1. Combined utility of maximum standardized uptake value and its change after neoadjuvant chemotherapy in predicting postoperative recurrence in esophageal cancer

Abstract

Background: Positron emission tomography（PET） is the standard method for metabolic and quantitative evaluation of therapeutic response to chemotherapy in solid tumors. This study determined the ability of PET parameters used in combination in predicting recurrence-free survival（RFS）after neoadjuvant chemotherapy （NAC）and operation for esophageal cancer. Methods: This single-center retrospective study included 96 patients who underwent 18F-fluorodeoxyglucose-PET/computed tomography before and after NAC. Maximum standardised uptake value（SUVmax）after NAC and the rate of change in SUVmax following NAC（ΔSUVmax）were determined to examine their relationship with postoperative recurrence and clinicopathological factors. Results: Receiver operating characteristic curves, with recurrence after NAC as the event, were used to determine optimal cut-off SUVmax and ΔSUVmax of 6.8 and 45.7, respectively. Using these cut-off values, the patients were classified into four groups: Group A, SUVmax > 6.8 and ΔSUVmax ≤ 45.7; Group B, SUVmax > 6.8 and ΔSUVmax > 45.7; Group C, SUVmax ≤ 6.8 and ΔSUVmax ≤ 45.7; and Group D, SUVmax ≤ 6.8 and ΔSUVmax > 45.7. Of the four groups, Group D had the longest RFS compared to the other groups. Conclusion: Combination of SUVmax with ΔSUVmax was useful for evaluating the effects of NAC in patients with esophageal cancer.

Abstract

The activation of melanocortin 1 receptor (MC1R) on melanocytes stimulates the production of eumelanin. A tridecapeptide α melanocyte-stimulating hormone (αMSH) is known to induce skin pigmentation.We characterised the properties of a novel oral MC1R agonist dersimelagon (MT-7117) with respect to its specific binding to MC1R, downstream signalling and eumelanin production in experimental models.The competitive binding and production of intracellular cyclic adenosine 3', 5'-monophosphate in cells expressing recombinant melanocortin receptors were examined. A mouse melanoma cell line B16F1 was used for the evaluation of in vitro melanin production. The in vitro activity of MT-7117 was determined with αMSH and [NleMT-7117 showed the highest affinity for human MC1R compared to the other melanocortin receptors evaluated and agonistic activity for human, cynomolgus monkey and mouse MC1R, with ECMT-7117 is a novel oral MC1R agonist that induces melanogenesis in vitro and in vivo, suggesting its potential application for the prevention of phototoxic reactions in patients with photodermatoses, such as erythropoietic protoporphyria and X-linked protoporphyria.

16. <Notes> Comparative study on white-collar workers career formation in the UK and Japan : main fact findings from the web-questionnaire survey about UK white collar workers in large firm

