guanine nucleotide binding protein, regulates cytoskeletal organization in a number of different types of cells. Evidence has indicated that Clostridium botulinum-derived ADP-ribosyltransferase (C3 exoenzyme) specifically ADP-ribosylates rho p21 at Asn41 and renders it functionally inactive. In this study, we examined the involvement of rho p21 in osteoclastic bone resorption using the C3 exoenzyme. When osteoclast-like multinucleated cells obtained from cocultures of mouse osteoblastic cells and bone marrow cells were placed on dentine slices, they formed ringed structures of podosomes containing F-actin (corresponding to the clear zone) within 8 hours. Many resorption pits were formed on dentine slices after culture for 24 hours. The C3 exoenzyme at 0.15-10 micrograms/ml added to the culture medium disrupted the ringed structure of podosomes in osteoclast-like cells in a dose-dependent manner. Correspondingly, pit formation by osteoclast-like cells on dentine slices was dose-dependently inhibited also by adding the C3 exoenzyme. Microinjection of the C3 exoenzyme into osteoclast-like cells placed on culture dishes completely disrupted the ringed podosome structure within 20 minutes. The amount of the rho p21 which was ADP-ribosylated by the C3 exoenzyme in vitro was much greater in purified osteoclast-like cells than in osteoblastic cells. Prior exposure of the purified osteoclast-like cell preparation to the C3 exoenzyme in vivo markedly decreased the amount of unribosylated rho p21. This indicated that the C3 exoenzyme incorporated into osteoclast-like cells effectively ADP-ribosylates rho p21 in vivo.(ABSTRACT TRUNCATED AT 250 WORDS)

Abstract

Three cases of successful pregnancies in renal transplant recipients who had undergone transplantation in the Okayama University Medical School Hospital are reported. Two of the women had received an organ from a living relative and one woman received a cadaveric organ graft. These patients, aged 28-37 at the time of the delivery, had received their transplants 2-5 years prior to their conception. The periods of gestation ranged between 35 and 40 weeks. The weight of the babies at birth ranged from 2,380g to 2,500g and the apgar score at 1 min was 8 or 9. None of the infants showed any congenital abnormalities. Lower-segment cesarean section was performed in all of three cases. Serum creatinine levels, an indicator of renal graft function, did not deteriorate during the pregnancy or after delivery. Although further work is needed to solve problems regarding pregnancy in renal transplant recipients, these results encouraged us to meet their hope for a baby.

Abstract

One-hundred-nine HLA-haploidentical living related renal transplants have been retrospectively analysed to compare the effect of donor-specific blood transfusion (DST) and different immunosuppressive regimens on graft survival and acute rejection. The recipients were divided into four groups according to the immunosuppressive therapy. Group 1 (n = 44): conventional therapy with posttransplant azathioprine (AZP) + methylprednisolone (MP). Group 2 (n = 25): pretransplant DST + posttransplant AZP + MP. Group 3 (n = 12): triple-drug therapy with posttransplant AZP + MP + cyclosporine (CS). Group 4 (n = 25): pretransplant DST + posttransplant AZP + MP + CS. The five-year actuarial survival rates for groups 1, 2, 3 and 4 were 48%, 73%, 79%, and 89%, respectively. The graft survival rate in group 3 was significantly (p less than 0.01) better than that in group 1. The transfusion effect was reduced, and appears as a 10% improvement in the graft survival in the cyclosporin era compared with a 25% improvement at pre-cyclosporin era. Furthermore, the incidence of the first rejection episode was decreased in recipients that received DST. The present study revealed that DST, as pretransplant conditioning has a definite impact on rejection-free long-term graft survival in HLA-haploidentical living-related kidney recipients and the most favorable outcome in such patients could be achieved by DST pretreatment in conjunction with posttransplant triple-drug therapy including cyclosporine.