Abstract

The purpose of this research note is to investigate how UK’s white-collar workers including managers form their career in organization, by comparing with previous international comparative studies, such as Storey et al. (1997) or Koike and Inoki (2002).The 1000 data-set of white-collar workers were gathered by web-questionnaire survey about white collar workers, consisting of about 400 managers being employed in UK firms (hereafter abridged as 2018 UK research). Although a large number of research about white-collar’s career formation, including new career theory such as boundaryless career as well as traditional career model, but unfortunately little is known about comparable data on real circumstance of white-collar workers career in the UK and Japan. The information collected in our research on white-collar workers seems to be of use in comparing with previous research, because 2018 UK research are designed to compare with previous research by making same question as the previous research.According to previous studies such as Koike and Inoki (2002), we should examine following hypotheses.1 While many UK managers would tend to change job frequently, Japanese managers don’ t tend to change job frequently.2 One of conditions that enable to change job would seem to be strong occupational relevance of school education in the UK, but it would not seem strong in Japan. In addition to this condition, it seems that manager’s job would be required official or social qualification because these qualifications will enhance portability of manager’s job.3 Manager’s career formation can be approached from the view point of horizontal breadth of organizational career. Previous research show that UK managers breadth of career in organization would be narrower than that of Japanese managers.4 Manager’s career formation can also be approached from the view point of vertical promotion of organizational career. Previous research show that speed of UK managers of vertical promotion would be faster than that of Japanese managers.Result of our examination based on the data of 2018 UK research and international comparison between UK and Japan are as follows., 1 The ratio of UK managers with non-experience working for other firms differs between UK and Japan. That ratio also means who entered as new school leaver and didn’t experienced job change (in other words, they are current firms is only firms worked for) is about 20 percent, which is much lower than about 80 percent of managers in Japan.2 Occupational relevance of school education in the UK is stronger than that of Japan.The result of UK managers answering question about useful things in coping with current manager’s job are educational content of advanced academic studies as well as Off-JT of firm. On the contrary, many of managers in Japan answered that many job experiences within current function, or experience in other function, or advice from their boss.3 Official or social certification requirement of UK’s manager job would be stronger than that of Japanese manager. While 67.4 percent of managers worked for large firms in the UK answered yes about the question whether there are any official or social qualifications that are essential or desirable to gain employment or to work in your current job, only 28.9 percent of Japanese manager answered yes.4 It was found that horizontal breadth of UK’ s managers career in large firm are narrower than that of Japanese managers. In case of UK managers, the ratio of function specific typed manager, which means 76 percent or more of one’s experience in a given work function (extensive experience in a given function), are 68.3 percent, which are higher than that of Japanese managers.5 Regarding with speed of vertical promotion, it was found that fast truck which means the route for candidate of future officer prevail much more in the UK than Japan. In addition to that, difference in promotion amongst employees hired during same year arise earlier than that of Japanese managers. UK managers promotional difference arise at 4.26 year, 7.86 years in case of Japanese managers.To sum up the major characteristics of UK manager’s career formation, their changing job orientation is relatively higher turnover rate than Japanese managers.The fact that their changing job orientation is relatively higher than that of manager s in Japan suggests that there would be suitable evidences to new career theory like boudaryless career in labor market in the UK.

Abstract

Background: Excessive alcohol intake has been shown to be associated with cardiovascular disease via metabolic pathways. However, the relationship between alcohol intake and obesity has not been fully elucidated. We aimed to examine the association of alcohol consumption with fat deposition and anthropometric measures. Methods: From 2006–2008, we conducted a cross-sectional study in a population-based sample of Japanese men aged 40 through 79 years. Areas of abdominal visceral adipose tissue (VAT) and subcutaneous adipose tissue (SAT) were calculated using computed tomography imaging. Based on a questionnaire, we classified participants into five groups according to weekly alcohol consumption, excluding former drinkers: non-drinkers (0 g/week), 0.1–160.9, 161–321.9, 322–482.9, and ≥483 g/week. Multivariable linear regression was used to estimate adjusted means of obesity indices for each group. Results: We analyzed 998 men (mean age and body mass index [BMI], 63.8 years and 23.6 kg/m2, respectively). 18. Relationships of Posterior Condylar Offset Change to Condylar Length and Pre/Postoperative Range of Motion in Total Knee Arthroplasty

Abstract

This study investigated the relationships of pre- to postoperative changes in posterior condylar offset（PCO）to differences in condylar length and range of motion in posterior stabilized（PS）-type total knee arthroplasty（TKA）. Subjects and Methods: We studied 40 knees of 40 patients（10 males and 30 females）treated by PS-type TKA. Slide calipers were used to measure condyle lengths intraoperatively and PCO was measured by standard X-ray. The patients were divided into two groups based on a pre- to postoperative change in PCO of ≥ 3mm or＜3mm, with the differences in both condylar length and range of motion compared between groups. The mean differences in condyle lengths were 2.6mm（33 knees）and 4.3mm（7 knees）in cases with PCO changes of＜3mm and ≥ 3mm, respectively. The means in the respective groups were −6° and −7° for preoperative extension, 116° and 118° for preoperative flexion, −3° and −4° for postoperative extension, and 131° and 129° for postoperative flexion. There was no significant difference in the range of motion between the groups. Cases with a large difference in condylar lengths were likely to have a small PCO postoperatively; however, the postoperative range of knee flexion was not significantly related to a small postoperative PCO. These findings suggested that preoperative range of knee motion, age, and type of TKA could influence both the postoperative range of motion and PCO.

Abstract

OBJECTIVE To investigate the clinical outcomes of patients who underwent cardioversion compared with those who did not have cardioverson in a large dataset of patients with recent onset non-valvular atrial fibrillation. DESIGN Observational study using prospectively collected registry data (Global Anticoagulant Registry in the FIELD-AF-GARFIELD-AF). SETTING 1317 participating sites in 35 countries. PARTICIPANTS 52 057 patients aged 18 years and older with newly diagnosed atrial fibrillation (up to six weeks' duration) and at least one investigator determined stroke risk factor. MAIN OUTCOME MEASURES Comparisons were made between patients who received cardioversion and those who had no cardioversion at baseline, and between patients who received direct current cardioversion and those who had pharmacological cardioversion. Overlap propensity weighting with Cox proportional hazards models was used to evaluate the effect of cardioversion on clinical endpoints (all cause mortality, non-haemorrhagic stroke or systemic embolism, and major bleeding), adjusting for baseline risk and patient selection. RESULTS 44 201 patients were included in the analysis comparing cardioversion and no cardioversion, and of these, 6595 (14.9%) underwent cardioversion at baseline. The propensity score weighted hazard ratio for all cause mortality in the cardioversion group was 0.74 (95% confidence interval 0.63 to 0.86) from baseline to one year follow-up and 0.77 (0.64 to 0.93) from one year to two year follow-up. Of the 6595 patients who had cardioversion at baseline, 299 had a follow-up cardioversion more than 48 days after enrolment. 7175 patients were assessed in the analysis comparing type of cardioversion: 2427 (33.8%) received pharmacological cardioversion and 4748 (66.2%) had direct current cardioversion. During one year follow-up, event rates (per 100 patient years) for all cause mortality in patients who received direct current and pharmacological cardioversion were 1.36 (1.13 to 1.64) and 1.70 (1.35 to 2.14), respectively. CONCLUSION In this large dataset of patients with recent onset non-valvular atrial fibrillation, a small proportion were treated with cardioversion. Direct current cardioversion was performed twice as often as pharmacological cardioversion, and there appeared to be no major difference in outcome events for these two cardioversion modalities. For the overall cardioversion group, after adjustments for confounders, a significantly lower risk of mortality was found in patients who received early cardioversion compared with those who did not receive early cardioversion. 21. Empathy with Human's and Robot's Embarrassments in Virtual Environments

Abstract

We feel embarrassed not only when we are embarrassed but also when we are watching others embarrassed. Humans show empathy for pain not only human others but also robots. However, it has not been investigated whether humans show empathy for robot's embarrassment. Thus, we aimed to test whether humans can empathize with robot's embarrassment in virtual environments. Four situations both of non-embarrassing and embarrassing stimuli were presented on an HMD, and participants were asked to rate their own feeling of embarrassment and the actor's feeling of embarrassment. We found that the own feeling of embarrassment was higher in human than robot actors, and higher in embarrassing than non-embarrassing conditions. The actor's feeling of embarrassment was rated higher in embarrassing than non-embarrassing conditions, and the effect was much larger in human than robot actors. Abstract

Lipomyces starkeyi is an oil-producing yeast that can produce triacylglycerol (TAG) from glycerol as a carbon source. The TAG was mainly produced after nitrogen depletion alongside reduced cell proliferation. To obtain clues for enhancing the TAG production, cell metabolism during the TAG-producing phase was characterized by metabolomics with 13C labeling. The turnover analysis showed that the time constants of intermediates from glycerol to pyruvate (Pyr) were large, whereas those of tricarboxylic acid (TCA) cycle intermediates were much smaller than that of Pyr. Surprisingly, the time constants of intermediates in gluconeogenesis and the pentose phosphate (PP) pathway were large, suggesting that a large amount of the uptaken glycerol was metabolized via the PP pathway. To synthesize fatty acids that make up TAG from acetyl-CoA (AcCoA), 14 molecules of nicotinamide adenine dinucleotide phosphate (NADPH) per C16 fatty acid molecule are required. Because the oxidative PP pathway generates NADPH, this pathway would contribute to supply NADPH for fatty acid synthesis. To confirm that the oxidative PP pathway can supply the NADPH required for TAG production, flux analysis was conducted based on the measured specific rates and mass balances. Flux analysis revealed that the NADPH necessary for TAG production was supplied by metabolizing 48.2% of the uptaken glycerol through gluconeogenesis and the PP pathway. This result was consistent with the result of the 13C-labeling experiment. Furthermore, comparison of the actual flux distribution with the ideal flux distribution for TAG production suggested that it is necessary to flow more dihydroxyacetonephosphate (DHAP) through gluconeogenesis to improve TAG yield.