Abstract

A simple method was devised for applying mechanical stretching to bone cell cultures. Bone cells cultured on the flexible plastic membrane of a Petriperm dish are placed over a template with a convex surface. A lead weight is then placed on top of the dish which causes the membrane and the tightly attached cells to be stretched. Mechanical stretching, applied either intermittently or continuously for a 2-hour period resulted in a 64% increase in the number of cells synthesizing DNA. Stretching the cells also significantly increased incorporation of tritiated proline and tritiated leucine. To assay the ratio of collagenous to noncollagenous protein, medium and cell layers of cultures labeled with tritiated leucine were incubated with collagenase and the digests chromatographed on PD 10 columns. The amount of collagen synthesized by stretched and unstretched cultures did not differ; but an increased synthesis of noncollagenous proteins was observed in the stretched cultures.

Abstract

The increasing residue levels of PCBs and DDE in the blubber of dalli -type Dall's porpoises were found to have a negative effect on the testosterone levels in blood. Decrease in the levels of testosterone was statistically significant with increase in DDE concentrations. The results obtained suggest that the present levels of environmental contamination by persistent organochlorines can cause an imbalance of sex hormones and subsequent reproductive abnormalities in wild. The other hormone measured, aldosterone, which has no sexual function, was independent of the effects of both PCBs and DDE.

Abstract

Orthotopic liver transplantations were performed on 12 pigs between November, 1986 and February, 1987. Survivals and causes of death were assessed according to the type of surgical procedure employed. Six operations were carried out according to the original procedure of Terblanche et al, whereas for other six animals, the transplantation was conducted by our modified procedure in which cannulation was made into the splenic vein without splenectomy at the veno-venous (v-v) bypass and a diaphragmatic cuff was used for the anastomosis of suprahepatic vena cava. 1/6 animals with original procedure and 5/6 with our modified procedure survived for more than 7 days. The total ischemic time was significantly short in the group subjected to the modified procedure. The major cause of death in the group on which the original procedure was conducted was gastric ulceration, accounting for 67% of the mortality, whereas in the modified procedure group, the mortality rate was reduced significantly to 17%. This operative technique has provided excellent survival rates in our transplanted animals.

Abstract

It is well recognized that hemorrhage or kinking of the suprahepatic vena caval anastomosis is the most fatal complication apparent within 24 hr after orthotopic liver transplantation in the pig. A secured technique for suprahepatic vena caval anastomosis is described in detail herein. Our technique include (1) the use of a diaphragmatic cuff, (2) the closure of phrenic vein orifices using transfixation technique, and (3) the application of "stay-suture" in the orifice of right phrenic vein. This technique has added significantly to the strength of the suprahepatic vena caval anastomosis, and there has been no experiences of uncontrollable bleeding and kinking of suprahepatic vena caval anastomosis in 48 porcine orthotopic transplants of the liver. In conclusion, we believe that this technique described herein is a simplified and secured method to avoid the hemorrhage and kinking of suprahepatic anastomosis in porcine liver transplantation.

An artificial liver support system for plasma exchange and plasma perfusion through BR-601 resin using a membrane separator was applied to 5 patients with postoperative liver failure. Percent absorption of total and direct bilirubin, and of bile acids were 77.1 +/- 6.4, 78.4 +/- 6.1, and 93.4 +/- 3.6%, respectively, when 250 ml of plasma was treated. Percent reductions in total and direct bilirubin, and in bile acids were 24.5 +/- 5.8, 25.5 +/- 5.8 and 30.9 +/- 8.5%, respectively. In contrast, percent reductions in total and direct bilirubin, and in bile acids by plasma exchange were 30.9 +/- 13.3, 34.5 +/- 12.5 and 24.2 +/- 8.5%, respectively. The coma grade was improved in 4 out of 5 cases, but unfortunately the patients did not recover. In conclusion, plasma perfusion through BR-601 resin is expected to play a promising role in artificial liver support systems because of its capacity to absorb bilirubin and bile acids.