Abstract

To develop an accurate diagnosis of “new ulcer disease” in koi carp, we produced four monoclonal antibodies (mAbs) against a strain of atypical Aeromonas salmonicida isolated from koi carp. These mAbs did not cross-react with an isolate of atypical A. salmonicida from Japanese flounder and other pathogenic bacteria. Re-isolation from artificially infected koi carp was achieved by selecting the blue colonies on agar medium containing Coomassie brilliant blue, and some of the colonies were detected by immunofluorescent staining using the mAbs. These results suggested that the mAbs can distinguish atypical A. salmonicida from koi carp from resident aeromonads.

25. <Notes> Labor Market in the UK will be ILM or OLM? : Review on international comparative studies

Abstract

Main focus of this paper is process of skill formation, surveying previous literatures on Internal Labor Market(hereafter abridged as ILM) and Occupational Labor Market (hereafter abridged as OLM), particularly focusing on international comparative study.There are some previous studies concerning this theme by Japanese scholars, such as Inagami (1997), The Japan Institute of labour (2003), The Japan Institute for Labour Policy and Training (2009), Ueda (2007), Syokugyodai (2011). However, these discuss mainly the process and structure on vocational education and training (hereafter VET) as foundation of OLM, but they don’t have a perspective on ILM. On the other hand, as regards ILM, there are some case studies on career and skill formation about white-collar career in large firms, such as The Japan Institute of labour (1998), Koike et al. (2002), but they don't have a perspective on OLM.Characteristics of this paper is to discuss recent trend of ILM and OLM as well as explore political implication for challenges labour market in Japan are facing now, resting on framework by Rubery and Grimshaw (2003) which classifies the UKs labour market as 'market-led weak, weak ILM/weak OLM approach' from international perspective.There are similarities between Japan and the UK, where both countries may have some challenges about not only connection's issues from school to work, but also problems about increasing of nonstandard workers and declining ILM. However, there are some differences in directing policy between two countries. In the UK, there may be political intention to vitalize apprenticeship system as a fundamental of OLM, so as to complement weakening ILM. In Japan, this political idea seems weak, although there is certain amount of policies in Japan. Therefore, we will need to examine how recent situation of OLM and ILM in the UK is, and to analyze which challenges they face and what similarities and differences between Japan and the UK. Being led by these interests, this paper will review concept of OLM and ILM through some previous studies, then present framework of this paper at section 2. Section 3 examines trend of OLM, focusing on apprenticeship system, and section 4 surveys previous studies about ILM, comparing with Japan. Lastly, this report will make a conclusion about our main findings, and discuss some political implication we will expect from our findings at section 5.Conclusion and implication are as follows.1 Regarding the trend of the Apprenticeship (=OLM), looking at researchers' evaluations, it cannot be said that they have worked well so far. In this paper, we pointed out the following factors that the apprenticeship did not work well. Firstly, background of the decline of apprenticeship lies in free-riding and poaching by employers, which creates a lack of employer' s commitment.Secondly, many apprentices or FE students on vocational courses are often unable to obtain the necessary work experience to obtain employment, which lower evaluation of VET system and create division between academic education and vocational education. So, at present, it is difficult to judge whether Post-16 Skills Plan can be an effective measure to overcome this division or not soon.Thirdly, apprenticeship depends on stable employment relations. One of important reasons for lack of employer commitment is the changing structure and labour process and the nature of the employment relationships. Fourthly, overcoming poaching, employer’s short-term interests, unstable employment relations need to build social partnerships. Whether apprenticeship as foundation of OLM work well will hinges on social partnership between employers, workers or unions and government.2 Regarding the trend of ILM, the following was pointed out.Firstly, even in the UK, there are about 30% long-term workers, and the proportion has not changed significantly in the past 10 years and it has been stable. Secondly, in terms of ILM structure, it can be said that the degree of internalization in the UK is somewhat shallower than that of Japan in terms of the retention of the initial career, and the percentage of managers who have changed jobs.Thirdly, the organization structure has been delayered due to recent organization restructuring and downsizing. Opportunities for career promotion in traditional large corporate managers are decreasing, and horizontal movement is emphasized increasingly instead. In that sense the growth of the carrier to the top is constrained and the top of the ILM is thinning. This is a common trend between Japan, the US and the UK.Fourth, the number of college graduates has increased recently, and it is inferred that the initial career path of internship experience from undergraduate age, training skills after joining, and training scheme for graduates in the next few years are becoming commonplace. In that sense the lower part of ILM is formed.

Abstract

Different total knee arthroplasty （TKA） designs showed different intra-operative kinematics when evaluated with a navigation system. This study compared the kinematics of three different prosthesis designs using navigation-based in vivo simulation. Studies were carried out on 18 osteoarthritic knees using a computed tomography （CT）-free navigation system. The TKA designs used for these patients were six each of cruciate-retaining （CR） type, posterior-stabilized （PS） type, and PS mobile type. Intraoperative knee kinematics were measured during a passive range of motion from full extension to 130 degrees of knee flexion. Three types of prosthesis were compared for the following factors: the presence of condylar lift-off （gap difference greater than 1mm between medial gap and lateral gap） and anterior-posterior （AP） displacement of the center of femur relative to the tibia. The lift-off was found in patients with the PS type between 120° and 130° of flexion, but not in the other two types of implant. The pattern of AP displacement was similar among the three implants. The femur moved to the anterior side from extension to 50-55° of flexion, after that, the femur moved to the posterior side to full flexion. The function of PCL could prevent lift-off in the CR type of implants. In the PS-mobile implants, the mechanism of rotation platform might have contributed to lift-off prevention. The paradoxical anterior slide of the femoral condyles during mid flexion was observed in all types of implant.

Abstract

We proposed and also preparing for a POP (Proof-of-Principle) demonstration of a completely non-destructive H$^{-}$ stripping method by using only lasers at J-PARC. To reduce the laser power, which is one of the critical issues in this case, we have studied an extensive manipulations of the H$^{-}$ beam as well advanced uses of the lasers in this research. These includes utilizing a dispersion derivative of the H$^{-}$ beam to cope with its large momentum spread, while multi-pass laser system to utilize multiple interactions of the H$^{-}$ beam with lasers. We have estimated that the laser power can be reduced to more than one order of magnitude by applying a combination of these two methods. The detail simulation and experimental results of the H$^{-}$ beam manipulations as well as development status of the multi-pass laser system are presented in this paper.

Abstract

Lympho-epithelial Kazal-type-related inhibitor (LEKTI) is a large multidomain serine protease inhibitor that is expressed in epidermal keratinocytes. Nonsense mutations of the SPINK5 gene, which codes for LEKTI, cause Netherton syndrome, which is characterized by hair abnormality, ichthyosis, and atopy. A single nucleotide polymorphism (SNP) of SPINK5, p.K420E, is reported to be associated with the pathogenesis of atopic dermatitis (AD). We studied all 34 exons of the SPINK5 gene in Japanese 57 AD patients and 50 normal healthy controls. We detected nine nonsynonymous variants, including p.K420E; these variants had already been registered in the SNP database. Among them, p.R654H (n=1) was found as a heterozygous mutation in the AD patients, but not in the control. No new mutation was detected. We next compared the data of the AD patients with data from the Human Genetic Variation Database provided by Kyoto University; a significant difference was found in the frequency of the p.S368N genotype distribution. PolyPhen-2 and SIFT, two algorithms for predicting the functional effects of amino acid substitutions, showed significant scores for p.R654H. Therefore, R654H might be a risk factor for epidermal barrier dysfunction in some Japanese AD patients.

Abstract

Steady-state hematopoiesis responds to extracellular stimuli to meet changing demands and also to pathologically altered intracellular signaling. Granulocyte production increases following infection or in response to cytokine stimulation, and activation of the CCAAT/enhancer-binding protein β (C/EBPβ) transcription factor is required for such stress-induced granulopoiesis, whereas C/EBPα plays a critical role in maintaining steady-state granulopoiesis. Different roles of these C/EBP transcription factors in different modes of hematopoiesis are evolutionally conserved from zebrafish to humans. In addition to reactions against infections, C/EBPβ is responsible for cancer-driven myelopoiesis, which promotes cancer progression, at least in part, by abrogating the immune response in the cancer microenvironment. The BCR–ABL fusion protein activates emergency-specific pathway of granulopoiesis by upregulating C/EBPβ. This in turn causes chronic phase chronic myeloid leukemia, which is characterized by myeloid expansion. The C/EBPβ transcription factor also plays a role in other hematological malignancies of both myeloid and lymphoid lineage origin. Thus, elucidation of the upstream and downstream networks surrounding C/EBPβ will lead to the development of novel therapeutic strategies for diseases mediated by non-steady-state hematopoiesis.

Abstract

Incorporation of imatinib into chemotherapeutic regimens has improved the prognosis of children with Philadelphia chromosome-positive acute lymphoblastic leukemia (Ph+ALL). We investigated a role of imatinib immediately before hematopoietic stem cell transplantation (HSCT). Children with Ph+ALL were enrolled on JPLSG Ph+ALL 04 Study within 1 week of initiation of treatment for ALL. Treatment regimen consisted of Induction phase, Consolidation phase, Reinduction phase, 2 weeks of imatinib monotherapy phase, and HSCT phase (Etoposide+CY+TBI conditioning). Minimal residual disease (MRD), the amount of BCR–ABL transcripts, was measured with the real-time PCR method. The study was registered in UMIN-CTR: UMIN ID C000000290. Forty-two patients were registered and 36 patients (86%) achieved complete remission (CR). Eight of 17 patients (47%) who had detectable MRD at the beginning of imatinib monotherapy phase showed disappearance or decrease in MRD after imatinib treatment. Consequently, 26 patients received HSCT in the first CR and all the patients had engraftment and no patients died because of complications of HSCT. The 4-year event-free survival rates and overall survival rates among all the 42 patients were 54.1 ± 7.8% and 78.1 ± 6.5%, respectively. Four of six patients who did achieve CR and three of six who relapsed before HSCT were salvaged with imatinib-containing chemotherapy and subsequently treated with HSCT. The survival rate was excellent in this study although all patients received HSCT. A longer use of imatinib concurrently with chemotherapy should eliminate HSCT in a subset of patients with a rapid clearance of the disease.

Abstract

The RCS vacuum system has been upgraded since the completion of its construction towards the objectives of both better vacuum quality and higher reliability of the components. For the better vacuum quality, (1) pressure of the injection beam line was improved to prevent the H⁻ beam from converting to H0; (2) leakage in the beam injection area due to the thermal expansion was eliminated by applying the adequate torque amount for the clamps; (3) new in-situ degassing method of the kicker magnet was developed. For the reliability increase of the components, (1) A considerable number of fluoroelastmer seal was exchanged to metal seal with the low spring constant bellows and the light clamps; (2) TMP controller for the long cable was developed to prevent the controller failure by the severe electrical noise; (3) A number of TMP were installed instead of ion pumps in the RF cavity section as an insurance for the case of pump trouble., Proceedings of the 8th Int. There are few data on the long-term clinical outcome of atrial fibrillation patients with a history of acute coronary syndrome. This is a 2-year study of atrial fibrillation patients with or without a history of acute coronary syndromes. METHODS: Adults with newly diagnosed atrial fibrillation and >/=1 investigator-defined stroke risk factor were enrolled in GARFIELD-AF between March 2010 and September 2015. The association between prior acute coronary syndromes and long-term outcomes was determined using a Cox proportional hazards model, adjusting for baseline risk factors, oral anticoagulation (OAC) +/- antiplatelet (AP) therapy, and usual care. RESULTS: Of 39,679 patients, 10.5% had a history of acute coronary syndromes. At 2-year follow-up, patients with prior acute coronary syndromes had higher adjusted risks of stroke/systemic embolism (hazard ratio [HR] 1.39; 95% confidence interval [CI], 1.08-1.78), major bleeding (HR 1.30; 95% CI, 0.95 -1.79), all-cause mortality (HR 1.34; 95% CI, 1.21 -1.49), cardiovascular mortality (HR 1.85; 95% CI, 1.51-2.26), and new acute coronary syndromes (HR 3.42; 95% CI, 2.62-4.45). Comparing antithrombotic therapy in the acute coronary syndromes vs no acute coronary syndromes groups, most patients received OAC +/- AP: 60.8% vs 66.1%, but AP therapy was more likely in the acute coronary syndromes group (68.1% vs 32.9%), either alone (34.9% vs 20.8%) or with OAC (33.2% vs 12.1%). Overall, 17.8% in the acute coronary syndromes group received dual AP therapy with (5.3%) or without OAC (12.5%). Among patients with moderate/high risk for stroke/systemic embolism, fewer in the acute coronary syndromes group received OAC with or without AP therapy (Congestive heart failure, Hypertension, Age 75 years, Diabetes mellitus, prior Stroke, TIA, or thromboembolism, Vascular disease, Age 65-74 years, Sex category [CHA2DS2-VASc] 2: 52.1% vs 64.6%; CHA2DS2-VASc >/=3: 62.0% vs 70.7%), and the majority with a Hypertension (uncontrolled systolic blood pressure >160 mm Hg), Abnormal renal or liver function, previous Stroke, Bleeding history or predisposition, Labile international normalized ratios, Elderly, and concomitant Drugs or alcohol excess (HAS-BLED) score >/=3 were on AP therapy (83.8% vs 65.5%). CONCLUSIONS: In GARFIELD-AF, previous acute coronary syndromes are associated with worse 2-year outcomes and a greater likelihood of under-treatment with OAC, while two-thirds of patients receive AP therapy. Major bleeding was more common with previous acute coronary syndromes, even after adjusting for all risk factors.

International audience; Ticagrelor may be a more effective antiplatelet therapy than aspirin for the prevention of recurrent stroke and cardiovascular events in patients with acute cerebral ischemia.; We conducted an international double-blind, controlled trial in 674 centers in 33 countries, in which 13,199 patients with a nonsevere ischemic stroke or high-risk transient ischemic attack who had not received intravenous or intraarterial thrombolysis and were not considered to have had a cardioembolic stroke were randomly assigned within 24 hours after symptom onset, in a 1:1 ratio, to receive either ticagrelor (180 mg loading dose on day 1 followed by 90 mg twice daily for days 2 through 90) or aspirin (300 mg on day 1 followed by 100 mg daily for days 2 through 90). The primary end point was the time to the occurrence of stroke, myocardial infarction, or death within 90 days.; During the 90 days of treatment, a primary end-point event occurred in 442 of the 6589 patients (6.7%) treated with ticagrelor, versus 497 of the 6610 patients (7.5%) treated with aspirin (hazard ratio, 0.89; 95% confidence interval [CI], 0.78 to 1.01; P=0.07). Ischemic stroke occurred in 385 patients (5.8%) treated with ticagrelor and in 441 patients (6.7%) treated with aspirin (hazard ratio, 0.87; 95% CI, 0.76 to 1.00). Major bleeding occurred in 0.5% of patients treated with ticagrelor and in 0.6% of patients treated with aspirin, intracranial hemorrhage in 0.2% and 0.3%, respectively, and fatal bleeding in 0.1% and 0.1%.; In our trial involving patients with acute ischemic stroke or transient ischemic attack, ticagrelor was not found to be superior to aspirin in reducing the rate of stroke, myocardial infarction, or death at 90 days. Abstract

International audience; A mammalian dentary discovered in the Coniacian Ashizawa Formation (Fukushima, northeastern Japan) is described. The specimen is a fragment of the horizontal ramus of a left edentulous dentary with five alveoli, the distal four of which are plugged with broken roots. Based on the morphologies of the dentary and the roots, it is considered to be of a therian mammal. This constitutes the first discovery of a Mesozoic mammal in northeastern Japan and highlights the potential for future mammal discoveries in the Cretaceous System in northeastern Japan, which will be significant for disclosure of the mammalian faunal evolution in East Asia during the Late Cretaceous.

42. Efficiency of Incentive Spirometry for Video-Assisted Thoracoscopic Surgery for Esophagectomy

Abstract

Transthoracic subtotal esophagectomy for esophageal cancer is a highly invasive procedure, associated with high mortality and morbidity rates. We examined the use of video-assisted thoracoscopic and laparoscopic surgery for esophagectomy (VATS-E). Further, incentive spirometry (IS) is commonly used in perioperative rehabilitation for esophagectomy. We investigated whether pulmonary complications after VATS-E are related to changes in the perioperative IS volumes and whether such changes could be predictive of these complications. This study included 63 patients who underwent VATS-E from June 2008 to December 2009. IS volumes before and after surgery were recorded for all patients. The perioperative IS volumes and clinicopathological factors were correlated with the incidence of postoperative pneumonia and atelectasis. Nine patients (14.5%) had postoperative pneumonia, and thirteen (22.2%) had atelectasis. Univariate analysis showed an increased risk of atelectasis in patients with diabetes and an increased risk of pneumonia in patients with a long operating time and for whom the lung was adhered to the thoracic wall. The vital capacity (VC) correlation coefficient was 0.674. Further, the risk of pneumonia was high in patients with 13% less than the minimum IS volume/preoperative VC ratio and 22% with less than the average IS volume/preoperative VC ratio. Multivariate regression models for pneumonia showed the same results regarding the IS volume/VC ratio. The results indicated that IS volumes could be used to predict the incidence of complications after VATS-E, and thereby facilitate early application of interventions to prevent pulmonary complications.

Abstract

This paper presents results of full-scale cyclic loading tests under constant deformation amplitude conducted to evaluate deformation capacity of welded beam-to-column connection adopted in moment frames. Test specimens are designed and fabricated to investigate the influence of flexural strength of beam end connection. The deformation capacity expressed by the relationships between ductility and cumulative plastic deformation is revealed from test results. The relationships between crack growth at welded joint and strength of connection is evaluated regarding deformation amplitude. Abstract

Seismic response of the passive controlled structure using buckling-restrained braces (BRB) can be ensured when the BRB connection have sufficient strength. Therefore, it is necessary to prevent the occurrence of slippage or yielding at the connections. In this paper, parametric seismic response analysis was conducted to clarify main factors concerning strength of connections that will affect seismic response of structures. In the analysis, strength variations of BRB and bolted connection are considered. 48. Effects of Discourse Function on the Processing of Relative Clauses in Japanese : Through the corpus research and behavioral experiment

Abstract

Previous researches have shown that subject relative clauses are easier to process than object relative clauses in many languages. Roland et al. (2007), however, claimed that the processing ease of subject relative clauses does not depend on structural complexity or linear distance, but it depends on the difference of discourse function between subject and object relative clauses. They confirmed the appropriate context reduced the processing load of object relative clauses. In this paper, we examined the validity of the discourse-function account (Roland et al. 2007, 2008) using a combination of a corpus and an experimental research. The results showed that the discourse-function account was not successful in explaining Japanese data. This suggests that the reduction of processing load in Roland et al. (2007) might be derived from the repetition of noun phrases rather than the relative clause processing itself.

Abstract

This paper addresses the derivation of collapse load of beam flange to circular tube column connections with exterior diaphragm based on plastic analysis. In order to verify the validity of the derived collapse load, component tests of the connections were conducted. Major findings obtained from tests are summarized as follows: (1) the yield strength obtained from the tests was about 80 to 90% of the calculated collapse loads, (2) the local deformation of the circular tube column that corresponds to the calculated collapse load is small enough for the yield strength, (3) the derived collapse load formula which is considering the shape of the connections correlated well with the test results, and captured the trend in a reasonable manner, (4) the maximum strength obtained from the tests was about 110 to 120% of the calculated maximum loads.
